- ALLGEMEINER STAND DER TECHNIK
- Eine Art endoprothetische Vorrichtung, die üblicherweise als Stent bezeichnet wird, wird in einer Vene, Arterie oder einem anderen röhrenförmigen Körperorgan angeordnet oder dorthinein implantiert, um Verschlüsse, Stenosen oder Aneurysmen des Gefäßes zu behandeln. Diese Stentvorrichtungen werden in röhrenförmige Gefäße implantiert, um zusammenfallende, teilweise verschlossene, geschwächte oder unnormal erweiterte Abschnitte der Gefäßwand zu verstärken. Stents wurden zur Behandlung der Dissektion von Blutgefäßwänden eingesetzt, die durch die ballonangioplastische Behandlung der Herzkranzarterien sowie von peripheren Arterien verursacht wurde und zur Verbesserung der Ergebnisse der Angioplastie durch Verhinderung der elastischen Rückverformung und Umgestaltung der Gefäßwand.
- Stents wurden auch bereits erfolgreich in den Harnwegen, dem Gallengang, der Speiseröhre und dem Tracheobronchialbaum zur Verstärkung dieser Körperorgane eingesetzt. Zwei randomisierte Versuche in mehreren Einrichtungen haben kürzlich im Vergleich zur ausschließlichen Ballonangioplastie eine niedrigere Restenoserate bei Herzkranzarterien ergeben, die mit Stents behandelt wurden (Serruys PW et al. New England Journal of Medicine 331:489–495, 1994, Fischmman DL et al. New England Journal of Medicine 331:496–501, 1994).
- Einer der Nachteile herkömmlicher Stents besteht darin, dass sie mit einer geraden röhrenförmigen Gestalt hergestellt werden. Bei der Verwendung dieser Stents zur Behandlung von Erkrankungen an oder in der Nähe eines Asts oder einer Gabelung eines Gefäßes besteht die Gefahr, dass der Grad der Durchgängigkeit des Hauptgefäßes und/oder seiner Äste oder Gabelung beeinträchtigt wird und wird auch die Einführbarkeit eines zweiten Stents in den Seitenast eingeschränkt, wenn das Ergebnis der Angioplastie nicht optimal ist. Dies kann als Ergebnis verschiedener Vorgänge geschehen, beispielsweise der Verlagerung von erkranktem Gewebe oder der Verlagerung der Plaque, Gefäßkrämpfen, Dissektion mit oder ohne Dissektionsmembran, Thrombose und Embolien.
- Die Gefahr der Beeinträchtigung eines Asts ist in zwei anatomischen Situationen erhöht. Erstens kann der Seitenast beeinträchtigt sein, wenn am Ursprung des Seitenasts eine Stenose vorliegt. Zweitens kann, wenn an der Stelle der Gabelung eine außermittige Läsion vorliegt, eine unsymmetrische Ausdehnung entweder zu einer Verlagerung der Plaque oder einer Dissektion am Ursprung des Seitenasts führen. Es sind Versuche zur Lösung dieses Problems durch Einführen eines Ballons durch die Stentstreben in den Seitenast hinein beschrieben; jedoch weist dieses Verfahren die Gefahr des Einschlusses des Ballons und anderer schwerer Komplikationen auf (Nakamura, S. et al., Catheterization and Cardiovascular Diagnosis 34:353–361 (1995)). Darüber hinaus ist eine angemessene Aufweitung des Seitenasts durch die elastische Rückverformung des Ursprungs des Seitenasts begrenzt. Außerdem kann der Stent eine Behinderung für den Blutstrom darstellen und den Zugang zum Seitenast einschränken. Zur Beschreibung dieser Vorstellung wird häufig der Begriff „Stentgefängnis" („stent jail") verwendet. In dieser Hinsicht wird insbesondere die röhrenförmige gelenkige Ausgestaltung des Palmaz-Schatz-Intrakoronarstents und Schlitzen als ungünstig bei Läsionen bei einem großen Seitenast erachtet und wird vermutet, dass sie eine größere Gefahr eines eingeschlossenen Seitenasts mit sich bringt, wobei der Stent den Zugang zum Seitenast verhindert oder einschränkt. Id.
- Ein übliches Verfahren zum Einpflanzen der Endoprothese oder des Stents besteht darin, den Bereich des Gefäßes zuerst mit einem Ballonkatheter zu öffnen und anschließend den Stent an einer Stelle anzuordnen, die den behandelten Bereich des Gefäßes überbrückt, um eine elastische Rückverformung und eine Restenose dieses Abschnitts zu verhindern. Die angioplastische Behandlung von Läsionen an einer Gabelung wurde herkömmlich unter Verwendung des Kissing-Balloon-Verfahrens durchgeführt, bei dem zwei Führungsdrähte und zwei Ballons eingeführt werden, einer in den Hauptast und der andere in den Seitenast. Bei der Anordnung des Stents in dieser Situation muss der Führungsdraht aus dem Seitenast herausgenommen und der Führungsdraht über die Stentstreben wieder eingeführt und ein Ballon durch die Streben des Stents eingeführt werden. Bei der Entfernung des Führungsdrahts besteht die Gefahr des Verschlusses des Seitenasts während der Entfaltung des Stents im Hauptast.
- Patente nach dem Stand der Technik betreffen den Aufbau und die Ausgestaltung sowohl des Stents als auch der Vorrichtung zum Anordnen des Stents im Gefäß. In einem typischen Patent (Chaudhury), der US-Patentschrift 4,140,126, ist ein Verfahren zum Anordnen eines länglichen zylindrischen Stents in einem Bereich eines Aneurysmas offenbart, um ein katastrophales Versagen der Blutgefäßwand zu verhindern. In dem Patent 4,140,126 wird ein Zylinder offenbart, der sich nach der Einführung mithilfe eines Katheters auf seine implantierte Anordnung ausdehnt. In der US-Patentschrift 4,503,569 (Dotter) ist ein Federstent offenbart, der sich bei einer Temperaturänderung auf eine implantierte Anordnung ausdehnt. Der Federstent wird in einer gewundenen Anordnung implantiert und anschließend erwärmt, damit sich die Feder ausdehnt. In der US-Patentschrift 4,733,665 (Palmaz) sind mehrere Stentanordnungen zur Implantierung mithilfe eines Katheters offenbart. Der Katheter weist eine Vorrichtung auf, mit der die Gefäßprothese oder der Stent, vorzugsweise auf einem füllbaren Abschnitt des Katheters, angebracht und gehalten werden kann. Die Stents werden implantiert, während sie auf einem Bildschirm abgebildet werden. Sobald der Stent richtig angeordnet ist, wird der Katheter ausgedehnt und der Stent vom Körper des Katheters getrennt. Der Katheter kann dann aus dem Patienten entnommen werden, wobei der Stent an der richtigen Stelle im Blutgefäß bleibt. In der US-Patentschrift 4,739,762 (Palmaz) ist eine ausdehnbare Intraluminalprothese offenbart. In der US-Patentschrift 4,413,989 (Schjeldahl et al.) sind verschiedene Ballonkatheteranordnungen offenbart. In der US-Patentschrift 5,456,712 (Maginot) und der US-Patentschrift 5,304,220 (Maginot) sind eine Prothesen- und Stentanordung und ein Implantierverfahren offenbart, wobei ein Stent zur Verstärkung einer Prothese verwendet wird, die bei einem Eingriff in ein Blutgefäß eingebracht wird, um einen Verschluss zu umgehen. Jedoch betrifft keines dieser Patente die Behandlung von Läsionen an Gabelungen noch offenbart es eine verzweigte Stentvorrichtung und ein Verfahren zu ihrer Entfaltung.
- In der US-Patentschrift 4,872,874 (Taheri), der US-Patentschrift 5,489,295 (Piplani et al.) und der US-Patentschrift 5,507,769 (Marin et al.) ist ein verzweigtes Prothesenmaterial offenbart, das mit Stents implantiert werden kann. Es wird jedoch keine Gabelung des Stents erwähnt und der Stent wird lediglich dazu verwendet, die Prothese in der Gefäßwand zu verankern. Er verstärkt weder die Gefäßwand noch verhindert er nach dem angioplastischen Eingriff Restenosen.
- In der US-Patentschrift 4,994,071 (MacGregor) ist ein verzweigter gelenkiger Stent offenbart. In dem Patent 4,994,071 ist, im Gegensatz zur vorliegenden Erfindung, ein Hauptstent mit zwei zusätzlichen Stents vorhanden, die an einem Ende befestigt sind, wodurch eine einzige Einheit mit einer Gabelung entsteht. Die zwei zusätzlichen Stents sind dauerhaft befestigt und können nicht vom Hauptstent entfernt werden. Daher kann diese Erfindung bei Gefäßen ohne Gabelung nicht verwendet werden. Außerdem haben Untersuchungen an Stents mit Gelenken ergeben, dass am Gelenkpunkt häufig Restenosen (Gewebewachstum) vorkommen, durch die die Verengung oder der vollständige Verschluss des Gefäßes bewirkt werden kann und damit der Blutstrom beeinträchtigt wird. Ferner ist diese Ausgestaltung verhältnismäßig groß, wodurch die Einführung in das Gefäß schwierig wird. Da die beiden zusätzlichen Stents mit dem Hauptstent verbunden sind, kann außerdem das Führen in einen Seitenast mit großem Winkel schwierig sein und die Gefahr der Dissektion der Gefäßwand mit sich bringen. Sobald die Vorrichtung des Patents 4,994,071 implantiert ist, ist es ferner unmöglich, den Stent für den Seitenast auszutauschen, sollte eine andere Stentgröße erforderlich werden. Der Stand der Technik gemäß WO-A-96 29955 (
14 bis16 ) betrifft einen Stent mit einer Öffnung in der Seitenwand, wobei sich in dieser Öffnung ein Stentabschnitt befinden kann. - Im Allgemeinen sollte bei der Behandlung von Läsionen an einer Gabelung unter Verwendung handelsüblicher Stent sehr darauf geachtet werden, dass der Ursprung des Asts bedeckt wird, da dieser Bereich anfällig für Restenosen ist, wenn er nicht bedeckt ist. Damit der Ursprung des Asts bedeckt wird, müssen herkömmliche Stents entweder vom Ast aus in den Hohlraum der Hauptarterie oder des Hauptgefäßes hineinragen (was eine Thrombose [Blutgerinnung] bewirken kann, die wiederum den Blutstrom beeinträchtigt) oder sie müssen vollständig im Ast angeordnet sein und bedecken im Allgemeinen nicht den Ursprung der Gabelung. Eine weitere häufige Komplikation, die bei der Verwendung von Stents an Gabelungen auftritt, umfasst die Verengung oder den Verschluss des Ursprungs eines Seitenasts, über den sich ein Stent erstreckt, der im Hauptast angeordnet ist. Zuletzt kann die Anordnung eines Stents in einem Hauptgefäß, wobei sich der Stent teilweise oder vollständig über die Öffnung eines Asts erstreckt, den späteren Zugang in diese abzweigenden Gefäße erschweren, wenn nicht gar unmöglich machen.
- Außerdem ist die herkömmliche Stenttechnologie als Mittel zur Behandlung von ostialen Läsionen unzulänglich. Ostiale Läsionen sind Läsionen am Ursprung eines Gefäßes. Beispielsweise können ostiale Läsionen in Nierenarterien entstehen, die Seitenkanäle sind, die von der Aorta ausgehen. Ostiale Läsionen sind aufgrund der elastischen Rückverformung des Hauptgefäßes wie der Aorta anfällig für Restenosen. Daher muss die Abdeckung durch den Stent die Wanddicke des Hauptgefäßes umfassen. Ohne dass der Stents in das Hauptgefäß hineinragt, ist dies äußerst schwer zu erreichen.
- Schließlich sind herkömmliche Stents während und nach der Entfaltung schwer sichtbar zu machen. Obwohl einige Ballonkatheter nach dem Stand der Technik am proximalen und distalen Ende des Ballons mit abbildbaren Stücken „markiert" sind, stehen derzeit keine Stents zur Verfügung, die von der FDA zugelassen sind, die mit gegenwärtig bekannten bildgebenden Verfahren, die beim Einführen der Stents in ein Gefäß verwendet werden, selbst abgebildet werden können.
- Folglich besteht Bedarf an einer verbesserten Stentvorrichtung und einem verbesserten Verfahren zu ihrer Entfaltung, die 1) zur wirksamen Behandlung von Läsionen an Gabelungen verwendet werden können, wodurch die Gefahr einer Restenose oder eines Verschlusses des Seitenasts verringert wird und wodurch Läsionen an Gabelungen vollständig mit der Stentvorrichtung bedeckt werden, 2) zur Behandlung von Läsionen in einem Ast einer Gabelung verwendet werden können, während für spätere Behandlungen der Zugang zum anderen Ast erhalten bleibt, 3) zur Behandlung von ostialen Läsionen verwendet werden können, 4) eine unterschiedliche Bemessung der Stents in einer verzweigten Stentvorrichtung ermöglicht, selbst nachdem der Hauptstent implantiert ist, und 5) ohne Weiteres mit derzeitigen oder zukünftigen Verfahren zur Sichtbarmachung sichtbar dargestellt werden kann.
- KURZDARSTELLUNG DER ERFINDUNG
- Die vorliegende Erfindung betrifft neuartige aufweitbare Stentvorrichtungen. Insbesondere betrifft die Erfindung eine Stentvorrichtung, die einen aufweitbaren Stent umfasst, der für die Behandlung von Läsionen an Gabelungen geeignet ist und der auch zur Behandlung von Läsionen am Ursprung eines Blutgefäßes oder anderen Organs verwendet werden kann. Wie er hier verwendet wird, bedeutet der Begriff „Gefäß" jedes röhrenförmige Gewebe und ist nicht auf Gefäße des Gefäßsystems beschränkt. Vorrichtungen, die erfindungsgemäß ausgeführt sind, umfassen, einzeln oder in Verbindung, einen trichterförmig erweiterten Stent, der einen zusammendrückbaren trichterförmig erweiterten Abschnitt an seinem proximalen Ende umfasst, wobei der trichterförmig erweiterte Abschnitt Haken, zusammendrückbare Maschen oder jedes beliebige andere Mittel umfassen kann, mit dem ein derartiger trichterförmig erweiterter Abschnitt am proximalen Ende des Stents hergestellt werden kann, und einen Hauptstent, der mindestens eine im Wesentlichen kreisförmige Öffnung umfasst, die zwischen seinem proximalen und distalen Ende liegt. Damit sie besser sichtbar sind, können sowohl der trichterförmig erweiterte Stent als auch der Hauptstent aus Werkstoffen bestehen, die abgebildet werden können, oder können die Stents der Erfindung an den Enden mit einem abbildbaren Stoff „markiert" sein und kann der Hauptstent auch an einer beliebigen Öffnung „markiert" sein. Es kann mindestens ein trichterförmig erweiterter Stent durch mindestens eine Öffnung des Hauptstents in mindestens ein abzweigendes Gefäß gesteckt werden, um geteilte oder verzweigte Läsionen zu behandeln, oder die Stents der Erfindung können einzeln eingeführt werden, um ostiale Läsionen oder Läsionen in der Nähe von Gabelungen zu behandeln, bei denen ein Stent entweder im Hauptgefäß oder dem abzweigenden Gefäß bei freiem Zugang zu dem Gefäß der Gabelung erforderlich ist, bei dem kein Stent verwendet wird. Das Verfahren umfasst einen zweiteiligen Vorgang, der zur Entfaltung sowohl des Hauptstents als auch des trichterförmig erweiterten Stents in einem verzweigten Gefäß oder zur alleinigen Entfaltung des Hauptstents in einem verzweigten Gefäß verwendet wird.
- Die Stentvorrichtung der Erfindung kann aus jedem Werkstoff entworfen werden, der keine Immunreaktion hervorruft und mit dem die Vorrichtung von einer Ausgangsform aus auf eine Form ausgedehnt werden kann, die sich der Form des Gefäßes oder der Gefäße anpasst, in das bzw. die die Vorrichtung eingeführt wird, einschließlich, jedoch nicht beschränkt auf alle die Werkstoffe, die bei den Stents nach dem Stand der Technik offenbart sind. Es wird vermutet, dass die Stentvorrichtungen der Erfindung ferner aus einem Werkstoff entworfen werden können, der mit bildgebenden Verfahren betrachtet werden kann, einschließlich, jedoch nicht beschränkt auf Magnetresonanz, Ultraschall, röntgenstrahlenundurchlässig oder Kontrastmittel, oder an bestimmten Punkten eines Stents der Erfindung, einschließlich, jedoch nicht beschränkt auf die Enden und um jede Öffnung oder jeden trichterförmig erweiterten Abschnitt herum, mit einem Werkstoff markiert werden können, der durch bildgebende Verfahren wie zuvor beschrieben erkennbar ist.
- Ein Stent, der in Übereinstimmung mit der Erfindung aufgebaut ist, eignet sich für die Implantierung in jedes Gefäß des Körpers, einschließlich, jedoch nicht beschränkt auf Gefäße des Systems des Herzens, des peripheren Gefäßsystems, die Karotis- und intrakranialen Gefäße, des venösen Systems, des Nierensystems, des Gallensystems, des Magen-Darm-Systems, des Tracheobronchialsystems, des Gallensystems und des Urogenitalsystems.
- Die Stents der Erfindung werden unter Verwendung einer Reihe von Führungsdrähten und Kathetern entfaltet, die anschließend, nach der Entfaltung der Stents, aus dem Patienten herausgezogen werden. Die Stents der Erfindung können selbstausdehnend sein, um sich der Form des Gefäßes des Patienten anzupassen oder können unter Verwendung von Ballonkathetern oder mit jedem Verfahren ausgedehnt werden, das derzeit bekannt oder später entwickelt wird, mit dem die Stents der Erfindung wirksam ausgedehnt werden. Der trichterförmig erweiterte Stent der Erfindung ist so entworfen worden, dass der trichterförmig erweiterte Abschnitt entlang der Wand des trichterförmig erweiterten Stents durch eine Hülle eingeengt wird, die bis zur Entfaltung parallel zur Längsachse des trichterförmig erweiterten Stents verläuft, während der die Hülle entfernt wird und der trichterförmig erweiterte Abschnitt ausgedehnt wird, sodass er eine Anordnung aufweist, bei der er sich zumindest teilweise von der Längsachse des trichterförmig erweiterten Stents aus radial aufweitet.
- Es ist damit eine Aufgabe der vorliegenden Erfindung, eine Doppelstentvorrichtung zu schaffen, durch die der Ursprung einer Läsion an einer Gabelung vollständig mit einer Stentvorrichtung bedeckt werden kann.
- Eine weitere Aufgabe ist es, eine Einzelstentvorrichtung und ein Verfahren zu ihrer Entfaltung zu schaffen, die zur Behandlung von lediglich einem Ast einer Läsion an einer Gabelung verwendet werden können, die spätere Behandlung des entsprechenden Asts jedoch erleichtern.
- Eine weitere Aufgabe ist es, eine Einzelstentvorrichtung zu schaffen, mit der wirksam ostiale Läsionen behandelt werden.
- Eine weitere Aufgabe ist es, ein Verfahren zum Einführen der aufweitbaren Doppelstentvorrichtung sowohl in das Hauptgefäß als auch das abzweigende Gefäß einer Läsion an einer Gabelung bereitzustellen.
- Außerdem ist es eine Aufgabe der Erfindung, eine Stentvorrichtung zu schaffen, die während und nach dem Einführen abgebildet werden kann.
- Diese Aufgaben und weitere Aufgaben, Vorteile und Merkmale der Erfindung werden anhand der ausführlichen Beschreibung der Erfindung und der zugehörigen Zeichnungen besser verständlich.
- KURZBESCHREIBUNG DER ZEICHNUNGEN
-
1 ist eine vereinfachte Darstellung der Doppelstentvorrichtung der vorliegenden Erfindung, bei der sowohl der Hauptstent als auch der trichterförmig erweiterte Stent vollständig aufgeweitet sind. -
2 ist eine vereinfachte Darstellung des entfalteten Hauptstents der Vorrichtung der Erfindung, ohne eingebrachten trichterförmig erweiterten Stent. -
3 ist eine vereinfachte Darstellung des entfalteten trichterförmig erweiterten Stents der Vorrichtung ohne den Hauptstent. -
4 ist eine vereinfachte Darstellung des Hauptstents der Vorrichtung, der sich entfaltet im Gefäß eines Patienten befindet. -
5 ist eine vereinfachte Darstellung der verzweigten Doppelstentvorrichtung, bei der der Hauptstent entfaltet ist und in der die Anordnung der trichterförmig erweiterten Stentvorrichtung vor der vollständigen Entfaltung des trichterförmig erweiterten Stents dargestellt ist. -
6 ist eine vereinfachte Darstellung des Verfahrens.6a zeigt die Einbringung des Hauptstents der verzweigten Stentvorrichtung in das Gefäß, zusammen mit der Einführung des Führungsdrahts und des Stabilisierungskatheters zum Einbringen des trichterförmig erweiterten Stents in das abzweigende Gefäß des Patienten. -
6b ist eine vereinfachte Darstellung des Schritts zum Füllen des Hauptstents der Erfindung. -
6c ist eine vereinfachte Darstellung der Entfaltung des trichterförmig erweiterten Stents über den Führungsdraht für den Seitenast durch eine Öffnung im Hauptstent und in das abzweigende Gefäß des Patienten. -
6d ist eine vereinfachte Darstellung der Entfernung der Schutzhülle des trichterförmig erweiterten Stents, wodurch sich der trichterförmig erweiterte Abschnitt vor der Anordnung und Entfaltung vollständig ausdehnen kann. -
6e ist eine vereinfachte Darstellung des vollständig aufgeweiteten trichterförmig erweiterten Stents, der mit dem Katheter in dem Ast angeordnet wurde, jedoch vor der vollständigen Entfaltung. -
6f ist eine vereinfachte Darstellung des vollständig aufgeweiteten Hauptstents und des endgültig angeordneten trichterförmig erweiterten Stents, wobei der trichterförmig erweiterte Stent durch Füllen des Ballons aufgeweitet wird. -
6g ist eine vereinfachte Darstellung des vollständig aufgeweiteten verzweigten Doppelstents der Erfindung, der in einem Gefäß eines Patienten in der Gabelung angeordnet ist. - Die geradlinigen Raster in den Zeichnungen sollen lediglich die Form der Oberflächen zeigen und veranschaulichen nicht die eigentlichen Oberflächenmuster oder die eigentliche äußere Erscheinung der Stentvorrichtungen der Erfindung.
- BESCHREIBUNG DER BEVORZUGTEN AUSFÜHRUNGSFORMEN
- Die verzweigte Doppelstentvorrichtung
10 der vorliegenden Erfindung umfasst einen im Allgemeinen zylindrischen Hauptstent12 und einen im Allgemeinen zylindrischen, trichterförmig erweiterten Stent15 , die vollständig aufgeweitet in einem Hauptgefäß8 eines Patienten und einem abzweigenden Gefäß7 eines Patienten dargestellt sind, wie in1 veranschaulicht ist. - Der Hauptstent
12 weist mindestens eine im Allgemeinen kreisförmige Öffnung16 auf, die sich zwischen dem proximalen Ende26 und dem distalen Ende28 des Hauptstents12 (2 ) befindet, wobei die Öffnung über der Öffnung48 eines abzweigenden Gefäßes einer Gefäßgabelung50 liegt, wie in2 dargestellt ist. Die Enden des Stents12 und die Öffnung werden in bildgebenden Verfahren dargestellt, indem die Marker56 um die Ränder der Öffnung16 im Hauptstent12 herum und am proximalen Ende26 und distalen Ende28 des Hauptstents angeordnet werden, wie in4 dargestellt ist. - Die trichterförmig erweiterte Stentvorrichtung
15 der vorliegenden Erfindung umfasst einen im Allgemeinen zylindrischen, trichterförmig erweiterten Stent, der ein proximales Ende30 und ein distales Ende32 umfasst, wie in3 dargestellt ist. Das proximale Ende30 umfasst einen trichterförmig erweiterten Abschnitt, der hier als die aufgeweiteten Schlaufen18 dargestellt ist, wobei der trichterförmig erweiterte Abschnitt im aufgeweiteten Zustand im Hohlraum58 des Hauptgefäßes8 (3 ) angeordnet ist. Die Enden des trichterförmig erweiterten Stents15 und der trichterförmig erweiterte Abschnitt18 werden in bildgebenden Verfahren dargestellt, indem die Marker56 um den trichterförmig erweiterten Abschnitt18 herum und am proximalen Ende30 und distalen Ende32 des trichterförmig erweiterte Stent angeordnet werden, wie in5 dargestellt ist. - Wie bei der Ausführungsform der Erfindung gezeigt ist, die in
4 dargestellt ist, wird vor der Einführung des Hauptstents12 ein Führungsdraht20 in das Gefäß8 eingeführt und dazu verwendet, den Hauptstent12 im Gefäß8 in die richtige Lage zu bringen. Vor der Einführung und Aufweitung wird der Hauptstent12 um das distale Ende eines Katheters48 herum angeordnet, der einen füllbaren Ballon24 aufweisen kann. Die Hauptstent-/Kathetervorrichtung wird anschließend auf den Hauptführungsdraht20 gefädelt und in das Gefäß8 hinein geführt. Der Hauptstent12 wird durch Füllen des Ballons24 aufgeweitet, bis er die Wände des Gefäßes8 dehnt und wird damit an der richtigen Stelle befestigt. - Wie bei der Ausführungsform der Erfindung gezeigt ist, die in
5 dargestellt ist, werden vor der Einführung des trichterförmig erweiterten Stents15 ein Führungsdraht36 und ein Stabilisierungskatheter44 durch die Öffnung16 im Hauptstent12 und in ein abzweigendes Gefäß eingeführt. Der Stabilisierungskatheter44 wird dazu verwendet, die Öffnung des Hauptstents12 über der Öffnung16 der Gabelung anzuordnen. Der Führungsdraht36 wird dazu verwendet, den trichterförmig erweiterten Stent15 innerhalb eines Gefäßes in die richtige Lage zu bringen. Während der Einführung und vor der Aufweitung wird der trichterförmig erweiterte Stent15 um das distale Ende eines Abzweigungskatheters54 herum angeordnet, der einen füllbaren Ballon25 aufweisen kann und wird der trichterförmig erweiterte Abschnitt18 des trichterförmig erweiterten Stents15 durch eine Schutzhülle34 in einem zusammengedrückten Zustand gehalten. - Bei der verzweigten Doppelstentvorrichtung
10 der Erfindung wird der trichterförmig erweiterte Stent15 , sobald der Hauptstent12 aufgeweitet und der Stabilisierungskatheter44 entfernt ist, über dem Führungsdraht36 für den Seitenast und durch die Öffnung16 des Hauptstents12 im Wesentlichen so eingeführt, wie es in5 dargestellt ist, und an der richtigen Stelle durch Ausdehnung des trichterförmig erweiterten Abschnitts18 befestigt, der am proximalen Ende30 des trichterförmig erweiterten Stents liegt, wie in1 und5 dargestellt ist. Der Winkel, in dem der trichterförmig erweiterte Stent15 befestigt wird, hängt vom Aufbau des Gefäßes ab, in den die verzweigte Stentvorrichtung10 eingeführt wird (1 ). - Das erfindungsgemäße Zweistufenverfahren zur Implantierung der neuartigen verzweigten Doppelstentvorrichtung
10 beginnt mit der Einführung des Hauptführungsdrahts20 in das Hauptgefäß8 des Patienten und über die Gabelung50 . Sobald sich der Hauptführungsdraht20 an der richtigen Stelle im Hauptgefäß8 befindet, wird der Hauptstent12 um einen Katheter48 herum angeordnet (der auch einen Ballon24 umfassen kann), und werden der Katheter48 und der Stent12 in das Hauptgefäß8 eingeführt. Der Stent12 wird so angeordnet, dass sich die Öffnung16 unmittelbar über dem Verzweigungspunkt50 im Gefäß des Patienten befindet (6a ). Als Unterstützung bei der Anordnung werden auch ein Führungsdraht36 für den Seitenast und ein Stabilisierungskatheter44 (wie in5 und6 dargestellt) durch die Öffnung16 des Hauptstents12 und in das abzweigende Gefäß7 eingeführt (6a ). - Bei einer wahlweisen Ausführungsform des Verfahrens können der Hauptstent
12 , die Katheter44 und48 und der Führungsdraht36 für den Seitenast vor der Einführung in den Patienten zusammengesetzt werden (wobei der Stabilisierungskatheter44 und der Führungsdraht für den Seitenast durch die Öffnung16 des Hauptstents hindurch angeordnet sind) und anschließend gleichzeitig in den Verzweigungspunkt50 des Hauptgefäßes8 eingeführt werden, wonach der Führungsdraht36 für den Seitenast und der Stabilisierungskatheter44 in das abzweigende Gefäß7 geführt werden, um die Öffnung16 im Hauptstent12 richtig auszurichten (6a ). - Um den Hauptstent
12 in der gewünschten Lage innerhalb des Gefäßes8 zu befestigen, kann der Stent12 durch Füllen des Ballons24 aufgeweitet werden, bis der Hauptstent12 die Wände des Gefäßes8 berührt (6b ). Sobald der Hauptstent12 aufgeweitet ist, werden die Katheter44 und48 entfernt, wobei der endgültig angeordnete Hauptstent12 und der Hauptführungsdraht20 im Hauptgefäß des Patienten bleiben und der Führungsdraht36 für den Seitenast im abzweigenden Gefäß des Patienten bleibt (6c ). - Im zweiten Schritt des Verfahrens der Entfaltung des verzweigten Doppelstents der Erfindung wird der Katheter
54 für den trichterförmig erweiterten Stent, der den zusammengedrückten trichterförmig erweiterten Stent15 in einer Schutzhülle34 enthält und ferner einen Ballon25 enthalten kann, der um den Katheter54 für den trichterförmig erweiterten Stent herum und in dem zusammengedrückten trichterförmig erweiterten Stent15 angeordnet ist, um den Führungsdraht36 für den Seitenast herum in das abzweigende Gefäß7 des Patienten eingeführt, wie in6c dargestellt ist. Der zusammengedrückte trichterförmig erweiterte Stent15 wird zu Beginn so angeordnet, dass sich das zusammengedrückte proximale Ende30 des trichterförmig erweiterten Stents in den Hohlraum42 des Hauptstents12 erstreckt, um die vollständige Ausdehnung des trichterförmig erweiterten Abschnitts18 nach dem Herausziehen der Schutzhülle34 vor dem endgültigen Anordnen des trichterförmig erweiterten Stents15 in dem Ast der Gabelung zu erleichtern (6c ). Das distale Ende32 des trichterförmig erweiterten Stents ist zu Beginn in dem abzweigenden Gefäß7 angeordnet (6c ). Nachdem das proximale Ende30 des zusammengedrückten trichterförmig erweiterten Stents richtig im Hohlraum42 des Hauptstents angeordnet ist, wird die Schutzhülle34 aus dem Gefäß8 herausgezogen und entspannt sich der trichterförmig erweiterte Abschnitt18 des trichterförmig erweiterten Stents15 , damit er sich zumindest teilweise von der Längsachse des trichterförmig erweiterten Stents15 aus radial aufweitet, wie in6d dargestellt ist. Nachdem der trichterförmig erweiterte Abschnitt18 des trichterförmig erweiterten Stents15 seine trichterförmig erweiterte Anordnung aufweist (wie es in6d dargestellt ist), wird der trichterförmig erweiterte Stent15 an seinem proximalen Ende30 in den Seitenast7 vorgeschoben, bis zumindest ein Abschnitt des trichterförmig erweiterten Abschnitts18 des trichterförmig erweiterten Stents15 zumindest einen Abschnitt des Rands der Öffnung16 des Hauptstents12 berührt, wie in6e dargestellt ist. Bei diesem Beispiel wird ein Ballon25 gefüllt, um den trichterförmig erweiterten Stent15 aufzuweiten, damit die Wände des trichterförmig erweiterten Stents15 die Wände des abzweigenden Gefäßes7 berühren, wie in6f dargestellt ist. Alle verbleibenden Katheter und Führungsdrähte werden anschließend aus dem Patienten herausgezogen, wobei die vollständig entfaltete verzweigte Doppelstentvorrichtung der Erfindung10 , die den Hauptstent12 mit der mindestens einen Öffnung16 umfasst und den trichterförmig erweiterten Stent15 , der durch die Öffnung16 hindurch in dem abzweigenden Gefäß7 angeordnet ist, so bleibt, wie in6g dargestellt ist. - Bei der Behandlung von ostialen Läsionen wird allein der trichterförmig erweiterte Stent
15 verwendet und unter Verwendung von Kathetern und Führungsdrähten wie zuvor beschrieben angeordnet, abgesehen davon, dass kein Stabilisierungskatheter verwendet wird, und der trichterförmig erweiterte Abschnitt18 des trichterförmig erweiterten Stents am Ostium eines Gefäßes anstatt durch die Öffnung16 eines Hauptasts hindurch in einen Seitenast hinein angeordnet wird. Nachdem der trichterförmig erweiterte Stent15 in der Nähe des Ostiums eines Gefäßes eines Patienten angeordnet ist, wird die Schutzhülle34 zurückgezogen, damit sich der trichterförmig erweiterte Abschnitt vollständig ausdehnen kann und wird der trichterförmig erweiterte Stent15 mit dem proximalen Ende des Katheters weiter vorgeschoben, bis die ausgebreiteten Haken18 die Wände des Gefäßes des Patienten berühren. - Alle Stents der Erfindung können unter Verwendung der Verfahren, die in den vorhergehenden Abschnitten erwähnt sind, ohne Verwendung eines Ballonkatheters entfaltet werden. Beispielsweise könnte sich ein selbstausdehnender zusammengedrückter Stent, der sich in einer Schutzhülle befindet, durch Zurückziehen einer Schutzhülle selbst aufweiten. Es können weitere Verfahren der Aufweitung der Stents der Erfindung vorhanden sein oder in Zukunft zur Verfügung stehen und diese Verfahren werden als in den Schutzumfang dieser Erfindung fallend betrachtet. Obwohl bei diesem Beispiel zur Veranschaulichung eines Mittels zum Herstellen eines trichterförmig erweiterten Abschnitts sich selbst ausbreitende Schlaufen verwendet wurden, wird jedes andere Mittel zum Herstellen einer trichterförmigen Aufweitung als in den Schutzumfang dieser Erfindung fallend betrachtet, beispielsweise jedoch nicht beschränkt auf die Herstellung einer Rolle im Stentwerkstoff, die dann zusammengedrückt wird.
- Die Erfindung soll alle Abwandlungen und Abänderungen der offenbarten Ausführungsformen umfassen, die in den Schutzumfang der Ansprüche der Erfindung fallen.
Claims (6)
- Sich aufweitende Doppelstentvorrichtung (
10 ) zum Anordnen in einem verzweigten Gefäß (7 ,8 ), umfassend: einen ersten im Allgemeinen zylindrischen Stent (12 ) mit Seiten, die zwischen dem ersten (26 ) und zweiten (28 ) gegenüberliegenden Ende verlaufen und mindestens einer Öffnung (16 ), die in einer Seite definiert ist; und einen zweiten im Allgemeinen zylindrischen Stent (15 ), der getrennt vom ersten Stent (12 ) gebildet ist und so angepasst ist, dass er sich durch eine der seitlichen Öffnungen (16 ) des ersten Stents (12 ) erstreckt; wobei der erste (12 ) und der zweite (15 ) Stent jeweils aus einem Werkstoff bestehen, durch den die Stents (12 ,15 ) ausgedehnt werden können, damit sie sich der Form des Gefäßes (7 ,8 ) des Patienten anpassen und wobei der zweite Stent (15 ) einen Abschnitt (18 ) an einem Ende (30 ) aufweist, der sich trichterförmig erweitern kann und so angepasst ist, dass er radial nach außen trichterförmig erweitert werden kann, um mindestens einen Abschnitt der Öffnung (16 ) der Seite des ersten Stents (12 ) zu berühren, wenn der zweite Stent (15 ) durch die Öffnung (16 ) des ersten Stents (12 ) nach außen entfaltet wird. - Stentvorrichtung (
10 ) nach Anspruch 1, wobei die Stentvorrichtung (10 ) aus einem biologisch akzeptablen Werkstoff besteht. - Stentvorrichtung (
10 ) nach Anspruch 1 oder 2, wobei die Stents (12 ,15 ) einen selbstausdehnenden Werkstoff umfassen. - Stentvorrichtung (
10 ) nach Anspruch 1 oder 2, wobei die Stents (12 ,15 ) einen Werkstoff umfassen, der mit einem Ballon ausgedehnt werden kann. - Stentvorrichtung (
10 ) nach einem der vorhergehenden Ansprüche, wobei mindestens ein Abschnitt der Stents (12 ,15 ) während und nach der Einführung abgebildet werden kann. - Stentvorrichtung (
10 ) nach Anspruch 5, die ferner für die Bildgebung Marker (56 ) um den Rand der seitlichen Öffnung (16 ) herum und über dem trichterförmig erweiterten Abschnitt (18 ) des zweiten Stents (15 ) umfasst.
