Abstract
본 발명은 인지 기능 장애 및 유사한 질환의 치료에 사용되는 스테로이드 구조를 갖는 많은 사포닌 및 사포게닌의 용도에 관한 것이다. 또한, 본 발명은 상기 질환의 치료 방법 및 치료를 위한 약제 조성물에 관한 것이다.
Description
본 발명은 막 결합된 수용체 및 이의 기능; 인지 기능 장애 및 유사 질환; 이의 치료 방법 및 이러한 치료에 사용하기 위한 조성물에 관한 것이다. 보다 구체적으로, 본 발명은 막 결합된 수용체의 수 또는 기능의 결핍에 의해 특징되는 질환의 치료에 관한 것이나, 이로 제한되는 것은 아니다. 하기에서, 본 발명은 주로 수용체 유형의 수적 결핍이 입증된 알츠하이머병(AD) 및 알츠하이머 유형의 노인성 치매(SDAT)에 대해 기술될 것이다. 그러나, 본 발명은 일반적으로 본래 병리적 질환 및/또는 열악한 환경 조건으로의 노출로 인한 질환으로서, 막 결합된 수용체의 수 또는 기능의 결핍 또는 뉴우런 사이의 접합점 또는 뉴우런과 이펙터 세포의 접합점에서의 전달력 결핍에 의해 특징되는 질환들을 치료하는 것에 관한 것이다.
상기 언급된 유형의 질환에는 파킨슨병, 루이 소체 치매(Lewi body dementia), 체위성 저혈압, 자폐증, 만성 피로 증후군, 중증 근무력증, 람베르트 이튼병(Lambert Eaton disease), 걸프 전쟁 증후군과 관련된 질병 및 문제, 유기인 화합물에 대한 직업적 노출 및 노화와 관련된 문제가 포함된다.
알츠하이머병(AD) 및 알츠하이머 유형 노인성 치매(SDAT)는 평균 수명의 증가 및 우발성 질환의 억제로 인해 인구 분포가 점차로 보다 노령의 인구로 점증적으로 확장되고 있기 때문에, 모든 사회에서 중요한 문제이다. AD/SDAT를 치료하거와 도움이 될 수 있는 제(agent)가 시급히 요구되고 있는 실정이다.
연령과 관련된 기억력 손상(AAMI)은 심리적 및 신체적으로는 정상이지만, 기억력 감소를 호소하는 노인 환자의 특징이다. 이것은 잘 정의되지 않은 증후군이나 AD/SDAT의 치료에 효과적인 제가 이들 환자에게도 가치가 있을 수 있다.
AD/SDAT에 대한 연구는 전통적이며 통상적인 의료 연구 방법 및 훈련에 의해 수행되고 있다. 통상적인 의학에서는 AD/SDAT의 치료에 대해 몇가지 접근 방법이 있다. 대뇌 피질의 기억력을 보조하는 생화학적 과정은 (적어도 부분적으로) 콜린에 의해 매개되는 것으로 공지되어 있다. 당해 기술자들은 "콜린에 의해 매개되는" 메카니즘이 직접적으로 수용체에 작용하는 아세틸콜린에 기인할 수 있으며, 이들은 직접적인 효과임을 알고 있을 것이다. 다르게, 임상적으로 유용한 효과가 또한 시냅스전 신경 말단으로부터의 아세틸콜린의 방출 조절 또는 아세틸콜린을 파괴하는 효소의 억제에 의해 유도될 수 있다. 이러한 조절 인자는 매개제가 비콜린성인 뉴우런을 통해 효력을 발휘할 수 있으며, 이들은 간접 효과로서 언급된다. 치료에 있어서 몇몇 시도는 중뇌 핵과 같은 뇌의 다른 영역에서 매개제인, 5-히드록시트립타민과 같은 다른 매개제의 역할에 초점을 맞추어 왔다. 그러나, 이러한 영역으로부터의 섬유소는 주요 전달제가 아세틸콜린인 대뇌 피질을 향해 돌출되어 있기 때문에, 적합한 치료제에 대한 연구는 이러한 매개제의 처리에 집중되었다.
AD/SDAT의 치료를 위한 콜린 방법은 방출된 아세틸콜린의 형성, 시냅스 방출 및 제거의 경로에 따라 몇몇 일면과 관련된다.
한 방법은 높은 투여량의 렉시틴과 아세틸콜린의 다른 전구체로 치료하는 것이 포함된다. 이는 인지 능력에서의 지속적인 향상에 제한적으로 사용된다.
또 다른 방법은 뇌에서의 신경 성장 인자(NGF) 분비 및 콜린-아세틸콜린 트랜스퍼라제(CAT) 활성을 증진시키는 것으로 나타난 폴리갈라에(Polygalae) 뿌리 추출물과 같은 식물성 약제의 사용을 포함한다. NGF의 경구 투여는, 이것이 혈뇌 장벽을 통과할 수 없는 고분자량 단백질이기 때문에 중추 신경계 뉴우런에 전혀 효과가 없다. 그러나, 혈뇌 장벽을 통과할 수 있고 중추 신경계의 NGF 합성을 촉진하는 효과가 있는 제는 기억력 관련 작용의 개선을 위해 제안되었다.
탁크린 히드로클로라이드와 같은 콜린스테라제 억제제를 사용하는 제 3의 임상 방법의 결과는 상기의 방법보다는 약간 더 긍정적이었다. 예를 들어, 후퍼진(huperzine), 갈란타민(galanthamine) 및 피소스티그민(physostigmine)과 같은 한방 및 양방에 사용되는 식물로부터 얻은 물질은 모두 어느 정도 임상 연구 및 실험적 모델에서 AD/SDAT의 치료에 제한적이긴 하나 어느 정도의 효과가 있는 것으로 나타났다. 상기 물질은 모두 아세틸콜린 에스테라제(AChE)의 억제제이다. AD/SDAT가 있는 환자들에게는 아세틸콜린(ACh) 합성의 감소, 시냅스전 저장물로부터의 ACh 방출 효율 감소 및 시냅스후(M1) 수용체의 수 또는 기능에서의 감소가 있을 수 있다. 시냅스전 M2 수용체의 감소도 또한 나타났다. AChE 억제제의 유리한 효과는 방출된 전달제의 파괴를 서서히 낮추므로써 뇌의 시냅스에서 아세틸콜린의 수준을 증진시키는 데 기인한다.
콜린 기능을 조절하는 조성물은 기억력 및 회상력에 영향을 미치는 것으로 공지되어 있다. 예를 들어, 니코틴은 니코틴 아세틸콜린 수용체를 자극하며, 흡연의 일시적인 기억력 증진 효과는 니코틴의 효과에 기인하는 것으로 여겨진다. 아세틸콜린의 길항제인 스코폴라민은 단순 반응 횟수의 연장으로서, 가능하게는 손상된 주의력의 결과로서, 정신 운동 시험으로 입증되는 건망증 및 손상된 인지 기능 장애를 일으킬 것이며, 보조적인 진통 치료제로서 상기 목적에 사용된다. 스코폴라민의 건망증 효과는 니코틴에 의해 길항될 수 있다.

AD/SDAT와 같은 질병에는 콜린성 신경 기능의 일반적인 뉴우런 손실 및 결함이 있음이 공지되어 있다. 남아있는 콜린성 뉴우런에서 높은 친화도의 니코틴 결합 부위가 이러한 질병의 치료시에 낮은 친화도의 결합 부위로 전환되므로써 전달제 방출을 지속시킬 수 있는 것으로 추측되었다. 니코틴 결합 부위의 높은 친화도를 낮추므로써, 빠른 탈감 과정이 피해진다.
뇌내의 니코틴 수용체에서의 효능제 활성화는 신속한 개시(onset) 및 상쇄(offset)를 갖는다. 니코틴 수용체의 감소된 친화성은 탈감 과정을 감소시킬 것이다. 슈바르츠 알.디.(Schwarz R.D.)등은 문헌[J. Neuro Chem 42, (1984), 1495-8]에서 니코틴 결합 부위가 콜린성(및 또한 5-히드록시트립타민성 및 카테콜라민성) 액손(axon) 말단상에서 시냅스전에 위치함을 밝혀냈다. AD/SDAT상에서의 고친화성 결합 부위의 변화는 니코틴 결합 부위가 다른 전달물질 시스템에 대해 가질 수 있는 조절 효과의 변화를 또한 유도시킬 수 있다.
시냅스전 콜린성 메카니즘은 GABA성 뉴우런에 의해 억제 조절을 또한 받으며, 이러한 억제는 AD/SDAT에서 강화되는 것으로 생각된다. 이러한 억제가 제거되거나 감소되면 시냅스전 피질 콜린성 활성이 강화되어, 인지 과정이 향상된다.
니코틴에 의해 신경지배된 뉴우런 사이의 섬유의 상호작용(결합 친화성을 감소시킴), 및 GABA성 섬유의 일시적 작동중단 둘 모두는 시냅스전 위치를 갖는다.
이것은 중추신경 전달의 극히 단순한 모델이지만, 중추신경 시냅스내의 아세틸콜린의 유효 농도를 증가시키려고 해왔던 실험을 이해하기 위한 구성을 제공한다. 이것은 직접 작용과 간접 작용의 개념을 추가로 예시한다. 상기 언급된 AD/SDAT 치료에 관한 종래의 세 가지 치료법, 즉, ACh 전구물질 보충법, 효능제 치환법 및 아세틸콜린 에스테라제 억제법은 단점을 수반한다. 이러한 치료는 시냅스후 수용체를 탈감시키는 피드백 메카니즘을 활성화시킬 수 있는 ACh의 유용성의 단기적 증가를 일으킬 수 있다. 이론적 이유로, 장기적 이점은 기대되지 않으며, 치료가 중단되는 경우, AD/SDAT 및 AAMI의 처리에서의 어떠한 이점도 사라져서 질환이 더욱 악화될 수 있다.
M1 효능제 및 M2/M3 길항제 활성을 갖는 화합물은 SDAT 환자에게서 인지 능력을 개선시키는 것으로 밝혀졌다 [참조: Sramak et al, Life Sciences vol. 2, No. 3, 195-202, 1997]. 그러나, 이러한 화합물은 피로, 설사 및 욕지기와 같은 부적절한 부작용을 일으킨다.
AD/SDAT 및 AAMI에 대한 보다 근본적인 방법은 뇌내에서 시냅스후(M1) 수용체의 수를 증가시키는 것이다. 사르사사포게닌(sarsasapogenin)(SaG)이 M1 콜린성 수용체를 상향 조절하고, 또한 β-아드레날린성 수용체를 하향 조절(즉, 정상 수준쪽으로 이동시킴)할 수 있으며, 이의 갯수는 AD/SDAT에서 병리학적으로 상승될 수 있는 것으로 중국 특허 제 CN1096031A로부터 공지되어 있다.
본 발명자들은 수용체를 조절할 수 있는 능력을 나타내는 다수의 사포닌과 사포게닌을 밝혀냈다. 따라서, 본 발명의 한 가지 일면에 따르면, 시냅스후 막 결합된 수용체의 수 또는 기능의 결핍을 특징으로 하는 질환을 치료하기 위한 약제를 제조하는데 사용되는 하나 이상의 스밀라게닌(smilagenin), 안주로게닌(anzurogenin) D 또는 아스트라갈로사이드(astragaloside)의 용도가 제공된다.
당업자는 사포닌과 이들의 사포게닌 사이의 관계를 인지할 것이며, 사포게닌의 원하는 효과가 상응하는 사포닌 또는 이들의 혼합물을 투여함으로써 환자에게서 나타날 수 있음을 인지할 것이다. 사포닌의 일부 이상의 가수분해가 위장관에서 일어난다. 당업자는 산 가수분해의 조건하에서 특정한 사포게닌의 에피머화를 또한 인지할 것이다.
모든 사포닌 및/또는 이들의 아글리콘(aglycones)이 AD/SDAT에 대해 유용한 치료제는 아니지만, 이들 중 몇몇, 예를 들어 디기탈리스(Digitalis)로부터의 사포닌과 사포게닌은 심근에 대해 강력한 근수축 작용을 갖는다. 사포닌의 이러한 군은 AD/SDAT에서의 치료적 용도를 나타내는 중추신경계(CNS)에 효과를 갖는 것으로 보이지 않으나; 고용량에서의 이들의 효능 및 독성은 또한 이로부터 배제된다.

다양한 유형의 사포게닌의 약리학적 특성 및 약동학적 작용의 변화는 AD/SDAT의 치료에 가장 유용한 이들 제제의 선택의 필요성을 강조한다. SaG의 작용에 대한 신규한 사실의 발견은 AD/SDAT 등의 치료에 가장 유용한 물질을 결정할 수 있게 하였다.
본 발명의 특정한 일면에서 가장 중요한 사포닌 및 사포게닌은 일정한 범위의 식물 종, 특히, 스밀락스(Smilax), 아스파라거스, 아네마레나(Anemarrhena), 유카(Yucca) 및 아가브(Agave) 속으로부터 자연 발생한다. 현재 가장 중요한 종으로는 스밀락스 레젤리 킬립 & 모르톤(Smilax regelii Kilip & Morton) (온두란 사르사파릴라(Honduran sarsaparilla)로서 일반적으로 공지되어 있음); 스밀락스 아리스톨로키애폴리아 밀러(Smilax aristolochiaefolia Miller) (멕시칸 사르사파릴라(Mexican sarsaparilla)로서 일반적으로 공지되어 있음); 스밀락스 오르나타 후커(Smilax ornata Hooker) (자마이칸 사르사파릴라(Jamaican sarsaparilla)로서 일반적으로 공지되어 있음); 스밀락스 아스페라(Smilax aspera) (스페니쉬 사르사파릴라(Spanish sarsaparilla)로서 일반적으로 공지되어 있음); 스밀락스 글라브라 록스버로우(Smilax glabra Roxburgh); 스밀락스 페브리푸가-쿤트(Smilax febrifuga-Kunth) (에쿠아도리안(Ecuadorian) 또는 페루비안(Peruvian) 사르사파릴라로서 일반적으로 공지되어 있음); 아네마레나 아스포델로이즈 번지(Anemarrhena asphodeloides Bunge); 유카 스키디제라 로에즐 엑스 오르트기스(Yucca schidigera Roezl ex Ortgies); 및 유카 브레비폴리아 엔젤름(Yucca brevifolia Engelm)이 있다. 중요할 수 있는 사포닌 및 사포게닌은 다른 속, 예를 들어 디오스코레아(Dioscorea), 트릴륨(Trillium), 솔라눔(Solanum), 스트로판투스(Stophanthus), 디기탈리스 및 트리고넬라(Trigonella)에서 또한 자연 발생한다. 상기 나타난 바와 같이, 이들 공급원으로부터의 몇몇 사포닌 및 사포게닌은 바람직하지 못한 특성을 가지므로, 본 발명에서 사용이 권장되지 않는다.

본 발명의 범위내에 상기한 조성물의 용도가 포함되는 것으로 이해된다. 이와 같이, 본 발명의 제 5면에 따르면, 본 발명은 인간 또는 동물에 본 발명의 조성물의 유효량을 투여하는 것을 포함하여, 인지 기능을 강화시키기 위한 방법을 제공한다.
본 발명은 또한 인간 또는 인간을 제외한 동물에서 인지 기능을 강화시키는 방법으로서, 유효량의 사포닌 또는 사포게닌, 바람직하게는 비에스트로겐성 사포닌 또는 사포게닌을 투여하는 것을 포함하는 방법을 제공한다.

본 발명자들은 또한 사르사사포게닌이 특정의 다른 사포게닌과 결합된 경우에, 예기치 않은 시너지 효과가 얻어짐을 발견했다.
이와 같이, 본 발명의 제 8면에 따르면, 시냅스후 막 결합된 수용체의 수 또는 기능의 결함을 특징으로 하는 질환의 치료용 조성물로서, 두개 이상의 사르사사포게닌, 스밀라게닌, 프라제리게닌, 아스트라갈로사이드, 티고게닌, 루스코게닌, 헤코게닌, 및 디오스게닌을 포함하는 조성물이 제공된다.
본 발명의 제 7면 및 제 8면에서 사용된 물질은 환자 체내에서 높은 에스트로겐 및/또는 안드로겐 및/또는 동화작용성 활성을 갖지 않는다. 그럼에도 불구하고, 몇몇 구체예에서, 낮은 수준의 에스트로겐 및/또는 안드로겐이 부가된다.
본 발명의 제 9면에 따르면, 조직, 기관, 세포형 또는 세포 기관내 막 결합된 수용체의 수 또는 기능의 결함을 특징으로 하는 질환을 치료하는 방법으로서,

놀랍게도, 본 발명자들은 방사 표지된 SaG가 뇌 세포의 핵내에 축적됨과, M 수용체 mRNA의 수준이 SaG 처리된 쥐에서 증가했음을 발견하였다. 본 발명자들은 어떠한 이론에 결부되는 것을 원하지 않으나, SaG가 DNA 발현을 조절함으로써 중국 특허 제 1096031A호에 기술된 효과를 나타내는 것으로 여겨진다.
본 발명에 따른 하나의 가능한 설명은 SaG가 스테로이드계 수용체, 가능하게는 에스트로겐 수용체의 세포내 효능제이거나, 전사 인자 또는 프로모터라는 것이다. 스테로이드와 SaG는 그 구조가 화학적으로 유사하고, 세포질로부터 핵으로의 SaG의 전달 메카니즘이 스테로이드의 경우와 동일하다. 이와 같이, 세포막을 거쳐 확산된 후에, SaG는 세포질에 존재하는 스테로이드 수용체와 결합하고, 수용체의 형태의 변화를 촉진시켜 높은 친화성의 핵 복합체가 핵 DNA 단백질 복합체상의 반응 부위로 전달된다. 여기서, 핵으로부터 리보좀으로 이동하는 mRNA의 전사를 증진시켜 그 결과 무스카린 수용체의 생성을 증가시킨다.

양자중 어느 것이든지, DNA와 SaG-수용체 복합체의 결합이 콜린성 수용체, 도파민성 수용체, 또는 아드레날린성 수용체 또는 다른 막 결합된 수용체를 코딩하는 mRNA의 발현을 증가시킬 수 있다.
또한, DNA와 SaG 수용체 복합체의 결합은, 결합 단백질 예를 들어, G 단백질의 생성을 증가시키거나; 이들의 퇴화를 저해하거나; 이러한 단백질와 관련된 수용체와의 결합을 늦추어, 수용체 수의 2차 변화를 일으킬 수 있다.
SaG의 효과는 하나 이상의 신경 영양 인자, 예를 들어, 신경 성장 인자(NGF)의 수준의 증가를 통해 조절될 수 있다.
또한, 신경성 및 콜린성 매개 시냅스 메카니즘 이외에, 산화질소(NO), 및 비콜린성 효능제가 콜린성 전달에 대한 조절 효과를 가질 수 있다.
효과를 발휘하도록 SaG가 결합하는 세포 성분의 정확한 특성이 무엇이든지, 이는 AD/SDAT, AAMI 등에 대한 강력한 처치가 표적화될 수 있는 신규한 경로를 제공할 수 있다.
SaG는 막 결합된 수용체 mRNA, 특히 m1 수용체 mRNA의 수준을 증가시키는 것으로 밝혀졌다. 따라서, 활성화되는 경우에, 사이토솔 또는 핵 수용체 또는 프로모터는 막 결합된 수용체를 코딩하는 조직, 기관, 세포 유형, 또는 세포 소기관내에서 mRNA 분자의 생성을 증가시키거나, 막 결합된 수용체를 코딩하는 조직, 세포 소기관, 세포 유형, 또는 세포 소기관내에서 mRNA 분자의 분해를 감소시키는 것이 가능하다.
활성화되는 경우에, 사이토솔 또는 핵 수용체는 또한 막 결합된 수용체를 코딩하는 조직, 기관, 세포 유형, 또는 세포 기관내에서 mRNA 분자의 전사를 증가시킬 수 있다.
상기한 바와 같이, 니코틴 수용체는 막 결합된 수용체의 수 및/또는 전환율을 조절할 수 있다. 따라서, 한 구체예에서 사이토솔 또는 핵 수용체의 작용은 니코틴 수용체의 적어도 부분적 효능제인 물질을 투여함에 의해 조절된다.
사이토솔 또는 핵 수용체의 작용은 이의 적어도 부분적 효능제인 물질을 투여함에 의해 조절되는 것이 바람직하다.
효능제는 사포닌 또는 사포게닌, 바람직하게는 사르사사포게닌, 스밀라게닌, 프라제리게닌, 아스트라갈로사이드, 티고게닌, 루스코게닌, 헤코게닌 및 디오스게닌중 하나 이상일 수 있다. 이러한 화합물들은 환자에서 명백히 높은 에스트로겐성 및/또는 안드로겐성 및/또는 동화작용성 활성을 갖지 않는다. 본 발명의 제 9면의 방법에서는 낮은 수준의 에스트로겐성 및/또는 안드로겐성 보충이 유리할 수 있다.
수용체는 조직, 기관, 세포 유형 또는 세포 소기관의 세포의 세포질에 존재할 수 있고, 효능제와 결합함으로써 활성화된 경우, 세포의 핵으로 이동한다. 수용체가 조직, 기관, 세포 유형 또는 세포 소기관의 세포의 핵에 존재하는 것도 가능하며, 효능제는 핵으로 확산되거나 또 다른 메카니즘에 의해 핵으로 이동된다.
본 발명의 제 1면에 따른 방법에서, 투여되는 물질이 세포질 또는 핵 수용체 자체에 직접 작용하는 것은 필수적이지 않다. 대신, 작용은 경로에서 세포질 또는 핵 수용체의 또는 프로모터 관련부의 상류 또는 하류에서 이루어질 수 있다. 따라서, 세포질 또는 핵 수용체의 작용은 막 결합된 수용체를 코딩하는 조직, 기관, 세포 유형 또는 세포 소기관내에서 mRNA 분자의 발현을 증가시키는 물질을 투여함으로써 조절될 수 있다.
SDAT에 대한 가능한 치료제로서 에스트로겐 및 기타 관련 화합물의 역할은 상당한 관심을 받아왔다. SDAT 환자에서 인지 기능에 대한 콜린에스테라제 억제제인 타크린의 효과를 조사하기 위하여 수행된 연구에서, 2차 분석은 호르몬(에스트로겐) 대체 요법(ERT)도 또한 받은 여성 환자에서 모든 개선점이 관찰되었음을 시사하였다. 역학적 자료 또한 ERT가 SDAT의 발병을 방어할 수 있음을 시사한다. 난소절제술이 인지 기능의 감소를 가져오며 이러한 효과가 에스트로겐의 투여에 의해 적어도 부분적으로 역전될 수 있음을 시사하는, 대규모 래트 연구가 있다. 본 모델에서 에스트로겐의 효과는 뇌의 특정 영역, 특히 해마에서 고친화성 콜린 흡수를 증가시킴으로써 콜린성 전달을 개선시키는 것일 수 있다. 동일한 모델에서, 에스트로겐의 투여는 적당한 원위치 하이브리드화 기술을 사용한 뇌 유도 신경 영양 인자(BDNF)에 대한 mRNA의 수준을 증가시키는 것으로 나타났다(Singh 1995).
에스트로겐 효과에 배후하는 가능한 메카니즘은 시험관내 실험에서 조사되었다. 이 연구들은 신경아세포종 세포주 및 상기 세포의 혈청 결핍에 대한 반응 또는 베타 아밀로이드(BA) 분획의 효과를 사용하여 수행되었다. 이 후자의 자극은 SDAT의 후기 단계에서 아밀로이드반의 융기 때문에 특정한 관련이 있는 것으로 생각된다. 혈청 결핍 및 BA 양자 모두는 세포 괴사를 유도한다. 17-β에스트라디올은 혈청 결핍 및 BA에 의해 유도된 세포 괴사를 방어하는 것으로 나타났다. 상기 방어 효과는 17-β에스트라디올이 에스트로겐 길항제인 타목시펜의 존재하에서 시험된 경우에 없어지지 않았다. 비에스트로겐성 에난티오머인 17-α에스트라디올은 세포 괴사를 저해하는데 있어서 효과적이었다. 후속 연구는 이러한 화합물의 방어 효과가 3 위치에서 완전히 탈포화된 페놀성 A-고리 및 차단되지 않는 히드록시기의 존재에 의존한다는 것을 시사하였다[참조: Simpkins 1997; Green 1997]. 신경아세포종 배양에서, 에스트로겐 화합물은 신경 성장 인자의 유리를 증가시키는 것으로 나타났다. 이러한 발견과 SDAT에서 에스트로겐의 효과와의 관련성은 불명확한 채로 남아있다.
SDAT를 포함한 질환의 치료에 있어서 스피로스탄, 푸로-스피로스탄, 스피로솔란 또는 솔라니딘 구조를 갖는 많은 스테로이드 사포게닌의 유용성을 청구하는 특허 출원이 공개되어 있다. 2개의 특허 공보가 본원과 특히 관련이 있다: 중국 특허 공개 제 CN 1096031A호는 SDAT의 치료에 있어서 스피로스탄 사포게닌인 사르사사포게닌의 용도를 청구한다. 그러나, 이 문헌에 개시된 내용은 간단하다. 관련된 다른 문헌인 특허 공개 제 DE 4303214A1호에는 발명자들이 바이러스 기원이라고 간주한 모든 범위의 질환의 치료에서 매우 광범위한 사포닌 및 사포게닌의 용도가 청구되어 있다. 그러나, 상기 문헌은 불충분한 시냅스 전달을 특징으로 하는 매우 많은 질환에 감염적인 요소가 없는 것으로 널리 인지되어 있으며, 이로써 주장된 발명의 기본 전제에 흠결이 있다는 점에서 그 가치가 의심스럽다. 또한, 이들 문헌에는 당업자가 청구된 다수의 화합물로부터 바람직한 화합물을 선택할 수 있게 하는 어떠한 종류의 데이터도 제시하지 못한다.
SDAT 및 수용체 수 또는 시냅스 전달의 감소를 특징으로 하는 기타 질환의 치료에 사용되는 화합물의 동정에서, 본 발명자들은 요망되는 효과를 가지지만, 특히 남성 환자에서 부적합하기 때문에 어떠한 에스트로겐 효과도 가지지 않는 화합물을 동정할 필요에 대하여 고려하였다. 특허 출원 제 DE 4303214A1호에서 활성을 갖는 것으로 청구된 많은 화합물은 에스트로겐 활성을 나타냈으므로 부적합하였다. 이 데이터를 표 1에 요약한다.

따라서, 에스트로겐 수용체에서 활성을 갖지 않는 것으로 나타난 화합물은 다른 중요한 스테로이드 수용체에서도 불활성이었다.
상기 선택된 화합물을 또한 많은 시험관내 검정에서 그들의 활성에 대하여 시험하였다. 막 결합된 수용체 수의 증가에 있어서 가능한 활성을 측정하는데 중요한 것으로 간주되는 검정/실험은 다음과 같다:
1. 무스카린 수용체를 코딩하는 DNA 단편으로 차이니즈 햄스터 난소(CHO) 세포를 트랜스펙션시켰다. 대다수의 실험에서 사용된 세포주는 m2 수용체를 발현시키는 세포주였다.
2. 신경세포기원의 배양 세포주에서 무스카린 수용체 발현의 효과를 조사하였다,
3. 배양된 심근 세포는 신생 스프라그 돌리(Sprague Dawley) 래트로부터 얻었다. 심근 세포는 무스카린 수용체, 전형적으로 m2를 발현시킨다. 이러한 수용체의 수준은 장기간 배양에서 감소하며 수용체 수의 감소를 방지하는 데에 있어서 관심 대상의 화합물의 효과를 조사하였다.
이러한 실험의 방법 및 결과를 하기에서 기술한다.
1. CHO 세포주 실험
m2 수용체에 대한 DNA로 트랜스펙션시킨 CHO 세포에서 m2 수용체의 발현에 대한 여러 화합물의 효과를 조사하였다. 삼중수소화된 QNB 결합을 사용하고 비특이적 결합을 공제함으로써 수용체 수를 분석하였다. 화합물을 DMSO에 용해시키고 DMSO를 대조표준으로서 사용하였다. 화합물을 최종 농도의 범위에서 시험하였다. 또한 에스트로겐 수용체 매개된 메카니즘과 구별하기 위하여 화합물을 타목시펜의 존재 하에, 그리고 부재 하에 시험하였다. 결과를 하기 표 2에 요약한다.

이와 같이, 실험은 여러 종의 화합물이 시험관내에서 배양된 CHO 세포의 표면상에서 발현된 무스카린 수용체의 수를 증가시킬 수 있었음을 나타낸다. 그 효과는 타목시펜(tamoxifen)에 의해 상쇄되지 않았는데, 이는 관련된 메카니즘이 에스트로겐 수용체를 포함하지 않았음을 나타낸다. 심프킨(Simpkin) 등에 의한 문헌에 기재된 실험과는 달리, 원형 그대로의 페놀 A-고리가 필요치 않음이 밝혀졌다. 그와 동시에, 스테로이드 사포게닌인 다수의 화합물에는 활성이 없었다. 더욱이, 또 다른 실험은 β-에스트라디올이 10-5M의 농도로 투여된 경우, 수용체 발현을 증가시키는데 있어서 유사한 효과를 갖는다는 것을 나타냈다.
2. 세포 생존에 대한 화합물의 영향
그 밖의 시험관내 검정은 활성 화합물과 비활성 화합물을 구별하는데 사용되었다. 특히, 파에크로모아시토마(phaechromoacytoma) 세포주 뿐만 아니라 SKN-SN 및 SH-SY5Y 세포를 나타내는 특정의 다양한 신경모세포종 세포주가 β-아밀로이드 단편의 존재하에 또는 혈청 고갈 상태에서 배양되었다. 배양된 세포를 보호하는데 있어 상기 화합물의 유효성을 입증하기 위해 다수의 기술을 사용하였다. 이들 기술은 트리판 청색 배제법, 화학발광법 및 락테이트 디히드로게나아제 방출법을 포함하였다. 가장 관심을 끄는 관찰 결과는 β-아밀로이드에 의한 세포, 특히 PC12 세포의 인큐베이션이 방사 표지된 리간드 결합 기술을 사용하여 측정된 무스카린 수용체의 수를 감소시켰다는 것이다. 이러한 수용체의 수적인 감소는 활성 화합물에 의해 개선되었음을 입증하는 것이다.
3. 배양된 심근 세포에 대한 화합물의 영향
갓 태어난 스프라그 돌리 래트의 심실 근육으로부터 표준 기술을 사용하여 심근 세포를 단리해냈다. 세포를 시험관내에서 배양시키고, 다양한 시점에서 채취한 세포의 균질화후에 세포 표면 막 단편상에서 발현된 무스카린 수용체의 수를 삼중수소화된 QNB의 특이적 결합을 사용하여 추산하였다. 예비 실험을 통해, 발현된 수용체의 수가 배양 10일 후에 감소하는 경향이 있음이 입증되었다. 따라서, 실험은 수용체 수의 이러한 감소를 제지하는데 있어 여러 화합물의 효과를 조사하도록 설계되었다.
이들 실험 결과는 표 3에 요약되어 있다.

놀랍게도, 본 발명자들은 사포게닌이 시험관내에서 배양된 세포의 핵에 우선적으로 축적된다는 것을 발견하였다. 이러한 사실은 상기된 바와 같이 사르사사포게닌 (SaG), 및 무스카린 수용체의 수를 증가시키는 것으로 확인된 일부 다른 화합물이 공지된 스테로이드성 수용체에 결합하지 않기 때문에 놀라운 것이다. 또한, 놀라운 것은 SaG가 핵에 우선적으로 취해진다는 것인데, 그 이유는 이들 화합물의 효과가, 무스카린 수용체를 발현시키지만 수용체에 대한 DNA가 세포질내로 트랜스펙션되어 정상적인 핵 조절 메카니즘하에 있지 않게되는 시험관내 검정 시스템에서 나타날 수 있기 때문이다.
시험되고 수용체의 수준을 상향 조절하는 것으로 확인된 SaG 및 다른 화합물은 모두 주요 공지된 부류의 막 결합된 수용체의 어떠한 것에도 직접 결합하지 않는 것으로 밝혀졌다. 따라서, 관찰된 효과는 예를 들어 니코틴 수용체에서의 작용 및 그 결과로서 발생하는 무스카린 수용체의 수적 증가에 기인하지 않는 것으로 가정될 수 있다. 이러한 설명은, 특정 화합물이 말초혈 림프구상에 발현되는 베타 아드레날린성 수용체의 수를 증가시키는 것으로 본 발명자들에게 확인되었다는 것을 고려한다면, 매우 그럴 듯 하지 않은 것으로 여겨진다(배제될 수는 없지만). 따라서, 메카니즘은 막 결합된 수용체의 조절에 대하여 보다 전반적인 영향을 미치는 것으로 여겨져야 할 것이다.
여기에서는 본 발명에 청구된 활성 화합물의 효과가 G 단백질에 대한 효과에 따라 달라질 수 있으며, 수용체의 수에 대한 효과가 G 단백질에 대한 효과에 대하여 부수적이라는 것이 고려된다. 막 결합된 G 단백질 결합된 수용체가 자극되는 경우, 두 가지 기본적인 사건, 즉, 이펙터 반응 및 수용체의 내재화가 개시된다. 또 다른 수용체 리간드와 상호작용할 수 있는 세포 표면 또는 다른 막 표면상에서 다시 일정하게 있게 되는 상태로 수용체를 후속 처리하는 것은 다수의 인자에 영향을 받기 쉬운 것으로 여겨진다. 다수의 이들 인자 또는 메카니즘은 G 단백질과 연계되어 있는 것으로 보인다. m3 수용체의 활성화가 G 단백질 발현 또는 수준에 영향을 미칠 수 있다는 것은 어느 정도 입증되어 있다. 본원에 기술된 화합물의 작용이 수용체 재생, G 단백질 연계 또는 G 단백질 항상성 과정에서의 상호작용에 기인될 수 있음이 고려된다.
대안적인 가설은 이들 화합물이 뇌 유도된 성장 인자와 같은 신경 영양 인자 및/또는 신경 성장 인자의 합성 또는 방출 또는 감소된 분해 속도를 증가시킬 것이라는 것이다. 성장 인자들에 대한 이들 효과는 화합물이 시토졸 또는 핵 수용체에 영향을 미치거나 프로모터 영역에 화합물이 결합한 결과로서 성장 인자에 대한 mRNA의 생성 속도에 직접적으로 영향을 미치는 것으로 인해 기인될 수 있거나 G 단백질과 같은 또 다른 물질 인자의 생성을 증가시키는 결과로서 기인될 수 있거나, 최종적으로 이들 효과는 수용체 또는 G 단백질 프로세션(procession)에 대한 효과에 대해 이차적일 수 있다.
아밀로이드 전구 단백질(APP)의 증가된 발현 및/또는 비정상적인 처리는 알츠하이머 질환의 주된 형태학적 홀마크(hallmark)인 아밀로이드 플라크 및 뇌혈관 아밀로이드 침착물의 형성과 관련한다. 특히 관심을 끄는 것은 아밀로이드성 단편과 비아밀로이드성 단편으로의 APP의 단백질 가수분해를 조절하는 방법이다. 단백질의 β-아밀로이드 서열내에서의 효소 α-세크레타아제에 의한 APP의 분해에 의해 비아밀로이드성 C-말단 단편과 가용성 APPsα 단편이 형성된다; 후자 단편은 벤트를 통해 뇌내(intra-cerebro-ventrically: ICV)로 주입한 경우 마우스의 기억력을 증대시킬 뿐만 아니라 신경 영양 및 신경 보호 활성을 갖는 것으로 입증되었다. 이와 대조적으로, β-세크레타아제에 의한 APP의 처리는 가변성 C 말단에서 γ-세크레타아제 분해에 의해 방출되는 β-아밀로이드의 N-말단을 노출시킨다. 39-43 개의 아미노산을 함유하는 생성된 β-아밀로이드 펩티드는 신경 독성이며 뉴우런 간의 상호연결을 방해하는 플라크를 축적시키는 것으로 확인되었다.
많은 연구는 단백질-키나아제(PKC) 결합된 무스카린 M1 및 M3 수용체의 자극이 α-세크레타아제 활성을 증가시킨다는 것을 보여주었다. APPsα로의 APP의 처리의 결과로 이것의 신경보호 효과는 증가된다. 동시에, β- 및 γ-세크레타아제에 의한 처리가 감소되고 이 결과 β-아밀로이드가 감소된다. 신경 성장 인자(NGF) 및 뇌 유도된 신경 영양 인자(BDNF) 뿐만 아니라 브라디키닌 및 바소프레신과 같은 다른 전달 물질들은 APPsα로 처리된 APP의 비를 증가시키는 동일한 효과를 가질 수 있다. 티로신 키나아제 수용체(TrkA)에 상기 인자의 결합, 후속하는 인산화와 함께 포스포리파제 Cγ의 자극, 단백질 키나아제 C(PKC)의 활성 및 α-세크레타아제의 활성도의 상대적인 증가를 포함할 수 있는 NGF의 효과와 관련된 수많은 인자들이 존재할 수 있다.
그러므로 뇌에서 선택적으로 단백질 키나아제 C의 활성도를 증가시키는 어떤 처리가 알츠하이머병의 관리에 유용하다고 예상된다. 최근까지 M1 수용체에서 선택적인 효능제는 입수할 수 없었다. 비선택적 효능제는 시냅스전 M2 수용체를 자극하여 네가티브 피드백을 유발시키고 따라서 무스카린 전달을 더욱 심하게 손상시키는 것으로 예상된다. M1 수용체에서 선택적인 효능제는 현재 이용가능하며(탈사클리딘) 이 약제는 AD의 치료를 위해 연구중에 있다. 그러나, 임의의 수용체 효능제의 장기간 투여의 경우에, 관찰된 임상적인 이점이 수용체 수의 감소 또는 감수성의 감소에 의한 이점의 크기 및 수용체 특이성의 결여에 의한 부작용의 측면에서 극히 제한될 것이라는 상당한 위험이 있다. 따라서 뇌에서 무스카린 M2 수용체 수에 거의 또는 전혀 영향을 끼치지 않으면서, 무스카린 M1 수용체 수를 선택적으로 증가시키는 본 발명에 기술된 바와 같은 화합물들은 무스카린 효능제에 의해 관찰되는 문제점들이 없을 것으로 예상되며, 따라서 특이적인 유용성을 가진다. 실제로 이러한 이점들은 다음과 같은 세 부분으로 살펴볼 수 있다.
1. 시냅스 전달을 증가시키는 M1 수용체 수에 있어서의 선택적인 증가. 선택적인 효능제의 장기간 투여는 전달상에 역효과를 갖지 않는다;
2. 증가된 수용체 수에 부수적인, α-세크레타아제 활성도의 증가와 함께 PKC의 자극 증가;
2.1 β-아밀로이드의 생성 감소 및 이에 따른 플라크 형성 및 신경세포의 손실의 감소;
2.2 APPsα의 증가 및 이에 따른 단기간 및 장기간의 기억력 향상에 의해 입증된 뇌 기능 향상.
마지막으로, 전달을 조절하는 GABA 시스템의 효과는 상기에 논의되어 있다. 벤조디아제핀, 클로라이드 및 GABA 결합 부위로부터 멀리 떨어져 있는 GABA 수용체상에 스테로이드 결합 부위가 존재한다는 것은 널리 공지되어 있다. 수많은 치료용 화합물들이 이 부위에 결합하는 것으로 공지되어 있으며 의식 수준을 향상시키거나 감소시키기 위해 사용되고 있다. 이 부위에 부분적인 효능제의 장기간 투여는 전달을 향상시킬 수 있다고 추측된다.
본 발명은 다음 실시예에서 상세히 기술될 것이다.
실시예 - 동일계내에서 하이브리드화를 사용한 mRNA 수준의 조사
20개월령 순수-라인 수컷 SD 래트를 2개의 군으로 임의로 나누었다. 한 군에 래트 당 하루에 평균 3 mg의 사르사사포게닌을 매일 먹이에 혼합하여 주었다. 대조군에는 정상적인 먹이와 물을 주었다. 4개월 후에, 어린 대조군으로서 사용된 4 내지 6개월이 지난 래트와 함께, 이들의 뇌를 하이브리드화 기술 실험에 사용하였다. 각 군을 위한 다른 먹이 장치는 완전히 동일하였다.
m1 및 m2 둘 모두의 mRNA에 각각 상응하는 cDNA 사슬을 합성하였다. m1은 수용체 단백질의 3-18개의 아미노산, 즉 TGG TGC CAA GAC AGT GAT GTT GGG ACT GAC AGC AGG GGG CAC TGA GGT에 상응하며 M2는 1-16개의 아미노산 서열, 즉 ATG AAT AAC TCA ACA AAC TCC TCG AAC AAT GGC TTG GCT ATT ACC AGT에 상응한다. 이 cDNA를 표지 물질로서 a-35S-dATP(8.9 TBq/mmol)를 포함하고 있는 3' 말단 표지 시약 킷을 사용하여 표지시켰다. 이 반응이 끝난 후에, 누클레오티드 컬럼을 사용하여 정제하였다. 회분의 특정 활성도는 (16.67-33.34) x 108 MBq/㎍. a-35S-dATP로 측정되었으며, 3' 말단 표지 시약 킷 및 누클레오티드 컬럼은 듀폰트사(Du Pont Co., USA)로부터 제공되었다.
매 시간에 각 군으로부터 래트 한마리를 꺼내어 동일한 실험을 수행하였다. 이 래트들의 목을 자르고 이들의 뇌를 온전하게 옮겼다. 15㎛ 두께의 원형 슬라이스를 일정한 냉동조건에서 저온 마이크로톰[참조 : AS-600 cryo-microtome, Anglia Scientific Co, UK]을 사용하여 준비하였다. 슬라이스를 다른 부위들(각 래트에 대해 동일 위치)로부터 취하여 폴리리신이 도말된 슬라이드 위에 올려놓고, 냉풍에서 건조시키고, 5분 동안 4% 파라포름알데히드(1 x 인산 완충염(PBS), pH 7.0을 함유하는)의 용액에서 고정시키고 난 후에 PBS중에서 2회 세척하였다. 그런 다음 이들을 10분 동안 0.25% 무수 아세트산 용액(0.1M 트리에탄올아민 하이드로클로라이드, pH 8.0 및 0.9% 염화나트륨을 함유하는)중에 위치시키고, 1분 동안 70%, 80%, 95% 및 100% 에틸 알콜중에서 탈수시키고, 5분 동안 클로로포름중에서 탈지시키고, 마지막으로 1분 동안 100% 및 95% 에틸 알콜중에서 연속적으로 처리하였다.
네가티브 대조군으로서 사용된 슬라이스를 취하여 상기에 상술된 바와 같이 에틸 알콜 등에서 탈수시키되, 37℃에서 2시간 동안 100 mg/ml RNase 및 2 x SSC 용액(300 mmol/L 염화나트륨 및 45 mmol/L 구연산나트륨을 함유하는 염/구연산나트륨 용액)중에서 추가로 처리하였다.
하이브리드화에 대해, 하이브리드화용 유동성 기질을 50% 탈이온 포름아미드, 4 x SSC, 10% 덱스트란 설페이트, 250 ㎍/㎕ 효모 tRNA, 5 x 덴하르트 용액, 500 ㎍/ml 변성 프로타민 DNA, 10 mmol/L 디티오트레이톨을 포함하여 신선하게 혼합하였다. 35S로 표지된 올리고누클레오티드 프로브[(16.67-33.34) x 10 MBq/50㎕]를 마지막으로 첨가하고 고르게 혼합하였다. 이 기질 50㎕를 각 슬라이스위에 적하하고 규산염 커버 글래스를 위에 가볍게 덮어 에어록이 없도록 하였다. 그런 다음, 습도를 유지시키기 위해 바닥에 2 x SSC를 함유한 하이브리드화 박스에 이 슬라이스를 얹고, 18 내지 24시간 동안 37℃에서 배양시켰다.
하이브리드화 후에, 이 슬라이드를 1 x SSC 용액중에 침지시키고 가볍게 흔들어 커버 글래스를 헹구었다. 이들을 1 x SSC 용액중에서 가볍게 세척하고 난 후에 4회 용액을 바꾸면서 20분 동안 37℃에서 50% 포름아미드를 함유한 2 x SSC중에서 부드럽게 흔든 다음 1 x SSC 용액으로 옮기고 30분 동안 실온에서 흔들었다(2회 반복). 마지막으로, 이 슬라이드를 2차 증류수를 사용하여 세척하고, 70% 에틸 알콜을 사용하여 탈수시킨 다음, 95% 에틸 알콜을 사용하여 탈수시키고 공기중에서 건조시켰다.
암실에서 자동방사선사진을 준비하고, 표본과 하이퍼필름 베타 맥스를 접촉 방법을 사용하여 서로 밀착시키고 건조제와 함께 카세트안에 넣고, 2내지 3주 동안 4℃에서 노출시켰다. 이들을 현상(D196)하고 고정(F5)시켰다. 마지막으로, 이 자동방사선사진을 컴퓨터를 이용한 이미지 분석기(VIDAS imaging analyser, Kontron, Germany)를 사용하여 분석하였다.
m2 프로브는 어떤 활성 부위를 나타내지 않았다. m1 프로브는 톱니모양의 핵, 대뇌 피질 및 주름에서의 활성을 보여주었다. 다른 동물군들에 대한 이들 세 부위의 비교를 표 4에 제시하였다:

어린 래트 대조군에 비해 어른 래트의 주름에서 m1에 대한 mRNA 발현이 현저하게 감소하였다. SaG의 투여는 처리된 동물을 어른 래트, 처리되지 않은 대조군과 비교한 경우, 동일한 뇌 부위에서 m1 수용체 mRNA를 현저하게 증가시켰다.
