백혈구는 감염 또는 염증의 제한부위에 대량으로 축적되기 때문에, 백혈구를 혈액으로부터 꺼내고, 이들을 적당한 지시제, 통상적으로 In-111로 표지시킨 다음 혈액에 재공급함으로써 이러한 부위를 검출할 수 있음은 이미 인식된 사실이다. 표지된 백혈구가 체내에 재분배되기에 충분한 시간이 지난 뒤, 피검자에게 적합한 장치를 주사(scanning)하여 표지된 백혈구의 위치를 검출할 수 있다. 상기 기술된 방법이 유효하긴 하지만, 이러한 방법은 백혈구를 분리하고, 표지시키며, 특히 재주입 후 이를 체내에 재분배시키는데 걸리는 시간이 있기 때문에 상당한 시간이 소요된다.
본 명세서내에 참조문헌으로 삽입된 미합중국 특허 제4,634,586호(구드윈(Goodwin) 등)에는, 환자에게 항-백혈구 단일특이적 항체 및 감마 방출 방사활성금속 킬레이트제의 면역반응성 백혈구비사멸(nonleukocidal) 결합체를 주입하고, 이 결합체가 백혈구상에 위치하기를 기다린 다음, 환자에게 결합체에 대한 항체를 주입하여 국부화되지 않은 표적대상 결합체의 혈액을 청정하게 하며, 신틸레이션 주사에 의해 백혈구를 가시화함으로써, 백혈구를 방사선 면역 영상화하는 것이 기술되어 있다.

발명의 목적
본 발명의 한 목적은 표적 대상물에 대한 표적물의 비가 증가된 염증성 또는 감염성 병변을 표적화하기 위한, 2가지 이상의 상이한 백혈구 세포 유형과 선택적으로 결합하는, 하나 이상의 진단제 또는 치료제와 결합된 다중특이적 항체를 제공하는 것이다.
본 발명의 또 다른 목적은 하나 이상의 진단제 또는 치료제와 결합된, 효능이 높고 표적 대상물에 대한 표적물의 비가 증가된 감염성 또는 염증성 병변에 대한 다중특이적 항체를 표적화하는 방법을 제공하는 것이다.
본 발명의 추가의 목적은 감염성 또는 염증성 병변을 더욱 유효하게 검출 및/또는 치료하는 시약 및 방법을 제공하는 것이다.

발명의 요약
본 발명의 이들 목적 및 기타 목적들은, 하나 이상의 영상화제와 결합된, 상이한 백혈구 세포 유형과 특이적으로 결합하는 실질적으로 단일특이적인 2가지 이상의 상이한 항체 또는 항체 단편을 함유하는, 백혈구 유착 부위를 표적화하는 다중특이적 항체-제제 결합제를 제공함으로써 성취된다.
본 발명은 또한, 포유동물에게 다중특이적 항체-제제 결합체를 표적화시키기에 유효한 양으로 비경구 주입시켜 염증성 또는 감염성 병변에 대한 전단제를 표적화하는 방법을 제공한다.

또한, 본 발명은 주입용 멸균 제제 및 전술된 방법을 실행하는데 사용하기 위한 키트를 제공한다.
발명의 상세한 설명
본 발명은 백혈구 유착 부위를 표적화하는 다중특이적 항체-제제 결합체를 사용함으로써 선행기술인 구드윈 등의 영상화 방법에 비해 개선점을 제공해 준다. 감염성 또는 염증성 병변의 발현과 관련된 상이한 백혈구 세포 유형(예 : 과립구, 단핵구, T-림프구 및 B-림프구)은 종종, 병변의 발현을 개시하는 제제의 특성 및/또는 병변의 경과시간에 따라서, 염증성 또는 감염성 병변에 현저하게 상이한 비율로 존재한다. 구드윈에 의해 교시된 바와 같이 단일특이적 항체를 사용한 결과, 병변 부위에 이쓴 백혈구 집단의 단지 일부분만이 표적화 항체와 결합할 경우 병변의 표적화가 비효율적일 것이며, 이는 표적 대상물에 대한 표적물의 비(또한, 국호화 비로 일컬음)을 감소시킬 것이다. 상이한 백혈구 특이성을 갖는 항체 혼합물을 사용하면, 정비례의 특이성이 이용되는 경우 표적 부위에 도달하는 주입용량의 비율은 증가될 수 있지만, 병변의 대부분이 단일 백혈구 세포 유형일 경우에는 비표적 백혈구와의 결합을 증가시킬 수 있다.

본 발명에 따른 영상화 시약은, 영상화 방사선 동위원소 또는 상자성 종과 결합된, 이중특이적, 삼중특이적, 또는 더욱 일반적으로, 다중특이적 항체/단편 복합물을 함유할 수 있다. 결합체중의 항체 성분은 완전한 항체 또는 항체 단편으로 만들어질 수 있다.

단편이 이들의 모(parent) 항체가 발생되는 것에 대항하는 백혈구에 대한 특이성을 보유하는 한, 중쇄를 분리시켜 일가 경-중쇄 단편을 형성시키고, 추가로 단편을 절단시키는 것과 같은 항체를 절단하는 기타 방법, 또는 기타 효소적, 화학적 또는 유전학적 기술을 또한 사용할 수도 있다.
백혈구 항원에 대한 항체는 숙주에게 환자종으로부터의 백혈구를 접종시켜 만들 수 있다. 예를 들면, 사람에 사용하기 위한 항체는 마우스 또는 기타 포유동물 종을 사람 백혈구로 면역시킴으로써 제조할 수 있다. 항-사람 백혈구 혈청을 숙주로부터 수집하고 친화성 정제시켜 복합물을 제조하기 위한 폴리클로날 항체를 수득할 수 있다. 또 다른 방법으로는, 비세포를 면역화된 숙주로부터 취하고, 체세포 하이브리드화 기술을 사용하여 적합한 중앙세포주와 융합시켜 항-백혈구 항체를 생성하는 하이브리도마를 제조할 수 있다. 이들 하이브리도마를 분리시키고, 아클로닝시킨 다음 배양하여 모노클로날 항체를 제조할 수 있다.

J., et al., eds, Plenum(1980)]에 기술된 체세포 하이브리드화 기술로부터 제조할 수 있다. 백혈구 항원에 대한 시판용 모노클로날 항체는 정상의 T-림프구와 결합하는 OKT 항-T 모노클로날 항체(Ortho Pharmaceutical Company사 제조), ATCC 기탁번호가 HB44, HB55, HB12, HB78 및 HB2인 하이브리도마에 의해 생성된 모노클로날 항체, G7E11, W8E7, NKP15 및 GO22(Becton Dickinson), NEN9.4(New England Nucler) 및 FNC11(Sera Labs)로 나타낸다.

일반적으로, 비교적 높은 면역반응성, 즉 약 105ℓ/mol 이상, 바람직하게는 약 107ℓ/mol 이상의 결함 상수 및 백혈구 항원에 대한 높은 면역특이성, 즉 약 40% 이상, 바람직하게는 약 60%, 더욱 바람직하게는 약 70 내지 95% 이상을 갖는 항체를 사용하는 것이 바람직하다.
본 발명에서 다소 낮은 결합상수를 갖는 항체를 사용하는 것이 특정 적용에 바람직할 수도 있다. 높은 결합 상수를 갖는 항체는 아마도, 염증 또는 감염 부위에 있는 백혈구뿐만 아니라, 순환 시스템, 즉 골수 또는 정상조직에 존재하는 백혈구와 단단하게 결합된다. 또 다른 한편으로, 낮은 결합상수를 갖는 항체는 덩어리 작용 효과의 한 유형으로인해, 병변 부위에 있는 농후한 백혈구 병소에 주로 유착되는 경향이 있을 것이다. 이로써, 영상화 표지 또는 후술될 치료용도에 있어서는 치료제의 조기 청정화 및 비표적 유착이 감소되므로, 병변을 표적화하는 유효량은 증가될 것이다.

본 발명에 따른 여러 가지 유형의 이중특이적 항체 복합물중에는 특정 용도에 특히 유용한 하기 복합물이 포함된다. 예를 들면, 골수염을 검출 및 치료하기 위한, 단핵구 및 과립구에 대해 특이적인 항체/단편적인 복합물, 예를 들면, 크론병(Crohn's disease)을 검출 및 치료하기 위한, B-세포 및 단핵구에 대해 특이적인 항체/단편의 복합물, 예를 들면, 유육종증을 검출 및 치료하기 위한, T-세포 및 B-세포에 대해 특이적인 항체/단편의 복합물 , 예를 들면, 결절성 병변을 검출 및 치료하기 위한, 단핵구 및 림프구에 대해 특이적인 항체/단편의 복합물 및 비공지된 기원의 고열과 연관된 감염, 예를 들면 육아종증 감염, 결절성 병변, 진균성 감염 등을 검출 및 치료하기 위한, Ia(Dr) 조직적합성 항원 및 과립구에 대해 특이적인 항체/단편의 복합물.
유사한 반응을 이용하여 복수의 항체 및/또는 항체 단편(예 : Fab 또는 F(ab')2단편)을 서로 결합시켜, 표적 방법에 대한 이의 결합 진화도 및 효능을 증가시키는 다중특이적 복합물 또는 백혈구 세포 유형에 대한 하나 이상의 에피토프 특이성을 갖는 복합물을 형성할 수 있다. 이중특이적 복합물은, 이중 이작용성 말레이미드-하이드록시 석신이미드 에스테르 링커를 사용하여 아민 그룹을 유도화한 다음, 이 유도체를, 임의로 시약(예 : 2-아미노티올란)과 함께 도입된, 자유 설프하이드릴 그룹을 갖는 단편과 반응시켜, 제3, 제4 또는 추가의 백혈구 세포 유형에 대해 특이적인 항체/단편과 연결시킬 수 있다. 또 다른 연결 양상은 이중특이적 복합물 형성에 대한 설명을 기초로 하여 통상의 전문가에게는 용이하게 명백할 것이며, 단지 이러한 방법의 약간의 변형 및 적응이 필요할 것이다.
본 발명에 따른 여러 가지 유형의 삼중특이적 또는 다중특이적 항체 복합물중에는 특정 용도에 특히 유용한 하기 복합물이 포함된다. 예를 들면, 조직이식 거부 침윤을 검출 및 치료하기 위한, T-세포, B-세포 및 단핵구에 대해 특이적인 세포, 단핵구 및 과립구에 대해 특이적인 항체/단편의 복합물, 예를들면, 갑상선염, 조직이식 거부 침윤 및 결절성 병변을 검출 및 치료하기 위한, Ia(DR) 항원, 과립구 및 T-1 항원에 대해 특이적인 항체/단편의 복합물.
이러한 항체 복합물을 진단용 영상화제로서 사용하기 위하여, 섬광 그래피적 영상화용 방사선 동위원소 또는 자기공명 영상 종진제로 표지시키거나, 이와 결합시키거나 결합하도록 변형시킬 수 있다. 생체내 용도의 단백질을 표지시키는데 적합한 통상의 모든 방사선 표지 방법이 일반적으로 상기 복합물을 표지시키는데 적합할 것이다. 이는, 통상의 기술 또는 더욱 복잡한 방법을 사용하여, 예를 들면 할로겐 또는 금속 이온의 방사선 동위원소로 직접 표지시키거나, 방사선 금속 또는 상자성 이온에 대한 킬레이터를 부착시킴으로써 성취될 수 있다. 이러한 킬레이터 및 항체에 대한 이들의 부착 형태는 통상의 전문가에게는 널리 공지되어 있으며, 특히 문헌[참조 : Childs et al., J. Nuc, Med., 26, 2939(1985)] 및 골든베르그의 미합중국 특허 제4331647호, 제4348376호, 제4361544호, 제4468457호, 제4444744호 및 제4624846호에 기술되어 있다. 에틸렌디아민테르라아세트산(EDTA) 및 디에틸렌트리아민펜타아세트산(DPTA)의 유도체가 일반적이다. 이들은 일반적으로, 킬레이터가 항체에 부착될 수 있는 측쇄상의 그룹을 갖는다. 한편으론, 킬레이터상의 카복실 또는 아민 그룹을 활성화한 다음 널리 공지된 방법으로 항체 복합물과 커플링시킬 수 있다. 예를 들면, Ga-67에 대한 킬레이터인 데퍼옥사민은 활성화 카복실, 이소티오시아네이트 또는 유사 그룹을 함유하도록 적합한 링커로 활성화되고, 이어서 항체 복합물 상의 아민과 커플링될 수 있는 자유 아민 그룹을 갖는다.
이러한 킬레이터는 항체상의 하나 이상의 작용성 그룹(예 : 아민, 카복실, 페닐, 티올 또는 하이드록실그룹)을 통하여, 직접 또는 단쇄 또는 장쇄 링커 잔기를 통하여 항체 복합물에 결합될 수 있다. 예를들면, 디이소시아네이트, 디이소티오시아네이트, 카보디이미트, 비스-하이드록시석신이미드 에스테르, 말레이미드-하이드록시석신이미드 에스테르, 글루타르알데히드 등과 같은 여러 가지 통상의 링커, 바람직하게는 미합중국 특허 제4680338호에 기술된 무수물-이소티오시아네이트 링커와 같은 선택적 연차적 링커를 사용할 수 있다.
요오드-131(Ⅰ-131) 또는 요오드-123(Ⅰ-123)을 사용한 표지화는 방사성 요오드화 칼륨 또는 나트륨과 항체와의 혼합물을, 예를 들면 문헌[참조 : Greenwood et al., Biochem. J., 89, 114(1963)]에 보고되고 문헌[참조 : McConahey et al., Int. Arch. Allergy Appl. Immunol., 29, 185(1969)]에 의해 변형된 바와같이, 클로르아민-T로 처리하는 산화적 방법을 사용하여 용이하게 수행된다. 이로써, 시약의 비율 및 반응 조건에 따라서, 항체 분자상, 아마도 티로신 잔기상, 가능하게는 또한 트립토판 및 심지어는 페닐알라닌 자기상의 수도 원자가 요오드 원자로 직접 치환된다. 또 다른 방법으로는, 문헌[참조 : Feteanu, 상기 참조, page 303] 및 본 명세서내에 인용된 참조문헌에 기술된 바와 같이, 락토퍼옥시다제 요오드화 반응을 사용할 수 있다.
더욱 향상된 표지의 방법중의 일부가 계류중인 미합중국 특허원 제742436호(6-7-85), 제084544호(8-12-87) 및 제176421(4-1-88)에 기술되어 있다. 전술된 모든 특허 및 특허원의 기술내용은 본 명세서내에 참조문헌으로 삽입된다. 광범위한 표지화 기술은 문헌[참조 : Feteanu, Labeled Abtibodies in Biology and Medicine, pages 214-309(McGraw-Hill Int. Book Co., New York et al., 1978)]에 기술되어 있다. 여러 가지 금속 방사선-동위원소의 도입은 문헌[참조 : Wagner et al., J. Nucl. Med., 20, 428(1979), Sundberg et al., J. Med. Chem., 17, 1304(1974), and Saha et al., J. Med., 6, 542(1976)]의 방법에 따라서 수행될 수 있다. 전술된 방법은 단지 당해분야에 공지된 수많은 단백질의 방사선 표지화 방법을 예사한 것이다.
MRI 영상 증신에 유용한 화합물의 예로는 상자성 이온(예 : Gd(Ⅲ), Eu(Ⅲ), Dy(Ⅲ), Pr(Ⅲ), Pa(Ⅳ), Mn(Ⅱ), Cr(Ⅲ), Co(Ⅲ), Fe(Ⅲ), Cu(Ⅱ), Ni(Ⅱ), Ti(Ⅲ) 및 V(Ⅳ) 이온), 또는 라디칼(예 : 니트록사이드)가 있으며, 이들을 상자성 이온 킬레이터를 가즌 기질과 결합시키거나, 킬레이트화 작용성 그룹(예 : SH, NH, COOH)을 이온에 또는 링커를 라디칼 부가물에 노출시킬 수도 있다. 이러한 MRI 증진제는, 환자 안전과 기계적 고안에 상당히 적합하고 부합하는 자기장 강도를 이용하여, 외부 카메라에 의해 검출할 수 있을 정도의 충분한 양으로 존재해야만 한다. 이러한 제제에 대한 요구사항은 배지중의 물 분자에 대한 이들의 효과를 나타내는 상기 제제에 대해 당해분야에 널리 공지되어 있고, 특히 문헌[참조 : Pykett, Scientific American, 246, 78(1982) 및 Runge et al., Am. J. Radiol., 141, 1209(1987)]에 기술되어 있다.
금속을 직접 또는 킬레이트의 형태로 항체내에 도입하는 다수의 동일한 방법이 MRI제를 본 발명에 따른 항체 복합물내로 도입시켜 감염성 병변에 대한 영상화제를 생성시키는데 적합한 것이라는 사실은 널리 인식되고 있다. MRI제는 유리하는 증진된 영상화를 위한 수많은 상자성 이온 또는 라디칼을 갖는다. 이러한 이온의 대다수를 도입시키기 위한 하나의 방법은 운반 중합체에 킬레이터를 부하하고, 이 분잔체를, 바람직하게는 복합물의 항원 결합 부위로부터 멀리 떨어진 위치에서 부위-특이적으로 항체 복합물과 연결시키는 것이다. 이러한 방법은 더 많은 킬레이터가 항체 자체상의 더 적은 부위에 있는 항체에 부착될 수 있으므로, 면역반응성이 심각하게 손상되지 않는다는 장점을 지닌다. 항체에 킬레이터를 부하하는데 유용한 중합체의 예는 예를 들면, 폴리올, 폴리사카라이드, 폴리펩타이드 등을 포함한다(참조 : 미합중국 특허 제4699784호(Shin et al.) 및 제4046722호(Rowland)), 폴리사카라이드의 한 유형은 덱스트란이다. 킬레이트는 덱스트란 하이드록실에 대한 반응성 그룹(예 : 무수물, 이소시아네이트 또는 이소티오시아네이트 등)을 함유하도록 작용할 수 있다. 또 다른 방법으로는, 덱스트란을 수많은 방법, 예를 들면 아미노덱스트란으로의 전환에 의해 우됴할 수 있다. 더욱 상세히 후술될 바와 같이, 이와 유사한 방법들이 대다수의 약제 분자를 항체 또는 항체 복합물상에 부하하는데 유용할 것임은 인지될 것이다.
아미노덱스트란(AD) 운반체와의 항체 결합체를 제조하는 방법은 통상적으로, 덱스타란, 중합체, 유리하게는 약 10,000 내지 100,000, 바람직하게는 약 10,000 내지 40,000, 더욱 바람직하게는 약 15,000의 평균 분자량(Mw)을 갖는 덱스트란으로 시작한다. 그 다음, 덱스트란을 산화제와 반응시켜 이의 탄수화물 환의 일부분이 제한 산화되어 알데히드 그룹을 생성시킨다. 이러한 산화반응은 통상의 방법에 따라서, 통상적으로 당분해작용성 화학적 시약(예 : NaIO4)을 사용하여 수행한다.
다른 평균 분자량의 덱스트란에 대해 알데히드 그룹의 비율을 거의 동일하게 하면서, 평균 분자량 약 40,000의 덱스트란에 대해 약 50 내지 150개, 바람직하게는 100개의 알데히드 그룹이 생성되도록 산화제의 양을 조정하는 것이 편리하다. 알데히드 그룹 및 연속적인 아민 그룹이 더 많이 존재하게 되면, 이때 중합체는 더욱더 폴리리신처럼 행동하기 때문에 덜 유리하다. 더 적게 존재하게 되면, 킬레이터 또는 첨가된 붕소의 부하가 덜 바람직하게 되며, 이는 유리할 수도 있다.
그 다음, 산화된 덱스트란을 폴리아민, 바람직하게는 디아민, 더욱 바람직하게는 모노 또는 폴리-하이드록시 다아민과 반응시킨다. 적합한 아민으로는 예를 들면, 에틸렌디아민, 프로필렌디아민 또는 유사한 폴리메틸렌디아민, 디에틸렌트리아민 또는 유사한 폴리아민, 1,3-디다미노-2-하이드록시프로판 또는 기타 유사한 하이드록실화 디아민 또는 폴리아민 등이 있다. 알데히드 그룹에 비해 과량의 아민을 사용하여 알데히드 작용기를 시프 염기(아민) 그룹으로 실질적으로 완전하게 전환시킨다.
생성된 중간물질의 환원적 안정화는 시프 염기 중간 물질을 환원제(예 : NaBH4, NaBh3CN 등)와 반응시켜 수행한다. 과량의 환원제를 사용하여 이민 그룹을 2급 이민 그룹으로 실질적으로 완전하게 환원시키고, 반응하지 않은 모든 알데히드 그룹을 하이드록실 그룹으로 실질저그로 완전하게 환원시킨다. 생성된 부가물을 통상의 크기별 분로 컬럼내로 통과시켜 교차-결합된 덱스트란을 제거함으로서 추가로 정제시킬 수 있다. AD상의 입수가능한 아미노 그룹의 일차적 수의 측정은 무게가 있는 샘플을 트리나트로벤젠설폰산과 반응시키고 420nm에서의 광학밀도를 표준치와 대비시킴으로써 수행될 수 있다. 이러한 방법을 수행한 결과, 통상적으로 계산치의 알데히드 그룹이 거의 완전하게 AD상의 1급 아민 그룹으로 전환되었다.
한편으로, 덱스트란을 아민 작용기를 도입하는 통상의 방법, 예를 들면 브롬의 시아노겐과 반응시킨 후 디아민과 반응시킴으로써 유도할 수 있다. AD는 통상의 방법, 예를 들면 디사이클로헥실카보디아미드(DCC) 또는 이의 수용성 변형체를 사용하여 제조된, 활성화 형태의 특정 약제 또는 킬레이터 유도체, 바람직하게는 카복실-활성화 유도체와 반응시켜야만 한다.
상기 언급된 방법을 이용하여 치료약제 및 방사성핵종 둘 다에 결합된 항체를 제조할 수 있다. 이러한 양태에서는, 상기 문장에 기술된 방법에 따라서, 항체/항미생물제 결합체를 초기에 제조한다. 대부분의 방사성핵종의 반감기가 비교적 짧기 때문에, 항체/항미생물제/방사성핵종의 결합체는 트여 직전에 통상적으로 제조된다.
치료상 방사성핵종의 예는 알파-방출제(예 : 비스무트-212 및 아스타틴-211), 베타-방출제(예 : 이트륨-90, 레늄-186, 레늄-188, 구리-67 및 요오드-131) 및 전자포획 방사성핵종(예 : 요오드-125)을 포함한다.
본 발명의 신티그래프 영상화 방법은 포유동물, 바람직하게는 사람에게 방사선 표지된 다중특이적 항-백혈구 항체 복합물을 신티그래프 영상화하는데 유요한 양으로 비경구 주입시켜 시행한다. 비경구적 투여는 예를 들어 정맥내, 동맥내, 협막내, 간질내 또는 동내투여를 의미한다. 피검자에게 방사선 표지된 결합체 약 1mCi 내지 50mCi의 용량을 투여하는 것으로 계획을 세우는데, 이때 상기 양은 특정 방사선 동위원소의 작용과 투여형태에 따라 결정된다. 정맥내 주시의 경우, 상기 양은 통상적으로, I-131 약 2 내지 10mCi, 바람직하게는 약 2 내지 5mCi, I-123 약 5 내지 10mCi, 바람직하게는 약 8mCi, Tc-99m 약 10 내지 40mCi, 바람직하게는 약 20mCi, In-111 또는 Ga-67 약 2 내지 5mCi, 바람직하게는 약 4mCi이다.
방사선 표지된 다중특이적 항-백혈구 항체 복합물은, 신티그래프 영상화제를 백혈구를 함유하는 감염성 또는 염증성 병변으로 표적화하기 위하여, 바람직하게는 약제학적으로 허용되는 멸균 주사 비히클, 바람직하게는 생리학적 pH 및 농도의 인산염-완충 식염수(PBS)중의 방사선 표지된 복합물의 유효량을 함유하는 멸균 주사 가능한 용액을 포함하는, 포유동물용의 주사가능한 제제, 바람직하게는 사람용의 멸균 주사가능한 제제로서 편리하게 제공된다. 약제학적으로 허용되는 기타 통상적인 비히클을 비경구 투여 부위에 요구되는 바에 따라 사용할 수 있다.
본 발명에 따라서 비경구 투여될 제제중 대표적인 것은, 유리하게는, 0.9% 염화나트륨을 함유하는 0.04M 인산염 완충제(pH 7.4, Bioware) ml당 사람 혈청 알부민(1% USP : Parke-Davis) 약 10mg을 함유하는 멸균용액중에 방사선 표지된 다중특이적 항체 복합물 약 0.1 내지 20mg, 바람직하게는 약 2mg을 통상 함유할 것이다.
일단 충분한 동위원소가 표적 부위에 침착되면, 통상의 평면 및/또는 SPECT 감마 카메라를 사용하거나, 염증 또는 병변을 국소화시키기 위해 외부적 또는 내부적으로 사용된 수동 조절 감마 프로브를 사용하여 주사(scanning)를 수행한다. 신티그램은 통상적으로, 50 내지 500KeV의 에너지 검색용 창을 하나 이상 갖는 감마 영상 카메라에 의해 취해진다. 표적 부위는 비교적 농축된 중심에 존재하는 백혈구를 갖는 모든 감염성 병변, 염증성 침착물 도는 잠재성 병변일 수 있다. 백혈구를 함유하는 병변의 국소화는 비경구투여시 다중특이적 항체-체제 결합체와 병변내에 거주하는 백혈구와의 반응성을 통하여 직접 이루어지거나, 또는 표지된 백혈구의 병변내로의 유입을 통하여 직접 이루어질 것이다.
자기공명 영상화(MRI)는 신티그래프 영상화와 유사한 방법으로 수행하는데, 단 영상화제는 방사선 동위원소보다는 MRI 증강 종을 함유할 것이다. 자기공명 현상이 신티그래프와는 상이한 원리를 근거로 작동된다는 사실을 인식하게 될 것이다. 통상적으로, 생성된 시그날은 영상화될 영역내에서 물 분자중의 수소원자 핵중 양자의 자기 운동의 완화시간과 상관이 있다. 자기공명 영상 증강제는 완속도를 증가시킴으로써, 영상화제가 유착되는 영역중의 물 분자와 그 밖의 체내의 물분자 사이의 대비를 증가시킴으로써 작용한다. 그런, 상기 제제의 효과는 T1과 T2모두를 증강시키는 것인데, 이때 T1을 증가시키면 더 큰 대비가 이루어지고, T2를 증가시키면 더 낮은 대비가 이루어진다. 따라서, 이러한 현상은 농도-의존성이며, 통상적으로 최대 효능을 얻기 위한 상사성 중의 최적농도가 존재하게 된다. 최적농도는 사용된 특정 제제, 영상화 위치, 영상화 방식, 즉 방식-반항, 포화0회수, 역전-회수 및 강격한 T1의존성 또는 T2의존성의 여러 가지 다른 영상화 기술 및 제제가 용해되거나 현탁되는 매질의 조성에 따라서 결정될 것이다. 이들 요소 및 이들의 상대적 중요성은 당해분야에 공지되어 있다(참조 : Pykett, 상기 참조 및 Runge et al., 상기 참조).
본 발명의 MRI 방법은 포유동물, 바람직하게는 사람에게 다중특이적 항-백혈구 항체 복합물과 MRi 증강제의 본 발명에 따른 결합체를 자기공명 영상화에 유용한 양으로 비경구 주입시켜 시행한다. 피검자에게, 병변 부위에 있는 MRI 시그날을 약 20% 이상, 바람직하게는 50 내지 500% 정도 증진시키는데 충분한 표지된 결합체의 용량을 투여하는데, 이때 상기 양은 특정 상자성 종의 작용과 투여방식에 따라서 결정된다.
다시, MRI 표지된 다중특이적 항-백혈구 항체 복합물은, MRI제를 백혈구를 함유하는 감염성 또는 염증성 병변으로 표적화하기 위하여, 바람직하게는 약제학적으로 허용되는 멸균 주사 비히클, 바람직하게는 생리학적 pH 및 농도의 인산염-완충 식염수(PBS)중에 표지된 복합물의 유효량을 함유하는 멸균 주사가능한 용액을 포함하는, 포유동물용의 주사가능한 제제, 바람직하게는 사람용의 멸균 주사가능한 제제로서 편리하게 제공된다. 기타 비경구 투여용의 통상적인 약제학적으로 허용되는 비히클을 필요에 따라, 비경구 투여 부위에 사용할 수도 있다.
본 발명에 따라서 비경구 투여되는 제제중 대표적인 것은, 유리하게는, 0.9% 염화나트륨을 함유하는 0.4M 이산염 완충제(pH 7.4, Bioware) ml당 사람 혈청 알부민(1% USP : Parke-Davis) 약 10mg을 함유한 멸균 용액중에 표지된 다중특이적 항체 복합물 약 0.1 내지 20mg, 바람직하게는 약 2mg을 통상 함유할 것이다. 일단 충분한 MRI제가 표적 부위에 침착되면, 병변을 영상화하는 통상의 MRi 카메라를 사용하여 주사를 수행한다.
본 발명의 바람직한 실시양태에서는, 일차적으로 표지된 다중특이적 항체-제제 결합체의 국소화 비는 골덴베르그의 미합중국 특허 제4624846호(이의 내용은 본 명세서내에 참조문헌으로 삽입된다)의 관련 영상화제에 대해 기술된 바와 같이, 표지되지 않은 제2항체를 사용하여 비-표적인 순환 결합체를 스캐빈지하고 이의 청정화를 향상시킴으로써 증진된다. 이러한 기술은 후술되는 바와 같이, 또한 치료약제에 결합된 다중특이적 항-백혈구 항체 복합물에 적용가능하다. 용어 국호화 비는 이의 통상적 의미, 즉 표적 항체 대 비료적 항체 결합체의 비로 사용된다. 일반적으로, 제2항체는 일차 항체 결합체의 국소화 비를 약 20% 이상, 일반적으로 50% 이상 증진시키는 양으로 사용한다.
제2항체는, 이것이 일차 항체 결합체와 결합하여 독립적으로 일차 항체 결합체보다 더욱 신속하게 순환 및 비표적 공간으로부터 청정해지는 복합체를 형성할 수 있는 한, 완전한 IgG 또는 IgM, 또는 IgG 또는 IgM의 단편일 수 있다. 바람직하게는, 제2항체는 완전한 IgG 또는 IgM일 것이다. 일차 항체가 IgG 또는 IgM의 단편일 경우, 일차/이차 복합체가 보체 캐스케이드를 활성화하는 능력을 보유하도록 제2항체는 완전한 IgG 또는 IgM인 것이 바람직하다. 이와는 반대로, 일차 항체가 완전한 IgG일 경우, 제2항체는 복합체가 여전히 보체-고정 능력을 보유한다면 임의의 단편일 수도 있다. 일차/이차 쌍중 적어도 하나는 완전한 IgG 또는 IgM인 것이 바람직하다. IgM을 사용할 경우의 한 가지 잇점은 이것이 일차 항체 또는 분리된 결합체와 고분자량의 복합체, 즉 진단 및/또는 치료 성분(예 : 약제, 킬레이트제, 방사성액종 등)을 형성한다는 것이다. 이는, 특히 혈액으로부터의 비표적 일차 항체 및/또는 성분의 청정속도 및 청정효과를 증가시킬 것이다. 제2항체는 전술된 골덴베르그의 제4624846호 특허에 기술된 방법으로 제조할 수 있다. 모모클로날 항-IgG 종도 또한 입수가능하며, 유리하게는 본 방법에서 제2항체로서 사용된다. 비금속성 결합체, 예를 들면 방사선 요오드화된 연결 그룹 또는 유기 상자성 종(예 : 니트록사이드)은 또한 제2항체가 특이적으로 작용하는 합텐일 수 있다.
일차 다중특이적 항체-제제 결합체를 비정구 투여하여 백혈구에 의한 이의 최대흡수가 이루어지기에 충분한 시간, 일반적으로 최초 투여한지 약 2 내지 72시간 후, 바람직하게는 투여한지 약 24 내지 48시간 후에, 제2항체를 피검자에게 주입한다. 일차 항체를 정맥내 투여하지 않을 경우, 제2항원의 적어도 일부분을 동일한 비정구 경로로 투여하는 것이 유리할 수도 있다. 그러나, 순환 시스템내로 확산된 일차 항체의 청정화를 촉진시키기 위해 제2항체의 적어도 일정 부분을 정맥내 주입하는 것이 유리하다.
순환성 표지된 일차 항체를 청정화하고 일차 항체의 국소화 비를 증진시키기 위한 제2항체의 용도는 전술된 골덴베르그의 특허 및 본 명세서내에서 인용된 참조문헌에 기술된 바와 같은 영상-증강 저지(subtration) 기술을 이용함으로써 주가로 향상된다. 이는 미분화 항체 또는 단편이 독립적으로 검출할 수 있는 방사성핵종으로 표지되는, 당해분야에 인식된 기술이다. 이러한 항체는 영상화하는데 필요한 기간 동안에 일차 항체와 실질적으로 동일한 분포력 및 기작을 갖는다. 통상의 저지제(예 : Tc-99m-표지된 혈청 알부민)가 표적 대상물을 보강해줌으로써 영상공정을 증진시키는데 사용하기 적합할 뿐만 아니라, 이러한 항체의 주입은 상기 저지제에 비해 바람직하다. 방사선 표지된 미분화 항체의 저지제로서의 용도는 순환시스템으로부터 항체 청정화에 영향을 끼치는 기관으로부터 비표적 대상물 방사선에 대한 컴퓨터 교정을 가능케 한다. 일차 모노클로날 항체 및 저지제로서 사용된 미분화 항체는, 제2항체가 실질적으로 동일한 속도로 일차 모노클로날 항체 및 비표적된 영역으로부터의 미분화 항체 면역글로블린을 청정화하도록 동일한 종 또는 골수종/하이브리도마로부터 유래된 것이 바람직하다는 사실이 당해 분야의 전문가에게는 인식될 것이다. 또한, 제2항체가 일차 및 미분화 면역글로블린 종의 일정 영역에 대해서 특이적인 것이 바람직하다.
도입된 제2항체의 양은 일반적으로, 순환성 일차 항체를 2 내지 72시간 내에 10 내지 85% 정도 감소시킬 수 있는 양일 것이다. 청정화에 영향을 미칠 제2항체 대 일차 항체의 비는 일차 및 이차 항체 쌍의 결합 특성에 따라서 결정될 것이다. 시험관내에서 환자 혈액을 예비 스크리닝시켜 적당한 비를 초기에 측정할 수 있다. 이러한 스크린을 사용하여 예를 들면, 겔 확산 시험으로 침강소 밴드를 수득하는데 필요한 제2항체 대 일차 항체의 비를 측정할 수 있을 것이다. 이는 제2항체 대 일차 항체의 일반적 몰비 범위를 지시하는데, 이는 생체내 적용이 상기 시험관내 시험에 의해 지시된 것보다 더 높은 제2항체 대 일차 항체의 비를 필요로 하기 때문에, 비에 대한 하한치의 측정치로서 제공된다.
실체적으로, 제2항체 대 일차 항체의 몰비 범위가 제한적인 것은 아닐지라도, 이는 일반적으로 약 5 내지 50의 범위일 것이다. 일차 및 제2항체 모두가 완전한 IgG일 경우, 15 내지 25, 바람직하게는 20 내지 25의 제2항체 대 일차 항체 비가 유리하다는 사실이 밝혀졌다.
사람을 감염시키고 병변을 유발시킬 수 있는 세포 또는 미생물에 대해 세포 독성 효과를 나타내는 수많은 약제가 공지되어 있다. 이들은 약제 및 독소에 관한 용이하게 입수가능한 당해분야-인식된 부록(예 : 머크 인덱스(Merck Index) 등)중의 어느 것에서도 발견될 수 있다. 이러한 항생제 모두를 항-백혈구 항체 또는 항체 복합물과 결합시켜 본 발명에 따른 치료제를 수득할 수 있으며, 감염성 또는 부위에서의 상기 결합체의 유효농도를 증가시키도록 이러한 부위에 대한 항생제의 표적화를 향상시키기 위한 상기 결합체의 용도도 본 발명의 한 부분이다. 치료 목적상 하나 이상의 항생제를 중합성 운반체와 결합시킨 다음 항체 또는 항체 복합물과 결합시킨다. 특정 경우에서는 항체 결합물의 일부로서 약제를 부분적으로 또는 완전하게 해독할 수 있는 반면, 순환시에는 약제의 전신 부작용을 저하시키고 약제의 전신 투여가 적합하지 않을 때에도 이를 이용할 수 있다. 항체와 가로 결합된 중합체와 결합된 더 많은 분자의 약제를 투여함으로써, 전신 독성을 경감시키면서 치료를 수행할 수 있다.
본 발명에 따른 방법은 제1항체 또는 항체 복합물을 항생제에 결합시킴으로써 감염성 병변을 치료하는데 적용가능하다. 항생제를 면역글로블린에 결합시키는 당해분야에 인식된 방법은, 예를 들면 문헌[참조 : the chapter by O'neill, entitled The Use of Antibodies as Drug Carriers, in Drung Carriers in Biology and Medicine, G. Gregoriadies, ed., Academic Press London, 1979, Arnon et al., Recent Results in Cancer Res. 75, 236(1980), and Moolton et al., Immunolog. Res. 62, 47(1982)]에 기술되어 있다. 이들 방법은 여러 가지 질병-유발 미생물(예 : 세균, 바이러스, 진균류 및 이들 미생물에 대해 발현된 항체에 대한 다양한 기생충), 이들의 병변과 연관된 생성물 또는 항원에 대하여 유효한 약제를 커플링하는데 사용된 방법과 다소 유사하다.
이러한 항세균, 구충, 항진균 및 관련 약제, 예를 들면 설폰아미드, 페니실린 및 세팔로스포린, 아미노글리코시드, 테트라사이클린, 클로람페니콜, 피페라진, 클로로퀸, 디아미노피라딘, 메트로니아지드, 이소니아지드, 리팜핀, 스트렙토마이신, 설폰, 에리트로마이신, 폴리믹신, 나이스타틴, 암포테리신, 5-플루오로사이토신, 5-요오드-2'-데옥시우리딘, 1-아다만타아민, 아데닌 아라비노사이드, 암만니틴 및 아지도티미딘(AZT)이 적합한 특이 항체/단편 및 항체/단편 복합물과 커플링시키는데 바람직하다. 본 발명에 사용하기 위한 기타 여러 가지 효능 있는 항미생물제가 문헌[참조 : Goodman et al., The Pharmacological Basis of Therapeutics, Sixth Edition, A.G. Gilman et al., eds., Macmillan Publishing Co., New York, 1098]에 예시되어 있다. 약제를 특이적 표적 부위로 표적화하는데 적당하고 바람직한 여러가지 조건은 예를 들어 문헌[참조 : Trouet et al., in Targeting of Drugs, G. Gregoriadis et al., eds., Plenum Press, New York and London, 1982, pp. 19∼30]에 보고되었으며, 여기에는 표적화가 감염성 병변으로 고통받고 있는 환자에게 어떻게 유익한 것인지에 관한 임상적 지식이 기술되어 있다.
병리학적 병변을 치료하는 하나의 방법으로서 방사선 치료가 널리 인식되고 있다. 치료학적 방사성핵종을, 문헌[참조 : Greenwood et al., 상기 참조]에 보고되고 문헌[참조 : McConahey et al., 상기 참조]에 의해 변형된 방법 및 전술되고 현재 계류중인 미합중국 특허원 제742436호, 제084544호 및 제176421호에 기술된 방법을 이용하여 면역글로블린에 결합시킬 수 있다.
또한, 종양 및 병리학적 병변의 중성자-활성화 방사선 치료법은 하우토론(Hawthorne)의 미합중국 특허 제4824659호(이의 내용은 본 명세서내에 참조문헌으로 삽입된다)에 보고되었다. 치료는, 다수의 붕소-함유 그룹과 결합된 다중특이적 항-백혈구 항체를 병리학적 병변 부위로 표적화하고, 비-표적화된 결합체를 적당히 청정화시키는데 필요한 시간 후에 표적 용역을 열 중성자 조사에 노출시켜 주변 조직상에 일파 입자의 치료학적 용량을 제공함으로써 성취된다.
본 발명의 방사선 치료법은 방사선 표지된 다중특이적 항-백혈구 항체 복합물을 치료학적 유효량으로 포유동물, 바람직하게는 사람에게 비정구 주입시켜 시행한다. 방사선 표지된 결합체의 투여용량은 사용된 특정 동위원소, 투여 빈도 및 피검자의 조혈상태에 따라서 결정될 것이다. 통상적으로 5 내지 250mCi의 방사선핵종이 지시된다. 특히, 이트륨-90의 통상 용량은 용량당 20 내지 30mCi인 반면, 요오드-131은 용량당 40 내지 100mCi로 투여된다. 기타 방사선 동위원소 및 붕소 결합체에 대한 용량은 통상적으로 전문 임상의에 의해 결정될 것이다.
방사선 표지된 다중특이적 항-백혈구 복합물은 치료용 방사선핵종을 백혈구를 함유하는 감염성 또는 염증성 병변으로 표적화하기 위하여, 바람직하게는 약제학적으로 허용되는 멸균 주입 비히클, 바람직하게는 생리학적 pH 및 농도의 인산염-완충 식염수(PBS)중에 방사선 표지된 복합물의 유효량을 함유하는 멸균 주입가능한 용액을 함유하는, 포유동물용의 주입가능한 제제, 바람직하게는 사람용의 멸균 주입가능한 제제로서 편리하게 제공된다. 기타 통상의 약제학적으로 허용되는 비히클을 필요에 따라 비경구 투여 부위에 사용할 수도 있다.
본 발명에 따라서 비경구 투여되는 제제중 대표적인 것은 통상적으로, 멸균 용액중의 방사선 표지된 다중특이적 항체 복합물 약 5.0 내지 150mg, 바람직하게는 약 10 내지 50mg, 더욱 바람직하게는 15 내지 25mg을 함유할 것이다.
표지된 다중특이적 항-백혈구 복합물을 최적 국소화시키는데 충분한 시간 후, 방사선핵종의 유해한 효과는 비표지된 제2항체를 사용하여 비표적된 순환성 결합체를 스캐빈지하고, 영상화 및 MRI제에 관해 전술한 바와 같이 이의 청정화를 향상시킴으로써 최소화할 수 있다.
본 발명의 바람직한 양태에서, 항미생물제 및 치료학적 방사선핵종과 결합된 항-백혈구 항체는 본 명세서내에서 언급된 영상화제 및 약제를 사용하여 결합체를 제조하는 것으로 기술된 방법에 의해 제조될 수 있다. 다중특이적 항체 복합물에 대한 두 가지 치료제의 결합으로, 단독으로 투여될 때 치료제의 통상 용량 수준의 50 내지 100%의 용량 수준을 가능케 하는 부가 및 상승효과를 얻을 수 있다. 이온화 방사선은 염증성 및 감염성 병변의 방관자(bystander) 방사선 및 이들의 유발성제(예 : 세균, 진균 및 바이러스)를 제공하면서, 다소 유효한 것으로 나타났다.
상기 기술된 바와 같은 제2항체의 사용은 진단용 영상화 결합체에 대해서와 동일한 방법으로 본 발명에 따른 치료제의 효능을 증가시킬 것이다. 치료제의 효능은 통상 사용되는 치료효과 대 바람직하지 않는 부자용의 비로서 정의되는 이의 치료 지수 용어로 표현된다. 이는 종종, 표준 모델 시스템에서 효능 대 독성의 정량적 측정치, 예를 들면 메디안 치사용량(LD50) 대 메디안 유효용량(ED50)의 비로서 정의된다. 본 명세서내에 기술된 제2항체의 사용은, 비표적 일차 항체 및/또는 분리된 치료 성분을 청정화함으로써 항-백혈구 항체 및 항체 복합물 결합체의 치료 지수를 증가시킨다. 상기 기술된 바와 같이 일차 모노클로날 항체에 대해 특이적인 것 외에도, 치료학적 제제의 경우에 제2항체는 치료제에 대해서도 특이적일 수 있다. 또한, 이는 치료제에 대한 운반제에 대해서도 특이적일 수 있다.
본 명세서내에서 제시된 치료학적 제제는 치료학적으로 유효한 약제와 결합된, 비경구 투여용으로 적합한 비히클중에 상기 언급된 단일특이적 항-백혈구 항체/단편을 함유한다. 또한, 치료학적 제제는 항생제와 결합된 다중특이적 항-백혈구 항체/단편 복합물을 함유할 수 있다. 또 다른 치료 제제는 비경구 투여용으로 적합한 비히클중에 단일특이적 항-백혈구 항체/단편과 결합된 치료학적으로 유효한 방사선핵종을 함유할 수도 있다. 추가의 치료학적 제제는 치료학적 방사성핵종과 결합된 다중특이적 항체/단편을 함유할 수도 있다. 부가적으로, 치료학적 제제는 비경구 투여용으로 적합한 비히클중에 치료학적 항미생물제 및 치료학적 방사성핵종과 결합된 단일특이적 항-백혈구 항체/단편을 함유할 수 있다. 또한, 또 다른 치료학적 제제는 항미생물제 및 치료학적으로 유효한 방사성핵종과 결합된 다중특이적 항-백혈구 항체/단편 복합물을 함유할 수 있다. 또한, 치료학적 제제는 상기 기술된 바와 같이 별도로 패키징된 제2항체를 포함할 수도 있다. 적합한 비히클은 당해분야에 널리 공지되어 있으며, 예를 들면, 전술된 바와 같이 진단 영상화제를 투여하는데 사용된 것과 유사한 멸균 PBS 용액을 포함할 수 있다.
또한, 본 발명에 따른 항-백혈구 다중특이적 영상화 결합체 및 단일특이적 또는 다중특이적 치료학적 결합체는 백혈구 병소를 함유하는 감염성 또는 염증성 병변에 항체를 표적화하기 위한 치료 또는 진단용 키트내에 편리하게 제공될 수 있다. 일반적으로, 이러한 키트는 동결건조된 제제 또는 주입 비히클로서, 본 발명에 따른 항체 결합체를 함유하는 바이알을 포함할 것이며, 결합체가 신티그래프 영상화에 사용될 경우, 이는 일반적으로 별도의 용기 내에 방사선 표지용 시약 및 보조제와 함께 차가운 결합체로서 제공되는 반면, MRI제 및 치료학적 결합체에는 일반적으로, 항체/단편 복합물 또는 단일특이적 항체/단편과 이미 결합된 상자성 종 또는 항생물질이 공급될 것이다. 항미생물제 및 방사성핵종으로 구성된 치료학적 결합체는 일반적으로 별도의 용기내에 방사선표지용 시약 및 보조제와 함께 항체/단편 복합물 또는 단일특이적 항체/단편과 이미 결합된 항미생물제로서 제공될 것이다. 키트는 항체 또는 단편 또는 치료제, 이에 대한 운반체, 또는 방사성핵종 또는 상자성 이온에 대한 킬레이드제에 대하여 특이적인 제2의, 별도로 패키징된, 비표지된 항체 또는 항체 단편을 추가로 함유할 수 있다.
본 발명에 따른 영상화 제제 및 방법은 적어도, In-111 표지된 백혈구를 사용하여 숨겨진 종기를 측정하는 통상의 제제와 같은 정도로 유효하며, 비용, 시약의 잠재적 독성 및 가장 중요한 사용 용이성의 측면에서 보면 통상의 제제보다 명백하게 유리하다. 본 발명에 따른 치료시약 및 방법은 염증 부위 및 미생물 감염 부위를 표적화하는 수단을 항생제 및/또는 방사성핵종에 제공하여 약제 및 방사성핵종의 치료 지수를 증가시키고, 전신 부작용을 감소시키며, 이들 효능을 증진시키게 한다.
추가의 노력 없이도, 당해분야의 전문가는 전술된 상세한 설명을 이용하여, 본 발명을 완전한 정도로 수행할 수 있으리라 여겨진다. 따라서, 하기 바람직한 특정 양태는 단지 예시적인 것이며, 어떠한 방법으로도 본 내용의 나머지 기술사항을 제한하지는 않는다. 하기 실시예에서, 달리 언급되지 않는 한, 모든 온도는 비교정된 섭씨도이고, 모든 부 및 퍼센트는 중량기준이다.
이중특이적 항-백혈구 복합물
이중특이적 항체 복합물은 과립구 세포에 대해 고 특이적 모노클로날 항체의 F(ab')2단편으로부터 제조한다. 쇄간 디설파이드 가교를, 경-중쇄 절단을 피하게끔 시스테인으로 조심스럽게 환원시켜 Fab'-SH 단편을 수득한다. 실시예 2
다중특이적 항-백혈구 복합물
다중특이적 항체 복합물은, Fab'2-SH 단편으로 전환시키고 비스-말레이미드 링커로 활성화시킨 다음, 2-이미노티올란을 사용하여 SH 그룹을 삽입시킴으로써 활성화시킨 상기 기술된 이중특이적 단편으로 커플링시킨, T-림프구에 특이적인 모노클로날 항체로부터 제조된다. RAID에 의한 감염성 병변 또는 염증성 퇴적물의 검출을 위해 영상화에 유용한 몇몇 방사성핵종(Tc-99m, I-123, Ga-67, In-111)중의 하나를 사용하여 다중특이적 복합물을 표지시킨다.
실시예 3
신티그래프 영상화 키트
진단용 영상화 키트는 하기를 함유한다. 고무 격막에 적합하고, 실시예 2의 티올-활성화 다중특이적 복합물을 함유하는, 동결건조 형태의 제제인 제1의 멸균 바이알, 인산염-완충 식염수(pH 7.4)를 함유하는 제2의 멸균(격막-밀봉됨) 바이알, 99m-퍼테크네테이트를 환원시키기 위한 제1주석 글로코헵토네이트 용액을 함유하는 제3의 멸균(격막-밀봉됨) 바이알 및 제제를 표지시키고 주입하기 위한 부가의 격막-밀봉된 멸균 바이알 및 멸균 시린지. 임의로, 이러한 키트는 국부화 후, 순환성 표지된 복합물을 신속히 청정화시키기 위한 제2의 항체, 예를 들면 진화-정제된 래빗 항-마우스 항체를 함유하는 별도의 멸균 바이알을 포함할 수 있다.
실시예 4
진단용 영상화
제왕절개로 남아를 출산한 24세 여성환자는 출산한지 1주일 후부터 고열과 복부통증을 느낀다. 이 환자에게 2주간 항생제를 정맥내 투여하여 치료하지만, 고열과 복부 통증은 지속된다. CAT 주사는 어떠한 비정상 덩어리도 입증하지 못한다. 실시예 3의 키트 성분 및 발전기-생성된 나트륨 퍼테크네테이트를 사용하여 Tc-99m 방사선 동위원소로 직접 표지시킨, 실시예 2의 다중특이적 항-백혈구 복합물을 사용하여 면역신티그래프 연구를 수행한다. 방사선 표지된 복합물 20mCi을 주입하고, 환자에게 SPECT 형태의 감마 카메라를 주사한다. 환자 복부의 주사는 Tc-99m의 측적 위치를 입증해준다. 수술하면, 종기는 Tc-99m 활성 부위에서 발견된다. 종기를 제거하고, 병리학은 화농 물질내에 수많은 과립구, 단핵구, B-림프구 및 활성화 T-림프구가 존재함을 입증해준다. 2일 후, 환자의 고열과 통증은 진정된다.
실시예 5
진단용 영상화
신우신염에 대해 치료를 받고 있는 58세 여성 환자에게서 고열 및 급성 척주취약(acute spinal tendernero) 현상이 나타난다. 추 골수염으로 추측되지만, 척주의 방사선 그래피는 정상이었다. 실시예 3의 키트 성분 및 발전기-생성된 니트륨 퍼테크네테이트를 사용하여 Tc-99m 방사선 동위원소로 직접 표지시킨, 실시예 2의 다중특이적 항-백혈구 복합물을 사용하여 면역신티그래프를 수행한다. 방사선 표지된 복합물 20mCi를 주입하고, 환자를 표지된 복합물을 투여한지 24시간 후에 평면 영상 형태의 감마 카메라로 주사한다. 이러한 주사는 척주 건 말단점 바로 위에 있는 Tc-99m의 위치를 강력히 나타낸다. 경막외극의 척추궁절제술 및 배역법을 표적 부위에서 수행한다. 배역법 유액중에서의 화농 물질의 병리학은 대부분 과립구 수가 보다 적은 수많은 단핵 림프양 세포를 함유하는 엑스유버런트(exhuberant) 과립화 조직임을 입증해준다.
실시예 6
치료
AIDS에 걸린 42세 남자는 통상의 광범위한 항생제 치료법에도 반응하지 못하는 양측 폐염을 나타낸다. 임상적 발현 및 객담 세포진단법은 계속 진행되고, 환자에게 심한 호흡기 장애를 일으키는 뉴모사이스티스 카리니어(Pneumocystis carinii)폐염을 제시해준다. 과립구 및 단핵구 항원과 결합하는 모노클로날 항체의 혼합물은 각각, 트리메토프림(Wellcome)(상기 인용된 특허(Shihetal)의 방법을 변형시킴으로써 운반체에 결합된 메토트렉세이트 유도체) 평균 약 25분자를 갖는 평균 분자량 약 15,000의 하나의 아미노덱스트란 운반체와 부위-특이적으로 결합된다. 항체 결합체 혼합물 약 25mg을 서서히 정맥내 주입하여 치료 용량의 항생제를 폐동맥 병변에 운반하고, 이를 3일 연속적으로 반복한다. 2일 후에, 환자는 회복되기 시작하고 그의 고열은 해소되는데, 이러한 사실은 3일 후 가슴의 X-선 사진의 호전에 의해 확인된다. 대부분의 폐염은 항체-약제 결합체로 치료한지 2주 내에 해소되며, 이때 환자는 이 기간 동안에 일반적 지지성 측정을 받는다.
이러한 환자 및 유사한 질환을 치료하기 위하여, 트리메토프림 결합체 대신 또는 이외에, 붕소 결합체를 포함한 치료학적 방사선 동위원소 및/또는 방사선 동위원소와 약제와의 배합물을 사용할 수 있다.
전술된 실시예에 사용된 것들을 일반적으로 또는 특정하게 기술된 본 발명의 반응물 및/또는 작동조건으로 대체함으로써 유사한 결과를 나타내는 실시예를 반복할 수 있다.

Claims (12)
- 영상화제와 결합되거나 치료학적 항미생물제, 붕소-함유 부가물 또는 방사성핵종과 결합된, 상이한 백혈구 세포 유형과 결합하는 2가지 이상의 상이한 단일특이적인(monospecific) 모노클로날 항체 또는 이의 단편으로 이루어진 다중특이적(polyspecific) 복합물을 포함하는, 백혈구 유착 부위를 표적화하기 위한 다중특이적 항-백혈구 결합체.
- 제1항에 있어서, 단일특이적인 모노클로날 항체 또는 이의 단편, 또는 다중특이적 복합물의 50 내지 5,000KeV 범위의 감마 방사선을 방출하는 방사선 동위원소 또는 자기공명 영상화 증강제인 영상화제와 결합된 항-백혈구 결합체.
- 제1항에 있어서, 단일특이적 모노클로날 항체 또는 이의 단편, 또는 다중특이적 복합물의 항세균제, 항바이러스제, 항진균제 및 구충제로 이루어진 그룹중에서 선택된 치료학적 항미생물제와 결합된 항-백혈구 결합체.
- 제1항에 있어서, 단일특이적인 모노클로날 항체 또는 이의 단편, 또는 다중특이적 복합물의 치료학적 방사성핵종과 결합된 항-백혈구 결합체.
- 제1항에 있어서, 단일특이적인 모노클로날 항체 또는 이의 단편, 또는 다중특이적 복합물의 다수의 붕소-10 원자를 함유하는 부가물과 결합된 항-백혈구 결합체.
- 제1항에 있어서, 단일특이적인 모노클로날 항체 또는 이의 단편, 또는 다중특이적 복합물이 치료학적 항미생물제와 결합되고 치료학적 방사성핵종 또는 붕소-함유 부가물과 결합된 항-백혈구 결합체.
- 제1항에 있어서, 각각의 백혈구 세포 유형의 과립구, 단핵구, B-림프구 및 T-림프구로 이루어진 그룹중에서 선택되는 항-백혈구 결합체.
- 두개의 별개의 제제로서, (a) 표적화 유효량의 제1항에 따른 항-백혈구 결합체 및 (b) 항-백혈구 결합체의 국소화 비를 약 20% 이상 증가시키는데 충분한 양 또는 치료학적 결합체의 경우에 이의 치료 지수를 2 내지 72시간 내에 약 10% 이상 증가시키기에 충분한 양의 항-백혈구 결합체에 결함하는 표지되지 않은 제2의 모노클로날 항체 또는 이의 단편을 함유하는 제제.
- 영상화제와 결합된 제1항에 따른 다중특이적인 항-백혈구 결합체를 약제학적으로 허용되는 멸균 주사용 비히클내에 함유하는, 염증성 또는 감염성 병변-함유 백혈구에 영상화제를 표적화시키기 위한 멸균 주사용 제제.
- 제9항에 있어서, 영상화제가 방사선 동위원소 또는 자기공명 영상화 증강제인 멸균 주사용 제제.
- 치료학적 항미생물제, 붕소-함유 첨가제 또는 방사선핵종과 결합된 치료량의 제1항에 따른 다중 특이적 항-백혈구 결합체를 약제학적으로 허용되는 멸균 주사용 비히클내에 함유하는, 염증성 또는 감염성 병변을 치료하기 위한 멸균 주사용 제제.
- 제1용기에는 제1항에 따른 다중특이적 항-백혈구 결합체를 포함하고, 제2용기에는 항-백혈구 결합체의 국소화 비를 약 20% 이상 증가시키거나 치료학적 결합체의 경우에 이의 치료 지수를 2시간 내지 72시간 내에 약 10% 이상 증가시키기에 충분한 양의 항-백혈구 결합체에 결합하는 제2의 표지되지 않은 모노클로날 항체 또는 이의 단편을 포함하는, 생체내에서 감염성 또는 염증성 병변-함유 백혈구를 검출하는데 사용하기에 적합한 키트.
