Abstract
Description
本願は、2016年3月22日に出願され、「抗LGR5モノクローナル抗体の投与」と題された米国特許仮出願第62/311,631号(参照することにより本明細書に組み入れられるものとする)の優先権を主張する。
本発明は、概して、がん生物学の分野に関する。本明細書中に提供される方法および組成物のいくつかの実施形態は、ロイシンリッチリピート含有Gタンパク質共役受容体5(LGR5)と特異的に結合するヒト化抗体またはその抗原結合性フラグメントを投与して特定のがんを治療することに関する。
本願は、電子フォーマット式配列リストと共に出願されるものである。配列リストは、約40Kbのサイズであり、2017年3月8日に作成し、BIONO14WOSEQLISTINGという題のファイルとして提供される。この配列リストの電子フォーマットの情報は、その全文を参照することにより本明細書に組み入れられるものとする。
体の用量、および適切な医薬用担体を含む医薬組成物を収容する容器であって、モノクローナル抗体の用量が約2.5mg/kg〜約15mg/kgである該容器が挙げられる。いくつかの実施形態では、医薬組成物は、静脈内投与に適している。
ンダムに標識化されたマウスと交配したストラテジーを使用して、これらのマウス腸管腫瘍の起源または根源であると確認された。APC欠失誘導4週間後の単色腫瘍(すなわち、全GFPまたは全RFP)の外観は、これらの腫瘍が1つのLGR5幹細胞に起因することを確証させた。さらに、このモデルは、LGR5幹細胞中の蛍光遺伝子タグが、異なる色に反転することを可能とし、その結果、赤い腫瘍を生成するRFP+LGR5がん幹細胞は、まだ腫瘍を播種しているが以前の全ての赤いGFP+腫瘍塊に浸潤する青い腫瘍細胞を今生じているECFP+LGR5がん幹細胞への途中の流れの中で形質転換することができた。この反転実験は、LGR5 CSCが腸管腫瘍の成長を開始および播種することができる腸管腫瘍の起源であることだけでなく、これらが腫瘍形成を継続的に維持する(すなわち、長期再構築能を有する)ことのさらなる確証を提供した。

実施形態では、抗体は抗体18G7H6A3であり、 配列番号14の軽鎖配列を含む。他の実施形態では、抗体は抗体18G7H6A3であり、 配列番号21の軽鎖可変ドメインを含む。
部または部分に沿ってランダムに変異を導入することができ、得られた変異体を生物活性に対してスクリーニングして活性を保持する変異体を同定することができる。変異誘発後、コードされた抗体を発現させることができ、抗体活性を決定することができる。
いくつかの実施形態は、抗LGR5抗体を用いたインビトロおよびインビボでのがん肝細胞成長の遮断に関する。いくつかの実施形態では、肝細胞増殖の遮断方法は、がん幹細胞への抗LGR5抗体の有効量を投与することを含み、該抗LGR5抗体の有効量は該がん幹細胞の成長を低下させるのに充分である。

本明細書中提供されるLGR5と特異的に結合するヒト化モノクローナル抗体またはその抗原結合性フラグメントを、投与に適した医薬組成物に組み込むことができる。このような組成物は、通常、ヒト化モノクローナル抗体またはその抗原結合性フラグメントおよび薬剤的に許容可能な担体を含む。本明細書中で使用されるとき、「薬剤的に許容可能な担体」という語は、医薬の投与に適合した、いずれかおよび全ての溶媒、分散媒体、コーティング、抗菌剤および抗真菌剤、等張および吸収遅延剤などを含むことを意図している。適切な担体は、本分野の標準参考書であるRemington’s Pharmaceutical Sciences(参照することにより本明細書に組み入れられるものとする)の最新版に記載されている。このような担体または希釈剤の好ましい例としては、水、生理食塩水、リンゲル液、デキストロース溶液、および5%ヒト血清アルブミンが挙げられるが、これらに限定されない。不揮発性油などのリポソームおよび非水性ビヒクルを使用してもよい。医薬活性物質のためのこのような媒体および薬剤の使用は、当技術分野で周知である。従来の媒体または薬剤が活性化合物と相溶性でない限りを除いて、組成

方法、組成物およびキットのいくつかの実施形態は、がんを患っている対象の治療方法を含む。いくつかのこのような方法は、LGR5と特異的に結合するヒト化モノクローナル抗体またはその抗原結合性フラグメントの有効量を、それを必要とする対象に投与することを含む。該対象は、哺乳類、例えば、ヒトであり得る。
モノクローナル抗体またはその抗原結合性フラグメントの投与量は、約2.5mg/kg〜約20mg/kg、または約2.5mg/kg〜約15mg/kgである。
ヒト生殖系列配列を、マウス抗体18G7.1のヒト化のため、アクセプターフレームワークとして使用した。密接な生殖系列配列を見出すために、最も類似の発現軽鎖および最も類似の重鎖を、NCI IgBLAST(ncbi.nlm.nih.gov/igblast/)により生殖系列配列データベースにおいて特定した。この検索において、18G7.1のCDR配列をマスクした。最適な発現配列の選択は、カノニカル(canonical)残基および界面(interface)残基の配列同定についてのチェック、ならびにCDRループ長の類似性についてのチェックを含んだ。
作製し、次いで、分子エネルギー最小化を行った。コンピュータソフトウェアPymolを用いた構造解析を使用して、適切なフォールディングに強力にネガティブに影響を及ぼし得る残基を特定した。
選択されたヒト化変異体18G7H6の親和性を向上するために、アラニン系統的変異導入法および飽和変異誘発の組合せを使用した。重鎖CDR1ならびに軽鎖CDR1およびCDR3中の残基をアラニンに変異させ、HEK293細胞にトランスフェクトして、得られた条件培地をフローサイトメトリーによりLGR5抗原結合活性についてアッセイした。重鎖CDR3に対して飽和変異誘発を行い、CDR3中の全ての残基をその位置におけるオリジナルアミノ酸同一性を除いて、19の天然アミノ酸の各々に変異させた。変異のそれぞれをHEK293細胞にトランスフェクトして、得られた条件培地をフローサイトメトリーによりLGR5抗原結合活性についてアッセイした。
18G7H6A1、18G7H6A3および18G7Chと名付けられたキメラ18G7.1(ヒトIgG1 Fcに融合された18G7.1由来のマウスFab)を構築し、増幅し、200mlの体積における発現評価のため、一過性トランスフェクションを用いてチャイニーズハムスター卵巣細胞(CHOK1SV GS−KO)に一過性にコトランスフェクトした。それから、18G7CHに対して5リットルならびに18G7H6A1および18G7H6A3の両方に対して2.5リットルの最終体積においてCHOK1SV GS−KO細胞の大規模一過性トランスフェクションを開始した。ワンステップのタンパク質Aクロマトグラフィーを用いて、清澄化された培養上澄み液を精製した。コントロールサンプルとして社内ヒト抗体を含む1mg/mlの濃度の精製物質を用いて、SE−HPLC、SDS−PAGEおよびエンドトキシン測定の形態での産生物の品質分析を行った。結果は、回収した産生物が高純度であることを示した(>95.7%)。
18G7H6A3抗体を発現する安定GS−CHOトランスフェクタントプールを、発
現ベクターp18G7H6A3/DGVを用いてCHOK1SV GS−KO宿主細胞のトランスフェクションにより作製した。この抗体をコードする遺伝子を含むDGVを構築し、トランスフェクトして、その後、得られたクローナルセルラインを、FACS法を用いてトランスフェクタントプールのシングルセルソーティングにより生成した。クローニング中に作製した96ウェルプレートを、シングルコロニーの存在について毎週スクリーニングした。約2週間後、1000コロニーからの上澄みをOctet(登録商標)システム法を用いて抗体産生についてスクリーニングした。スクリーニングした1000コロニーのうち、991コロニーは検出可能レベルの抗体を産生した。Octetデータを順位づけし、最も高く産生したコロニーを進行のため選択した。
FACSベースアッセイを使用して、CHO細胞表面で過剰発現された組換えヒトLGR5と、精製18G7H6A1および18G7H6A3の結合を測定した。CHOおよびCHO−LGR5細胞を、4℃において18G7H6A1または18G7H6A3の連続希釈で染色し、表面染色をPE結合抗ヒトIgG二次抗体で検出して、FACScaliburで分析した。ヒトLGR5結合に対する18G7H6A1および18G7H6A3のEC50は、<10nMであった。抗体コントロール(MOPC)をこの実験におけるネガティブコントロールならびにLGR5を含まない野生型CHOとして使用した。18G7H6A3は、野生型CHOとの結合を示さず、アイソタイプコントロールはヒトLGR5とのいかなる測定可能な結合を示さなかった。
ヒトLGR5と、18G7H6A1および18G7H6A3の結合を、ELISAベースプレート結合アッセイを用いてインビトロでアッセイした。アッセイは、ELISAプレート結合精製組換えLGR5外部ドメイン−IgG−Fc融合物と結合している抗体を
、西洋わさびペルオキシダーゼ共役抗ヒトIgG−CH1二次抗体を含むLGR5結合抗体の検出で測定した。ヒトLGR5−Fcに対する18G7H6A3のEC50は、300pMであることが分かった。
異なるレベルのLGR5を発現するヒトがんセルラインに対する18G7H6A3の結合特性をフローサイトメトリーにより分析して、異種性腫瘍集団に対する18G7H6A3のあり得る標的化特性を確定した。複数の腫瘍セルラインにおけるLGR5の発現レベルを、フローサイトメトリーにより定量化した。
CT1原発性CRC異種移植モデルは、ステージIV転移性結腸がん患者由来であった。この腫瘍のDNAシークエンシングは、K−Ras、PI3K、PTEN、p53およびAPCを含む複数の遺伝子における通常の結腸がん変異と同定した。無血清条件下の培養液中に維持される低継代のCT1腫瘍様塊を、0日目にマトリゲル中のSCID/Bgマウスに皮下注射し、腫瘍サイズおよび体重について週2回モニターした。25日目、腫瘍が120mm3に達した場合、CT1皮下腫瘍を、10匹のマウスの群にランダムに分けた。マウスを、PBS、抗体コントロールMOPC、18G7H6A1、18G7H6A3またはヒト/マウスキメラ18G7Chのいずれかで治療した。マウスに、2.5週間、15mg/kgで週2回投与した(合計5回投与)。
CT3原発性CRC異種移植モデルは、K−Ras、H−Ras、APC、PI3K、PTEN、STK11、RB1、TP53、FGFR2、VANGL2、およびISCOにおける変異を有するステージIII mCRC患者由来であった。低継代凍結保存CT3原発性異種移植腫瘍フラグメントを、5匹のSCID/Bgマウスに移植した。5匹のCT3原発性異種移植片担持SCIDマウスからプールした平均約1150mm3の腫瘍を、移植後41日目に取り出し、分離してマトリゲル中のCB.17 SCIDマウスの皮下に再移植し、腫瘍サイズおよび体重について週2回モニターした。腫瘍が平均130mm3に達した場合、マウスをランダムに分けた(移植後34日目)。マウスを、PBS
、抗体コントロールMOPC、18G7H6A3、18G7H6A1またはヒト/マウスキメラ18G7Chのいずれかで治療した。34日目でスタートし、マウスに、2.5週間(5回投与)、15mg/kgで週2回投与した。全マウスを、体重および腫瘍サイズ、ならびに健康全般および外観について、終了するまで、週2回モニターした。
CB.17 SCIDマウスに、CSC条件下成長したCT3細胞を移植した。移植後40日目、腫瘍が約160mm3に達した場合、i)PBS、ii)15日間5日毎(合計3回投与)のFolFiri(5FU 30mg/kg、ロイコボリン 90mg/kgおよびイリノテカン 24mg/kg)、ならびにiii)FolFiri(iiと同様に)および18G7H6A3(15mg/kg、週2回)を含む治療群にランダムに分けた。腫瘍体積の分析から、18G7H6A3およびFolFiriの併用は、FolFiriレジメンと比較してCT3腫瘍の成長を低下させることが分かった。併用治療は、61日目、65日目、68日目、71日目および75日目の腫瘍体積を、それぞれ、約58%、53%、45%、33%および37%減少させた(図2)。
単剤または標準治療との併用における18G7H6A3の有効性を評価するため、膵がん異種移植モデルを試験した。CB.17 SCIDマウスに、AsPC−1細胞を移植した(マトリゲル+RPMI(比1:1)中)。移植後20日目、腫瘍を5つの群:i)PBS、ii)MOPC(15mg/kg、週2回、腹腔内)、iv)ゲムシタビン(90mg/kg、週2回、腹腔内)ならびにv)上記投与でのゲムシタビンおよび18G7H6A3の同時併用にランダムに分けた。
このインビボ試験を、低継代トリプルネガティブ乳がん細胞(ER−、PR−、非HER2過剰発現)を用いて行った。MDA−MB−231−LM3細胞を、DMEM/10
%FBS/Anti−Anti培地を含む接着培養液中で維持した。0日目、CB.17
SCIDマウスに、RPMI:マトリゲル(1:1)中のMDA−MB−231−LM3細胞を、4番目の乳腺脂肪体に注射し、腫瘍サイズおよび体重について週2回モニターした。27日目、腫瘍が約155mm3に達した場合、MDA−MB−231−LM3腫瘍を、10匹のマウスの4つの群にランダムに分けた。マウスを、PBS、抗体コントロールMOPC、または18G7H6A3で治療した。マウスに、3.5週間、15mg/kgで週2回投与した(7回投与)。抗体18G7H6A3は、PBS(60.7%腫瘍成長阻害)またはMOPC抗体(49.3%腫瘍成長阻害)コントロールと比較して有意な抗腫瘍活性を示すことを見出した(図3)。
DLD1、HCT116、LS174t、LoVo、SW48、SW480およびSW620を含むCRCセルラインのパネルを、PI3K/mTOR二重阻害剤(NVP)またはSN38(イリノテカンの活性代謝物)もしくは5FU(5−フルオロウラシル)を含む2つの異なる細胞傷害性薬物で治療した。細胞を1uMで上記薬剤を用いて治療し、72時間後に回収した。それから、細胞をAlexa Fluor647と結合した抗LGR5 Mabで染色し、FACScaliburを用いてフローサイトメトリーによりデータ解析した。
上記発見をさらに発展させるため、CRCセルラインに加えて、nab−パクリタキセル、ゲムシタビンおよびタキソールを含む関連標準治療、ならびにPI3K、MEKおよびGSK3βの阻害剤などの膵がんにおける大部分の関連経路を標的化する小分子阻害剤も用いて治療された一連の膵臓セルラインにおいて、LGR5の発現を調査した。試験された膵臓セルラインとしては:AsPc1、HPAFII、PANC1、BxPC3、CFPAC、PANC10.05、Capan2およびSW1990が挙げられる。nab−パクリタキセルを用いた治療は、フローサイトメトリーにより評価されるとき、PANC1、BxPc3およびPANC10.05におけるLGR5の上方制御をもたらす。ゲムシタビン治療は、PANC1におけるLGR5を上方制御し、タキソール治療はHPAFIIにおけるLGR5の発現の増加をもたらす。PI3K/mTOR治療は、CFPACにおけるLGR5の上方制御をもたらし、MEK阻害剤はHPAFIIおよびSW1990においてLGR5を上方制御する。
化学療法が大腸腫瘍においてLGR5の発現を変化させるか否かを調査するため、マウスを、5FU(30mg/kg、腹腔内)、ロイコボリン(90mg/kg)およびイリノテカンの2つの異なる用量(24mg/kgまたは8mg/kg)を用いて、5日毎に治療した。これらの試験結果は、CT3腫瘍は化学レジメンに感受性があるが、CT1腫瘍は完全に消失せず、レジメンに対する抵抗をいくらか示すことを示した(図4)。LGR5の発現のFOLFIRI治療の効果を検査するため、CT1およびCT3患者由来腫瘍およびLGR5の発現から、全mRNAを抽出し、qRT−PCRにより決定し、その対応するGAPDH転写産物から各サンプルのLGR5のCt値(サイクル閾値)を差し引いてDCT(ΔCt)値を得ることにより解析した。データは、2のDCT乗として表される。LGR5の存在量の分析は、対応する生理食塩水治療された腫瘍と比較して、CT1腫瘍(約2倍)およびCT3腫瘍(約3.5倍)の両方において、LGR5転写産物が増加することを示した。
膵がんに対する標準治療の化学療法治療が膵腫瘍においてLGR5の発現を変化させるか否かを調査するため、ゲムシタビンおよびnab−パクリタキセルの併用で、週2回マウスを治療した(JH109原発異種移植)。末端基分析において、腫瘍cDNAを用いたqRT−PCRデータは、対応する生理食塩水治療された腫瘍と比較して、化学療法治療腫瘍におけるLGR5の発現の著しい増加を示し、標準治療での治療が腫瘍細胞においてLGR5の上方制御をもたらすことを示している。
18G7H6A3の有効性も、膵がん異種移植モデルにおいて調査した。CB.17 SCIDマウスに、PANC1細胞(1E6/マウス、皮下、マトリゲル+RPMI(比1:1))を移植し、移植後41日目に治療群:i)PBS、ii)IgGコントロール(15mg/kg、週2回、腹腔内)、iii)18G7H6A3(15mg/kg、週2回、腹腔内)、iv)ゲムシタビン(90mg/kg、週2回、腹腔内)ならびにv)ゲムシタビンおよび18G7H6A3の同時併用(15mg/kg、週2回、腹腔内)にランダムに分けた。3週間に割り当てられた群において、ゲムシタビンを投与して腫瘍成長を阻害した。全マウスを、体重および腫瘍サイズ、ならびに健康全般および外観について、週2回モニターした。

セルラインに加えて、本発明者らは、単剤または膵がんのJH109原発性患者由来異種移植片モデルにおける標準治療との併用での18G7H6A3の有効性も調査した。JH109異種移植モデルは、5−FU、ゲムシタビン、エルビタックスおよび放射線療法を含む4つの治療レジメンを受けた患者由来である。元の患者腫瘍は、インビトロ培養に暴露しないで継続的に免疫不全マウス内で継代された。JH109モデルにおいて18G7H6A3の有効性を試験するため、腫瘍担持マウス(n=7)を、コントロールIgG(15mg/kg、腹腔内、週2回)、18G7H6A3(15mg/kg、腹腔内、週2回)単剤、標準治療の化学療法(ゲムシタビン(50mg/kg、腹腔内、週1回;およびnab−パクリタキセル 30mg/kg、静脈内、週1回)の併用、化学療法およびコントロールIgGの併用、ならびに化学療法および18G7H6A3の併用で治療した。単剤の18G7H6A3 mAbは、腫瘍成長に影響を及ぼさず、18G7H6A3とNab−パクリタキセルとの併用、およびゲムシタビン化学療法は、化学療法単独と比較して有意に大きな程度の腫瘍阻害をもたらした。化学療法と併用した18G7H6A3は、化学療法単独と比較して77%より強力に腫瘍成長阻害をもたらした。18G7H7A3化学療法併用で治療した3匹のマウスは、その腫瘍を完全に根絶した(測定可能な腫瘍は検出されなかった)。18G7H6A3化学療法併用群は、治療中断後でさえ腫瘍成長を抑制し続け、1匹のマウスは化学療法休止後3か月でも測定可能な腫瘍はなかった。この実施例では、化学療法(ゲムシタビン+nab−パクリタキセル)と併用で投与された場合に観察された18G7H6A3の有意な活性は、ゲムシタビンnab−パクリタキセル治療に応答して標的抗原LGR5の発現増加に起因し得るが、化学療法治療後インビボで原発腫瘍の再成長または再発を阻止して原発腫瘍の大部分(tumor bulk)を根絶することの実証である。
フローサイトメトリー分析のため、5つの個別の腫瘍由来細胞を、幹細胞特定マーカーCD44、およびCD166に対する様々な抗体で染色した。腫瘍を分離し、マウス細胞のため枯渇し、それから、生存可能な細胞についてカウントした。分離した細胞を、フローサイトメトリーによる細胞表面幹細胞マーカー発現の分析に使用した。
FolFiriと併用した18G7H6A3の効果を、結腸がんCT3モデル(実施例10)で試験した。この原発腫瘍有効性試験の結果は、3サイクルのFOLFIRI大群と併用した18G7H6A3は、腫瘍成長の低下において、FolFiri単独より効果的であることを示した。18G7H6A3 FOLFIRI併用レジメンががん幹細胞(CSC)発生頻度の低減においても効果的であるか否かを決定するため、78日目の腫瘍を回収し、分離し、プールして、腫瘍ナイーブCB17.Scidマウスの新しいコホートに、10、30、100細胞/横腹の限界希釈アッセイで再移植した。それから、マウスを、腫瘍成長について週2回モニターし、腫瘍はさらなる治療なしで成長を可能とした。
成能を非常に低下させた(図8)。加えて、18G7H6A3 FOLFIRI併用からの再移植細胞は、FOLFIRI単独と比較して、有意により遅い腫瘍成長プロファイルおよび進行までの時間の遅延(図9)を有した。最終的に、18G7H6A3治療は、40日目において、線形回帰分析によりがん幹細胞発生頻度を6倍低減した(1/856.3 18G7H6A3/FOLFIRI対1/138.6(FOLFIRI))。これらのデータは、FOLFIRIと併用した18G7H6A3は腫瘍開始またはがん幹細胞集団を効果的に標的化することを示している。68日目は、30細胞/動物データの最終日であった。データは、p=0.0039で有意である。
ゲムシタビンと併用した18G7H6A3の効果を、膵がんPANC1モデルで試験した。この試験は、ゲムシタビンと併用した18G7H6A3が、ゲムシタビン単独と比較してPANC1モデルにおける腫瘍成長を有意に阻害することを示した。これらの治療群からの腫瘍細胞を回収し、分離し、プールして、限界希釈アッセイ(500、1500、4500または13500細胞/動物)においてCB.17 SCIDマウスの新しいコホートに再移植し、治療なしで成長させた。
パクリタキセルと併用した18G7H6A3の効果を、トリプルネガティブ乳がんMDA−MB−231−LM3モデル(実施例12)で試験した。この試験は、パクリタキセルと併用した18G7H6A3は、パクリタキセル単独と比較して、腫瘍成長において最小の相加的阻害を有した。これらの腫瘍を回収し、分離し、プールして、10、30、100細胞/横腹で限界希釈アッセイにおいてCB.17 SCIDマウスの新しいコホートに再移植し、治療なしで成長させた。
細胞は、パクリタキセル単独と比較して、有意により遅い腫瘍成長プロファイルおよび進行の遅延時間を有した。最終的に、18G7H6A3+パクリタキセル治療は、線形回帰解析により、がん幹細胞発生頻度を低減した。これらのデータは、パクリタキセルと併用した18G7H6A3は腫瘍開始またはがん幹細胞集団を効果的に標的化することを示している。
大腸がん患者の肝転移由来の低継代大腸がん細胞(BMCRC086)を用いて、インビボ試験を行った。0日目、BMCRC086細胞を解凍し、RPMI:マトリゲル(1:1)に懸濁し、CB.17 SCIDマウスの腹に皮下注射した。腫瘍サイズおよび体重について、動物を週2回モニターした。7日目、マウスを、PBS、18G7H6A3、FOLFIRIまたは18G7H6A3と併用したFOLFIRIで治療した。マウスに、7.5週間15mg/kgで週2回、PBSおよび18G7H6A3を投与した(16回投与)。マウスに、4週間、FOLFIRI(7日目、12日目、17日目、22日目、27日目および32日目に30mg/kgフルオロウラシルおよび90mg/kgロイコボリン;8日目、13日目、18日目、23日目、28日目および33日目に24mg/kgイリノテカン)を投与した(6回投与)。FOLFIRIと併用した18G7H6A3は、FOLFIRI単独と比較して、有意な抗腫瘍活性を示した(図10)。
インビボ腫瘍有効性試験において結腸がんCT1(実施例8)およびCT3(実施例9)からの18G7H6A3治療した腫瘍を、ウェスタンブロット解析により特徴づけした。各治療マウスからの腫瘍サンプル(群当たりn=5〜10マウス)を殺処分後切除し、液体窒素冷却した乳鉢内で直ちに凍結し、乳棒で粉砕(凍結粉砕)し、液体窒素中で急速冷凍して使用するまで−80℃で貯蔵した。凍結粉砕した腫瘍を、時々ボルテックスしながら、氷上で30分間、氷冷溶解緩衝液(ホスファターゼおよびプロテアーゼ阻害剤を含有するRIPA緩衝液を減らして)で溶解した。腫瘍ライセートタンパク質を含む上澄みを、SDS−PAGEゲル上で分析し、次いで、Wntシグナルタンパク質(およびそのリン酸化体)の数についてウェスタンブロッティングした。多くの治療群間の有意差を、CT1およびCT3腫瘍のウェスタンブロットで観察した。図11ではホスホ−Thr41/Ser45−β−カテニン(Wntシグナルタンパク質)は、18G7H6A3がインビボで腫瘍細胞におけるLGR5シグナル伝達を阻害することができることを示すタンパク質の不活性型、およびその後の分解型のマーカーである。
親HEK−293T細胞およびLGR5を安定に発現するHEK−293T細胞に、TCF−LEFレポーターベクター含有レンチウイルス(GFP Cignal、QIAGEN)を形質導入し、レポーターの安定発現について選択した。親およびLGR5の発現安定レポーター系統を、96ウェルプレート中25,000/ウェルで播種し、オーバーナイトで取り付け、血清飢餓させて、抗体またはビヒクルで6時間治療してから、組換えヒトWnt3a(3nM)および組換えヒトR−スポンジンで18時間治療した。各R−スポンジン1〜3の2つの濃度およびR−spo4の1つの濃度を、TCF/LEFレポーターセルラインの活性化において異なるR−スポンジンの活性の本発明者らの分析に基づいて試験した(100pM、300pM、1nM、3nMまたは10nM)。レポーター誘導GFPシグナルをプレートリーダーで測定した。示した全実験は試験した各R−スポンジンについての3つの独立した実験(2回繰り返して各実験を行った)からプールしたデータである(データは平均値+SD)。
RSPO2またはRSPO3の併用によるTCF/LEFプロモーター誘導GFP発現の誘導に影響を及ぼさなかった。RSPOおよびWntの存在下でLGR4およびLGR5両方によりシグナル伝達活性の誘導を阻害することが分かっているポジティブコントロール抗体76C12も示す。このデータは、抗LGR5抗体18G7H6A3がRSPO誘導TCF/LEFプロモーター活性を遮断しないことを示している。
CHO−LGR5細胞をコンフルエントまで成長させ、遠心沈殿させて、PBS中に再懸濁し、カウントした。細胞のアリコート(約100k)を100μM前以て温めた(37℃)CFSE(カルボキシフルオレセインスクシンイミジルエステル)を含む別のチューブに添加し、混合物をセルインキュベーター内で15分間インキュベートした。最終CFSE濃度は約1μMであった。次に、細胞を洗浄し、前以て温めた培地に再懸濁し、インキュベーター内でさらに30分間入れた後、PBSで洗浄した。それから、染色した細胞を18G7H6A3(100μM)で染色した。抗体のCHO−LGR5細胞との結合を保証するため、いくつかの試験では、細胞のアリコートを、二次ヤギ抗ヒトPE共役抗体でも染色し、実験室においてcalibur機で分析した。U937細胞を、DDAO−SE(DDAOスクシンイミジルエステル;100K細胞に対して2μMの染料)で15分間、実験室内の遮光場所で、かつ、室温において染色した。それから、細胞は1mlのFBS(ウシ胎仔血清)であり、次いで、遮光場所で5分間インキュベートした。次に、細胞を、FBS(10%)を添加したPBSで洗浄し、FBS(2.5%)を添加したRPMI中に再懸濁した。CHO−LGR5−18G7H6A3およびU937−DDAO−SE標識化細胞両方をセルインキュベーター内で5時間、同時インキュベートし、実験室においてcalibur機で分析した。ネガティブコントロールとして、CHO−LGR5−CFSE細胞(18G7H6A3染色なし)のアリコートもU937と同時インキュベートし、calibur機で分析した。
18G7H6A3の内部移行を、LGR5を過剰発現するCHO細胞に対して検査した。細胞を100nM抗体で4℃において30分〜2時間染色し、過剰Abを洗い落とし、染色した細胞を4℃または37℃のいずれかでインキュベートした。細胞をAlexaFluor488標識二次抗体で様々な時点において染色し、細胞表面結合抗体の内部移行をモニターした。37℃におけるインキュベーション時、内部移行した割合は、6.703±1.282分の表面局在に対して実測値t1/2であった。表面結合抗体のいくらか減少が観察されたが、内部移行は4℃におけるインキュベーションにより多くが遮断された。

ヒトR−スポンジン1/2/3/4タンパク質の存在下、ビオチン−18G7H6A3とhLGR5−Fcとの相互作用を、競合ELISAフォーマットを用いて検査した。LGR5−Fcを96ウェル高結合ELISAプレート上に2μg/mLで塗布し、4℃オーバーナイトでインキュベートした。プレートをPBS+1%BSAで遮断した。ビオチン−18G7H6A3を結合緩衝液中1μg/mLまで希釈した。LGR5−Fcおよびビオチン−18G7H6A3間の前の直接結合ELISAから濃度を選択して、EC50濃度超の頑強なシグナルを得た。競合タンパク質を、様々な濃度においてビオチン−18G7H6A3と同時にELISAプレートに添加した。1:1,000希釈のストレプトアビジン−HRP(R&D Systems、カタログ#890803)を検出に使用した。プレートをTMB(Thermo)で発色し、SpectraMax Plus384プレートリーダーで450nmにおいてデータを収集した。GraphPad Prism6プログラムを用いてデータ解析を行った。ビオチン−mAbおよび競合濃度のいくつかを変更してELISAを3回繰り返した。
リガンド単独(RSPOまたはNorrin)とLGR5との結合は、LGR5シグナル伝達を誘導するのに充分ではない。代わりに、LGR5は、複数の共受容体と三元複合体を形成してシグナル伝達を誘導する。LGR5三元複合体の形成に対する18G7H6A3の効果を検査するため、R−スポンジン1/2/3/4およびNorrinの存在下、LGR5と、RNF43、ZNRF3、およびLRP6との結合を、ELISAフォーマットを用いて検査した。RNF43−Fc、ZNRF3−Fc、およびLRP6−Fcを、1×PBS中の4μg/mLで96ウェル高結合プレート上に塗布した。プレートを4℃オーバーナイトでインキュベートし、PBS+1%BSAで遮断した。LGR5−Fcを、R−スポンジン1/2/3/4の1μg/mLまたはNorrin 0.5μg/mLの存在下または非存在下全てで、一次緩衝液中1μg/mLまで希釈した。R−スポンジン1/2/3/4またはNorrinを、ELISAプレートに添加する前に、hLGR5−Fcと一緒にプレインキュベートした。各条件について三組のウェルを使用し、三組を同じ条件で試験した。1:2,000抗FLAG mAb(Cell Signaling)を使用して結合したhLGR5−Fcを検出した。抗マウスIgG HRP(JIR)の1:10,000希釈を検出のために使用した。プレートをTMB(Thermo)で発色し、SpectraMax Plus384プレートリーダーで450nmにおいてデータを収集した。GraphPad Prism6プログラムを用いてデータ解析を行った。LGR5、リガンドRSPOまたはNorrin、および共受容体(RNF43−Fc、ZNRF3−Fc、およびLRP6−Fc)との三元複合体の形成が観察された。

抗体18G7H6A3が結合するLGR5の特定領域をさらに特徴づけするため、水素重水素交換質量分析法を用いてエピトープマッピング実験を行った。水素−重水素交換実験を行う前に、塩酸グアニジン(GdnHCl)の様々な濃度中の非重水素化緩衝液で調製した試験消化物を作製して、LGR5単独の最適なペプチドカバレッジ(peptide coverage)のためのタンパク質分解条件を最適化した。DXMSのペプシン消化のため、サンプルを5℃で解凍してから直ちに0.05%トリフルオロ酢酸で100μl/分の流速で、ブタペプシン(Sigma)を充填したプロテアーゼカラムで消化した。消化性フラグメントをC18トラップカラムに回収し、6〜38%の直線的アセトニトリルグラジエントを用いてC18逆相カラム(Vydac)で分離した。カラム溶出物をLCQ Classic(Thermo Finnigan,Inc.)またはQ−TOF質量分析計(Micromass)に直接エレクトロスプレーした。MS/MSデータセットからのペプシン生成ペプチドの決定を、SEQUEST(Thermo Finnigan,Inc.)の使用により容易にした。それから、この組のペプチドを、DXMS Explorer(Sierra Analytics Inc.、カリフォルニア州モデスト)によりさらに検証した。異なる濃度のGdnHClのペプチドカバレッジマップを比較して、各個別のタンパク質またはタンパク質複合体の最適カバレッジマップを、その後の重水素交換実験に使用した。前に記載したように0℃で全ての工程を行った。

LGR5転写産物発現を、LGR5特異的プローブを含むRNAscope技術を用いて調査した。LGR5転写産物は、結腸、腸管、小脳および膵臓を含む組織で検出可能であった。LGR5転写産物は、CT1 CRCおよびJH109膵腫瘍を含む患者由来異種移植片(PDX)組織でも検出可能であった。LGR5の発現を、初期(グレードI)対進行性(転移性)病変を含む腫瘍形成能の異なる段階において単離されたCRC患者サンプルで調査した。LGR5転写産物は、CRCグレードI、IIおよびII病変において発現され、CRC転移性病変において高度に発現された。
転移性膵臓患者由来異種移植片におけるLGR5の発現を、定量的ポリメラーゼ連鎖反応(QPCR)を用いて調査した。腫瘍組織のサンプルを急速凍結またはRNAlater(Qiagen、カリフォルニア州)を含むクライオバイアル(cryovial)に添加し、数時間4℃でインキュベートした後−70℃に移動した。Qiagen RNeasy抽出キット(Qiagen、カリフォルニア州)を用いて、全RNAを抽出し、SuperScriptIIIキット(Life Technologies、カリフォルニア州)および製造者により提供されたプロトコールを用いてcDNAを合成した。ヒトLGR5転写産物存在量を、ヒト特異的LGR5およびGAPDHプライマーならびにStepOne Thermocycler(Life Technologies、カリフォルニア州)の次の熱的条件:50℃(2分);90℃(2分)および90℃(15秒)の40サイクルおよび60℃(1分)および融解曲線アセスメント(65℃〜95℃)を用いて測定した。LGR5存在量を、式2^δCtを用いて定量した。
18G7H6A3のためのWnt経路における可能性ある標的を調査した。Wnt QPCRプレート(Qiagen、カリフォルニア州)を、96ウェルPCRプレート中の約80Wnt経路遺伝子に対するプライマーを用いて調製した。18G7H6A3またはMOPC(コントロール)治療腫瘍からのcDNAをプールし、WntプレートにおけるQPCRを行った。各プレートのデータを対応するGAPDHに対して正規化し、各遺伝子の存在量を式2^δCtを用いて測定した。倍差を測定するため、各18G7H6A3治療腫瘍におけるデータを、MOPC治療群からの対応する値により割った。1より大きいかまたは1より小さい値は、それぞれ、18G7H6A3治療群における上方制御かまたは下方制御を示した。上方制御または下方制御された遺伝子数の初期評価は、両方の腫瘍モデル(CT1およびCT3)において、上方制御された遺伝子より下方制御された遺伝子が多いことを示し、18G7H6A3は遺伝子発現に対する阻害効果を有することを示唆した。詳細な分析は、FZDB、FZD7、WNT7B、FBXW11、FZD1、DVL1、CSNK2A1およびCTNNB1を含むいくつかの差次的に発現された遺伝子を同定した。
するQPCRデータを確証し、これらの腫瘍におけるWnt経路活性の阻害を示唆した(図16)。
18G7H6A3標的遺伝子の発現を、初期対後期腫瘍形成能において調査した。マウスは、CT1を移植したマウスであり、3日目、10日目および17日目においてコントロール、18G7H6A3群、FOLFIRI群または化学療法群から腫瘍を回収した。3日目の各腫瘍からの全RNAを回収し、Illumina human chipsを用いて遺伝子アレイハイブリダイゼーションのために調製した。差次的に発現された遺伝子(1.5または2倍より多い、p<0.05)の全体的分析は、18G7H6A3で治療した(単剤またはFOLFIRIと併用して)腫瘍において、上方制御された遺伝子より下方制御された遺伝子が多かった。これは、18G7H6A3を用いた治療は、全体的細胞転写装置に対するより大きな抑制効果を有した可能性があることを示唆した。PCA(主成分分析)は、18G7H6A3およびコントロール治療腫瘍での全体的遺伝子発現における近接効果も示した。しかしながら、18G7H6A3をFOLFIRIに添加した場合(すなわち、併用群)、併用対FOLFIRI間の明白な分離があり、LGR5の標的化は、FOLFIRI治療腫瘍における遺伝子発現を有意に変化させた可能性があることを示唆した。
るDAB1、MIR655、NKX1−2などのいくつかの腫瘍サプレッサーを同定した(図18)。逆に、FOLFIRI治療は、腫瘍プロモーター(FBN2、HKDC1、ABCB1、FGF2)ならびにTRIB3、ATF3およびTIMP3などのいくつかの腫瘍サプレッサーも上方制御するように思われる(図19)。FOLFIRIおよび18G7H6A3の併用は、ALDOC、CDH5、ITGA2などのより多くの腫瘍プロモーターの下方制御、ならびにZBTB11、ITPKA、PSMC3IPおよびBAK1などのより多くの腫瘍サプレッサーの上方制御をもたらした(図20)。
原発腫瘍成長および転移の阻害における18G7H6A3の役割を調査するため、一連の膵臓患者由来異種移植片サンプル、ならびにPANC1424細胞およびPANC1427細胞においてLGR5の発現を検査した。
LGR5の発現を、フローサイトメトリーおよびRNAscopeを用いて、カニクイザル(Cynomolgus macaques)(Cynos)由来の皮膚サンプルで調査した。Cynosからの皮膚サンプルを、0日目、7日目、14日目および21日目においてビヒクルまたは18G7H6A3の様々な用量(G2:10mg/kg;G3:50mg/kg;およびG4:150mg/kg)で治療した。試験終了時、皮膚サンプルを、抗菌剤(ペニシリンおよびストレプトマイシン)および抗真菌液剤(Anti−Anti 100×、Life Technologies、カリフォルニア州)を添加し
たDMEM中で準備した。皮膚サンプルを、コラゲナーゼおよびサーモリシン(リベラーゼ、Roch Inc、カリフォルニア州)の混合液を用いて消化した。皮膚前駆細胞(SP)をリベラーゼと一緒に、機械的破砕をしながらオーバーナイトのインキュベーション後に単離した。SPを、ラット抗ヒトLGR5(AF647、BD Pharmingen、カリフォルニア州)で染色し、実験室においてcalibur機で分析した。FCS Express(Denovo Software、カリフォルニア州)を用いたデータ解析は、LGR5はCynos SPにおいて検出可能であるが、18G7H6A3(異なる用量)対ビヒクル治療群間のLGR5頻度の有意な差はないことを示した。RNAscopeを用いて、LGR5は、特に毛包幹細胞およびずっと程度は低いが皮膚上皮細胞において皮膚領域で検出可能であった。ビヒクル対18G7H6A3治療サンプルのLGR5ポジティブ領域の有意な差はなかった。
患者由来小細胞肺がん異種移植モデル。BLG293腫瘍から分離した腫瘍細胞を、マトリゲル中のCB.17 SCIDマウスの皮下に移植し、腫瘍サイズおよび体重について週2回モニターした。腫瘍が平均130mm3に達した場合、マウスをランダムに分けた。マウスを、PBS、抗体コントロールMOPC、または18G7H6A3のいずれかで治療した。マウスに、15mg/kgで週2回投与した。全マウスを、体重および腫瘍サイズ、ならびに健康全般および外観について、終了するまで、週2回モニターした。
Panc1427(UCSD1427)腫瘍は、化学療法(ゲムシタビン/アブラキサ
ン)および18G7H6A3を用いた治療により完全に減量(消失)した。腫瘍が退消失した場合、化学療法は止め、マウスを18G7H6A3で治療したかまたは治療しなかった。18G7H6A3で治療した動物は、コントロール動物と比較して、著しく健康であったが、いく匹かのマウスは跛行または体重損失などの重症な健康観察のせいで安楽死させなければならなかった。150日目、18G7H6A3および化学療法で治療した7/8のマウスは生存し、対して、化学療法単独で治療したマウスは4/8が生存した。図23に結果をまとめている。
転移性大腸がん患者のBNC101(抗LGR5ヒト化モノクローナル抗体)のフェーズI、用量漸増試験を下記のように行う。最終製品名:注入用BNC101液剤。活性成分名:BNC101、18G7H6A3、ET101、LGR5抗体。
最大耐用量(MTD)を決定するため転移性大腸がん患者に静脈内投与されたBNC101の推奨フェーズII用量(RP2D)、安全性、耐容性および薬物動態(PK)プロファイル。主目的は転移性大腸がん患者におけるBNC101単剤および化学療法との併用の両方のMTDを決定することである。第二目的は以下の通りである。転移性大腸がん患者におけるBNC101単剤および化学療法との併用の両方のRP2Dを決定すること。BNC101の安全性および耐容性[有害事象(AE)、投薬欠落または投薬遅延]を評価すること。BNC101の免疫原性(BNC101に対する抗体の産生)について評価すること。BNC101単剤および化学療法との併用の両方の薬物動態(PK)(半減期、分布およびクリアランスの体積)を決定すること。BNC101で治療された転移性大腸がん患者の全奏効率(ORR)、無進行生存率(PFS)および全生存率(OS)の初期評価を行うこと。探索目的は以下の通りである。疾患関連バイオマーカーの変化(CEA)を評価すること。活性のバイオマーカー[薬力学、例えば、循環腫瘍細胞(CTC)、LGR5+細胞、循環腫瘍DNA]を評価すること。
治療誘発事象(臨床データおよび実験室データ)を評価すること。投薬中断および投薬中止を評価すること。
1.署名された書面によるインフォームドコンセント。 2.年齢>18歳。 3.米国東海岸がん臨床試験グループ(ECOG)パフォーマンスステータススコア0〜1。 4.転移性疾患に対して少なくとも2回の化学療法(単剤治療コホート)または少なくとも1回の化学療法(併用治療コホート)に抵抗性のあった組織学的または細胞学的に確認された大腸がん患者、かつ、医師および患者双方の意見において実験的治療を試行することが不当でない。この6か月以内のアジュバントFOLFOX(フォリン酸;フルオロウラシル(5−FU);およびオキサリプラチン)が治療法と見なされる。第一治療後の維持ストラテジーが追加の治療法として考えられていない。 5.患者は、該患者またはその治療を危険にさらすことがない生検を行い易い利用可能な腫瘍病変を有しなければならない。単剤漸増コホート3以降の患者、単剤拡大コホート、および全併用治療患者は、2回連続の腫瘍病変生検(可能ならいつでも最低限2回の新鮮なコア/パンチが好ましい)を提供することに同意かつ提供する意思がある。原発性腫瘍の代わりに、肝転移から生検することができる。新鮮な生検の腫瘍組織の存在は、適切な準備なしの組織学的または細胞学的手順を用いて訓練された病理学者により認定されるものとする。 6.固形がんの治療効果判定のためのガイドライン(RECIST)1.1版による測定可能な疾病。 7.既知の脳転移がない。 8.少なくとも≧12週の平均余命。 9.正常な臓器および髄機能: a.登録前に過去14日間成長因子の助けなしに好中球絶対数>1,500
/mL。 b.登録前に過去14日間輸血なしで血小板>100,000/mL。 c.ヘモグロビン≧9.0g/dL−患者は輸血されていてもよく、この規準を満足する赤血球新生治療を受けていてもよい。 d.全ビリルビン<1.5×正常の施設基準値上限(ULN)(ジルベール症候群を有する対象では<2×ULN)。 e.血清アルブミン≧3g/dL。 10.アスパラギン酸アミノ基転移酵素(AST)(血清グルタミン酸オキサロ酢酸トランスアミナーゼ、SGOT)およびアラニンアミノ基転移酵素(ALT)(血清グルタミン酸ピルビン酸アミノ基転移酵素、SGPT)<2.5×施設基準ULN(肝臓への侵襲がある対象では<5×施設基準ULNだが、ビリルビン上昇と関連し得ない)。 11.生検を受ける患者のため、プロトロンビン時間(PT)および活性化部分トロンボプラスチン時間(APTT)/正常範囲内(±15%)の国際標準化比(INR)。 12.クレアチニン<1.5×施設基準ULNまたはクレアチニンクリアランス>60mL/分/1.73m2(国際標準より上のクレアチニンレベルを有する対象)。 13.妊娠する/妊娠させる可能性のある女性/男性の充分な避妊。
交互に、2つの分離した組みのコホートにおいて漸増を行う:単剤BNC101用量漸増は併用化学療法コホートにおける用量漸増に先行する。後者の場合、BNC101は、FOLFIRIと併用して用量漸増される。併用化学療法コホートは、RP2Dが単剤治療コホートにおいて決定されるまで治療を開始しない。併用化学療法コホートにおける漸増は、単剤コホートの単剤RP2Dより低い1つのレベルで開始する。化学療法とBNC101の併用からさらなる毒性は予期されないが、初回用量より低い2つの追加の段階的縮小(de-escalation)レベルを使用するが、これらは必要なことである。
標準3+3フェーズI試験デザインを使用する。初回用量は、動物において最大無毒性量(NOAEL)の用量(ヒトで2.5mg/kg)の30分の1であり、受容体結合における種の違いを考慮して算出した。その後のBNC101用量レベルは5、10、および15mg/kgである。
新たに登録した対象に対する用量漸増を、コホートの全対象がその28日目DLTアセスメントを完了した後で行う。不変の疾患または反応する疾患を有する対象または56日目(2サイクル)より後の対象は、疾患が進行するまでBNC101を毎週投与することを許容することができる。
BNC101単剤治療の用量漸増が完了し、安全を宣言してRP2Dに達した後、転移性大腸がん患者は、単剤で特定されたRP2Dより低い1用量レベルのBNC101を用いたこれらの併用化学療法コホートで開始することができる。これらのコホートはそれぞれ3対象を含む。漸増は単剤治療コホートで使用したのと同じルールにより進める。この初回BNC101用量は、最大6患者のうち2人のDLTを示すべきであるが、3+3ルールに従って、2人の追加の段階的縮小コホートを準備して化学療法と併用したRP2Dを特定する。
DLTは、いずれかの所与の漸増コホートのサイクル1(0〜28日目)において起こる下記のもののいずれかと定義される:NCI CTCAE v4.0を用いたグレード3または4の非血液毒性(アナフィラキシー反応を含む);48時間より長く持続するグレード3の吐き気または適切な治療にもかかわらず起こるグレード4の嘔吐もしくはグレード≧3の下痢のいずれか;いずれもの持続期間のグレード4の血小板減少症および単剤治療コホートにおいていずれもの持続期間または併用化学療法コホートにおける>7日持続するグレード4の無併発性好中球減少症(すなわち、熱も感染症もない)。入院を必要とするグレード4の発熱性好中球減少症および3週間を超える治療遅延およびQTcインターバルの≧500ミリ秒への延長またはベースライン平均QTcインターバルから60ミリ秒の増長を必要とするいずれかのグレード3の血液毒性。最大の医学的管理を受けていない対象の高脂血症またはサプリメントの服用で抑えられている電解質異常を除いた、
いずれもの他の薬物関連≧グレード3の非血液有害事象(アナフィラキシー反応を含む)。全てのAEは、疾患進行(PD)に対する観察された毒性間の明快な関係性がない限り、強力に治療に関連すると見なされる。これらの判断基準を満足する有害事象は、DLTの報告またはMTDの決定の目的のためと見なされない。
患者は、どれでも最初に起こる、PD、容認されない毒性、同意の撤回、またはスポンサーによる試験停止まで治療される。
約54人までの患者がこの試験において治療され、単剤および化学療法との併用の両方で、毒性プロファイル、DLT、およびMTDおよび/またはBNC101のRP2Dを決定する。用量レベルコホート当たり1〜6人の評価可能な患者がBNC101の毒性およびPKプロファイルを評価するために十分なデータを提供すると見込まれる。一旦、RP2Dを特定したら、これらのコホートの拡張(少なくとも9人までの追加患者)は、単剤治療群および併用化学療法群の両方で行われる。試験した最大BNC101用量においてDLTが報告されない場合、実験動物において示される最大暴露に匹敵するBNC101暴露を含むPKプロファイルを示す患者のコホートは、RP2Dである。
4週サイクルの各週に全7(±3)日。生存の経過観察情報およびその後の抗がん治療は、死、追跡不能、患者の同意撤回、またはスポンサーによる試験停止まで、治療の休止後3か月毎に収集される。
0〜28日目(サイクル1)のDLTについて対象を評価する。最後の投薬後、30日にわたって有害事象を報告する。安全性を継続的に報告する。最後の投薬後28日目と30日目の間で起こる有害事象を報告するが、MTDの決定に不可欠なものでない。パフォーマンスステータスを毎週スコアし、ベースライン、各サイクルの開始時(例えば、4週間毎)、研究終了(EOS)訪問時およびいずれものAEの回復時に身体検査を行う。
全12リードECGを、3回、少なくとも5分離して得るものとする。ベースライン(第一投与に先んじて14日中に)、サイクル1の1日目および15日目に集中的に、ならびに8日目および22日目の投薬前に、ECGを得る。サイクル2およびその後のサイクル(複数)の1日目の投薬前にECGを得る。EOS ECGは回収しなければならない。アセスメントのために、地域の研究室にECGを送付する。
コンピュータ断層撮影(CT)スキャンを、ベースラインおよび8週間毎に行う。第一応答アセスメントは56日目(2サイクル後)である。抗腫瘍応答のための放射線医学試験を、前の28日以内に行われない場合にEOS訪問において繰り返す。記録の電話経過観察またはレビューを、6か月間毎月およびその後の18か月間は3か月毎に行う(合計24か月)。
薬物動態サンプルを、サイクル1の1日目および15日目ならびに疎らに8日目および22日目に集中的に得る。サイクル2およびその後のサイクル(複数)の1日目に疎らにサンプルを得る。BNC101の点滴中、および点滴後(点滴終了後)、投薬前(点滴開始前)に血液サンプルを採血する。
患者は、ベースライン、サイクル1の間毎週、それぞれその後のサイクルの1日目および治療終了時に血液を採取して、薬力学効果の指標としてCTCおよびバイオマーカー(LGR5を含むがこれらに限定されない)のレベルを測定する。患者は、ベースラインおよびサイクル1の22日目に2回の対応皮膚生検も受ける(各生検は2回の新鮮なコア/パンチである)。患者は、追加の毛髪サンプル(毛包幹細胞の収集を含む)を採取してもよい。
単剤治療漸増コホート1および2の生検は必要でない。対応生検(ベースラインおよび22日目)は、単剤治療漸増コホート3以降、および単剤治療拡大コホートには必須である。対応生検は、全併用治療患者には必須である。各生検は、好ましくは可能ならいつでも、最低限2回の新鮮なコア/パンチである。原発性腫瘍の代わりに、肝転移から生検することができる。新鮮な生検の腫瘍組織の存在は、適切な準備なしの組織学的または細胞学的手順を用いて訓練された病理学者により認定されるものとする。全サンプルを匿名化する。個体の特定は、調査またはスポンサーにより確認することができない。中央標準研究所に属するパスワード保護されたデータは、患者またはこの患者を治療する医師に公表されない。患者情報は、リサーチデータの無記名の報告の中で厳密に使用される。このようなリサーチデータは、患者のチャート内に記載されないし、臨床医にも利用できず、診断のためにも治療のためにも使用せず、かつ、使用することができない。
抗BNC101抗体の存在を、ベースライン、各投薬前、治療終了時、および12週間治療中止後4週毎に試験する。単剤治療対併用治療コホートにおける患者の異なる性質および予測される漸進的変化のせいで、異なる数のサンプルをそれぞれにおいて得ることができる場合がある。

- ロイシンリッチリピート含有Gタンパク質共役受容体5(LGR5)と特異的に結合するヒト化モノクローナル抗体の有効量を、それを必要とするヒト対象に投与することを含む転移性大腸がんを患っている前記ヒト対象の治療方法であって:
前記モノクローナル抗体は、配列番号13を含む重鎖および配列番号14を含む軽鎖を含み;

- ロイシンリッチリピート含有Gタンパク質共役受容体5(LGR5)と特異的に結合するヒト化モノクローナル抗体またはその抗原結合性フラグメントの有効量を、それを必要とする対象に投与することを含むがんを患っている前記対象の治療方法であって:
前記モノクローナル抗体は、配列番号13を含む重鎖および配列番号14を含む軽鎖を含み;
前記モノクローナル抗体は、少なくとも4週間、毎週静脈内に投与され;
前記モノクローナル抗体の投与量は、約2.5mg/kg〜約15mg/kgである、前記方法。 - 前記モノクローナル抗体を、化学療法薬と併用して投与する、請求項5に記載の方法。
- 前記化学療法薬が、フォリン酸、フルオロウラシル、イリノテカン、ゲムシタビンおよびナノ粒子アルブミン結合パクリタキセル(アブラキサン)からなる群から選択される、請求項6に記載の方法。
- 前記モノクローナル抗体の初回用量を、化学療法薬の投与前に投与する、請求項7に記載の方法。
- 前記モノクローナル抗体を、フォリン酸、フルオロウラシル、およびイリノテカンと併用して投与する、請求項7に記載の方法。
- 前記モノクローナル抗体の初回用量を、フォリン酸、フルオロウラシル、およびイリノテカンの投与前に投与する、請求項7に記載の方法。
- 前記イリノテカンの初回用量は約180mg/m2であり、約90分かけて投与する、請求項7に記載の方法。
- 前記フォリン酸の初回用量は約400mg/m2であり、約120分かけて、かつ、前記イリノテカンの初回用量と同時に投与する、請求項11に記載の方法。

- 前記モノクローナル抗体を、ベバシズマブ、アフリベルセプト、セツキシマブ、およびパニツムマブからなる群から選択される追加の治療薬と併用して投与する、請求項1〜14のいずれか一項に記載の方法。
- 前記がんが、固形腫瘍を含む、請求項5〜14のいずれか一項に記載の方法。
- 前記がんが、結腸がん、大腸がん、膵がん、乳がん、および肺がんからなる群から選択される、請求項5〜14のいずれか一項に記載の方法。
- 前記がんが、APC変異を含む結腸がん、KRAS変異を含む結腸がん、転移性大腸がん、転移性膵がん、トリプルネガティブ乳がん、および小細胞肺がんからなる群から選択される、請求項5〜14のいずれか一項に記載の方法。
- 前記がんが、転移性大腸がんである、請求項5〜14のいずれか一項に記載の方法
- 前記対象が、前記モノクローナル抗体の投与前に転移性疾患の化学療法による少なくとも1例の無効例の前治療歴;既知の脳転移がないこと;平均余命12週間以上であること;前記モノクローナル抗体の投与前の前記14日間中に成長因子の助けなしに約1500細胞/mLを超える好中球絶対数を有すること;前記モノクローナル抗体の投与前の前記14日間中に輸血なしで100,000血小板/mLを超える血小板数を有すること;9.0g/dL以上のヘモグロビンを有すること;および3g/dL以上の血清アルブミンを有することからなる群から選択される特徴を有する、請求項1〜14のいずれか一項に記載の方法。
- 前記対象が、哺乳類である、請求項1〜14のいずれか一項に記載の方法。
- 前記対象が、ヒトである、請求項1〜14のいずれか一項に記載の方法。
- ロイシンリッチリピート含有Gタンパク質共役受容体5(LGR5)と特異的に結合する用量のヒト化モノクローナル抗体、および適切な医薬用担体を含む医薬組成物を収容する容器であって、前記用量のモノクローナル抗体が約2.5mg/kg〜約15mg/kgである、前記容器。
- 前記医薬組成物が静脈内投与に適している、請求項23に記載の容器。
- 転移性大腸がんの治療用途のロイシンリッチリピート含有Gタンパク質共役受容体5(LGR5)と特異的に結合するヒト化モノクローナル抗体であって、
