Description
本発明は、全般的には医療装置および方法に関する。より具体的には、本発明は補綴心臓弁と、体内管腔のスキャホールド(scaffoldong)を提供する構造と、これらの弁および構造を送達および配置するための装置および方法とに関する。
心臓弁の疾患および他の障害は、心臓からの正常な血流に影響する。心臓弁の疾患は、狭窄および機能不全の2種類である。狭窄とは、弁組織の硬化によって弁が十分に開かないことである。機能不全とは、弁が心臓内の血液の逆流を許すために血液循環の効率が下がることである。
本発明は、支持構造を体内管腔内に配置するための装置および方法を提供する。これらの方法および装置は、経皮的大動脈弁置換での使用に特に適している。これらの方法および装置は、周縁脈管構造、腹脈管構造のほか、胆管、ファロピー管などの管、および患者の体内の同様の管腔構造なども使用しうる。これらの装置および方法は、人体内の管腔での使用に特に適しているが、動物の治療にも使用しうる。
本発明を説明する前に、本発明は本願明細書で説明する具体的な実施形態に限定されるものではなく、当然変更しうるものであることを理解されたい。また、本発明の範囲は添付請求の範囲によってのみ限定され、本願明細書で使用する用語は具体的な実施形態を説明することのみを目的としており、限定するためのものではないことを理解されたい。
最初に図1Aにおいて、補綴弁の一実施形態が図示されている。補綴弁30は、置換用大動脈弁としての使用に特に適しているが、他の用途にも使用しうる。図示のように、補綴弁30は、略円筒形の支持部材32と、この支持部材の内面に取り付けられた弁体34とを有する。略円筒形の支持部材が図示されているが、この補綴弁または支持構造を使用する治療部位および環境の性質によっては、断面が円形以外、たとえば長円形、楕円形、または不整形などの支持部材を設けてもよい。
補綴弁を体内管腔内の治療部位に送達するための各装置とその使用方法とを以下に説明する。本願の各送達装置は、経皮的大動脈弁置換などの低侵襲的介入処置に特に適している。図14Aおよび15Aは、送達装置の2つの実施形態を示す。送達装置300は、近位端304と遠位端306とを有する細長い送達用カテーテル302を含む。送達用カテーテルの近位端には、ハンドル308が設けられている。ハンドル308には、補綴弁送達処理を行うためにカテーテルを制御および操作するノブ310、アクチュエータ、スライダ、他の制御部材、またはこれらの組み合わせを設けてもよい。カテーテルの長さの少なくとも一部にわたって後退可能な外装シース312を延在させてもよい。カテーテルの遠位端から近位方向にガイドワイヤ用管腔を延在させることが好ましい。オーバーザワイヤ方式の場合は、ガイドワイヤ用管腔をカテーテルの全長にわたって延在させてもよい。ラピッドエクスチェンジ方式の場合は、カテーテルの近位端より遠位端に近い近位出口ポートをガイドワイヤ用管腔に設けてもよい。カテーテルの遠位部306は、収縮状態の補綴弁を収容および保持すると共に、この補綴弁を体内管腔内の治療部位に配置するようになっているキャリアを含む。
Claims (53)
- 補綴心臓弁であって、
複数のセグメントと、2つの隣接セグメントを連結する少なくとも1つの折り畳み可能な接合部材とを備え、拡張状態と収縮状態とを有する支持部材であって、各セグメントがパネルを備え、前記折り畳み可能な接合部材がヒンジを備える支持部材と、
前記支持部材に取り付けられている複数の弁葉を有する弁体とを備え、前記支持部材の収縮状態において、前記複数のセグメントの各々が内側に反転されるか、または前記複数のセグメントの1つが内側に反転されて残りの内側に反転されないセグメントと入れ子式配列を形成する、補綴心臓弁。 - 請求項1に記載の補綴心臓弁であって、前記支持部材が少なくとも3つのセグメントを備える補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、拡張状態のときに前記支持部材が略管状である補綴心臓弁。
- 請求項3に記載の補綴心臓弁であって、拡張状態のときに前記支持部材が略円筒形である補綴心臓弁。
- 請求項3に記載の補綴心臓弁であって、前記支持部材が前記拡張状態のときに、前記支持部材の断面形状が略長円形である補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記ヒンジが機械ヒンジ、膜状ヒンジ、または一体ヒンジのうちの1つを備える補綴心臓弁。
- 請求項6に記載の補綴心臓弁であって、前記ヒンジが、取り外し可能なピンを有する機械ヒンジを備える補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記支持部材が固定用部材をさらに備える補綴心臓弁。
- 請求項8に記載の補綴心臓弁であって、前記固定用部材が送達用姿勢と配置用姿勢とを有し、前記固定用部材をその送達用姿勢からその配置用状態に移行させるようになっているアクチュエータをさらに備える補綴心臓弁。
- 請求項8に記載の補綴心臓弁であって、前記固定用部材が送達用姿勢と配置用姿勢とを有し、前記固定用部材がその送達用姿勢からその配置用姿勢に自身で移行する補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記支持部材がその収縮状態にあるとき、前記複数のセグメントのすべてが1つの略管状形状に形成されている補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記支持部材がその収縮状態にあるとき、前記複数のセグメントがそれぞれ内側に反り、各折り畳み可能な接合部材が細長い頂点を形成している補綴心臓弁。
- 請求項12に記載の補綴心臓弁であって、前記支持部材がその収縮状態にあるとき、各隣接セグメント対がローブを形成している補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記弁体の前記複数の弁葉が分離している補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記弁体が動物組織を備える補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記弁体がヒト組織を備える補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記弁体の前記複数の弁葉が被覆された基材を備える補綴心臓弁。
- 請求項17に記載の補綴心臓弁であって、前記基材がメッシュ材料を備える補綴心臓弁。
- 請求項17に記載の補綴心臓弁であって、前記弁体がポリマー材料で被覆された基材を備える補綴心臓弁。
- 請求項19に記載の補綴心臓弁であって、前記ポリマー材料がポリウレタンを含む補綴心臓弁。
- 請求項1に記載の補綴心臓弁であって、前記弁体が前記支持部材の内部に取り付けられている補綴心臓弁。
- スキャホールド力を体内管腔の内部に提供するようになっている支持部材であって、
複数の周縁セグメントと、隣接する2つの周縁セグメントを連結する少なくとも1つの折り畳み可能な接合部材とを備え、第1の最大断面寸法を有する拡張状態と、第2の最大断面寸法を有する収縮状態とを有し、前記第1の最大断面寸法が前記第2の最大断面寸法より大きく、前記支持部材の収縮状態において、前記複数の周縁セグメントの各々が内側に反転されるか、または前記複数の周縁セグメントの1つが内側に反転されて残りの内側に反転されない周縁セグメントと入れ子式配列を形成し、各周縁セグメントがパネルを備え、前記折り畳み可能な接合部材がヒンジを備える、支持部材。 - 請求項22に記載の支持部材であって、少なくとも3つの周縁セグメントを備える支持部材。

- 請求項36に記載のシステムであって、前記補綴心臓弁を取り付けることが、前記補綴心臓弁を前記収縮状態にするために、前記弁拘束部材と前記補綴心臓弁との間で相対的回転を引き起こすことを含むシステム。
