Abstract
Description
本出願は、2008年7月15日出願の米国仮出願61/134995号優先権の利益を主張し、この内容全体を本明細書において援用する。
膨張可能な弁の設計では重要な課題がある。たとえば、弁は体内への進入を容易にするために比較的小さな直径まで潰れ可能であるのが望ましい。これにより、カフの設計、たとえばカフ内に組み込み可能な材料の厚みに深刻な制限が課される。しかし、ステント体が所定位置で弁を保持するようにステント体の周囲の自然組織にしっかり係合する作動、膨張状態まで、ステント体は膨張可能でなければならない。通常弁周囲逆流と呼ばれる、人工弁の外側部分周りの漏出を防ぐように、ステント体とステント体上に達したカフは、その周囲の自然組織とともに良好な封止部を形成しなければならない。しかし、ステント体とカフは、自然弁の弁輪に過度な力を印加してはならない。自然大動脈弁の弁輪に過度な力が印加されると、心臓の電気伝導系を乱し、僧帽弁の機能を損なわせる可能性もある。これらの問題は、病変した自然弁尖や他の病変組織により埋め込み部位が不規則になるという事実によって複雑化している。たとえば、石灰化または狭窄化した大動脈弁は、現在の潰れ可能な弁設計では十分に取り扱われない場合があって、(1)弁周囲逆流(PV漏出)、(2)弁移動、(3)僧帽弁衝突、(4)伝導系混乱などの問題に遭遇する場合もある。これらのすべてが不都合な臨床転帰に繋がりかねない。これらの不都合な事象を低減するために光学的弁が用いられ、この光学的弁は、隣接する組織や生理機能を阻害しうる過度な半径方向の力を必要とせずに十分な封止機能および固定機能を果たす。
Claims (32)
- 自然弁輪を有する自然心臓弁を置換する人工心臓弁であって、
(a)近位遠位軸線を有するほぼ管状の輪状部を含むとともに、半径方向潰れ状態および半径方向膨張状態を有するステント体であって、該半径方向潰れ状態から該半径方向膨張状態へと遷移する間に、前記輪状部の直径が増加する、ステント体と、
(b)前記ステント体に取り付けられた1以上の人工弁要素であって、前記輪状部を通る近位方向の流れを許容するものの、該輪状部を通る該近位方向の流れを実質的には遮断するように作動する人工弁要素と、
(c)前記ステント体に保持されたカフであって、
(i)前記輪状部の少なくとも一部を覆い、前記自然弁輪に配置される第1のカフ部であって、該輪状部が前記半径方向膨張状態にある際に第1の半径を有する第1のカフ部と、
(ii)前記第1のカフ部に近接しているとともに、前記輪状部が前記半径方向膨張状態にある際に第2の半径を有する第2のカフ部であって、該第2の半径が前記第1の半径よりも大きく、前記自然弁輪に近接した自然組織に係合するようにされた第2のカフ部と、
を有するカフと、

人工心臓弁。 - 前記ステント体は、前記輪状部に近接したフレア部を含み、
該フレア部は、前記ステント体が前記膨張形態にある際に前記輪状部よりも直径が大きく、
前記第2のカフ部の少なくとも一部は、前記ステント体の前記フレア部に沿って延びている、
請求項1に記載の弁。 - (a)近位遠位軸線を有するほぼ管状の輪状部を含むとともに、半径方向潰れ状態および半径方向膨張状態を有するステント体であって、該半径方向潰れ状態から該半径方向膨張状態へと遷移する間に、前記輪状部の直径が増加し、該輪状部の軸線方向長さが減少するステント体と、
(b)前記ステント体に取り付けられた1以上の人工弁要素であって、前記輪状部を通る近位方向の流れを許容するものの、該輪状部を通る該近位方向の流れを実質的には遮断するように作動する人工弁要素と、
(c)前記ステント体に保持され、前記輪状部を取り囲んでいるカフであって、前記ステント体が前記半径方向潰れ状態から前記半径方向膨張状態へと遷移する際に軸線方向には潰れ、半径方向には膨張するようにされた1以上のひだを有するカフと、
を有する、
人工心臓弁。 - (a)近位遠位軸線を有するほぼ管状の輪状部を含むステント体と、
(b)該ステント体に取り付けられた1以上の人工弁要素であって、前記輪状部を通る近位方向の流れを許容するものの、該輪状部を通る該近位方向の流れを実質的には遮断するように作動する人工弁要素と、
(c)前記ステント体に保持され、前記輪状部を取り囲んでいるカフと、
(d)前記カフから離れ、前記ステント体と前記カフとに機械的に接続された1以上の付勢要素であって、前記ステント体に対して外方に前記カフの少なくとも一部を付勢するようにされた付勢要素と、
を有する、
人工心臓弁。 - 前記ステント体は、半径方向潰れ状態および半径方向膨張状態を有する膨張可能な金属製ステント体であり、
該半径方向潰れ状態から該半径方向膨張状態へと遷移する間に、前記輪状部の直径が増加する、
請求項6に記載の弁。 - 前記1以上の付勢要素は1以上のばねを含む、請求項5に記載の弁。
- 前記ばねは前記ステント体と一体的に形成されている、請求項6に記載の弁。
- 前記ばねは複数のコイルばねを含み、各コイルばねは、ばね軸線と、該ばね軸線の周りの複数の湾曲部と、該複数の湾曲部内を延びる細長い部材と、を有する、請求項6に記載の弁。
- 各ばねの前記湾曲部は、前記ばね軸線を横切る軸線の周りに湾曲しているとともに前記ステント体の前記近位遠位軸線に平行である、請求項8に記載の弁。
- 前記1以上の付勢要素は吸湿材を含む、請求項5に記載の弁。
- 前記1以上の弁要素は、前記輪状部内の少なくとも一部に配置された複数の人工弁尖を含み、かつ、
前記ステント体が少なくとも前記半径方向潰れ状態にある際に、前記吸湿材は、前記近位方向または遠位方向において前記弁尖からずれている、
請求項10に記載の弁。 - 前記吸湿材は、前記輪状部の周りを延びる螺旋状の吸湿要素を含む、請求項10に記載の弁。
- 前記1以上の付勢要素は、前記輪状部の周りを周方向に延びる弾性リング部を含み、
該リング部は、前記ステント体に保持された基部と、前記ステント体から離脱した自由縁部と、を有し、
前記リング部が半径方向に膨張する際には、該リング部が変形し、該変形によって前記自由縁部を前記基部および前記ステント体に対して外方に付勢するように前記リング部が構成されている、
請求項5に記載の弁。 - (a)近位遠位軸線を有するほぼ管状の輪状部を含むとともに、半径方向潰れ状態および半径方向膨張状態を有するステント体であって、該半径方向潰れ状態から該半径方向膨張状態へと遷移する間に、前記輪状部の直径が増加する、ステント体と、
(b)前記ステント体に取り付けられた1以上の人工弁要素であって、前記輪状部を通る近位方向の流れを許容するものの、該輪状部を通る該近位方向の流れを実質的には遮断するように作動する人工弁要素と、
(c)前記輪状部の周りを延びる前記ステント体に保持され、該ステント体の周りを周方向に延びるリング部を含むカフであって、該リング部が、前記ステント体に保持された基部と、前記ステント体から離脱した自由縁部と、を有し、前記リング部が半径方向に膨張する際には、該リング部が変形し、該変形によって前記自由縁部を前記基部および前記ステント体に対して外方に付勢するように前記リング部が構成されている、カフと、
を有する、
人工心臓弁。 - (a)近位遠位軸線を有するほぼ管状の輪状部を含むとともに、半径方向潰れ状態および半径方向膨張状態を有するステント体であって、該半径方向潰れ状態から該半径方向膨張状態へと遷移する間に、前記輪状部の直径が増加する、ステント体と、
(b)前記ステント体に取り付けられた1以上の人工弁要素であって、前記輪状部を通る近位方向の流れを許容するものの、該輪状部を通る該近位方向の流れを実質的には遮断するように作動する人工弁要素と、
(c)前記輪状部の周りを延びる前記ステント体に保持され、可動部を有するカフであって、前記ステント体が前記半径方向潰れ状態にある際に、該カフの該可動部が前記ステント体の前記輪状部から軸線方向においてずれるように、かつ、前記カフの前記可動部が前記輪状部の周りを延びる位置である作動位置まで前記可動部が変位可能であるように、前記可動部が前記ステント体に対して軸線方向に移動可能である、カフと、
を有する、
人工心臓弁。 - 前記カフはほぼ管状の壁を有し、該管状壁は、前記ステント体に取り付けられた固定端部と、前記ステント体が前記半径方向潰れ状態にある際に、前記輪状部から軸線方向に突出する自由端部と、を有し、
前記カフの前記可動部は、前記管状壁の前記自由端部を含み、
前記管状壁は、その前記自由端部を前記作動位置へと運ぶように裏返しになって回転できるように構成されており、

(c)前記輪状部の外側に1以上のポケットを定めるカフであって、各ポケットは外壁と、第1の軸線方向を向く開放側面と、該第1の軸線方向の反対側に位置する第2の軸線方向を向く閉鎖側面と、を有し、該第2の軸線方向における血流が前記ポケット内に強制的に流される傾向になるとともに、該血流が前記ステント体に対して前記ポケットの前記外壁を外方に付勢する傾向になる、カフと、
を有する、
人工心臓弁。 - 前記1以上のポケットは、遠位方向を向く開放側面を有する複数のポケットを含む、請求項20に記載の弁。
- (a)近位遠位軸線を有するほぼ管状の輪状部を含むステント体と、
(b)前記ステント体に取り付けられた1以上の人工弁要素であって、前記輪状部を通る近位方向の流れを許容するものの、該輪状部を通る該近位方向の流れを実質的には遮断するように作動する人工弁要素と、
(c)周方向に互いに離間して配された複数の領域を有し、前記輪状部の周りを延びるカフであって、該複数の領域の各々は半径方向厚みを有し、該カフが作動形態にある際に、前記複数の領域の少なくとも1つの領域の前記半径方向厚みが、該複数の領域の少なくとも1つの他の領域の該半径方向厚みとは異なっている、カフと、
を有する、
人工心臓弁。 - 前記複数の領域の少なくとも幾つかは中空チャンバを含み、該中空チャンバを膨張させることで前記カフを前記作動形態に導くことが可能である、請求項22に記載の弁。
- 前記中空チャンバの少なくとも1つは、他の前記中空チャンバの少なくとも1つと独立して膨張可能であるように構成されている、請求項23に記載の弁。
- 前記複数の領域は、周方向に互いに離間して配置された3つの隆起領域および該隆起領域間に配置された中間領域を含み、該隆起領域の半径方向厚みは、該中間領域の半径方向厚みよりも大きい、請求項22に記載の弁。
- 前記複数の領域は、周方向に互いに離間して配置された2つの隆起領域および該隆起領域間に配置された中間領域を含み、該隆起領域の半径方向厚みは、該中間領域の半径方向厚みよりも大きい、請求項22に記載の弁。
- 前記1以上の弁要素は複数の可撓性人工弁尖を含み、該複数の可撓性人工弁尖は前記輪状部内に少なくとも部分的に配置されている、請求項1から26のいずれか1項に記載の弁。
- (a)ステント体と、該ステント体に接続された1以上の人工弁要素と、前記ステント体の少なくとも一部上を延びているカフと、を含む人工弁を前記ステント体および前記カフが半径方向潰れ状態にある間に、患者内に挿入する挿入ステップと、
(b)前記ステント体および前記カフを半径方向膨張状態に誘導し、前記カフの第1の部分が自然弁輪に係合するとともに前記カフの第2の部分が該自然弁輪に隣接した自然組織に係合するように、前記ステント体および前記カフを位置決めする誘導、位置決めステップと、
(c)前記カフの前記第2の部分を前記自然組織に取り付ける取り付けステップと、
を有する、
患者の治療方法。 - 前記カフの前記第2の部分は、縫合またはステープル留めによって前記自然組織に取り付けられる、請求項28に記載の方法。
