Description
- Die Erfindung bezieht sich auf eine Lösung von Taxol mit verbesserter Stabilität.
- Taxol ist eine Verbindung, extrahiert aus der Rinde einer westlichen Eibe, Taxus brevifolia, und bekannt für seine anti-neoplastische Aktivität. Es ist z. B. im Merck Index, 11. Ausgabe, 1989, Monographie 9049 beschrieben.
- 1977 wurde Taxol ausgewählt zur Entwicklung eines anti-neoplastischen Mittels aufgrund seines einzigartigen Mechanismus der Wirksamkeit und guten zytotoxischen Aktivität gegen IP-implantierte D16-Melanome und der menschlichen X-1 Säugertumor-Fremdverpflanzung.
- Es wird angenommen, daß Taxol als ein Gift der mitotischen Spindel und als ein starker Inhibitor der Zellreplikation in vitro wirkt. Andere Mitose-Spindel-Gifte (Colchicin und Podophyllotoxin) inhibieren den Mikrotubuli-Zusammenbau. Taxol zeigt einen etwas anderen Wirkungsmechanismus, da es das Gleichgewicht der Polymerisation/Depolymerisation zugunsten des Polymer-Zusammenbaus verschiebt und die Mikrotubuli gegen die Depolymerisation stabilisiert unter Bedingungen, die eine schnelle Auflösung der Mikrotubuli verursachen würden. Die Störung des Polymerisations/Depolymerisationszyklus in Zellen scheint sowohl die Replikation als auch die Wanderung der Zellen zu stören.
- Nach extensivem präklinischen Screening in Maus-Tumormodellen trat Taxol 1983 in die klinische Prüfung ein. Über die letzten Jahre hat Taxol gute Wirkungen bei der Behandlung sowohl von Ovar- als auch Brustkrebspatienten gezeigt, die nicht von der Vinca-Alkaloid- oder Cisplatin-Therapie profitierten. Es zeigte ebenfalls ermutigende Ergebnisse bei Patienten mit anderen Formen des Krebses, einschließlich Lungenkrebs, Melanoma, Lymphoma, Haut- und Halskrebs.

National Cancer Institute, überarbeitet im Juli 1991 und auf Veröffentlichungen, präsentiert auf dem Zweiten National Cancer Institute Workshop über Taxol und Taxus, gehalten in Alexandria, Virginia, USA, am 23.-24. September 1992.
- Es ist ein Nachteil der bekannten Formulierung, daß das Taxol darin degradiert, mit dem Ergebnis, daß die Halbwertzeit der Formulierung unbefriedigend ist. Die US-PS 4,960,790 beschreibt Zusammensetzungen, umfassend Taxol-Derivate, die gute Stabilität bei pH-Werten, geeignet für pharmazeutische Formulierungen (pH 3-4), zeigen und bei physiologischem pH abgebaut werden, um als mögliche Taxol-Arzneistoffvorstufe zu wirken. Jedoch wird die Stabilisierung von Zusammensetzungen, enthaltend unmodifiziertes Taxol, nicht beschrieben. Es besteht ein Bedarf für eine Taxol-Lösung mit verbesserter Stabilität
- Entsprechend wird im Hauptaspekt der Erfindung eine Lösung bereitgestellt, enthaltend Taxol (Paclitaxel) Cremphor ELTM und Ethanol, dadurch gekennzeichnet, daß der pH-Wert der Lösung in einem Bereich von 1 bis 8 durch Zugabe einer Säure eingestellt wurde. Säuren in Form von Pulvern sind bevorzugt, z. B. Zitronensäure, über solche, die Wasser enthalten, z. B. Schwefelsäure. Die besonders bevorzugte Säure zur Verwendung gemäß der vorliegenden Erfindung ist Zitronensäure, aber ein großer Bereich von Säuren kann verwendet werden einschließlich der folgenden:
- Zitronensäure - Monohydrat
- Zitronensäure - wasserfrei
- Zitronensäure - wasserhaltig
- Essigsäure
- Ameisensäure
- Ascorbinsäure
- Asparaginsäure
- Benzolsulfonsäure
- Benzoesäure
- Salzsäure
- Schwefelsäure
- Phosphorsäure
- Salpetersäure
- Weinsäure
- Diatrizoesäure
- Glutaminsäure
- Milchsäure
- Maleinsäure
- Bernsteinsäure
- Wegen seiner begrenzten Löslichkeit in Wasser wird Taxol gewöhnlich hergestellt und verabreicht mit einem Träger, enthaltend Cremophor ELTM (ein polyethoxyliertes Castoröl, das als Lösungsvermittler wirkt) und Ethanol. Eine käufliche Lösung, hergestellt von Bristol-Myers Squibbs (BMS) wird mit diesen Komponenten formuliert und hat einen pH-Wert von 9,1.
- Wie vorstehend beschrieben, lehrt die Erfindung im wesentlichen das Hinzufügen einer Säure zu einer Taxol-Formulierung, um deren pH auf einen Wert im Bereich von 1 bis 8, vorzugsweise 5 bis 7 einzustellen. Eine Ausführungsform der Erfindung ist die Verwendung eines Säuerungsmittels als ein Stabilisator für Paclitaxel in einer pharmazeutischen Zusammensetzung, wobei die Zusammensetzung einen resultierenden pH-Wert von weniger als 8,1 auf weist. Die Erfindung umfaßt ebenfalls die Verwendung eines Säuerungsmittels zur Herstellung eines Medikaments, das Paclitaxel, polyethoxyliertes Castoröl und das Säuerungsmittel umfaßt, wobei das Medikament einen pH-Wert von weniger als 8,1 hat.
- In einem bevorzugten Verfahren, das bei dem Anmelder angewendet wurde, wurden die folgenden Schritte durchgeführt. Das Verfahren ist nicht einschränkend zu verstehen.
- Zitronensäure wurde in absolutem Alkohol aufgelöst, wobei 8 ml absoluter Alkohol zu 1 g Zitronensäure gegeben wurde und die Lösung für 15 Minuten gerührt wurde.
- Cremophor ELTM wurde in das Hauptrührgefäß eingewogen.
- Lösung 1 wurde zu Lösung 2 hinzugegeben und der Behälter, verwendet für Lösung 2, wurde mit einer minimalen Menge von absolutem Alkohol gewaschen, um den vollständigen Transfer der Zitronensäure sicherzustellen. Lösung 3 wurde gemischt und Stickstoff wurde für mindestens 15 Minuten eingeblasen. Das Taxol wurde abgewogen und aufgeschlämmt unter Verwendung von absolutem Alkohol unter Verwendung eines Verhältnisses von absolutem Alkohol zu 1 g Taxol. Das aufgeschlämmte Taxol wurde zu Lösung 3 hinzugefügt und das Aufschlämmungsgefäß wurde mit einer minimalen Menge an absolutem Alkohol gewaschen. Lösung 3 wurde auf 75% des erforderlichen Volumens eingestellt unter Verwendung von absolutem Alkohol und für mindestens 45 Minuten bis zu vollständigen Lösung gerührt. Nach dem vollständigen Auflösen wurde das Volumen überprüft und falls notwendig aufgefüllt mit absolutem Alkohol und die endgültige Lösung wurde für 5 Minuten gerührt.
- Eine Lösung wurde hergestellt mit der folgenden Formulierung:
- Formulierung: (Probe 1)
- Cremophor ELTM 0,5 ml
- Zitronensäure (wasserfrei) 2,0 mg
- Taxol 6,0 mg
- absoluter Alkohol auf 1,0 ml
- Der pH-Wert dieser Lösung wurde mit 6,1 bestimmt.
- Die Stabilität dieser Probe wurde verglichen mit einer Probe, hergestellt durch die Formulierung gemäß der NCI Taxol Clinical Broschüre (wie folgt) mit einem pH-Wert von 9,1 (Probe 2).
- Probe 2 pro ml
- Taxol 6 mg
- Cremophor ELTM 0,5 ml
- absoluter Alkohol auf 1 ml
- Die Lösungen wurden in klare 5 ml Glasgefäße (Typ 1) gefüllt und mit Gummistopfen verschlossen.
- Die Lösungen wurden bei 40ºC 7 Tage gelagert und die Stabilitätsergebnisse sind in Tabelle 1 gezeigt.
- Es ist klar zu erkennen, daß Probe 1 signifikant gestiegene Stabilität gegenüber Probe 2 zeigte.
- Eine Lösung wurde hergestellt mit folgender Formulierung:
- Formulierung: Probe 3
- Cremophor ELTM 0,5 ml
- Taxol 6,0 mg
- absolutes Ethanolauf 1 ml
- pH eingestellt auf 6, 6 mit 1,0 M Essigsäure.
- Die Lösung wurde in klare 5 ml Glasgefäße (Typ 1) gefüllt und mit Gummistopfen verschlossen. Die Lösung wurde für 7 Tage bei 40ºC gelagert.
- Die erhaltenen Stabilitätsergebnisse wurden mit denen der Probe 2 verglichen.
- Wiederum ist die signifikante Stabilitätsüberlegenheit der erfindungsgemäßen Formulierung (Probe 3) offensichtlich.
- Die vorliegende Erfindung ist nicht auf die speziellen Ausführungsformen beschränkt.
Claims (21)
1. Eine pharmazeutische Zusammensetzung, geeignet zur Verwendung bei der
Behandlung von Krebs und umfassend Paclitaxel (Taxol), polyethoxyliertes Castoröl und ein
Säuerungsmittel, wobei die Komponenten der Zusammensetzung in solchen Mengen vermischt
werden, daß die Zusammensetzung einen resultierenden pH-Wert von weniger als 8,1 hat.
2. Pharmazeutische Zusammensetzung gemäß Anspruch 1, wobei der Teil des
Säuerungsmittels derart ist, daß der pH-Wert der Zusammensetzung zwischen 5 und 7 liegt.
3. Pharmazeutische Zusammensetzung gemäß Anspruch 1 oder Anspruch 2 und Ethanol
oder einen Alkohol als einen Bestandteil hiervon einschließend.
4. Pharmazeutische Zusammensetzung, wie in einem der Ansprüche 1 bis 3 beansprucht,
wobei das Säuerungsmittel Ameisensäure, Ascorbinsäure, Aspartatsäure, Benzolsulfonsäure,
Benzoesäure, Hydrochloridsäure, Schwefelsäure, Phosphorsäure, Salpetersäure, Weinsäure,
Diatrizoesäure, Glutaminsäure, Milchsäure, Maleinsäure oder Succinsäure ist.
5. Pharmazeutische Zusammensetzung, wie in einem der Ansprüche 1 bis 3 beansprucht,
wobei das Säuerungsmittel Essigsäure oder Zitronensäure ist.
6. Pharmazeutische Zusammensetzung, wie in Anspruch 5 beansprucht, wobei das
Säuerungsmittel wasserfreie Zitronensäure ist.
7. Pharmazeutische Zusammensetzung, wie in einem der vorhergehenden Ansprüche
beansprucht, wobei das Säuerungsmittel eine Säure in partikulärer fester Form ist.
8. Pharmazeutische Zusammensetzung, wie in Anspruch 1 beansprucht und umfassend:
(i) Paclitaxel,
(ii) polyethoxyliertes Castoröl als ein Lösungsvermittler für das Paclitaxel,
(iii) Zitronensäure als ein Säuerungsmittel, vorhanden in einer Menge, so daß die
Zusammensetzung als ganzes einen pH-Wert von 1 bis 8 hat und
(iv) gegebenenfalls Ethanol.
9. Die Verwendung eines Säuerungsmittels als ein Stabilisator für Paclitaxel in einer
pharmazeutischen Zusammensetzung, in der das Paclitaxel umgesetzt wird durch
polyethoxyliertes Castoröl, wobei das Säuerungsmittel derart und in solchen Teilen verwendet wird, daß
die Zusammensetzung einen resultierenden pH-Wert von weniger als 8,1 hat.
10. Verwendung wie in Anspruch 9 beansprucht, wobei das Säuerungsmittel, wie in einem
der Ansprüche 4 bis 7 beansprucht, ist.
11. Verwendung wie in Anspruch 9 oder 10 beansprucht und einschließend Ethanol als ein
Bestandteil der pharmazeutischen Zusammensetzung.
12. Verwendung wie in einem der Ansprüche 9 bis 11 beansprucht, wobei der Teil des
Säuerungsmittels so ist, daß der pH-Wert der angesäuerten stabilisierten pharmazeutischen
Zusammensetzung zwischen 5 und 7 liegt.
13. Verwendung wie in Anspruch 9 beansprucht, wobei die angesäuerte stabilisierte
pharmazeutische Zusammensetzung umfaßt:
(i) Paclitaxel,
(ii) polyethoxyliertes Castoröl als ein Lösungsvermittler für das Paclitaxel,
(iii) Zitronensäure als das Säuerungsmittel, wobei das Säuerungsmittel in einer Menge
vorhanden ist, so daß die angesäuerte stabilisierte pharmazeutische Zusammensetzung
als ganzes einen pH-Wert von 1 bis 8 hat und
(iv) gegebenenfalls Ethanol.
14. Verwendung eines Säuerungsmittels für die Herstellung eines Medikaments, das
umfaßt Paclitaxel und polyethoxyliertes Castoröl genauso wie das Säuerungsmittel, wobei das
Medikament einen pH-Wert geringer als 8,1 hat und zur Behandlung von Krebs dient.
15. Verwendung von Zitronensäure, Paclitaxel und polyethoxyliertem Castoröl zur
Herstellung eines Medikaments zur Verwendung bei der Behandlung von Krebs, wobei das
Medikament in der Form einer Zusammensetzung ist, umfassend Zitronensäure, Paclitaxel und
polyethoxyliertes Castoröl und einen pH-Wert von weniger als 8,1 hat.
16. Ein Verfahren zur Formulierung einer Taxollösung zur Injektion, wobei das Verfahren
umfaßt:
(i) Mischen eines sauren Materials mit einem Trägermaterial, zur Bildung einer
Trägerlösung und
(ii) Mischen von Paclitaxel mit der Trägerlösung, um eine Paclitaxellösung mit einem pH-
Wert von weniger als 8,1 zu bilden, wobei das Paclitaxel in der Paclitaxellösung nicht
ohne weiteres degeneriert.
17. Verfahren wie in Anspruch 16 beansprucht, wobei die Paclitaxellösung einen pH-Wert
von 5 bis 7 hat.
18. Verfahren wie in Anspruch 16 oder Anspruch 17 beansprucht, wobei das saure
Material ein Säuerungsmittel, wie in einem der Ansprüche 4 bis 7 beansprucht, ist.
19. Verfahren, wie in einem der Ansprüche 16 bis 18 beansprucht, wobei das
Trägermaterial polyethoxyliertes Castoröl umfaßt.
20. Verfahren, wie in Anspruch 19 beansprucht, wobei das Trägermaterial Ethanol
einschließt als ein Bestandteil hiervon.
21. Verfahren, wie in einem der Ansprüche 16 bis 20 beansprucht und den Schritt des
Aufschlämmens des Paclitaxel in Ethanol vor dem Beimischen des Paclitaxel zur
Trägerlösung einschließt.
