Description
Foreliggende oppfinnelse angår oralt administrerbare terapeutiske farmasøytiske kombinasjoner omfattende en effektiv mengde av CPT-11 i kombinasjon med en effektiv mengde av capecitabin. Kombinasjonen av nevnte to forbindelser er nyttig ved behandling av cancer, spesielt coloncancer.

Disse camptotecinderivater er anticancermidler som inhiberer topoisomerase I, blant hvilke irinotecan, der X-CO-O- er [4-(l-pipeirdin-l-piperidin)]karbonyloksy, er et aktivt prinsipp som er særdeles effektivt ved behandling av solide tumorer, og i særdeleshet colorectal cancer.
Den europeiske patentsøknad EP 74 256 beskriver også andre camptotecinderivater som også nevnes som anticancer medikamenter, i særdeleshet derivater av en strukturanalog til strukturen gitt ovenfor, og der X-CO-O- er erstattet med et radikal -X'R' der X' er O eller S og R' er et hydrogenatom eller et alkyl- eller acylradikal.
Andre camptotecinderivater er også beskrevet, for eksempel i patentene eller patentsøknadene EP 56 692, EP 88 642, EP 296 612, EP321 122, EP 325 247,

Camptotecinderivater administreres vanligvis ved injeksjon, i særdeleshet intravenøst i form av en steril løsning eller en emulsjon. Camptotecinderivatene kan imidlertid også administreres oralt i form av faste eller flytende sammensetninger.
CPT-11 er en av de mest aktive nye midlene ved colorectal cancer. Colorectal cancer er en ledende årsak til morbiditet og mortalitet med ca. 300 000 nye tilfeller og 200 000 dødsfall i Europa og USA hvert år [se P. Boyle, Some Recent Developments in the Epidemiology of Colorectal Cancer, s. 19-34 i Management of Colorectal Cancer. Bleiberg H., Rougier P., Wilke H. J., red., (Martin Dunitz, London 1998); og - Midgley R. S., Kerr D. J., Systemic Adjuvant Chemotherapy for Colorectal Cancer, s. 126-27 i Management of Colorectal Cancer. Bleiberg H., Rougier P., Wilke H. J., red., (Martin Dunitz, London 1998)]. Selv om omtrent 50% av pasientene kureres ved kirurgi alene, vil den andre halvparten evt. dø på grunn av metastatisk sykdom, som inkluderer omtrent 25% av pasientene som har tegn på metastaser ved diagnosetidspunktet.
Hos pasienter med colorectal cancer som er resistente mot 5-FU, resulterte enkeltmedikamentet CPT-11 utprøvet i to store fase Ill-randomiserte studier i en lengre overlevelse og en bedre livskvalitet, sammenliknet med kun lindrende behandling (D. Cunningham, S. PyrhSnen, RD. James et al, The Lancet, 352 (9138):1413-1418 (1998)) og også i en lengre overlevelse uten forringelse av livskvalitet, sammenliknet med de beste 5-FU/FA-infusjonsregimer (P. Rougier, E. van Cutsem et al, The Lancet, 352
(9138):1407-1418 (1998)). CPT-11 er derfor referansebehandlingen ved metastatisk colorectal cancer (MCRC) etter mislykket tidligere FU-5-behandling.
CPT-11 er også vist å være minst like aktiv som den såkalte standard 5-FU/FA-bolus hos kjemoterapinaive pasienter med MCRC [Proe. Am. Soc. Clin. Oncol., bind 13
(1994), (Abstr. nr. 573); J. Clin Oncol, 14(3):709-715 (1996); J. Clin Oncol, 15(1):251-260 (1997).
Kombinasjoner av irinotecan (CPT-11) og 5-FU er allerede studert i fase-I-studier i Japan, der foreløpige resultater indikerer at samtidig administrering er hensiktsmessig med hensyn til sikkerhet (L. Saltz et al., Eur. J. Cancer 32A, tillegg 3: s. 24-31 (1996)). En studie angående CPT-11 publisert av D. Cunningham, Eur. J. Cancer 32A, tillegg 3: s. 1-8 (1996) konkluderte med at CPT-11 gir en forskjellig cytotoksisk timærming som kan komplettere anvendelsen av 5-FU/folinsyre ved colorectal cancer. For å vise effektiviteten ved en kombinasjon, kan det være nødvendig å sammenlikne den maksimalt tolererte dose av kombinasjonen med den maksimalt tolererte dose av hver av de enkelte bestanddeler i den aktuelle studie. Effektiviteten kan kvantifiseres, for eksempel med logio til drepte celler, som bestemmes ved den følgende formel:
logio av drepte celler = T-C(dager)/3,32 x Td
der T-C representerer den tiden det tar for celler å vokse, som er gjennomsnittstid i dager for tumorene i den behandlede gruppe (T) og tumorene i den behandlede gruppe (C) å nå en forutbestemt verdi (for eksempel 1 g), og Td representerer tiden i dager nødvendig for volumet av tumoren hos kontrolldyrene (T. H. Corbett et al., Cancer, 40,
2660-2680 (1977); F. M. Schabel et al., Cancer Drug Development, Part B, Methods in Cancer Research, 17,3-51, New York, Academic Press Inc. (1979)). Et produkt vurderes å være aktivt dersom logio av døde celler er større eller lik 0,7. Et produkt er vurdert å være svært aktivt dersom logio til døde celler er større enn 2,8.
Effektiviteten til en kombinasjon kan også vises ved bestemmelse av den terapeutiske synergi. En kombinasjon manifesterer terapeutisk synergi dersom den er terapeutisk bedre enn en eller flere av bestanddelene anvendt ved dens optimale dose (T. H. Corbett et al., Cancer Treatment Reports, 66,1187 (1982)).
Det er nå funnet at kombinasjonen av CPT-11, som er et camptotecinderivat, og capecitabin, som er et pyrimidinderivat, er spesielt effektiv ved behandling av solide tumorer, slik som ovarial cancere, NSCLC og colorectal cancer. Blant de effektive pyrimidinderivater er gemcitabin, MTA og capecitabin.
Gemcitabin har antitumoraktivitet. Saltet av gemcitabin, 2'-deksy-2',2'-difluorcytidinmonohydroklorid er frembrakt for klinisk anvendelse som en intravenøs løsning for behandling av solide tumorer, slik som ikke-småcellet lungecancer
(NSCLC).
Gemcitabin har cellefasespesifisitet, og dreper først og fremst celler som gjennomgår DNA-syntese (S-fase) og blokkerer også progresjon av celler gjennom Gl/S-faseovergangen. Gemcitabin metaboliseres intracellulært av nukleosid kinaser til det aktive difosfat (dFdCDP)- og trifosfat (dFdCTP)-nukleosider. Den cytotoksiske effekt til gemcitabin er forbundet med en kombinasjon av to virkninger av difosfat- og trifosfatnukleosidene, som fører til inhibisjon av DNA-syntese. For det første inhiberer gemcitabindifosfat ribonukleotidreduktase, som er ansvarlig for å katalysere reaksjonene som genererer deoksynukleosidtrifosfater til DNA-syntese. Inhibisjon av dette enzym av difosfatnukleosid forårsaker en reduksjon i konsentrasjoner av deoksynukleotider, inkludert dCTP. For det andre konkurrerer gemcitabintrifosfat med dCTP for inkorporering i DNA. Reduksjonen i den intracellulære konsentrasjon av dCTP (ved effekt av difosfatet) øker inkorporeringen av gemcitabintrifosfat i DNA (selv-potensierende). Etter at gemcitabinnukleotidet er inkorporert i DNA, inkorporeres kun ytterligere ett nukleotid til DNA. Etter denne tilførsel er ytterligere DNA-syntese inhibert.
Gemcitabin har vist seg å være lovende i kombinasjon med CPT-11 som en behandling ved pancreascancer i fase II-studier.
MTA (multi-targeted antifolat) er en antimetabolitt som er en folatantagonist, dihydrofolatreduktaseinhibitor og tymidilat syntaseinhibitor. Den frembringes for anvendelse som en intravenøs løsning og er funnet å inhibere tumorvekst hos mus. Den testes for tiden på mennesker ved behandling av ikke-småcellet lungecancer, mesoteliom, melanom, blærecancer, brystcancer, pancreascancer, colorectal cancer og andre solide tumorer.
Capecitabin er et fluorpyrimidinkarbamat med antineoplåstisk aktivitet. Det er et oralt administrert promedikament av 5'-deoksy-5-fluoruridin (5'-DFUR), som konverteres til 5-fluoruracil i kroppen. Ved prekliniske studier har capecitabin vist aktivitet ved colorectal-, bryst-, hode- og nakkekarsinom, inkludert de som er resistente mot 5-FU.
Capecitabins kjemiske navn er 5'-deoksy-5-fluor-N-[(pentyloksy)karbonyl]-cytidin,
med molekylvekt på 359,35. Capecitabin har den følgende strukturelle formel: Capecitabin har en unik aktiveringsmekanisme som utnytter de høye konsentrasjoner av enzymet tymidinfosforylase i tumorvev, sammenliknet med friskt vev, hvilket fører til tumorselektiv dannelse av 5-FU.
To randomiserte fase Ill-studier har vist at oral capecitabin er en effektiv førstelinje-terapi for metastatisk colorectal cancer, der det oppnås en bedre responsrate og en minst ekvivalent overlevelse og tidsperiode frem til sykdomsprogresjon, sammenliknet med intravenøs (i.v.) 5-FU/leucovorin (Mayo Klinikk-regimet). Capecitabin viste også en mer fordelaktig sikkerhetsprofil sammenliknet med Mayo Klinikk-regimet.
Foreliggende oppfinnelse illustreres, men begrenses ikke, ved eksemplene nedenfor.
EKSEMPEL 1
Administrering av oral CPT-11 og oral capecitabin, alene og sammen, ble evaluert mot den humane colonkarsinomstammen HCT-116 implantert i Swiss-nakne mus. Når de to forbindelsene ble administrert sammen, ble de gitt med en times mellomrom, og CPT-11 ble gitt først. Administreringsvolumet av hver forbindelse var 0,2 ml p.o. Tabellen nedenfor viser at de maksimalt tolererte doser av begge enkeltmidlene og de kombinerte forbindelsene var svært aktive.
Disse resultater antyder at andre tidsskjemaer kunne optimalisere kombinasjonen og eksempel 2 nedenfor viser virkelig at delvis samtidig administrering av capecitabin og CPT-11 resulterer i en terapeutisk kombinasjon som er svært aktiv mot den undersøkte tumoren; dvs. capecitabin/CPT-11-kombinasjonen ga en logio av døde celler-verdi større enn 2,8.
EKSEMPEL 2
Samtidig administrering av oral CPT-11 og oral capecitabin ble evaluert i Swiss-nakne mus som hadde et tidlig stadium av HCT-116-colonkarsinom. Tre dosenivå av CPT-11 ble gitt alene på dagene 18-22 for å etablere den høyeste ikke-toksiske dosen. Tre dosenivå av capecitabin ble gitt alene på dagene 18-22 og 25-29. Ved kombinasjonen ble to delvis samtidige tidsskjemaer undersøkt. Det første tidsskjemaet involverte administrering av fire dosenivå av CPT-11 på dagene 18-22 med administrering av fire dosenivå av capecitabin på dagene 18-22 og 25-29.1 det andre tidsskjemaet ble capecitabin administrert først på dagene 18-22, etterfulgt av samtidig administrering med fire dosenivå av CPT-11 på dagene 25-29.
De to kombinerte rekkefølger ble funnet å være så aktive som det beste enkeltmiddel og 42% og 55% av den høyeste ikke-toksiske dose for hvert enkeltmiddel kunne administreres uten overlapp i vertens toksisitet. Rekkefølgen på administreringen synes å indusere en forskjellig toleranse av de kombinerte medikamenter. Når capecitabin ble administrert først synes resultatet mindre toksisk og bedre tolerert enn når CPT-11 ble administrert først. Ved begge administrasjonsmåler forble imidlertid effektiviteten til kombinasjonen den samme.
Resultatene oppnådd i studien av enkeltmidlene CPT-11 og capecitabin og kombinasjonen av disse vises nedenfor i den følgende tabell. Tre kombinasjoner var svært aktive med en log-celledød større enn 2,8.
EKSEMPEL 3
Sikkerheten og effektiviteten til to regimer av irinotecan (CPT-11) administrert i kombinasjon med standard dosen av intermitterende capecitabin ble studert hos pasienter med fremskredet/metastatisk colorectal cancer.
Hovedhensikten med studien var å sammenlikne sikkerhetsprofilene til behandlingsregimene. Den andre hensikten var å sammenlikne tumor responsrater ved de to behandlingsmåter.
Pasienter mottok irinotecan i.v. 300/mg/m ved ett av to doseregimer i kombinasjon med intermitterende, oral capecitabin i en 21-dagers behandlingssyklus (figurene 1 og 2).
Totalt 19 pasienter med ubehandlet eller forbehandlet fremskreden colorectal cancer ble innskrevet i denne pilotstudie. Behandlingen ble administrert frem til sykdomsprogresjon oppsto eller maksimalt 10 behandlingssykluser hos pasienter med tumorrespons eller stabil sykdom.
Pasienter med målbar fremskreden eller metastatisk colorectalt adenokarsinom var kvalifisert for innskrivning. Disse inkluderer pasienter i alderen 18-75 år, med en forventet levetid på minst 3 måneder og ECOG-status 0-2. Pasienter i alderen 70-75 år måtte nødvendigvis ha ECOG-status på 0-1.
Utgangsegenskapene til de 19 pasientene er vist i tabell 3.
Sikkerheten ble evaluert hos alle pasienter som fikk minst en dose av studiemedisinering, med bivirkninger gradert ifølge "National Cancer Institute Common Toxicity Criteria (NCICTC)". "Hånd-fot-syndromet" ble gradert 1-3. Tumorer ble vurdert av forskeren ved utgangspunktet og ved 9 ukers intervaller, basert på kriteriene til World Health Organization. Forekomst av behandlingsrelaterte, skadelige hendelser hos de 19 pasientene behandlet frem til nå, vises i tabell 4. Det var ingen 4. grads behandlingsrelaterte skadelige hendelser. Kun hos en pasient var det nødvendig med modifisering av dose for å regulere toksisiteter.
Atten pasienter kunne evalueres for respons (Tabell 5).
Disse foreløpige data viser at de to 21-dagers regimene som kombinerer intermitterende, oral capecitabin med i.v. irinotecan 300 mg/m administrert i en enkelt dose (dag 1) og oppdelt i to like doser (dag 1 og 8) har fordelaktige sikkerhetsprofiler og viser lovende antitumoraktivitet hos pasienter med fremskreden metastatisk colorectal cancer. Av de 10 pasientene i A, hadde 8 partielle responser og 2 ble stabilisert. Ingen viste progressiv sykdom.
Følgelig resulterer kombinasjonen av CPT-11 og pyrimidinderivatet capecitabin i en svært aktiv kombinasjon for behandling av cancer, slik som colorectal cancer.

1.
Oralt administrerbar terapeutisk farmasøytisk kombinasjon, karakterisert ved at den omfatter en effektiv mengde av CPT-11 i kombinasjon med en effektiv mengde av capecitabin.
2.
Anvendelse av CPT-11 og capecitabin for fremstilling av et farmasøytisk preparat for behandling av coloncancer.
3.
Anvendelse ifølge krav 2, der nevnte farmasøytiske preparat administreres i samme sammensetning eller i hver sin sammensetning, samtidig eller etter hverandre i en hvilken som helst rekkefølge.
