Oppfinnelsen vedrører analogifremgangsmåte til fremstilling av det nye 2-/ 2 1-hydroksy-3'-(1,1-dimetylpropylamino)-propoksy/-3-fenylpropiofenon med formel I
og dets syreaddisjonssalter. Forbindelsen er i det følgende kalt diprafenon. Tysk patent nr. 2 001 431 vedrører 2-(2'-hydroksy-3'-alkylaminopropoksy)-3-fenylpropiofenoner med den generelle formel
og dets syreaddisjonssalter. Resten R i denne formel betyr n-propylgruppe (-CH2-CH2-CH3), n-butylgruppen (-CH2-CH2-CH2-CH3), 1-metyl-propylgruppe (-CH(CH3)-CH2~CH3), eller tert.-butylgruppen (-CCCH^)^).
Dette patent vedrører også en fremgangsmåte for fremstilling av disse forbindelser. Nevnte kjente forbindelser og deres salter er farmasøytika. Undersøkelser på isolerte marsvinhjerter (metode ifølge Langendorff) har vist at disse forbindelser bevirker en tydelig økning i korronarstrømmen. Videre, er n-propylamino-forbindelsen (R=n-C3H^, i det følgende kalt propafenon) den eneste av de omtalte forbindelser som i tillegg viser en antiaryt-misk effekt. I eksperimenter med dyr (hunder, katter og kaniner) viser modell arrytmi en antiarrytmisk effekt av propafenon som opptrådte etter intravenøs (1 mg/kg)
og oral (2-10 mg/kg) administrering, se H J. Hapke og

I
De kunstige arrytmier ble indusert ved følgende forholdsregler: Infusjon 'av adrenalin pluss kloroform-inhal ering eller ved infusjon av digoksin, kalsiumklorid eller aconitin eller ved okklusjon av venstre koronar-arterie. I gjennomsnitt (1-3 mg/kg) og høyere (5 mg/kg) intravenøse doser propafenon har en hypotensiv negativ inotropisk effekt og dilaterer koronar-arteriene. Med doser som bevirker en antiarrytmisk effekt er i hjertehastigheten ikke vesentlig påvirket.
For tiden benyttes propafenon i humanmedisinen som antiarrytmisk middel med lokal anestetisk guinidin-lignende og også 3-reseptor blokkerende effekt for behandling av hjertehastighetsforist<y>rrelser<,> se eksempelvis

Undersøkelser med iide andre tre 2-(2'-hydroksy-3'-alkylaminopropoksy)-g-fenylpropionfenon-forbindelser er kjent fra tysk patent nr. 2 '001 431, dvs. n-butylamino, 1-metylpropylamino og tert.-jbutylamino-f orbindelser har vist at disse forbindelser praktisk talt ikke bevirker antiarrytmisk effekt når de ladministreres gjennomsnittlig (1-3 mg/kg) intravenøst i do'ser i prøver med dyr som har arrytmi.
Det er hensikten med oppfinnelsen å tilveiebringe et nytt 2-(2'-hydroksy-3'-alkylaminopropoksy)-g-fenyl-propionfenon og dets syreaddisjonssalter som adskiller seg ved vesentlig høyere antiarrytmisk virkning, men ikke
i
vesentlig høyere toksisk effekt enn propafenon.
Oppfinnelsen vedrører analogifremgangsmåte til fremstilling av det teraputisk aktive 2-/2 *-hydroksy-3'-(1,1-dimetylpropylamino) -prop^oksy7-g-fenylpropiofenon (diprafenon) av ovennevnte formel I og dets syreaddisjonssalter, idet fremgangsmåten er karakterisert ved at en fenoleter med den generelle formel II og B er en avspaltbar gruppe, omsettes med 1,1-dimetylpropylamin med formel III ;1,1-dimetylpropylamino-forbindelsen som dannes, overføres eventuelt med en syre til et syreaddisjonssalt. Omsetningen kan eksempelvis utføres i henhold til fremgangsmåten omtalt i tysk patent 2 001 431 og tysk Offenlegungs-schrift 2 007 751. ;Fortrinnsvis er den avspaltbare gruppe B et halo-genatom, spesielt klor, brom eller jodatom. Aromatiske eller alifatiske sulfonsyregrupper er også anvendelige som p-toluen-sulfonsyregruppen, p-brombenzen-sulfonsyregruppen eller metansulfonsyregruppen. Reaksjonen utføres ved en temperatur fra 10 til 120°C, dvs. ved værelsestemperatur eller en høyere temperatur, fortrinnsvis ved en temperatur i området fra 50 til 120°C, ved atmosfærisk trykk eller i et lukket kar, ved forhøyet trykk. ;Utgangsforbindelsene med formel II og III kan omsettes uten fortynningsmidler eller oppløsningsmidler. Imidlertid utføres reaksjonen fortrinnsvis i nærvær av et inert fortynnings- eller oppløsningsmiddel som en laverealkohol med 1 til 4 karbonatomer, f.eks. metanol, etanol eller propanol, fortrinnsvis isopropanol eller etanol, ;en lavere mettet dialkyleter, dialkylglykoleter eller cyklisk eter, som dietyleter, 1,2-dimetoksyetan, tetra-hydrofuran elle dioksan, et benzen-hydrokarbon, som benzen selv eller en alkyl-benzen, spesielt toluen eller xylen, ;eller et alifatisk hydrokarbon som heksan, heptan eller oktan, dimetylsulfoksyd eller i nærvær av vann eller blandinger av overnevnte oppløsningsmiddel. | ;Når det benyttes!i overskudd kan 1,1-dimetylpropylamin (2-metyl-2-aminobutan) ogås være et egnet fortynnings-eller oppløsningsmiddel. ;Laverealkoholer, i spesielt etanol eller isopropanol, er foretrukkete oppløsningsmidler ved omsetning av forbindelsene ;med formel II hvori A betyr resten ;;med aminet med for- ;mel III, idet reaksjonen fortrinnsvis utføres ved temperaturer i området 50 til 120°C. Reaksjonen kan eventuelt utføres i et lukket kar under trykk. ! ;I nukleofil substituering av resten B, foretrekkes ;som oppløsningsmiddel et syklisk alifatisk eter, spesielt tetra-hydrofuran, eller dioksan elller dimetylsulfoksyd ved temperaturer i området fra 90 til 120°C. jReaksjonen kan eventuelt utføres i nærvær av en katalytisk mengde av natrium eller kaliumjodid. ;Fenoleteren med den generelle formel II kan even- ;tuelt også benyttes i form av en blanding av et epoksyd med et halohydrin, da ved den tekniske fremstilling av utgangsforbindelsen slike blandinger kan eventuelt, dannes. ;I en foretrukket utførelse av den nukleofile erstatning av resten B, ved aminer med formel III utføres reaksjonen i nær- ;vær av en base som syrebindende middel. Foretrukkede baser er alkalimetallhydroksyder, alkalimetallkarbonater, alkalimetallbikarbonater, alkalimetallalk<p>holater, og spesielt metylater og etylater, eller et tertiært amin, som pyridin eller trialkylamin som dimetylamin eller trietylamin. Blant alkaliforbindelsene er natrium og kalium spesielt foretrukket. Basen benyttes i støkio-metrisk mengde eller i et svakt overskudd. ;I ;Når de benyttes i,overskuddsmengde kan det anvendte ;amin for reaksjonen samtidig tjene som syrebindende middel. ;Fullstendigheten av reaksjonen avhenger av reaksjons-temperaturen og oppnås vanligvis i løpet av 2 til 15 timer. Reak-sjonsproduktet kan utvinnes på vanlig måte, eksempelvis ved filt-rering eller avdestillering av fortynningsmidlet fra reaksjonsblandingen. Den dannede forbindelse renses på vanlig måte, eksempelvis ved omkrystallisering fra et oppløsningsmiddel overføring til et syreaddisjonssalt eller ved kolonnekromatografi. ;Fenoleteren med den generelle formel II kan dannes ved alkylering av 2-hydroksy-g-fenylpropiofenon med formel IV ;;med et epihalohydrin eller en a,u)-dihalo-2-propanol. Eksempler på epihalohydriner er epiklorhydrin, epibromhydrin og epijod-hydrin. Eksempler på et, (jo-dihalo-2-propanoler er spesielt 1,3-diklor-2-propanol og 1,3-dibrom-2-propanol. ;Omsetningen av forbindelse IV for fremstilling og utgangsforbindelsene med formel II er spesielt utført ved temperaturer fra 0 til 120°C og normal trykk, eller i et lukket kar med forhøyet trykk. Egnede oppløsningsmidler eller fortynningsmidler er lavere alifatiske ketoner som aceton, metyl, etyl, ;keton eller metylisobutylketon, en lavere alkohol med 1 til 4 karbonatomer, som metanol, etanol, propanol eller butanol, en lavere alifatisk eller syklisk eter, som dietyleter, tetrahydro-furan eller dioksan, et dialkylformamid, som dimetylformamid eller dietylformamid eller dimetylsulfoksyd eller heksametylfosforsyre-triamid eller en overskuddsmengde av det alkylerende middel. ;Reaksjonen utføres fortrinnsvis i nærvær av en base, som syrebindende middel. Egnede baser er alkalimetallkarbonater, alkalimetallbikarbonater, alkalimetallhydroksyder, alkalimetall-hydrider eller alkalimetallalkoholater, spesielt av natrium eller kalium, basiske oksyder som aluminiumoksyd eller kalsiumoksyd, organiske tertiære baser, som pyridin, lavere trialkylaminer, ;som trimetylamin eller trietylamin, eller piperidin. Basen ;kan benyttes i katalytiske eller støkiometriske mengder, eller i svakt overskuddsmengder, med hensyn til det anvendte alkalylerings-middel. ;2-hydroksy-3-fenylpropiofenon omsettes fortrinnsvis med epiklorhydrin, epibromhydrin eller 1,3-dibrompropanol-2 i et polart aprotisk oppløsningsmiddel, spesielt dimetylsulfoksyd, ved ;temperaturer fra 0 til 50°C|i nærvær av minst en molekvivalent base, spesielt natriumhydrid basert på det alkylerende middel. ;Utgangsforbindelise<n> med formel IV, dvs. 2-hydroksy-3-fenylpropiofenon fremstilles etter kjent metode. ;Forbindelsen med formel I viser et chiralitetssenter på karbonatomet i 2-stilling av alifatisk- sidekjede. Vanligvis oppnås den som racemat som kan spaltes i de optiske aktive anti-poder, ved kjente metoder, f. eks. ved dannelsen av diastereo-mere salter med optisk aktive syrer, som dibensoyl-vinsyre, kam-fer-10-sulfonsyre, ditouyl-yinsyre eller 3-brom-kamfer-8-sulfon-syre. ;Forbindelsen med iformel I fremstilles ifølge oppfinnelsen overføres eventuelt i et syreaddisjonssalt, fortrinnsvis i et salt av en fysiologisk tålbar^ >syre. Vanlig ^fysiologisk tålbare uorganiske eller organiske syrer, er f. eks. hydroklor-syre, hydrobromsyre, fosforsyre, svovelsyre, oksalsyre, maleinsyre, fumarsyre, melkesyre, vinsyre, maleinsyre, sitronsyre, ;i ;salisylsyre, adipinsyre og bensosyre. Andre egnede syrer er omtalt eksempelvis i Fortschritte der Arzneimittelforschung, vol. ;10, side 224-225, Birkhauser| publishers, Basel og Stuttgart, 1966, og J. og Pharmaceutical Sciences, vol. 66, side 1-5 (1977). Hydro-klorsyre foretrekkes. ;Syreaddisjonssaltene oppnås normalt på vanlig måte ;ved å blande den fri base eller dens oppløsninger med tilsvarende syre eller dens oppløsning il et organisk oppløsningsmiddel, f. eks. en laverealkohol som metanol', etanol, n-propanol, eller isopropanol, eller et lavere keton som aceton, metyletylketon eller metyl-isobutylketon, eller en eter som dietyleter, tetrahydro-furan eller dioksan. Blandinger av nevnte oppløsningsmidler kan også benyttes for bedre krystallavsetning. Videre kan farma^ søytisk godtagbare vandige oppløsninger av syreaddisjonssalter av forbindelsen med formel I|fremstilles i en vanlig sur opp-løsning. ;Syreaddisjonssaltene av forbindelsene med formel I ;kan overføres i fri baser påiJ<i> og for seg kjent måte, som eksempelvis ved alkali eller ioneutvekslere. Videre kan salter oppnås fra den fri base ved omsetning méd uorganiske eller organiske syrer, ;spesielt med dem som er egnet for dannelse av terapeutiske anvendelige salter. Disse og andre salter av nye forbindelser som pikratene kan også benyttes til rensning av den fri base, hvori den fri base overføres til et salt som adskilles og basen frigjøres igjen fra saltet. ;Forbindelsene fremstilt ifølge oppfinnelsen benyttes ;til farmasøytiske sammensetninger for oral, rektal, intravenøs eller intramuskulær applikasjon, som i tillegg til vanlige bærere og fortynningsmidler inneholder forbindelsen med formel I eller dets syreaddisjonssalter, som aktiv bestanddel. Den nye forbindelse og dens fysiologisk godtagbare salter benyttes til behandling av hjerterytmeforstyrrelser. ;De farmasøytiske sammensetninger fremstilles på vanlig måte med de vanlige faste eller flytende bære- eller fortynningsr midler, og vanlige farmasøytiske tilsetninger i en egnet dose over-ensstemmende med den ønskede applikasjonsform. Den foretrukkede preparat er sammensetninger i dosisenhetsform for oral applikasjon. Slike farmasøytiske former er f. eks. tabletter, filmbelagte tabletter, drageer, kapsler, piller, pulvere, oppløsninger eller suspensjoner, eller depotpreparater. ;Parenterale preparater som injeksjonsoppløsninger, er selvsagt også egnet. Andre farmasøytiske former som skal nevnes er f. eks. suppositorier. ;Tilsvarende tabletter kan oppnås eksempelvis ved i blanding av aktiv middel med kjente hjelpemidler, f. eks. ved inerte fortynningsmidler, som dekstrose, sukker, sorbitol, mannitol, poly-vinylpyrolidon, sprengmiddel, som stivelse eller alginsyre, binde-midler som stivelse eller gelatin, smøremidler som magnesiumstearat eller talkum, og/eller midler for å oppnå en depoteffekt, som )karboksypolymetylen, karboksymetylcellulose, cellulose, acetatphtalat eller polyvinylacetat. Tablettene kan omfatte flere lag. ;Drageer kan fremstilles tilsvarende ved å belegge kjerner som er fremstillet analogt til tablettene med sammensetninger som vanligvis benyttes ved dragering som polyvinylpyrrolidon eller jshellak, gummi arabikum, talkum, titandioksyd eller sukker. Drage-skallene kan også bestå av flere lag, hvori kan benyttes hjelpemidler nevnt ovenfor i forbindelse med tablettene. ;Oppløsninger eller suspensjoner som omfatter det aktive stoffet ifølge oppfinnelsen kan i tillegg inneholde smaks-stoffer som sakkarin, cyklamat eller sukker, og eksempelvis aromatiske som vanilin eller appélsinekstrakt. Videre kan de inneholde suspensjonshjelpemidler som natriumkarboksymetylcellulose, eller konserveringsmidler som p-hydroksybensoat. Kapsler som inneholder det aktive stoff kan eksempelvis fremstilles ved å blande det aktive stoff med inert bærer, som laktose eller sorbitol, ;og fylles i gelatinkapsler. ;i ;Egnede suppositorier kan fremstilles ved å blande det aktive stoff med tilsvarende!bærere som nøytralt fett eller poly-etylenglykol eller deres deri<i>vater. ;På mennesker er den enkle dose ;;Under overnevnte forbindelser beskrevet av H.J. Hapke og E. Prigge, Arzneimittelforsch. (Drug Res.) nevnt ovenfor i frembragt arrytmi, viser forbindelsene med formel I, dvs. 2-£ 2'-hydroksy-3'-(1,1-dimetylpropylamino)-propoksy?-B-fenylpropiofenon (diprafenon) overraskende allerede en antiarrytmisk effekt ved 0,05 mg/kg i intravenøs applikasjon, dvs. tyvendedelen av den nødvendige dose av ref eranses'tof f et propafenon. ;Denne overraskende høye antiarrytmiske effekt er ikke fullt en tilsvarende stor økning av toksisitet. Dette kan sees fra en sammenligning av LD^g-<y>erdiene hos rotte og hund som er oppstillet i tabell I ;;Herav kan det sees at den terapeutiske kvotient av diprafenon økes fra 10 til 20-ganger sammenlignet til propafenon. ;Den antiarrytmiske effekt av forbindelsen med formel I ble undersøkt på hunder ifølge metoden av Hapke og Prigge, Arzneimittelforsch. (Drug Res.), sitert ovenfor. En frembragt arrytmi var vesentlig karakterisert ved en ventrikulær ekstra- ;i ;i ;systoler ved abs. arrytmi ble dannet ved infusjon av kloroform og adrenalin (spinefrin). Ved starting av disse rytmeforstyrrelser ble forbindelsene ifølge oppfinnelsen injisert intra-venøst som vandig oppløsning av hydrokloridet. ;En intravenøs dose så liten som 0,05 mg/kg var helt effektiv i løpet av 45 sekunder, alle hunder behandlet med den, dvs. sinus-rytmen i elektrokardiogrammet (ECG) ble normalt. ;På hunder kunne den gode antiarrytmiske effekt også iakttas etter oral administrering, Rytmeforstyrrelser ble dannet ved okklusjon av en gren av korronarkaret. Forbindelsen ifølge oppfinnelsen ble administrert intragastrinalt i form av hydrokloridet neste dag. Ekstrasystolene ble iakttatt uendret i 10 minutter etter applikasjon av 10 mg/kg. 30 minutter etter applikasjon kunne ingen ekstrasystoler iakttas. ECG forble normalt i 24 timer hvorunder de ble observert. ;Tabell II viser ytterligere resultater av farmakologiske prøver på hunder, med forbindelser ifølge oppfinnelsen i hydro-kloridform og med propafenon-HCl. For prøvemetoden se H.J. Hapke og E. Prigge, Arzneimittelf. (Drug Res.), sitert ovenfor. ;Alle oppførte prøveresultater viser at forbindelsen med formel I (diprafenon) kan antas å være også nyttige i humanmedisinen, som antiarrymikum, som propafenon, imidlertid i vesentlig lavere doser og følgelig med lavere toksisitet. ;Det er overraskende og kunne ikke ventes at når det ;ble benyttet de sammenlignbare doser, har forbindelsene ifølge oppfinnelsen ikke noen virkning på den perifere og koronare sirkulasjon, på tross av dens likhet med de kjente 2-(2'-hydroksy-3<1->alkylaminopropoksy)-3-fenylpropiofenon-forbindelser, og viser en omtrent 20 ganger større virkning ved hjerterytmeforstyrrelser enn propafenon på tross av omtrent samme hemodynamiske aktivitet. Forbindelsen med formel I kan derfor benyttes i vesentlig lavere doser enn propafenon. Som et resultat er bivirkningene ved behandling av hjerterytmeforstyrrelser vesentlig redusert. ;Videre er det å bemerke at i motsetning til propafenon frembringer forbindelsene ifølge oppfinnelsen allerede en reduksjon av hjerteslaghastigheten ved doser på 0,5 mg/kg i.v., hvilket er ønskelig i tachykardiske hjerterytmeforstyrrelser. ;Oppfinnelsen skal forklares nærmere ved hjelp av noen eksempler. ;Eksempel 1 ;a> Fremstilling av 2-(21,3 *-epoksypropoksy)-3-fenyl-propiof enon.
22,6 g (0,1 mol) 2-hydroksy-(3-f enylpropiof enon opp-løses i 15 0 ml l-klor-2,3-epoksypropan. Etter tilsetning av 12 g kaliumkarbonat oppvarmes reaksjonsblandingen under omrøring og tilbakeløp inntil reaksjonen har vist seg fullstendig ved høytrykksvæskekromatografi. Reaksjonsblandingen avkjøles deretter. Kaliumklorid som dannes filtreres. Filtratet kondenseres med redusert trykk, mens overskytende l-klor-2,3-epoksypropan derved avdestilleres fra det resulterende 2-(2<1>,3'-epoksypropoksy)-3-fenylpropiofenon. Råproduktet som oppnås er en gulaktig olje som stivner ved værelsestemperatur når den har stått noen tid. Utbytte 28 g, 98%. Produktet behøver ikke å renses for neste trinn. Det rene stoff oppnådd ved krystallisering fra en 4-gangers mengde metanol har et sm.p. på 56°C.
b) Fremstilling av 2-/~2'-hydroksy-3'-(1,1-dimetylpropyl-amiho)- propbksy7- 3~ fenylpropiofenon- hydroklorid ( diprafenon- HCl)
28 g (0,1 mol) av forbindelsen oppnådd ifølge eksempel
I
la) oppløses i 100 ml metanol og behandles med 26 g 1,1-dimetylpropylamin (2-metyl-2-aminobutan). Blandingen oppvarmes under tilbakeløp i 4 timer under omrøring. Reaksjonsblandingen for-dampes deretter under nedsatjt trykk. Det dannede residuet opp-løses i 100 ml isopropanol under oppvarming, og innstilles til pH 1 med kons. 36 % hydrokloirsyre. Blandingen hensettes ved værelsestemperatur mens hydroklorid utfelles i form av krystaller. Det dannede råprodukt (ca. 36 g) omkrystalliseres fra det samme mengde isopropanol. Hydrokloridet oppnås i et utbytte på omtrent 33,0 g (81,3 %), i form av et hvitt krystallinsk stoff med et sm.p. på 130 til 131 o C. Ana'l<y>severdie<n> av det rene stoff til-svarer de teoretiske verdier.;
Eksempel 2 (
2- (2 1 -hydroksy-3 ' i-brompropoksy) -3-f enylpropiof enon omsettes med en ekvivalent mengde 2-metyl-2-aminobutan i nærvær av dimetylformamid som oppløsningsmidler og natriumkarbonat som syre-l
akseptor under tilbakeløp i flere timer. Diprafenon-hydroklorid med et smeltepunkt på 130°C oppnås etter at reaksjonsblandingen er opparbeidet ifølge eksempel lb.

Eksempel 2 gjentas med 2-(2<1->hydroksy-3<1->kloropropoksy)-3-fenylpropiofenon, idet det dannes diprafenon-hydroklorid med sm.p. 130-131°C.

- Analogifremgangsmåte til fremstilling av terapeutisk aktivt 2-^2'-hydroksy-3'-(1,1-dimetyl-propylamino)-propoksy/-3- fenylpropionfenon med formel I og dens syreaddisjonssalter,karakterisert ved at en fenoleter med den generelle formel II hvori A betyr gruppenellerog B er en avspaltbar gruppe omsettes med 1,1-dimetylpropylamin med formel III og at forbindelsen med formel I som dannes eventuelt overføres med en syre til syreaddisjonssaltet.
