ALPには、4つの公知のアイソザイム、つまり組織非特異的アルカリホスファターゼ(TNALP、下記の考察参照)、胎盤型アルカリホスファターゼ(PALP)(NCBI参照配列[NP_112603]及び[NP_001623])、生殖細胞型アルカリホスファターゼ(GCALP)(NCBI参照配列[P10696])、並びに小腸型アルカリホスファターゼ(IALP)(NCBI参照配列[NP_001622])がある。これらの酵素は、非常に類似した三次元構造を有する。それらの触媒部位は、それぞれ酵素活性に必要な金属イオン(2つのZn2+及び1つのMg2+)のための4つの金属結合ドメインを含んでいる。これらの酵素は、リン酸モノエステルの加水分解を触媒し、また、高濃度のリン酸アクセプタの存在下でトランスリン酸化反応を触媒する。例えば、PALPは、TNALPの天然基質として公知のPEA、PPi及びPLP(Whyte, 1995、Zhang, 2004)に対して、生理学的に活性である。
TNALPは、そのC末端へ糖脂質を介してアンカーされている膜結合タンパク質である(Swiss-Prot、P05186)。このグリコホスファチジルイノシトール(GPI)アンカーは、疎水性C末端が除去された後に、翻訳後的に付加され、これは、一時的膜アンカー及びGPIの付加のためのシグナルの両方として作用する。本明細書に記載の組換えTNALPは、例えば、TNALPの可溶性部分を含む。より詳細な例としては、疎水性C末端配列の第1のアミノ酸、つまりアラニンがストップコドンに置き換えられているヒトTNALPを含む組換えTNALPが挙げられる。そのように形成された可溶性TNALP(本明細書でsTNALPと称する)は、触媒部位の形成に必要なTNALPの天然アンカー形態の全てのアミノ酸を含んでいるが、GPI膜アンカーは欠いている。公知のTNALPとしては、例えば、ヒトTNALP[NCBI参照配列NP_000469、AAI10910、AAH90861、AAH66116、AAH21289及びAAI26166]、アカゲザルTNALP[XP_001109717]、ラットTNALP[NP_037191]、イヌTNALP[AAF64516]、ブタTNALP[AAN64273]、マウス[NP_031457]、ウシ[NP_789828、NP_776412、AAM 8209、AAC33858]、並びにネコ[NP_001036028]が挙げられる。本開示におけるTNALP又は他の遺伝子若しくはタンパク質に対して使用される用語「野生型」又は「野生型配列」は、正常ヒト、非ヒト哺乳動物又は他の生体生物において天然に存在するそのような遺伝子又はタンパク質の通常の形態を指す。野生型配列は、遺伝子については、遺伝子座での標準の「正常」対立遺伝子、又はポリペプチド若しくはタンパク質については、標準「正常」一次アミノ酸配列(場合により、標準の「正常な」翻訳後修飾及び/又はアミノ残基酸間の鎖内結合及び/若しくは相互作用を有するもの)を指し、非標準「変異」対立遺伝子又はアミノ酸配列/修飾/相互作用によって生成されるものと対比して参照されうる。「変異」対立遺伝子は、大規模に変化する可能性があり、集団内で遺伝的シフトが生じた場合には野生型にさえなる。ほとんど又は全ての遺伝子座(ほとんどのポリペプチド配列について頻度は低いが可能性はある)は、様々な対立遺伝子形態で存在し、その頻度は種の地理的範囲を通じて変化し、統一の野生型は必ずしも存在しない可能性があることが今や認識されている。しかしながら、一般に、最も広く存在する対立遺伝子又はアミノ酸配列、つまり正常な個体のヒト又は他の生物の間で最も頻度が高い対立遺伝子又はアミノ酸配列が、本開示において、野生型と考えられる。本明細書におけるヒト又は他の生物について使用される用語「正常」は、別途特定される場合を除いて、関連遺伝子又はポリペプチド/タンパク質の異常な活性により生じる又は関連する(例えば、遺伝子又はタンパク質産物の欠損若しくは不足、及び/又は遺伝子又はタンパク質産物の機能の欠陥若しくは不足によるものでありうる)、何らかの疾患(例えば、HPP)、障害、及び/又は症状若しくは生理学的帰結(例えば、石灰化障害、発作など)を有しないヒト又は他の生物を指す。正常なヒトについての最も明白な例は、HPP又はHPP症状を有さず、及びHPP関連症状をもたらしうるALPL遺伝子及びALPタンパク質に変異又は修飾を有しないヒトである。ALP機能に着目する別のシナリオにおいて、本開示における「正常」なヒトの範囲は、異常な内在性アルカリホスファターゼ活性(これは、例えば、他の健常又は正常なヒトにおける基質(PPi、PEA及びPLP)レベルによって試験され、及び相当する活性と比較されてもよい)を有しない任意のヒトを含むように拡大されてもよい。
本明細書に記載の方法は、特定された対象集団の処置のために、組換えALP(例えば、TNALP)融合タンパク質を利用することができる。融合タンパク質は、本明細書に開示のALP又はバリアントの全長又は断片を含み、生物学的活性を維持しうる。理論に限定されないが、融合タンパク質は、例えば、アルカリホスファターゼ機能を維持又は改善するために、他の部分、例えば、任意のポリペプチド、脂質、ヌクレオチド、又は他の成分を含んでもよい。例えば、融合タンパク質は、ALPのin vivoの安定性又は保持時間を増加させるために(例えば、より長い半減期を伴う)、断片結晶性領域(Fc)又は他の免疫グロブリンの全長若しくは断片を含んでもよい。同様に、アルブミン融合技術を使用して、循環ALPの半減期を向上させてもよい(Schulte 2011 Thromb Res. 128:S9-12)。さらに、融合タンパク質は、ALPを特定の組織、器官又は細胞に向かわせる標的化部位を含んでもよい。
本開示の有用なFc断片としては、ヒンジ、CH2及びCH3ドメインを含むIgGのFc断片が挙げられる。例えば、IgG-1、IgG-2、IgG-3、IgG-3及びIgG-4を使用することができる。本開示のALPを有する融合タンパク質で使用されるFc断片の例示的なアミノ酸配列は、配列番号3に示される。同様に、Fc断片について他のアミノ酸配列、例えば、配列番号3と少なくとも70%、75%、80%、85%、90%、95%、96%、97%、98%、99%又はそれを超える配列同一性を有するアミノ酸配列も、本開示に含まれる。
本開示のALP融合タンパク質は、骨標的化ポリペプチド、例えば、負に荷電したペプチドなどを含みうる。負に荷電したペプチドは、D10、D8、D11、D12、D13、D14、D15、D16、E10、E8、E11、E12、E13、E14、E15、及びE16からなる群から選択されるポリアスパルテート若しくはポリグルタメート又は当該分野で公知の任意の他の形式、例えば、PCT公開第WO2008/138131号に記載のものでありうる。
本開示は、例えば、HPP患者の特定された対象集団及び部分集団を処置するための、組換えALP構築物又はALP融合タンパク質を提供する。そのような組換えALP構築物又は融合タンパク質は、可溶性ALP(sALP、例えば、sTNALP)イソ酵素及びFc断片の全長又は断片を、負に荷電したペプチドをさらに伴って又は伴わずに含みうる。これらの成分は、5'末端から3'末端へ任意の配列で融合することができ、但し、得られた融合タンパク質がアルカリホスファターゼ活性を維持又は改善することを条件とする。融合タンパク質の例示的な形式としては、例えば、限定されないが、sALP-Fc-D10、sALP-Fc、D10-Fc-sALP、Fc-sALP、D10-sALP-Fc、Fc-sALP-D10、Fc-D10-sALPなどが挙げられる。これらの形式において、D10は、任意の他の負に荷電したペプチド又は標的化部分で任意に置換することができ、Fc断片は、任意の機能性IgG又は免疫グロブリンで置換されうる。
ALP融合タンパク質の異なる成分(例えば、断片又は部分)は、隔てているリンカー又はスペーサーを通じて融合してもよい。一部の実施形態では、スペーサーは、少なくとも2、3、4、5、6、7、8、9又は10以上のアミノ酸を含む短いポリペプチドでありうる。一実施形態では、sALP(例えば、sTNALP)融合タンパク質におけるFc断片は、タンパク質をより効率的に折り畳ませることができるスペーサーとして作用する。このことは、sTNALP-Fc-D10の発現が、sTNALP-D10の発現よりも高かったという発見により確認された(PCT公開第WO2008/138131号の実施例2)。理論に限定されないが、Fc断片は、sALP配列のC末端にD10配列が付加する強い負の電荷の存在により生じる反発力を軽減するよう作用しうる。他の有用なスペーサーとしては、限定されないが、sALP配列に加えられた強い負に荷電した配列(例えば、D10などのポリアスパルテート)の存在により生じる反発力を軽減することができるポリペプチドが挙げられる。
通常の抗てんかん薬は、ナトリウムチャネルを遮断することができる又はGABA機能を増強しうる。電位開口型ナトリウムチャネル及びGABA系の成分に加えて、他の標的としては、GABAA受容体、GAT-1 GABAトランスポーター及びGABAトランスアミナーゼが挙げられる。さらなる標的としては、電位開口型カルシウムチャネル、SV2A及びα2δが挙げられる。例示的な抗発作薬としては、例えば、アルデヒド(例えば、パラアルデヒド)、芳香族アリルアルコール(例えば、スチリペントール)、バルビツレート(例えば、フェノバルビタール、メチルフェノバルビタール及びバルベキサクロン)、ベンゾジアゼピン(例えば、クロバザム、クロナゼパム、クロラゼペート、ジアゼパム、ミダゾラム、ロラゼパム、ニトラゼパム、テマゼパム及びニメタゼパム)、臭化物(例えば、臭化カリウム)、カルバメート(例えば、フェルバメート)、カルボキサミド(例えば、カルバマゼピン、オクスカルバゼピン及びエスリカルバゼピン酢酸エステル)、脂肪酸(例えば、バルプロエート、ビガバトリン、プロガビド及びチアガビン)、フルクトース誘導体(例えば、トピラメート)、GABA類似体(例えば、ガバペンチン及びプレガバリン)、ヒダントイン(例えば、エトトイン、フェニトイン、メフェニトイン及びホスフェニトイン)、オキサゾリジンジオン(例えば、パラメタジオン、トリメタジオン及びエタジオン)、プロピオネート(例えば、ベクラミド)、ピリミジンジオン(例えば、プリミドン)、ピロリン(例えば、ブリバラセタム、レベチラセタム及びセレトラセタム)、スクシンイミド(例えば、エトスクシミド、フェンスクシミド及びメスクシミド)、スルホンアミド(例えば、アセタゾラミド、スルチアム、メタゾラミド及びゾニサミド)、トリアジン(例えば、ラモトリギン)、ウレア(例えば、フェネツリド及びフェナセミド)、バルプロイルアミド(バルプロエートのアミド誘導体)(例えば、バルプロミド及びバルノクタミド)、並びに他の薬物(例えば、ペランパネル)が挙げられる。
本明細書に記載の治療剤、例えば、組換えALPは、例えば、経口的に、経鼻的に、静脈内に、筋肉内に、皮下に、舌下に、髄腔内に、又は皮内に投与されうる。投与の経路は、様々な因子、例えば、環境及び治療目的などに依存しうる。
具体的な投与量は、例えば、投与様式、並びに対象の年齢及び体重などの多くの因子に依存する。通常、単回用量内に含有される骨標的化又はタグ化されていないALPの量は、顕著な毒性を誘導せずに、発作を効果的に防止、遅延又は修復する量である。通常、本開示に従うsALP又はその融合タンパク質は、0.001〜500mg/kg/日の範囲の用量で、より特定の実施形態では、約0.1〜約100mg/kg/日の範囲の用量で、さらに特定の実施形態では、約0.2〜約20mg/kg/日の範囲の用量で投与されうる。アロメトリックスケーリング法(Mahmoodら 2003 Clin. Pharmacol.、43(7):692-7及びMahmood 2009 J. Pharma. Sci.、98(10):3850-3861)を使用して、マウスの用量をヒトに外挿してもよい。投与量は、通常の因子、例えば、疾患の程度及び患者からの異なるパラメータにしたがって、臨床医により適合されてもよい。
sALP又はその融合タンパク質を含有する製剤は、薬学的に許容される滅菌水性又は非水性溶媒、懸濁液又はエマルジョンと組み合わせて、患者に提供されてもよい。非水性溶媒の例は、プロピレングリコール、ポリエチレングリコール、植物油、魚油、及び注射可能な有機エステルである。水性担体としては、水、水-アルコール溶液、エマルジョン又は懸濁液、例えば、食塩水、並びに緩衝化医学的非経口ビヒクル、例えば塩化ナトリウム溶液、リンゲルデキストロース溶液、デキストロース+塩化ナトリウム溶液、ラクトース含有リンゲル溶液、又は不揮発性油が挙げられる。静脈内ビヒクルとしては、流体及び栄養補充剤、電解質補充剤、例えば、リンゲルデキストロースなどに基づくものが挙げられる。
本開示はまた、減少したALP活性を示す対象の集団を特定するため及び/又は特定した集団を処置するためのキットに関する。キットは、例えば、骨石灰化障害を示さない対象を特定することができ、例えば、発作(例えば、組換えTNALPなどの組換えALPなどにより処置されるB6耐性発作)を処置するための治療剤及び製剤を含むことができる。キットは、減少したALP活性に関連する疾患、障害又は症状、例えば、HPP及びHPP関連症状を修復又は防止するために、対象に組成物又はベクターを投与するための指示書をさらに含みうる。
組換えsTNALP-FcD10の発現及び精製
組換えTNALPの発現及び精製を容易にするために、TNALPにおけるGPI-アンカー結合を特定する疎水性C末端配列を消去して可溶性分泌酵素とした(Di Mauroら 2002 Journal of Bone and Mineral Research 17:1383-1391)。また、TNALP外部ドメインのコーディング配列を、ヒトIgG1(γ1)(Swiss-Prot P01857)のFc領域で延長させた。これにより、プロテインAクロマトグラフィーにおける組換え酵素の迅速な精製と、驚くべきことに、その発現の増加が可能となった。さらに、組換えTNALPを骨組織へ標的化するために、デカアスパルテート(D10)配列を、Fc領域のC末端に結合させた。sTNALP-FcD10と称される、TNALPのこのキメラ形態は、人工基質p-ニトロフェニルリン酸を使用してpH9.8でアッセイするとき、及び生理学的基質として無機ピロリン酸(PPi)を使用してpH7.4でアッセイするときの両方の全酵素活性を保持している。TNALPの天然形態と同様に、N末端シグナルペプチドは、粗面小胞体にわたるタンパク質の翻訳同時転位の間に切断される。その設計及び構造を、図1に模式的に例証する。融合タンパク質(シグナルペプチドを含む)のアミノ酸配列は、図2に示される。分泌された融合タンパク質(つまり、シグナルペプチドを含まない)のアミノ酸配列を図3に示す。代表的なsTNALP-FcD10融合タンパク質の発現方法及び特徴についての完全な記述については、例えば、PCT公開第WO2008/138131号を参照。
「非罹患」のHPPマウス部分集団
Akp2-/-マウスは、全体的にヒトHPP患者と類似した生理学的症候を示す。例えば、そのようなマウスは、1)ALP血漿活性がはるかに少なく(例えば、1%未満)、2)出生時には正常にみえるが、(表現型に応じて)6日齢又は11日齢で明らかな骨格疾患を発症し、3)PPi、PLP及びPEAの少なくとも1つの血漿レベルが上昇しており、4)進行性のくる病変化、骨減少症を有し、骨折し易く、5)てんかん性発作及び無呼吸を有し、6)栄養不良及び/若しくは不十分な体重増加を有し、並びに/又は、7)21日齢までに死亡する。しかしながら、Akp2-/-マウスは、様々な程度の表現型発現を有しうる。例えば、Waymire 1995において、Akp2-/-マウスはいずれも石灰化障害がなかった。加えてB6補助剤(ピリドキサール又はPLであるが、PNでない形態)も、発作表現型を救済し、C57BL/6、129/Sv(B6129)ハイブリッド遺伝的背景を有するAkp2-/-マウスの約67%で生存を促進したが、救済された動物は、後に欠陥歯列を発症した。残りの33%は、PL注射に対して応答不良であるか又は非応答性であった。しかしながら、近交系129/Sv遺伝的背景を有するAkp2-/-マウスは全て、PL注射に対して応答不良であるか又は非応答性であった。
Akp2-/-マウスは、組換えsTNALP-FcD10の毎日の処置により修復可能な減少した脳ガンマ-アミノ酪酸(GABA)を有する。
アルカリホスファターゼ、例えば、TNALP、PLALP、GALP及びIALPは、それらの基質ホスホエタノールアミン(PEA)、無機ピロリン酸(PPi)及びピリドキサール5'-リン酸(PLP)に対して生理学的に活性である。TNALPノックアウト又は変異対象における減少したアルカリホスファターゼ活性により、細胞外PLP及びPPiの蓄積がもたらされる。全身性PLPの蓄積は、随伴して細胞内PLPの低下をもたらしたが、その理由は、PLPはそのリン酸化状態で細胞膜を通過して細胞に入ることができないためである。PLPは、ビタミンB6の触媒的活性型であり、アミン及びアミノ酸代謝に関与する140を超える様々な酵素反応において補因子として作用する(Percudani、R. & Peracchi、A.、EMBO Rep.、4:850 4、2003)。例えば、細胞内PLPが低いと、CNS GABAの減少につながる可能性があり、これは、発作を導く可能性がある。さらに、高濃度のPPiは、くる病(骨軟化症)、頭蓋骨癒合症、呼吸器不全、腎石灰化症、又は筋力低下をもたらす可能性がある。重症の場合には、これらの合併症により、死に至りうる。低下したアルカリホスファターゼ活性によるアミン及びアミノ酸の代謝異常は、HPPに特徴的な病態形成及び表現型に寄与する。
臨床治験の改善
臨床治験により、hsTNALP-FcD10(アスホターゼアルファ、配列番号1)の処置がHPP患者の発作を改善するかどうかを試験した。ビタミンB6応答性発作の経歴を有する全ての患者は、HPPにより非常に重度の影響を受け、そのうち2人がhsTNALP-FcD10を用いた処置の開始後に死亡した。患者の1人は出生時に発作があり、hsTNALP-FcD10処置の数年後に死亡した。ビタミンB6を患者に与えたが、発作を防止するには不十分であった。次いで、クロナゼパムを投与したが、患者は脳浮腫、発熱、差次的イオン不均衡、及び潜在的心筋症を伴う発作を経験した後に死亡した。
マウス及びヒトにおける発作の処置
軽症、非罹患又は検出不能の石灰化障害を有するAkp2-/-マウス及び野生型同腹仔を、可溶性組換えALP構築物(例えば、sTNALP、sTNALP-Fc、sTNALP-FcD10又は他の組換えアイソザイム構築物)で処置する。処置前及び処置後の発作を、ビデオカメラ又は他の生理学的方法で記録する。脳GABA、セリン、及びシスタチオニンの濃度(血清又は尿供給源に由来するものでもよい)を、公知の手法で試験し、比較する。組換えALPの治療効果は、Akp2-/-マウスにおける処置後の発作の減少及び/又はGABA/セリン濃度の回復の発見により示される。様々な投与レジメンを試験し、比較する。ビタミンB6単独又は組み合わせ療法も、あらゆる見込まれる相乗的効果のために試験する。
患者におけるバイオマーカーのモニタリングによる発作処置の最大化
発作を有する患者の特定の部分集団を、特徴的な、アルカリホスファターゼ基質(例えば、PPi、PEA、及びPLP)の正常を超えるレベルにより特定する。それらの基質の豊富なDNA、RNA、及び/又はタンパク質レベルは、当該分野で公知の通常の方法、例えば、定量PCR、サザンブロット、ノザンブロット、PAGE、SDS-PAGE、ウェスタンブロットなどにより検出する。この特定の患者の部分集団を特定した後、本明細書に開示の組換えALP構築物を、発作症状の緩和のために投与する。場合により、1つ以上の他の抗発作薬、例えば、ビタミンB6ビタマーを、少なくとも一定期間、同時投与してもよい。そのような単独又はALP補充を用いた併用療法のために、患者を、処置前、処置中及び処置後に、内在性バイオマーカーについて密接にモニターする。例えば、ビタミンB6を同時投与するとき、医師又は他の薬物投与者は、レシピエント患者におけるバイオマーカーのレベルをモニターし、好ましくは各患者の個々の状況に基づいて、同時投与するビタミンB6の時点、B6及び/又は組換えALPの投与レジメン、同時投与の期間の長さ、並びにビタミンB6同時投与を停止する時点などを決定する。
ビタミンB6でAkp2-/-マウスにおける発作を処置することの不十分性
ビタミンB6処置は、標準的な発作処置である。しかしながら、多くのHPP患者及びAkp2-/-マウスが、B6耐性発作を有し、この発作は、ビタミンB6投与では修復できず、死に至る可能性もある。Akp2-/-マウスに対するビタミンB6の効果の不足を確認するためのさらなる研究を実施した。この研究において、Akp2-/-マウスを、325ppmの食事性ビタミンB6(ピリドキシン)補助剤で処置した。しかしながら、発作は、正常な骨石灰化又は重度の石灰化障害を有するAkp2-/-マウスにおいて依然として観察された。いずれの石灰化表現型を有するノックアウトマウスも25日を超えて生存しなかった(図13)。
[1]異常なアルカリホスファターゼ活性を有する対象における発作を処置する方法であって、治療有効量の少なくとも1つの組換えアルカリホスファターゼを対象に投与することを含む、方法。
[2]正常レベルを超える少なくとも1つのアルカリホスファターゼ基質を有する対象における発作を処置する方法であって、治療有効量の少なくとも1つの組換えアルカリホスファターゼを対象に投与することを含む、方法。
[3]対象における発作を処置する方法であって、
(i)異常なアルカリホスファターゼ活性を有する、発作を患う又は患うリスクのある対象の集団を特定すること、及び
(ii)治療有効量の少なくとも1つの組換えアルカリホスファターゼを、前記集団における対象に投与すること
を含む、方法。
[4]対象における発作を処置する方法であって、
(i)正常レベルを超える少なくとも1つのアルカリホスファターゼ基質を有する、発作を患う又は患うリスクのある対象の集団を特定すること、及び
(ii)治療有効量の少なくとも1つの組換えアルカリホスファターゼを、前記集団における対象に投与すること
を含む、方法。
[5]対象が低ホスファターゼ症(HPP)を有すると診断されており、かつ、対象が軽症又は検出不能の骨石灰化障害を有する、[1]〜[4]のいずれか一項に記載の方法。
[6]対象がHPPを有すると診断されていない、[1]〜[4]のいずれか一項に記載の方法。
[7]アルカリホスファターゼ活性が、ピリドキサール5'-リン酸(PLP)、無機ピロリン酸(PPi)、及びホスホエタノールアミン(PEA)の少なくとも1つに対するアルカリホスファターゼ酵素活性により測定される、[1]又は[3]に記載の方法。
[8]少なくとも1つのアルカリホスファターゼ基質が、PLP、PPi、及びPEAからなる群から選択される、[2]又は[4]に記載の方法。
[9]対象が、増加した血清ピリドキサール5'-リン酸(PLP)を有する、[1]〜[8]のいずれか一項に記載の方法。
[10]対象が、減少した細胞内ピリドキサール5'-リン酸(PLP)を有する、[1]〜[9]のいずれか一項に記載の方法。
[11]対象が、減少した脳ガンマ-アミノ酪酸(GABA)及び減少した脳セリンの少なくとも1つを有する、[1]〜[10]のいずれか一項に記載の方法。
[12]対象が、増加した脳及び尿中シスタチオニンの少なくとも1つを有する、[1]〜[11]のいずれか一項に記載の方法。
[13]少なくとも1つの組換えアルカリホスファターゼが、少なくとも1週間、1ヶ月間、3ヶ月間、6ヶ月間又は1年間毎日対象に投与される、[1]〜[12]のいずれか一項に記載の方法。
[14]少なくとも1つの組換えアルカリホスファターゼの投与が、脳GABAを上昇させる、[1]〜[13]のいずれか一項に記載の方法。
[15]治療有効量の少なくとも1つの付加的治療剤が、対象に組み合わせて投与される、[1]〜[14]のいずれか一項に記載の方法。
[16]少なくとも1つの付加的治療剤が、抗発作薬である、[15]に記載の方法。
[17]少なくとも1つの付加的治療剤と、少なくとも1つの組換えアルカリホスファターゼの同時投与を、所定の期間維持すること、及び
対象への少なくとも1つの付加的治療剤の投与は停止するが、少なくとも1つの組換えアルカリホスファターゼの投与は維持すること
をさらに含む、[15]に記載の方法。
[18]少なくとも1つの付加的抗発作薬及び少なくとも1つの組換えアルカリホスファターゼが、対象に、少なくとも1ヶ月間、少なくとも6ヶ月間、又は少なくとも1年間、同時投与される、[15]に記載の方法。
[19]少なくとも1つの付加的治療剤が、ビタミンB6(ピリドキシン)及びビタミンB6ビタマーの少なくとも1つである、[15]〜[18]のいずれか一項に記載の方法。
[20]少なくとも1つの組換えアルカリホスファターゼが、組織非特異的アルカリホスファターゼ(TNALP)、胎盤型アルカリホスファターゼ(PALP)、生殖細胞型アルカリホスファターゼ(GCALP)、小腸型アルカリホスファターゼ(IALP)、又はそれらの生物学的に機能的な断片、融合物若しくはキメラ構築物の少なくとも1つを含む、[1]〜[19]のいずれか一項に記載の方法。
[21]少なくとも1つの組換えアルカリホスファターゼが、TNALP、PALP、GCALP又はIALPの可溶性断片である、[20]に記載の方法。
[22]少なくとも1つの組換えアルカリホスファターゼが、配列番号2に示されたアミノ酸配列を含む、[1]〜[21]のいずれか一項に記載の方法。
[23]少なくとも1つの組換えアルカリホスファターゼが、融合タンパク質である、[1]〜[22]のいずれか一項に記載の方法。
[24]少なくとも1つの組換えアルカリホスファターゼが、免疫グロブリン分子に融合される、[23]に記載の方法。
[25]免疫グロブリン分子が、断片結晶性領域(Fc)である、[24]に記載の方法。
[26]Fcが、配列番号3に示されたアミノ酸配列を含む、[25]に記載の方法。

[43]少なくとも1つの組換えアルカリホスファターゼが、静脈内に投与され、次いで皮下に投与される、[42]に記載の方法。
[44]対象がヒトである、[1]〜[43]のいずれか一項に記載の方法。
Claims (21)
- 少なくとも1つの組換えアルカリホスファターゼを含む、ヒト対象における低ホスファターゼ症(HPP)関連てんかん性発作の処置における使用のための組成物であって、前記対象は正常な血中及び/又は尿中レベルを超える、ピリドキサール5'-リン酸(PLP)、無機ピロリン酸(PPi)、及びホスホエタノールアミン(PEA)から選択される少なくとも1つのアルカリホスファターゼ基質を有し、前記対象は発作に対するビタミンB6処置に対し非応答性であると判定されている、組成物。
- 前記対象は:
(i)欠損したアルカリホスファターゼ活性を有し、発作を患う又は患うリスクのあるものとして特定される;及び/又は、
(ii)正常レベルを超える少なくとも1つのアルカリホスファターゼ基質を有し、発作を患う又は患うリスクのあるものとして特定される、
請求項1に記載の使用のための組成物。 - 前記ヒト対象がHPPを有すると診断されており、かつ、対象が軽症又は検出不能の骨石灰化障害を有する、請求項1又は2に記載の使用のための組成物。
- 前記ヒト対象がHPPを有すると診断されていない、請求項1又は2に記載の使用のための組成物。
- 前記ヒト対象が、増加した血清ピリドキサール5'-リン酸(PLP)を有する、請求項1〜4のいずれか一項に記載の使用のための組成物。
- 前記ヒト対象が、減少した細胞内ピリドキサール5'-リン酸(PLP)を有する、請求項1〜5のいずれか一項に記載の使用のための組成物。
- 前記ヒト対象が、減少した脳ガンマ−アミノ酪酸(GABA)及び減少した脳セリンの少なくとも1つを有する、請求項1〜6のいずれか一項に記載の使用のための組成物。
- 前記ヒト対象が、増加した脳及び尿中シスタチオニンの少なくとも1つを有する、請求項1〜7のいずれか一項に記載の使用のための組成物。
- 前記組成物が、以下のa)〜c):
a)少なくとも1週間、1ヶ月間、3ヶ月間、6ヶ月間又は1年間毎日;
b)少なくとも1つの付加的治療剤と組み合わせて;及び
c)約0.1mg/kg/日〜約20mg/kg/日の投与量、又は同程度の毎週の投与量で
のうち少なくとも1つでヒト対象に投与するために製剤化されている、請求項1〜8のいずれか一項に記載の使用のための組成物。 - 前記少なくとも1つの付加的治療剤が、抗発作薬、又はビタミンB6(ピリドキシン)及びビタミンB6ビタマーの少なくとも1つである、請求項9に記載の使用のための組成物。
- 抗発作薬及び少なくとも1つの組換えアルカリホスファターゼが、ヒト対象に、少なくとも1ヶ月間、少なくとも6ヶ月間、又は少なくとも1年間、同時投与するために製剤化されている、請求項10に記載の使用のための組成物。
- 前記組成物が、所定の期間前記少なくとも1つの付加的治療剤と同時投与するために製剤化されている、請求項9〜11のいずれか1項に記載の使用のための組成物。
- 前記組成物が、約0.5mg/kg/日〜約5mg/kg/日の投与量、又は同程度の毎週の投与量で投与するために製剤化されている、請求項9〜12のいずれか1項に記載の使用のための組成物。
- 前記組成物が、投与により脳GABAを上昇させることを特徴とする、請求項1〜13のいずれか一項に記載の使用のための組成物。
- 少なくとも1つの組換えアルカリホスファターゼが、以下のa)〜e):
a)組織非特異的アルカリホスファターゼ(TNALP)、胎盤型アルカリホスファターゼ(PALP)、生殖細胞型アルカリホスファターゼ(GCALP)、小腸型アルカリホスファターゼ(IALP)、又はそれらの可溶性断片、生物学的に機能的な断片、融合物若しくはキメラ構築物の少なくとも1つを含む;
b)配列番号2に示されたアミノ酸配列を含む;
c)融合タンパク質である;
d)免疫グロブリン分子又は負に荷電したペプチドに融合される;及び
e)sALP-Fc-D10を含み、sALPは組換えアルカリホスファターゼの可溶性形態であり、Fcは断片結晶性領域(Fc)であり、前記D10は10個のポリアスパラギン酸残基を含む、
のうち少なくとも1つである、請求項1〜14のいずれか1項に記載の使用のための組成物。 - 前記免疫グロブリン分子が、断片結晶性領域(Fc)である、請求項15に記載の使用のための組成物。
- 前記Fcが、配列番号3に示されたアミノ酸配列を含む、請求項15又は16に記載の使用のための組成物。
- 前記負に荷電したペプチドが、D10、D16、E10、及びE16の少なくとも1つである、請求項15〜17のいずれか1項に記載の使用のための組成物。
- 少なくとも1つの組換えアルカリホスファターゼが、配列番号1又は配列番号4に示されたアミノ酸配列を含む、請求項1〜18のいずれか1項に記載の使用のための組成物。
- 前記組成物が、静脈内、筋肉内、皮下、舌下、髄腔内又は皮内投与のために製剤化されている、請求項1〜19のいずれか一項に記載の使用のための組成物。
- 前記組成物が、静脈内投与のために製剤化されている、請求項20に記載の使用のための組成物。
