Description
全長タンパク質が、配列番号1の全長アミノ酸配列に少なくとも90%の配列同一性を有するアミノ酸配列を含み、
ただし、位置279のアミノ酸がロイシン(L)であり、位置328のアミノ酸がバリン(V)であり、位置478のアミノ酸がロイシン(L)である単離タンパク質を提供する。
好ましい実施形態において、前記最低配列同一性(%)の計算は、空隙を導入せずに実施される。整列についての方法およびコンピュータプログラムは、当該分野において周知であり、たとえば、「Align2」または米国国立生物工学情報センター(NCBI)のBLASTサービスなどである。
ステージ1:わずかな機能低下;通常の、または比較的高いGFR(≧90mL/分/1.73m2)による腎障害腎障害とは、血液もしくは尿検査、または画像診断における異常などの、病理学的異常、または障害のマーカとして定義される。

亜鉛欠乏
ヒト組換えアルカリホスファターゼバッチの酵素活性およびタンパク質含有量の双方が測定された。続いて、100μg/mLタンパク質溶液が各条件について作製され、表2に概説されるようにsALPI−ALPP−CD、およびLVL−recAPバッチについて亜鉛欠乏が測定された。作製された試料は、室温で保存され、T=0、T=2時間、およびT=24時間において酵素活性を分析した。
タンパク質濃度は、OD280測定によって測定された(εrecAP 1.01mL/mg/cm OD280)。
緩衝液中におけるLVL−RecAPの安定性
材料:

第2の独立実験において、sALPI−ALPP−CDおよびLVL−recAPの安定性は、表4に概説されたようないくつかの条件について試験された。
この実験における両方の組換えAPバッチについて、0.025Mグリシン緩衝液 pH9.6における100μg/mLタンパク質溶液、ヒト血清、およびヒトクエン酸血漿が、各緩衝条件について作製され、安定性を測定された。
全ての作製された組換えAP試料は、37℃で培養され、T=0、T=0.5時間、T=1時間、T=2時間、およびT=24時間において酵素活性を分析した。
LVL−RecAPの熱的安定性、およびPLP動態
方法
タンパク質発現
分泌された、FLAGエピトープタグ化sALPI−PLAP−CD、およびLVL−RecAPを含む発現プラスミドは、以前に記載されたように行われた(Kozlenkov et al. J Biol Chem 277, 22992-22999 (2002) and Kozlenkov et al. J. Bone Miner. Res. 19, 1862-1872 (2004))。FLAG−タグ化酵素は、COS−1細胞に一過的にトランスフェクトされ、以前に記載されたように(Narisawa et al. Am. J. Physiol. Gastrointest. Liver Physiol. 293, G1068-1077 (2007))、培地が無血清Opti−MEM(Life Technologies社)で置換されたとき、続いてDMEM培地において24時間増殖された。分泌タンパク質を含むOpti−MEMは、トランスフェクションの60時間後に集められ、続いて2μmのセルロースアセテートフィルタを通して濾過され、1mM MgCl2および20μM ZnCl2を含むTBSに対して透析された。
FLAGタグ化酵素の相対的酵素活性を測定するために、マイクロタイタープレートは、0.2〜0.6μg mL−1において抗FLAG M2抗体で被覆された(Sigma-Aldrich社)。これらのプレートは、FLAGタグ化LVL−RecAP、またはsALPI−PLAPの飽和濃度で室温において3時間にわたってインキュベートされ、その後、プレートは、0.008% Tween−80を含むPBSを用いて洗浄され、PLPについての相対的活性は、M2飽和された酵素と比較された。物理的基質ピリドキサール−5’−リン酸塩(PLP)(Sigma-Aldrich社)の加水分解は、標準的アッセイ緩衝液(50mM Tris−HCl緩衝液、100mM NaCl、1mM MgCl2、および20μM ZnCl2)中で、pH7.4において測定された。放出されたホスファターゼの濃度は、Piカラーロックゴールド(Innova Bioscienses社)を用いて650nm(A650)における吸光度を測定することによって測定された。リン酸塩の濃度増加についての検量線は、0〜50μMの間で直線であり、全ての実験は、加水分解されたリン酸塩濃度のこの範囲内に入るように計画された。[Pi]s−1として表わされるモル反応速度は、示された基質濃度範囲について計算され、1つの結合部位モデル(GraphPad Prism)対[基質]に合わされ、Kmが算出された(ラインウィーバーバークプロットは、非常に低い基質濃度における相反する転換の精度が欠如しているので適用されなかった)。
FLAGタグ化酵素の作製
LVL−RecAPの動態特性をsALPI−PLAPと比較するために、PLAPおよびTNAPの比較研究について以前に行われたように、FLAGタグ化配列が双方のcDNAに付加された。cDNAが、COS−1細胞において発現され、分泌酵素を含む培養上清が回収された。連続的な発現、および回収は、抗FLAG抗体ウエスタンブロット解析によって確かめられた。
LVL−RecAP、およびsALPI−PLAPのリン酸加水分解酵素特性は、炎症と癲癇とを関係付ける生理学的基質、特に、ビタミンB6ビタマーPLPについて調べられた。LVL−RecAPは、生理的pH(7.4)において、sALPI−PLAPよりも低いKmを示し(30.1μM 対 60.7μM)、改良されたLVL−RecAPは、生理的pHにおいてsALPI−PLAPよりもPLPについて高い親和性を有することを示す。
LVL−RecAPにおけるアミノ酸変異が酵素の全体的な安定性にもたらす影響は、熱失活試験によって調べられた。sALPI−PLAPは、既に高度に改良された耐熱性(77.8℃において50%不活性化)を有するが、LVL−RecAPは、さらに熱不活性化に対する抵抗性が高い(80.6℃において50%不活性化)。
ラットにおけるLVL−RecAPの薬物動態的分布
Aパート ヨウ素125を用いる放射標識
sALPI−PLAP−CDタンパク質、およびLVL−RecAPタンパク質は、Greenwoodら*によって記載されたクロラミンT法を用いてヨウ素125で放射標識された。標識化の原理は、ヨウ素を原子ヨウ素への「in situ」酸化、およびチロシン残基の水酸基のオルト位置におけるフェノール環へのその求核置換に基づく。sALPI−PLAPタンパク質は、〜0.5mCi/mgの最終比活性と、〜1mg/mL(NaCl 0.9%)最終濃度を得るために、クロラミンT法を用いて放射ヨウ素標識された。

sALPI−PLAPアルカリホスファターゼ(496.7U/mg)、およびLVL−RecAP(624U/mg)は、25%グリセロールw/v,5mM Tris,2mM MgCl2,50μM ZnCl2,pH8.0中において、6.34mg/mL濃度でAM-Pharma社によって提供された。sALPI−PLAPタンパク質、およびLVL−RecAPタンパク質は、4℃で保存された。
ヨウ素125放射性核種は、10−5N水酸化ナトリウムにおけるヨウ化ナトリウムとしてPerkin Elmer社から購入された(比活性:643.8 GBq/mg 放射性核種純度:99.95%)。
クロラミンT(N−クロロ−p−トルエンスルホアミド、MW227.6g/mol)、トリクロロ酢酸、二亜硫酸ナトリウム(MW 190.1g/mol)、およびチロシンは、Sigma社から購入された。
Tris緩衝液5mM pH8は、Chelatec研究室において作製された。0.9%NaClは、Versol(登録商標)社によって提供された。
850μgのタンパク質、約600μCiのNa125I、50μLのTris緩衝液、および10μLのクロラミンT(404.8nmol、50当量/タンパク質)は、1.5mLのLo−結合エッペンドルフチューブに連続的に添加された。室温において1分間撹拌されて反応させた。2μLの放射標識化培地は、5%MBS溶液に混合され、放射線標識化効率は、10%TCAを溶出液として用いるインスタント薄層クロマトグラフィー(ITLC)によって評価された(ストリップの底部においてアルカリホスファターゼが、およびストリップの上部で遊離の125−ヨウ素が溶出する)。30μLのチロシン溶液(水中で10mg/mL)を未精製混合物に添加した後、ヨウ素化sALPI−PLAP、およびヨウ素化LVL−RecAPは、0.9%NaClを用いて溶出されるゲル濾過(G10、GE Healthcare社)によって精製された。0.2mLの画分は、試験管に集められた。各画分の放射活性は、ヨウ素125放射性核種に較正された自動ガンマカウンター(Wallace Wizard 2470 Perkin Elmer社)で測定された。所望の放射ヨウ素標識製品を含む画分が貯えられた。
放射標識化化合物の放射化学的純度は、ITLCによって検証された。
ITLCによって測定された放射標識の効率は、双方のタンパク質について85%を超えるものであった。G10精製後、標識化反応の放射性純度は、双方のタンパク質について97%を超えるものであった。
放射標識化sALPI−PLAPの酵素活性は、ELISAによって評価された。
アルカリホスファターゼ比色分析キット(参照 ab83369 バッチ番号:GR118166−3)
0.9%NaCl中において0.25mg/mLで希釈された未標識組換えヒトアルカリホスファターゼ
0.9%NaCl中において0.25mg/mLで希釈された放射標識化組換えヒトアルカリホスファターゼ
Abcam社のキットは、APによって脱リン酸化されたときに黄色(ラムダマックス=405nm)に変わるホスファターゼ基質としてp−ニトロフェニルホスフェート(pNPP)を使用している。キットは、試料中の10〜250μU APを検出することが可能である。アルカリホスファターゼ比色分析アッセイは、未標識、および放射標識化sALPI−PLAPにおいて実施された。アッセイは、Abcam社によって提供されるプロトコールに記載されたように実施された。
要約すると、標準曲線は、pNPP標準物質の0〜20nmol/ウェルから作成された(最終体積:120μL)。10μLのAP酵素溶液は、各ウェルに添加された。平行して、sALPI−PLAP試験試料は、アッセイ緩衝液中で15000および8000倍に希釈された。10および20μLの各希釈液が添加され、最終体積は、アッセイ緩衝液によって80μLとされた。続いて、50μLのpNPP溶液が、試験試料を含む各ウェルに添加された。標準物質、および試料反応物は、暗所にて25℃で60分間インキュベートされた。全ての反応は、20μLの停止液を用いて停止された。405nmにおけるO.D.は、マイクロプレートリーダにおいて測定された。pNP標準曲線が、作成された。試料の読み取り値は、標準曲線に適用され、AP試料によって生じたpNP量を得た。試験試料のAP活性は、算出することが可能である。

B.1 材料
本試験において使用されたラット系統の特徴を以下に示す。

環境 :気温は、毎日記録された。室温は、22〜24℃であった。人為的な照射サイクルを、自動タイマーを用いて制御した(10時間照射、14時間闇)
人員 :関与する仲間を、適宜選抜し、訓練した。
選別 :動物は、試験管理者によって受領したときに試験された。健康な動物のみが選択された。動物における炎症反応の任意の徴候(たとえば、腫瘍、皮膚炎など)について特に注意を払った。
ヨウ化sALPI−PLAP、およびLVL−RecAP溶液は、in vivo投与の直前に0.9%NaCl中において0.25mg/mLで希釈された。
実験の時点において、Sprague Dawleyラットの平均体重は、約240gであった。無麻酔のラットは、ラット毎に96μgのタンパク質量と、ラット毎に約58μCiの活性とに対応する400μg/kgの投与量で外側尾静脈(左側)に静脈内注射された。注入量は、約380μLであった。個々の投与量は、処置の日における各ラットの個々の体重を用いて算出された。ラットは、競合装置に置かれた。尾部の血管を拡張するために、尾部は、温かい水(45℃)に浸され、続いてアルコールで殺菌された。投与溶液は、尾静脈にゆっくりと注入された。各ラットが受けた実際の投与量を算出するために、注射器は、処置の前後で計量され、投与溶液の一定分量がガンマカウンターで計測された。
屠殺時において、動物は、ケタミン塩酸塩(50mg/mL)とキシラジン塩酸塩(20mg/mL)との混合物の2.5mL/kg体重の腹腔内注射によって麻酔された。ラットは、続いて、心臓穿刺による失血によって迅速に屠殺された。標的の器官は、摘出され、肝臓、胃、小腸、および大腸などの、2gを超える小片に切断された。各片は、その後、柔らかいティシュペーパーで水分をふき取られる前に生理的血清で洗われ、別々に計量され、計数された。選択された器官は、肝臓、腎臓、心臓、肺、脾臓、骨格筋、大腿骨、脳、甲状腺、胃、内容物を含む小腸、内容物を含む大腸、皮膚、および腎臓周囲の脂肪である。
また、組織に残った放射活性と、血液中の放射活性との間の割合が計算された(割合 器官/血液)。最後に、sALPI−PLAPの器官/血液比、LVL−RecAPの器官/血液の割合との間の割合が計算された(図2)。
血液および血清の放射活性レベル
屠殺の時点において、血液試料は、PBS中におけるケタミン塩酸塩とキシラジン塩酸塩との混合物の腹腔内注入によって麻酔されたラットの心臓内穿刺による全採血から得られた。
血液試料は、30分間にわたって室温においてインキュベートされ、続いて、それらは、5分間にわたって10000gで遠心され、血清を作製した。血清は、予め計量されたチューブに集められ、ガンマカウンターにおいて計側された。
データ分析は、全血液および血清について注入され計算された百分率を含む。
幼児型低ホスファターゼ症のAkp2−/−マウスモデル
幼児型低ホスファターゼ症Akp2−/−マウスモデルは、当該技術分野において公知である(J Dent Res 90(4):470-476, 2011)。簡潔に述べると、Akp2−/−マウスは、検出可能なTNALPmRNAまたはタンパク質をもたらさない、機能的に不活性化されたAkp2遺伝子への相同組換えによって、マウスTNALP遺伝子(Akp2)のエクソン6にNeoカセットの挿入によって作製された。動物の使用、および組織の摘出方法は、サンフォード・バーナム医学研究所動物倫理委員会の承認された手順に従った。動物は、ベヒクル(N=10)、1mg LVL−RecAP/kg/日(N=10)、8mg LVL−RecAP/kg/日(N=8)、または16mg LVL−RecAP/kg/日(N=10)で処置された。生存期間が測定され、骨格の発達が評価された。腎臓の石灰化が評価された。PPi、血漿ピリドキサール、血漿カルシウム、およびリン酸塩レベルが測定され、異なる処置群について大腿骨および/または脛骨の長さが、測定された。マイクロCTデータは、各投与量で残余の過類骨症の分析について集められた。
向上した長期生存期間 16mg/kg/日において処置された(図3)。
最も高い投与量でさえ、長骨において骨格異常の(不完全な)レスキューが存在した(データは示されていない)。
歯の表現型においていくらかの回復が認められた(データは示されていない)。
頭蓋縫合早期癒合症の回復があると思われる(データは示されていない。以後確認される)。

材料、および方法

コントロール、および試験品の作製
新鮮なコントロール品、LVL−RecAP希釈液(プラセボ)は、各投与量の投与に先だって試験における使用のために作製され、使用されないときは2〜8℃で冷蔵保存された。
集められ、0日目(群 3〜7)、および7日目(群7)における製造の各日の投薬に先立って、最終投与製剤0.5mL試料が2回集められた。試料は、中央の層から集められ、起こり得る将来の分析のために凍結して(−50〜−90℃)保存された。
動物の取得および順化
雄の実験的に天然のDomestic Yorkshire交雑ブタ(農場ブタ)(約8〜10週齢、受入時)は、Midwest Research Swine, Gibbon, Minnesotaから受け取った。10〜28日間の順化期間において、動物は、一般的な健康状態、および任意の疾患の徴候について毎日観察された。便試料における卵白アルブミン、および寄生虫の評価が行われ、全ての結果が、試験に採用された動物について陰性であった。
腎臓虚血/再灌流傷害は、Leeらによって公開された手順を用いて誘導された(J. Vet. Med. Sci 72(1): 127-130)。一度麻酔され、全ての動物は、背側横臥位に配置され、手術場所は、クロルヘキシジンスクラブおよび溶液で交互に拭き取り準備された。正中線開腹手術が行われ、両方の腎臓が暴露された。超音波所見に基づいて、左腎、または右腎が除去された。
0日目において、コントロール、または試験品は、それぞれ0、0.32、および1.6mg/kgの全投与量において再灌流する直前に、第1群、第5群、および第6群の全ての動物に静脈内に投与された。全ての投与量は、0.333mL/kgの投与量で投与された。0日目において、試験品は、2つの別の半分の投与量0.16mg/kg、0.167mL/kgの組み合わせた0.32mg/kgの投与量で、第7群の動物全てに静脈内に投与された。第1投与量は、再灌流の直前に投与され、第2投与量は、再灌流後、約8(±2)時間において投与された。0.32mg/kg/日の全投与量(0.333mL/kg)を、1日目〜7日目に、最初の0日目とおおよそ同じ時間に(±2時間)第7群の動物全てに投与した。
3点またはそれより多くの、試験後の時間間隔で測定された評価項目について、反復測定分析(混合モデル)が行われた。各評価項目について、前記モデルは、処置、時間、ならびに処置および時間の相互作用の効果について試験された。試験前データ(投与前の最後の測定値)は、共変数としてモデルに含まれた。
血清クレアチニン
図4に示されるように、予備試験値と比較して、全ての期間において、軽度から中程度まで増加したクレアチニンがあった。クレアチニンは、再灌流後24時間で最大に増加し、続いて後の期間にわたって徐々に減少する。クレアチニンの変化は、処置に伴う腎障害に続く糸球体濾過の低下に一致していた(データは示されていない)。大部分の投与群、および大部分の間隔において、試験品の投与は、クレアチニンの手順関連上昇を減弱させる傾向を有し、LVL−RecAPの保護的効果を示す。図5において示されるように、予備試験値と比較して、再灌流後24時間で試験品を受けるすべての処置群におけるアルカリホスファターゼ(ALP)活性の投与量に依存する上昇があった。ALP活性は、≦1.6mg/kgを受けている処置群において徐々に減少したが、0.32mg/kg/日を受けている動物において連続的に増加し続けた。
ヒトにおける安全性試験
材料、および方法
目的:
健常な対象の静脈内(i.v.)注入によって投与された組換え改良アルカリホスファターゼ(LVL−recAP)の単回、および複数回の投与の安全性、および認容性を評価すること。 LVL−recAPを健常な対象に、単回および複数回i.v.注入した後の、LVL−recAPの薬物動態(PK)を測定すること。
これは、計画人数50人の健康な対象における2部構成、単一施設試験である。A部は、任意に抽出された、二重盲検の、プラセボ対照の、単回投与試験(SAD)、8人の健康な男性および女性(6人のLVL−recAP、および2人のプラセボ)を対象とするそれぞれ連続した4つまでの、群における研究である。試験は、各群ごとに最小で2人、最大で4人の女性とともに、等しい数の男性および女性の被験者を各処置群に含めるように行われる。LVL−recAP、またはプラセボの単一用量は、1時間の静脈内注入によって投与される。パートBは、9人の健康な男性および女性の各対象(6人は、LVL−recAP、および3人はプラセボである)の最大2つの群における、ランダム化、二重盲検、プラセボ制御、反復投与試験(MAD)である。等しい数の男性および女性の対象を各処置群に含めるように試みられる。対象は、1、3および5日目において、LVL−recAP、またはプラセボの1時間静脈内注入を受ける。以下の処置が行われる。

Bパート:−1日目から最後の薬剤投与後48時間(第7日目)まで。第8日目、第10日目、第13日目、および第19日目に、短期外来患者が臨床研究センターを訪問。

材料および方法
マウス
Alpl−/−マウスの作製および特徴は、以前に報告された(Narisawa et al., 1997)。Alpl−/−マウスは、TNAPの包括的な欠乏を含む幼児型HPP、PPi蓄積、および石灰化異常のフェノタイプを示す。ビタミンB6を伴う栄養素補充は、短時間で癲癇を抑制し、出生後18日〜22日まで生存期間を延ばすが、石灰化不全および類骨の蓄積は、加齢とともに悪化し続ける(Narisawa et al., 1997; Fedde et al., 1999; Narisawa et al., 2001; Millet al., 2008)。したがって、本試験における全ての動物(ブリーダー、看護母親、仔、および離乳仔)は、高レベル(325ppm)のピリドキシンを含むlaboratory rodent diet 5001に自由にアクセスすることができた。遺伝子型判定は、以前に記載されたように(Yadav et al., 2011)、ゲノムDNAにおいてPCRによって行われた。動物実験委員会(IACUC)は、全ての動物実験を承認した。
25%グリセロールw/v、5mM トリス/HCl、2mM MgCl2、50μM ZnCl2において10.1mg/mlのLVL−RecAP溶液、pH 8.0が使用された。酵素は、高圧液体クロマトグラフィーによって測定された>99.99%の純度を有していた。
Alpl−/−マウスは、5つのコホートに分けられた。ベヒクル処置:Alpl−/−マウスは、ベヒクルのみで処置された(n=14)。LVL−RecAP1:Alpl−/−マウスは、1mg/kg/日でLVL−RecAPを用いて処置された(n=14)。LVL−RecAP8:Alpl−/−マウスは、8mg/kg/日でLVL−RecAPを用いて処置された(n=12)。LVL−RecAP16:Alpl−/−マウスは、16mg/kg/日でLVL−RecAPを用いて処置された(n=10)。Alpl−/−マウスの野生型同腹仔は、参照動物として用いられ、注入を受けなかった(n=14)。ベヒクル、またはLVL−RecAPコホートは、毎日肩甲部に皮下注入された。注入薬は、AM8:00〜11:00に投与された。投与された量は、注入前に測定された体重に基づいて計算された。全ての処置は、出生後1日から始まり、53日目まで、または剖検時まで毎日繰り返された。
剖検は、出生後53日目(p53)、実験プロトコールを終了した動物についてLVL−RecAPの最後の注入後24時間、または病状末期と考えられる動物に直ちに行われた。アベルチンは、安楽死の前に腹腔内に投与された。血液は、心臓穿刺によってリチウムヘパリンチューブに集められた。剖検は、肉眼所見試験、およびX線から構成された。
骨格のX線撮影画像は、20kVのエネルギーを用いて、Faxitron MX-20 DC4 (Chicago, IL, USA)で得られた。片側下顎骨は、30kVでスキャンされた。
p21マウスから遊離された全ての頭蓋骨は固定され、続いてeXplore Locus SP イメージングシステム(GE Healthcare Pre-Clinical Imaging, London, ON, Canada)を用いて18μm 等方性ボクセル分解能でスキャンされた。測定値は、試料を180度プラス20度の送風角度で回転させるパーカー法スキャン技術を用いて1600msの暴露時間を用いて、80kVの作動電圧、および80mAの電流で取得された。スキャンは、ヒドロキシアパタイトファントムに較正され、3D画像は、18μm3の有効ボクセルサイズで再構築された。1400ハンスフィールドユニットの固定閾値が、石灰化組織を識別するために使用された。頭頂骨および前頭骨の関心領域(ROI’s)は、長さ1mm、幅1mm、骨の厚さに等しい深さ、ならびに矢状、および冠状縫合から0.75mmで始まる位置において、以前に記載されたように設定された(Liu et al., 2014)。骨の、体積、密度、および構造のパラメータは、Microview version 2.2 ソフトウェア(GE Healthcare Pre-Clinical Imaging, London, ON)を用いて測定され、アルゴリズムを設定された(Meganck et al., 2009; Umoh et al., 2009)。スチューデントt検定は、遺伝型の間における統計学的に有意な相違を立証するために行われた。μCTの骨データは、分析され、Bouxseinら 2010 (Bouxsein et al., 2010)の推奨に従って報告されている。
骨試料は洗浄され、4%パラホルムアルデヒド/リン酸緩衝液生理食塩水中において3日間にわたって4℃で固定され、続いて70%エタノールに移され4℃で保存された。プラスチック切片を以前に記載したように作製した(Yadav et al., 2012)。フォンコッサ、およびヴァンギーソンのトリクローム染色を、以前に記載されたように(Narisawa et al., 1997)プラスチック切片に行った。フォンコッサ、またはヴァンギーソン染色された切片は、ScanScopeXTシステム(Aperio社, Vista, CA, USA)によってスキャンされ、画像は、Bioquant Osteoソフトウェア(Bioquant Osteoanalysis社, Nashville, TN, USA)を用いることによって分析された。組織学に用いられた右側下顎骨は、以前に記載されたように(Foster, 2012)、ブアン固定液中で24時間にわたって固定され、続いてAFS溶液(酢酸、ホルムアミド、塩化ナトリウム)中で鉱質除去され、連続切片作製のためにパラフィン中に包埋された。ピクロシリウスレッド染色のために、脱パラフィン化組織切片は、以前に記載されたように(Foster, 2012)、リンモリブデン酸水和物、0.4%ダイレクトレッド80、および1.3% 2,4,6−トリニトロフェノール(Polysciences社, Warrington, PA)の0.2%水溶液を用いて染色された。ピクロシリウスレッド染色された切片は、顕微鏡写真撮影のために偏光下で観察された。
筋組織の除去後、大腿骨、脛骨、上腕骨、および橈骨の長さは、カリパスを用いて測定された。骨は、凍結され、脱水を回避するために生理食塩水を含むガーゼで包まれた。分離された大腿骨および脛骨は、以前に記載されたように(Huesa et al., 2011)、Instron 1101 万能材料試験機を用いて、3点屈曲試験で評価された。骨は、試験の前にゆっくり解凍され、室温で維持された。未処置の大腿骨および脛骨は、15mmの距離で離れた2つの支持体上の試験装置に置かれ、骨幹の中央に負荷が加えられ、このようにして2mm・min−1の速度における3点屈曲試験を構築した。
血漿におけるPPi濃度は、記載された(Hessle et al., 2002; Yadav et al., 2014)ように、反応産物6−ホスホ[6−3H]グルコネートからUDP−D−[6−3H]グルコース(Amersham Pharmacia社)の活性炭上の示差吸収によって測定された。
異なる生存率をもたらすLVL−RecAPの異なる濃度を考慮して、年齢をマッチさせた方式で3つの処置コホートを比較することが可能である。この理由によって、スチューデントt不対パラメトリック、両側試験が行われ、処置されたAlpl−/−マウスとWTコホートとを比較した。p<0.05である場合、相違は、有意であると考えられる。処置コホート間の生存曲線における相違を比較するために、Gehan-Breslow-Wilcoxon検定が行われた。
LVL−RecAP処置されたAlpl−/−マウスにおける生存期間および体重の増加
LVL−RecAPの、8mg/kg/日(LVL−RecAP8)、または16mg/kg/日(LVL−RecAP16)を受けたマウスにおける生存期間は、ベヒクル処置された、および1mg/kg/日(LVL−RecAP1)コホートに比べて有意に向上した(p=0.001)(図6A)。処置されたコホートの生存曲線を互いと比較したとき、相違は統計学的に有意であった(全ての比較について、p<0.0001)。生存期間中央値は、LVL−RecAP8、LVL−RecAP1、およびベヒクル処置コホートにおいて、それぞれ44日、22日、および19日であった。LVL−RecAP16コホートについて生存期間中央値は、算出されなかった。なぜなら、動物は、53日目の実験終了まで生存したからである。Alpl−/−動物の重量は、p7あたりから始まり、それらのWT同腹仔よりも軽量である。1mg/kg/日のLVL−RecAPを用いる処置は、ベヒクル処置マウスに比べて統計学的に有意な体重増加をもたらし、処置の18日目においてWT同腹仔に比べて有意でない相違をもたらした。Alpl−/−マウスは、ビタミンB6食餌におけるp18〜24で通常死亡し、したがって、より長く生存するLVL−RecAP8と、LVL−RecAP16コホートとの比較は不可能であり、これらは、代わりにWTマウスと比べられた。LVL−RecAP8コホートの動物は、p18からそれらのWT同腹仔よりも有意に軽量であった(図6B)が、LVL−RecAP16コホートにおけるマウスは、体重における標準化を示し、p53におけるWTマウスから区別されなかった(図6B)。
未処置のAlpl−/−マウスのX線写真(図7A)は、以前に記載されたように(Yadav et al., 2011)、低下した組織ミネラル密度、および骨折などの重度の骨格異常を示した。我々は、18日間にわたって1mg/kg/日のLVL−RecAPを受けたAlpl−/−マウスにおいて骨格表現型が改善しており(図7A)、利益は、LVL−RecAP8、およびLVL−RecAP16コホートにおいてより顕著である(図7B)ことを見出した。四肢遠位部は、投与量に関わらず正常の形態を示したが、全ての処置コホートについて、脊椎、前肢、後肢、および胸郭において部分的な補正が観察された。LVL−RecAP8およびLVL−RecAP16マウスは、大腿骨および脛骨において、軽いクラックまたは骨折も示した。膝および肘の間接外形は、p44およびp53において、それぞれLVL−RecAP8およびLVL−RecAP16における不規則性を示した(図7)。
骨軟化症の改善の程度を評価するために、我々は、LVL−RecAP−処置されたマウス、およびWTコントロールマウスの後肢のプラスチック包埋脱灰切片の組織形態計側的分析を行った(図8Aおよび9A)。我々は、骨および類骨体積を測定し(図8B、8C)、血清PPiレベルを分析した(図9B、9C)。初期の知見(Yadav et al., 2011)と一致して、フォンコッサ染色は、Alpl−/−マウスが、石灰化、薄い皮質骨、減少した海綿骨、および損傷した骨化中心において重度の異常を示すことを明らかにした(図8A)が、LVL−RecAP8およびLVL−RecAP16処置された動物は、双方とも、顕著に、増加した皮質骨、および改善した二次骨化中心を示した。組織形態計側(図8B、8C)は、BV/TV割合(図8B)が、LVL−RecAP8およびLVL−RecAP16コホートの双方において、同一齢のWTコントロールよりも顕著に低いままであることを明らかにした(0.0321、および0.0302、N=9)が、OV/BV百分率(図8C)は、双方の処置コホートにおいて、WTコントロールに比べて顕著に高かった(0.0001および0.0175、N=9)。LVL−RecAP8およびLVL−RecAP16処置コホートの間の相違は、統計学的に有意ではなかった。後肢のプラスチック切片の組織学的なトリクローム染色(図9A)によって、これらの知見が裏付けられた。Alpl−/−マウスは、類骨蓄積によって特徴付けられる皮質骨および海綿骨における欠損とともに、重度に減少した石灰化骨質量(緑染色領域)を有する。これに対して、53日齢のWTコントロールは、二次骨化中心、および骨表面における少ない類骨中に、頑強な皮質骨、海綿骨を有する。LVL−RecAP8およびRecAP16コホートにおけるAlpl−/−マウスは、特にLVL−RecAP16コホートにおいて、皮質骨の有意な改善を示し、WTマウスに比べて主な相違を示さなかった。Alpl−/−マウスにおける二次骨化中心における海綿骨形成は処置によって増加するが、LVL−RecAP8およびLVL−RecAP16マウスは、類骨の正常領域(赤色)よりも大きいままであった。骨格の知見と一致して、我々は、全ての処置コホートにおける血清PPi濃度が補正されていることを見出した。LVL−RecAP1動物(図9B)は、ベヒクル処置されたコントロール動物に比べて、有意に減少したPPiレベルを有していると思われる。LVL−RecAP16処置動物は、WT同腹仔と統計学的に区別することができないPPiレベルを有する。
Alpl−/−マウスは、顔面頭蓋形状異常、および冠状縫合融合の特徴を示す(Liu et al., 2014)。Alpl−/−マウスにおける処置によって影響を受ける顔面頭蓋骨格の範囲を測定するために、我々は、前頭骨および頭頂骨のμCTに基づく分析を行った。p21における結果は、ベヒクル処置Alpl−/−マウスの前頭骨および頭頂骨の双方において、WT、またはAlpl−/−マウスに比べて、骨体積画分、骨塩量、骨密度、組織ミネラル含有量、および組織ミネラル密度が有意に減少したことを示す(表15)。これに対して、処置されたAlpl−/−マウスの前頭骨も頭頂骨も、野生型のものと有意に異なるものではなかった。LVL−RecAP16処置されたAlpl−/−マウスの成体頭蓋骨は、寸法および形状の点でWTと相違がないようであった。
マウスにおけるAlpl遺伝子の除去は、HPPに罹患したヒト被験者の症例報告と一致して、セメント質、象牙質、歯槽骨、およびエナメル質における発育上のミネラル欠乏をもたらす(Foster et al., 2014a; Foster et al., 2014b; McKee et al. 2011; Yadav et al., 2012)。無細胞セメント質の欠如は、HPPの特徴である、歯根表面への歯根膜付着の損失、および未成熟な歯の脱落をもたらす。LVL−RecAP8処置された、およびベヒクル処置されたAlpl−/−マウスは、大臼歯形成の完了近くに、p25〜26のWTと比較された。X線検査、およびμCT画像は、未処置のAlpl−/−マウスの下顎骨が、コントロールに比べて(図11A、D)、全体的に石灰化不全の骨、薄い象牙質および広い髄腔の短い大臼歯、ならびに重度に欠損した切歯の特徴を示す(図11B、F)。組織学的検査によって、Alpl−/−マウスの歯は、無細胞セメント質が無いことによって特徴付けられ、歯槽骨類骨は、歯根膜(PDL)腔に侵潤し、強直、および機能的な歯根膜の損失をもたらす(図11G 対 図11E)。8mg/kg/日のLVL−RecAP投与は、p26における、大臼歯の高さ、象牙質の厚さ、および骨石灰化のX線検査の外観を改善するが、切歯は、欠損したままである(図11C、H)。組織学的に、LVL−RecAPのこの投与量は、無細胞セメント質を回復しないが、大臼歯に付随するPDL腔と、歯槽縁は、良好に維持された(図11I)。LVL−RecAP16コホートは、p50〜53においてWTと比較され、成熟した歯の構造および機能における影響を判断された。X線検査およびμCT画像は、Alpl−/−マウスの下顎骨における大臼歯周囲の、歯槽骨および隣接面の骨石灰化の低下を示した(図11J〜M)。大臼歯の形状および象牙質は、LVL−RecAP16マウスにおいて大部分標準化されたように見え、このことは、第1大臼歯のμCT分析によって確認された。LVL−RecAP16群におけるエナメル質は、BV/TVまたはTMDにおいて、WTと相違がなかった。大臼歯の歯冠および歯根は、WTに比べて、BV/TVおよびTMDにおいて4〜10%の、軽度であるが、有意な減少を示し、このことは、象牙質の石灰化が完全に回復しなかったことを意味し、歯根の厚さは、15%減少した。歯槽骨の石灰化は、WTに比べて、LVL−RecAP16群においてより重度に欠損したままであり、BV/TVは、27%減少し、TMDは13%減少した。処置されたAlpl−/−マウスにおける切歯も、重度に影響を受けたままであった(図11K,M)。 組織学的検査によって、LVL−RecAP16マウスは、石灰化された歯槽骨および類骨の混合、ならびに低下したが、維持されたPDL腔の特徴を示した(図11N,P)。p53までにLVL−RecAP16群において、歯の損失は確認されなかったが、セメント質の欠如、PDL組織崩壊、および歯根表面からの脱離、ならびに付着上皮の成長鈍化によって証明されるように、歯周組織の付着は欠損したままであった。しかし、歯にPDLが付着した小さな領域が、組織化されたPDL繊維と関連して確認され(図11O,Q)、このことは、大臼歯を保持するように機能し得るいくらかの易感染性付着の存在を示している。

方法
細胞培養
通常、ciPTECは、33℃で培養された(Wilmer, 2010)。細胞は、サルウイルス40T抗原と、ヒトテロメラーゼの必須触媒サブユニットとともに遺伝子導入され、一定に増殖された。細胞は、ITS(5μg/ml インスリン、5μg/ml トランスフェリン、5ng/ml セレニウム;Sigma-Aldrich社, Zwijndrecht, The Netherlands)を追加した、フェノールレッドフリーのDMEM/Ham F−12(Gibco社, Paisly, United Kingdom)において培養された。実験の前に、細胞をウェルプレートに播種した(48400細胞/cm2)、33℃で1日間、続いて37℃で7日目の成熟期まで培養した。実験の日、細胞は、2時間にわたってLVL−RecAPを用いて(1、5、または10U/ml、AM-Pharma社, Bunnik, The Netherlandsから提供された)プレインキュベートされ(Kiffer-Moreira, 2014)、続いて10mM HEPES HBSS、pH7.4(HEPES: Roche Diagnostics社, Mannheim, Germany; HBSS: Gibco社)に溶解された10μg/ml LPSを用いて24時間にわたってインキュベートされた。また、10U/ml LVL−RecAP(〜17μg/ml)を、LPS誘導細胞に対して、同時、または2時間後に投与した。コントロール細胞は、培養培地と共に単独でインキュベートされた。DLPS(E. coli 055: B5; Sigma-Aldrich社, 10μg/ml)および不活性のLVL−RecAP(17μg/ml、AM-Pharma社から提供された)は、ネガティブコントロールとして使用された。異なる実験において、LPSは、ヒトTNF−α組換えタンパク質、またはPBMCの上清に置換され、LPSの有無で24時間にわたって前刺激された。全ての実験(n=5)は、最小限で行われ、二度実施された。
PBMCは、Ficoll-Pague Plus(GE Healthcare社, Diegem, Belgium)を用いる分画遠心分離によって健常な血液ドナー(blood bank Nijmegen, n=5)から得られた軟膜から単離された。PBMCは、0.5mg/mlゲンタマイシン(Sigma-Aldrich社)、1mM ピルビン酸(Gibco社)、および2mM glutamax(Gibco社)で強化されたRPMI−1640培地(Gibco社)において再懸濁された。細胞は、96ウェルプレートに0.5×106細胞/ウェルの密度で播種され、AP(10ユニット/ml)の有無で2時間プレインキュベートされ、続いて24時間にわたってLPSインキュベーション(1ng/ml)された。全ての実験は、二度行われた。
上清は、LVL−RecAP前処理の有無に関係なくLPS投与の30分後に集められ、続いて、製造業者のプロトコールに従ってATPバイオルミネッセンスアッセイキットCLS II(Roche Diagnostics社)を用いてATP産生を直接測定した。細胞生存は、MTTアッセイを行うことによってLPS培養の24時間後に評価した。要約すると、培地は、37℃で3時間にわたって予め温められた100μlのMTT溶液(Sigma-Aldrich社; 培養培地において0.5mg/ml)に置き換えられ、続いて200μlのDMSOを、可溶化された細胞内沈殿ホルマザン結晶に添加した。色素の消退は、670nmの波長補正を用いて570nmにおいて測定された。
動物実験は、国立衛生研究所のガイドラインに従って行われ、プロトコールは、動物実験審査委員会によって承認された。特定病原菌フリーの雄スプラーグドーリーラット(RjHan:SD; Janvier, France)は、プラセボ(n=6)、LPS(n=6)、またはLPS + LVL−RecAP(n=6)の3つの群に分けられた。基準の血漿試料(リチウム−ヘパリン血液)は、実験の7日前にMultivette(Sarstedt社, Etten-Leur, the Netherlands)を用いる尾静脈穿刺によって集められた。実験の3日前に、基準の腎臓機能をFITC−シニストリン t1/2として評価した(Schock-Kusch, 2011)。t=0において、プラセボ(0.9% NaCl、生理食塩水)、または0.3mg/kg BW LPS(生理食塩水に溶解されたE. coli 0127:B8)を、静脈内ボーラスとして投与し、LPS誘導腎不全を誘導した。t=1.5時間において、上述のように、血漿を得た。t=2時間で、ラットは、プラセボまたはLVL−RecAPの静脈内ボーラス(生理食塩水に希釈された1000U/kg BW)を受け、腎臓機能の第2測定がそれに続いた。t=5時間で、全ての動物は、脱水症状を防ぐために5mlの生理食塩水(皮下注入)を受け、16時間の尿採取期間がそれに続いた。t=21.5時間において、第3の経皮的測定が行われた。t=24時間において、ラットを麻酔し(腹腔内、3mg/kg BW キシラン、および80mg/kg BW ケタミン 10%)、血漿を得るために球後リチウム−ヘパリン血液試料を取り除き、全身灌流を開始した(6分間、生理食塩水+50IU/ml ヘパリン、210mbar;3分間、4%パラホルムアルデヒド(PFA;210mbar))。生理食塩水の灌流後、右腎は、注意深く取り除かれ、迅速に凍結され、処理まで−80℃で保存された。左腎は、PFA灌流後に除去され、組織学的検査および免疫組織化学的検査のための処理まで4%PFA中で4℃において保存された。LPS + LVL−RecAP群に由来する1匹の動物と、プラセボ群に由来する1つの尿試料とは、それぞれ注入および回収の困難性が原因で排除された。
腎機能は、以前に発表されたように(Schock-Kusch 2011, Schock-Kusch 2009)、新規の測定装置を用いることによって、GFRの市販マーカ、FITC−シニストリンの、経皮的に測定された排泄動態によってフリームービング覚醒ラットにおいて評価された(Fresenius Kabi社, Linz, Austria)。要約すると、ラットをイソフルラン吸入によって麻酔し(5%誘導、1.5〜2%維持;Abbott Laboratories社, Illinois, USA)、背中の毛を剃った。装置の光学部は、特に設計された両面接着パッチを用いてこの脱毛領域に固定された(Lohmann社, Neuwied, Germany)が、装置の電子部は、げっ歯類用ジャケットに組み込まれた(Lomir Biomedical社, Malone, USA)。基準シグナルの設定後、FITC−シニストリン(100g BWあたり5mg、緩衝生理食塩水中に希釈した)を尾静脈に注入した。その後、注入後約120分間にわたって測定を継続する間に動物を麻酔から回復させた。T1/2は、経皮的に測定されたFITC−シニストリン排泄動態に適用された一分画モデルによって計算された(Schock-Kusch, 2009)。また、腎機能のパラメータは、Hitachi 704自動分析機(Boehringer Mannheim社, Mannheim, Germany)を用いて、血漿および尿試料において測定された。分画された尿素排出量および内在性クレアチニンクリアランス値は、t=1.5およびt=24の平均血漿値を用いて計算された。
少なくとも24時間の固定後、組織は処理され、paraplast中に包埋され、3μMの厚さで切片にされた。通常の組織学的検査のために、HE染色を腎臓組織に行った。腎障害は、0〜4のスケールで点数化して評価された(0=変化無し、4=重度の損傷、たとえば顕著な尿細管細胞変化)。KIM−1は、ヤギ抗ラットKIM−1一次抗体(1:50; AF3689, R&D Systems社, Abingdon, UK)、およびウサギ抗ヤギIgG(1:200; P0449, DAKO社, Heverlee, Belgium)によって検出された。免疫染色は、VECTASTAIN Eline ABCシステム試薬(Vector Labs社, Amsterdam, Netherlands)、および3,3’−ジアミノベンジジン(DAB, Sigma-Aldrich社)、続いてヘマトキシリン対比染色によって可視化された。全ての点数化は、盲検法で行われた。
ヒトELISAキット(R&D Systems社)は、製造業者の説明書に従って、上清におけるTNF−α、IL−6、およびIL−8を測定するために使用された。血漿サイトカインレベル(IL−1β,IL−6,IL−10,TNF−α,INF−γ)は、製造業者の説明書(Millipore社, Cork, Ireland)に従って、同時ルミネックスアッセイによって測定された。KIM−1およびNGALは、製造業者の説明書に従ってELISA(R&D Systems社)によって測定された。
迅速に凍結された腎臓は、製造業者の説明書に従って、コンプリートEDTAフリープロテアーゼインヒビターカクテルタブレット(Roche Applied Science社, Almere, The Netherlands)を添加した組織タンパク質抽出試薬(T-PER; Thermo Scientific社, Rockford, USA)中において、TissueLyser LT(Qiagen社, Venlo, The Netherlands)によって均質化された。総タンパク質含有量は、ビシンコニン酸タンパク質アッセイキット(Thermo Scientific社)を用いて測定され、試料は、−80℃でアッセイまで保存された。
RNAは、凍結された細胞ペレット、または微粉砕された腎臓からトリゾール試薬を用いて抽出された(2000,30秒; Mikro-dismembrator U, Sartorius Stedim Biotech社, Aubagne Cedex, France)。RNAは、モロニーマウス白血病ウイルス(M−MLV)逆転写酵素(Invitrogen社, Breda, The Netherlands)を用いてcDNAに逆転写された。リアルタイム定量PCR(RQ−PCR)は、Taqman(登録商標)(Applied Biosystems社, Carlsbad, USA)を用いて行われた。遺伝子を増幅し、GAPDHの発現について標準化した(ciPTEC:Ct:18.9±0.1;腎臓組織:Ct:24.8±0.2)。PCR反応は、50℃の2分間インキュベーションステップで開始し、続いて10分間95℃の初期変性、および15秒間95℃の40サイクル、および1分間60℃で行った。群の間の相違は、比較ΔΔCt法によって計算された。プライマー/プローブの組み合わせは、表17にまとめた。
尿中の、アデノシン、AMP、ADP、ATP、およびcAMP含有量は、HPLCによって測定された。要約すると、4体積の尿を1体積のクロロアセトアルデヒド(1M 酢酸緩衝液に6×希釈、pH4.5、Sigma-Aldrich社)に混合し、誘導体化(60分間、70℃、500rpm)、および遠心分離(3分間、室温、13400rpm)し、その後、上清をHPLCバイアルに移し、注入した。プリン体は、溶出液A(0.1M K2HPO4,10mM TBAHS(pH6.5)、および2% MeOH)、および溶出液B(H2O:ACN:THF;50:49:1)を用いるグラジエント溶出によって、Polaris C18-A カラム(150×4.6mm)を用いるHPLCシステム(Thermo Scientific社)によって分離された保持時間は、7.1分間(アデノシン)、8.4分間(AMP)、12.5分間(ADP)、16.2分間(ATP)、および14.8分間(cAMP)であった。定量化は、試料のピーク領域、および蛍光によって測定された標準試料に基づいた(励起:280nm;発光:420nm)。
データは、平均値±SEM、または中央値として表わされる[第25パーセンタイル、第75パーセンタイル]。データの正規性は、コルモゴロフ−スミルノフ検定によって評価された。群間の統計学的な相違を、ボンフェローニの多重比較試験を用いるポストホック比較によるANOVAによって、またはDunnのポスト試験によるKruskal-Wallis試験によって、評価した。0.05未満の両側p値は、統計学的に有意であるとみなされた。全ての試験は、WindowsのGraphpad Prism 5.00を用いて行われた(Graphpad Software社 San Diego, CA, USA)。
LVL−RecAPの腎臓保護機能をさらに調べるために、ciPTECは、仔ウシIAPによって脱リン酸化することができないプロ炎症性サイトカインTNF−αと共にインキュベートされた(Chen, 2010)。IL−6およびIL−8のTNF−α誘導サイトカイン産生も、LVL−RecAP前処理によって減弱したが、不活性LVL−RecAPは効果が無かった(図13A)。敗血症随伴性AKIの病態形成において、LPSは、PTEC上に発現したTLR4に対する結合を介して局所的な炎症反応を誘導する(ciPTEC Ct: 30.5±3.9;n=5)。疾患の他の特徴は、全身の炎症反応であり、腎臓上皮、および内皮細胞の双方に影響を与え、AKIの発症の原因となる(Peters, 2014)。このエンドトキシン誘導腎臓炎症を模倣するために、ciPTECを、LPS刺激された末梢血単核球細胞の上清とともにインキュベートした(PBMC、1ng/ml LPS)。これは、IL−6およびIL−8の産生を誘導し、ciPTECがLVL−RecAPとともに前処理されたとき減少した(図13B、TNF−αは検出されなかった)。TNF−αによって媒介される炎症性反応は、LVL−RecAP処置によって減弱するとの知見と共に、このことは、LVL−RecAPによって標的とされる他の介在物質の存在を示唆する。これに対して、LVL−RecAPを用いるPBMCの前処理は、これらの細胞におけるLPS誘導炎症反応に影響を与えなかった(図13C)。このことは、LVL−RecAPの効果が腎臓特異的であることを示している。
LVL−RecAPの第2の潜在的標的は、たとえば炎症および低酸素症によってもたらされる細胞ストレスの間に放出されたATPである(Eltzschig, 2012)。細胞外ATPは、有害な効果を有するが、エクトヌクレオチダーゼ(たとえば、AP)によって、ADP、AMP、および最終的にアデノシンに変換することができ、アデノシン受容体A1、A2A、A2B、およびA3の1つに対する結合を介して抗炎症および組織保護効果を発揮する(Bauerle, 2011, Di Sole, 2008)。興味深いことに、ciPTECにおけるアデノシン受容体A1、A2B、およびA3の発現は、LPSインキュベーションによって影響を受けない(データは示されていない)が、A2A発現は、LPS刺激下において上方制御された(倍率:4.1±0.4;プラセボに比べてp<0.001)。この効果は、LVL−RecAP同時処理によって減弱し(倍率:2.9±0.2;プラセボに比べてp<0.001;LPSに比べてp<0.05)、LVL−RecAPの保護効果におけるアデノシン経路の役割を示唆する。さらに、我々は、LPSインキュベーションに続いて細胞外ATP濃度増加を観察した。これは、より高いLPS濃度でさらに明確であったが、LVL−RecAPのプレインキュベーションによって反転した(図14)。LPSは、24時間まで細胞生存率に影響を与えなかった(データは示されていない)。このことは、LVL−RecAPは、ATP/アデノシン経路を介する腎臓保護効果を発揮し得るとの仮説を支持する。
in vivoにおけるLVL−RecAPの有利な効果を確認するために、AKIは、ラットにおいてLPS(0.3mg/kg BW)によって誘導され、腎臓機能は、以前に報告されたように(Schock-Kusch, 2011)、フルオレセインイソチオシアネート(FITC)−標識化シニストリン動態の経皮測定によって評価された。FITC−シニストリンは、濾過のみによって腎臓で除去され、血漿画分からのその消失は、リアルタイムで経皮的に測定することができる。このことは、一般的に使用されるクレアチニンクリアランス値に比べてより正確な方法でAKIの進行を調べることを可能にする。LPS注入前に、ベースライン血液試料は、臨床パラメータ、および血漿サイトカインを測定するために取得され、ベースラインFITC−シニストリン半減期(t1/2)は、群間の均一性を確認するために、測定された動態から決定された(データは、示されていない)。1.5時間後、LPS処置は、血漿サイトカインレベルの増加、いくつかの血漿パラメータにおける異常(表18)、立毛、下痢、および自発活性の低下をもたらしたので、全身性炎症の存在を裏付けている。LPS投与の2時間後、ラットは、LVL−RecAP(1000U/kg BW)またはプラセボ(生理食塩水)で処置され、次いで経皮的な腎機能測定が行われた。LPSは、FITC−シニストリン t1/2を有意に遅延し、腎機能の有意な減少を明らかにした。この傾向は、LVL−RecAP処置によって減弱された(図15A)。全ての群において、腎機能は、24時間以内に完全に回復した(図15B)。さらに、血漿尿素レベルは、LVL−RecAPなしにLPSを受けた動物に比べて、LVL−RecAP処置された動物において有意に低下した(表19)。また、LVL−RecAP処置は、分画された尿素排出量のLPS誘導性増加(図15C)と、内在性クレアチニンクリアランスのLPS誘導性減少(図15D)とを抑制した。LVL−RecAP生理活性は、血漿において確かめられ、注入の22時間後に8倍の増加を示した(プラセボ:293±12U/ml; LPS: 260±13U/ml; LPS+AP:2150±60U/ml;p<0.0001)。
LPS誘導性AKIにおけるLVL−RecAPの腎臓保護効果は、腎臓の組織学的検査、および特定の尿細管損傷マーカの評価によって、さらに調べられた。処置された群の間において、組織学的検査における相違は、見出されなかった。損傷無しの範囲内の変化(0)、近位尿細管細胞の、泡沫状の外観、およびわずかな膨張のような変性変化まで(1)、泡沫状の外観および中程度の膨張、ならびに少数のアポトーシス(2)(プラセボ:1[0.75−2];LPS:1.5[0−2];LPS + LVL−RecAP:1[0−1.0])。LPS処置は、腎臓のIL−6発現レベルの有意な増加をもたらすが、他のサイトカインおよび損傷マーカ(MPO、ミエロペルオキシダーゼ;BAX、Bcl2随伴性Xタンパク質;iNOS、誘導型一酸化窒素合成酵素)は、影響を受けなかった(表20)。LVL−RecAPは、腎臓のIL−6発現レベルを減少させることができなかったが、抗炎症性サイトカインIL−10の腎臓における発現を強く増強した(表20)。さらに、LPS投与は、腎臓遺伝子発現レベルの増加に付随する、腎臓損傷分子(KIM)−1、および好中球ゼラチナーゼ関連リポカイン(NGAL)の尿中排出量における有意な増加をもたらした。この効果は、LVL−RecAPの同時投与によって妨げられる(図16A−B、表20)。LVL−RecAPの同様の効果は、血漿NGALレベルについて(図16C)、近位尿細管上皮細胞の頂端膜側に主に局在したKIM−1の腎臓タンパク質レベルについて(図16D)観察された。
LVL−RecAPの腎臓保護機能をさらに解明するために、我々は、ATP−アデノシン経路の役割を調べた。LPS処置は、4つ全てのアデノシン受容体について腎臓における遺伝子発現レベルを減少させる傾向があり、そのうちアデノシン受容体A3のみが統計学的に有意に達した(表20)。興味深いことに、LPS処置は、cAMP、ATP、ADP、およびAMPの排出量を変えることなく(データは、示されていない)、アデノシンの尿中排出量を有意に減少させた(プラセボ:68.0±7.8pgアデノシン/10μg クレアチニン;LPS:19.4±6.3pg アデノシン/10μg クレアチニン;p<0.001)。このことは、腎臓がLPS誘導性AKIの間にアデノシンを利用していることを示唆している。LVL−RecAP処置は、LPS単独に比べて、アデノシン受容体遺伝子発現(表20)、または尿中のアデノシン排出量に影響を与えなかった(LPS + LVL−RecAP:16.7±6.8pg アデノシン/10μg クレアチニン: プラセボに比べてp<0.001)。

10.7.1 溶液の作製
10.7.1.1 2M 水酸化ナトリウム
2モルの水酸化ナトリウム水溶液は、8gの水酸化ナトリウムを約90mlのミリQ水に溶解させ、室温に冷却し、体積を100mLに調整することによって作製された。溶液は、室温で、最大1月まで保存された。
1モルの塩化マグネシウム溶液は、4.06gの塩化マグネシウム6水和物を、約16mLのミリQ水に溶解させることによって作製された。 溶解後、体積は、20mLに調整された。溶液は、表示値+4℃で、最大1月まで保存された。
0.1モルの塩化亜鉛溶液は、272.5mgの塩化亜鉛を、約16mLのミリQ水に溶解させることによって作製された。溶解後、体積は、20mLに調整された。溶液は、表示値+4℃で、最大1月まで保存された
pH9.6 0.025モルのグリシン溶液は、3.76gのグリシンを、1800mLのミリQ水に溶解させることによって作製された。溶液は、25℃に温められ、2M 水酸化ナトリウムを用いてpH9.6に調整された(セクション10.7.1.1を参照)。体積は、2000mLまで作製され、pHは、再度確認された。pHは、25℃においてpH9.6であるべきである。溶液は、表示値+4℃で、最大1週間まで保存された。
酵素希釈緩衝液は、0.5mLの1M 塩化マグネシウム(セクション10.7.1.2を参照)を、0.5mLの0.1M 塩化亜鉛(セクション10.7.1.3を参照)、およびpH9.6 0.025M グリシン溶液(セクション10.7.1.4を参照)に混合することによって作製された。この溶液のために、5.00gのマンニトールと、0.25gの仔ウシ血清アルブミンとを添加し、攪拌しながら溶解した。pHは、再度確認され、必要がある場合、2Mの水酸化ナトリウムを用いて、25℃でpH9.6に調整された。酵素希釈緩衝液は、毎日新たに作製された。
pH9.6 0.0103M p−ニトロフェニルリン酸塩は、1528mgのp−ニトロフェニルリン酸塩を、約360mLのpH9.6 0.025M グリシン溶液に溶解させることによって作製された(セクション10.7.1.4を参照)。pHは、再度確認され、必要である場合、2M 水酸化ナトリウムを用いて、25℃でpH9.6に調整された(セクション10.7.1.1を参照)。pHを再度確認した後、体積は、pH9.6 0.025M グリシン溶液を用いて400mLに調整された。溶液は、表示値+4℃で、最大5日まで保存された。
使用基質は、120mLの pH9.6 0.0103M p−ニトロフェニルリン酸塩溶液(セクション10.7.1.6を参照)を、1.25mLの1M 塩化マグネシウム溶液(セクション10.7.1.2を参照)に混合することによって作製された。この溶液に、約15mLの pH9.6 0.025M グリシン溶液(セクション10.7.1.4を参照)が添加され、pHは、再度確認され、必要であれば、2M 水酸化ナトリウムを用いて、25℃でpH9.6に調整された(セクション10.7.1.1を参照)。体積は、pH9.6 0.025M グリシン溶液を用いて145mLに調整された。使用基質は、毎日新たに作製された。

recAPスパイク溶液は、252.5μL LVL−RecAP(セクション10.6.1)を、酵素希釈緩衝液(セクション10.7.1.5)を用いて5.00mLに希釈することによって作製された。スパイク溶液の最終濃度は、500μg/mLであり、この溶液は、recAP添加されたヒト血漿試料の作製に使用された(セクション10.7.4)。
RecAPスパイク溶液は、188.0μLのRecAP(セクション10.6.1)を、酵素希釈緩衝液(セクション10.7.1.5)を用いて5.00mLに希釈することによって作製された。スパイク溶液の最終濃度は、500μg/mLであり、この溶液は、RecAP添加されたヒト血清試料(セクション10.7.5)の作製に使用された。
LVL−RecAPおよびRecAP酵素活性のための試料溶液は、酵素希釈緩衝液(25℃法についてのセクション10.7.1.5 または37℃法についてのセクション10.7.1.9)を用いる、LVL−RecAP産物試料、またはRecAP産物試料の希釈によって作製された。
LVL−RecAPおよび/またはRecAP試料に由来する試料1は、125μLのLVL−RecAPおよび/またはRecAP血清試料を取得し、酵素希釈緩衝液を用いて2.50mLに希釈することによって希釈され、LVL−RecAPまたはRecAP酵素活性のための最終試料溶液を作製した。
LVL−RecAPおよび/またはRecAP試料に由来する試料2は、125μLのLVL-RecAPおよび/またはRecAP血清試料を取得し、酵素希釈緩衝液を用いて2.00mLに希釈することによって希釈され、LVL−RecAPまたはRecAP酵素活性のための最終試料溶液を作製した。
LVL−RecAPおよび/またはRecAP試料に由来する試料3は、167μLのLVL-RecAPおよび/またはRecAP血清試料を取得し、酵素希釈緩衝液を用いて2.00mLに希釈することによって希釈され、LVL−RecAPまたはRecAP酵素活性のための最終試料溶液を作製した。
LVL−RecAPおよび/またはRecAP試料に由来する試料4は、250μLのLVL-RecAPおよび/またはRecAP血清試料を取得し、酵素希釈緩衝液を用いて2.00mLに希釈することによって希釈され、LVL−RecAPまたはRecAP酵素活性のための最終試料溶液を作製した。
LVL−RecAPおよび/またはRecAP試料に由来する試料5は、500μLのLVL-RecAPおよび/またはRecAP血清試料を取得し、酵素希釈緩衝液を用いて2.00mLに希釈することによって希釈され、LVL−RecAPまたはRecAP酵素活性のための最終試料溶液を作製した。
LVL−RecAPおよび/またはRecAP試料に由来する試料6は、1000μLのLVL-RecAPおよび/またはRecAP血清試料を取得し、酵素希釈緩衝液を用いて2.00mLに希釈することによって希釈され、LVL−RecAPまたはRecAP酵素活性のための最終試料溶液を作製した。
LVL−RecAPおよび/またはRecAP試料に由来する内在性(試料7)は、1000μLのLVL-RecAPおよび/またはRecAP血清試料を取得し、酵素希釈緩衝液を用いて2.00mLに希釈することによって希釈され、LVL−RecAPまたはRecAP酵素活性のための最終試料溶液を作製した。
10.8.1 装置および設定
以下の方法および設定が使用された。
分光光度計 :シングルセルペルチェヒータを備えるThermo Fisher Evolution 300 UV/VISを、マグネチックスターラを用いて、25℃(AN−18−3)、または37℃(AN−18−4)に設定した。
波長 : 405nm
測定 : 3分間、各15秒で測定。最初の1分間は、計算のために考慮なかった。
キュベット型 : ガラス
以下の溶液は、ガラスキュベットに滴下された。溶液の温度は、AN−18−3について25℃±0.5℃、またはAN−18−4について37℃±0.5℃であった。
実験計画は、2つの個別の希釈結果を認めるために、2つの個別の結果(酵素活性)についての相違が≦5.0%であるべきであることを記載しているが、各測定についての相関係数(r)≧0.990の全ての結果が評価に使用された。なぜなら、酵素活性法は、25℃±0.5℃においてLVL−RecAP産物について検証され、他のアルカリホスファターゼ起源、および/または他の条件については、検証されなかったからである。
Claims (15)
- ホスファターゼ活性を有する単離されたタンパク質であって、
配列番号1の全長アミノ酸配列に100%の配列同一性を有するアミノ酸配列を有することを特徴とする単離されたタンパク質。 - 請求項1に記載の単離されたタンパク質をコードする核酸配列を含むことを特徴とするポリヌクレオチド。
- 請求項2に記載のポリヌクレオチドを含むことを特徴とするベクター。
- 医薬としての使用のための、請求項1に記載の単離されたタンパク質、請求項2に記載のポリヌクレオチド、または請求項3に記載のベクター。
- 炎症性疾患、腎臓病、または低ホスファターゼ症の、予防、または処置のための、請求項4に記載の使用のための、単離されたタンパク質、ポリヌクレオチド、またはベクター。
- 前記炎症性疾患は、自己免疫疾患、慢性関節リウマチ、喘息、慢性閉塞性肺疾患、アテローム性動脈硬化症、消化管の炎症性疾患、感染症、敗血症、神経皮膚炎、炎症性肝疾患、炎症性肺疾患、および炎症性腎臓病から選択されることを特徴とする、請求項5に記載の使用のための単離されたタンパク質、ポリヌクレオチド、またはベクター。
- 前記腎臓病は、腎障害、急性腎障害、慢性腎臓病、腎不全、急性腎不全、虚血性腎臓病、および虚血/再灌流腎損傷から選択されることを特徴とする請求項5に記載の使用のための単離されたタンパク質、ポリヌクレオチド、またはベクター。
- 前記低ホスファターゼ症は、周産期型低ホスファターゼ症、幼児型低ホスファターゼ症、小児型低ホスファターゼ症、および成人型低ホスファターゼ症から選択されることを特徴とする請求項5に記載の単離されたタンパク質、ポリヌクレオチド、またはベクター。
- 請求項1に記載のタンパク質、請求項2に記載のポリヌクレオチド、または請求項3に記載のベクターを含むことを特徴とする宿主細胞。
- CHO細胞であることを特徴とする請求項9に記載の宿主細胞。
- 請求項9または10の宿主細胞を培養し、当該宿主細胞に前記タンパク質を産生させることを含むことを特徴とする請求項1に記載の単離されたタンパク質を生産する方法。
- 請求項1に記載の、および/または請求項11に記載の方法によって得ることができる単離されたタンパク質を含むことを特徴とする組成物。
- 医薬としての使用のための、請求項12に記載の組成物。
- 炎症性疾患、腎臓病、または低ホスファターゼ症に付随する疾患の、予防、または処置における使用のための、請求項12に記載の組成物。
- 低ホスファターゼ症の前記予防または処置が、生存期間の延長、体重の増加、改善した骨格表現型、顔面頭蓋欠損の減弱、歯−歯槽表現型の改善、ならびに/または血漿PPiレベルの減少をもたらすことを特徴とする、請求項5もしくは8に記載の使用のための単離されたタンパク質、ポリヌクレオチド、もしくはベクター、または請求項14に記載の使用のための組成物。
