I.概要
本発明は、タウに結合する抗体を提供する。いくつかの抗体は、配列番号1の残基23〜46内のエピトープに特異的に結合する。いくつかの抗体は、リン酸化状態に関わりなくタウに結合する。本発明のいくつかの抗体は、タウに関連する病状およびタウに関連する徴候の悪化を抑制するかまたは遅らせるのに役立つ。機構を理解することは本発明の実施に必要ではないが、他のメカニズムの中で、非毒性高次構造を安定化するか、または病原性タウ形態の細胞間もしくは細胞内伝達を抑制することによって、毒性の低下がタウの抗体誘発性食作用の結果として生じ、タウが分子間もしくは分子内凝集するのを抑制するか、または他の分子に結合するのを抑制し得る。このような抗体を誘導する本発明の抗体または薬剤は、アルツハイマー病およびタウと関連する他の疾患の治療法または効果的な予防法において使用できる。

キメラ抗体、(ベニヤ抗体を含む)ヒト化抗体、またはヒト抗体の重鎖および軽鎖の可変領域は、ヒト定常領域の少なくとも一部に連結できる。定常領域の選択は、ある程度、抗体依存性補体および/または細胞媒介性細胞傷害が所望されるか否かに依存する。例えば、ヒトアイソタイプのIgG1およびIgG3は補体媒介細胞傷害を有するが、ヒトアイソタイプIgG2およびIgG4は、補体媒介細胞傷害をほとんどまたは全く有していない。軽鎖定常領域は、ラムダまたはカッパであり得る。抗体は、2つの軽鎖および2つの重鎖を含む四量体として、別々の重鎖、軽鎖として、Fab、Fab'、F(ab’)2、およびFvとして、または重鎖および軽鎖の可変領域がスペーサーを介して連結されている一本鎖抗体として発現させることができる。

上記のように、抗体は、最初に意図した結合特異性についてスクリーニングされ得る。能動免疫原は、同様に、そのような結合特異性を有する抗体を誘導する能力についてスクリーニングされ得る。この場合、能動免疫原を用いて実験動物を免疫し、得られた血清を適切な結合特異性について試験する。
神経原線維変化の存在は、アルツハイマー病、ダウン症候群、軽度認知障害、脳炎後のパーキンソニズム、外傷後の認知症もしくはパンチドランカー、C型ニーマン・ピック病、核上性麻痺、前頭側頭型認知症、前頭側頭葉変性症、筋萎縮性側索硬化症/グアムのパーキンソン認知症症候群、およびPSP進行性核上性麻痺を含むいくつかの疾患において見出されている。本投与計画はまた、これらの疾患のいずれかの治療または予防にも使用できる。神経疾患および病状とタウの間の広範囲に及ぶ関係のために、本投与計画は、神経疾患のない個人の平均値と比較して、(例えば、CSF内の)タウもしくはリン酸化タウのレベルの上昇を示す任意の対象の治療または予防に使用できる。本投与計画は、神経疾患と関連するタウの変異を有する個人における神経疾患の治療または予防にも使用できる。本方法は、アルツハイマー病、および特に患者における治療または予防に特に適している。
予防的適用では、抗体もしくは抗体を誘導するための薬剤またはその医薬組成物は、疾患(例えば、アルツハイマー病)の影響を受けやすい患者、またはそうでなければ疾患のリスクのある患者に、疾患のリスクを低減する、疾患の重症度を軽減する、または疾患の少なくとも1つの徴候もしくは症状の発症を遅延させるのに有効な投与計画(投与の用量、頻度および経路)で投与される。特に、投与計画は、好ましくは脳内でタウもしくはリン酸化タウおよびそれから形成される対のフィラメントを抑制もしくは遅延させるか、かつ/またはその毒性作用を抑制もしくは遅延させるか、かつ/または行動障害の発症を抑制もしくは遅延させるのに有効である。治療用途では、抗体または抗体を誘発する薬剤は、疾患の少なくとも1つの徴候または症状の改善または少なくともさらなる悪化を抑制するのに効果的な投与計画(投与の用量、頻度および経路)で疾患(例えば、アルツハイマー病)の疑いのある患者、または既に疾患に罹患している患者に投与される。特に、この投与計画は、好ましくは、タウ、リン酸化タウ、もしくはそれから形成される対のフィラメントのレベルのさらなる上昇、関連する毒性および/または行動障害を低減または少なくとも抑制するのに効果的である。
本発明は、患者におけるタウタンパク質沈着物(例えば、神経原線維変化およびタウ封入体)をインビボイメージングする方法を提供する。本方法は、タウに結合する抗体(例えば、マウス、ヒト化、キメラまたはベニヤ16B5抗体)などの薬剤を患者に投与し、その後、タウに結合した薬剤を検出することによって動作する。アミノ酸24〜46内のタウのエピトープに結合する抗体が好ましい。いくつかの方法において、抗体は、アミノ酸25〜44内またはアミノ酸30〜39内のエピトープに結合する。投与した抗体の除去反応は、Fabなどの全長の定常領域を欠く抗体断片を使用することによって回避または低減することができる。いくつかの方法において、同一の抗体は、治療および診断試薬の両方として機能し得る。
実施例1.抗体16B5の作製
リン酸化されているタウまたはされていないタウを認識するパン抗体16B5を精製したタウに対して作製し、ELISAアッセイでの高い親和性捕捉特性に基づいて選択した。
RNAを、トリゾールLS(Invitrogen社)を用いて、16B5抗体を発現するペレット化細胞から抽出した。RNA濃度を、Quant−ITキット(Invitrogen社)を用いて測定した。

ペプチド断片分析によるエピトープの同定
N末端インサートを含まず、4つの微小管結合リピートおよびP301L変異(rタウ)を含むヒトのタウ配列を大腸菌内で発現させ、精製した。タウのこの形態は、タウの最長のアイソフォームに基づく番号付け規則を使用してP301Lに相当する)243位のロイシンがプロリンに置き換わった配列番号3の配列を有する。200μgのタウの酵素消化を次の4つの異なるプロテアーゼのいずれかを用いて行った:(アルギニンおよびリジンのカルボキシル末端で切断する)トリプシン、(チロシン、トリプトファン、フェニルアラニンおよびロイシンのカルボキシル末端で主として切断する)キモトリプシン、(リジンのカルボキシル末端で切断する)LysC、または(グルタミン酸残基およびまれにアスパラギン残基の後ろで切断する)Gluc。全てのプロテアーゼをThermo Scientific社から入手し、消化を37℃で16時間行った。得られたペプチド断片を10μgの16B5と共にインキュベートし、プロテインG磁気ビーズ(NEB)を用いて沈殿させた。沈殿物を完全に300mMのNaClおよび0.5%NP−40を含有するPBS中で洗浄し、次いで、100mMグリシン中1MのNaCl(pH2.8)で溶出した。溶出物を真空乾燥し、0.1%トリフルオロ酢酸(TFA)に再懸濁した。再懸濁した溶出液を4.6×50mmのC18カラム上にロードし、その後、0.075%TFAとアセトニトリルの直線勾配を用いてHPLC(Agilent1260インフィニティ・システム)により分画した。ピーク画分を収集し、乾燥させ、蒸留水に再懸濁した。ペプチドの質量および同一性を、MALDI−TOF/TOFによって決定した。配列番号1の残基25〜44に対応するピークは、LysCのMSスペクトルで同定された。残基配列1の番号25〜44および24〜44に対応するピークを、トリプシンのMSスペクトルで同定した。キモトリプシンおよびGluC消化物からのシグナルが得られなかったことは、いくつかのエピトープが、配列番号1の残基29および/または配列番号1の残基37を含むことができることを示唆する。
(上述の)ペプチド断片分析によって決定された結果を用いて、rタウの欠失変異誘発を標準的な分子生物学的方法を用いる全プラスミドの増幅により行った。タンパク質を少量の細菌培養物中で発現させ、浄化した細菌溶解物の等容量を電気泳動し、ブロットし、16B5抗体で染色した。試料のローディングの対照には、タウのC末端領域(C末端エピトープ)に特異性を有する抗体のタウ46を用いて、重複ブロットを染色した。両方の抗体を0.2μg/mlの濃度で使用した。Licorオデッセイ蛍光スキャナーを用いて画像を取得した。タウの次の欠失変異体を作製し、このようにして分析した:Δ5−24、Δ23−32、Δ25−44、Δ30−39、およびΔ37−46。図1および図2に示すように、タウのΔ25−44およびΔ30−39欠失変異体は16B5抗体により検出されず、16B5によって認識されるエピトープがそれらの残基内にある証拠が得られた。タウのΔ37−46欠失変異体は16B5でごくわずかに検出可能であり、37−46内の残基のいくつか(例えば、残基37)が16B5のタウへの結合において役割を果たし得るという証拠が得られた。16B5抗体は、タウのΔ23−32欠失変異体をΔ5−24欠失変異体より少ない程度、かつΔ25−44およびΔ30−39欠失変異体よりも多い程度に染色し、16B5がまた、残基33〜36、30〜36、33〜37、30〜37または33〜39を含むペプチドに結合することができるという証拠が得られた。全体として、タウ欠失変異体から得られたデータは、16B5により認識されるエピトープが配列番号1の残基23〜32の一部または全部および配列番号1の残基37〜46の一部または全部を含み得ることを示唆する。例えば、16B5は、配列番号1の残基32〜38または28〜41内のエピトープを認識できる。
次に、残基30〜42に及ぶタウの領域内の単一残基を、PCR変異誘発を用いてアラニン変異させた。変異したタンパク質を発現させ、上記のように溶解物を電気泳動によって分離し、16B5抗体またはタウ46抗体のいずれかを用いてブロットした。この分析の結果を図3に示す。T30A、M31A、H32A、Q33A、D34A、Q35A、E36A、G37A、D38A、T39A、D40A、A41L、およびG42Aを含む分析した特定の点変異体をブロット上に列挙する。特に関心のある残基を各ブロット上にボックスで囲んである。16B5の検出可能な結合は、Q33Aタウ変異によって完全に除去され、G37Aタウ変異によって実質的に減少し、残基33およびより少ない程度の残基37が16B5によって認識されるエピトープの重要な構成要素であり得る証拠が得られた。他の残基は、ビアコア分析において16B5によって認識されるエピトープの重要な構成要素であり得る。
免疫
FVB/N遺伝的バックグラウンドの3ヶ月齢のhタウ.P301L−Tgの雌マウスをこの研究に使用した。10mg/kgの試験抗体および対照抗体の投与を、週に一度、腹腔内に行った。治療期間は約5ヶ月であった。23回の注射の後、マウスを屠殺して本研究を終えた。本研究で投与した試験抗体および対照抗体を表1に記載する。
従来のSDS−PAGEおよびウェスタンブロッティングの適用のために、試料を変性させ、10分間95℃でインキュベートすることによって還元し、7.5%のTris−HClゲル(Criterion XTプレキャストゲル、26ウェルコーム、15μl、1.0mm;Biorad社)上で分離した。PVDF−膜(iBlot(商標)ゲルトランスファースタック(Gel Transfer Stack)、PVDF、レギュラー、Invitrogen社)を乾燥電気泳動転写(dry electrotransfer(iBlot(商標)Invitrogen社))した後、膜を30分間、0.4%PFA中で洗浄し、その後、トリス緩衝食塩水で洗浄した。次に、膜を5%(w/v)脱脂粉乳および0.1%(v/v)Tween−20を含有するトリス緩衝生理食塩水(TBS、pH7.6)中で1時間インキュベートした。ブロットを、表3に示した作業濃度で、様々な抗タウ一次抗体と一晩インキュベートした。洗浄し、抗マウスまたは抗ウサギHRP結合二次抗体(ヤギ抗マウスまたはヤギ抗ウサギIgG、DAKO)とインキュベートした後に、ブロットをECL検出システム(スーパーシグナルウェストフェムト最大感度基質(SuperSignal West Femto Maximum Sensitivity Substrate)、製品番号34096、Thermo Scientific社)で発色させた。異なる露光時間で画像をデジタル記録(VisionWorks Acquisition、UVP)し、専用ソフトウェア(VisionWorks Analysis、UVP)をブロットの分析のために使用した。比較のために、インターゲルリファレンスゲルの泳動を行い、比較のために4画分のアリコートを各ゲル上で移動させた。検出のために使用した抗タウ一次モノクローナル抗体には、AT100(リン酸化タウ、Thermo Scientific社、希釈1:250)、AT8(リン酸化タウ、Thermo Scientific社、希釈1:500)、HT7(パンタウ、Pierce社、希釈1:1000)、および1F5(試験施設には知られていないエピトープ、Neotope、希釈3:500)が含まれていた。ブロットを、ローディングコントロールとして抗GAPDHで再プローブした(Abcam9485;希釈1:2500)。パンタウ抗体は、リン酸化タウに特異的ではない。
視床下核アネックス不確帯(STH/ZI)および小脳の挿入核、前方および後方部、アネックス外側小脳核(IntA/P/LAT)において、抗リン酸化タウ抗体を用いた免疫組織化学的分析を行った。矢状ビブラトーム切片(40μm)を0.1%アジ化ナトリウムを含むPBS中で、使用するまで4℃で保存した。示したブレグマで、マウスあたり8つの切片をmAbのAT8、AT100または1F5でフリーフローティング染色した。切片を、以下の表4に記載の示した抗体での染色のために選択した。特定の染色のために選択した全ての動物の切片を、染色および盲検定量化のために無作為化した。
配列解析により、16B5抗体が、マウスカバットサブグループ1に属し、ヒトカバットサブグループ4に対応する配列番号16の配列を有する可変カッパ(Vk)ドメインを有することが示される。カバットCDRに下線が引いてある。16B5抗体の可変重鎖(Vh)ドメインは、マウスカバットサブグループ2bに属し、ヒトカバットサブグループ1に対応する配列番号10の配列を有する(Kabat et al.(1991)、免疫学的に興味のあるタンパク質の配列(Sequences of Proteins of Immunological Interest)、第5版、NIH公開番号91−3242)。カバットCDRに下線が引いてある。
ので、位置98にはFを選択した。
バージョン1(L1)は、配列番号21の配列を有し、3つの復帰突然変異のDIN、M4L、およびY36Fを含む。HCDR2のN61と潜在的な水素結合を形成するため、1位にNを選択した。LCDR3のK96、Q97およびS98と接触するので、4位にLを選択した。これは界面の残基のF104とも接触する。Fはできないが、YがHCDR3のD106と水素結合できるので、36位にFを選択した。水素結合は、HCDR3機能に影響を与える可能性がある余分な相互作用を構成し、したがって、好ましくは回避される。
H1L1またはH1L2設計を有するヒト化16B5抗体についての結合データを以下の表7に示す。比較のために、キメラ16B5についての結合データも示す。ビアコア装置を用いてデータが得られた。バージョンH1L2は、キメラ16B5のものと本質的に同じである最も強い親和性を有すると結論付けられた。ヒト化16B5バージョンH1L1およびH1L3も十分な親和性を有していた。
6のBraakスコアを有するアルツハイマー病患者の前頭皮質の死後試料を、次の順序で、可溶化力を増加させる緩衝液で連続的に抽出した:(i)高塩緩衝液(20mMのトリス、5mMのEDTA、1mMのDTT、10%スクロース、7500mMのNaCl pH7.4)、(ii)トリトン緩衝液(20mMのトリス、5mMのEDTA、1mMのDTT、10%スクロース、1%のトリトンX100、500mMのNaCl pH7.4)、(iii)サルコシル緩衝液(10mMのトリス、5mMのEDTA、1mMのDTT、10%スクロース、500mMのNaCl、1%サルコシル、pH7.4)。
マウスモノクローナル抗タウ抗体16B5ならびにその2つのヒト化変異型であるh16B5−H1L2およびh16B5−N1Dを、アルツハイマー病のドナーおよび非罹患高齢者対照からのヒト脳皮質の新鮮凍結切片上で免疫組織化学的試験も行った。
ヒト脳組織
前頭皮質を、サン衛生研究所(Sun Health Research Institute)から入手した。症例には、アルツハイマー病と診断され、死後の神経病理学的評価で確認された6人の患者(平均年齢86.8±0.40SEM)、および3人の非罹患高齢者対照(平均年齢77±9.7SEM)が含まれた。症例の人口統計学データを以下の表8に記載する。免疫組織化学を、スライドにマウントし、軽くアセトン固定した10μmの凍結切片上で行った。
免疫ペルオキシダーゼ法は、ヤギ抗マウス免疫グロブリンにコンジュゲートしたペルオキシダーゼ標識ポリマー(EnVision+システムHRP標識ポリマー;Dako K4001)または直接ビオチン化したヒト化抗体用のベクターABC増幅システム(ABC Elite Standard;PK−6100;Vector Laboratories)のいずれかからなる主要な検出システムであった。染色をDAB色原体(液体DAB+基質色素原システム;Dako K3468)を用いて可視化し、茶色の堆積物が生成した。
二重免疫蛍光染色を行い、抗体のマウス変異型およびヒト化変異型、様々なリン酸化エピトープを認識する他のタウ抗体と、アミロイドベータの関係を決定した。組織切片を、ビオチン化またはFITCタグ付きヒト化16B5変異型(1μg/mL)およびマウス抗体(モノクローナル16B5(1μg/mL)、AT8(1:1000)、AT100(1:1000)、または3D6(μg/mL)のいずれか)を含む抗体カクテルで並行して染色した。マウス抗体は、488または635フルオロフォア(Invitrogen社)にコンジュゲートしたヤギ抗マウス二次抗体で検出した。ビオチニル化ヒト化抗体は、ストレプトアビジン635で検出した。
標的抗原に対する抗体の特異性を評価するために、1μg/mLの16B5抗体を4℃で一晩、50μg/mLの精製ヒトP301Lタウまたは野生型シヌクレイン(無関係なタンパク質)でプレ吸収した。次いで、これらの抗体を組織に適用し、上記のように免疫組織化学手順を行った。
スライドをオリンパスBX61顕微鏡、オリンパスNanozoomer 2.0HT、またはライカSPEスペクトル共焦点システムのいずれかを用いて画像化した。画像は、TIFFファイルとして収集した。
以下の表9に示すように、マウスモノクローナル抗体16B5および両方のヒト化変異型は、アルツハイマー病の組織上で応答性を示し、神経網スレッドを顕著に、神経原線維変化(主に球形)およびいくつかのタウ陽性老人斑をある程度染色した。ほとんどの16B5AD−線維性病変は灰白質に限られていたが、ある程度の応答性が白質でも検出された。対照的に、非疾患対照組織は、拡散背景(diffuse background)応答性を示したが、AD組織で検出される病状については陰性であった。
