Abstract
Description
本発明は概して、医療用デバイスおよび方法に関する。さらに詳細には、本発明は、人工心臓弁、身体管腔のスキャフォールディング(scaffolding)を提供するための構造、ならびにこれらの弁および構造を送達、配備するためのデバイスおよび方法に関する。
心臓弁の疾患および他の障害は、心臓からの血液の正しい流れに影響を与える。心臓弁疾患の2つのカテゴリーは、狭窄および機能不全である。狭窄とは、硬直した弁組織により、弁が完全に開くことができないことを指す。機能不全とは、心臓において血液の逆流を許すことにより、非効率な血液の循環を引き起こす弁を指す。
シースと、複数のグリッパと、エキスパンダと、複数のストラットとを含む。上述されたような、随意的な非外傷性の先端またはノーズコーンも、デバイスの遠位端に固定され得る。グリッパの各々は、チューブ内を動くワイヤと、チューブの遠位端における先端354とを含む。各グリッパのワイヤは、複数のセグメントを有する人工弁支持部材の頂点と係合し、収縮された状態に人工弁を選択的に制止するように適合される。エキスパンダは、それが、ハンドルに位置するアクチュエータを介してユーザによってアクチュエートされるとき、グリッパを半径方向外向きに選択的に拡張させるように適合される。
本発明が記述される前に、本発明は、記述される特定の実施形態に限定されないことが理解されるべきである。なぜならば、記述される特定の実施形態が、もちろん変化し得るからある。本明細書に使用される用語は、特定の実施形態のみを記述するためのものであり、限定するものとしては意図されていないことを理解されるべきである。なぜならば、本発明の範囲は、添付された請求項によってのみ限定されるからである。
最初に図1Aを参照して、人工弁の実施形態が示される。人工弁30は、取替の大動脈弁として使用されるように特に適合されるが、他の症状に対しても使用され得る。図示のように、人工弁30は、ほぼ円筒状の支持部材32と支持部材の内面に取り付けられた弁本体34とを含む。ほぼ円筒状の支持部材が示されているが、人工弁または支持構造が使用されるように意図されている治療位置および環境の性質によっては、円形の断面形状以外、例えば長円形、楕円形、または不規則な形状を有する支持部材も提供され得る。
人工弁を身体管腔の治療位置に送達するためのデバイスが以下に記述され、それらの使用方法も記述される。送達デバイスは、例えば経皮的な大動脈弁取替のような最少に侵襲性の治療的処置における使用に対して特に適合される。図14Aおよび図15Aは、デバイスの2つの実施形態を例示する。送達デバイス300は、近位端304および遠位端306を有する細長い送達カテーテル302を含む。ハンドル308が、送達カテーテルの近位端に提供される。ハンドル308には、人工弁送達処置を実行するようにカテーテルを制御、操作するためのノブ310、アクチュエータ、スライダ、他の制御部材、またはこれらの組み合わせが提供され得る。引込み可能な外側シース312が、カテーテルの長さの少なくとも一部分にわたって延長し得る。好ましくは、ガイドワイヤ管腔が、カテーテルの遠位端から近位方向に延び得る。ガイドワイヤ管腔は、ワイヤの上での適用のために、カテーテルの全長を貫通し得るか、またはガイドワイヤ管腔は、迅速交換の適用に対して使用されるために、カテーテルの近位端よりも遠位端により近い近位の出口ポートを有し得る。カテーテルの遠位部分306は、収縮された状態の人工弁を受け取り、保持し、かつ身体管腔内の治療位置に人工弁を配備するように適合されたキャリアを含む。

複数の偏向可能なパネル区間および2つの隣接するパネル区間の間に位置する少なくとも1つのヒンジを備えた弁支持構造を備えている、デバイス。 - 前記ヒンジはリビングヒンジを備えている、請求項1に記載の人工弁デバイス。
- 前記ヒンジは、ワイヤループである、請求項1に記載の人工弁デバイス。
- 少なくとも1つのパネルに結合された補強部材をさらに備えている、請求項1に記載の人工弁デバイス。
- 前記補強部材は、前記パネルの外側に結合される請求項4に記載の人工弁デバイス。
- 前記補強部材は、アンカー区間を備えている、請求項5に記載の人工弁デバイス。
- 前記補強部材は、前記パネルの内側に結合される、請求項4に記載の人工弁デバイス。

- 前記補強部材は、前記ヒンジを備えている、請求項4に記載の人工弁デバイス。
