Description
Bakgrunn for oppfinnelsen
Kopolymer-1 er en syntetisk polypeptidanalog av myelingrunnprotein (MBP), som er en naturlig bestanddel i myelinkjeden. Denne er foreslått som et potensielt terapeutisk middel mot multippel sklerose (Eur. J. Immunol. [1971] 1:242; og J. Neurol, Sei. [1977] 31:433). Interesse for kopolymer-1 som immunterapi mot multippel sklerose har sitt opphav i observasjoner som først ble gjort på 1950-tallet, som viste at myelinkomponenter, slik som MBP, forebygger eller stopper eksperimentell autoimmun encefalomyelitt (EAE). EAE er en sykdom som ligner multippel sklerose, som kan induseres i mottakelige dyr.
Kopolymer-1 ble utviklet av Drs. Sela, Aron, og deres medarbeidere ved Weizmann Institute (Rehovot, Israel). Denne ble vist å undertrykke EAE (Eur. J. Immunol. [1971] 1:242; U.S. patent nr. 3849550). I den senere tid har kopolymer-1 vist seg å være fordelaktig for pasienter med forverret remitterende form av multippel sklerose (N. Engl. J. Med. [1987] 317:408). Pasienter behandlet med daglige injeksjoner av kopolymer-1 hadde færre forverringer og lavere økning i deres uførhetsstatus enn kontrollpasientene.
Kopolymer-1 er en blanding av polypeptider satt sammen av alanin, glutaminsyre, lysin og tyrosin i et molart forhold på henholdsvis omtrent 6:2:5:1. Den syntetiseres ved kjemisk polymerisering av de fire aminosyrer for dannelse av produkter med gjennomsnittlige molekylvekter på 23 000 dalton (U.S. patent nr. 3849550).
Det er en målsetning med foreliggende oppfinnelse å tilveiebringe et forbedret preparat av kopolymer-1.
Oppsummering av oppfinnelsen
Foreliggende oppfinnelse omfatter kopolymer-1-fraksjon, fremgangsmåte for fremstilling av denne, anvendelse, sammensetning og fremfangsmåte for fremstilling av et preparat for behandling av multippel sklerose samt kopolymer-1-preparat.
Foreliggende oppfinnelse vedrører et preparat av kopolymer-1 som er vesentlig fritt for utgaver av kopolymer-1 med en molekyl vekt over 40 kilodalton (KDa).
Foreliggende oppfinnelse vedrører videre en kopolymer-1 som har over 75 % av sin molare fraksjon innen
molekylvektsområdet fra ca. 2 KDa til ca. 20 KDa.
I tillegg vedrører foreliggende oppfinnelse en kopolymer-1 med en gjennomsnittlig molekylvekt fra ca. 4 til ca. 8,6 KDa.
Dessuten vedrører foreliggende oppfinnelse et farmasøytisk preparat og en fremgangsmåte for fremstilling av et preparat for behandling av multippel sklerose, ved anvendelse av den ovennevnte kopolymer-1.

Detaljert beskrivelse av oppfinnelsen
Foreliggende oppfinnelse omfatter kopolymer-l-fraksjon kjennetegnet ved at fraksjonen inneholder mindre enn 5% av utgaver av kopolymer-1 som har en molekylvekt på over 40 kilodalton og hvori over 75% av fraksjonen er innen et molekylvektsområde fra 2 kilodalton til 20 kilodalton.
Foreliggende oppfinnelse vedrører således et preparat av kopolymer-1, som i det vesentlige er fritt for utgaver av kopolymer-1 med en molekyl vekt over 40 kilodalton (KDa). Fortrinnsvis inneholder preparatet mindre enn 5 % av utgaver av kopolymer-1 med en molekylvekt på 40 KDa eller mer. Mer foretrukket inneholder preparatet mindre enn 2,5 % av utgaver av kopolymer-1 med en molekylvekt på 40 KDa eller mer.
Foreliggende oppfinnelse vedrører dessuten en kopolymer-1 med over 75 % av sin molare fraksjon innen molekylvektsområdet fra ca. 2 KDa til ca. 20 KDa.
I tillegg vedrører foreliggende oppfinnelse en kopolymer-1 med en gjennomsnittlig molekylvekt på mellom ca. 4 til ca. 8,6 KDa. Særlig vedrører foreliggende oppfinnelse en kopolymer-1 med en gjennomsnittlig molekylvekt fra ca. 4 til ca. 8 KDa, og en kopolymer-1 med en gjennomsnittlig molekylvekt fra ca. 6,25 til ca. 8,4 KDa.
Foreliggende oppfinnelse omfatter også sammensetning for behandling av multippel sklerose kjennetegnet ved at den omfatter en kopolymer-1-sammensetning hvorved sammensetningen inneholder mindre enn 5 % av utgaver av kopolymer-1 som har en molekylvekt på mer enn 40 kilodalton og 75 % av nevnte kopolymer-1 i sammensetningen er innen et molekylvektsområde fra 2 til 20 kilodalton.

Kopolymer-1, ifølge foreliggende oppfinnelse, kan fremstilles ved fremgangsmåter kjent innen fagfeltet, for eksempel fremgangsmåten gjort rede for i U.S. patent 3849550, hvori N-karboksyanhydridene av tyrosin, alanin, ybenzylglutamat og E-N-trifluor-acetyllysin polymeriseres ved omgivelsestemperatur i vannfritt dioksan med dietylamin som initiator. Avblokkeringen av y-karboxylgruppen til glutaminsyren effektueres ved hjelp av hydrogenbromid i iseddik og etterfølges av fjerning av trifluoracetylgruppene fra lysinrestene ved hjelp av IM piperidin. I forbindelse med formålene med søknaden må uttrykkene "omgivelsestemperatur" og "romtemperatur" forstås å bety en temperatur som varierer fra ca. 20 til ca. 26°C.
Kopolymer-1 med den ønskede molekylvektsprof il. kan erholdes ved hjelp av kjente fremgangsmåter. Slike fremgangsmåter omfatter kromatografi av kopolymer-1 inneholdende utgaver med høy molekylvekt og oppsamling av fraksjoner uten de uønskede utgaver, eller ved hjelp av delvis syre- eller enzymatisk hydrolyse for fjerning av utgavene med høy molekylvekt etterfulgt av rensing ved hjelp av dialyse eller ultrafiltrering. En ytterligere fremgangsmåte for erholdelse av kopolymer-1 med den ønskede molekylvektsprofil, er ved fremstilling av de ønskede utgaver mens aminosyrene fortsatt er beskyttet, og så erholde de korrekte utgaver, direkte etter fjerning av beskyttelsen. Preparatene ifølge foreliggende oppfinnelse kan utarbeides ved hjelp av vanlige fremgangsmåter kjent innen fagfeltet. Fortrinnsvis lyofiliseres og dannes preparatet til en vandig løsning egnet for subkutan injeksjon. Alternativt kan kopolymer-1 utarbeides i enhver av utgavene kjent innen fagfeltet, for fremstilling av orale, nasale, buccale eller rektale preparater av peptidmedikamenter.
Vanligvis administreres kopolymer-1 daglig til pasienter som lider av multippel sklerose i en dose på 20 mg.
Foreliggende oppfinnelse vil eksemplifiseres, men begrenses nødvendigvis ikke av de følgende eksempler.
Eksempel 1
Kromatografisk fremgangsmåte for fremstilling av kopolymer- 1 med lav toksisitet
To satser av kopolymer-1 ble fremstilt ifølge fremgangsmåter kjent innen fagfeltet, for eksempel U.S. patent nr. 3849550.
En sats ble utsatt for krormatografisk separering, som beskrevet nedenfor.
En kolonne for gelfiltrering FRACTOGEL TSK HW55 (600 x 26 mm) ble fremstilt i en Superformance 26 Merck beholder, ifølge produsentens instruksjoner. Kolonnen ble ekvilibrert med vann, og acetonløsning ble tilsatt for bestemmelse av totalt volum. Kolonnen ble ekvilibrert med 0,2 M ammoniumacetatbuffer pH 5,0. Kopolymer-1-prøver på 30 ml (20 mg/ml, i 0,2 M ammoniumacetat pH 5,0) ble anbrakt på kolonnen og fraksjoner ble oppsamlet hvert tiende minutt. En fraksjon med en gjennomsnittlig molekylvekt på 7-8 KDa ble isolert mellom 120-130 minutter.

Molekylvektanalyser
UV-absorbans ved 275 nm ble bestemt i et UVIKON 810 spektrofotometer. Prøver ble fortynnet for å erholde en UV-absorbans som var lavere enn 1 absorpsjonsenhet. Molekylfordelingen til de to satser ble bestemt på en kalibrert gelfiltreringskolonne (Superose 12).
Kopolymer-1 sats A ble funnet å ha en gjennomsnittlig molekylvekt på 7-8 KDa. 2,5 % av denne sats hadde en molekylvekt over 32 KDa, men ingen kopolymer-1-utgaver til stede i denne sats hadde en molekylvekt over 40 KDa.
Den andre satsen av kopolymer-1, som ikke ble underkastet for kromatografi, hadde en gjennomsnittlig molekylvekt på 12 KDa. 2,5 % av satsen hadde en molekylvekt over 42 KDa og 5 % av de totale kopolymer-1 utgaver i denne satsen hadde en molekylvekt på over 40 KDa.

Toksisitetsanalvser
A: In Vivo
Tre satser av kopolymer-1, med en gjennomsnittlig molekylvekt på 7,3 og 8,4 KDa (mindre enn 2,5 % kopolymer-1-utgaver over 40 KDa) og 22 KDa (mer enn 5 % kopolymer-1-utgaver over 40 KDa) ble underkastet toksisitetstesten beskrevet under. I hvert tilfelle ble 5 mus anvendt i hver forsøksgruppe.
Fremgangsmåte
Kopolymer-1 ble oppløst i destillert vann for å oppnå en løsning på 2 mg/ml av den aktive bestanddel. Hver mus ble injisert med 0,5 ml av test løsningen inn i den laterale halevene. Mus ble observert med hensyn til mortalitet og relevante kliniske tegn over en 48 timers periode. Observasjoner ble registrert i 10 minutter, 24 timer og 48 timer etter injeksjon. Dersom alle dyrene var i live og ingen skadelige tegn var observert etter 48 timer, ble satsen angitt som "ikke-toksisk". Dersom imidlertid én eller flere av musene var døde eller viste tegn på- skade, ble satsen angitt som "toksisk".
Satsene med gjennomsnittlig molekylvekt på 7,3 og 8,4 KDa ble begge angitt som "ikke toksisk", mens i satsen med gjennomsnittlig molekylvekt på 22 KDa, var 3 av 5 mus døde etter 48 timers perioden, og den ble derfor kalt "toksisk".

RBL - degranuleringstest
I. Introduksj on
Histamin (eller serotonin) frigjørelse fra basofile celler er en in vitro-modell for umiddelbar hypersensitivitet. Basofilleukemicellelinjen fra rotte (RBL-2H3) ble utviklet og karakterisert som et høysensitivt, enhetlig, lett å opprettholde i kultur og reproduserbart system (E.L. Basumian, C. Isersky, M.G. Petrino og R.P. Siraganian. Eur. J. Immunol. II, 317 (1981) . De fysiologiske stimuli for histaminfrigjøring involverer binding av antigenet til membranbundne IgE-molekyler, noe som resulterer i de sistnevntes kryssbinding og som en konsekvens, utløsning av en intrikat biokjemisk kaskade. Ved siden av disse fysiologiske, immunoglobulinformidlende utløsere, kan degranulering induseres ved ulike ikke-IgE-formidlede stimuli. Blant disse er ulike peptider og syntetiske polymerer, for eksempel polylysin (R.P. Siraganian. Trends in Pharmacological Sciences, oktober 432 (1983)). RBL-degranuleringstesten er derfor anvendt for å skille ut de satser av kopolymer-1 som fremkaller betydelig degranulering, og som på denne måte kan frembringe uønskede lokale og/eller systemiske bivirkninger.
II. Prinsipp bak testmetoden
Basofile leukemiceller fra rotte (RBL-2H3) anbringes med (3H] -serotonin, etterfulgt av inkubering med 100 /xg av kopolymer-1 som skal testes. Satser av kopolymer-1 som induserer ikke-spesifikk degranulering, frigjør (<3>H]-serotonin til mediet. Radioaktiviteten i mediet måles ved hjelp av en skintillasjonsteller, og den totale mengde av radiomerket serotonin inkorporert i cellene bestemmes i de pelleterte celler. Prosentvis degranulering beregnes i form av prosenten av serotonin frigjort i forhold til det totalt inkorporerte.
III. Resultater
Fire satser av kopolymer-1, med gjennomsnittlig molekylvekt mellom 6250-14500, ble analysert med hensyn til både % av utgavene med molekylvekt over 4 0 KDa og med hensyn til degranulering av RBL'er. Resultater er oppsummert i den etterfølgende tabell.
Som det kan observeres, når % av høye molekylvektsutgaver er lav (< 2,5), er frigjøringsprosenten til serotonin, indikerende for toksisitet, lav, og vise versa.
Eksempel 3
Fremstilling av trifluoracetvl- kopolvmer- 1
Beskyttet kopolymer-1 fremstilles som beskrevet av Teitelbaum et al. Eur. J. Immun. vol. 1 s. 242 (1971) fra N-karboksyanhydrider av tyrosin (18 g), alanin (50 g), ybenzylglutamat (35 g) og trifluoracetyllysin (83 g) oppløst i 3,5 liter dioksan.
Polymeriseringsprosessen startes ved tilsetning av 0,01-0,02 % dietylamin. Reaksjonsblandingen røres ved romtemperatur i 24 timer og helles så i 10 liter vann. Produktet (beskyttet
kopolymer-1) filtreres, vaskes med vann og tørkes. Fjerning av gamma-benzylblokkeringsgruppene fra glutamatrester utføres ved behandling av den beskyttede kopolymer-1 med 33 % hydrobromsyre i iseddik ved romtemperatur i 6-12 timer med omrøring. Produktet helles i overskuddsvann, filtreres, vaskes og tørkes, for å gi trifluoracetylkopolymer-1.
Eksempel 4
Fremstilling av trifluoracetyl- kopolvmer- 1
Beskyttet kopolymer-1 fremstilles som beskrevet av Teitelbaum et al. Eur. J. Immun. vol. 1 s. 242 (1971) fra N-karboksyanhydrider av tyrosin (18 g), alanin (50 g), x-benzylglutamat (35 g) og trifluoracetyllysin (83 g) oppløst i 3,5 liter dioksan.
Polymeriseringsprosessen startes ved tilsetning av 0,01-0,02 % dietylamin. Reaksjonsblandingen røres ved romtemperatur i 24 timer, for så å helles i 10 liter vann. Produktet (beskyttet kopolymer-1) filtreres, vaskes med vann og tørkes.
Beskyttet kopolymer-1 behandles med 33 % HBr i eddiksyre, som fjerner de omegabenzylbeskyttende grupper fra 5-karboksylatet i glutamatresten og bryter opp polymeren til mindre polypeptider. Tiden som trengs for å erholde kopolymerl med molekylvekt 7000 ± 2000 Da, avhenger av
reaksjonstemperaturen og størrelsen på beskyttet kopolymer-1.
Ved temperaturer mellom 20-28 °C, utføres en testreaksjon på hver sats ved ulike tidsperioder, for eksempel fra 10-50 timer.
Resultatene med hensyn til molekylvektene ved disse reaksjoner i liten skala beregnes og en kurve av molekylvekt mot tid tegnes. Tiden som trengs for å erholde molekylvekt 7000 + 2 000 Da kalkuleres fra kurven og utføres i reaksjoner i større skala. Ved arbeid i 26°C er tidsperioden 17 timer i gjennomsnitt. Fremstilling av lavtoksisitetskopolymer- 1
20 g trifluoracetyl-kopolymer-1 dispergeres i 1 liter vann og det tilsettes 100 g piperidin. Blandingen røres i 24 timer ved romtemperatur og filtreres. Løsningen av urenset kopolymer-1 fordeles i dialyseposer og dialyseres ved 10-20 °C mot vann inntil pH = 8 er oppnådd. Det dialyseres så mot ca. 0,3 % eddiksyre og på nytt mot vann inntil pH = 5,5-6,0 oppnås. Denne løsning konsentreres så og lyofiliseres til tørrhet.
