カフは様々に機能する。カフの第1の機能は、人工弁周囲での血液の弁周囲漏出/逆流を阻止することである。弁輪および心房の凹凸のある輪郭にわたって撓み、封止することによって、漏出が最小限化および/または防止される。
カフは、縁またはへりを形成するように管状ステントの直径を超えて突出する実質的に平坦なプレートである。本明細書で使用される、カフ、フランジ、カラー、ボンネット、エプロンまたはスカーティングという用語は、機能的に等しいとみなされる。管状ステントが僧帽弁開口部、僧帽弁輪を通ってテザー・ループによって左心室の方向に引っ張られると、カフは、管状ステントが僧帽弁開口部を通ってそれ以上移動しないように止めるカラーとして作用する。左心房に据え付けられたカフと僧帽弁輪との間の長手方向の力および左心室に取り付けられた心室テザーによって、人工弁全体が保持される。
好ましくは、ニチノール(商標)ワイヤなどの超弾性金属ワイヤが、ステント、ステント内に配設される内部ワイヤに基づいた弁葉アセンブリ、およびカフのワイヤ・フォームに使用される。上記のように、Cu−Zn−Al−Ni合金およびCu−Al−Ni合金などの他の形状記憶合金を任意で使用することが、本発明の範囲内として企図されている。ステントを編組ステントまたはレーザ切断ステントとして作製することができることが企図されている。そのようなステントは、Pulse Systems社など、いくつもの製造業者から市販されている。レーザ切断ステントは、好ましくはニッケル−チタン(ニチノール(商標))から形成されるが、ステンレス鋼、コバルトクロム、チタン、および他の機能的に等しい金属および合金からも形成されるがこれに限定されず、または固定具または心棒上で熱処理することによって形成されるPulse Systems社の編組ステントである。
ステントの1つの可能な構成は、細い等直径のニチノール管のレーザ切断を想定している。レーザ切断によって、細いニチノール管に規則的な切欠きを形成する。次に、管を所望の形状のモールド上に置き、マルテンサイト温度に加熱し、冷却する。ステントをこのように処理することによって、形状記憶特性を有し、較正温度で記憶形状に容易に戻るステントまたはステント/カフを形成する。
ステントは、単純な編組技術を使用して作製することができる。ニチノール・ワイヤ(例えば0.012インチ・ワイヤ)および単純な編組固定具を使用して、一本のワイヤから等直径の管が形成されるまで、単純な上/下編組パターンでワイヤを編組固定具上に巻き付ける。ワイヤの2つの結んでいない端部は、結んでいない端部を中に置いて圧着する、ステンレス鋼またはニチノールの結合管を使用して結合する。約60度の編組角度が、特に有用であることが明らかになっている。次に、編組ステントを形成固定具上に置き、特定温度のマッフル炉内に置いて、所望されるマルテンサイトまたは超弾性特性を発揮するように、ステントを所望の形状に形成する。
弁葉は、弁葉アセンブリによって、または弁葉アセンブリ内に、保持される。本発明の1つの好ましい実施形態では、弁葉アセンブリは弁葉が取り付けられる弁葉ワイヤ支持構造を含み、弁葉アセンブリ全体がステント本体内に収容される。この実施形態では、アセンブリは、弁葉を取り付けるための適切なプラットホームを形成するように、ワイヤおよび安定化させた組織から構成される。この態様では、ワイヤおよび安定化させた組織は、人工弁が展開カテーテル内で圧縮されているとき弁葉構造が圧縮され、展開中に人工弁が開くとき適正な機能的形状にばねによって開くことを可能にする。この実施形態では、弁葉アセンブリを、安定化させた組織または材料によって形成された別個の円筒形ライナー内に任意で取り付け、収容することができ、次いで、ステント本体の内側を裏打ちするようにライナーを取り付ける。
人工心臓弁は、一実施形態では、カテーテル・システムを使用して、心臓の左心室心尖部を通って経心尖的に送り込まれる。心尖部送達の1つの態様では、カテーテル・システムは、肋間送達によって心臓および心膜腔にアクセスする。別の送達アプローチでは、カテーテル・システムは、可撓性カテーテル・システムを使用して、一般に使用される剛性の管システムを使用せずに、順行性または逆行性送達アプローチのいずれかを使用して、人工心臓弁を送り込む。別の実施形態では、カテーテル・システムは、経中隔アプローチによって心臓にアクセスする。
1つの好ましい実施形態では、心臓内の1つまたは複数の組織繋留位置に延びる、人工心臓弁に取り付けられたテザーがある。1つの好ましい実施形態では、テザーは左心室を通って下向きに延び、心臓の心尖部で左心室から出て、心臓の外側の心膜面に締結される。同様の繋留は、三尖弁、または人工弁を必要とする他の弁構造に関連するので、本明細書で企図されている。好ましくはステントに取り付けられた、2〜8のテザーを設けることができる。
一実施形態では、心尖部の送達システムの進入位置で組織が裂開する可能性を制御するために、円形、半円形または多部品プレジェットが使用される。プレジェットは、PFTEフェルトなどの半剛性材料から作製することができる。送達システムが心尖部を穿刺する前に、心尖部が中心に位置するように、フェルトを心臓に確実に取り付ける。次に、送達システムを、プレジェットの中心部、またはオリフィスを通して適宜導入する。このようにして配置され、取り付けられると、プレジェットは心尖部の裂開の可能性を制御するように作用する。
別の実施形態では弁は、歯または棘を使用して弁輪内に据え付けることができる。これらは、1つまたは複数のテザーとともに、または1つまたは複数のテザーの代わりに使用することができる。歯または棘は、隣接する組織への取り付けを行うように位置している。1つの好ましい実施形態では、歯は任意でステントとカフとの間の湾曲/移行領域の周りに周方向に位置する。そのような歯は、バルーン・カテーテルの使用など、機械的手段によって、環状組織内に押し込まれる。1つの非限定的な実施形態では、歯は、任意で、ステント本体が拡張すると、環状組織を確実に穿通し、その中に回転し、保持する、半円形のフックとすることができる。
一実施形態では、複数のタイプの組織および生体適合性材料を使用して、カフを覆い、弁葉を形成し、ワイヤレス弁葉アセンブリを形成し、および/またはステントの内側および/または外側側壁を裏打ちすることができることが、企図されている。上記で述べたように、弁葉部品は、ワイヤを使用せず、安定化させた組織のみから、弁葉アセンブリおよび弁葉を形成するように作製することができる。この態様では、組織のみの弁葉部品を、ワイヤ・フォームを使用して、またはワイヤ・フォームを使用せずに、ステントに取り付けることができる。好ましい実施形態では、1、2、3または4弁葉のいずれか、または弁尖を設けることができる。
好ましい実施形態では、蒸気架橋した超薄の安定化させた生体弁または植込み組織材料が、企図されている。(a)組織標本をアルデヒド、エポキシド、イソシアナート、カルボジイミド、イソチオシアナート、グリシジルエーテルおよびアシルアジドからなる群から選択される架橋剤の蒸気に曝露することによって、消化前の圧縮された組織標本を蒸気架橋させるステップ、および(b)蒸気架橋された組織標本を、アルデヒド、エポキシド、イソシアナート、カルボジイミド、イソチオシアナート、グリシジルエーテルおよびアシルアジドからなる群から選択される架橋剤の液体相を含む架橋浴などの水性架橋浴に、あらかじめ定められた時間にわたって曝露することによって、化学的に架橋するステップを含む方法を使用して、0.003インチ(0.0762mm)〜約0.010インチ(0.254mm)を有する組織を形成することができる。[para15]そのような組織は、ブタ、ヒツジ、ウマまたはウシ由来とすることができ、好ましくは初期材料は30日齢以下のウシから採取されるが、それより成長した動物からの組織も、本発明の範囲内として企図されている。1つの好ましい実施形態では、組織標本は、架橋前に化学的脱水/圧縮および機械的圧縮を施す。
別の実施形態では、最小侵襲心臓カテーテル技術を使用して、手術による展開を中止するとき、人工弁を迅速に除去するための回収システムが企図されている。この実施形態では、テザーは、スネア取付具を有するカテーテルによって捕えられる。テザーが捕えられると、心室内漏斗型取付具が、テザーを引っ張ることによって、したがって、その後で取り出すために、圧縮された人工弁を除去カテーテル内まで引っ張ることによって、人工弁を折りたたまれた圧縮構造に導く。
ここで図を参照すると、図1は、一端ではテザー取付構造114を有し、他端では管状ステント112がカフ116に取り付けられている管状ステント112を含む、本発明による人工心臓弁110の一実施形態を示す。弁葉アセンブリ118(図示せず)がステント112内に配設され、弁葉120(同じく図示せず)を支持する。カフ116は、独立した連接するワイヤのループ122およびカバー124を有する。
Claims (20)
- 人工僧帽弁または人工三尖弁としての人工心臓弁であって、
カフとカフカバーと拡張可能な内部弁葉アセンブリとを有する拡張可能な管状ステントを含み、
前記カフは、径方向に延びる複数のループから構成され、前記ループの径方向外縁は、前記カフの外周縁に向かって延び、前記カフは、遠位対向側と近位対向側とを有し、前記カフカバーは、前記カフ上に配置され、前記カフの前記遠位対向側、前記近位対向側および前記外周縁を覆って、前記ループの前記径方向外縁間に、隣り合うループ間の空間にわたって延びており、前記カフは、個々の前記ループが生来の僧帽弁または三尖弁の弁輪の輪郭に一致する形状を心周期中維持するよう独立して動くように、構成されている、
人工心臓弁。 - 前記ループは異なる長さを有し、横長形状のカフを規定する、請求項1に記載の人工心臓弁。
