본 발명은 캄프토테신 또는 캄프토테신 유도체 유효량과 함께 피리미딘 유도체 유효량을 포함하고, 암의 치료에 유용한 치료적 약학 조성물에 관한 것이다.
본 발명은 암, 특히 충실성 종양의 캄프토테신 유도체와 기타 항암 약제의 배합물로의 치료방법 및 개선된 치료를 위한 이러한 배합물의 사용에 관한 것이다.
좀더 구체적으로 말하면, 본 발명은 이리노테칸(CPT-11; CamptosarR), 토포테칸, 9-아미노캄프토테신 및 9-니트로캄프토테신과 같은 캄프토테신 유도체와 피리미딘 유도체의 배합물로의 항암 치료에 관한 것이다. 피리미딘 유도체로는 우라실, 티민, 시토신, 메틸시토신 및 티아민 함유 화합물이 포함된다. 이러한 피리미딘 유도체의 예로는 카페시타빈, 젬시타빈 및 페미트렉스트(pemetrexed)로도 알려져 있는 복수-표적화 항폴린산제(MTA)가 있다.

CPT-11은 결장직장암에 가장 활성인 신규 제제중 하나이다. 결장직장암은 매년 유럽과 미국에서 신규 약 300,000건의 질병률과 200,000건의 사망률의 주요 원인이다(참조: P. Boyle, Some Recent Developments in the Epidemiology of Colorectal Cancer, pages 19-34 inManagement of Colorectal Cancer, Bleiberg H., Rougier P., Wilke H.J., eds, (Martin Dunitz, London 1998); 및 MidgleyR.S., Kerr D.J., Systemic Adjuvant Chemotherapy for Colorectal Cancer, pages 126-27 inManagement of Colorectal Cancer, Bleiberg H., Rougier P., Wilke H.J., eds, (Martin Dunitz, London 1998)). 환자의 약 50%는 수술만으로도 치료되지만, 진단시 전이의 증거가 보이는 환자 대략 25%를 포함하는 나머지 절반은 전이성 질환으로 인해 결국 사망할 것이다.
5-FU에 내성인 결장직장암 환자에서, 2 가지의 대형 III기 무작위 실험에서 시험된 단독 제제 CPT-11은 단독 지지 치료(supportive care)와 비교하여 더 장기의 생존과 더 높은 생활의 질을 초래하고(D. Cunningham, S. Pyrhonen, RD. James 등, The Lancet,352(9138): 1413-1418 (1998)) 또한 5-FU/FA 최상의 주입 요법과 비교하여 생활의 질을 저하시키지 않으면서 더욱 장기의 생존을 초래한다(P. Rougier, E. van Cutsem 등; The Lancet,352(9138): 1407-1418 (1998)). 따라서 CPT-11은 선행 5-FU 치료가 실패한 후에 전이성 결장직장암(MCRC)의 참조 치료이다.
CPT-11은 또한 적어도 MCRC에 걸린 화학요법 나이브(naive) 환자에서 이른바 표준 5-FU/FA 거환만큼 활성인 것으로 보여졌다[Proc. Am. Soc. Clin. Oncol., vol 13 (1994), (Abstr. #573); J. Clin Oncol,14(3): 709-715 (1996); J. Clin Oncol,15(1): 251-260(1997)].
이리노테칸(CPT-11)과 5-FU의 배합물은 이미 일본에서 I기 연구에서 연구되었고, 이것은 예비 결과에서 동시 투여가 안전성 측면에서 실현 가능하다는 것을 나타낸다(L. Saltz 등, Eur. J. Cancer 32A, suppl 3: S24-31(1996)).
D. Cunningham, Eur. J. Cancer, 32A suppl. 3:S1-8 (1996)에 의해 공개된 CPT-11에 관한 연구가 CPT-11이 결장직장암에서 5-FU/폴린산의 사용을 보완할 수 있는 상이한 세포독성의 접근법을 제공한다고 결론 내렸다.
배합물의 효능을 설명하기 위해, 문제의 연구에서 배합물의 최대 용인 투여량을 각 개별 구성물의 최대 용인 투여량과 비교할 필요가 있을 수 있다. 이 효능은 예를 들면, 하기식으로 결정되는 log10세포사에 의해 정량화될 수 있다:

log10세포사가 0.7 이상이면 산물이 활성인 것으로 간주된다. log10세포사가 2.8보다 크면 산물이 매우 활성인 것으로 간주된다.
배합물의 효능은 또한 치료 상승작용의 측정에 의해 설명될 수 있다. 배합물이 최적의 투여량으로 사용되는 구성물중 하나 또는 그 나머지보다 치료적으로 우수하면 그것은 치료 상승작용을 증명한다(T. H. Corbett 등, Cancer TreatmentReports, 66, 1187 (1982)).
본 발명에 이르러 캄프토테신 유도체의 피리미딘 유도체와의 배합물이 난소, NSCLC 및 결장직장암과 같은 충실성 종양의 치료에 특히 효과적이라는 것이 밝혀졌다. 그 중에서도 효과적인 피리미딘 유도체는 젬시타빈, MTA 및 카페시타빈이다.
젬시타빈이 항종양 활성을 보인다. 젬시타빈의 염인 2'-데옥시-2',2'-디플루오로시티딘 모노하이드로클로라이드는 작지 않은 세포 폐암(NSCLC)과 같은 충실성 종양의 치료를 위한 정맥주사액으로서의 임상적 사용을 위해 제공된다.

MTA(복수-표적화 항폴린산제)는 폴레이트 길항제인 대사길항물질, 디하이드로폴레이트 환원효소 억제제 및 티미딜레이트 합성효소 억제제이다. 이것은 정맥주사액으로 사용하기 위해 제공되고 마우스에서 종양 성장을 억제하는 것으로 밝혀졌다. 이것은 현재 작지 않은 폐암, 중피종, 흑색종, 방광암, 유방암, 췌장암, 결장직장암 및 기타 충실성 종양의 치료를 위해 인간에서 시험되고 있다.

카페시타빈은 5-FU의 종양-선택적 생성을 유도하는, 건강한 조직과 비교하여 종양 조직에서 효소 티미딘 포스포릴라제의 고농도를 이용하는 특이한 메카니즘의활성화를 가진다.
두 무작위 III기 연구가 경구 카페시타빈이 정맥내(i.v.) 5-FU/루코보린(Mayo Clinic 요법)과 비교하여 우수한 반응속도 및 적어도 동등한 생존과 질환 진행 시간을 달성하는, 전이성 결장직장암을 위한 효과적인 제일선의 치료요법임을 보여준다. 카페시타빈은 또한 Mayo Clinic 요법과 비교하여 더욱 호적한 안전성 프로파일을 보인다.
본 발명은 하기의 실시예에 의해 제한받지 않고 설명된다.
실시예 1
경구 CPT-11과 경구 카페시타빈의 투여(단독 및 함께)가 Swiss-nude 마우스에 이식된 인간 결장 암종 스트레인 HCT-116에 대해 평가된다. 두 화합물이 동시-투여되면, 이들은 1시간 간격으로 주어지고, CPT-11이 1차로 주어진다. 각 화합물의 투여 용적은 0.2 ㎖ p.o.이다. 하기의 표는 단독 제제와 배합된 화합물 모두의 최대 용인 투여량이 매우 활성이라는 것을 보여준다.

이러한 결과는 다른 계획이 배합물을 최적화할 수 있다는 것을 나타내고, 실제로 하기 실시예 2가 카페시타빈과 CPT-11의 반-동시 투여가 시험되는 종양에 대해 매우 활성인 치료 배합물을 초래한다는 것을 보여주는데; 즉, 카페시타빈/CPT-11 배합물이 2.8보다 큰 log10세포사 값을 제공한다.
실시예 2
경구 CPT-11과 경구 카페시타빈의 동시-투여가 초기 HCT-116 결장 암종을 지니고 있는 Swiss-nude 마우스에서 평가된다. 최대 비독성 투여량을 정하기 위해 CPT-11의 3 투여량 수준이 18-22일째에 단독으로 주어진다. 카페시타빈의 3 투여량 수준이 18-22 및 25-29일째에 단독으로 주어진다. 배합 아암(arm)에서, 두 가지의 반-동시 계획이 연구되었다. 첫번째 계획은 CPT-11의 4 투여량 수준을 28-22일째에투여하고 카페시타빈의 4 투여량 수준을 18-22일째 및 25-29일째에 투여하는 단계를 포함한다. 두번째 계획에서는, 카페시타빈을 18-22일째에 1차 투여한 다음, CPT-11의 4 투여량 수준을 25-29일째에 동시 투여한다.
두 조합된 결과가 최상의 단독 제제만큼 활성인 것으로 밝혀졌고 각 단독 제제의 최대 비독성 투여량의 42% 및 55%가 주 독성에서 오버랩(overlap) 없이 투여될 수 있다. 투여 순서가 배합된 약제의 내성 차를 유도하는 것 같다. 카페시타빈이 1차 투여되면, 결과는 CPT-11이 1차 투여되었을 때보다 덜 독성이고 더욱 내성인 것으로 생각된다. 그러나, 두 투여 모드 모두에서, 배합물의 효능은 동일하다.
단독 제제 CPT-11 및 카페시타빈과 CPT-11/카페시타빈 배합물의 연구에서 수집되는 결과가 하기 표에 나타나 있다. 3 가지 배합물이 2.8보다 큰 log 세포사로 매우 활성이다.

실시예 3
간헐성 카페시타빈의 표준 투여량과 함께 투여되는 이리노테칸(CPT-11)의 두 가지 계획의 안전성 및 효능이 진행/전이성 결장직장암에 걸린 환자에서 연구된다.
연구의 주목적은 치료 계획의 안전성 프로파일을 비교하는 것이다. 제 2의 목적은 두 가지 치료 아암에서 종양 반응 속도를 비교하는 것이다.
두 가지 투여 계획중 하나에서 환자에게 21일 치료 사이클로 간헐성 경구 카페시타빈과 함께 이리노테칸 i.v. 300/mg/m을 투여한다(도 1 및 2).

치료하지 않았거나 예비치료한 진행된 결장직장암에 걸린 총 19명의 환자가 이 준비 연구에서 등록된다. 질환의 진행이 일어날 때까지 또는 종양 반응을 보이거나 안정한 질환을 앓고 있는 환자에서 최고 10 치료 사이클 동안 약제를 투여한다.
측정 가능하고, 진행되었거나 전이성인 결장직장 선암종에 걸린 환자가 등록하기에 적격이다. 여기에는 적어도 3개월의 평균 여명과 ECOG 동작 상태 0-2를 갖는 18-75세의 환자가 포함된다. 70-75세의 환자들은 ECOG 동작 상태 0-1을 갖도록 요구된다.

안전성이 National Cancer Institute Common Toxicity Criteria(NCI CTC)에 따라 등급을 매긴 부작용으로 연구 약물중 적어도 하나의 투여량을 투여받은 모든 환자에서 평가된다. 핸드-풋 증후군(hand-foot syndrome)을 1-3으로 등급을 매긴다. 종양이 연구원에 의해 세계 보건 기구 기준에 따라 초기에 및 9주 간격으로 평가된다. 지금까지 치료된 19명의 환자에서 치료-관련 부작용의 발생률이 표 4에 나타나 있다. 등급 4의 치료-관련 부작용은 없었다. 환자 한명 만이 독성의 관리를 위해 투여량 수정을 요하였다.

이러한 예비 데이타가 간헐성 경구 카페시타빈을 i.v. 이리노테칸 300 mg/m과 배합하여 1회 투여하거나(1일) 동일한 2회 투여량(1일 및 8일)으로 나눠 투여하는 두 가지의 21일 요법이 호적한 안전성 프로파일을 가짐을 보여주고 진행된 전이성 결장직장암에 걸린 환자에서 항종양 활성을 촉진함을 보여준다. 아암 A의 10명의 환자 중에서, 8명이 부분적인 반응을 보이고 2명이 안정화되었다. 진행성 질환은 보이지 않았다.
따라서, CPT-11과 피리미딘 유도체인 카페시타빈의 배합은 결장직장암과 같은 암의 치료를 위해 매우 활성인 배합물을 초래한다.

- 암의 치료를 위한 피리미딘 유도체 유효량과 함께 캄프토테신 또는 이의 유도체 유효량을 포함하는 치료적 약학 배합물.
- 암의 치료를 위한 피리미딘 유도체 유효량과 함께 CPT-11 유효량을 포함하는 치료적 약학 배합물.
- 제 1 항 또는 제 2 항에 있어서, 피리미딘 유도체가 카페시타빈인 치료 배합물.
- 제 1 항 또는 제 2 항에 있어서, 피리미딘 유도체가 젬시타빈인 치료 배합물.
