Abstract
【解決手段】システムは、少なくとも注入手順の一部に基づいて注入手順の少なくとも1つのフェーズのパラメータを決定するパラメータ生成システムを含む。パラメータ生成システムは、少なくとも一部が薬剤中の作用物質の濃度に基づき、且つ少なくとも一部が患者パラメータに基づき、患者のパラメータとともに変化する関数に基づいて、患者に送出されるべき薬剤の体積を決定する。患者のパラメータは例えば、体重、肥満度指数、体表面積又は心拍出量である。薬剤は例えば、画像化手順に用いる造影強化物質を含む。

Description
本発明は、2006年12月29日に出願した米国仮特許出願第60/877,779号、及び2007年7月28日に出願した米国仮特許出願第60/976,002号の利益を要求し、それらの内容は引用を以て、本願への記載加入とする。
本願は、2005年11月23日に出願された公開された国際出願WO/2006/058280(国際特許出願PCT/US05/042891)、及び公開された国際出願WO/2006/055813(国際特許出願PCT/US2005/041913)に開示され及び/又は権利要求された事項に関する事項を含み、それらの開示は引用を以て、本願への記載加入とし、本願の一部とする。
本発明は、流体を搬送する装置、システム及び方法に関し、特に患者に薬剤流体を送出する装置、システム及び方法に関し、特に医療注入手順中に患者に造影剤を送出する装置、システム及び方法に関する。
一態様に於いて、本発明は、少なくとも1部が注入手順のタイプに基づいて、注入手順の少なくとも第1フェーズ(例えば、複数のフェーズ)のパラメータを決定するパラメータ生成システムを含むシステムを提供する。パラメータ生成システムは、少なくとも1部が薬剤内の物質濃度に基づいて、及び少なくとも1部が患者パラメータに依存し患者パラメータとともに変化する機能に基づいて、患者に送出されるべき薬剤の体積を決定する。患者パラメータは例えば、体重、肥満度指数、体表面積又は心拍出量である。薬剤は例えば、画像化手順に用いられる造影強化物質を含む。
V1=体重×X×Y
を用いて計算され、ここでV1は第1部分の体積であり、Xは体重の関数、Yは薬剤中の造影強化物質の濃度の関数である。Xは例えば、アルゴリズムから特定の患者の体重(又は他の患者パラメータ)について決定され、Xは体重の関数として演算される。Xは代わりに、テーブルから特定の患者の体重(又は他の患者パラメータ)について決定され、Xは体重の関数として示される。
注入手順に関してここで用いられるように、語句「プロトコル」は、注入手順時に患者に送出されるべき流体の量を規定する流量、注入される体積、持続時間等のようなパラメータの群を指す。そのようなパラメータは注入手順の経過に亘って、変化することができる。ここで用いられるように、語句「フェーズ」は一般に、或る期間(又はフェーズ持続時間)中に患者に送出されるべき流体の量を規定するパラメータの群を指し、該期間は注入手順の総持続時間よりも短くなり得る。このようにして、フェーズのパラメータは、フェーズの持続時間に対応した時刻に亘って注入を記述する。特定の注入手順についての注入プロトコルは例えば、単フェーズ(単一のフェーズ)、2フェーズ(2つのフェーズ)又は多フェーズ(2以上のフェーズであるが、一般的に2を超えるフェーズである)として記載される。多フェーズ注入はまた、少なくとも1部の注入手順に亘ってパラメータが連続的に変化する注入を含む。

Claims (18)
- (a) ユーザインターフェイスと、
(b) メモリシステムと、
(c) 前記ユーザインターフェイスと前記メモリシステムとに動作可能に接続されたプロセッサと、
を備えたパラメータ生成システムにおいて、
前記プロセッサは、前記メモリーシステムに格納されたアルゴリズムを実行し、
前記アルゴリズムは、注入手順の少なくとも第1フェーズのパラメータを、前記ユーザインターフェイスを介して特定された前記注入手順のタイプに少なくとも部分的に基づいて決定し、
前記パラメータ生成システムは、前記ユーザインターフェイスを介して特定された薬剤中の作用物質の濃度に少なくとも部分的に基づいて、且つ、所定の関数Xに少なくとも部分的に基づいて、患者に送られる薬剤の量を決定し、前記所定の関数Xは、前記患者のパラメータに依存しており、前記患者のパラメータによって変化し、
前記関数Xは、アルゴリズム又はテーブルで与えられ、
前記患者のパラメータは、体重、肥満度指数、体表面積及び心拍出量の少なくとも1つであり、
前記薬剤のみが注入される前記第1フェーズにて注入される前記薬剤の量V1は、以下の式、
V1=体重*X*Y
で決定され、Yは、前記薬剤の作用物質の関数であるパラメータ生成システム。 - 前記薬剤の作用物質は、画像化手順に用いられる造影増強物質である、請求項1に記載のパラメータ生成システム。
- 前記所定の関数Xは、前記患者の体重、肥満度指数、体表面積及び心拍出量の少なくとも1つについて、前記アルゴリズムから決定される、請求項2に記載のパラメータ生成システム。
- 前記所定の関数Xは、前記患者の体重、肥満度指数、体表面積及び心拍出量の少なくとも1つについて、前記テーブルから決定される、請求項2に記載のパラメータ生成システム。
- 前記パラメータ生成システムはまた、少なくとも第2フェーズで送られる前記薬剤の量V2を決定し、前記第2フェーズでは前記薬剤及び希釈液の両方が患者に送られる、請求項2に記載のパラメータ生成システム。
- 前記第1フェーズでの前記薬剤の流量は、前記第2フェーズでの前記薬剤及び前記希釈液の混合物の流量にほぼ等しい、請求項5に記載のパラメータ生成システム。
- 前記薬剤の流量は、V1を前記第1フェーズの注入持続時間で除することによって計算される、請求項6に記載のパラメータ生成システム。
- 前記第1フェーズの注入持続時間は、画像化システムの走査持続時間に因子Kを加えることによって決定される、請求項7に記載のパラメータ生成システム。
- 前記因子Kは0から10秒の範囲である、請求項8に記載のパラメータ生成システム。
- 前記第1フェーズの最短の注入持続時間が、前記パラメータ生成システムに入力され得る、請求項9に記載のパラメータ生成システム。
- 前記第1フェーズの流量は、V1を前記第1フェーズの注入持続時間で除することによって計算される、請求項10に記載のパラメータ生成システム。
- 前記第2フェーズにて前記患者に送られる造影剤の量V2は、少なくとも部分的に前記患者のパラメータの少なくとも1つに基づいて演算される、請求項6に記載のパラメータ生成システム。
- 前記パラメータ生成システムは、前記注入手順の全てのフェーズに於いて、前記患者に送出される薬剤の総量が、前記患者に患者に送るのに利用できる前記薬剤の量を超えているか否かを決定し、前記パラメータ生成システムは更に、決定された総量が利用できる量を超えていれば、前記薬剤の総量を減らすルールを具えている、請求項12に記載のパラメータ生成システム。
- 前記注入手順の全てのフェーズで前記患者に送られる前記薬剤の総量が決定される、請求項5に記載のパラメータ生成システム。
- 前記総量を決定するために前記パラメータ生成システムによって決定された初期プロトコルは、前記患者の心臓血管系の描写に基づいて調整される、請求項14に記載のパラメータ生成システム。
- 前記薬剤のテストボーラスが、前記患者の心臓血管系を描写するために前記患者に実行される、請求項15に記載のパラメータ生成システム。
- 前記初期プロトコルは、少なくとも部分的には前記テストボーラスの増強ピークに至る時間に基づいて、調整され得る、請求項16に記載のパラメータ生成システム。
- 前記初期プロトコルは、少なくとも部分的には前記テストボーラスの増強ピークに至る時間及び増強ピークの増強レベルに基づいて調整される、請求項16に記載のパラメータ生成システム。
