Description
- Die Erfindung betrifft orale pharmazeutische Zusammensetzungen, die ein Rapamycin in einer festen Dispersion enthalten.
- Rapamycin ist ein immunsuppressives Lactammakrolid, das beispielsweise durch Streptomyces hygroscopicus herstellbar ist. Die Struktur von Rapamycin ist angegeben in H. Kessler et al, 1993, Helv. Chim. Acta, 76: 117. Rapamycin ist ein äußerst starkes Immunsuppressivum, welches auch über eine antitumorale und antifungale Wirksamkeit verfügt. Sein Einsatz als Pharmazeutikum ist aber beschränkt durch seine sehr niedrige und variable Bioverfügbarkeit. Darüber hinaus ist Rapamycin hoch unlöslich in wässrigen Medien, wie Wasser, so dass es sich nur schwer zu stabilen galenischen Zusammensetzungen formulieren lässt. Es sind auch zahlreiche Derivate von Rapamycin bekannt. Bestimmte Rapamycine, die in der Stellung 16-O substituiert sind, werden in WO 94 02 136 A1 beschrieben. In Stellung 40-O substituierte Rapamycine werden beispielsweise beschrieben in
US 5 258 389 A und WO 94 09 010 A (O-Aryl- und O-Alkylrapamycine), WO 92 05 179 A (Carbonsäureester),US 5 118 677 A (Amidester),US 5 118 678 A (Carbamate),US 5 100 883 A (fluorierte Ester),US 5 151 413 A (Acetate),US 5 120 842 A (Silylether), WO 93 11 130 A (Methylenrapamycin und Derivate), WO 94 02 136 A (Methoxyderivate), WO 94 02 385 A und WO 95 14 023 A (Alkenylderivate), wobei alle diese Dokumente durch die erfolgte Bezugnahme hier eingeführt werden. 32-O-Dihydro- oder substituiertes Rapamycin werden beispielsweise beschrieben inUS 5 256 790 A , wobei dieses Patent hiermit ebenfalls eingeführt wird. - Weitere Rapamycinderivate werden auch in WO 96 02 441 A beschrieben, beispielsweise das 32-Deozorapamycin gemäß dessen Beispiel 1 und das 16-Pent-2-inyloxy-32(S)-dihydrorapamycin gemäß dessen Beispielen 2 und 3.
- Rapamycin und seine strukturell ähnlichen Analogen und Derivate werden hierin kollektiv auch als Rapamycine bezeichnet.
- In
EP 0 240 773 A wird eine feste Dispersionszusammensetzung beschrieben, die eine FR-900 506 genannte Substanz und ein in Wasser lösliches Polymer umfasst. - Bei einer oralen Verabreichung an Menschen können feste Rapamycine, wie Rapamycin selbst, aber nicht in einem signifikanten Ausmaß in den Blutstrom absorbiert werden. Bekannt sind auch einfache Gemische von Rapamycinen, beispielsweise von Rapamycin selbst, mit herkömmlichen pharmazeutischen Hilfsstoffen, doch haben diese Zusammensetzungen den Nachteil unvorhersagbarer Auflösungsraten, irregulärer Bioverfügbarkeitsprofile und einer Instabilität. Es gibt daher bis heute keine bequem verabreichbare orale feste Formulierung von Rapamycin oder einem Derivat hiervon.
- Ein Aspekt der vorliegenden Erfindung ist daher die Schaffung einer pharmazeutischen Zusammensetzung in Form einer festen Dispersion, die ein Rapamycin und ein Trägermedium umfasst.
- Die erfindungsgemäßen Zusammensetzungen sorgen für eine Bioverfügbarkeit der Wirksubstanz, sind bequem verabreichbar und auch stabil.
- Das bei den erfindungsgemäßen Zusammensetzungen zu verwendende Rapamycin kann irgendein Rapamycin oder Derivat hiervon sein, wie es beispielsweise oben erwähnt oder auch in den oben genannten Patentdokumenten beschrieben worden ist.
- Beim Rapamycin, das in den erfindungsgemäßen festen Dispersionszusammensetzungen zur Anwendung gelangt, kann es sich somit handeln um Rapamycin oder ein O-substituiertes Derivat hiervon, worin die. Hydroxylgruppe am Cyclohexylring des Rapamycins ersetzt ist durch eine Gruppe -OR1, worin R1 für Hydroxyalkyl, Hydroxyalkoxyalkyl, Acylaminoalkyl oder Aminoalkyl steht, wobei in WO 94 09 010 A Beispiele hierfür beschrieben werden, wie 40-O-(2-Hydroxy)ethylrapamycin, 40-O-(3-Hydroxy)propylrapamycin, 40-O-[2-(2-Hydroxy)ethoxy]ethylrapamycin und 40-O-(2-Acetaminoethyl)rapamycin. Das Rapamycinderivat kann auch ein in Stellung 26 oder 28 substituiertes Derivat sein.
- Zu bevorzugten Rapamycinen, welche bei den erfindungsgemäßen festen Dispersionszusammensetzungen zur Anwendung gelangen können, gehören Rapamycin, 40-O-(2-Hydroxy)ethylrapamycin, 32-Deoxorapamycin und 16-Pent-2-inyloxy-32(S)-dihydrorapamycin. Ein stärker bevorzugtes Rapamycin ist 40-O-(2-Hydroxy)ethylrapamycin, wobei dieses Rapamycin im folgenden auch als Verbindung X bezeichnet wird.
- Die hierin verwendete Nummerierung der Rapamycinderivate bezieht sich auf die Struktur, wie sie der Formel A auf Seite 4 von WO 9613 273 A entspricht.
- Unter der hierin verwendeten Bezeichnung feste Dispersion versteht man ein Copräzipitat aus einem Rapamycin, beispielsweise 40-O-(2-Hydroxy)ethylrapamycin oder Rapamycin selbst, mit dem Trägermedium. In der festen Dispersion liegt das Rapamycin in einer amorphen oder im wesentlichen amorphen Form vor und ist physikalisch an das Trägermedium gebunden.
- Die erfindungsgemäßen Zusammensetzungen können in jeder bequemen Form verabreicht werden, beispielsweise in Form einer Tablette, einer Kapsel, eines Granulats oder eines Pulvers, beispielsweise als Sachet (Säckchen).
- Das Rapamycin kann in der Zusammensetzung in einer Menge von etwa 0,01 bis etwa 30 Gew.-%, bezogen auf das Gewicht der Zusammensetzung (% w/w) vorhanden sein und liegt darin vorzugsweise in einer Menge von 1 bis 20 w/w, bezogen auf das Gesamtgewicht der Zusammensetzung, vor.
- Das Trägermedium ist in einer Menge von bis zu 99,99 Gew.-% vorhanden, beispielsweise in einer Menge von 10 bis 95 Gew.-%, bezogen auf das Gesamtgewicht der Zusammensetzung.
- Bei einer Ausführungsform umfasst das Trägermedium ein in Wasser lösliches Polymer, vorzugsweise ein Cellulosederivat, wie Hydroxypropylmethylcellulose (HPMC), Hydroxypropylmethylcellulosephthalat oder Polyvinylpyrrolidon (PVP). Gute Ergebnisse lassen sich erzielen unter Verwendung von HPMC mit einer niedrigen scheinbaren dynamischen Viskosität, beispielsweise von unter 100 cP, ge messen bei 20°C anhand einer 2 Gew.-%igen wässrigen Lösung, beispielsweise von unter 50 cP, vorzugsweise von unter 20 cP, beispielsweise von HPMC mit 3 cP. Bei HPMC handelt es sich um ein wohl bekanntes und ausführlich beschriebenes Produkt, wozu beispielsweise hingewiesen wird auf Handbook of Pharmaceutical Excipients, Zweite Auflage, Herausgeber Pharmaceutical Society of Great Britain und American Pharmaceutical Association, 1994, Seiten 229 bis 232, wobei der Inhalt dieser Literaturstelle hiermit eingeführt wird. HPMC unter Einschluss von HPMC 3 cP ist im Handel unter der Bezeichnung Pharmacoat 603 von der Firma Shinetsu erhältlich.
- PVP ist beispielsweise unter der Bezeichnung Povidon erhältlich (Handbook of Pharmaceutical Excipients), wobei ein PVP mit einem mittleren Molekulargewicht von etwa 8000 bis etwa 50000 Dalton bevorzugt ist.
- Weitere Ausführungsformen des Trägermediums umfassen
- – Hydroxypropylcellulose (HPC) oder ein Derivat hiervon. Zu Beispielen für HPC Derivate gehören solche mit einer niedrigen dynamischen Viskosität in wässrigen Medien, wie Wasser, beispielsweise von unter etwa 400 cP, beispielsweise von unter 150 cP, gemessen bei 25°C anhand einer zweiprozentigen wässrigen Lösung. Bevorzugte HPC Derivate haben einen niedrigen Substitutionsgrad und ein mittleres Molekulargewicht von unter etwa 200000 Dalton, beispielsweise von 50000 bis 150000 Dalton. Zu Beispielen für im Handel erhältliche HPC's gehören Klucel LF, Klucel EF und Klucel JF von der Firma Aqualon und Nisso HPC-L von der Firma Nippon Soda Ltd.,
- – Polyethylenglycole (PEG), wobei Beispiele für solche PEGs ein mittleres Molekulargewicht von 1000 bis 9000 Dalton haben, beispielsweise von etwa 1800 bis 7000 Dalton, wie PEG 2000, PEG 4000 oder PEG 6000 (Handbook of Pharmaceutical Excipients),
- – gesättigte polyglycolierte Glyceride, wie sie unter der Bezeichnung Gelucir, beispielsweise Gelucir 44/14, 53/10, 50/13, 42/12 oder 35/10 von der Firma Gattefossé erhältlich sind, oder
- – Cyclodextrine, beispielsweise ein β-Cyclodextrin oder ein α-Cyclodextrin. Zu Beispielen für geeignete β-Cyclodextrine gehören Methyl-β-cyclodextrin, Dimethyl-β-cyclodextrin, Hydroxypropyl-β-cyclodextrin, Glycosyl-β-cyclodextrin, Maltosyl-β-cyclodextrin, Sulfo-β-cyclodextrin, Sulfoalkylether von β-Cyclodextrin, wie Sulfo-C1-C4-Alkylether. Zu Beispielen für α-Cyclodextrine gehören Glucosyl-α-cyclodextrin und Maltosyl-α-cyclodextrin.
- Das Trägermedium kann ferner eine in Wasser lösliche oder in Wasser unlösliche Saccharose enthalten oder einen sonstigen annehmbaren Träger oder einen Füllstoff, wie Lactose oder mikrokristalline Cellulose. Die Menge an Füllstoff beträgt, falls überhaupt vorhanden, allgemein bis zu etwa 30 Gew.-%, beispielsweise 0,5 bis 20 Gew.-%, vorzugsweise etwa 5 bis etwa 15 Gew.-%, der Zusammensetzung.
- Eine mikrokristalline Cellulose ist im Handel beispielsweise von der FMC Corporation unter der Bezeichnung Avicel erhältlich.
- Das Trägermedium kann ferner ein oder mehr Tenside enthalten, beispielsweise nicht ionische, ionische, anionische oder amphotere Tenside. Zu Beispielen für geeignete Tenside gehören
- – Polyoxyethylen-Polyoxypropylen-Copolymere und Block-Copolymere, wie sie beispielsweise bekannt sind unter den Handelsnamen Pluronic oder Poloxamer, und beispielsweise beschrieben werden in H. P. Fiedler, Lexikon der Hilfsstoffe für Pharmazie, Kosmetik und angrenzende Gebiete, Editio Cantor, D 88326 Aulendorf, dritte überarbeitete und erweiterte Auflage (1989), dessen Inhalt hiermit durch Bezugnahme eingeführt wird. Ein bevorzugtes Polyoxyethylen-Polyoxypropylen-Blockpolymer ist Poloxamer 188, welches von der Firma BASF erhältlich ist.
- – ethoxylierte Cholesterine, wie sie beispielsweise unter dem Warennamen Solulan bekannt sind, beispielsweise Solulan C24, welches von der Firma Amerchol erhältlich ist,
- – Vitaminderivate, beispielsweise Derivate von Vitamin E, wie Tocopherolpolyethylenglycolsuccinat (TPGS), das von der Firma Eastman erhältlich ist,
- – Natriumdodecylsulfat oder Natriumlaurylsulfat,
- – eine Gallensäure oder ein Salz hiervon, beispielsweise Cholsäure, Glycolsäure oder ein Salz hiervon, beispielsweise Natriumcholat, oder
- – Lecithin.
- Sind in den erfindungsgemäßen Zusammensetzungen Tenside vorhanden, dann beträgt deren Menge im allgemeinen bis zu 20 Gew.-%, beispielsweise 1 bis 15 Gew.-%.
- In den erfindungsgemäßen Zusammensetzungen können auch ein oder mehr Zerfallhilfsmittel enthalten sein. Zu Beispielen für Zerfallhilfsmittel gehören Polyplasdon (Handbook of Pharmaceutical Excipients), das im Handel von der Firma ISP erhältlich ist, Natriumstärkeglycolat, welches im Handel von der Firma Generichem erhältlich ist, und Crosscarmelosenatrium, das von der Firma FMC unter der Marke Ac-di-sol erhältlich ist. In der erfindungsgemäßen Zusammensetzung können auch ein oder mehr Gleitmittel enthalten sein, beispielsweise Magnesiumstearat oder kolloidales Siliciumdioxid, und zwar in einer Menge von bis zu etwa 5 Gew.-%, beispielsweise 0,5 bis 2 Gew.-%, bezogen auf das Gewicht der Zusammensetzung.
- In den erfindungsgemäßen Zusammensetzungen können vorteilhafterweise auch ein oder mehr Aromastoffe enthalten sein.
- Erfindungsgemäß sind auch gute Ergebnisse unter Verwendung von Rapamycinzusammensetzungen erhalten worden, die kein Tensid enthalten. Ein anderer Aspekt der Erfindung ist daher eine an Tensid freie feste Dispersionszusammensetzung, die ein Rapamycin der hierin beschriebenen Art enthält.
- In den efindungsgemäßen Zusammensetzungen können auch Antioxidantien und/oder Stabilisatoren enthalten sein, und zwar in Mengen von bis zu 1 Gew.-%, beispielsweise zwischen 0,05 und 0,5 Gew.-%. Zu Beispielen für Antioxidantien gehören butyliertes Hydroxytoluol, DL-α-Tocopherol, Propylgallat, Ascorbylpalmitat und Fumarsäure. Malonsäure ist ein geeigneter Stabilisator.
- Bei einer Ausführungsform dieser Erfindung enthält die Zusammensetzung bis zu 30 Gew.-%, beispielsweise 1 bis 20 Gew.-%, 40-O-(2-Hydroxy)ethylrapamycin und bis zu 95 Gew.-%, beispielsweise 30 bis 90 Gew.-%, HPMC.
- Das Gewichtsverhältnis aus dem Rapamycin und dem Trägermedium in den erfindungsgemäßen Zusammensetzungen macht im allgemeinen mehr als 1 : 3 aus und liegt vorzugsweise bei unter 1 : 4.
- Ein weiterer Aspekt dieser Erfindung ist ein Verfahren zur Herstellung einer festen Dispersionszusammensetzung, wie sie hierin beschrieben wird.
- Bei einer Ausführungsform lassen sich die erfindungsgemäßen Zusammensetzungen erhalten durch Auflösung oder Suspendierung des Rapamycins und des Trägermediums in einem Lösemittel oder einem Lösemittelgemisch. Das Lösemittel kann dabei ein einzelnes Lösemittel oder ein Lösemittelgemisch sein, wobei die Reihenfolge der Auflösung und Suspendierung des Rapamycins mit dem Trägermedium variiert werden kann. Lösemittel, die sich zur Verwendung bei der Herstellung erfindungsgemäßer fester Dispersionszusammensetzungen eigenen, sind organische Lösemittel, wie Alkohole, beispielsweise Methanol, Ethanol oder Isopropanol, Ester, beispielsweise Ethylacetat, Ether, beispielsweise Diethylether, Ketone, beispielsweise Aceton, oder Halogenkohlenwasserstoffe, beispielsweise Dichlorethan. Ein bequemes Lösemittelgemisch ist ein Gemisch aus Ethanol und Aceton, in welchem die Komponenten Ethanol und Aceton in einem Gewichtsverhältnis von etwa 1 : 10 bis etwa 10 : 1, beispielsweise von 1 : 5 bis 5 : 1, vorhanden sind.
- Typischerweise sind das Rapamycin und das Trägermedium mit dem Lösemittel in einem Gewichtsverhältnis von 1 : 0,1 bis 1 : 20 vorhanden. Das Lösemittel kann abgedampft und das Rapamycin mit dem Trägermedium coausgefällt werden. Der erhaltene Rückstand kann getrocknet, beispielsweise unter verringertem Druck, gesiebt und vermahlen werden. Die vermahlene Dispersion kann mit anderen Hilfsstoffen kombiniert und beispielsweise zu Tabletten verpresst oder in Säckchen oder Gelatinekapseln abgefüllt werden.
- Bei einer anderen Ausführungsform kann die Herstellung der festen Dispersionszusammensetzungen erfolgen durch Schmelzen des Trägermediums unter Bildung einer Schmelze und Kombination der Schmelze mit dem Rapamycin, beispielsweise durch Verrühren, und zwar optional in Anwesenheit eines Lösemittels oder Lösemittelgemisches der oben beschriebenen Art.
- Alternativ können die erfindungsgemäßen festen Dispersionen auch hergestellt werden durch Sprühtrocknungstechniken, wie sie beispielsweise beschrieben werden in Theory and Practice of Industrial Pharmacy, Lachmann et al, 1986. Eine in obiger Weise gebildete Suspension wird durch eine Düse in eine Kammer dispergiert, die beispielsweise auf 20 bis 80°C gehalten wird. Während des Durchlaufens der Düse wird das Lösemittel verdampft, und es werden fein dispergierte Teilchen gesammelt.
- Die erfindungsgemäßen Zusammensetzungen haben nach dem Vermahlen typischerweise eine mittlere Teilchengröße von weniger als etwa 0,5 mm, beispielsweise weniger als etwa 350 μm, beispielsweise etwa 100 bis 300 μm.
- Die oralen erfindungsgemäßen Zusammensetzungen sind für die bekannten Indikationen von Rapamycin brauchbar, beispielsweise für die folgenden Zustände:
- a) Behandlung und Verhinderung einer Organ- oder Gewebe-Allo- oder – Xenotransplantatabstoßung, beispielsweise zur Behandlung von Empfängern von beispielsweise Herz-, Lungen-, kombinierten Herz-Lunge-, Leber-, Niere-, Pancreas-, Haut-oder Cornealtransplantaten. Sie sind auch angezeigt zur Verhinderung einer Transplantat-gegen-Wirt-Erkrankung, wie einer Knochenmarktransplantation.
- b) Behandlung und Verhinderung einer Autoimmunerkrankung und inflammatorischer Zustände, insbesondere inflammatorischer Zustände mit einer Ätiologie unter Einschluss einer Autoimmunkomponente, wie Arthritis, beispielsweise rheumatoide Arthritis, Arthritis chronica progrediente und Arthritis deformans, und rheumatische Erkrankungen. Zu speziellen Autoimmunerkrankungen, für welche die erfindungsgemäßen Verbindungen verwendet werden können, gehören autoimmune hämatologische Störungen, unter Einschluss von beispielsweise hämolytischer Anaemie, aplastischer Anaemie, Anaemie der reinen roten Blutkörperchen und idiopathischen Thrombocytopenie, systemischer Lupus enthematosus, Polychondritis, Sclerodermie, Wegener-Granulomatose, Dermatomyositis, chronische aktive Hepatitis, Myasthenia gravis, Psoriasis, Steven-Johnson-Syndrom, idiopathische Sprue, autoimmune inflammatorische Darmerkrankung, unter Einschluss von beispielsweise ulcerativer Colitis und Crohn-Krankheit, endocrine Ophthalmopathie, Graves-Krankheit, Sarcoidose, multiple Sclerose, primäre billiäre Cirrhose, juveniler Diabetes, Diabetes mellitus Typ I, Uveitis, anterior und posterior, Keratoconjunctivitis sicca und vernale Keratoconjunctivitis, interstitielle Lungenfibrose, psoriatische Arthritis, Glomerulonephritis, mit und ohne nephrotischem Syndrom, unter Einschluss von beispielsweise idiopathischem nephrotischem Syndrom oder minimal veränderlicher Nephropathie, und juvenile Dermatomyositis.
- c) Behandlung und Verhinderung von Asthma.
- d) Behandlung von Multi-Drug-Resistenz (MDR). MDR ist besonders problematisch bei Krebspatienten und AIDS Patienten, die auf eine herkömmliche Chemotherapie nicht ansprechen, da die Medikation Pgp aus den Zellen gepumpt wird. Die Zusammensetzungen eignen sich daher auch zur Verbesserung der Wirksamkeit anderer chemotherapeutischer Mittel bei der Behandlung und Kontrolle Multi-Drug resistenter Zustände, wie Multi-Drug resistentem Krebs oder Multi-Drug resistentem AIDS.
- e) Behandlung proliferativer Störungen, beispielsweise von Tumoren, hyperproliferativer Hautstörung und dergleichen.
- f) Behandlung fungaler Infektionen.
- g) Behandlung und Verhinderung von Entzündungen, insbesondere zur Potenzierung der Wirkung von Steroiden.
- h) Behandlung und Verhinderung von Infektionen, insbesondere Infektionen durch Pathogene mit Mip- oder Mip-ähnlichen Faktoren.
- i) Behandlung von Überdosen von FK-506 und anderen Makrophilin bindenden Immunsuppressiva.
- Liegt die erfindungsgemäße pharmazeutische Zusammensetzung in Einheitsdosierungsform vor, beispielsweise als Tablette, Kapsel, Granulat oder Pulver, dann enthält jede Einheitsdosierung zweckmäßigerweise zwischen 1 mg und 100 mg Wirkstoff, vorzugsweise zwischen 10 und 50 mg, beispielsweise 15, 20, 25 oder 50 mg. Solche Einheitsdosierungsformen eignen sich für eine ein- bis fünfmal tägliche Verabreichung in Abhängigkeit vom jeweiligen Zweck der Therapie, der Therapiephase und dergleichen.
- Die genaue Menge der zu verabreichenden Zusammensetzungen ist von mehreren Faktoren abhängig, beispielsweise der gewünschten Dauer der Behandlung und der Freisetzungsrate des Rapamycins.
- Die Brauchbarkeit der pharmazeutischen Zusammensetzungen lässt sich durch übliche klinische Versuche ermitteln, beispielsweise in bekannten Indikationen an Wirkstoffdosierungen, welche äquivalente Blutkonzentrationen des Wirkstoffs ergeben, beispielsweise unter Anwendung von Wirkstoffdosen im Bereich von 1 mg bis 1000 mg, beispielsweise 5 mg bis 100 mg des jeweiligen Wirkstoffs pro Tag bei einem 75 kg wiegenden Erwachsenen und in standardisierten Tiermodellen. Die erhöhte Bioverfügbarkeit der Wirksubstanz, welche sich durch die vorliegenden Zusammensetzungen ergibt, kann in üblichen Tierversuchen und klinischen Versuchen beobachtet werden.
- Die verwendete Dosierungsform, beispielsweise eine Tablette, kann beschichtet sein, beispielsweise mit einem enterischen Überzug versehen sein. Geeignete Beschichtungen umfassen Celluloseace tatphthalat, Hydroxypropylmethylcellulosephthalat, ein Polymethacrylsäurecopolymer, beispielsweise Eudragit L, S, oder Hydroxypropylmethylcellulosesuccinat.
- Das in den efindungsgemäßen Zusammensetzungen verwendete Rapamycin, beispielsweise 40-O-(2-Hydroxy)ethylrapamycin oder Rapamycin selbst, kann vor der Bildung der festen Dispersion in kristalliner oder amorpher Form vorliegen. Ein Vorteil der vorliegenden Erfindung ist daher, dass das Rapamycin nicht kristallin sein muss. Somit kann das Rapamycin direkt in Kombination mit beispielsweise einem Lösemittel verwendet und muss daher vorher nicht isoliert werden. Ein weiterer Vorteil der Erfindung besteht darin, dass die Auflösungsraten der festen Dispersion höher sind als die Auflösungsraten für ein kristallines Rapamycin oder ein amorphes Rapamycin in einem einfachen Gemisch.
- Die Erfindung wird im folgenden anhand von Beispielen für erfindungsgemäße feste Dispersionszusammensetzungen näher erläutert.
- Beispiel 1
- Es wird eine feste Dispersionszusammensetzung hergestellt, welche die folgenden Bestandteile in Gewichtsteilen enthält:
Verbindung X 9,1 HPMC 3 cP 81,8 Lactose 20 mesh 9,1 - Die Zusammensetzung (Form A) wird hergestellt durch Auflösung des Rapamycins und des Trägermediums in einem Ethanol/Aceton-Gemisch. Es wird absolutes Ethanol in einem Gewichtsverhältnis von 1 : 1 mit dem Aceton verwendet. Sodann verdampft man die Lösemittel und vermahlt den erhaltenen trockenen Rückstand zu einem feinen Pulver mit einer mittleren Teilchengröße von < 0,5 mm.
- Beispiel 2
- Es wird eine feste Dispersionszusammensetzung hergestellt, welche die folgenden Bestandteile in Gewichtsteilen enthält:
Verbindung X 16,7 HPMC 3 cP 66,7 Poloxamer 188 (von BASF) 16,7 - Die Zusammensetzung (Form B) wird in analoger Weise wie beim Beispiel 1 hergestellt.
- Beispiel 3
- Es wird eine feste Dispersionszusammensetzung hergestellt, welche die folgenden Bestandteile in Gewichtsteilen enthält:
Verbindung X 16,7 HPMC 3 cP 66,7 TPGS* 16,7 - Die Zusammensetzung (Form C) wird in analoger Weise wie beim Beispiel 1 hergestellt.
- Beispiel 4
- Es wird eine feste Dispersionszusammensetzung hergestellt, welche die folgenden Bestandteile in Gewichtsteilen enthält:
Verbindung X 10 HPMC 3 cP 80 Solulan C24 (von Amerchol) 10 - Die Zusammensetzung (Form D) wird in analoger Weise wie beim Beispiel 1 hergestellt.
- Die obigen Zusammensetzungen gemäß der Formen A bis D können zu Tabletten geformt, in Kapseln abgefüllt oder pulverisiert und in Säckchen abgepackt werden.
- Pharmakokinetiken nach Verabreichung von 40-O-(2-Hydroxy)ethylrapamycin an Ratten
- a) Verabreichung des Arzneimittels
- Es werden wässrige Dispersionen von jeweils 0,5 ml der die Verbindung X enthaltenden Zusammensetzung, was 4,0 mg Wirkstoff pro Ratte entspricht, durch Gastrointubation während einer kurzen Inhalationsanästhesie mit einer 1 ml Spritze verabreicht, die an einem Polyethylenschlauch befestigt ist. Für jede Zusammensetzung der Formen A, B, C und D werden sechs Tiere verwendet.
- b) Blutproben
- Einen Tag vor diesem Versuch wird den Tieren eine Permanentkanüle in die Vena jugularis gesetzt. Jeder Ratte werden 0,5 ml venöses Blut (Vena jugularis) entnommen, das in zwei 2,5 ml EDTA Schläuchen aufbewahrt wird. Die Blutproben von zwei Tieren (1 und 2, 3 und 4, 5 und 6) werden zusammengefasst und bis zur Arzneimittelanalyse bei –80°C aufbewahrt. Proben werden vor der Verabreichung und 10, 30, 60, 120, 300, 480 und 1440 min nach der Wirkstoffverabreichung gezogen.
- c) Bioanalytik
- Die Blutproben werden durch HPLC mit Umkehrphase analysiert.
- Die folgende Tabelle zeigt die pharmacokinetischen Daten, die nach Verabreichung der Verbindung X an Ratten erhalten werden.
- Die Form A ergibt Blutspiegel, die höher sind als diejenigen nach Verabreichung von Zusammensetzungen, welche ein Tensid enthalten.
- Studie an Hunden
- Nach den obigen versprechenden Ergebnissen wird eine relative Bioverfügbarkeitsstudie an ausgehungerten Beagle-Hunden unter Anwendung einer Dosis von 1 mg/kg Körpergewicht durchgeführt. Hartgelatinekapseln, die jeweils 10 mg der Verbindung X enthalten, werden jeweils acht Hunden nach einem Vierwegplan in Form eines Lateinischen Quadrats verabreicht. Die Hunde erhalten 6 Stunden nach der Verabreichung der Kapseln Futter, wobei die Blutspiegel der Verbindung X während einer Zeitdauer von 48 Stunden bestimmt werden. Bei allen Hunden sind ähnliche Blutkonzentrationsprofile der Verbindung X zu beobachten, wobei die terminate Halbwertszeit der Verbindung X im Blut zwischen 10 und 40 h liegt. Es werden mittlere Peak-Spiegel von 140 ng/ml und mittlere AUC Spiegel von 0 bis 48 h mit etwa 1600 ng h/ml beobachtet.
Claims (27)
- Pharmazeutische Zusammensetzung zur oralen Verabreichung in Form einer festen Dispersion, wobei diese feste Dispersion ein Copräzipitat von Rapamycin oder einem Derivat hiervon mit einem Trägermedium umfasst.
- Zusammensetzung nach Anspruch 1, worin das Trägermedium ein wasserlösliches Polymer oder ein Cyclodextrin umfasst.
- Pharmazeutische Zusammensetzung nach Anspruch 1 oder 2, worin das Rapamycinderivat ausgewählt ist aus 40-O-(2-Hydroxy)ethylrapamycin, 32-Desoxorapamycin oder 16-Pent-2-inyloxy-32(S)-dihydrorapamycin.
- Zusammensetzung nach Anspruch 2 oder 3, worin das Polymer Hydroxypropylmethylcellulose oder Polyvinylpyrrolidon ist.
- Zusammensetzung nach einem der vorhergehenden Ansprüche, umfassend bis zu 30 Gew.-% Rapamycin oder eines Derivats hiervon.
- Zusammensetzung nach einem der Ansprüche 2, 3 oder 4, worin das wasserlösliche Polymer Hydroxypropylmethylcellulose in einer Menge von bis zu etwa 95 Gew.-% ist.
- Zusammensetzung nach einem der Ansprüche 2 bis 6, worin das Gewichtsverhältnis von Rapamycin oder eines Derivats hiervon zum Trägermedium weniger als 1 : 4 beträgt.
- Zusammensetzung nach einem der vorhergehenden Ansprüche, die frei an Tensid ist.
- Zusammensetzung nach einem der vorhergehenden Ansprüche, worin das Rapamycin oder Derivat hiervon in amorpher oder im wesentlichen amorpher Form vorliegt.
- Zusammensetzung nach einem der vorhergehenden Ansprüche in Einheitsdosierungsform.
- Zusammensetzung nach einem der vorhergehenden Ansprüche in Form einer Kapsel oder Tablette.
- Zusammensetzung nach einem der Ansprüche 10 oder 11, die ferner einen enterischen Überzug umfasst.
- Zusammensetzung nach einem der vorhergehenden Ansprüche, die ferner ein Antioxidant umfasst.
- Zusammensetzung nach Anspruch 13, worin das Antioxidant ausgewählt ist aus der Gruppe, die besteht aus butyliertem Hydroxytoluol, DL-α-Tocopherolpropylgallat, Ascorbylpalmitat und Fumarsäure.
- Zusammensetzung nach einem der Ansprüche 4, 5 und 7 bis 14, worin das Polymer Polyvinylpyrrolidon ist.
- Zusammensetzung nach einem der vorhergehenden Ansprüche, die ferner eine wasserlösliche Saccharose umfasst.
- Zusammensetzung nach einem der vorhergehenden Ansprüche, die ferner eine mikrokristalline Cellulose umfasst.
- Zusammensetzung nach einem der Ansprüche 1 bis 7 oder 9 bis 17, die ferner ein Polyoxyethylen-Polyoxypropylen-Copolymer oder ein Blockcopolymer hiervon umfasst.
- Verfahren zur Herstellung einer pharmazeutischen Zusammensetzung, die Rapamycin oder ein Derivat hiervon und ein Trägermedium enthält, durch (a) Suspendierung von Rapamycin oder eines Derivats hiervon in einem Lösemittel unter Bildung einer Suspension, (b) Kombination einer oder mehrerer Trägerkomponenten mit dem Lösemittel und (c) Sprühtrocknung der Suspension unter Bildung der pharmazeutischen Zusammensetzung.
- Verfahren nach Anspruch 19, worin die pharmazeutische Zusammensetzung anschließend zu einer Einheitsdosierungsform geformt wird.
- Verfahren nach Anspruch 19 oder 20, worin die pharmazeutische Zusammensetzung anschließend zu einer Tablette geformt oder in eine Kapsel abgefüllt wird.
- Verfahren nach Anspruch 19, worin das Rapamycin oder ein Derivat hiervon nach einer Sprühtrocknung zu einer festen Dispersion geformt wird.
- Verfahren nach Anspruch 19, worin das Lösemittel ein organisches Lösemittel oder ein Gemisch organischer Lösemittel ist.
- Verfahren nach einem der Ansprüche 19 bis 23, worin die Trägerkomponente ein Polyoxyethylen-Polyoxypropylen-Copolymer oder ein Blockcopolymer hiervon umfasst.
- Verfahren nach einem der Ansprüche 19 bis 24, worin die Trägerkomponente ein Polyvinylpyrrolidon umfasst.
- Verfahren nach einem der Ansprüche 19 bis 25, worin die Trägerkomponente eine mikrokristalline Cellulose umfasst.
- Verfahren nach einem der Ansprüche 19 bis 26, worin die Trägerkomponente eine wasserlösliche Saccharose umfasst.
