ここに記載するものは、インプラントを体の開口領域および中実領域内に送達すための装置、システムおよび方法である。ここで使用する用語「領域」とは、体の管腔構造ならびに中実器官および中実組織を指しており、それらが病気であるか病気でないかは問わない。管腔構造の例としては、これらに限定されないが、血管、動静脈奇形、静脈奇形、動静脈フィステル、心室、胆管および乳管のような管、ファロピー管、尿管、大および小気道、ならびに胃、腸および膀胱のような中空器官が挙げられる。中実器官または組織としては、これらに限定されないが、皮膚、筋肉、脂肪、脳、肝臓、腎臓、脾臓ならびに良性および悪性腫瘍が挙げられる。
送達ガイドは細長であり、比較的小さい有効直径を有する。それは選択部位へのインプラントの送達を可能にし、配置および埋め込みする間、インプラントを閉じた形に維持する機能を持つ。送達ガイドは、通常は非膨張性である。それは、必要とされる可撓度、関係する解放機構のタイプ、素材等の要因に応じて、中実であってもよく、またはその中を貫通して伸びるルーメンを有してもよい。送達ガイドの先端は、医師の好み、問題の管状器官もしくは領域の解剖学的構造、必要とされる剛度等によってテーパーを付けてもよく、および/または直線、湾曲またはJ字形でもよい。送達ガイドは、例えばX線透視装置で見るために、外側スプリングコイルを含んでもよく、また含まなくともよい。
インプラントそのものは、特定の目的を達成するのに合わせた形をしてよい。他に記載したように、インプラントの目的が動脈または管のような解剖学的構造に開通性を与えるか、または維持することであれば、埋め込んだ後のインプラントの形そのものが管状である。形は、目的に合えば、対称的でも非対称的でもよい。
図1Aに示すように、一般的なインプラント送達システムの変形の1つでは、インプラントは送達ガイド100を含む。送達ガイド100は近位部102および遠位部104を有する。インプラントは、この例ではステント106として描かれており、送達ガイドの遠位部104の一部を取り囲んでおり、送達ガイドの遠位部104に解放可能に取付けられている。インプラント106は、図1Bに示すように、送達ガイド100に同心円状に隣接している。インプラント(106)は図1Aおよび1Bではステントして示されているが、この形に描写したことは、遠位および近位インプラント解放機構(109、111)を持つ送達ガイド100上でのインプラント106の設定を図示することだけを目的としたものである。様々なインプラント解放機構または構造について、以下に詳しく論ずる。
ここに記したインプラント送達装置は、複数のインプラントを、単一の送達ガイドに含めてもよく、または適切な場合には、他の装置と共に使用して標的部位を処置してもよい。一般的に、関心対象の管状器官へは経皮的に到達するが、到達方法は通常、器官の解剖学的状態、処置対象となる医学的状態、被験者の健康状態等に依存するだろう。従って、腹腔鏡または切開法によって到達することもある。
インプラント送達システムは、哺乳動物被検体、好ましくはヒトに使用できる。哺乳動物としては、これに限定されないが、霊長類、家畜、競技用動物、ネコ、イヌ、ウサギ、マウスおよびラットを挙げることができる。
このようなシステムに関しては、システムの構築に通常の材料および技術が使用できることを理解すべきである。この点に関しては、可動構成要素間に潤滑コーティングまたはカバーを提供し、内部システムの摩擦を減らすことが所望されることが多いだろう。
前記本発明を、明瞭に理解することを目的として例示および例を挙げて詳細に記載したが、添付の請求の精神および範囲から逸脱することなしに本発明にある変化および変更を加えることができることは、本発明の教示に照らせば、当業者には容易に明らかになるだろう。本発明の範囲は、別途の特許請求の範囲の文言上または均等な範囲によってのみ限定されるものである。

Claims (35)
- 本質的に以下からなる、管状器官内の標的部位を処置するためのシステムであって、送達ガイド部材がバルーンカテーテルのルーメン内での受け取りに適合しているシステム:
外面および内面を有する少なくとも1つのインプラントをユーザの操作によって解剖学的処置部位に向けるように形成され、近位端および遠位端を有する細長い送達ガイド部材であって、少なくとも1つのインプラントは、少なくとも1つのインプラントの解放前は送達直径を有していて、解放前は送達ガイド部材の遠位端に隣接して配置されており、そして少なくとも1つの解放可能なジョイントが、少なくとも1つの解放可能なジョイントが解放されるまでは少なくとも1つのインプラントの少なくとも1つの区画を送達直径に維持するように形成されている送達ガイド部材;ならびに
バルーンカテーテル。 - 請求項1記載のシステムおよびガイドワイヤから本質的になるシステムであって、ガイドワイヤがバルーンカテーテルのルーメン内で受けられるのに適合しているシステム。
- 請求項1記載のシステムおよび閉塞フィルターから本質的になるシステムであって、閉塞フィルターが送達ガイド部材の近位端に取り付けられるシステム。
- 送達ガイド部材が、バルーンカテーテルのルーメン内に配置される、請求項1記載のシステム。
- 以下を含む、インプラント送達システムであって、送達ガイド部材が少なくとも1つのインプラント送達直径に本質的に等しい少なくとも1つのインプラントの直径を遠位および近位に有し、これにより本質的に非外傷性であるインプラント送達システムが提供されるインプラント送達システム:
外面および内面を有する少なくとも1つのインプラントをユーザによる送達ガイド部材の操作によって解剖学的処置部位に向けるように形成され、近位端および遠位端を有する細長い送達ガイド部材;
少なくとも1つのインプラントの解放前には送達直径を有していて、解放前は送達ガイド部材の遠位端に隣接して配置されている、少なくとも1つのインプラント;ならびに
少なくとも一つの解放可能なジョイントが解放されるまでは少なくとも1つのインプラントの少なくとも1つの区画を送達直径に維持するように形成されている少なくとも1つの解放可能なジョイント。 - 以下を含む、解放可能なジョイントの連続的解放に適合しているインプラント送達システム:
外面および内面を有する少なくとも1つのインプラントをユーザの操作によって解剖学的処置部位に向けるように形成され、近位端および遠位端を有する細長い送達ガイド部材;
少なくとも1つのインプラントの解放前には送達直径を有していて、解放前は送達ガイド部材の遠位端に隣接して配置されている、少なくとも1つのインプラント;ならびに
複数の解放可能なジョイントであり、各ジョイントが解放可能なジョイントの解放まで少なくとも1つのインプラントの区画を送達直径に維持するよう形成されているジョイント。 - 以下を含む、管状器官内の標的部位を処置するためのシステム:
請求項6または7記載のインプラント送達システム;および
バルーンカテーテル。 - 送達ガイド部材が、その遠位端で閉じられている、請求項1、5または6記載のシステム。
- 遠位端が、非外傷性先端で閉じられている、請求項8記載のシステム。
- 少なくとも1つのインプラントの送達ガイド部材遠位側および近位側直径が、約0.010インチ〜約0.020インチであり、これにより薄型の送達システムが提供される、請求項1、5または6記載のシステム。
- 送達ガイド部材が、その近位端から遠位端への通路を有していない、請求項1、5または6記載のシステム。
- 少なくとも1つの解放可能なジョイントを解放するためのアクチュエータを更に含む、請求項1、5または6記載のシステムであって、送達ガイド部材がその近位端からその遠位端に通ずる通路を1本だけ有し、その通路がアクチュエータを含んでおり、そのアクチュエータが送達ガイド部材の遠位端を超えて伸びないシステム。
- 少なくとも1つの解放可能なジョイントを解放するためのアクチュエータが、送達ガイド部材の遠位端に取り付けられている、請求項12記載のシステム。
- 送達ガイド部材が遠位ガイド区画、近位ガイド区画、および遠位ガイド区画と近位ガイド区画の間の隙間を含み、少なくとも1つのインプラントが遠位ガイド区画と近位ガイド区画の間に配置されている、請求項1、5または6記載のシステム。
- 送達ガイド部材が、非膨張型である、請求項1、5または6記載のシステム。
- 送達ガイド部材が管形であり、その中にルーメンを有している、請求項1、5または6記載のシステム。
- 送達ガイド部材ルーメン内に、少なくとも一部が滑動的に配置され、送達ガイド部材内の軸方向の運動によって少なくとも1つの解放可能なジョイントを解放するように形成されているアクチュエータを更に含む、請求項16記載のシステム。
- 送達ガイド部材ルーメン内に、少なくとも一部が滑動的に配置され、送達ガイド部材内の回転運動によって少なくとも1つの解放可能なジョイントを解放するように形成されているアクチュエータを更に含む、請求項16記載のシステム。
- 少なくとも1つの解放可能なジョイントが、送達部材ルーメンに液圧を加えることで解放するように形成されていて、かつ、送達ガイド部材ルーメン内に少なくとも一部が滑動的に配置されていて、流体を選択された少なくとも1つの解放可能なジョイントに向け、それを解放するように形成されている流体検出装置を更に含む、請求項16記載のシステム。
- 少なくとも1つの解放可能なジョイントが、少なくとも1つの解放可能なジョイントに好適なDC電流が流されると解放するように形成されていて、かつ、好適なDC電流を供給して、それによって少なくとも1つの解放可能なジョイントを解放する、送達ガイド部材ルーメン内に少なくとも一部が配置されている導電体を更に含む、請求項16記載のシステム。
- 少なくとも1つの解放可能なジョイントおよび導電体が、二つ以上の解放可能なジョイントを同時に解放するように形成されている、請求項20記載のシステム。
- インプラントが、ステントである、請求項1、5または6記載のシステム。
- インプラントが、閉塞コイルである、請求項1、5または6記載のシステム。
- インプラントが、治療薬を更に含む、請求項1、5または6記載のシステム。
- 送達ガイド部材が、放射線不透過性マーカーを更に含む、請求項1、5または6記載のシステム。
- ユーザが、その近位端から送達ガイド部材を操作するだけで、冠動脈内にガイド部材の遠位端を向け、少なくとも1つのインプラントを導入することができるような可撓性および遠隔指向性を有する、請求項1、5または6記載のシステム。
- システムが、ガイドワイヤレスである、請求項1、5または6記載のシステム。
- 以下を含む、インプラント送達システム:
自己拡張型ステント;
近位端および遠位端を有し、小径の遠位区画および非外傷性先端を含むガイドワイヤ;ならびに
ステントを小径の区画の上に折りたたんだ形に保持するシース。 - 本質的に以下からなる、インプラント送達システム:
自己拡張型ステント;
近位端および遠位端を有し、小径の遠位区画および非外傷性先端を含むガイドワイヤ;ならびに
ステントを小径の区画の上に折りたたんだ形に保持するシース。 - シースが、バルーンカテーテルを含む、請求項28記載のシステム。
- 以下を含む、管状器官内の標的部位を処置するシステム:
請求項28、29または30記載のインプラント送達システム;および
シースを引っ込めるためのハンドル。 - 以下の段階を含む、インプラントを保持する方法:
2つの端部を有する、放射状に拡張可能なインプラントを提供する段階;
端部をとらえる段階;および
端部を引き、捻って、解放されるまでインプラントを折りたたんだ形に維持する段階。 - とらえる段階を不動部材で行う、請求項32記載の方法。
