病変に対する少なくとも1つの診断的又は治療的薬剤を
目的とする薬剤に関する。
数集積するので、血液から白血球を取り出して適当な指
示物質(In-111が都合よく用いられる)で白血球を標識
し、その後それを血液にもどすことにより、そのような
部位の検出が可能である。体内に標識化白血球が再分布
するのに十分な一定時間の後、被験体を適切な装置で走
査し、標識化白血球の局在性を検出する。前記方法は有
効ではあるが、白血球の分離、標識化及び特に再注入後
の体内への再分布に時間がかかるために多くの時間が消
費される。
米国特許第4,634,586号[グッドウィン(Goodwin)等]
には、抗白血球単特異抗体とγ−線放射性金属キレート
との免疫反応性非白血球殺性結合体を患者に注射し、前
記結合体が白血球に局在化するのを待って患者に前記結
合体に対する抗体を注射してバックグラウンドの非局在
化結合体の血液をクリアーし、シンチスキャンにより白
血球を明視化することによる、白血球の放射線免疫影像
化が記載されている。

ンドに対する標的割合はなお問題がある。従って、標的
部位に結合されていないバックグラウンド中の特定の白
血球細胞に対して非常に反応性であり特異的である多く
の抗体が存在する。なぜなら感染又は炎症部位において
特定な白血球細胞型が測定できる濃度存在しないからで
ある。
治療を行うために高度に有効なそしてバックグラウンド
に対する増大した標的割合を有する、炎症又は感染病変
に対する影像剤又は治療剤を標的用にする方法の必要性
が存在し続けている。

を維持する限り、重鎖を分離して一価の軽−重鎖フラグ
メントを形成させ、さらにフラグメントを分解させるよ
うな抗体を分解する他の方法又は他の酵素的、化学的又
は遺伝子的操作も用いられる。白血球抗原に対する抗体
は患者からの白血球を宿主に接種させることによって生

産されるモノクローナル抗体; G7E11、W8E7、NKP15及
びGO22[ベクトン・ディッキンソン(Becton Dickinso
n);NEN9.4[ニュー・イングランド・ニュークレアー(N
ewEngland Nuclear)];及びFMC11[セラ・ラボラトリーズ
(Sera Labs)]で表される。

米国を含む世界的に販売をしている]のカタログに、市
販のモノクローナル抗白血球抗体−それらの多くは本発
明の治療薬剤を製造するのに適している−が載ってい
る。これらはT細胞、活性化T細胞、B細胞、単核白血
球及び顆粒性白血球に特異的に結合する抗体を含み、そ
れらの部分母集団も含む。一定の抗体は1より多い型の
白血球、例えば、単核白血球及び顆粒性白血球、B細胞
及び顆粒性白血球、T細胞及びB細胞、T細胞及び顆粒
性白血球、T細胞及び単核白血球、及び顆粒性白血球及
びB細胞等に共通した抗原に結合する。イムノテックに
より製造され、販売された抗体は他の市販されているク
ローンからの他の抗体と同様である。
D6、CD7又はCD8抗原に結合する抗体を含む。好ましい抗
T細胞抗体はCD3抗原及びCD5抗原に結合するものであ
る。単核白血球及び顆粒性白血球の両方に結合する好ま
しい抗体は特にCDW14抗原に特別に結合するモノクロー
ナルである。B細胞に結合する好ましい抗体はCD19又は
CD21抗原に結合する抗体を含む。活性化T細胞に結合す
る抗体はCD25又はCD26抗原に結合するモノクローナルを

抗体は、本発明の結合体を製造するのに用いられる適し
た抗体が、種又はIgクラスに関して限定されることを意
図していないにもかかわらず、一般的にはハツカネズミ
又はラット由来のものであり、一般的にはIgGs又はIgMs
である。一般的に、抗体は、当技術分野において公知の
多くの従来技術を用いて、ほとんどの抗原に対して生成
される。哺乳動物の体での炎症又は感染部位において十
分の濃度見出だされる白血球抗原に結合する抗体は本発
明において用いられる標的用単特異抗体をつくるのに用

抗体複合体は単純なグルタルアルデヒド結合から、より
精密さと簡潔さを備えた、官能基間の特異的結合までの
種々の従来方法により製造される。抗体及び/又は抗体
フラグメントは好ましくは相互に直接、又は短い又は長
いリンカー部分を通して、抗体/フラグメントの1以上
の官能基、例えばアミン、カルボキシル、フェニル、チ
オール又は水酸基を通して共有結合する。グルタルアル
デヒドの他に種々の従来のリンカー、例えばジイソシア
ネート、ジイソチオシアネート、ビス(ヒドロキシスク
シンイミド)エステル、カルボジイミド、マレイミド−
ヒドロキシスクシンイミドエステル等が用いられる。

(ハイブリッド)抗体フラグメントは、異なる抗体の還
元的開裂により生じるFab′フラグメントの酸化的結合
によって製造される。これらの一部は、原抗体が生じる
両方の抗原に特異的なフラグメントを含む。このこと
は、2つの異なる抗体のペプシン消化により生じる2つ
の異なるF(ab′)2フラグメントを混合し、Fab′フラグ

キシスクシンイミドエステルのような異種二官能価のリ
ンカーを用いて達成される。後者の抗体/フラグメント
との反応は抗体/フラグメントのアミン基を誘導し、そ
の誘導体は次に、遊離のメルカプト基を有する抗体Fab
フラグメント[又は、より大きなフラグメント又は、例

な抗体/フラグメントの複合体。
フラグメント、例えばFab又はF(ab′)2フラグメントを
互いに結合させて多特異複合体又は白血球細胞型に対し
て1つより多いエピトープ特異性を有する多特異複合体
又は複合体を生成するのに用いられ、標的病変に対して
結合親和力又は効率を増加させる。二特異複合体は例え
ば異種二官能性マレイミド−ヒドロキシスクシンイミド
エステルリンカーを用いて、第3、4又はそれ以上の白
血球細胞型に特異的な抗体/フラグメントに結合され、
アミン基を誘導させ、次にその誘導体を遊離のメルカプ
ト基を有するフラグメントで反応させ、適宜、2−イミ
ノチオラン(2-iminothiolane)のような作用剤を導入さ
せる。他の結合方法は二特異複合体形成の記載を基に当
業者には容易に明白になるものであり、少しの変更とそ
のような方法の適用を必要とするのみである。
異抗体複合体の種々の型のなかで包含されるものは下記
のものであり、それらは一定の用途には特異に有用であ
る:例えば移植片拒絶反応浸潤の検出及び治療用の、T
細胞、B細胞及び単核白血球に特異的な抗体/フラグメ
ントの複合体;例えば慢性感染の検出及び治療用の、B
細胞、T細胞、単核白血球及び顆粒性白血球に特異的な
抗体/フラグメントの複合体;例えば甲状腺炎、移植片
拒絶反応浸潤及び結核病変の治療用のIa(DR)抗原、顆粒
性白血球及びT−1抗原に特異的な抗体/フラグメント
の複合体。

のに適する、放射性同位元素を使って識別する従来の方
法は、複合体を標識するのに一般的に適している。この
方法は、例えば、従来技術を用いるか又はより精巧な方
法学を用いて、ハロゲン又は金属イオンの放射性同位体
で直接標識するか又は放射性金属又は常磁性イオン用の
キレート化剤を付着させることによって達成される。そ

は当業者によく知られた方法であり、とりわけ、チルズ

は、エチレンジアミン四酢酸(EDTA)、ジエチレントリ

スクシンイミドエステル、グルタルアルデヒド等の種々
の従来のリンカーを用いることができ、好ましくは、米
国特許第4,680,338号に記載の無水イソチオシアネート
リンカーのような選択性逐次的リンカーである。

常磁性イオン、例えば、Gd(III)、Eu(III)、Dy(III)、P
r(III)、Pa(IV)、Mn(III)、Cr(III)、Co(III)、Fe(II
I)、Cu(II)、Ni(II)、Ti(III)、及びV(IV)イオン又は
基、例えばニトロキサイドを包含し、これらは、常磁性
イオンキレート化剤又はイオンに対しての露出した例え
ば、SH、NH2、COOHのキレート化官能基を担持する支持
体、又はラジカルアデンド用のリンカーに結合する。そ
のMRI強化剤は、適度に達成でき、患者の安全性及び器
械的設計と矛盾しない磁場強さを用いて、外部カメラに
よって検出が可能になるような十分な量を存在させなけ
ればならない。そのような強化剤の要件は、媒体中の水
分子に効果を有するようなものとして当業者に公知であ
り、それらは、とりわけ、例えば、ピケット(Pykett)
による「サイエンティフィック・アメリカン(Sientifi
c American)、第246巻、第78頁(1982年)及びランゲ
(Runge)等によるアメリカン・ジャーナル・オブ・ラジ
オロジイ(Am. J. Radiol.)第141巻、第1209頁(1987
年)に記載されている。
導入する多くの同じ方法は、MRI剤を特願平1-508790号
の発明の抗体複合体に導入し、感染病変に対する影像剤
を形成させるのに適していることがよく理解できるであ
ろう。MRI剤は、多くの常磁性イオン又は影像強化用の
基を都合よく有する。複数のそのようなイオンを導入す
る1つの方法は、キレートを担体ポリマーに載せ、その
担体を抗体複合体に、好ましくは、前記複合体の抗原結
合部位から離れた部位で部位特異的に連結させる方法で
ある。この方法は、多くのキレート化剤が抗体のより少
ない部位において抗体に結合でき、免疫反応性がひどく
譲歩されないという長所を有する。抗体にキレート化剤
を載せるのに有用なポリマーの例として例えば、ポリオ
ール、多糖類、ポリペプチド等があげられる[米国特許
第4,699,784号{シフ(Shih)等}及び第4,046,722号{ロ
ウランド(Rowland)}を参照]。多糖類の1つの型はデ
キストランである。キレート化剤は、より機能的にさ
れ、デキストランヒドロキシルの方に反応基、たとえば
無水物、イソシアネート又はイソチオシアネート等を含
有する。その代わりに、デキストランは、多くの方法、
例えばアミノデキストランへの変換により誘導される。
下記でより詳しく述べるように、同様な方法が、複数の
薬物分子を抗体に載せるのに有用であることが正しく認
識されるであろう。
体を製造する方法は、通常、デキストランポリマー、都
合よくは、平均分子量(MW)が約10,000-100,000、好ま
しくは約10,000-40,000、より好ましくは約15,000のデ
キストランから出発する。デキストランを次に酸化剤と
反応させ、そのカルボキシレート環の一部の制御された
酸化をもたらし、アルデヒド基を発生させる。その酸化
は、従来の方法によって糖分解的化学的作用剤、例えば
NaIO4で都合よく達成される。

らす。

キシジアミンと反応させる。適したアミンは、例えばエ
チレンジアミン、プロピレンジアミン又は同様のポリメ
チレンジアミン、ジエチレントリアミン又は同様のポリ
アミン、1、3−ジアミノ−2−ヒドロキシプロパン又
は他の同様のヒドロキシル化ジアミン又はポリアミン等
を含む。アルデヒド官能基のシッフ塩基(イミン)基へ
の、実質的に完全な変換を確保するためにアルデヒド基
に比較して過剰のアミンが用いられ得る。

とによって達成される。イミン基の第2アミン基への実
質的に完全な還元を、そして非反応アルデヒド基のヒド
ロキシル基への還元を確保するために過剰の還元剤が用

ラムに通して、架橋デキストランを除去することによっ
てさらに精製できる。AD上の利用できるアミノ基のおお
よその最初の数の推定は、秤量された試料のトリニトロ
ベンゼンスルホン酸との反応及び420nmでの光学濃度の
標準との相関により達成される。この方法は通常、計算
された数のアルデヒド基のAD上の初期アミン基への本質
的に完全な変換を生じる。

る。この態様において、抗体/抗微生物剤結合体は、先
の段落で詳述した方法によって最初に製造される。ほと
んどの放射性核種が比較的短い半減期を有するために、
抗体/抗微生物剤/放射性核種結合体は、通常投与直前
に製造される。
ー、例えばビスマス−212及びアスタチン−211;βエミ
ッター、例えばイットリウム−90、レニウム−186、レ
ニウム−188、銅−67及びヨウ素−131;及びその代わり
に、ヨウ素125のような電子捕獲核種があげられる。
影法は哺乳動物に、好ましくはヒトに非経口的に放射性
元素で標識した多特異抗白血球抗体複合体をシンチグラ
ム造影に有効な量注射することによって行われる。非経
口的にとは、例えば静脈内、動脈内、鞘内内(intrathec
ally)、組織間腔内(interstitially)又は体腔内にとい
うことを意味する。被験者は、放射性同位元素で標識さ
れた結合体を約1mCi−50mCiの量受ける。その量は特定
の放射性核種及び投与の形態の関数である。静脈内注射
ではその量は、通常、約2−10mCi、好ましくは約2−
5mCiのI-131、約5−10mCi、好ましくは約8mCiのI-12
3、約10−40mCi、好ましくは約20mCiのTc-99m、約2−
5mCi、好ましくは約4mCiのIn-111又はGa-67である。
抗体複合体は、シンチグラム造影法影像剤を白血球を含
む感染性又は炎症性病変への標的にするための、哺乳動
物用の注射製剤として、好ましくは、ヒト用の無菌の注
射製剤として、都合よく供給され、前記注射製剤は、生
理的pH及び濃度で、薬学的に許容できる無菌の注射用
賦形剤、好ましくはリン酸緩衝塩水溶液(PBS)中に有効
量の放射性同位元素で標識された複合体を含有する無菌
の注射溶液から成る。非経口投与の部位のために必要と
する、他の従来の薬学的に許容できる賦形剤を用いるこ
とができる。
与される代表的な製剤は、通常、例えば、0.9%塩化ナ
トリウムを含む0.04Mリン酸緩衝溶液[pH7.4、バイ
オウエア(Bioware)]ml当たり約10mgのヒト血清アルブ
ミン[1%米国薬局方、パークデービス(Parke-Davi
s)]も有利に含む無菌溶液中に放射性同位元素で標識し
た多特異抗体複合体を約0.1−20mg、好ましくは約2mg
含有する。

ラを使用することによって、又は炎症又は病変の起源を
突き止めるために外部的に又は内部的に用いる手で支え
るガンマプローブ(gamma probe)スキャニングが達成さ
れる。シンチグラムは、通常、50−500 KeVの範囲のエ
ネルギーを探知する1以上の窓を有するγ線影像カメラ
により撮られる。標的部位は、比較的濃厚な病巣に存在
する白血球を有する感染病変、炎症性堆積物又は肉眼で
見えない病変のいずれかになり得る。白血球を含有する
病変の局部限定は、病変への標識化白血球の侵入と同様
に非経口投与時に多特異抗体−作用物質複合体と病変に
存在する白血球との反応性によって直接起きる。
素でなくMRI強化物質を含有する以外はシンチグラフィ
ー影像と類似の方法で達成される。磁気共鳴現象をシン
チグラフィーと異なる原則で達成させてもよい。通常
は、発生する信号は、像形成される病変の水分子の水素
原子の核中の陽子の磁気モーメントの緩和回数と相関す
る。磁気共鳴影像増強剤は緩和割合を増加させることに
よって作用し、それによって影像剤が付着した病変中の
水分子と体の他の場所の水分子とのコントラストを増
す。しかし、その作用剤の効果は、T1及びT2の両方を
増加させる。前者はより大きなコントラストになり、後
者はより小さなコントラストになる。よって、その現象
は、濃度に依存し、通常最大の効力に対する常磁性物質
の最適濃度がある。最適濃度は用いる特定の作用剤、影
像の中心、影像の形態、例えば、スピンエコー、飽和回
復(saturation-recovery)、反転回復(inversion-reco
very)及び種々の他の強いT1依存又はT2依存影像技術
によって、そしてその作用剤を溶解させる又は懸濁させ
る媒体の組成によって異なる。それらの因子及び相関的
重要性は当技術分野において公知である(例えば、ピケ
ットによる前記文献及びランゲ等による前記文献)。
動物、好ましくはヒトに非経口的に多特異抗白血球抗体
複合体とMRI増強剤の、この発明による複合体の磁気共
鳴影像用の有効量注射することによって行う。患者(被
験者)は、標識した複合体を、病変部位でのMRI信号を
少なくとも約20%、好ましくは50−500%増強させるの
に十分な量投与される。その量は、特定の常磁性物質及
び投与形態の相関関係である。
抗体複合体は、白血球を含む感染性又は炎症性病変に対
するMRI剤を標識するための、哺乳動物用の注射製剤と
して、好ましくは、ヒト用の無菌の注射製剤として、都
合よく供給され、無菌注射溶液は、生理的pH及び濃度
で、薬学的に許容できる無菌の注射用賦形剤、好ましく
はリン酸緩衝塩水(PBS)中に有効量の放射性同位元素で
標識された複合体を含有する無菌の注射溶液から成る。

的に許容できる賦形剤を用いることができる。

第2の抗体は第1の抗体複合体の局在化割合を少なくと
も約20%、一般的には50%以上高めるような量で用いら
れる。
標的区域から除去(clear)される複合物を形成するため
に第2抗体が第1抗体結合体と結合できる限り、第2の
抗体は、完全IgG又はIgM、又はIgG又はIgMのフラグメン
トでよい。好ましくは、第2の抗体は完全IgG又はIgMで
ある。第1の抗体がIgG又はIgMのフラグメントである場
合、第2の抗体は、第1/第2複合物が補体カスケード
を活性化する能力を維持するように完全IgG又はIgMであ
るのが好ましい。逆に、第1の抗体が完全IgGである場
合、もし複合体が補体結合能力をなお維持するなら第2
の抗体はフラグメントであり得る。少なくとも1つの第
1/第2の一対は完全IgG又はIgMであるのが好ましい。
IgMを用いる1つの長所は、IgMが第1の抗体又は分離し
た結合体、すなわち、薬剤、キレート化剤、放射性核種
等のような診断及び/又は治療主成分と高分子量複合体
を形成することである。このことは、特に血液からの、
非標的第1抗体及び/又は主成分のクリアランスの割合
及び効率を増加させる。第2の抗体は前記のゴールデン
バーグの米国特許第4,624,846号に記載の方法によって
製造される。モノクローナル抗種IgGも又、本方法にお
ける第2抗体として都合よく用いられる。非金属性結合
体、例えば、放射性ヨウ化結合基又はニトロキサイド
(nitroxide)のような有機常磁性物質も又、第2抗体が
特異的なヘプテンになり得る。

ましくは、約24−48時間後投与で第2抗体を被験者に注
射する。第1抗体が静注投与されなければ、第2抗体の

リアランスを促進するために第2抗体の少なくとも一部
は静注投与されるのが有利である。

められている技術である。その抗体は、画像化に要する
時間の間、第1抗体と実質的に同じ、分布及び代謝の動
態を有する。そのような抗体の注射は、バックグラウン
ドを補正することによって画像処理を増大させるのに用
いるのにとりわけ適した、Tc−99m−標識化血清アルブ
ミンのようなサブトラクション剤と比較して好ましい。
サブトラクション剤としての、放射性元素で標識した無
作用の抗体の使用は、循環系からの抗体のクリアランス
を有効にする臓器からの非標的バックグラウンド放射の
ためのコンピューター補正をさせる。第2抗体が第1モ
ノクローナル抗体及び非標的区域からの無作用の抗体免
疫グロブリンを実質的に同じ割合で消去するように第1
モノクローナル抗体及びサブトラクション剤として用い
られる無作用の抗体は、好ましくは同じ種または骨髄腫
/ハイブリドーマからのものであることは当業者に正し
く認識されるであろう。第2抗体は第1のそして無作用
の免疫グロブリン種の一定の領域に特異的であることが

見出だされる。そのような抗生物質薬剤は抗白血球抗体
と結合され、本発明による治療剤を形成し、そしてその
部位での有効濃度を増加させるように感染病変への抗生
物質薬剤の標的を改良するそのような結合体の使用は本
発明の一部である。1以上の抗生物質は、治療用に、ポ
リマー担体に結合され、その担体は次に抗体に結合され
る。ある場合において、抗体の結合体の部分としての薬
剤を一部又は完全に非毒性化することが可能であり、一
方、体に循環しているときに薬剤の組織的副作用を減ら
し、そしてその薬剤の組織的投与が容認できないときに
その使用を可能にすることができる。ポリマーに結合
し、そのポリマーがさらに抗体に結合した薬剤のより多
い分子の投与は組織的毒性を移動させながら治療を可能
にする。本発明の方法体系は、第1抗体を抗生物質薬剤
に結合することによって感染病変の治療処置に適用でき
る。抗生物質薬剤を免疫グロブリンに結合する、当技術
分野で認められた方法は、例えば当技術分野で公知の、
ドラッグ・キャリアース・イン・バイオロジィ・アンド
・メディスン(Drug Carriersin Biology and Medicine
s)中の「ザ・ユース・オブ・アンティボディース・アズ
・ドラッグ・キャリアース(TheUse of Antibodies as D

虫剤、抗糸状菌剤及び関連薬剤、例えばスルホンアミド
類、ペニシリン類及びセファロスポリン類、アミノグリ
コシド類、テトラサイクリン類、クロラムフェニコー
ル、ピペラジン、クロロキン、ジアミノピリジン類、メ
トロニアジッド、イソニアジッド、リファンピン類、ス
トレプトマイシン類、スルホン類、エリスロマイシン
類、ポリミキシン類、ナイスタチン、アンフォテリシン
類、5−フルオロサイトシン、5−ヨ−ド−2´−デオ
キシウリジン、1−アダマンタンアミン、アデニンアラ
ビノシッド、アマニチン類及びアジドチミジン(AZT)
が、適した複数特異抗体/複数フラグメントに結合させ

量が示されれる。特に、イットリウム−90の通常量は1
投与量20−30 mCiであり、一方、ヨウ素−131は40−100
mCi投与される。他の放射性元素及びホウ素結合体の用
量は、熟練した臨床医により慣習的に決定される。

な他の従来の薬学的に認容される賦形剤を用いることが

を無菌溶液中に含有する。

剤及び治療的放射性核種と結合した抗白血球抗体は、本
明細書中に記載された影像剤及び薬剤との結合体を製造
するために記載された方法によって製造される。両治療
剤の多特異抗体複合体への結合は、単独で投与されると
きの治療剤の通常用量の50−100%の用量にする付加的
なそして相乗的な効果を生じる。イオン化放射物は遠距
離において、炎症性及び感染性病変及びそれらの原因物
質、例えば細菌、糸状菌及びウイルスの「バイスタンダ
ー(bystander)」放射物を備えることによって効果的で
あることが示されてきた。

療的製剤は非経口投与用の適した賦形剤中の単特異抗体
/フラグメントに結合した治療的に有効な放射性核種を
含み得る。他に、治療的製剤は、非経口投与用の適した
賦形剤中の、治療的抗微生物剤及び治療的放射性核種に
結合した単特異抗白血球抗体/フラグメントを含み得
る。治療的製剤は又、上記のような別に充填された第2
抗体も含み得る。適した賦形剤とは、当技術分野におい
てよく知られており、例えば先に述べたように、診断的
影像剤の投与用に用いられる無菌PBS溶液の類似物を包
含する。
治療結合物質は、白血球の中心を含む、感染性又は炎症
性病変を標的とする抗体用の治療的又は診断的キットに
おいて都合よく提供される。典型的にはそのようなキッ
トは、凍結乾燥した製剤として又は注射用賦形剤中の本
発明の抗体結合体を含有するガラス瓶から成り、治療的
結合剤は、すでに単特異抗体/フラグメントと結合させ
た常磁性体種又は抗生物質とともに一般に供給される。
抗微生物剤及び放射性核種から成る治療的結合体は一般
に、単特異抗体/フラグメントにすでに結合させた抗微
生物剤として、放射性同位元素使用標識用試薬及び付属
物質とともに別々の容器で供給される。そのキットはさ
らに、第2の別々に包装された標識されていない、抗体
又は抗体フラグメント又は治療薬剤に特異的な抗体又は
抗体フラグメント、そのための担体又は放射性核種又は
常磁性イオン用のキレート化剤を含有する。影像製剤及
び本発明の方法は、少なくともIn−111で標識した白血
球を用いる消失膿瘍の決定用の従来の物質と同様に効果
があり、コスト、試薬の潜在的毒性及び最も重要な使用
容易さに関しては明らかにより有利である。治療的薬剤
及び本発明の方法は、抗生物質試薬及び/又は放射性核
種を用いて炎症及び微生物感染の部位を標的にする手段
を与え、薬剤及び放射性核種の治療係数を改良し、組織
的副作用を減らし効力を高める。
を用いて本発明を完全に用いることができると考える。
従って、下記の好ましい特定な態様は単なる例示的なも
のと解釈されるべきであり、他の記載を何ら制限するも
のではない。下記の実施例において、すべての温度は他
に示されなければ℃で訂正されないで示され、すべての
部及び百分率は重量によって示される。
抗生物質の治療に反応しなかった両側性(bilateral)肺
炎が現れた。臨床的所見及び痰細胞学では進行しそして
深刻な呼吸困難を起こしているニューモサイスチス(Pn
eumocystis)カリニイ肺炎であることを示唆した。顆粒
性白血球と単核白血球抗原に結合するモノクローナル抗
体の混合物は、約15,000分子量(MW)であり、平均約25
分子のトリメトプリム(ウエルカム社製)を有する平均
1つのアミノデキストラン担体にそれぞれ部位特異的に
結合され、メソトレキセート誘導体がその担体にシッフ
等の前記特許の方法学を適用することによってその担体
に結合された。抗体複合体混合物の約25mgのゆっくりの
静脈内点滴により肺病変に治療用量の抗生物質を運び3
日毎日続けて繰り返した。2日後、患者は改善を示し始
め、熱は低くなり、このことは3日後に胸のX線像に改
善により確認された。肺炎のほとんどが抗体−薬剤結合
体処置の2週間以内に消失した。患者はこの間、一般
の、体力を持たせるのに有効な処置を受けていた。ホウ
素結合体を含有する治療的放射性同位元素及び/又は放
射性同位元素及び薬剤の組み合わせがトリメトプリムの
代わりに又はそれに加えてこの患者の治療及び類似の条
件のために用いられた。

同様に成功して繰り返すことができる。

Claims (10)
- 【請求項1】 少なくとも1つの、免疫反応性の少なく
とも1つの白血球細胞型に特異的に結合する実質的に単
特異抗体又は抗体フラグメントを含有し、治療的抗微生
物剤、ホウ素含有アデンド(addend)又は放射性核種に
結合された、白血球集積(accretion)の中心を標的とす
るための治療的抗白血球結合体。 - 【請求項2】 前記免疫反応性の実質的に単特異抗体又
は抗体フラグメントが、抗菌剤、抗ウイルス剤、抗真菌
剤および駆虫剤からなる群から選択される治療的抗微生
物剤に結合された、請求項1に記載の抗白血球結合体。 - 【請求項3】 前記免疫反応性の実質的に単特異抗体又
は抗体フラグメントが治療的放射性核種に結合された、
請求項1に記載の抗白血球結合剤。 - 【請求項4】 前記免疫反応性の実質的に単特異抗体又
は抗体フラグメントが複数のホウ素−10原子を含むアデ
ンドに結合された、請求項1に記載の抗白血球結合体。 - 【請求項5】 前記免疫反応性の実質的に単特異抗体又
は抗体フラグメントが治療的抗微生物剤及び治療的放射
性核種又はホウ素含有アデンドに結合させた、請求項1
に記載の抗白血球結合体。 - 【請求項6】 前記白血球細胞型の各々が顆粒性白血
球、単核白血球、B−リンパ球又はT−リンパ球から成
る群から選択される、請求項1乃至5のいずれか1請求
項に記載の抗白血球結合体。 - 【請求項7】 前記抗体又は抗体フラグメントの各々が
モノクローナル抗体又は抗体フラグメントである、請求
項1乃至6のいずれか1請求項に記載の抗白血球結合
体。 - 【請求項8】 別々の製剤として、(a) 標的用としての
有効量の請求項1乃至7のいずれか1請求項に記載の抗
白血球結合体及び(b) 前記抗白血球結合体の局在化割合
を少なくとも約20%増加させるか、又は治療的結合体の
治療係数を2−72時間以内に少なくとも約10%増加させ
るのに十分な量の、前記抗白血球結合体に特異的に結合
する第2の標識されない抗体又は抗体フラグメントを含
む作用剤。 - 【請求項9】 1以上の免疫反応性の実質的に単特異抗
体又は抗体フラグメントを、標的用とするために有効な
量、含み、治療のために有効な量の治療用抗微生物剤、
ホウ素含有アデンド又は放射性核種に結合された、抗白
血球結合体及び薬剤的に許容できる賦形剤を含む、炎症
性又は感染性病変の治療用の及び前記病変を有する哺乳
動物の非経口的注射用剤。 - 【請求項10】 前記注射用剤が、別々の製剤として、
前記抗白血球結合体に特異的に結合する第2の標識され
ていない抗体又は抗体フラグメントを抗白血球結合体の
局在割合を2乃至72時間以内に少なくとも約20%増加さ
せるのに十分な量又は、治療的結合体の場合は、その治
療係数を少なくとも約10%増加させるのに十分な量、付
加的に含む、請求項9に記載の非経口的注44射用剤。
