캄프토테신 또는 캄프토테신 유도체, 및 피리미딘 유도체를 포함하는 암 치료용 약제 배합물 치료제가 기술되어 있다. 일 양태에서, 피리디민 유도체는 카페시타빈이고 캄프토테신 유도체는 CPT-11이다.

Description
본 출원은 2000년 2월 28일자로 출원된 미국가출원 제 60/185,378 호 및 2000년 6월 5일자로 출원된 미국가출원 제 60/208,938 호의 이점을 청구한다.
본 발명은 캄프토테신 또는 캄프토테신 유도체 유효량과 함께 피리미딘 유도체 유효량을 포함하는, 암의 치료에 유용한 약제 배합물 치료제에 관한 것이다.
본 발명은 암, 특히 충실성 종양의 캄프토테신 유도체와 기타 항암 약제의 배합물로의 치료방법 및 치료의 개선을 위한 이러한 배합물의 사용하는 것에 관한 것이다.
좀더 구체적으로 말하면, 본 발명은 이리노테칸(CPT-11; CamptosarR), 토포테칸, 9-아미노캄프토테신 및 9-니트로캄프토테신과 같은 캄프토테신 유도체와 피리미딘 유도체의 배합물로의 항암 치료에 관한 것이다. 피리미딘 유도체로는 우라실, 티민, 시토신, 메틸시토신 및 티아민 함유 화합물이 포함된다. 이러한 피리미딘 유도체의 예로는 카페시타빈, 젬시타빈 및 페미트렉스트(pemetrexed)로도 알려져 있는 복수-표적화 항폴린산제(MTA)가 있다.

이러한 캄프토테신 유도체는 토포이소머라제 I을 억제하는 항암제이고, 그 중에서도 X-CO-O-가 [4-(1-피페리디노-1-피페리디노]카보닐옥시인 이리노테칸이 충실성 종양, 구체적으로 말하면 결장직장암의 치료에 특히 효과적인 활성 성분이다.
유럽 특허 출원 EP 74,256은 또한 항암제로도 언급되는 다른 캄프토테신 유도체, 구체적으로 말하면 상기에 도시된 구조와 유사한 구조를 가지나 X-CO-O-가 라디칼 -X'R'(여기에서, X'는 O 또는 S이고 R'는 수소 원자나 알킬 또는 아실 라디칼이다)로 대체되는 유도체를 기술하고 있다.

캄프토테신 유도체는 통상적으로 주사액으로, 좀더 구체적으로 말하면 멸균액 또는 멸균 에멀션 형태로 정맥내로 투여된다. 그러나, 캄프토테신 유도체는 또한 고체형 또는 액체형 조성물로 경구 투여될 수도 있다.
CPT-11은 결장직장암에 가장 활성이 있는 신규 제제중 하나이다. 결장직장암은 매년 유럽과 미국에서 신규 약 300,000건의 질병률과 200,000건의 사망률의 주요 원인이다(참조: P. Boyle, Some Recent Developments in the Epidemiology of Colorectal Cancer, pages 19-34 in Management of Colorectal Cancer, Bleiberg H., Rougier P., Wilke H.J., eds, (Martin Dunitz, London 1998); 및 Midgley R.S., Kerr D.J., Systemic Adjuvant Chemotherapy for Colorectal Cancer, pages 126-27 in Management of Colorectal Cancer, Bleiberg H., Rougier P., Wilke H.J., eds, (Martin Dunitz, London 1998)). 환자중 약 50%는 수술만으로도 치료되지만, 진단시 전이의 증거가 보이는 환자 대략 25%를 포함하는 나머지 절반은 전이성 질환때문에 인해 결국 사망할 것이다.
5-FU에 대해 내성을 갖는 결장직장암 환자에서, 2번에 걸친 대규모 III기 무작위 실험에서 시험한 단독 제제 CPT-11은 보조적인 치료(supportive care)와 비교하였을 때 생존기간이 더 길며 생활의 질이 더 높아졌고(D. Cunningham, S. Pyrhonen, RD. James 등, The Lancet, 352(9138): 1413-1418 (1998)), 또한 5-FU/FA을 이용한 주입 요법과 비교하였을 때 생활의 질은 저하됨이 없이 생존 기간을 더욱 연장시켰다(P. Rougier, E. van Cutsem 등; The Lancet, 352(9138): 1407-1418 (1998)). 따라서 CPT-11은 선행의 5-FU로의 치료가 실패한 후에도 전이성 결장직장암(MCRC)에 대한 대조 치료가 될 수 있다.
또한, CPT-11은 화학요법 치료경험이 없는 전이성 결장직장암(MCRC) 환자에 대해서만은 이른바 표준 5-FU/FA 거환약 만큼 활성이 있는 것으로 보고되었다[Proc. Am. Soc. Clin. Oncol., vol 13 (1994), (Abstr. #573); J. Clin Oncol, 14(3): 709-715 (1996); J. Clin Oncol, 15(1): 251-260(1997)].
이리노테칸(CPT-11)과 5-FU의 배합물은 이미 일본에서의 I기 연구에서 연구된 바 있는 것으로서, 예비 결과로부터 안전성이란 측면으로는 동시투여가 실현가능하다는 것을 알 수 있다(L. Saltz 등, Eur. J. Cancer 32A, suppl 3: S24-31(1996)).
D. Cunningham, Eur. J. Cancer, 32A suppl. 3:S1-8 (1996)에 의해 공개된 CPT-11에 관한 연구는 CPT-11이 결장직장암에서 5-FU/폴린산의 사용을 보완할 수 있는 상이한 세포독성의 접근법을 제공한다고 결론 내렸다.
배합물의 효능을 설명하기 위해, 문제의 연구에서 배합물의 용인되는 최대 투여량을 각 개별 구성물의 용인되는 최대 투여량과 비교하는 것이 필요할 수 있다. 이러한 배합물의 효능은 예를 들면, 하기식으로 결정되는 log10 세포사에 의해 정량화할 수 있다:

상기식에서,
T-C는 처리 그룹(T)의 종양 및 처리 그룹(C)의 종양이 예상치(예를 들면, 1 g)에 도달하는 평균 시간(일)인 세포가 성장하는 데 걸린 시간을 나타내고,

log10 세포사가 0.7 이상이면 산물이 활성이 있는 것으로 간주된다. log10 세포사가 2.8보다 크면 산물이 매우 활성이 있는 것으로 간주된다.
배합물의 효능은 또한 치료제의 상승작용을 측정함에 의해 설명될 수 있다. 배합물이 최적의 투여량으로 사용되는 한 구성물 또는 다른 구성물보다 치료제로서의 효과가 우수하면 치료제로서 상승작용이 있음을 입증된다(T. H. Corbett 등, Cancer Treatment Reports, 66, 1187 (1982)).
본 발명은 캄프토테신 유도체와 피리미딘 유도체의 배합물이 난소암, 폐암 및 결장직장암과 같은 충실성 종양의 치료에 특히 효과적이라는 것이 밝혀내어 본 발명을 완성하였다. 피리미딘 유도체 중에서도, 젬시타빈, MTA (복수-표적화 항폴린산제) 및 카페시타빈이 효과적이다.
젬시타빈이 항종양 활성을 보인다. 젬시타빈의 염인 2'-데옥시-2',2'-디플루오로시티딘 모노하이드로클로라이드는 작지 않은 세포 폐암(NSCLC)과 같은 충실성 종양의 치료를 위한 정맥주사액으로서의 임상적 사용을 위해 제공된다.
젬시타빈은 주로 DNA 합성을 시행하는 세포(S기)를 사멸시키는 세포 주기 특이성을 보이고 또한 G1/S기의 경계에서 세포의 발달을 차단한다. 젬시타빈은 뉴클레오사이드 키나제에 의해 활성 디포스페이트(dFdCDP) 및 트리포스페이트(dFdCTP) 뉴클레오사이드로 세포내에서 물질대사로 전환된다. 젬시타빈의 세포독성 효과는 DNA 합성의 억제를 유도하는, 디포스페이트 및 트리포스페이트 뉴클레오사이드의 두 작용의 조합에 기인하는 것이다. 첫째, 젬시타빈 디포스페이트는 DNA 합성을 위한 데옥시뉴클레오사이드 트리포스페이트를 생성하는 반응을 촉매하는 기능이 있는 리보뉴클레오타이드 환원효소를 억제한다. 이 효소의 디포스페이트 뉴클레오사이드에 의한 억제는 dCTP를 포함하는 데옥시뉴클레오타이드 농도의 감소를 초래한다. 둘째, 젬시타빈 트리포스페이트는 DNA에의 도입을 위해 dCTP와 경쟁한다. dCTP의 세포내 농도의 감소(디포스페이트의 작용에 의한)는 젬시타빈 트리포스페이트의 DNA에의 도입(자기-잠재력)을 증가시킨다. 젬시타빈 뉴클레오타이드가 DNA에 도입된 후에, 부가의 뉴클레오타이드 하나만이 DNA에 도입된다. 이러한 첨가 이후에, DNA 합성이 추가로 억제된다.
젬시타빈은 II기 연구에서 CPT-11과 배합되어 췌장암의 치료 가능성을 보였다.
MTA(복수-표적화 항폴린산제)는 폴레이트 길항제인 대사길항물질, 디하이드로폴레이트 환원효소 억제제 및 티미딜레이트 합성효소 억제제이다. 이것은 정맥주사액으로 사용하기 위해 제공되고 마우스에서 종양 성장을 억제하는 것으로 밝혀졌다. 이것은 현재 작지 않은 폐암, 중피종, 흑색종, 방광암, 유방암, 췌장암, 결장직장암 및 기타 충실성 종양의 치료를 위해 인간에서 시험되고 있다.
카페시타빈은 항신생물 활성이 있는 플루오로피리미딘 카바메이트이다. 이것은 체내에서 5-플루오로우라실로 전환되는 5'-데옥시-5-플루오로우리딘(5'-DFUR)의 경구 투여되는 전구약물이다. 예비임상 연구에서, 카페시타빈은 5-FU에 대해 내성을 갖는 암종을 포함하여 결장직장, 유방 및 머리와 목의 암종에 대해 활성이 있음을 보였다.
카페시타빈은 화학명이 5'-데옥시-5-플루오로-N-[(펜틸옥시)카보닐]-시티딘이고 분자량이 359.35이다. 카페시타빈은 하기 화학식을 가진다:

카페시타빈은 건강한 조직과 비교하였을 때 종양 조직에서 효소 티미딘 포스포릴라제의 고농도를 이용하는 특이한 활성화 메카니즘을 가져서, 5-FU의 종양-선택적 생성을 유도하게 된다.
두 무작위 III기 연구에 의해면, 카페시타빈의 경구 투여가 5-FU/루코보린(Mayo Clinic 요법)의 정맥내 투여에 비교하여 반응속도가 우수하며 생존과 질환 진행 시간이 적어도 동등하게 되는, 가장 효과적인 전이성 결장직장암 치료요법임을 알 수 있다. 카페시타빈은 또한 Mayo Clinic 요법과 비교하여 더욱 호적한 안전성 프로파일을 보인다.
본 발명은 하기의 실시예에 의해 설명되나 그에 의해 제한되지는 않는다.
실시예 1
경구 CPT-11 및 경구 카페시타빈의 투여(단독으로 및 병용하여)를 Swiss-nude 마우스에 이식시킨 인간 결장 암종 스트레인 HCT-116에 대해 평가한다. 두 화합물이 동시-투여되는 경우, 이들은 1시간 간격으로 투여하고, CPT-11을 1차로 투여한다. 각 화합물의 투여 용량은 0.2 ㎖ p.o.이다. 하기의 표로 부터 단독 제제와 병용투여된 화합물 둘다에 대해 최대로 용인되는 투여량이 가장 활성이 있다는 것을 알 수 있다.

이러한 결과를 통해 다른 계획으로도 배합물을 최적화할 수 있다는 것을 알 수 있고, 실제로 하기 실시예 2에 의해 카페시타빈과 CPT-11의 반-동시 투여가 시험되는 종양에 대해 매우 활성이 있는 치료 배합물을 유도한다는 것을 알 수 있는데; 즉, 카페시타빈/CPT-11 배합물은 log10 세포사 값이 2.8보다 크다.
실시예 2
경구 CPT-11과 경구 카페시타빈의 동시-투여를 초기 HCT-116 결장 암종을 지니고 있는 Swiss-nude 마우스에 대해서 평가한다. 무독성의 최대 투여량을 정하기 위해 CPT-11을 단독으로 18-22일째에 3개의 투여량으로 투여한다. 카페시타빈을 단독으로 18-22 및 25-29일째에 3개의 투여량으로 투여한다. 배합 아암(arm)에서, 2개의 반-동시 계획을 연구하였다. 첫번째 계획은 CPT-11을 28-22일째에 4개의 투여량으로 투여하고 카페시타빈의 18-22일째 및 25-29일째에 4개의 투여량으로 투여하는 단계를 포함한다. 두번째 계획에서는, 카페시타빈을 18-22일째에 1차로 투여한 다음, CPT-11의 25-29일째에 4개의 투여량으로 동시 투여한다.
두 조합된 결과가 최상의 단독 제제만큼 활성이 있는 것으로 밝혀졌고 각각의 단독 제제에 대한 무독성의 최대 투여량의 42% 및 55%가 피검자에게 상응하는 독성을 나타냄이 없이 투여될 수 있다. 투여 순서는 배합된 약제의 내성에 있어서의 차이를 유도하는 것 같다. 카페시타빈이 1차로 투여되면, 결과는 CPT-11이 1차로 투여되었을 때보다 독성은 감소하나 내성은 증가하는 것으로 여겨진다. 그러나, 두가지 투여 모드 둘다에 있어서, 배합물의 효능은 동일하다.
단독 제제 CPT-11 및 카페시타빈과 CPT-11/카페시타빈 배합물에 대한 연구를 통해 수집한 결과를 하기 표에 명기하였다. 3 가지 배합물은 log 세포사 값이 2.8보다 큰 매우 활성이 있는 것이다.

실시예 3
간헐적으로 카페시타빈을 표준 투여량으로 투여하면서 이리노테칸(CPT-11)을 투여하는 두 가지 계획의 안전성 및 효능을 진행성/전이성 결장직장암 환자에 대해서 연구하였다.
연구의 주목적은 치료 계획의 안전성 프로파일을 비교하는 것이다. 제 2의 목적은 두 가지 치료 아암에서 종양 반응 속도를 비교하는 것이다.
두 가지 투여 계획중 하나에서 환자당 21일의 치료 주기를 가지고 카페시타빈을 간헐적으로 경구 투여하면서 이리노테칸 300/mg/m을 정맥주사로 투여한다(도 1 및 2).

치료하지 않았거나 예비치료한 진행성 결장직장암 환자 총 19명이 이러한 치료계획의 연구를 위해 등록되었다. 질환의 진행이 일어날 때까지 또는 종양 반응을 보이거나 안정한 질환을 앓고 있는 환자에서 최고 10의 치료 주기 동안 약제를 투여한다.
측정 가능하고, 진행되었거나 전이성인 결장직장 선암종 환자가 적격의 등록요건을 갖춘다. 여기에는 적어도 3개월의 평균 여명과 ECOG 동작 상태 0-2를 갖는 18-75세의 환자가 포함된다. 70-75세의 환자들은 ECOG 동작 상태 0-1을 갖는 것이 요구된다.

*치료 아암 B의 환자 한 명이 2 치료 주기 후에 사망하였다(치료와 무관) *환자 한명이 두 셋팅 모두에서 치료를 받았다 |
National Cancer Institute Common Toxicity Criteria(NCI CTC)에 따라 등급을 매긴 부작용으로 연구 약물중 적어도 하나의 투여량을 투여받은 모든 환자에 대해 안정성을 평가한다. 핸드-풋 증후군(hand-foot syndrome)을 1-3으로 등급을 매긴다. 연구원은 종양을 세계 보건 기구 기준에 따라 초기에 및 9주 간격으로 평가한다. 지금까지 치료한 19명의 환자중에서 치료-관련 부작용의 발생률을 표 4에 명기하였다. 등급 4의 치료-관련 부작용은 없었다. 환자 한명 만은 독성의 관리를 위해 투여량 수정이 필요하였다.

이러한 예비 데이타로 부터 페시타빈을 간헐적으로 경구투여하면서 이리노테칸 300 mg/m을 정맥주사로 병용하여 1회 투여하거나(1일) 동일양을 2회 투여량(1일 및 8일)으로 나누어 투여하는 두 가지의 21일 요법이 호적한 안전성 프로파일을 가짐을 보여주고 진행된 전이성 결장직장암 환자에서 항종양 활성을 촉진함을 보여준다. 아암 A의 10명의 환자 중에서, 8명이 부분적인 반응을 보이고 2명이 안정화되었다. 진행성 질환은 보이지 않았다.
따라서, CPT-11과 피리미딘 유도체인 카페시타빈을 병용하여 사용하면 이 배합물은 결장직장암과 같은 암 치료에 매우 활성이 있게 된다.

- 캄프토테신 유효량 및 카페시타빈 유효량을 포함하는 암 치료용 약제 배합물 치료제.
- 캄프토테신-11(Camptothecin-11; CPT-11) 유효량 및 카페시타빈 유효량을 포함하는 암 치료용 약제 배합물 치료제.
