Abstract
Un aparato (10) para separar porciones de hueso y médula ósea de un hueso (2) para fines de diagnóstico o terapéuticos, que comprende: un alojamiento (12); un árbol motriz hueco (14) operable para acoplarse con un conjunto de engranajes (37); comprendiendo el árbol motriz hueco (14) un primer extremo (22) operable para penetrar en el hueso; comprendiendo además el árbol motriz hueco un canal interior (15) operable para conducir porciones de hueso y médula ósea; un trocar separable (9) que se puede introducir en el canal interior del árbol motriz hueco (14), comprendiendo dicho trocar un primer extremo (24) operable para penetrar en el hueso y un segundo extremo; siendo el conjunto de engranajes (37) operable para acoplarse con el árbol motriz hueco; un motor (38), acoplado al árbol motriz hueco, operable para accionar el trocar al interior de la médula ósea mediante rotación en el árbol motriz hueco; y una fuente de alimentación eléctrica (39) y circuitería asociada operable para accionar el motor, caracterizado porque el árbol motriz hueco comprende: un segundo extremo operable para permitir la recuperación de porciones de hueso y médula ósea, en donde el trocar separable comprende además un canal interior operable para conducir porciones de hueso y médula ósea.
Description
Aparato para poder acceder a la médula ósea.
La presente invención se refiere a un aparato
para extraer muestras del hueso o médula ósea. El aparato se puede
emplear para extraer células madre o médula ósea para fines de
transplante o de diagnóstico.
Existen muchos estados clínicos en donde es
importante poder acceder y recuperar médula ósea. En algunos casos,
puede ser necesario el tratamiento de enfermedades con un
transplante de médula ósea o células madre para restaurar las
células sanguíneas en funcionamiento en el cuerpo después de una
quimioterapia en altas dosis. Dichos estados pueden incluir
leucemias agudas, tumores de cerebro, cáncer de mama, enfermedad de
Hodgkin, mieloma múltiple, neuroblastoma, linfomas no relacionados
con Hodgkin, cáncer ovárico, sarcoma y cáncer testicular. En otros
casos es necesario acceder a la médula ósea para obtener una muestra
de la médula para llevar a cabo ensayos de diagnóstico. Estos
estados pueden incluir cáncer de cualquier tipo y enfermedad
hematológica de cualquier origen.
Las técnicas actuales para poder tener acceso a
la médula ósea pueden ser difíciles, traumáticas y ocasionalmente
peligrosas, dependiendo del sitio elegido para su recogida, la
experiencia del cirujano y la anatomía del paciente. En general,
los dispositivos disponibles para poder acceder a la cavidad medular
del hueso, en donde está ubicada la médula ósea, incluyen una aguja
acoplada con un trocar, con asideros para facilitar la aplicación
de presión y rotación. Estos tipos de dispositivos requieren una
fuerza importante para penetrar a través de la corteza exterior del
hueso mediante una técnica de fracturación. El hecho de ejercer una
elevada presión sobre la aguja causa dolor en el paciente y con
frecuencia daños en la punta de la aguja. Esto es particularmente
un problema cuando la recogida se efectúa desde el esternón de un
paciente debido a que un exceso de fuerza puede causar una
penetración a través del esternón y puede dañar las estructuras
subyacentes tales como el corazón y los vasos grandes.
Otro inconveniente de las técnicas actuales para
poder acceder a la médula ósea es que frecuentemente se requiere
más de un sitio de penetración en el hueso para recuperar suficiente
médula ósea para llevar a cabo ensayos de diagnóstico o para fines
de transporte. Para recuperar una muestra adecuada de médula ósea
para un transplante de médula ósea o de células madre, el médico
puede que tenga que colocar la aguja en diversas partes diferentes
de la pelvis, lo cual puede requerir hasta seis sitios o más de
punzamiento con la aguja. Este requisito de múltiples pasos puede
ser extremadamente doloroso para el paciente y puede hacer que la
gente sea reacia a donar médula ósea. Esta técnica de utilizar
múltiples pasos también puede causar fatiga en los cirujanos más
bajos los cuales pueden carecer de la suficiente resistencia para
completar procedimientos de múltiples pasos.
De manera similar, también es difícil la
recuperación de muestras de hueso para fines de diagnóstico.
Ocasionalmente, no se recupera la muestra de núcleo del hueso
debido a que no puede extraerse con éxito por medio de una aguja de
biopsia estándar. De este modo, pueden ser necesarios múltiples
intentos para obtener una biopsia satisfactoria de hueso o de
médula ósea.

de biopsia sean obligadas mediante presión manual al interior del
hueso. Estas técnicas pueden tener efectos secundarios indeseables
tales como daños en la punta de la aguja o pueden hacer que la
aguja se deslice del objetivo óseo propuesto y vaya a parar al
interior de órganos o tejidos blandos.
La WO980633 describe un dispositivo y un método
de administración de fármacos en tejidos duros en donde se perfora
el tejido duro y se proporciona un paso de administración de
fármacos al tejido subyacente.
La FR 853349 describe un dispositivo para
obtener una muestra de tejido de órganos humanos o animales.
Existe la necesidad de disponer de un aparato y
método para poder acceder a la médula ósea que sea mínimamente
traumático para el paciente y que permita separar una cantidad
suficiente de médula ósea en la primera vez que es penetrado el
hueso. De acuerdo con las enseñanzas de la presente invención, se
proporcionan un aparato y un método para separar del hueso
porciones de médula ósea.
La invención proporciona un aparato de acuerdo
con la reivindicación 1 y un conjunto de árbol motriz hueco de
acuerdo con la reivindicación 16.
Según una modalidad, el aparato incluye un
alojamiento y un árbol motriz hueco operable para acoplarse con un
conjunto de engranajes. El árbol motriz hueco incluye un primer
extremo operable para penetrar en el hueso y un segundo extremo
operable para permitir la recuperación de porciones de hueso y
médula ósea. El árbol motriz hueco también incluye un canal
interior operable para conducir porciones de hueso y médula ósea. El
aparato también incluye un trocar separable con un primer extremo
operable para penetrar en el hueso y un segundo extremo. El trocar
separable incluye un canal interior operable para conducir porciones
de hueso y médula ósea. El conjunto de engranajes es capaz de
acoplarse con el árbol motriz hueco y hacerlo girar. También se
incluyen un motor operable para acoplarse con el conjunto de
engranajes y accionar el árbol motriz hueco al interior de la
médula ósea mediante rotación del árbol motriz hueco, y una fuente
de alimentación eléctrica y circuitería asociada operable para
accionar el motor.
Según otra modalidad de la invención, se
proporciona un conjunto de árbol motriz hueco operable para separar
hueso o tejido de médula ósea. Se incluyen un árbol motriz hueco
exterior y un trocar interior separable y operable para penetrar en
la médula ósea. El árbol motriz hueco exterior está acoplado de
forma liberable con un conjunto de engranajes y el árbol motriz
hueco exterior incluye un canal interior operable para conducir
porciones de hueso y médula ósea.
En una modalidad particular de la invención, el
sitio de recogida dentro de la médula ósea se puede cambiar
mediante un engranaje provisto de trinquete y unido al segundo
extremo del trocar. El engranaje se puede emplear para cambiar la
profundidad de rastreo en la médula ósea.
Las presentes modalidades y ventajas de las
mismas se pueden entender de forma más completa y profunda haciendo
referencia a la siguiente descripción considerada en combinación con
los dibujos adjuntos, en donde números de referencia iguales
indican características también iguales y en donde:

Modalidades preferidas de la invención y sus
ventajas se podrán entender mejor con referencia a las figuras
1A-5C en donde números similares se refieren a
partes iguales y similares.
La figura 1A ilustra un ejemplo de un aparato
para separar en la ósea de un hueso. El aparato 10 se puede emplear
para obtener una muestra de médula ósea de cualquier hueso adecuado
del cuerpo, tal como la cresta ilíaca o el esternón. Según una
modalidad, el aparato 10 incluye un alojamiento 12, un árbol motriz
hueco 14, un trocar separable 19, un conjunto de engranajes 37, un
motor 38 y una fuente de alimentación eléctrica 39. El alojamiento
12 puede incluir un conmutador on/off 3, un asidero 5 y un protector
6. El asidero 5 puede estar angulado hacia abajo para permitir una
fácil operación y también para permitir que el extremo del árbol
motriz hueco 14 gire sin que se vea obstruido por una mano. El
alojamiento 12 incluye una fuente de alimentación eléctrica 39, un
motor 38 y circuitería asociada 18, un árbol motriz hueco 14 y un
conjunto de engranajes 37. El árbol motriz hueco 14 incluye un
canal interior 15 y collarín 16. El engranaje 17 del conjunto de
engranajes 37 se acopla con un collarín 36 del árbol motriz hueco
14 y con ello hace girar al árbol motriz hueco 14.
El trocar separable 19 se puede introducir en el
canal interior 15 del árbol motriz hueco 14. El trocar 19 es hueco
y tiene un canal interior operable para conducir muestras de hueso y
de médula ósea. El trocar 19 puede tener un asidero 8 que se puede
emplear para apretar el trocar 19 en su sitio o para retirarlo del
canal interior 15 del árbol motriz hueco 14. El árbol motriz hueco
14 puede incluir un conector de bloqueo luer 4 en el punto en donde
sale el alojamiento 12. El conector de bloqueo luer puede permitir
la conexión del árbol motriz hueco con un aparato de aspiración tal
como un tubo o una jeringa o a cualquier otro aparato adecuado que
ayude a obtener una muestra de biopsia de hueso o médula ósea. Un
orificio de acceso, tal como un orificio de aspiración, puede
también estar unido de forma liberable a un extremo del árbol motriz
hueco en donde sale del alojamiento 12, por ejemplo a un conector
de bloqueo luer. Dicho acoplamiento puede ser un tapón, un
orificio, un aparato de aspiración, un orificio giratorio u otro
adaptador.
La figura 1B muestra el árbol matriz hueco 14
separado del aparato 10. Según una modalidad, el árbol motriz hueco
14 puede incluir uno o más cojinetes de empuje 21. Los cojinetes de
empuje pueden absorber presión procedente del empuje de un árbol
motriz al interior del hueso durante la perforación. En el árbol
motriz hueco también están incluidos orificios laterales mostrados
con mayor detalle en la figura 2. La figura 1C ilustra el trocar
separable 19 que incluye un asidero 8. El asidero 8 puede estar
configurado en una forma que se pueda sujetar y girar fácilmente
durante el procedimiento de obtención de una biopsia. El trocar
separable 19 puede incluir uno o más orificios laterales operables
para acceder a una muestra de médula ósea, ilustrados con mayor
detalle en la figura 2.
Según una modalidad, mostrada en la figura 1D,
el alojamiento 12 puede incluir una compuerta o puerta liberable
12a que se puede abrir para permitir que el árbol motriz hueco y el
engranaje acoplado 17 puedan retirarse después de utilizar el
aparato 10. Esto puede ser deseable cuando el aparato 10 es
reutilizable y el árbol motriz hueco 14 y el trocar separable 19
son desechables. La compuerta o puerta liberable también puede ser
deseable para poder limpiar el interior del aparato 10.
En otra modalidad de la presente invención,
mostrada en la figura 1E, un penetrador desunible 20 puede estar
acoplado al árbol motriz hueco 14 por medio de un conector 36 y que
perfora al interior del hueso 2. En esta modalidad, el trocar
separable 19 se puede introducir en el canal interior 15 del árbol
motriz hueco 14 y al interior del canal hueco del penetrador 20. El
penetrador 20 puede incluir orificios laterales para poder acceder
a las muestras de hueso y médula ósea durante una biopsia o
procedimiento de recogida de médula ósea. Una ventaja de una
modalidad que incluye un penetrador desunible consiste en que
permite unir penetradores de varios tamaños y configuraciones al
árbol motriz hueco 14. En esta modalidad, el conector 36 incluye un
canal interior operable para poder recuperar las muestras de hueso y
médula ósea.
Las figuras 2A-D muestran un
extremo 22 de un árbol motriz hueco o de un penetrador que es
adecuado para penetrar en el hueso. La figura 2A ilustra el extremo
22 del árbol motriz hueco o penetrador que puede incluir múltiples
orificios de muestreo 23 a través de los cuales se puede aspirar
material de médula ósea o cualquier otro material de biopsia. Los
orificios de muestreo 23a, 23b y 23c son cada uno de ellos operables
para recuperar una porción de médula ósea. Cuando el extremo 24 del
trocar separable está en su posición dentro del canal interior del
extremo 22 del árbol motriz hueco, el orificio de muestreo 13 del
extremo 24 del trocar separable puede quedar alineado con el
orificio de muestreo 23a, 23b o 23c. El cirujano puede determinar el
nivel en la médula ósea en donde se toma una muestra o puede
determinar qué orificio de muestreo (23a, 23b o 23c) puede
alinearse con el orificio de muestreo 13 del trocar. El extremo 22
del árbol motriz hueco o penetrador puede incluir una punta
aserrada 25 como se muestra en la figura 2B o cualquier otra
configuración adecuada para el muestreo del hueso o médula
ósea.
La figura 2C muestra el extremo 24 del trocar
separable que tiene un orificio de muestreo 23 cerca de la punta 27
a través del cual se puede recuperar una muestra del hueso o médula
ósea al igual que a través de un orificio de muestreo 23a, 23b o
23c. Cuando el orificio de muestreo 23a, 23b o 23c del árbol motriz
hueco o penetrador 22 queda alineado con el orificio de muestreo 13
del trocar separable 24, se puede aspirar del hueso una porción de
médula ósea. El trocar separable 24 puede ser separado permitiendo
que la médula ósea sea aspirada a través de uno o más orificios de
muestreo 23 del árbol motriz hueco o penetrador 22 en diferentes
sitios de una cavidad de médula ósea de manera secuencial. La
figura 2D muestra otro ejemplo de la punta 25 del extremo 22 del
árbol motriz hueco o penetrador que tiene una rosca interna 26 que
puede extraer una muestra de hueso y núcleo a medida que el árbol
motriz hueco o penetrador 22 perfora en el hueso. La rosca 26 puede
acoplarse y adherirse a una porción de hueso o a una sustancia
semisólida tal como médula ósea y mantener el contacto con la
muestra de manera que pueda ser recuperada con éxito.
La figura 3A muestra el mecanismo operativo 32
que puede ser incluido en algunas modalidades de la invención y que
puede unirse al árbol motriz hueco 14 y ser manipulado para cambiar
la profundidad en la médula ósea en donde se produce el muestreo.
El mecanismo operativo 32 incluye un asidero 35 y un engranaje 33.
El engranaje 33 se acopla con el engranaje 34 unido al trocar 19.
El orificio de aspiración 28, ilustrado unido al trocar 19, se
puede emplear para recuperar porciones de médula ósea del canal
interior 15 del árbol motriz hueco 14.
La figura 3B muestra un ejemplo de orificio de
aspiración 28 que se puede conectar al trocar 19 en el punto en
donde sale del alojamiento 12. En una modalidad de la invención, al
árbol motriz hueco 14 se puede conectar también un aparato con
orificio de aspiración o aparato giratorio de aspiración. Esta
característica permite utilizar, para obtener muestras de médula
ósea, un aparato de aspiración del tipo bien conocido para el
experto en la materia. El orificio de aspiración 28 puede también
estar configurado para unirse directamente a un penetrador, por
ejemplo en una modalidad de la invención en donde el penetrador está
desunido del alojamiento con anterioridad a su acceso para
recuperar médula ósea. En la figura 3B se muestra también un aparato
giratorio de aspiración 45 que permite unir un tubo de aspiración
41 al orificio de aspiración 28. El aparato giratorio de aspiración
45 permite que el orificio de aspiración 28 permanezca unido al
punto de aspiración 41 mientras un aparato de perforación anexo
está perforando en la médula ósea sin que el tubo de aspiración 41
se alavee o torsione. En la figura 3B se muestra también un
receptáculo 43 que está interpuesto entre el tubo de aspiración 41
y una fuente de aspiración, de manera que una muestra de médula ósea
puede ser recuperada con éxito al interior del receptáculo 43 y no
se produce pérdida alguna de la misma dentro de la máquina de
aspiración.
Las figuras 4A-D muestran un
ejemplo de extremo de árbol motriz hueco o extremo de penetrador 33
y de trocar interior 42 que tiene una configuración de aguja
dividida. El trocar interior 42 está introducido en un extremo 44
del árbol motriz hueco o penetrador. El trocar interior 42 mostrado
en la posición abierta en la figura 4A se hace avanzar más allá del
extremo del extremo 44 del árbol motriz hueco o penetrador, en donde
puede ser abierto e introducido en el hueso, médula ósea o en
cualquier otro tejido. El trocar interior 42 puede ser cerrado,
como se muestra en las figura 4B, de manera que una muestra queda
retenida en su asidero a medida que se extrae del cuerpo. La figura
4C muestra una sección transversal del trocar interior de aguja
dividida 42 en donde la punta del trocar interior 42 se encuentra
en la posición abierta. La figura 4D muestra una sección
transversal del trocar 42, ilustrado en la posición cerrada.
El aparato puede o no incluir un conjunto de
engranaje reductor. El conjunto de engranaje reductor puede incluir
un conjunto de engranaje helicoidal mostrado con mayor detalle en la
figura 5A y puede incluir un primer conector 50 que conecta el
árbol 52 del motor 38 al engranaje helicoidal 54. Se puede emplear
un conjunto de engranaje reductor para disminuir las RPMs entre el
motor y el conjunto del penetrador para proporcionar unas RPMs
óptimas en el punto de inserción del conjunto del penetrador en el
hueso. La figura 5B ilustra otra modalidad de un conjunto de
engranaje reductor en donde un primer engranaje recto 57 se acopla
con un segundo engranaje recto 58. La figura 5C ilustra una
modalidad alternativa de un conjunto de engranaje reductor en donde
el engranaje recto 56 está desviado en 45º con respecto al árbol
motriz hueco 14 lo cual puede ser preferible en ciertas modalidades
de la presente invención. En esta modalidad, el engranaje recto 58
puede estar desviado en cualquier ángulo y no queda limitado a una
desviación de 45º. Se pueden emplear otros engranajes en el conjunto
de engranaje reductor, por ejemplo un engranaje planetario (no
mostrado de manera expresa) se puede emplear solo o en combinación
con un engranaje helicoidal o un engranaje recto.
Aunque la presente invención y sus ventajas han
sido descritas con detalle, deberá entenderse que pueden llevarse a
cabo diversos cambios, sustituciones y alternativas sin desviarse
por ello del alcance de la invención tal como queda definida por
las siguientes reivindicaciones.
Claims (20)
1. Un aparato (10) para separar porciones de
hueso y médula ósea de un hueso (2) para fines de diagnóstico o
terapéuticos, que comprende:
- un alojamiento (12);
- un árbol motriz hueco (14) operable para acoplarse con un conjunto de engranajes (37);
- comprendiendo el árbol motriz hueco (14) un primer extremo (22) operable para penetrar en el hueso;
- comprendiendo además el árbol motriz hueco un canal interior (15) operable para conducir porciones de hueso y médula ósea;
- un trocar separable (9) que se puede introducir en el canal interior del árbol motriz hueco (14), comprendiendo dicho trocar un primer extremo (24) operable para penetrar en el hueso y un segundo extremo;
- siendo el conjunto de engranajes (37) operable para acoplarse con el árbol motriz hueco;
- un motor (38), acoplado al árbol motriz hueco, operable para accionar el trocar al interior de la médula ósea mediante rotación en el árbol motriz hueco; y
- una fuente de alimentación eléctrica (39) y circuitería asociada operable para accionar el motor,
caracterizado porque el árbol motriz
hueco comprende:
- un segundo extremo operable para permitir la recuperación de porciones de hueso y médula ósea, en donde el trocar separable comprende además un canal interior operable para conducir porciones de hueso y médula ósea.
2. Un aparato según la reivindicación 1, en
donde el árbol motriz hueco (14) y el trocar separable (19) son
desechables y operables para ser retirados del aparato a través de
un panel de acceso acoplado de forma liberable en el
alojamiento.
3. Un aparato según la reivindicación 1, en
donde el árbol motriz hueco (14) es operable para poderse unir a un
orificio de acceso.
4. Un aparato según la reivindicación 1, en
donde el árbol motriz hueco es operable para poderse unir a un
orificio de aspiración (28).
5. Un aparato según la reivindicación 1, en
donde el segundo extremo del trocar (29) está acoplado a un
engranaje provisto de trinquete (33), siendo operable el engranaje
para alinear los orificios de muestreo del trocar con los orificios
de muestreo del árbol motriz hueco.
6. Un aparato según la reivindicación 1, en
donde el trocar separable (29) comprende una punta (27) que tiene
una acanaladura longitudinal operable para liberar trozos de hueso
de un sitio de inserción en la médula ósea y en donde el penetrador
exterior incluye un filo de corte.
7. Un aparato según la reivindicación 1, en
donde el alojamiento (12) comprende una porción de una base montada
sobre una superficie.
8. Un aparato según la reivindicación 1, en
donde el árbol motriz hueco (14) comprende una punta (25) que tiene
una rosca interna (26) operable para acoplarse con una muestra de
biopsia de hueso o médula ósea a medida que se retira esta de la
médula ósea.
9. Un aparato según la reivindicación 1, en
donde el trocar separable (19) comprende una punta (27) que tiene
dos piezas longitudinales separadas que pueden moverse o separarse
de forma conjunta.
10. Un aparato según la reivindicación 1, que
comprende además un conjunto de engranaje reductor operable para
girar el árbol motriz hueco (14).
11. Un aparato según la reivindicación 1 o 16,
en donde el árbol motriz hueco (14) comprende al menos un cojinete
de empuje (21).
12. Un aparato según la reivindicación 1, en
donde el árbol motriz hueco (14) comprende al menos un orificio
lateral (23a) operable para poder acceder a una porción de hueso o
médula ósea.
13. Un aparato según la reivindicación 1, en
donde el trocar separable (19) comprende al menor un orificio
lateral (13) operable para alinearse con un orificio lateral (23a)
del árbol motriz hueco (14) para poder acceder a una porción de
hueso o médula ósea.
14. Un aparato según la reivindicación 1 o 16,
en donde el árbol motriz hueco (14) comprende un conector de bloqueo
luer (4).
15. Un aparato según la reivindicación 1, en
donde el trocar interior (19) comprende dos cuerpos longitudinales,
teniendo los cuerpos longitudinales puntas con mordazas operables
para sujetar una porción de hueso o médula ósea y en donde los
cuerpos longitudinales son operables para separarse o juntarse
dependiendo de su posición con respecto al árbol motriz hueco
(14).
16. Un conjunto de árbol motriz hueco operable
para separar hueso o médula ósea de un hueso (2) para fines de
diagnóstico o terapéuticos, que comprende:
- un árbol motriz hueco exterior (14) y un trocar interior separable (19) operable para penetrar en la médula ósea;
- estando el árbol motriz hueco exterior acoplado de forma liberable a un conjunto de engranajes (37); y
- comprendiendo además el árbol motriz hueco exterior un canal interior (15) operable para conducir porciones de hueso y médula ósea;
- caracterizado porque el trocar separable comprende un canal interior operable para conducir porciones de hueso y médula ósea.
17. Un conjunto de árbol motriz hueco según la
reivindicación 16, en donde el árbol motriz hueco (14) comprende
una punta (25) con dientes aserrados, comprendiendo además el árbol
motriz hueco una rosca interna (26) operable para acoplarse con una
porción de hueso o médula ósea.
18. Un conjunto de árbol motriz hueco según la
reivindicación 16, en donde el árbol motriz hueco (14) comprende un
cuerpo con al menos un orificio lateral (23a) operable para obtener
porciones de médula ósea.
19. Un conjunto de árbol motriz hueco según la
reivindicación 16, en donde el trocar interior (19) comprende una
punta (27) o un filo de corte y comprende además un cuerpo con al
menos un orificio lateral (13) operable para obtener tejido de
médula ósea de la médula ósea.
20. Un conjunto de árbol motriz hueco según la
reivindicación 16, en donde el trocar interior (19) comprende dos
cuerpos longitudinales, comprendiendo además los cuerpos
longitudinales puntas con mordazas curvadas; y pudiéndose operar
los cuerpos longitudinales para separarlos o montarlos dependiendo
de su posición con respecto al penetrador exterior, y pudiendo ser
operados además para sujetar la porción de hueso o médula ósea entre
las mordazas curvadas.
