技术领域

背景技术
慢性炎性疾病每日影响数以百万计的人。患者进行炎性疾病治疗存 在不同的结果,且在其整个寿命期间存在局限性或全身疾病的复发。尽 管在过去的十年中在免疫学领域已经取得了巨大进展,但是该领域中仍 然存在障碍需要克服,包括,例如通过靶向特异性鉴定疾病的有效且优 选非手术方式改善患者的生活质量。
尽管免疫系统通常提供针对外部病原体的防线,但是仍然存在许多 情况,其中免疫应答自身牵涉疾病进展。因人自身免疫应答导致或恶化 的疾病是自身免疫疾病,例如多发性硬化、红斑狼疮、银屑病、肺纤维 化和类风湿性关节炎;和其中免疫应答促成发病机制的疾病,例如动脉 粥样硬化、炎性疾病、骨髓炎、溃疡性结肠炎、克罗恩病和通常导致器 官移植排斥的移植物抗宿主病(GVHD)。另外的示例性疾病状态包括纤维 肌痛、骨关节炎、肉瘤样病、系统性硬化病、干燥综合征、皮肤炎症(例 如银屑病)、肾小球肾炎、增殖性视网膜病、再狭窄和慢性炎症。
活化的炎性细胞例如巨噬细胞可以在一些情况中促成疾病的发病 机制,导致病原体降解。来自降解的蛋白质的肽类可以展示在所述炎症 细胞表面上,其中它们可以被T细胞识别且它们可以直接与B细胞表面 上的抗体发生相互作用,从而导致T和B细胞活化并且进一步刺激免疫 应答。与炎性疾病状态相关的炎性细胞类型包括巨噬细胞、单核细胞和祖细胞,包括内皮祖细胞。
叶酸受体(FR)是以高度亲和力(<1nM)结合维生素叶酸的38KDa GPI-锚定蛋白。在受体结合后,快速胞吞将维生素递送入细胞,其中它 在低pH下卸入内体隔室。重要地,小分子、蛋白质且甚至主合同与叶酸 共价结合不改变维生素结合叶酸受体的能力,且由此叶酸-药物缀合物6 易于通过受体-介导的胞吞进入细胞。

尽管SPECT扫描并不昂贵,但是与PET扫描相比时,SPECT扫描提 供低分辨率图像。因此,在前期临床阶段评价多种叶酸-靶向的PET成像 剂,包括几种氟-18-标记的试剂。然而,用于PET成像的放射性核素因 其长半衰期[例如86Y(t1/2~14.7h)、64Cu(t1/2~12.7h)和66Ga(t1/2~ 9.49h)]而可能存在剂量测定问题或因其短半衰期[例如11C(t1/2~22 分钟)、13N(t1/2~10min)和15O(t1/2~2min)]导致的就地生产(on-site production)需求。因此,在放射性成像中存在明显缺陷。
由于放射性成像剂可以损伤可能导致癌症的DNA,所以在监测对疗 法的响应时,多剂量放射性成像剂的应用是不期望的。放射性成像剂的 另一个缺点在于患者必须停留在医院,直到放射性成像剂通过身体清除 为止。此外。放射性成像剂可能是昂贵的,可能需要就地生产,因放射 性核素的半衰期而不能储存。因此,对于研发用于检测的更好的和更为 安全的方法存在高度的医学需求。在这方面,光学成像提供了超过放射 成像的优势,例如,易于合成、高纯度、储存期间的长期稳定性、制备 过程中的稳定性和用于其合成和纯化的合理方法。此外,光学成像剂在 临床应用中是安全的。
常规的荧光技术使用在可见光谱(~400-600nm)中的探针。这种波 长是对于使炎性疾病成像并非最佳,因为它与相对高水平的非特异性背 景干扰相关,这归因于组织中的胶原蛋白。因此,来自这些常规化合物 的信噪比低。此外。由生物发色团、特别是血红蛋白吸收可见光将荧光 影像透入的穿透深度限制到几个毫米。因此,组织中埋藏深度高于几个毫米的疾病部位通常仍然无法检测到。
可见光谱(<600nm)中血红蛋白与IR范围(>900nm)内的水和脂质 的光吸收的组合提供了约650-900nm的光学显像窗,其中组织的吸收系 数最小。在可见范围发射光的染料的适合的可替代选择可以用于研发可 以用于近红外(NIR)范围的染料,因为近红外区内的光几乎不诱导自体荧 光并且非常有效地透入组织。近-IR荧光技术的另一个有益性在于来自 从激发源的散射光的背景明显减少,因为散射强度与波长的四次方的倒 数成正比。低背景荧光是高灵敏度检测所必需的。此外,生物组织中近 -IR区(650nm至900nm)内的光学透明观察孔线(optically transparent window)使得NIR荧光成为用于体内成像和亚细胞检测应用的有价值的 技术,所述的体内成像和亚细胞检测应用需要光通过生物成分传送。
尽管光在用于较深组织成像的NIR范围中的应用优先针对可见光谱 中的光,但是目前本领域中使用的NIR成像染料存在大量挑战和缺陷。 它们包括对于光漂白敏感。化学稳定性差、落入与许多生理学分子相同 范围的吸收和发射光谱(导致高背景信号和自身荧光)。此外,大部分NIR 染料在合成过程不稳定,至少并非配体与胺连接基缀合产生多种不需要 的副产物这一事实所致。因此,采用配体-靶向的NIR成像剂可能是昂贵 的。所以,目前使用NIR荧光探针成像的用于治疗炎性疾病的方法存在 湖各缺陷,包括对于深部组织(距离表面>5mm)成像无效;在哺乳动物组 织中定量荧光信号无效和在增加临床前期至临床转换时间的生产成本方 面无效。
使用近红外光谱范围发出荧光的光学从成像剂取得了进展。预先的 进展还包括在约650-约1000nm的NIR-荧光光谱范围使用靶向染料作为 最佳造影剂鉴定炎性疾病,包括类风湿性关节炎。这包括使用吲哚三碳 氰化物(indotricarbocyanides)类花菁染料,例如已知在心血管诊断和 肝功能试验中有效量的吲哚花青绿(ICG)。尽管已经证实非靶向荧光染料 被动地蓄积在一些组织中,但是得到的组织与背景比通常较差且炎症部 位与健康组织之间的边界可能难以确定。
目前在NIR-荧光方面的研究已经产生了炎性疾病靶向成像的新方 法。研究了体内蛋白酶-活化的NIRF探针通过靶向特定裂解部位并且导 致荧光活化在使关节中存在的胶原诱发性关节炎成像中的能力(Andreas Wunder等人Arthritis&Rheumatism,50(8):2459-2465(2004))。另 外,测试了近期研发的NIR2-叶酸缀合物通过靶向发炎细胞中活化的巨 噬细胞上丰富的叶酸受体使关节炎体内成像(Wei-Tsung Chen等人 ArthritisResearch&Therapy,7(2):R310-R317(2005))。
然而,尚未研究用于靶向炎症发病疾病组织的包含带有连接基的可 选叶酸衍生物作为配体以制备更好和更明亮染料的NIR荧光染料-连接 基-配体缀合物。
因此,对于基于可选叶酸衍生物的染料物质的应用仍然存在需求, 其可以用于特异性地靶向炎症发病状态并且增加在体内成像中应用的稳 定性和光亮性。
发明内容
本文提供用于治疗炎性疾病的方法,通过使用有效量的氨基酸连 接基来进行,所述氨基酸连接基与用于发炎组织和淋巴结靶向成像的化 合物缀合。
在本发明的一个实施方案中,本文涉及使炎性疾病成像的方法, 包括以下步骤:a)对需要这种方法的患者施用有效量的能够结合炎性 细胞的化合物或其药学可接受的盐或同位素,该化合物具有下式:

所成像的炎性疾病可以选自溃疡性结肠炎、类风湿性关节炎、肺 纤维化和动脉粥样硬化。
化合物的施用可以通过任意常规使用的途经进行。例如,可以通过 静脉内(I.V.)注射、腹膜内(I.P.)注射、口服施用等进入炎性疾病侵害 的身体部分来进行。关注施用途经可以是局部、肠或胃肠外。在本发明 的第二个实施方案中,本文提供使炎性疾病成像的方法,包括对需要有 这种方法的患者施用有效量的化合物,该化合物具有下式:

此外,荧光强度大于使用其它对于FR同种型使用叶酸靶向的近红 外染料预先观察到的强度。这种增强的强度允许靶向和清楚地鉴定来自 所监测的组织的较小的生物样品区域(例如较小的发炎区域)。此外,本 文的化合物和成像方法的增加的强度提供附加的优势在于可以施用较低 剂量/用量的染料且仍然产生有意义的结果。因此,使用本文的化合物使 炎性疾病成像的方法产生了更经济的成像技术。此外,存在附加的优点, 即于常规成像化合物相比本文的化合物的剂量较低将因给身体施用外部 物质附带的毒性和其它副作用减少到最低限度。
此外,鉴定炎症侵害的组织导致对卫星病毒疾病(satellite disease)的早期、更精确和更有效的诊断、成像和鉴定。这些优点各自 与所治疗患者的更好的临床结果积极相关。
在具体的实施方案中,发现赖氨酸作为连接基的应用导致近红外荧 光缺失。然而,关注除酪氨酸和酪氨酸衍生物外,具有半胱氨酸或半胱 氨酸衍生物的近红外染料的蝶酰基化合物液可能是有用的。此外。关注 蝶酰基或叶酸部分与染料直接连接或染料与蝶酸或叶酸通过胺连接基连 接也产生了来自化合物的荧光强度缺失,而在蝶酰基(靶向部分)与近红 外染料(荧光部分)之间存在酪氨酸或酪氨酸衍生物作为连接部分对于维 持或增强缀合化合物的荧光是有益的。本文的基于酪氨酸的化合物无需 额外的胺连接基连接至化合物S0456且还因为缀合通过酪氨酸的酚部分 缀合导致荧光增强。
所述化合物可以与荧光介导的分子体层造影成像系统一起使用,例 如为在深部组织中检测近红外荧光活化设计的那些。所述化合物提供分 子和组织特异性、产生高荧光对比、更明亮的荧光信号和减少背景自体 荧光,从而使得体内患病组织(例如癌症或炎性疾病)的早期检测和分子 靶标评价改善。所述化合物可以用于深部组织三维成像、靶向手术和用 于对生物样品中的靶细胞类型的量定量。
缀合化合物

化合物的合成

然而,在特别优选的实施方案中,本文提供用于生成本文所述化合 物(即式I的化合物)的更有效的合成方法,包括具有下式的化合物:

反应路线I示例预先用于生成式I的化合物的合成反应路线,其中 目标配体包含芳族蝶呤部分,例如叶酸或蝶酸。反应路线I特别示例了 式I的化合物,其中目标配体包含通过氨基酸(赖氨酸)连接至染料分子 的叶酸。简言之,通过连接至氨基酸的氨基修饰的叶酸配体与染料的桥 连的醚衍生物在产生产物(3)和(4)的条件下反应。然而,值得注意的是, 化合物3是优选期望的化合物,而合成路径导致存在不期望的副产物4 作为主要产物,其不具有NIR特性。此外,其光谱特性是pH依赖性的。 因此,该反应路线显示醚桥连的染料的主要缺陷。在这些染料的常规生 产过程中,30-60%的收率是期望的产物的收率,而40-70%的收率是不 期望的副产物的收率。
反应路线II提供合成途经,其包括仅3个反应步骤且提供高收率 的产物化合物(5)(高于98%)。简言之,将靶向配体(1)(示例在反应路 线II中,带有蝶酰基)和任选地包括保护基以避免不期望的与非氨基酸 的氨基的基团的反应的氨基酸或氨基酸衍生物(2)在HATU[(O-(7-氮杂苯 并三唑-1-基)-N,N,N′,N′-四甲基脲六氟磷酸盐)]/DIPEA(二异丙基 乙胺)/DMF(二甲基甲酰胺)溶剂系统中混合,在室温反应足够时间(5分 钟),以使(2)通过氨基官能团与配体(1)偶合,得到(3)。可以通过将稀 酸添加到该反应混合物包括其它溶剂、例如二甲亚砜(DMSO)中有利地沉 淀化合物(3)。更具体地,用1N HCl(盐酸)沉淀化合物3,得到最终化 合物,纯度超过98%,在这些实施方案中,避免了使用昂贵的HPLC或柱 色谱步骤。通过使化合物在室温下在TFA(三氟乙酸):水:TIPS(三异 丙基硅烷)溶剂系统中反应使化合物(3)反应以除去化合物的氨基酸部分 上的保护基,得到化合物(4)。通过用乙醚或甲基叔丁基醚沉淀纯化化合 物4,在不进行HPLC(高效液相色谱法)或柱色谱法的情况下得到超过 98%的纯度。使化合物(4)在碱水系统(例如NaOH,氢氧化钠)中反应,以 便除去保护基官能团,随后在稍摩尔过量的情况下与染料(S0456)在水中 在70-100℃温度下反应15分钟时间,以允许染料与(4)偶合,得到最终 化合物(5)。用丙酮沉淀化合物5,得到超过98%纯的Pte-Tyr-S0456。 当使用NaOH时,产生Pte-Tyr-S0456的钠盐。
反应路线II

成像方法
正如上文注意到的,对于特异性地靶向至组织内的区域的近红外染 料化合物存在需求。即所述化合物可以用于成像技术并且辅助炎性疾病 的诊断和治疗干预。正如上文详细讨论的,本文提供的化合物用作染料 和NIR光谱区的中的成像剂。照此,所述化合物广泛适用于众多成像、 诊断和靶向治疗方法。
在具体的实施方案中,本文涉及治疗炎性疾病的方法,保护对需要这种方法的患者施用有效量的并入本文公开的化合物的至少一种的化合物(例如式I的化合物可以用于特异性地和灵敏地鉴定组织内的炎性疾病。在其它实施方案中,本文涉及命名为Pte-Tyr-S0456(OTL-0038)的化合物在诊断和成像中的用途,所述诊断和成像用途涉及影像引导的1) 交杯酒成像、2)炎性疾病、3)动脉粥样硬化、4)感染性疾病和/或5) 手术以及其它用途。在其它方面,Pte-Tyr-S0456衍生物可以是 Pte-D-Tyr-S0456、Pte-homo-Tyr-S0456、Pte-β-homo-Tyr-S0456、 Pte-(NMe)-Tyr-S0456、Pte-Tyr(OMe)-S0456、Pte-Tyr(OBn)-S0456、Pte-NHNH-Tyr-OAc-S0456、其盐或衍生物。按照这种方式,本文的化合 物可以用于荧光引导的全身炎症成像,如果必要,还有疾病状态、淋巴 结等的手术切除。或者,本文的化合物易于用于全身成像,其中将所述 化合物施用于受试者并且荧光定位有利于鉴定发炎组织或癌症部位。

具有600nm-850nm波长范围的光属于光谱的近红外范围,与可见 光相对比,可见光属于约400nm-约500nm的范围。因此,用于实施本 文的诊断方法的激发光包含至少一种波长的光以便在红外波长处照射组 织,从而激发化合物以便得自吸收本文的化合物的区域的荧光清楚地显 现且不同于周围组织的自体荧光。按照这种方式,本文的化合物是有利的,因为它们消除了对于使用过滤机制的需求,所述过滤机制可以用于 得到期望的诊断图像,只要荧光探针是在低于约600nm的波长处发出荧 光。按照这种方式,本文的化合物避免了模糊的诊断图像,这些模糊的 诊断图像是作为可以从健康组织反射出的并且导致荧光图像分辨率缺失 的波长的激发光的结果产生的。
可以给诊断实验室、临床医师诊室和外科手术用手术室配备高架 光,这些高架光产生在用于实施本文的诊断方法的光学发射光谱中的光 波长,例如产生适合的波长的光的灯。这种光仅可以用于通过在手术室 中切换成其它光(以消除可能在研究中从身体部分的组织中反射出的可 见的外部光)并且将近红外波长的激发光照入体腔或实施开放式手术来 实施本文的诊断方法,使得观察者(例如外科医师)的眼直接接收的荧光 图像主要是来源于视野中的荧光团的荧光图像。来源于600nm-850nm 范围、优选750nm-850nm范围的来源的光可以用于由观察者直接达到显 影的目标,使得从身体部分反射出的而非来自发射荧光的部分的光被减 至最少或被消除。
因此,使患病组织(和结合的或吸收的靶向构建体)"暴露"于激发光 (例如通过手术生成的开放区或光的内窥镜递送至内部位置。这些成像方 法的公开特别适合于位于受试者的体内部位上的患病组织的体内检测, 例如天然体腔内或手术生成的开放区域内,其中患病组织是"平坦的视图 "形式(即暴露于人眼)以有利于活组织检查操作或手术切除因吸收本文 的化合物而突出显示的区域。因为患病或发炎的组织的精确位置和/或表 面积易于通过本文的化合物的吸收确定,所以使用本文的化合物的方法 为病理学者、免疫学者、技师和外科医师等提供了有价值的指引,他们 需要实时"观察"到发炎区域的肿块的确切外形、大小等,以便诊断和成 像,如果必要,用于手术。

按照类似方式,本文的化合物用于鉴定生物样品中的靶细胞类型, 通过使生物样品接触这种化合物来进行,其接触时间和条件允许该化合 物结合所述靶细胞类型的至少一种细胞。然后对于结合的化合物进行光 学检测,使得存在的来源于本文的结合的靶向化合物的近红外波长的荧 光表示靶细胞类型存在于生物样品中。该方法由此提供所评价的组织内 靶向的靶向细胞类型的图像。最优选地,所述靶向细胞类型是活化的炎 性细胞,包括、但不限于巨噬细胞、单核细胞和祖细胞,包括内皮祖细 胞。
本文公开的有效量的缀合化合物的最适合的施用途经根据所治疗 的疾病状态或所诊断的疑似病症的位置的不同而改变。它包括、但不限 于胃肠外,例如通过皮内、皮下、肌内、腹膜内或静脉内。在其它实施 方案中,可以通过其它医学上有用的方法将所述缀合物施用于患者,且 可以使用任意有效剂量和适合的治疗剂型,包括延长释放剂型。示例性地,本文所述的方法可以于生物疗法联用,例如其它免疫疗法,包括、 但不限于单克隆抗体疗法、使用免疫调节剂治疗和预防接种。例如,为 了治疗炎性病症,局部施用、包括通过直接注入激发光照射的身体部位 (例如通过腔内)提供了如下优点:可以以高浓度施用靶向构建体(例如荧 光标记的抗体),没有可能伴随其全身施用的并发症风险。然而,也关注 口服、局部和胃肠外应用。
当施用含本文的化合物的组合物时,这些方法有利地提供对受试者 的炎性疾病实施图像引导的诊断和治疗的改进的方法,所述施用的条件 和时间足以使所述化合物蓄积在指定组织部位上,这有助于临床医师使 所治疗的组织显影。
如果推定的患病部位是天然体腔或手术产生的体内部位,则可以任 选内窥镜装置用于将激发光递送至该部位,接收来源于体腔内该部位的 荧光并且辅助来自患病组织的荧光的直接图像形成。例如,内窥镜装置 中的镜头可以聚焦于检测的荧光,因为有助于图像形成。认为本文所用 的这种内窥镜递送的荧光是由医师"直接观察的",且靶向寡聚体结合或 其中吸收它的组织对于内窥镜必需是"平坦的视野"形式,因为用于本文 的诊断方法的光不含透入组织的光波长,例如近红外范围的波长。或者, 可以通过任意便利的方式将激发光导入体腔或包含如本文所述施用的靶 向构建体的手术开放区域,且由此产生的荧光图像可以无来自内窥镜的 辅助下由观察者的眼直接观察到。使用或不使用来自任意类型的内窥镜 装置的辅助,通过本文方法产生的荧光图像使得可以在没有图像加工装 置的辅助下观察到该荧光图像,所述图像加工装置例如CCD照相机、TV 检测器、光子采集装置等。
关注本文的诊断或成像方法能够使得外科医师/医师通过开放式手 术同时观察/观测/显示患病或异常组织,以有利于活组织检查操作或手 术切除。因为患病组织的部位和/表面区域易于通过本文的使用本文所述 的化合物的诊断方法测定,所以本文的方法对于外科医师而言是有价值 的引导,所述外科医师需要知晓肿块的确切外形、大小等,以便在手术 进行时切除。特别地,注意到本文的化合物在近红外范围发出强度大于 预先所述的荧光。照此,有利地,关注需要较少的该化合物实现诊断成 像。此外,本文的化合物可以透入深部组织且由此本文能够有利地提供 采取对所述炎性疾病适当治疗的更大的精确度。
在一些实施方案中,在一些实施方案中,单一类型的荧光部分依赖 于从照射的身体部分生成的荧光(即来自结合患病组织或被其吸收的荧 光靶向构建体)和是所述靶向构建体接触来自近红外光谱的光源。
在其它实施方案中,关注多种(即两种、三种、四种或多种)靶向构 建体用于得到诊断图像。这种另外的靶向构建体可以是不同于第一种这 样的本文的另外的化合物。或者,所述另外的靶向构建体可以包含本文 所述的染料,但蝶酰基部分被非叶酸受体的另一种受体的配体替代。在 其它实施方案中,所述另外的靶向部分可以是其它的发出荧光的靶向构 建体(例如抗体或其生物活性片段,其连接荧光团),它们结合所成像的 肿瘤或组织上的其它受体或抗原(例如动脉粥样硬化、感染、心血管疾病、 神经变性疾病、免疫疾病、自身免疫疾病、呼吸系统疾病、代谢病、遗 传疾病、感染性疾病、骨病和环境性疾病等的部位)。可以使用特异性靶 向肿瘤或组织上的特定部位的任意另外的靶向部分,只要它对于所监测 的部位具有特异性。另外的发出荧光的靶向构建体的目的在于增加荧光 在所监测部位上的强度,由此有助于检测身体部位中的患病或异常组织。 例如,指定患病组织可以具有大量标记,且除本文的化合物外,还提供 荧光部分的混合物,其对指定部位具有特异性,使得来源于该组织的信 号由一种以上的靶向所关注的组织部位且定位于该部位的化合物或荧光 部分生成。
实际上,本领域技术人员可以单独地或作为靶向可检测部分的混合 物施用本文的化合物并且能够使得这些化合物和靶向部分结合可以存在 于研究部位上的任意靶向组织和/或被其吸收,且然后提供光源供给。典 型地,本文的化合物和任意另外的靶向部分可以在手术前施用一定时间, 并且可以在允许本文的荧光化合物以及任意另外的荧光构建体被靶组织 吸收的组合物形式施用一定时间。
本领域技术人员能够设计依次施用的发出荧光的靶向构建体的组 合,它们各自可以特异性地结合靶部位。优选用于这种混合物的发出荧 光的用于鉴定靶组织的所有靶向构建体包含与本文化合物相同波长带内 或相同波长处的荧光团(例如对于本文的化合物中的近红外光波长敏感 性地发出荧光),以便将需要用于同时激发来自用于实施本文方法的所有 不同靶向构建体的荧光的不同光源数量减至最少。然而,关注非本文的 化合物的另外的靶向部分可以发出荧光作为对与本文的荧光化合物不同 的颜色的照射光(即具有不同波长)的响应。来源于本文的化合物和另外 的靶向化合物的荧光的颜色差异可以有助于观察者确定患病组织的位置 和大小。在一些实例中,期望在靶向至靶正常组织的靶向构建体中包括 荧光团和靶向患病组织的本文的化合物,使得患病组织与正常组织之间 的对比进一步得到增强以便有助于观察者确定靶组织的位置和大小。除 本公开为文本的化合物外这种另外的荧光团和靶向剂的应用提供如下优 点:来源于正常组织的任意天然荧光被来源于靶向至身体部分内靶组织 的补充靶向构建体中的荧光团的荧光掩蔽。来源于正常与靶组织的荧光 之间的颜色差异越大,则对于观察者而言使靶组织的外形和大小的显影 越方便。例如,包含对靶组织(即异常组织)产生来自本文的化合物的红 外光的荧光团和对健康组织产生绿色光的荧光团的发出荧光的靶向构建 体有助于观察者区分靶组织与正常组织。本领域技术人员易于选择提供 不同可见颜色对比的荧光团的组合。
选择用于实施本文方法的光的光谱以包含至少一种相当于靶向构 建体或靶向构建体内包含的生物相容性发出荧光的部分的主要激发波 长。一般地,用于实施本文方法的激发光包含约600nm-约850nm范围 的近红外波长光的至少一种波长。
然而,当在不同波长处发出荧光的靶向配体的组合用于在本文中实 施时,激发光的光谱必须宽至足以提供用于所用荧光团各自的至少一种 激发波长。例如,当选择不同颜色的荧光团以区分正常与患病组织时, 特别有益的是光的是激发光谱包括靶向至正常和靶组织的荧光团的激发 波长。
正如本文注意到的,本文的化合物通过作为本文的化合物的部分的 蝶酰基或叶酸配体特异性地靶向至叶酸受体。在其中使用另外的靶向部 分的实施方案中,选择这种另外的靶向部分的靶向构建体以结合所关注 靶组织和/或特异性地被其吸收,例如结合特征在于靶组织内的疾病或异 常状态的细胞上或其内的抗原或其它表面特征。正如在其它诊断试验中, 期望靶向构建体选择性地结合靶组织或被其吸收或结合与疾病或异常状 态相关的抗原;然而,包含也结合健康组织或细胞结构或被其吸收的配 体部分的靶向构建体可以用于实施本文的方法,只要在视野中靶组织中 的抗原浓度或靶向构建体对靶组织的亲和力足够大于对健康组织,使得 代表靶组织的荧光图像可以清楚地在视野中显示为不同于来源于健康组 织或结构的任意荧光。
本文方法检测的疾病或异常状态可以是任意类型,其特征在于对于 已知特异性结合配体存在已知的靶组织。例如,不同心脏病的特征在于 对于已知特异性结合配体产生坏死或缺血性组织或产生动脉粥样硬化组 织。关注靶组织的特征可以在于产生针对已知结合配体的表面抗原或胞 内标记(即抗原)的细胞,因为许多靶向构建体透入细胞膜。可以使用本 文方法鉴定的有代表性的疾病状态包括例如不同类型的肿瘤、细菌、真 菌和病毒感染等这样的不同病症。本文所用的"异常"组织包括癌前状态、 坏死或缺血性组织和与结缔组织疾病和自身免疫疾病相关的组织等。此 外,适合于使用本文方法诊断或检查的靶组织的类型的实例包括心脏、 乳腺、卵巢、子宫、肺、内皮、血管、胃肠道、结肠直肠、前列腺组织、 内分泌组织等及其任意两种或多种的组合。
单纯地作为实例,一些常见恶性肿瘤的抗原和通常发现它们的身体 部位是本领域技术人员已知的,且靶向配体例如抗体或用于这些抗原的 抗体或实际上其中抗原是受体的配体是本领域已知的。
用于实施本文方法的靶向构建体和补充靶向构建体可以通过本领 域技术人员公知的任意途经施用,例如通过局部、关节内、池内、眼内、 心室内、向鞘内、静脉内、肌内、腹膜内、皮内、气管内、腔内等以及 通过它们的任意两种或多种的任意组合。
最适合的施用途经根据所治疗的疾病状态或所诊断的疑似病症或 肿瘤的位置的不同而改变。例如,为了使炎性病症和不同肿瘤成像,局 部施用包括通过直接注入激发光照射的身体部位施用(例如通过腔内)提 供如下优点:可以以高浓度施用靶向构建体(例如荧光标记的抗体),没 有可能随其全身施用附带的并发症风险。
可以以"有效量"施用本文的化合物和包含本文的化合物的诊断混 合物中使用的任意另外的靶向构建体用于诊断。有效量是有助于使位于 受试者的研究中的身体部分内的任意靶组织直接显影必不可少的靶向构 建体用量。关注作为本文所用的术语"受试者"包括任意的哺乳动物,例 如家养宠物、农场动物或动物园动物,但优选是人。有效地用于诊断应 用的量当然根据所研究的身体部分的大小和位置、靶向构建体对于靶组 织的亲和力、靶组织的类型以及施用途经的不同而不同。靶向构建体的 局部施用典型地需要小于任意全身施用模式的剂量,不过,在一些情况 中,靶向构建体的局部浓度在局部施用后高于全身施用时安全达到的浓 度。
所施用的缀合化合物的有效量取决于患者病情、所治疗的疾病状 态、缀合物的分子量、其施用途经和组织分布以及与例如放疗这样的治 疗共同应用的可能性。施用于患者的有效量基于体表面积、患者体重和 临床医师对于患者病情的判断。在不同的示例性实施方案中,使用或不 使用赋形剂/载体执行有效剂量,包括、但不限于盐水。由于个体受试者可能存在症状的严重性的宽范围改变,且每种靶向构建体具有其独特的 诊断特征,包括靶向构建体对靶标的亲和力、靶向构建体被身体过程的 清除速率、包含在其中的荧光团的特性等,所以本领域技术人员可以加 权这些因素并且由此改变剂量。
可以根据公知方法、使用适合的分散剂或湿润剂和助悬剂将本文的 化合物和包含这些化合物的混合物配制成无菌可注射混悬液。无菌可注 射制剂还可以是在无毒性的胃肠外可接受的稀释剂或溶剂中的无菌可注 射溶液或混悬液,例如为在1,4-丁二醇中的溶液。无菌固定油常用作溶 剂或助悬介质。对于该目的,可以使用任意温和的固定油,包括合成单 酸甘油酯类或二酸甘油酯类、脂肪酸(包括油酸)、天然存在的植物油如 芝麻油、椰子油、花生油、棉籽油等或合成脂肪媒介物如油酸乙酯等。 可以根据需要掺入缓冲剂、防腐剂、抗氧化剂等,或它们可以包含在制 剂中。
提供下文的实施例仅用于示例本文的具体实施方案的目的,而不预 以限定待批权利要求的范围。正如本文所讨论的,可以按照不同方式改 变所公开的化合物和方法的具体特征,这些不同方式所提供的不一定具 有可操作性或优点。例如,化合物可以并入各种氨基酸和氨基酸衍生物 以及靶向配体,这取决于所用化合物的具体用途。本领域技术人员可以 理解,这种变型包括在待批权利要求范围中。
实施例
实施例1:使用Pte_L_Tyr_S0456(OTL-0038)的溃疡性结肠炎的 体内研究:
给维持叶酸缺乏膳食的7周龄Balb/c小鼠(Harlan Laboratories) 施用5%的在其饮用水中的葡聚糖硫酸钠(DSS)以诱发溃疡性结肠炎。健 康对照组小鼠接受标准饮用水。
8天后,给小鼠经腹膜内注射10nmol OTL-0038。4小时后,对小 鼠实施安乐死,并且使用IVIS lumina II成像(如图1A-C中所示)。
实施例2:使用Pte_L_Tyr_S0456(OTL-0038)的胶原诱发性关节炎 (CIA)(作为用于类风湿性关节炎的模型)的体内研究:
给6-7周龄雌性DBA/1小鼠(Jackson Laboratories)维持叶酸缺乏 膳食(Harlan-Teklad)。给小鼠尾底部免疫接种100μg在弗氏完全佐剂 中乳化的牛II型胶原蛋白(Chondrex,Inc.,Redmond,WA,USA)。然后 在21天后给小鼠加强类似注射100μg在弗氏不完全佐剂中乳化的牛II 型胶原蛋白。4天后(第25天),在具有腹膜内注射溶于盐水的25μg脂多糖(LPS)的所有小鼠中同步发作关节炎。健康对照组小鼠不进行关节炎 诱发。
在第39天时,小鼠接受腹膜内注射10nmol OTL-0038。4小时后, 对小鼠实施安乐死,并且使用IVIS lumina II成像(如图2A-2B中所示)。
实施例3:使用Pte_L_Tyr_S0456(OTL-0038)的动脉粥样硬化的体 内研究:
5周龄雄性ApoE-/-小鼠购自Jackson Laboratories并且置于可调 解的热量膳食上(42%来自脂肪,Harlan Laboratories)。给健康对照组 小鼠维持正常动物膳食。
20周后,小鼠接受i.p.注射10nmol OTL38。4小时后,对小鼠实 施安乐死,并且使用IVIS lumina II成像(如图3A-3B中所示)。
实施例4:使用Pte_L_Tyr_S0456(OTL-0038)的肺纤维化的体内研 究:
用异氟烷麻醉维持叶酸缺乏膳食的6周龄雌性C57BL/6小鼠 (HarlanLaboratories),并且将溶于盐水的50μL博来霉素(2U/kg体 重)经气管内输注入每只小鼠。健康对照组小鼠类似地经气管内输注50 μL盐水。
第15天时,小鼠接受i.p.注射10nmol OTL38。4小时后,对小鼠 实施安乐死,并且使用IVIS lumina II成像(如图4中所示)。
本领域技术人员显而易见,可以在不脱离其精神和范围的情况下 对本文进行各种变型,因此,本文除本说明书特别公开的那些外还包括 仅在待批权利要求中所示的实施方案。
Claims (27)
1.具有下式的化合物或其药学上可接受的盐或同位素在制备炎性疾病成像剂中的用途,
其中:
X选自酪氨酸和酪氨酸衍生物,且
Y是在近红外(NIR)范围具有荧光激发和发射光谱的染料,其中Y由下式表示:
其中,X′独立地选自O、S、N和C,
R′独立地选自CH2和CH2CH2,且
Y通过与X的酪氨酸侧链的酚部分形成醚键而连接到X上,
所述化合物维持或增强染料的荧光,且所述化合物结合炎症细胞。
2.权利要求1的用途,其中Y选自LS288、IR800、SP054、S0121、KODAK、IRD28、S2076、S0456及其衍生物。
3.权利要求2的用途,其中Y是S0456。
4.权利要求1的用途,其中所述炎症细胞选自活化的巨噬细胞、单核细胞和祖细胞。
5.权利要求4的用途,其中所述炎症细胞是活化的巨噬细胞。

16.具有下式的化合物在制备炎性疾病成像剂中的用途,
其中W、X、Y、Z各自是H+、Na+、K+或NH4 +,
