Description
Foreliggende oppfinnelse vedrører anvendelse av en forbindelse som angitt i krav 1 ved fremstilling av et medikament for å fremme neuronal vekst og regenerering, for å behandle
en neurologisk sykdom eller å forhindre neurodegenerering. Slike forbindelser er neurotrofiske forbindelser som har en affinitet med immunofiliner av FKBP-typen, og kan anvendes
som hemmere av enzymaktivitet som er forbundet med immunofilinproteiner, og spesielt hemmere av peptidylprolylisomerase- eller rotamaseenzymaktivitet.
Beskrivelse av teknikkens stand
Begrepet immunofilin vedrører et antall proteiner som
tjener som receptorer for de primære immunosuppressive legemidler, nemlig cyklosporin A (CsA), FK506 og rapamycin. Kjente immunofilin-klasser er cyklofiliner og FK506-binden-
de proteiner, såsom FKBP. Cyklosporin A bindes til cyklo-filin, mens FK506 og rapamycin bindes til FKBP. Disse immunofilin/legemiddel-komplekser samvirker med diverse intracellulære signalomsetningssystemer, spesielt i immunsystemet og nervesystemet.
Immunofiliner er kjent for å ha peptidylprolylisomerase-(PPIase-) eller rotamaseenzymaktivitet. Man har påvist at rotamaseaktiviteten spiller en rolle ved katalysering av den innbyrdes omdannelse av cis- og trans-isomerene av immunofilinproteiner.
Immunofiliner ble opprinnelig oppdaget og undersøkt i immunvev. Det ble opprinnelig postulert av fagmenn at en hemming av immunofilinenes rotamaseaktivitet fører til en hemming av T-celleproliferasjonen og dermed forårsaker den immunosuppressive virkning som oppvises av immunosuppres-
sive legemidler, såsom cyklosporin A, FK506 og rapamycin. Nærmere undersøkelser har vist at hemmingen av rotamaseaktiviteten i og for seg ikke er tilstrekkelig for å utøve en immunosuppressiv virkning. Schreiber et al., Science, 1990,
vol. 250, s. 556-559. Man har vist at immunofilin/legemiddel-kompleksenes virkningsmåte er å samvirke med ternære målproteiner. Schreiber et al., Cell, 1991, vol. 66, s. 807-815. I tilfellet av FKBP-FK506 og FKBP-CsA, bindes legemiddel/immunofilin-kompleksene til enzymet kalsineurin, og en hemmende T-cellereceptor-signalisering fører til T-celleproliferasjon. På lignende måte samvirker komplekset av rapamycin og FKBP med RAFTl/FRAP-proteinet og hemmer signaliseringen fra IL-2-receptor.
Man har funnet at immunofiliner foreligger i høye konsentrasjoner i det sentrale nervesystem. Immunofilinene er 10-50 ganger mer anriket i det sentrale nervesystem enn i immunsystemet. Innen neurale vev virker immunofilinene å påvirke en forlengelse av neuronale tapper, nitrogenoksyd-syntesen og frigivningen av neurotransmittere.
Man har funnet at picomolare konsentrasjoner av et immuno-suppressivt middel, såsom FK506 og rapamycin, stimulerer neurittutveksten i PC12-celler og sensoriske nerveceller, nemlig ryggrotganglieceller (DRG1 er). Lyons et al.. Proe. of Nati. Acad. Sei., 1994, vol. 91, s. 3191-3195. I eksperimenter på hele dyr, har FK506 vist seg å stimulere nerve-regenerasjonen etter skade på nervus facialis, og fører også til en funksjonell bedring i dyr med isjiasnervelesjoner.
Man har overraskende funnet at legemidler med en høy affinitet for FKBP er sterke rotamasehemmere som forårsaker en neurotrofisk virkning. Lyons et al., Proe. of Nati. Acad. Sei., 1994, vol. 91, s. 3191-3195. Disse funn tilsier anvendelsen av immunosuppressive midler ved behandling av forskjellige perifere neuropatier og bedring av en neuronal gjenvekst i det sentrale nervesystem (CNS). Undersøkelser har vist at neurodegenerative lidelser, såsom Alzheimers sykdom, Parkinsons sykdom og amyotrofisk lateralsklerose (ALS) kan opptre på grunn av et tap, eller en nedsatt tilgjengelighet, av en neurotrofisk substans som er spesi-fikk for en bestemt neuronpopulasjon som rammes av lidelsen.
Man har identifisert flere neurotrofiske faktorer som påvirker bestemte neuronale populasjoner i det sentrale nervesystem. Man har f.eks. stilt opp en hypotese om at Alzheimers sykdom forårsakes av en nedsatt tilgjengelighet eller et tap av nervevekstfaktor (NGF). Man har derfor foreslått å behandle pasienter som lider av Alzheimers sykdom, med eksogen nervevekstfaktor eller andre neuro-trof iske proteiner, såsom hjerneavledet nervefaktor (BDNF), gliaavledet nervefaktor, ciliær neurotrofisk faktor og neurotropin-3, for å øke overlevelsesraten av degenererende neuronale populasjoner.
En klinisk anvendelse av disse proteiner i forskjellige neurologiske sykdomstilstander forstyrres av vanskeligheter forbundet med tilførselen og biotilgjengeligheten av store proteiner til tilsiktede områder av nervesystemet. Derimot er immunosuppressive legemidler som har neurotrofisk virkning, relativt små, og de oppviser en utmerket biotilgjen-gelighet og spesifisitet. Når de administreres kronisk, oppviser de immunosuppressive midler imidlertid et antall potensielt alvorlige bivirkninger, omfattende nefrotoksisi-tet, såsom forstyrrelse av glomerulærfiltrasjonen og en irreversibel interstitiell fibrose (Kopp et al., 1991, J. Am. Soc. Nephrol. 1:162); neurologiske mangler, såsom ufrivillig skjelving, eller ikkespesifikk cerebral angina, såsom ikkelokalisert hodepine (De Groen et al., 1987, N. Engl. J. Med. 317:861); og vaskulær hypertensjon med kom-plikasjoner som resulterer derav (Kahan et al., 1989, N. Engl. J. Med. 321:1725).
For å forebygge bivirkningene som er forbundet med bruken av immunosuppressive forbindelser, frembringer foreliggende oppfinnelse ikke-immunosuppressive forbindelser som inneholder småmolekylære FKBP-rotamasehemmere for å fremme den neuronale vekst og regenerasjonen i forskjellige neuropato-logiske situasjoner hvor neuronal reparasjon kan fremmes, bl.a. perifer nerveskade etter fysisk skade eller en syk-domstilstand såsom diabetes, fysisk skade på det sentrale nervesystem (ryggmarg og hjerne), hjerneskade forbundet med slag og for behandling av neurologiske forstyrrelser som er forbundet med neurodegenerasjon, bl.a. Parkinsons sykdom, Alzheimers sykdom og amyotrofisk lateralsklerose.
SAMMENFATNING AV OPPFINNELSEN
Foreliggende oppfinnelse vedrører anvendelse av en type neurotrofiske forbindelser som har en affinitet for immunofiliner av FKBP-typen. Når de neurotrofiske forbindelser vel er bundet til dette protein, blir de til sterke hemmere av enzymaktiviteten som er forbundet med immunofilinproteiner, og spesielt av rotamaseenzymaktiviteten, og stimulerer dermed en neuronal regenerasjon og utvekst. Et hoved-trekk er at disse forbindelsene ikke utøver noen vesentlig immunosuppressiv virkning i tillegg til sin neurotrofiske aktivitet.

Slike forbindelser har en affinitet for immunofiliner av FKBP-typen og hemmer rotamaseaktiviteten av immunofilinet.
En annen foretrukken utførelse er en neurotrofisk N-glyok-sylprolylesterforbindelse med formel:

KORT BESKRIVELSE AV TEGNINGENE
Figur 1 er et mikrofotografi av kylling-ryggrotganglia etter behandling med de angitte forskjellige konsentrasjoner av forbindelsen fra Eksempel 17. Figur 1 viser at forbindelsen fra Eksempel 17 er en sterk fremmer av neurittutvekst i sensoriske neuronalkulturer. Dyrkede kulturer som ble isolert fra kylling-ryggrotganglia på embryodag 9-10, ble behandlet med de angitte forskjellige konsentrasjoner av forbindelsen fra Eksempel 17. 48 timer senere telte man antallet neuritter som var lengre enn en DRG-kultur. Antallet neuritter som ble uttrykt i ubehandlet DRG, ble subtrahert fra neurittantallet for prøver som var blitt behandlet med forbindelsen fra Eksempel 17, for å gi den Eksempel 17-avhengige spesifikke neurittutvekst. Mikro-fotograf ier av DRG'er som er behandlet med forbindelsen fra Eksempel 17, samt den kvantitative doseavhengige neurittutvekst som utløses av forbindelsen fra Eksempel 17, vises. Figur 2 er en kurve som viser kvantifiseringen av neurittutveksten i kylling-ryggrotganglia som er blitt behandlet med de angitte forskjellige konsentrasjoner av forbindelsen fra Eksempel 17. Figur 2 viser at forbindelsen fra Eksempel 17 er en sterk fremmer av neurittutvekst i sensoriske neuronalkulturer. Figur 3 viser at forbindelsen fra Eksempel 1 fremmer en neuronal regenerasjon etter isjiasnervelesjoner. Isjias-nervene av 150 g tunge hannlige Sprague-Dawley-rotter ble knust på høyde med hoften. Man administrerte forbindelsen fra Eksempel 1 (30 mg/kg s.c), et ikke aktivt stoff (30 mg/kg s.c.) eller intralipid-bærerstoff én gang daglig i de følgende 21 dager. Dyrene ble avlivet, isjiasnerven ble fjernet og nervesegmenter som lå 2 mm distalt fra knuse-punktet, ble snittet opp og farget med Holmes sølvfarge (for å bestemme aksonantallet) og Luxol rask blåfarge (for å bestemme remyelinasjonen). Mikrofotografiene viser isjiasnervesnitt av liksom-opererte rotter, bærerstoff-behandlede lesjonerte dyr, dyr som ble behandlet med forbindelsen fra Eksempel 1, og dyr som ble behandlet med et ikke-aktivt bærerstoff, i 630 gangers forstørrelse. Eksperimentene ble utført med fire dyr pr. gruppe. Figur 4 er en kurve som viser [<3>H]-CFT-bindingen pr. ug tverrstripet membranprotein. Figur 4 viser at neuroimmuno-filinligander fjerner restitusjonen av dopaminneuroner etter MPTP-behandling av mus. CDl-mus (25 g) ble behandlet daglig med 30 mg/kg MPTP (i.p.) i 5 dager. Dyrene ble også behandlet daglig med intralipid-bærerstoff, forbindelsen fra Eksempel 1 (100 mg/kg s.c.) eller forbindelsen fra Eksempel 17 (40, 20, 10 mg/kg s.c, som angitt) samtidig med MPTP-behandlingen og i ytterligere 5 dager. Etter 18 dager ble musene avlivet, corpus striatum fra 5 dyr pr. gruppe ble samlet og bearbeidet til et vasket membran-preparat. Bindingen av [<3>H]-CFT til disse preparater av tverrstripet membran fra de forskjellige grupper ble kvantifisert for å bestemme dopamintransportørnivåer på levedyktige nerveender. Bindingen i nærvær av 10 uM umerket CFT gav en vurdering av den ikke-spesifikke binding, som ble trukket fra den samlede binding for å bestemme den spesifikke I<3>H]-CFT-binding. Verdien for bindingen ble normalisert i forhold til proteininnholdet i striatal-membranene fra hver eksperimentgruppe. Koronal- og sagit-tal-hjernesnitt fra MPTP- og legemiddelbehandlede dyr ble farget med anti-tyrosinhydroksylase (TH)-lg for å kvantifisere nivåene av TH i aksonale og nigrale bunter hos den tverrstripede midlere forhjerne, som er et tegn på funksjonelle dopaminergiske neuroner. Figur 5 er et søylediagram av [3H] -CFT som er plottet for 200 ug membranprotein. Figur 5 viser at neuroimmunofilin-ligander fremmer restitusjonen av dopaminneuroner etter MPTP-behandling av mus ifølge fremgangsmåten som ble beskrevet for Figur 4. Figur 6 er et mikrofotografi, med 630 gangers forstørrelse, av koronale og sagittale hjernesnitt. Figur 6 viser hjernesnitt fra MPTP- og legemiddelbehandlede dyr, farget med anti-tyrosinhydroksylase (TH)-lg for å kvantifisere de striatale TH-nivåer, som er et tegn på funksjonelle dopaminergiske neuroner. DETALJERT BESKRIVELSE AV OPPFINNELSEN
De angitte neurotrofiske forbindelser er relativt små molekyler i forhold til andre kjente forbindelser som bindes til immunofiliner at FKBP-typen, såsom rapamycin, FK506 og cyklosporin.
De neurotrofiske forbindelser har en affinitet for de FK506-bindende proteiner, såsom FKBP-12. Når de neurotrofiske forbindelser bindes til FKBP, har de overraskende vist seg å hemme prolylpeptidyl-cis-trans-isomeraseaktvi-teten, dvs. rotamaseaktiviteten, av det bindende protein og å stimulere neurittvekst, mens de ikke oppviser noen immunosuppressiv virkning.
Forbindelsene som er beskrevet ovenfor, foreligger som stereoisomere former, enten enantiomerer eller diastereoisomerer. Stereokjemien i stilling 1 (Formel 1) er R eller S, hvor S foretrekkes. Forbindelsene kan foreligge som enantiomerene, den racemiske form og diastereoisomere blandinger. Enantiomerer og diastereoisomerer kan adskilles ved fremgangsmåter som er kjent for fagmannen.
Det er kjent at immunofiliner, såsom FKBP, foretrukket gjenkjenner peptidsubstrater som inneholder Xaa-Pro-Yaa-mønstre, hvor Xaa og Yaa betyr lipofile aminosyreresiduer. Schreiber et al., 1990, J. Org. Chem. 55, 4984-4986; Harri-son og Stein, 1990, Biochemistry, 29, 3813-3816. Således bør modifiserte prolylpeptidomimetiske forbindelser som bærer lipofile substituenter, bindes med høy affinitet til den hydrofobe kjene av FKBP-aktiv stilling og hemme dennes rotamaseaktivitet.
Foretrukne forbindelser inneholder Rx-grupper som ikke er sterokjemisk omfangsrike i forhold til den kjente form og størrelse av den hydrofobe kjerne av FKBP-aktiv stilling. Således vil meget store og/eller høyt substituerte H^-grupper bindes til den aktive stilling av FKBP med en lavere affinitet.

Forbindelsene kan anvendes i form av salter som avledes fra uorganiske eller organiske syrer og baser. Blant slike syresalter finnes de følgende: acetat, adipat, alginat, aspartat, benzoat, benzensulfonat, bisulfatbutyrat, citrat, kamferat, kamfersulfonat, cyklopentanpropionat, diglukonat, dodekylsulfat, etansulfonat, fumarat, glukoheptanoat, glycerofosfat, hemisulfat, heptanoat, heksanoat, hydro-klorid, hydrobromid, hydrojodid, 2-hydroksyetansulfonat, laktat, maleat, metansulfonat, 2-naftalensulfonat, niko-tinat, oksalat, pamoat, pektinat, propionat, suksinat, tartrat, tiocyanat, tosylat og undekanoat. Basesalter omfatter bl.a. ammoniumsalter, alkalimetallsalter, såsom natrium- og kaliumsalter, alkalijordmetallsalter, såsom kalsium- og magnesiumsalter, salter med organiske baser, såsom dicykloheksylaminsalter, N-metyl-D-glukamin og salter med aminosyrer, såsom arginin, lysin osv. Dessuten kan de basiske nitrogenholdige grupper kvaterniseres med midler såsom lavere alkylhalogenider, såsom metyl-, etyl-, propyl-og butylklorider, -bromider og -jodider; dialkylsulfater, såsom dimetyl-, dietyl-, dibutyl- og diamylsulfater; langkjedede halogenider, såsom dekyl-, lauryl-, myristyl-og stearylklorider, -bromider og -jodider; aralkylhalo-genider, såsom benzyl- og fenetylbromider og andre. Således erholdes produkter som er oppløselige eller dispergerbare i vann eller olje.
De neurotrofiske forbindelser kan administreres periodisk til en pasient som gjennomgår behandling av en neurologisk lidelse eller i hvilken det av andre grunner er ønskelig å stimulere en neuronal regenerasjon og vekst, såsom i forskjellige perifere neuropatiske og neurologiske lidelser som vedrører neurodegenerasjon. Forbindelsene kan også administreres til andre pattedyr enn mennesker, for å behandle forskjellige neurologiske lidelser hos pattedyr.
De aktuelle forbindelsene er sterke hemmere av rotamaseaktivitet, og de har en utmerket grad av neurotrofisk aktivitet. Denne aktivitet er nyttig ved stimulering av skadede neuroner, fremmingen av neuronal regenerasjon, forebyggelse av neurodegenerasjon og ved behandling av diverse neurologiske lidelser som er kjent for å være forbundet med neuronal degenerasjon og perifere neuropatier. De neurologiske lidelser som kan behandles, omfatter, men er ikke begrenset til: trigeminusneuralgi, glossopharyngeal neuralgi, Bells paralyse, myasthenia gravis, muskeldystrofi, amyotrofisk lateralsklerose, progressiv muskelatrofi, arvelig progressiv bulbærmuskel-atrofi, herniert, sprengt eller prolaperende mellomvirvel-skive-syndrom, cervikalspondylose, pleksusforstyrrelser, brystutløps-ødeleggelsessyndromer, perifere neuropatier, såsom de som forårsakes av bly, dapson, blodmidd, pro-phyria, eller Guillain-Barrés syndrom, Alzheimers sykdom og Parkinsons sykdom.
Med dette formål kan forbindelsene administreres oralt, parenteralt, i inhalasjonsspray, topisk, rektalt, nasalt, bukkalt, vaginalt eller via en implantert beholder, i doseringsformuleringer som inneholder konvensjonelle ikke-toksiske, farmasøytisk akseptable bærerstoffer, hjelpe-stoffer og vehikler. Begrepet parenteralt som anvendt heri, omfatter subkutane, intravenøse, intramuskulære, intraperi-toneale, intratekale, intravéntrikulære, intrasternale og intrakraniale injeksjons- eller infusjonsteknikker.
For å være terapeutisk virksomme på målområder i det sentrale nervesystem, bør immunofilin/legemiddel-komplekset lett kunne gjennomtrenge blod-hjernebarrieren når det administreres perifert. Forbindelser som ikke kan gjennomtrenge blod-hjernebarrieren, kan virksomt administreres via en intraventrikulær rute.
De farmasøytiske blandinger kan foreligge i form av sterile injiserbare preparater, f.eks. i form av en steril injiserbar vandig eller oljeaktig suspensjon. Denne suspensjon kan formuleres ifølge teknikker som er kjent innen faget, under anvendelse av egnede dispergeringsmidler eller fuktemidler og suspensjonsmidler. Det sterile injiserbare preparat kan også være en steril injiserbar oppløsning eller suspensjon i et ikke-toksisk parenteralt akseptabelt tynningsmiddel eller oppløsningsmiddel, f.eks. i form av en oppløsning i 1,3-butandiol. Blant de akseptable bærerstoffer og oppløs-ningsmidler som kan anvendes, kan nevnes vann, Ringers oppløsning og en isotonisk natriumkloridoppløsning. I tillegg anvendes ofte sterile fete oljer som oppløsnings-middel eller suspensjonsmedium. Med dette formål kan hvilken som helst mild fet olje anvendes, bl.a. syntetiske mono- eller diglycerider. Fettsyrer, såsom oleinsyre og dens glyceridderivater, finner anvendelse ved fremstilling av injiserbare preparater, liksom også olivenolje eller ricinusolje, spesielt i sine polyoksyetylerte former. Disse oljeoppløsninger eller -suspensjoner kan også inneholde et langkjedet alkoholtynningsmiddel eller dispergeringsmiddel.
Forbindelsene kan administreres oralt i form av f.eks. kapsler eller tabletter, eller i form av en vandig suspensjon eller oppløsning. I tilfellet av tabletter for oral bruk, er laktose og kornstivelse blant de vanligvis anvend-te bærerstoffer. Smøremidler, såsom magnesiumstearat, tilsettes også vanligvis. For oral administrasjon i kapsel-form er f.eks. laktose eller tørket kornstivelse nyttige tynningsmidler. Når det kreves vandige suspensjoner for oral bruk, kombineres den aktive ingrediens med emulge-rings- og suspensjonsmidler. Om ønsket, kan man tilsette bestemte søtstoffer og/eller smakstoffer og/eller farge-stoffer.
Forbindelsene kan også administreres i form av supposi-torier for en rektal administrasjon av legemidlet. Disse sammensetninger kan fremstilles ved å blande legemidlet med et egnet ikke-irriterende bærerstoff som er fast ved romtemperatur men flytende ved rektaltemperaturen og som derfor vil smelte i rektum for å frigi legemidlet. Slike materialer omfatter bl.a. kakaosmør, bivoks og polyetylen-glykoler.
Forbindelsene kan også administreres topisk, spesielt når lidelsen som skal behandles, omfatter områder eller organer som er lett tilgjengelige ved topisk applikasjon, bl.a. neurologiske forstyrrelser i øyet, huden eller den lavere tarmtrakt. Egnede topiske formuleringer kan lett fremstilles for hvert av disse områder.
For en oftalmisk bruk kan forbindelsene formuleres som finpulveriserte suspensjoner i isotonisk, pH-innstilt sterilt saltvann, eller fortrinnsvis som oppløsninger i isotonisk, pH-innstilt sterilt saltvann, enten med eller uten et konserveringsmiddel, såsom benzylalkoniumklorid. Alternativt kan forbindelsene for oftalmisk bruk formuleres i en salve, såsom vaselin.
For topisk påføring på huden kan forbindelsene formuleres i en egnet salve som inneholder forbindelsen suspendert eller oppløst i f.eks. en blanding av et eller flere av de føl-gende stoffer: mineralolje, parafinolje, vaselin, propylen-glykol, polyoksyetylen/polyoksypropylen-forbindelse, emul-gerende voks og vann. Alternativt kan forbindelsene formuleres i et egnet hudvann eller en krem som inneholder den aktive forbindelse suspendert eller oppløst i f.eks. en blanding av et eller flere av de følgende stoffer: mineralolje, sorbitanmonostearat, polysorbat 60, cetylestervoks, cetearylalkohol, 2-oktyldodekanol, benzylalkohol og vann.
En topisk applikasjon til den lavere tarmtrakt kan utføres i en rektal suppositorieformulering {jfr. ovenfor) eller i en egnet klysterlavementformulering.
Doseringsmengder i størrelsesorden på ca. 0,1-10.000 mg aktiv ingrediensforbindelse er nyttige ved behandling av de ovennevnte tilstander, hvor det foretrukne område er 0,1-1.000 mg. Mengden aktiv ingrediens som kan kombineres med bærerstoffene for å gi en enkelt doseringsform, vil variere avhengig av verten som skal behandles, og den bestemte administrasjonsform.
Det er imidlertid underforstått at den bestemte dosering for en bestemt pasient vil være avhengig av tallrike faktorer, bl.a. aktiviteten av den bestemte forbindelse som skal anvendes, alderen, kroppsvekten, allmenntilstanden, kjøn-net, kostholdet, administrasjonstidspunktet, utsondrings-hastigheten, legemiddelkombinasjonen og alvoret av den bestemte sykdom som skal behandles, samt administrasjons-formen.
Forbindelsene kan administreres sammen med andre neuro-trof iske midler, såsom neurotrofisk vekstfaktor (NGF), gliaavledet vekstfaktor, hjerneavledet vekstfaktor, ciliær neurotrofisk faktor og neurotropin-3. Doseringsmengden av de andre neurotrofiske legemidler vil være avhengig av de ovennevnte faktorer samt av den neurotrofiske effekt av 1egemidde1kombinasj onen.
K, - testprosedvre
Hemmingen av peptidylpropylisomeraseaktiviteten (rotamaseaktiviteten) med forbindelsene, kan bedømmes ved kjente metoder som beskrives i litteraturen (Harding, M.W. et al., Wature 341:758-760 (1989); Holt et al., J. Am. Chem. Soc. 115:9923-9938). Disse verdier erholdes som tilsynelatende Ki-verdier, og de er oppført i Tabell I. Cis- trans-iso-meriseringen av en alanin-prolin-binding i et modellsub-strat, nemlig N-suksinyl-Ala-Ala-Pro-Phe-p-nitroanilid, overvåkes spektrofotometrisk i et chymotrypsinkoblet assay, som frigir para-nitroanilid fra trans-formen av substratet. Man bestemmer hemmingen av denne reaksjon som forårsakes av tilsetningen av forskjellige konsentrasjoner av hemmer, og dataene analyseres som en endring av første ordens hastig-hetskonstanten som en funksjon av hemmerkonsentrasjonen for å gi de tilsynelatende Ki-verdier.
Til en liten plastskål tilsettes 950 ml iskald assaybuffer (25 mM HEPES, pH 7,8, 100 mM NaCl), 10 ml FKBT (2,5 mM i 10 mM Tris-Cl pH 7,5, 100 mM NaCl, 1 mM ditiotreitol), 25 ml chymotrypsin (50 mg/ml i 1 mM HCl) og 10 ml testforbindelse i forskjellige konsentrasjoner i dimetylsulfoksyd. Reaksjonen utløses ved tilsetning av 5 ml substrat (suksinyl-Ala-Phe-Pro-Phe-para-nitroanilid, 5 mg/ml i 2,35 mM LiCl i trifluoretanol).
Absorbansen ved 390 nm over tid overvåkes i 90 sekunder ved anvendelse av et spektrofotometer, og hastighetskonstantene bestemmes utifrå dataene over absorbansen i forhold til tiden.
Dataen for disse eksperimenter er oppført i Tabell I.
I pattedyrceller danner FKBP-12 et kompleks med inositol-trifosfatreceptor (IP3R) og ryanodinreceptor (RyR). Man tror at de aktuelle neurotrofiske forbindelser løser FKBP-12 fra disse komplekser og dermed forårsaker "lekka-sjer" i kalsiumkanalen (Cameron et al., 1995). Kalsiumfluks er innblandet i neurittforlengelser, således at IP3R-recep-tor og ryanodinreceptor kan være innblandet i de neuro-trof iske virkninger av legemidlene. Fordi legemidlene bindes til den samme stilling av FKBP-12 som IP3R-receptor, kan man anta at legemidlene forskyver kanalene fra FKBP-12.
Kulturer av kyllincr- ryggrotgancrlier og neurittutvekst
Ryggrotganglier ble dissekert fra kyllingembryoer på ruge-dag 10. Hele gangliekulturer ble dyrket på tynnskikts "Matrigel"-bestrøkne 12 brønners plater med "Liebovitz L15" plus høyt glukoseholdig medium supplementert med 2 mM glutamin og 10% føtalt kalveserum som også inneholdt 10 uM cytosin-B-D-arabinofuranosid (Ara-C) ved 37°C i et miljø som inneholdt 5% C02. 24 timer senere ble DRG'ene behandlet med forskjellige konsentrasjoner av nervevekstfaktor, immunofilinligander eller kombinasjoner av NFG plus legemidler. 48 timer etter legemiddelbehandlingen ble gangliene synliggjort under fasekontrastmiddel eller Hoffman modula-sjonskontrastmiddel med et "Zeiss Axiovert" invertert mikroskop. Mikrofotografier av kulturene ble gjort, og neurittutveksten ble kvantifisert. Neuritter som var lengre enn DRG-diameteren, ble ansett for å være positive, og det samlede antall neuritter ble kvantifisert for hver eksperi-mentbetingelse. 3-4 DRG'er ble dyrket pr. brønn, og hver behandling ble utført to ganger.
Dataene for disse eksperimenter er oppført i Tabell II. Representative mikrofotografier for forbindelsen fra Eksempel 17 vises på Figur 1; en doseresponskurve for dette Eksempel vises på Figur 2.
Isi iasnerve- akBotomi
Seks uker gamle hannlige Sprague-Dawley-rotter ble bedøvet, og isjiasnerven ble frilagt og knust med pinsett på høyde med hoften. Testforbindelser eller bærerstoff ble administrert subkutant rett før lesjonen og daglig i de følgende 18 dager. Snitt av isjiasnerven ble farget med Holmes sølv-farge for å bestemme antallet aksoner og med Luxol rask blåfarge for å bestemme myelinasjonsgraden. 18 dager etter lesjonen var antallet aksoner i dyr som var behandlet med bærerstoff, signifikant nedsatt (50% senking sammenlignet med den ikke-lesjonerte kontrollgruppe), liksom også myelinasjonsgraden (90% senking sammenlignet med den ikke-lesjo-ner te kontrollgruppe).
Administrasjon av forbindelsen fra Eksempel l (30 mg/kg s.c.) rett før lesjonen og daglig i 18 dager etter lesjonen, førte til en signifikant regenerasjon av både aksonantallet (5% senking sammenlignet med den ikke-lesjonerte kontrollgruppe) og myelinasjonsgraden (50% senking sammenlignet med kontrollgruppen) sammenlignet med dyrene som ble behandlet med kun bærerstoff. Den signifikante effekt av forbindelsen fra Eksempel 1 stemmer overens med dens sterke aktivitet ved hemming av rotamaseaktiviteten og stimulasjon av neurittutveksten i kylling-DRG. Disse resultater vises på Figur 3. "Liksom" betyr kontrolldyr som ble behandlet med bærerstoff, men som ikke ble lesjonert; "bærerstoff" betyr dyr som ble lesjonert og kun ble behandlet med bærerstoff (dvs. ikke med legemiddel). Bildet for forbindelsen fra Eksempel 1 oppviser en påfallende likhet med bildet for liksom-gruppen, og dette tyder på at disse forbindelser har en sterk neuroregenerativ virkning in vivo. "Ikke aktiv" betyr en forbindelse som ikke har noen FKBP-12-hemmende virkning. Resultatene for dyr som ble behandlet med denne forbindelse, lignet på resultatene for de lesjonerte dyr som ble behandlet med bærerstoff, og dette tyder på at resultatene som ble observert for forbindelsen fra Eksempel 1, forårsakes direkte av dennes hemming av PKBP12. En kvan-tifisering for disse data vises i Tabell III.
MPTP- modell av Parkinsons sykdom i mus
MPTP-lesjonering av dopaminergiske neuroner i mus ble anvendt som dyremodell for Parkinsons sykdom. Fire uker gamle hannlige CDl-hvite mus fikk dosert 30 mg/kg MPTP i.p. i 5 dager. Forbindelsen fra Eksempel 17 (10-40 mg/kg) eller bærerstoff ble administrert s.c. sammen med MPTP i 5 dager og i ytterligere 5 dager etter opphørt MPTP-behandling. 18 dager etter MPTP-behandlingen ble dyrene avlivet, og corpus striatum ble dissekert og homogenisert. En binding av [3H] - CFT, en radioligand for dopamintransportøren, til de tverrstripede membraner ble utført for å kvantifisere nivået av dopamintransportøren (DAT) etter lesjonen og legemiddelbehandlingen. Immunofarging ble utført på sagittale og koronale hjernesnitt ved anvendelse av anti-tyrosinhydroksylase- lg for å kvantifisere overlevelsen og bedringen av dopaminergiske neuroner. I dyr som ble behandlet med MPTP og bærerstoff, observerte man et signifikant tap av funksjonelle dopaminergiske terminaler sammenlignet med de ikke-lesjonerte dyr. Lesjonerte dyr som ble behandlet med forbindelsen fra Eksempel 17, viste en nærmest kvantitativ bedring av de TH-fargede dopaminergiske neuroner.
Figurene 4 og 5 viser kvantif ikasjonen av DAT-nivåene, mens Figurene 6-8 er mikrofotografier som viser de regenerative virkninger av forbindelsen fra Eksempel 17 i denne modell. Figur 4 viser den signifikante bedring i funksjonelle dopaminergiske terminaler, ifølge assay med [<3>H]-CFT-bin-
ding, i forhold til dyr som ble behandlet med MPTP, men ikke med foreliggende forbindelser. På Figur 5 presenteres denne data i form av et søylediagram. Det vises at dyr som ble behandlet med 40 mg/kg forbindelse fra Eksempel 17 i tillegg til MPTP, oppviste en gjenvinning av [<3>H]-CFT-bindingen på over 90%. Som vist på Figurene 6-8, viser immunofarging for tyrosinhydroksylase (en markør for levedyktige dopaminergiske neuroner) i corpus striatum, nigra og den midtre forhjernebunt en klar og tydlig gjenvinning av funksjonelle neuroner i dyr som ble behandlet med forbindelsen fra Eksempel 17, sammenlignet med dyr som ble behandlet med lesjoneringsmiddel, men ikke med legemiddel (MPTP/bærerstoff).

er midlere gjennomsnittlige molekylvekter. Alle prosent-andeler er basert på vektprosentandelen av det endelige tilførselssystem eller den endelige formulering hvis intet annet er nevnt, og alle summer er lik 100 vekt%.
EKSEMPLER
De aktuelle forbindelsene kan fremstilles ved forskjellige syntesesekvenser som utnytter etablerte kjemiske omdan-nelser. Den generelle vei til foreliggende forbindelser beskrives på Skjema I. N-glyoksylprolinderivater kan fremstilles ved å omsette L-prolinmetylester med metyloksalylklorid som vist på Skjema I. De resulterende oksamater kan omsettes med forskjellige karbonnukleofiler for å erholde mellomprodukter. Disse mellomprodukter omsettes deretter med forskjellige alkoholer, amider eller beskyttede aminosyreresiduer for å erholde de aktuelle propylesterne og amidene.
EKSEMPEL 1
Syntese av 3- fenvl- l- propvl-( 2S)- 1-( 3. 3- dimetyl- l, 2- diokso-pentvl)- 2- pyrrolidinkarboksylat ( Eksempel 1)
Syntese av metyl- ( 2S)- 1- ( 1, 2- diokso- 2- metoksyetyl)- 2- pyrro-1 idinkarboksylat
En oppløsning av L-prolinmetylesterhydroklorid (3,08 g; 18,60 mmol) i tørt metylenklorid ble avkjølt til 0°C og behandlet med trietylamin (3,92 g; 38,74 mmol; 2,1 ekv.). Etter omrøring av den dannede oppslemming under en nitrogenatmosfære i 15 minutter, tilsatte man dråpevis en opp-løsning av metyloksalylklorid (3,20 g; 26,12 mmol) i metylenklorid (45 ml). Den erholdte blanding ble omrørt ved 0°C i 1,5 time. Etter filtrering for å fjerne de faste stoffer, ble den organiske fase vasket med vann, tørket over MgSO« og inndampet. Råresiduet ble renset på en kiselgelkolonne under eluering med 50% etylacetat i heksan for å gi 3,52 g (88%) av produktet i form av en rødaktig olje. Syntese av metyl-( 2S)- 1-( 1, 2- diokso- 3, 3- dimetylpentyl)- 2-pyrrolidinkarboksylat
En oppløsning av metyl-(2S)-1-(1,2-diokso-2-metoksyetyl)-2-pyrrolidinkarboksylat (2,35 g; 10,90 mmol) i 30 ml tetrahydrofuran (THF) ble avkjølt til -78°C og behandlet med 14,2 ml 1,0 M oppløsning av 1,1-dimetylpropylmagnesium-klorid i THF. Etter omrøring av den erholdte homogene blanding ved -78°C i 3 timer, ble blandingen helt i mettet ammoniumklorid (100 ml) og ekstrahert i etylacetat. Den organiske fase ble vasket med vann, tørket og inndampet, og råmaterialet som ble erholdt etter fjerning av oppløsnings-midlet, ble renset på en kiselgelkolonne under eluering med 25% etylacetat i heksan for å gi 2,10 g (75%) av oksamatet i form av en fargeløs olje. <*>H NMR (CDC13) : d 0,88 (t, 3H) ; 1,22 og 1,26 (s, 3H hver); 1,75 (dm, 2H); 1,87-2,10 (m, 3H); 2,23 (m, 1H); 3,54 (m, 2H); 3,76 (s, 3H); 4,52 (dm, 1H, J=8,4 og 3,4).

En blanding av metyl-(2S)-1-(1,2-diokso-3,3-dimetylpentyl)-2-pyrrolidinkarboksylat (2,10 g; 8,23 mmol), 1 N LiOH (15 ml) og metanol (50 ml) ble omrørt ved 0°C i 30 minutter og ved romtemperatur over natten. Blandingen ble surgjort til pH 1 med 1 N HC1, fortynnet med vann og ekstrahert i 100 ml metylenklorid. En blanding av (2S)-1-(1,2-diokso-3,3-dimetylpentyl)-2-pyrrolidinkarboksylsyre (600 mg; 2,49 mmol), 3-fenyl-l-propanol (508 mg; 3,73 mmol), dicykloheksylkarbodiimid (822 mg; 3,98 mmol), kamfersulfonsyre (190 mg; 0,8 mmol) og 4-dimetylaminopyridin (100 mg; 0,8 mmol) i metylenklorid (2 0 ml) ble omrørt over natten under en nitrogenatmosfære. De nødvendige substituerte alkoholer kan fremstilles ved et antall fremgangsmåter som er kjent for personer med kunn-skaper innen organisk syntese. Alkoholer med lengre kjeder kan fremstilles ved homologe-ring av benzyl- og høyere aldehyder. Alternativt kan disse aldehyder fremstilles ved omdannelse av de tilsvarende fenyleddiksyrer og høyere syrer, samt fenetyl- og høyere alkoholer.
Generell fremgangsmåte ved fremstilling av akrylestere, eksemplifisert for metyl - ( 3, 3, 5- trimetoksy) -trans-cinnamat : En oppløsning av 3,4,5-trimetoksybenzaldehyd {5,0 g; 25,48 mmol) og metyl{trifenylfosforanyliden)acetat (10,0 g; 29,91 mmol) i tetrahydrofuran (250 ml) ble oppvarmet under til-bakeløp over natten. Etter avkjøling ble reaksjonsblandingen fortynnet med 200 ml etylacetat og vasket med 2 x 200 ml vann, tørket og inndampet under vakuum. Det ubehandlede residuum ble kromatografert på en kiselgelkolonne under eluering med 25% etylacetat i heksan, hvilket gav 5,63 g (88%) av cinnamatet i form av et hvitt krystallinsk fast Stoff. <X>H NMR (300 MHz, CDC13) : d 3,78 (s, 3H) ; 3,85 (s, 6H); 6,32 {d, 1H, J=16); 6,72 (s, 2H); 7,59 (d, 1H, J=16).
Generell fremgangsmåte ved fremstilling av mettede alkoholer fra akrylestere, eksemplifisert for ( 3, 4, 5- trimetoksy) - fenylpropanol.
En oppløsning av metyl(3,3,5-trimetoksy)-trans-cinnamat (1,81 g; 7,17 mmol) i tetrahydrofuran (30 ml) ble tilsatt dråpevis til en oppløsning av litiumaluminiumhydrid (14 mmol) i THF (35 ml), under omrøring og under en argonatmos-fære. Etter avsluttet tilsetning ble blandingen oppvarmet til 75°C i 4 timer. Etter avkjøling ble reaksjonen stoppet ved forsiktig tilsetning av 15 ml 2N NaOH etterfulgt av 50 ml vann. Den erholdte blanding ble filtrert gjennom "Celite" for å fjerne de faste stoffer, og filterkaken ble vasket med etylacetat. De sammenslåtte organiske fraksjoner ble vasket med vann, tørket, inndampet under vakuum og renset på en kiselgelkolonne under eluering med etylacetat, hvilket gav 0,86 g (53%) av alkoholen i form av en klar olje. <l>H NMR (300 MHz, CDC13) : d 1,23 (br, 1H) ; 1,87 (m, 2H); 2,61 (t, 2H, J=7,l); 3,66 (t, 2H); 3,80 (s, 3H); 3,83 (s, 6H); 6,40 (s, 2H).
Generell fremgangsmåte ved fremstilling av trans- allylalko-holer fra akrylestere, eksemplifisert for ( 3, 4, 5- trimetoksy) fenylprop- 2-( E)- enol.
En oppløsning av metyl-(3,3,5-trimetoksy)-trans-cinnamat (1,35 g; 5,35 mmol) i toluen (25 ml) ble avkjølt til -10°C og behandlet med en oppløsning av diisobutylaluminiumhydrid i toluen (11,25 ml 1,0 M oppløsning; 11,25 mmol). Reaksjonsblandingen ble omrørt i 3 timer ved 0°C, og deretter ble reaksjonen stoppet ved tilsetning av 3 ml metanol etterfulgt av l N HC1 inntil pH-verdien var 1. Reaksjonsblandingen ble ekstrahert i etylacetat, og den organiske fase ble vasket med vann, tørket og inndampet. Rensing på en kiselgelkolonne under eluering med 25% etylacetat i heksan gav 0,96 g (80%) av en tykk olje. <X>H NMR (360 MHz, CDC13) : d 3,85 (s, 3H) ; 3,87 (s, 6H) ; 4,32 (d, 2H, J=5,6); 6,29 (dt, 1H, J=15,8 og 5,7); 6,54 (d, 1H, J=15,8); 6,61 (s, 2H).
Claims (10)
1. Anvendelse av en forbindelse av formel
eller et farmasøytisk akseptabelt salt eller hydrat derav, hvor
Rx representerer
en Ci-Cg rett eller forgrenet alkyl- eller C2-C9 rett eller forgrenet alkenylgruppe eventuelt substituert med C3-C„-cykloalkyl,
C3 - C6 - cykl oalkyl,
eller Ar^ hvor Ar i er 2-furanyl, 2-tiazolyl, 2-t ienyl, 3-1 ienyl, fenyl,
X er oksygen;
Y er oksygen; og
Z representerer
en C2-C6 rett eller forgrenet alkyl eller alkenyl, hvor nevnte alkyl eller alkenyl er substituert i en eller to posisjoner med Ar2 hvor Ar2 er fenyl som eventuelt er substituert med fra 1-3 C^C, alkoksygrupper, eller hvor to nabosubstituenter på fenylringen utgjør en metylendiok-sygruppe, eller Ar, er 2-, 3- eller 4-pyridyl, C3-Cg-cykloalkyl forbundet med en C±- C6 rett eller forgrenet alkyl eller med C2-Ce rett eller forgrenet alkenyl ved fremstilling av et medikament for å fremme neuronal vekst og regenerering, for å behandle en nevro-logisk sykdom eller for å forhindre neurodegenerering.
2. Anvendelse ifølge krav 1, hvor medikamentet er egnet for å stimulere veksten av skadede perifere nerver.
3. Anvendelse ifølge krav 1, hvor den nevrologiske sykdom er valgt fra gruppen omfattende perifere neuropatier, og nevrologiske patologier relatert til neurodegenerering.
4. Anvendelse ifølge krav 3, hvor den nevrologiske sykdom er Alzheimer's sykdom.
5. Anvendelse ifølge krav 3, hvor den nevrologiske sykdom er Parkinson's sykdom.
6. Anvendelse ifølge krav 3, hvor den nevrologiske sykdom er amyotrofisk lateral sklerose.
7. Anvendelse ifølge ethvert av kravene 1 til 6 av en tilsvarende forbindelse hvor: Ri representerer - en Cj-Cs rett eller forgrenet alkyl- eller Ca-Cs alkenylgruppe hvilken gruppe eventuelt er substituert med C3-Cs cykloalkyl, eller - Arx hvor Arx er 2-furanyl, 2-tienyl eller fenyl; og Z representerer - en C2-C6 rett eller forgrenet alkyl- eller alkenylkjede hvor alkyl- eller alkenylkjeden er substituert i en leier flere posisjoner med Ara, hvor Ar2 er fenyl som eventuelt er substituert med en til tre Cx- C3 alkoksygrupper, eller Ar2 er 2-pyridyl, 3-pyridyl eller 4-pyridyl.
10.
