技术领域
本发明涉及检测、诊断、成像和治疗受试者的癌症的方法。
发明背景
胆囊收缩素B受体(也称为CCK2R、CCKBR和胃泌素受体)是一种跨膜G蛋白偶联受体。CCK2R主要在(i)大脑和中枢神经系统,以及(ii)胃肠道粘膜,包括壁细胞和ECL细胞中表达。据报道,它在哺乳动物大脑皮层中的表达最为丰富,在那里其与记忆/学习和对应激的反应的调节有关。在消化道中,已经在GI组织以及外分泌和内分泌胰腺中检测到了它的存在。相互矛盾的报道表明,该受体与CCK1R—一种与CCK2R有50%同源性的G蛋白偶联受体也一起在外周组织如脂肪细胞中表达。CCK2R的天然配体是胃泌素和胆囊收缩素肽。
已经发现许多不同类型的人肿瘤以高密度或以高频率过表达或异位表达CCK2R,这些肿瘤包括甲状腺髓样癌、胰岛素瘤、小细胞肺癌、支气管和回肠类癌、GIST肿瘤和结肠癌、肝细胞癌和胰腺癌。CCK2R信号传导的失调可能有助于肿瘤形成或生长的一种方式与胃泌素通过受体的信号传导有关。胃泌素是一种肽激素,其在结合CCK2R后刺激胃壁细胞分泌胃酸(HCI)。已经报道了胃泌素是癌细胞凋亡的抑制剂,可能是通过其诱导丝氨酸/苏氨酸蛋白激酶PKB/Akt活化的能力10。几项报道已经显示,抑制CCK2R受体和胃泌素自分泌环诱导细胞凋亡并在体内抑制癌细胞系的增殖。
已经报道,在肿瘤组织中发现的CCK2受体包括正常蛋白质,以及组成型活性的CCK2R受体剪接变体(CCK2i4svR),其具有不适当保留的第四内含子,导致在受体的第三个细胞内环结构域中添加69个氨基酸,已知该结构域对信号传导很重要。
靶向CCK2R的细胞毒剂可能用于阻断肿瘤中受体的不受控制的激活。有益的是,由于CCK2R在正常脑组织中表达,血脑屏障将阻断可能影响大脑中受体的正常活动的极性化合物。因此,在CCK2R及其剪接变体通过肿瘤在脑外表达的情况下,该肿瘤将是CCK2R和CCK2i4svR特异性细胞毒剂靶向的唯一组织。
已经开发出CCK2R特异性拮抗剂,并且已经报道了广泛的结构-活性关系。体外和体内研究均表明,在选择性CCK2R拮抗剂存在下,胃泌素的生长增强作用可以被消除。一种这样的拮抗剂是Z-360—一种口服活性的CCK2R拮抗剂(Zeria Pharmaceuticals Co.,Ltd.,Tokyo,Japan),其Ki=0.47nmol/L,相对于CCK1R的选择性比率=672。临床前研究已经表明,与单独使用任一种药剂相比,Z-360与化疗剂吉西他滨一起口服给予显著抑制皮下PANC-1肿瘤生长(27.1%抑制),并且与载体相比,显著提高生存率。该拮抗剂与吉西他滨的联合治疗目前处于胰腺癌治疗的II期人临床试验中。
选择性靶向CCK2R的其它药剂的开发将拓宽可用于治疗其中表达CCK2R的肿瘤的治疗药物武器库。这些药剂也可用于受试者的癌症的诊断和成像。
发明简要概述
本发明涉及一小组高度相关的配体,其选择性地靶向并结合CCK2R和剪接变体CCK2i4svR。所述配体与NIR成像剂连接,并且这些化合物用于表达CCK2R和剪接变体CCK2i4svR的肿瘤的治疗、诊断和成像。

在本发明的一些实施方案中,红外光波长在约600至1000纳米范围内。在一些实施方案中,本发明的方法使用的红外光波长在约670至850纳米范围内。
本发明的一些实施方案提供了诊断受试者的疾病的方法,包括:
a)向需要诊断的受试者给予一定量的CCK2R和/或CCK2i4svR靶向NIR染料化合物一段时间,并且是在允许所述化合物与至少一种表达CCK2R或CCK2i4svR或两者的细胞结合的条件下;
b)测量来自生物样品中存在的化合物的信号;

在所提供的方法的另一个实施方案中,表达CCK2R和/或CCK2i4svR的癌细胞是肿瘤。在所提供的方法的又一个实施方案中,表达CCK2R和/或CCK2i4svR的癌症是肿瘤。

在一个说明性实施方案中,所述接头L可以是可释放的或不可释放的接头。在一方面,接头L的长度为至少一个氨基酸或4个原子。在一个变型中,接头L的长度为至少约10个原子。
在供选择的方面,接头L的长度为至少约5埃。

本发明基于以下发现:有用的成像剂(活性部分)可以通过接头与选择性地结合CCK2R和剪接变体CCK2i4svR的靶向配体复合。通过不懈的努力,本发明人开发了接头并鉴定了靶向配体上的位置,活性部分可以连接该位置而不会干扰靶向配体选择性结合CCK2R和/或CCK2i4svR的能力。因此,本发明人开发了包含与活性部分连接的靶向配体的化合物,其中靶向配体是选择性结合至CCK2R和/或CCK2i4svR的2,3,4,5-四氢-1H-1,4-苯并二氮卓化合物,并且活性部分是治疗剂或成像剂。

III.检测、诊断和成像方法

IV.药物制剂
使用所述化合物的本发明方法可以在一种或多种体外、体内和离体应用中实施。当体内和离体使用时,所述化合物可以制备成包含所述化合物和一种或多种药学上可接受的载体和/或稀释剂的药物组合物。技术人员将理解,包含药物组合物的具体要素将取决于包括化合物中靶向配体和活性部分的特性(identity)、待检测或治疗的癌症或肿瘤的特性、癌症或肿瘤在受试者中的位置、检测成像剂的可用手段,以及用于向受试者给予化合物的方法的因素。
本发明的药物组合物可以配制成,例如,用于口服、舌下、鼻内、眼内、直肠、透皮、粘膜、肺部、局部或肠胃外给予。肠胃外给予方式包括但不限于皮内、皮下(s.c.,s.q.,sub-Q,Hypo)、肌肉内(i.m.)、静脉内(i.v.)、腹膜内(i.p.)、动脉内、髓内、心内、关节内(关节)、滑膜内(关节液区)、颅内、脊柱内和鞘内(脊髓液)。用于药物组合物的肠胃外注射或输注的任何已知装置可用于实现这种给予。
肠胃外剂型的实例包括化合物在等渗盐水、5%葡萄糖或其它众所周知的药学上可接受的液体载体如液体醇、二醇、酯和酰胺中的水溶液。根据本发明的肠胃外剂型可以是包含一定剂量化合物的可重构冻干物的形式。在本实施方案的一个方面,化合物可以配制成延长或延迟释放制剂,如第4713249、5266333和5417982号美国专利中描述的可生物降解的碳水化合物基质,其中每一篇的公开内容都过引用的方式整体并入本文,供选择地可以使用缓释泵(例如,渗透泵)。

VI.给予受试者
A.检测、诊断和成像方法
本文所述的检测、诊断和成像方法中使用的化合物的量还将取决于多种因素,包括靶向配体的特性、成像剂的特性、待检测的癌症或肿瘤的特性、癌症或肿瘤在受试者中的位置、用于检测成像剂的可用的手段、以及用于向受试者给予化合物的方法。
技术人员将理解,检测、诊断和成像的方法中的每一种都可以使用一种类型的化合物或多于一种,如两种、三种、四种或更多种类型的化合物来实施。当使用两种或更多种类型的化合物时,所述方法可以通过向受试者同时或顺序给予两种或多种类型的化合物来实施,根据所实施的方法,时间间隔为5、10、15、20、25、30或更多分钟。举例说明,例如,可以给予受试者在共同给药方案中具有不同靶向配体但具有相同活性部分的化合物。在其它实施方案中,可以给予受试者包含与不同活性部分连接的相同靶向配体的化合物,或包含与不同活性部分连接的不同靶向配体的化合物。
当用于检测、诊断和成像方法时,优选将化合物肠胃外,例如皮内、皮下、肌肉内、腹膜内、静脉内或鞘内给予受试者。然而,技术人员将理解,在一些情况下,给予可以是口服、舌下、鼻内、眼内、直肠、透皮、粘膜、肺或局部的。
除了检测所述化合物之外,技术人员将理解,可以将检测到的信号量与其它值,如与来自已知没有肿瘤或癌症的受试者的对照值,或者来自同一受试者的较早日期获得的值比较。因此,本发明的每种方法可供选择地包括测量受试者中化合物的量,而不是简单地检测它,并且任选地进一步将测量的量与对照值或在同一受试者中较早时间获得的值比较。
用于检测化合物的手段基于多种因素变化,这些因素包括成像剂的特性,无论所述方法是在体外、体内还是离体实施,以及当在体内实施时,受试者中被可视化的位置。
VII.实施例
通用方法:HEK-CCK2细胞(用CCK2受体转染的人胚胎肾细胞)是来自MarkHellmich博士(University of Texas Medical Branch,Galveston,TX)的慷慨赠送,其长成单层,使用Dulbecco改进Eagles培养基(GIBCO)(Gibco,NY)10%热灭活的胎牛血清(Atlanta Biological,GA)和1%青霉素链霉素(Gibco,NY),在5%二氧化碳:95%湿空气的气氛中于37℃下至少6次传代,然后用于测定。
无胸腺雌性裸(nu/nu)小鼠(5周龄,18-20g)购自Envigo(Indianapolis,IN)并维持正常的啮齿动物饮食(Teklad,WI)。以5只/笼将动物饲养在带栅栏(barrier)、无病原体的有遮盖的架(cloaked rack)中。根据需要给予高压蒸汽处理的自来水和食物。在研究期间,以标准的12小时光亮-黑暗循环将动物饲养在无菌环境中。通过耳朵打孔逐一识别小鼠。所有动物程序均经Purdue Animal Care and Use Committee批准。根据国家和国际动物人道待遇指南进行动物护理和研究。
体外结合和特异性:将过表达CCK2的HEK细胞接种到T75烧瓶中并使其在48小时内形成单层。胰蛋白酶消化后,将释放细胞转移到离心管(1×106个细胞/管)中并离心。在存在或不存在100倍过量配体—高亲和力CCK2受体拮抗剂的情况下,用含有升高浓度的所需CCK2R-配体-NIR染料化合物的新鲜培养基置换培养基,并在37℃孵育1小时。用新鲜培养基(2×1.0mL)和PBS(1×1.0mL)冲洗后,将细胞重悬于PBS(1.0mL)中,并使用荧光计(Cary,Agilent)分析细胞结合荧光(100000个细胞/样品)。使用GraphPad Prism 4以细胞结合荧光百分比相对测试物品的对数浓度的图来计算相对结合亲和力。
全身成像:在肩上用HEK-CCK2细胞(5.0×106/小鼠,在GIBCO培养基中的50%高浓度基质胶中)皮下接种6周龄雌性nu/nu小鼠。使用卡尺每2天在垂直方向上测量肿瘤的生长(按照相同的时间表监测体重),并且肿瘤的体积计算为0.5×L×W2(L=最长轴,并且W=垂直于L的轴,以毫米计)。一旦肿瘤体积达到400mm3,给动物(5只小鼠/组)静脉注射含10nmol所需的CCK2R-配体-NIR染料化合物的磷酸盐缓冲盐水(100μL)。对于全身成像和生物分布研究,在给予感兴趣的化合物2小时后通过CO2窒息对动物实施安乐死。
然后使用具有Living Image 4.0软件的Caliper IVIS Lumina II ImagingStation(PerkinElmer Inc,MA)进行全身成像(完整肿瘤)实验。
成像设置:-灯级:中等;激发:745nm;发射:ICG(830nm);落射照射(epiillumination);合并(binning):4(M),FOV=12.5;f-stop=2;采集时间=1秒。对于时间依赖性研究,使用异氟醚在麻醉下对动物进行成像。然后使用具有Living Image 4.0软件的Caliper IVIS Lumina II Imaging Station进行全身成像(完整肿瘤)实验(PerkinElmerInc,MA)。
组织生物分布:在全身成像后,对动物进行解剖,像前面一样使用IVIS成像器分析选择的组织(心脏、肺、肝脏、脾脏、肾脏、胃、小肠、大肠、肌肉、皮肤和肿瘤)的荧光活性。成像设置:-灯级:中等;激发:745nm;发射:ICG;落射照射;合并:4(M),FOV=12.5;f-stop=2;采集时间=1秒。
实施例1:具有单个氨基酸间隔基团的CCK2R靶向NIR化合物的临床前评估
体内研究的结论:化合物40和41之间的差异是化合物41在酪氨酸和脲部分之间具有另外的苄基残基。与化合物40相比,化合物41表现出更高的特异性、更高的积聚、快速的皮肤清除,表明化合物41中的另外的苄基残基在最终化合物的受体结合、特异性和药代动力学性质方面起作用。
实施例2:具有短氨基酸间隔基团的CCK2R靶向NIR化合物的临床前评估
体外研究:

体外研究的结论:体外结合亲和力数据显示:化合物21和43对CCK2R具有非常高的亲和力,而与化合物21和43相比,化合物42和44对CCK2R具有相对较弱的亲和力。全身成像和生物分布数据表明,所有化合物都积聚在CCK2R阳性的肿瘤中。然而,化合物21和41在肿瘤中具有非常高的荧光强度,表明这些化合物对CCK2R的高效力。化合物肿瘤摄取的头对头比较表明化合物21在所有四种肿瘤中具有最高的肿瘤摄取,表明它将是用于CCK2R阳性的癌症的图像引导手术的良好临床候选物。
实施例3:具有手性修饰的短氨基酸间隔基团的CCK2R靶向NIR化合物的临床前评估

体外研究的结论:体外结合亲和力数据显示:化合物10对CCK2R具有非常高的亲和力,而化合物8、9和11对CCK2R具有相对弱的亲和力。这可能是由于配体从D-异构体到L-异构体的手性变化。然而,与化合物21相比,化合物10对CCK2R的亲和力较低。因此,决定围绕化合物21进行先导优化。
实施例4:化合物21的扩展临床前评估
结论:化合物21在776nm处被激发并在796nm处发射(图12),表明20nm斯托克斯(Stokes)位移,具有很好的NIR特性。由研究得到的解离常数(KD)3计算为1nM(图13),表明对CCK2受体的非常高的亲和力。观察到化合物3有效地标记HEK-CCK2细胞(图14c和d)。IHC能力分析表明超过60%的样本具有中等至强的CCK2R表达水平,而健康的肺不表达CCK2R(图14a-b)。此外,携带HEK-CCK2R肿瘤异种移植物的小鼠的全身成像(图14c)和它们的离体组织生物分布(图14f)表明,化合物21仅在CCK2R阳性的肿瘤中被摄取而在其它组织中没有积聚,表明非常高的肿瘤-背景比。

14.一种检测受试者的肿瘤的方法,包括以下步骤:选择疑似患有肿瘤的受试者,给予所述受试者权利要求1所述的化合物,用所述化合物可吸收波长的激发光照射组织;以及检测由受试者的组织或细胞中的所述化合物发出的光信号。
15.权利要求14所述的方法,其中所述受试者是动物或人。
16.权利要求14所述的方法,其中权利要求1所述的化合物还包含药学上可接受的稀释剂或载体。
17.权利要求14所述的方法,其中所述组织或细胞是恶性的并且与选自以下的癌症相关:肾癌、甲状腺髓样癌、胰岛素瘤、小细胞肺癌、支气管和回肠类癌、GIST肿瘤和结肠癌、肝细胞癌和胰腺癌。
