Description
入された後の回収の方法に関する。詳細には本発
明は、Giantruco式Expandable Wire Stent(膨
張可能なワイヤステント)の改良に関する。
膨張させ、あるいは導管を通つて開いた管腔を維
持する手段を提供する必要がある。そのような状
況は例えば、血管を通る血流を制限しあるいは閉
塞する動脈硬化、あるいは尿道を通る流れを制限
しあるいは閉塞する前立腺の疾病に関して生ず
る。
ステントは閉鎖ジグザグパターンに構成されたス
テンレスワイヤから形成され、より詳細には、米
国特許第4580568号明細書に説明されている。
GianturcoのステントはZステントとしても知ら
れているが、圧縮されて寸法が縮小し、その外径
は膨張した時の数分の1の外径となつている。前
記ステントは鞘により、その中に収縮した寸法形
状で保持されて管腔に配置される。鞘が引き抜か
れる時に、プツシヤあるいは平らな端を有するカ
テーテルが鞘を通つてステントを管腔内に保持す
るために使用され、それによりステントが管腔内
で膨張した形状に膨張し、管腔を開きかつ拡大す
る。かくてZステントは自動膨張手段を提供して
開いた管腔を維持する。第1図は体内の管腔内部
で公知のZステントの使用方法を描いている。
使用されるいくつかのZステントを描いている。
描かれている構成においては、2つのステントが
重なり合つた構成で配置され、その間に第3のス
テントが残りの2つのステントからわずかに下流
に配置されている。上記に参照されている米国特
許第4580568号明細書に完全に説明されているよ
うに、3つのステントはそれぞれ独立に上述の鞘
及びプツシヤを使用して挿入されなければならな
い。
来、Zステントは最初は管腔内で膨張した時の最
大外径よりもいくらか少ない外径に膨張するけれ
ども、時間が経過すると膨張し続け、それの最大
外径にまで膨張することが発見されている。その
ため、場合によつてはステントは管腔の壁に深く
埋め込まれてしまう結果となる。この特別な問題
に対応する努力において、他の当業者は膨張可能
なZステントをジグザグ構造の折り曲げあるいは
接合で円形に形成する改良を行つた。次に単線フ
イラメントがステントの一端にある各小孔に通さ
れ、ワイヤステントの膨張を制限する連続する柔
軟なリングが形成される。単線フイラメントはそ
の拡張方向に十分弾性があり、ステントが最適、
所望の直径に膨張するように制御する。

能な自動膨張経皮ステントの需要は残つている。
同様に、従来技術の膨張可能なステントにより可
能なものより長い体内の管腔の長さに沿つて挿入
するための数個のステントセクシヨンを含む経皮
ステントの需要も存在する。更に、挿入操作のた
めに複数のカテーテル操作を必要としない複数ス
テントセクシヨンの組み合せを可能とするステン
トに対する需要も存在する。
曲げにより結合された直線セクシヨンの無限連続
を形成する閉じたジグザグ形状に形成されたワイ
ヤを含む。折り曲げ部ごとに小孔が形成され、こ
れが同様に形成された自動膨張ステントの連結の
ために使用可能であり、あるいは同様の構造のス
カートステントの連結に使用可能である。多数の
自動膨張ステントが小孔においてステントを結合
することにより組み合せ可能である。各ステント
は導入鞘のような体内の管腔に挿入するために最
初の小さな形状に弾性的に圧縮可能である。一旦
体内の管腔に挿入されると、ステントはそれより
大きい第2形状に自動膨張可能であり、そこで直
線セクシヨンが管腔の壁を押圧してそれを開いた
状態に維持する。
シヨンを結合する折り曲げ部に小孔を有する、閉
じたジグザグ形状の自動膨張ステントを含む。単
線フイラメント糸がステントの一端にある各小孔
を通り、一旦各小孔を通つて360度のループを作
り、次に再び小孔のいくつかを通つて180度のル
ープを作る。単線フイラメントの後尾の自由端は
ステントの直径の反対側の側面に残され、体内の
管腔の外側にある導入鞘を通つて伸びている。単
線フイラメントは体内の管腔内部で自動膨張ステ
ントの膨張を制限するために、外部に結びつけら
れている。
テントを回収することを可能とするためにステン
トの直径を縮小するためにも使用可能である。か
くて、本発明の方法においては、単線フイラメン
トの自由端を越えて管に糸が通され、前記管が膨
張ステントに隣接するところまで管腔を沿つて前
進する。次に自由端が引つ張られ、それによりス
テントの外径が、前記管の直径にほぼ等しくなる
まで収縮される。次に鞘が管及び収縮したステン
トの直径部分をを越えて導入され、更にステント
の残りの長さを押し潰す。次に管、鞘、ステント
及び単線フイラメントを含む組立体全体が体内の
管腔から除去される。

内部に配置された自動膨張ステントの除去の方法

れ、このステントは一定の長さ11の閉じたジグ
ザグ構造に形成されたステンレスワイヤから形成
されている。ワイヤの両端は、連続あるいは無限
構造を生ずるようにワイヤの両端を溶接し、ある
いは強く圧搾して結合したスリーブ12により閉
じている。ワイヤの長さ11は多数の並んだ直線
セクシヨン13のように構成されている。隣接す
るセクシヨン13は先端14により隣接してい
る。

4580568号明細書に説明されているZステントに
類似している。特に、本発明の特定の実施例は、
0.018インチ(約0.45mm)の直径を有するステン
レスのワイヤ11を含む。隣接する直線セクシヨ
ン13を連結する先端14は、ほぼ半径およそ
0.08インチ(0.2cm)を越えない円形を描いてい
る。このような形状により、ステント10は、カ
テーテルあるいは鞘を使用して対体の管腔内部に
ステントを挿入可能にするために、第3−4図に
示されている外径よりずつと小さく圧縮すること
が可能となる。
トの先端14は多数の小孔20に形成されてい
る。好ましい実施例においては、前記小孔は連続
するワイヤ11に形成され、先端14において隣
接する直線セクシヨン13の間の交点は溶接ある
いははんだ付けされて小孔20のために閉じたル
ープを提供している。本発明の特定の1実施例に
おいては、前記小孔はおそよ前記ワイヤの直径の
3倍、あるいは0.054インチ(約1.35mm)の直径
である。

成される。ワイヤ11′の長さがジグザグに形成
されるけれども、先端14′は直ちにははんだ付
けされない。ワイヤ11′の長さの自由端はスリ
ーブ12′により結合される。一旦ステント1
0′が形成されると、第2ステント10″がステン
レスワイヤの第2の長さ11″から形成される。
第2の長さ11″はジグザグと小孔構造に形成さ
れ、それにより前記ステントの一端に形成される
小孔20″は第1ステント10′の小孔20′と相
互結合される。一旦第2ステント10″が完成す
ると、スリーブ12″がワイヤ11′の長さの両端
を連結するために使用され、次に先端14′及び
14″が結合あるいははんだ付けされる。

テント16′により結合するために類似の製造方
法が使用可能である。この特定の実施例において
は、単体のスカートステント16′は前記スカー
トの自由端に形成された多数の小孔を有していな
い。スカート16′の自由端は、一旦その組み合
せが体内管腔内部に配置された場合に、ステント
とスカートとの移動を防ぐためにフツク構造に形
成可能である。ここで了解すべきことは、第5図
及び6図の実施例において、ステントあるいはス
カートの一端にある小孔は折り曲げあるいは先端
だけをその連結端に有するステントに、第6図に
示されているスカート16′と類似のスカート1

ト30の対向する小孔35に180度の方向で配置
される。次に糸40は糸40の両方の自由端40

る。自由端40aと40bとが同時に引つ張られ
ると、糸40の直径が縮小し、そのためステント
40の一端の直径が縮小する。代替的には、糸4

する直径を制限するように使用可能である。

テント55の各端に含み、第7A図に示されてい
るステント30とすべての点において同一であ
る。単線フイラメント糸56はステント50の一
端で小孔55を通過する。挿入の方法において
は、ステントはその収縮した直径に圧縮され、第
8図に示されているように、鞘60内に配置され
る。フイラメント56の自由端56a及び56b
は鞘60の外部に伸び、ステントが挿入された後
に出入させるために患者の身体の外部に伸びてい
る。例えば、ステントが前立腺ステントとして使
用される場合、フイラメント56はステント50
から各自由端56a及び56bまで測定した長さ
が30〜40cmであり、それにより前記自由端は前立
腺から尿道まで伸長可能である。ステント50が
血管ステントとして使用される場合、端56a及
び56bが刺し穴の外に配置され、皮膚の近くに
係留するに十分な長さがあることを前提として、
フイラメント56の長さは比較的短い。

れると、プツシヤ61も除去され、それにより第
9図に示されているように、ステント50が体内
の管腔内部に残留する。単線フイラメント56は
ステントに従い、上述のように体外へ通過して出
る。
一時的に体内の管腔内部に配置されるだけで用が
足りることがある。かくて、本発明の他の方法に
おいては、第10A〜10C図を参照して描かれ
ているように、ステント50が体内の管腔から引
き出され、除去される。この実施例の方法におい
ては、管70が単線フイラメント56の自由端を
通され、第10A図に示されているように、埋め
込まれたステント50に隣接するまで単線フイラ
メント56に沿つて体内の管腔へ挿入される。一
旦管70が直接ステント50に隣接して配置され
ると、単線フイラメント56の自由端は管70を
通して引つ張られ、第10B図に示されているよ
うに、それによりステント50の一端を圧縮ある
いは収縮させて、直径を縮小させる。かくて、ス
テント50の端を圧縮することにより、第10C
図に示されているように、鞘75が体内の管腔に
導入可能となる。鞘75はステント50の端の縮
小した直径よりも大きい内径を有している。鞘7
5は、それが自動膨張ステント50の残りの長さ
に接触しこれを圧縮するまで、連続して体内の管
腔内に運ばれる。一旦鞘75が完全にステント5
0を覆い、あるいは包むと、つまりステント50
が完全に鞘75内部に配置されると、組立体全体
が体内の管腔から除去可能となる。本発明の方法
の好ましい実施例においては、管70と鞘75と
は8ポリエチレンチユービングのような医療用プ
ラスチツクからなる。
説明されたが、これは例示的であつて制限的と解
釈されてはならず、単に好ましい実施例が示され
説明されただけであり、本発明の精神の範囲内に
あるすべての変更及び改良が保護されるべきであ
ると理解されるべきである。
ワイヤステントを有する体内管腔の側面断面図、
第2図は、従来技術による体内管腔内部の数個の
ステントを使用しているところを示す第1図に類
似の図、第3図は、本発明の好ましい実施例の側
面図、第4図は、第3図の構造の立面図、第5図
は、本発明のその他の実施例の側面図、第6図
は、本発明のその他の実施例の側面図、第7A図
は、その膨張した状態を示す本発明のその他の実
施例の側面図、第7B図は、その一端が収縮した
状態にある第7A図の構造の側面図、第8図は、
本発明の自由膨張ステントの挿入の方法を示す体
内管腔の断面図、第9図は、自動膨張ステントの
挿入に続く第8図に類似の体内管腔の断面図、第
10A図は、本発明の1実施例の自動膨張ステン
トの回収の方法の1段階を示す、第9図に示され
ている体内管腔の側面図、第10B図は、回収の
方法の他の段階を示す、第10A図に類似の図、

Claims (1)
- 【特許請求の範囲】 1 ステント組立体であつて、 閉じたジグザグ構造に形成された第1ワイヤ
が、 対向する両端を有する直線セクシヨンの無限連
続状態であり、前記直線セクシヨンがステントを
形成するために前記対向する両端において折り曲
げにより連結され、 前記対向する両端の1つにおいて数個の前記折
り曲げ部において1組の小孔を形成し、 前記小孔の組を連続して通る糸を備え、前記糸
が前記ステントから引つ張る一対の自由端を含
み、 前記ステントが体内の管腔を通つて運ばれるた
めに最初の小さい形状に弾力的に収縮可能であ
り、そして、 前記ステントが、前記直線セクシヨンが体内の
管腔の壁を押圧する第2の形状に弾力的に膨張可
能であることを特徴とするステント組立体。 2 請求項1に記載のステント組立体において、
前記ステントの前記第2の形状が前記対向する両
端の前記1つにおいて円周を含み、前記小孔が前
記円周に配置され、そして、 前記糸が連続して小孔を少なくともその円周の
周囲に360度通過することを特徴とするステント
組立体。 3 請求項2に記載のステント組立体において、
前記糸が連続する小孔を通してほぼ540度、前記
円周を通過することを特徴とするステント組立
体。 4 請求項1に記載のステント組立体において、
前記自由端が十分長く、前記ステントが管腔内に
配置されている時に体内の管腔の外部に伸びてい
ることを特徴とするステント組立体。 5 請求項1に記載のステント組立体において、 閉じたジグザグ形状に形成された第2ワイヤ
が、 対向する両端を有する直線セクシヨンの第2無
限連続部を備え、前記直線セクシヨンが折り曲げ
部により前記対向する両端において連結されて第
2ステントを形成し、 前記対向する両端の1つにおいて数個の前記折
り曲げ部に形成された第2組の小孔を備え、 前記第2ステントが体内の管腔を通つて運ばれ
るように、より小さい第1形状へと弾力的に収縮
可能であり、 前記第2ステントが第2形状へ弾力的に膨張可
能であり、その状態で直線セクシヨンが体内の管
腔の壁を押圧し、そして 前記第2ワイヤの前記第2組の小孔が前記対向
する両端の1つにある前記第1ワイヤの周囲に係
合することを特徴とするステント組立体。 6 請求項5に記載のステント組立体において、 前記第1ワイヤが前記第1ワイヤの前記対向す
る両端の他端の数個の折り曲げ部に形成される第
3組の小孔を含み、そして 前記第2ワイヤの前記第2組の小孔が前記第3
組の小孔に相互に係合することを特徴とするステ
ント組立体。 7 体内の管腔内へ挿入するためにステント組立
体を形成するため、第1及び第2自動膨張ステン
トを組合せる方法において、 複数の折り曲げ部によりその両端で結合された
直線セクシヨンの無限の連続を有する、閉じたジ
グザグ構造に形成された連続する第1の長さのワ
イヤから第1ステントを形成する段階と、 複数の折り曲げ部によりその両端で結合された
直線セクシヨンの無限の連続を有する、閉じたジ
グザグ形状へ形成された連続する第2の長さのワ
イヤから第2ステントを形成し、一端にある前記
折り曲げ部が、第2ステントの直線セクシヨンで
開いている小孔を形成する段階と、 第1ステントの一端にある折り曲げ部の周囲に
ある前記第2ステントの一端において、前記小孔
と係合する段階と、そして 前記第2ステントの前記一端で前記小孔を閉じ
る段階とを含むことを特徴とする第1及び第2自
動膨張ステントを組合せる方法。 8 請求項7に記載の方法において、 第1ステントを形成する段階が第1ステントの
一端に第1ステントの直線セクシヨンで開いてい
る小孔を形成する段階を含み、そして 第2ステントの一端に小孔を係合する段階が第
2ステントの小孔を第1ステントの小孔に固定し
第1ステントの一端で前記小孔を閉じる段階を含
むことを特徴とする方法。 9 体内の管腔内部に配置されたステントを回収
する方法において、前記ステントが閉じたジグザ
グ構造に形成された弾性ワイヤを含み、前記ステ
ントの一端に形成された1組の小孔を有し、前記
ステントの連続する小孔を通過する糸を有し、前
記糸に体内の管腔の外へ出てくる2つの自由端が
あり、前記ステントが最初は圧縮された形状を有
し、弾性ワイヤが体内の管腔の壁に接触する第2
の形状を有する方法において、 前記ステントを体内の管腔内部の適所に維持し
たまま、管を通して前記糸の自由端を引つ張つて
ステントの一端の直径を縮小させる段階と、 体内の管腔を通つて前記ステントを移動させる
に十分なだけ、前記ステントの残りの直径を縮小
させる段階と、 収縮したステントを管腔を通つて後退させる段
階とを含むことを特徴とする方法。 10 請求項9に記載の回収する方法において、
前記自由端を引つ張る段階が更に、 ステントの最初の圧縮された直径よりも小さい
内径を有する管を前記糸の自由端に従い通す段階
と、 前記管を体内の管腔に導き、前記管を前記ステ
ントに隣接する位置に運ぶ段階と、 前記管を通して前記糸の自由端を引つ張り、前
記ステントの一端の直径を縮小させる段階とを含
むことを特徴とする方法。 11 請求項10に記載の回収する方法におい
て、前記ステントの残りの直径を縮小させる方法
が更に、 前記管と同心に鞘を体内の管腔へ導く段階と、 体内の管腔を通つて前記管及び前記ステントの
縮小した直径にかぶるように前記鞘を運ぶ段階
と、 更に前記鞘を前記ステントの残りの部分へ運
び、前記鞘の残りの部分を圧縮し、体内の管腔の
壁と係合している前記ステントを引き抜く段階と
を含むことを特徴とする方法。 12 請求項9に記載の方法において、前記ステ
ントの残りの部分の直径を収縮させる段階が更
に、 前記ステントの最初の圧縮された形状の直径よ
りも大きいが、前記ステントの第2の膨張した直
径よりも小さい内径を有する鞘が前記糸の前記自
由端を越えて通される段階と、 前記鞘を体内の管腔に導き、前記鞘を体内の管
腔及び前記ステントの端の縮小した直径を通して
運ぶ段階と、 更に前記鞘を前記ステントの残りの部分を越え
て運び、前記鞘の残りの部分を圧縮し、体内の管
腔の壁と係合している前記ステントを引き抜く段
階とを含むことを特徴とする方法。
