Claims (34)
- 罹患血管を治療するためのシステムであって、
第1の位置から第2の位置へ半径方向に拡張するように構築および配列された拡張可能デバイスであって、前記拡張可能デバイスは、少なくとも2つの部分を備え、第1の部分は、前記第1の位置から前記第2の位置への半径方向拡張中に、塑性的に変形可能であり、第2の部分は、前記第2の位置で弾性的に付勢される、拡張可能デバイスと、
前記拡張可能デバイスの拡張に応答して拡張可能な膜であって、前記膜は、複数のポリマーストリップを備え、前記複数のポリマーストリップは、円周方向に前記拡張可能デバイスに巻き付けられ、前記拡張可能デバイスに固着され、前記膜は、1つ以上の細孔を備える少なくとも1つの多孔性部分を備える、膜と、
前記拡張可能デバイスの外面が血管の内面と係合し、前記血管を通る流体経路を維持するように、前記拡張可能デバイスを位置付けるように構築および配列された送達デバイスであって、前記送達デバイスは、近位部分と、前記近位部分よりも可撓である遠位区画とを有するカテーテルアセンブリを備え、前記カテーテルアセンブリは、
第1の管腔を含む第1のシャフトと、
第2のシャフトであって、前記第1のシャフトの側壁を通って退出する第2の管腔と、前記拡張可能デバイスの近位端において前記第2のシャフト上に位置付けられた第1のマーカーと、前記拡張可能デバイスの遠位端において前記第2のシャフト上に位置付けられた第2のマーカーとを含む第2のシャフトと
を含み、前記第1の管腔は、膨張管腔であり、前記第2の管腔は、ガイドワイヤ管腔である、送達デバイスと、
前記拡張可能デバイスの前記近位端に位置する少なくとも1つの近位リングおよび前記拡張可能デバイスの前記遠位端に位置する少なくとも1つの遠位リングと
を備え、前記少なくとも1つの近位リングおよび前記少なくとも1つの遠位リングの各々は、前記拡張可能デバイスを血管壁に係留するように構成されており、
前記膜は、前記少なくとも1つの近位リングおよび前記少なくとも1つの遠位リングの一方または両方に含まれず、これにより、前記膜を含まない前記近位端または前記遠位端が、前記膜を含む前記拡張可能デバイスの部分よりも大きく半径方向に変位することを可能にし、前記少なくとも1つの近位リングおよび前記少なくとも1つの遠位リングは、前記送達デバイスに含まれる前記第1のマーカーおよび前記第2のマーカーを固着するように構成された歯を含む、システム。 - 前記システムは、親血管から生じる頭蓋内動脈瘤を治療するように構築および配列され、前記親血管は、2.0mmから5.0mmの直径を備える、請求項1に記載のシステム。
- 前記拡張可能デバイスは、前記第1の位置から前記第2の位置への半径方向拡張中に、塑性的に変形させられるように構築および配列される、請求項1に記載のシステム。
- 前記拡張可能デバイスは、前記第2の位置で弾性的に付勢される、請求項1に記載のシステム。
- 前記拡張可能デバイスは、前記送達デバイス上に装填され、前記送達デバイスは、マイクロカテーテルと、ワイヤとを備える、請求項1に記載のシステム。
- 前記拡張可能デバイスは、第1の拡張可能デバイスであり、前記システムは、第1の位置から第2の位置へ半径方向に拡張するように構築および配列された第2の拡張可能デバイスをさらに備える、請求項1に記載のシステム。
- 前記第1の拡張可能デバイスは、自己拡張可能デバイスを備え、前記第2の拡張可能デバイスは、バルーン拡張可能デバイスを備える、請求項6に記載のシステム。
- 前記拡張可能デバイスは、2.0mmから5.0mmに及ぶ拡張外径を備える、請求項1に記載のシステム。
- 前記拡張可能デバイスは、7.0mmから40.0mmに及ぶ長さを備える、請求項1に記載のシステム。
- 前記拡張可能デバイスは、8本から38本のワイヤを備える、請求項1に記載のシステム。
- 前記少なくとも8本から38本のワイヤは、0.0005インチ(12.7μm)から0.004インチ(101.6μm)の間の直径を伴う少なくとも2本のワイヤを備える、請求項10に記載のシステム。
- 前記少なくとも2本のワイヤは、0.003インチ(76.2μm)の直径を伴うワイヤを備える、請求項11に記載のシステム。
- 前記拡張可能デバイスは、ワイヤフレームを備え、前記ワイヤフレームは、金属、形状記憶合金、形状記憶ポリマー、白金、タングステン、コバルト、クロム、およびそれらの組み合わせから成る群から選択される材料を有する、請求項1に記載のシステム。
- 前記拡張可能デバイスは、織物構成で少なくとも2本のワイヤを備えるフレームを備える、請求項1に記載のシステム。
- 前記織物構成は、0.26mm以下の幅を伴う菱形セルを画定する、請求項14に記載のシステム。
- 前記菱形セルの幅は、0.053mmから0.15mmに及ぶ、請求項15に記載のシステム。
- 前記菱形セルの幅は、0.13mmに近似する、請求項16に記載のシステム。
- 前記第1のマーカーおよび前記第2のマーカーは、X線または蛍光透視法を用いて視認することができる放射線不透過性マーカー、可視腔内カメラを用いて視認することができる可視マーカー、赤外線腔内カメラを用いて視認することができる赤外線マーカー、外部超音波または血管内超音波を用いて視認することができる超音波マーカー、MRIを用いて視認することができる磁気マーカー、およびそれらの組み合わせから成る群から選択される、請求項1に記載のシステム。
- 前記拡張可能デバイスは、ポリマー、ポリマーを含む膜、またはポリマーコーティングのうちの少なくとも1つを備え、前記ポリマーは、フッ素重合体、ポリイミド、シリコーン、ポリウレタン、ポリウレタンエーテル、ポリウレタンエステル、ポリウレタンポリカーボネート、ポリウレタン尿素、生体分解性ポリラクチド、ポリエーテル、ポリエチレングリコール(生体安定)、ポリ(DL−ラクチド−コ−カプロラクトン)(PLC)、ポリ(DL−ラクチド−コ−グリコリド)(PLGA)、ポリエステル、ポリカーボネートジオール、これらの共重合体、およびそれらの組み合わせから成る群から選択される、請求項1に記載のシステム。
- 前記ポリマーは、メチレンジイソシアネート、トルエンジイソシアネート、ヘキサメチレンジイソシアネート、ジイソシアネート、アルキルトリオール、トリアミン、オルトギ酸、リン酸塩、カルシトリオール、環状ポリオール、シケリトール、短鎖官能化アミノ酸、3つのヒドロキシル基によって特徴付けられるポリケチド、リピドイドCI2−200、フルオロアルカン、フルオロアルカノール、およびそれらの組み合わせから成る群から選択される、少なくとも1つの鎖延長剤を含む、請求項19に記載のシステム。
- 前記ポリマーは、少なくとも1つの末端基を備え、前記少なくとも1つの末端基は、前記ポリマーに組み込む前に官能化されている、請求項19に記載のシステム。
- 前記ポリマーは、アリルアルキル水酸化物またはアミン、シロキシ含有反応機能性、ポリメチオキシまたはポリエチオキシ低分子量錯体、およびそれらの組み合わせから成る群から選択される、反応的官能化ポリマーを含む、請求項19に記載のシステム。
- 前記拡張可能デバイスは、ハロゲン、セラミック、金属、放射線透過性および/または放射線不透過性材料を含むゲル、放射線透過性および/または放射線不透過性材料を含むゲル・ゾル、およびこれらの組み合わせから成る群から選択される、1つ以上の放射線透過性または放射線不透過性材料を含む、請求項1に記載のシステム。
- 前記膜の少なくとも一部分は、生体分解性材料を含む、請求項1に記載のシステム。
- 前記拡張可能デバイスは、薬剤、試薬、およびそれらの組み合わせから成る群から選択される、1つ以上の作用物質を含むポリマーを含む、請求項1に記載のシステム。
- 前記ポリマーは、デンドリマー型ポリマーを含み、前記デンドリマー型ポリマーは、デンドリマーまたは樹状突起を含む、請求項25に記載のシステム。
- 前記1つ以上の作用物質は、抗増殖剤、抗炎症剤、細胞再生促進剤、再狭窄阻害剤、ナノ粒子、薬剤溶出ナノ粒子、ナノ粒子ゲル、およびそれらの組み合わせから成る群から選択される、請求項25に記載のシステム。
- 前記膜は、0.0005インチ(12.7μm)から0.005インチ(127μm)の厚さを備える、請求項1に記載のシステム。

- 前記送達デバイスはさらに、前記第1の位置から前記第2の位置へ前記拡張可能デバイスを拡張するように構築および配列された膨張可能要素を備える、請求項1に記載のシステム。
- 血管壁と前記拡張可能デバイスとの間の管腔を排除するように構築および配列された事後拡張バルーンをさらに備える、請求項1に記載のシステム。
- 前記事後拡張バルーンは、前記拡張可能デバイスの遠位部分を拡張することに先立って、前記拡張可能デバイスの近位部分を拡張するように構成される、請求項33に記載のシステム。
