Abstract

El sitio de remate 5’ de un ARNm comprende un resto de guanosina metilada en N7 unido al resto del extremo 5’ del ARNm por medio de un enlace 5'-5' trifosfato. La región de remate 5’ de un ARNm se considera que
65 incluye la estructura de remate 5’ en sí misma así como los primeros 50 nucleótidos adyacentes al sitio de remate.

5 Los compuestos de la presente invención son útiles para investigación y diagnóstico, debido a que estos compuestos se hibridan con ácidos nucleicos que codifican apolipoproteína C-III. Por ejemplos, los oligonucleótidos que se ha demostrado que se hibridan con la eficacia y en las condiciones que se divulgan en el presente documento como para ser inhibidores eficaces de la apolipoproteína C-III también serán cebadores y sondas eficaces en condiciones que favorecen la amplificación o la detección de genes, respectivamente. Estos cebadores y sondas son útiles en procedimientos que requieren la detección específica de moléculas de ácidos nucleicos que codifican la apolipoproteína C-III y en la amplificación de dichas moléuclas de ácidos nucleicos para la detección o para su uso en estudios posteriores de la apolipoproteína C-III. La hibridación de los oligonucleótidos antisentido, particularmente los cebadores y sondas, de la invención con un ácido nucleico que codifica la apolipoproteína C-III puede detectarse por medios conocidos en la técnica. Dichos medios pueden incluir la conjugación de una enzima al
15 oligonucleótido, radiomarcado del oligonucleótido o cualquier otro medio de detección adecuado. También pueden prepararse los kits que usan dichos medios de detección para detectar el nivel de apolipoproteína C-III en una muestra.
También se divulga un procedimiento para preparar un compuesto de la invención que comprende hibridar específicamente in vitro una primera cadena de bases nitrogenadas de al menos 8 bases nitrogenadas contiguas de la secuencia establecida en SEQ ID Nº: 4 y/o la SEC ID N.º: 18 a una segunda cadena de bases nitrogenadas que comprende una secuencia lo suficientemente complementaria a dicha primera cadena de tal modo que se permita una hibridación estable.

También se divulga en el presente documento el uso de un compuesto o composición de la divulgación en la fabricación de un medicamento para el tratamiento de todas las afecciones divulgadas en el presente documento y de cualquiera de las mismas.
Entre los usos de diagnóstico está la medición de apoliproteína C-III en pacientes para identificar a los que pueden beneficiarse de una estrategia de tratamiento dirigida a reducir los niveles de apolipoproteína C-III. Dichos pacientes adecuados para diagnóstico incluyen pacientes con hipertrigliceridemia (por ejemplo, para tendencias de diagnóstico para enfermedad arterial coronaria), metabolismo de lípidos anormal, obesidad, hiperlipidemia, entre otros
35 trastornos.
La especificidad y sensitividad de antisentido también se usan de un modo controlado por los expertos en la técnica para usos terapéuticos. Los compuestos antisentido se han usado como restos terapéuticos en el tratamiento de estados patológicos en animales, incluidos seres humanos. Los fármacos de oligonucleótidos antisentido, incluidos ribozimas, se han administrado de un modo inocuo y eficaz a seres humanos y actualmente se están llevando a cabo numerosos ensayos clínicos. De este modo, se ha establecido que los compuestos antisentido pueden ser útiles en modalidades terapéuticas que puede configurarse que son útiles en regímenes de tratamiento para el tratamiento de células, tejidos y animales, especialemente seres humanos.
45 Para terapia, un animal, preferentemente un ser humano, sospechoso de tener una enfermedad o trastorno que puede tratarse modulando la expresión de la apolipoproteína C-III se trata administrando compuestos antisentido según la divulgación. Por ejemplo, en una realización no limitante, los procedimientos comprenden la etapa de administrar al animal con necesidad de tratamiento una cantidad terapéuticamente eficaz de un inhibidor de la apolipoproteína C-III. Los inhibidores de la apolipoproteína C-III de la presente divulgación inhiben eficazmente la actividad de la proteína apolipoproteína C-III o inhiben la expresión de la proteína apoliproteína C-III. Por ejemplo, un compuesto de este tipo que reduzca los niveles de apolipoproteína C-III es útil para prevenir la morbimortalidad de sujetos con transtornos cardiacos. Por ejemplo, tal como se demuestra en los ejemplos, la reducción de apolipoproteína C-III puede dar como resultado la reducción de los niveles de colesterol, triglicéridos y glucosa en el suero. De este modo, los inhibidores de la apolipoproteína C-III son útiles en el tratamiento de hipertrigliceridemia,

En una realización, la actividad o expresión de la apolipoproteína C-III en un animal se inhibe aproximadamente el 10 %. Preferentemente, la actividad o expresión de la apolipoproteína C-III en un animal se inhibe aproximadamente el 30 %. Más preferentemente, la actividad o expresión de la apolipoproteína C-III en un animal se inhibe aproximadamente el 50 %. De este modo, los compuestos oligoméricos modulan la expresión de ARNm de apolipoproteína C-III en al menos el 10 %, en al menos el 20 %, en al menos el 25 %, en al menos el 30 %, en al menos el 40 %, en al menos el 50 %, en al menos el 60 %, en al menos el 70 %, en al menos el 75 %, en al menos

Las uniones entre nucleósidos (esqueleto) son fosforotioato (P=S) a lo largo del oligonucleótido. Todos los restos de citidina son 5-metilcitidinas. Los compuestos se analizan para determinar su efecto sobre niveles de ARNm de apoliproteína C-III humana mediante PCR cuantitativa en tiempo real tal como se describe en otros ejemplos en el presente documento. Los datos son datos promedio de los tres experimentos en los que se trataron células HepG2 con los oligonucleótidos antisentido. El control positivo para cada dato puntual se identifica en la tabla por el número de secuencia ID. Si está presente, "N.D." indica "sin datos".

Se desiñó una segunda serie de compuestos antisentido para dirigirla a diferentes regiones del ARN de apolipoproteína C-III de ratón usando secuencias publicadas (número de acceso del GenBank L04150.1, que se incorpora al presente documento como secuencia SEC ID Nº: 11). Los compuestos se muestran en la tabla 2. "Sitio diana" indica el primer nucleótido (extremo 5’) numerado en el ácido nucleico diana particular al que se une el compuesto. Todos los compuestos de la tabla 2 son oligonucleótidos quiméricos ("gapmers") de 20 nucleótidos de longitud, compuestos por una región “hueco” que consta de diez 2’-desoxinucleótidos, que están flanqueados por ambos lados (direcciones 5’ y 3’) por “flancos” de cinco nucleótidos. Debido a que se ha encontrado por experimentación que estos “segmentos diana preferentes” están abiertos a, y accesibles a, hibridación con los compuestos antisentido de la presente divulgación, un experto en la técncia reconocerá o será capaz de determinar, usando nada más que experimentación de rutina, otras realizaciones que abarcan otros compuestos que se hibridan específicamente con estos segmentos diana 45 preferentes y, en consecuencia, inhiben la expresión de la apolipoproteína C-III.

Además, los oligonucleótidos antisentido dirigidos contra apolipoproteína C-III son capaces de disminuir los niveles de ARNm de apolipoproteína C-III in vivo en una medida similar en ratones alimentados con una dieta rica en grasas y ratones magros. Estos resultados sugieren un efecto de toxicidad en hígado secundario para ISIS 167880 en ratones alimentados con una dieta rica en grasas pero ninguna toxicidad para ISIS 167880 o 167879 en ratones alimentados con una dieta de roedor normal.
Ejemplo 25: Inhibición antisentido de ARNm de apolipoproteína C-III in vivo
55 Se usaron ratones C57BL/6, una cepa de la que se ha informado que es susceptible a la formación inducida por hiperlipidemia de placas ateroescleróticas, en los estudios siguientes para evaluar oligonucleótidos antisentido de apolipoproteína C-III como agentes potenciales para reducir los niveles de colesterol y triglicéridos. Se analizaron ratones C57BL/6 machos (n=8; 7 a 8 semanas que edad) que recibieron una dieta rica en grasas (60 % kcal de grasas) para evaluar la expresión de ARNm de apolipoproteína C-III en hígado 6 semanas después del trtamiento con oligonucleótidos antisentido dirigidos a apolipoproteína C-III. Los ratones recibieron dos veces a la semana inyecciones intraperitoneales a una dosis de 25 mg/kg de ISIS 167880 (SEC ID Nº: 117), ISIS
65 167875 (SEC ID Nº: 113), ISIS 167878 (SEC ID Nº: 115) o ISIS 167879 (SEC ID Nº: 116). Los animales control
recibieron tratamiento de solución salina dos veces a la semana durante un periodo de 6 semanas.
Al terminar el estudio, cuarenta y ocho horas después de las inyecciones finales, los animales se sacrificaron y se evaluaron para determinar la expresión de ARNm de apolipoproteína C-III en el hígado. El ARN se
5 aisló del hígado y el ARNm se cuantificó tal como se describe en el presente documento. Se hizo la media de los niveles de ARNm de apolipoproteína C-III de cada grupo de tratamiento (n=8). Con relación a los animales tratados con solución salina, el tratamiento con ISIS 167875, ISIS 167878, ISIS 167879 e ISIS 167880 dieron como resultado una reducción del 24 %, 56 %, 50 % y 77 % de los niveles de ARNm de apolipoproteína C-III, respectivamente, lo que demuestra que estos compuestos reducen significativamente la expresión de ARNm de apolipoproteína C-III en el hígado.

Se observó una reducción significativa de los niveles de triglicéridos en suero después del tratamiento con ISIS 167875, ISIS 167878, ISIS 167879 e ISIS 167880, que redujeron los niveles de triglicéridos el 22 %, 40 %, 52 % y 58 %, respectivamente. Esta reducción de triglicéridos en suero se correlaciona con la reducción de la expresión de ARNm de apolipoproteína C-III en hígado. Se diseñó una serie adicional de oligonucleótidos para diferentes regiones diana del ARNm de aplipoproteína C-III de hámster usando secuencias descritas en el presente documento (SEC ID Nº: 352). Los oligonucleótidos se muestran en la tabla 19. "Sitio diana" indica el primer nucleótido (extremo 5’) numerado en la secuencia diana particular 45 al que se une el compuesto. Todos los compuestos de la tabla 19 son oligonucleótidos quiméricos ("gapmers") de 20 nucleótidos de longitud, compuestos por una región “hueco” que consta de diez 2’-desoxinucleótidos, que está flanqueada por ambos lados (direcciones 5’ y 3’) por “flancos” de cinco nucleótidos. Ejemplo 40: Inhibición antisentido de apolipoproteína C-III de hámster mediante oligonucleótidos de fosforotioato quiméricos que tienen flancos 2’-MOE y un hueco desoxi. Estudios de respuesta a la dosis en hepatocitos de hámster primarios
Se seleccionaron seis oligonucleótidos dirigidos a apolipoproteína C-III de hámster para los estudios de respuesta a la dosis adicionales. Los hepatocitos de hámster primarios se trataron con una concentración 50, 150 y 300 nM de ISIS 352939 (SEC ID Nº: 369), ISIS 352952 (SEC ID Nº: 380), ISIS 352962 (SEC ID Nº: 389), ISIS 55 352963 (SEC ID Nº: 390), ISIS 352971 (SEC ID Nº: 397) o ISIS 352977 (SEC ID Nº: 403) y se midieron los niveles de ARNm 24 horas después del tratamiento con oligonucleótidos tal como se describe en otros ejemplos del presente documento. Las células no tratadas sirvieron como control con respecto al que se normalizaron los datos. Los resultados de estos estudios se muestran en la tabla 20. Los datos se promediaron a partir de tres experimentos y se expresaron como porcentaje de inhibición con relación a los controles no tratados.
