Esillä olevan keksinnön kohteena on suonikatetriside, johon kuuluu kantokalvo jonka toisella puolella on tarrakerros, jota peittää suo-jaava, irtirepäistävä päästökalvo jolloin kantokalvon, tarrakerroksen ja päästökerroksen läpi on järjestetty rako siten toisesta reunasta sen keskiosaan,
Suonikatetri koostuu tavallisesti ohuesta letkusta, jonka toinen pää on pujotettu paksumpaan putkeen, jossa on kaksi sivuttain kulkevaa siipeä. Putkessa voi olla joko hana tai se voidaan sulkea ja avata tulpalla, joka putkesta irti ollessaan on sijoitettu toisessa siivessä olevaan tulpan pitimeen. Katetrin letku pistetään neulan avulla ihon läpi ja potilaan laskimoon. Tämän jälkeen putken läpi voidaan hanan ollessa auki ja putken ilman tulppaa ottaa verinäytteitä ja ruiskuttaa lääkkeitä, nukutusaineita tai ravintoliuoksia. Tällainen katetri voi olla paikallaan potilaassa lyhyemmän tai pidemmän ajan.
Eräänä suurena ongelmana suonikatetrien kiinnityksessä on pitää pisto-kohdan ympäristö steriilinä ja puhtaana verestä ja pitää samanaikaisesti katetri paikallaan liian suurten letkun sisään- ja ulosmenevien 2 69756 liikkeiden välttämiseksi. Näistä syistä on tähän mennessä käytetty laastareiden ja löysien kääreiden yhdistelmää, jotka vaihdetaan tarvittaessa kerran tai pari vuorokaudessa. Käyttämällä useita komponentteja steriliyden ylläpito vaikeutuu. Eri henkilöt, jotka kiinnittävät tai vaihtavat näitä komponentteja, käyttävät edelleen eri määriä laastareita ja siteitä ja kiinnittävät niitä eri tavoin. Tästä syystä katetri kiinnittyy paikalleen eri tavoin eri kerroi1 la.
Yhtenäistä sidettä, joka samanaikaisesti pitää pistohaavan kuivana ja bakteerittomana, ei ole olemassa. Useita ratkaisuja on ehdotettu katetriletkuiksi ja vuotoletkuiksi, joissa ei ole yhdysputkia eikä hanaa tai tulppaa pistokohdan läheisyydessä. USA-patenttiju1 kaisussa 3 138 158 on esitetty kolmi- tai nelihaarainen laastari, jonka keskiosassa on kolme reikää. Yksi reikä asetetaan suoraan pistokohdan päälle ja katetriletku pudotetaan ylöspäin reiän läpi, alaspäin toisen reiän läpi ja jälleen ylöspäin kolmannen reiän läpi. Tällä tavoin arvellaan saatavan aikaan letkun varma kiinnittyminen. Eräs toinen laastarivaihtoehto on suorakulmio, jossa on liuskoilla tai kaistaleilla varustettu keskeinen reikä ja rako lähimpään sivureunaan. Katetriletku pujotetaan raon avulla reiän läpi ja liuskat kiinnitetään letkuun, jolloin letkun oletetaan pysyvän paikallaan. Tässä ei ole kuitenkaan mahdollisuutta pitää haavaa puhtaana verestä tai imeä pois esimerkiksi sisäänruiskutettua ravintoliuosta, joka tunkeutuu ylöspäin pistoreiän kautta. Olisi erittäin vaikeata yhdistää side tällaiseen laastariin. Edelleen esitettyä laastaria ei voi käyttää sellaisen katetrin yhteydessä, jossa on yhdysputki ja hana.
USA-patenttijulkaisussa 3 683 911 on esitetty bakteereja ehkäisevä kiinnityslaite yksinkertaiseen katetriletkuun. Laitteeseen kuuluu pöhjalaastari, josta pistää putki ylöspäin, laastarin alasivu on päällystetty liimalla ja bakteereja tappavalla aineella. Sama päällys on putken sisäpuolessa. Putkessa ja laastarissa on rako, joka mahdollistaa laitteen sijoittamisen katetri letkun ympärille. Laastarin ja putken liiman avulla katetriletku pysyy paikallaan eivätkä bakteerit pääse tunkeutumaan pistohaavaan. Tässä ei kuitenkaan ole mahdollisuutta yhdistää laitetta, imevään kappaleeseen, esimerkiksi siteeseen, joka pitää haavan verestävänä. Laitetta ei myöskään voida käyttää yhdys-putkilla ja hanalla varustetuissa katetreissa ja lisäksi on erittäin vaikeata vaihtaa tällainen laastari ja samanaikaisesti irroittaa sekä 3 69756 letku laitteen putkesta että laastari potilaan ihosta siirtämättä letkua samanaikaisesti haitallisesti ja ärsyttämättä pistohaavaa
Esillä olevan keksinnön tarkoituksena on saada aikaan suonikatetri-side, jossa on putken muotoinen yhdyskappale ja siihen kuulUVa tulppa tai hana, jossa siteessä ei ole mainittuja haittapuolia ja joka on helppo kiinnittää ja vaihtaa ja jonka avulla pistokohdan alue pysyy helposti steriilinä.
Keksinnön mukainen tarkoitus saavutetaan siteellä, jolle on tunnusomaista se, että tarrakerroksen ja päästökerroksen väliin on järjestetty nestettä absorboiva imukerros, että sopivimmin imukerroksen ja päästökerroksen väliin on järjestetty ei-imevä nesteen läpäisemä kalvo, joka peittää imukerroksen, jolloin imukerros tai kyseessä olevassa tapauksessa ei-imevä kalvo, sidettä käytettäessä on tarkoitus asettaa suoni katetrin sisäänpistokohtaa vastaan, että imukerros on oleellisesti pienempi kuin puolet kantokalvosta ja kokonaisuudessaan sijoitettu kantokalvon toiselle puoliskolle, kun se jaetaan puoliksi, ja välimatkan päähän kantokalvon reunoista, että päästökerros on jaettu kahdeksi päästökalvoksi, jotka ulottuvat kantokalvon molemmista päädyistä ja kohtaavat imukerroksen kohdalla ja muodostavat kaksi tartuntaliuskaa, toinen taaksepäin käännettynä ja toinen sen päällä, ja että rako ulottuu siten siitä päästä, joka on kauempana imuker-roksesta välimatkan päähän imukerroksesta.
Side on edullisesti suorakulmainen. Imukerros pitää asettaa keskelle päästökalvon toista puoliskoa ja se kattaa esimerkiksi noin kolmasosan siteen leveydestä ja esimerkiksi noin viidesosan sen pituudesta.
Siteen pituus on sopivasti noin puolitoista kertaa sen leveys.
Jotta sidettä voitaisiin käyttää nk. suonieflonkatetrissa, johon kuuluu katetri letku, putkimainen yhdyskappale, jossa on kaksi sivulle suuntautunutta siipeä ja toisen siiven ulko-osassa tulpan pidin ja tulppa, järjestetään sopivasti esimerkiksi nelikulmainen repäisyperfo-rointi, jonka tarkoituksena on muodostaa vastaava aukko sulkukalvon, kiinnityskerroksen ja toisen päästökalvon läpi siteen toiseen sivu-reunaan sijoitettuna siten, että imemätön nestettä läpipäästävä kalvo sijaitsee katetrin pistokohdan päällä ja siteessä oleva rako on katetrin liitosputken kohdalla, katetrin tulppapidin asettuu repäisy-perforoinnin avulla saatuun aukkoon.
69756 Käytettäessä esillä olevaa sidettä poistetaan ensin se päästökalvo, jossa ei ole rakoa, mikä tapahtuu helposti käyttämällä päästökalvon tartuntaliuskaa. Tämän jälkeen side sijoitetaan siten, että nestettä läpäisevä imemätön kalvo ja imukappale tulevat pistokohdan päälle, jonka jälkeen toinen suojakalvo poistetaan samanaikaisesti kuin siteessä olevan raon muodostavat kaksi liuskaa kiinnitetään katetrin siipien päälle ja putkimaisen yhdyskappaleen sivuun. Suonieflon-katetriin muodostetaan aukko, jonka repäisyperforointi muodostaa siteen sivuun, repäisemällä perforointi irti ennen päästökalvojen poistoa. Tämän jälkeen side kiinnitetään kuvatulla tavalla ja siten, että aukko sijaitsee tulppapltimen ympärillä.
Keksinnön mukainen side päkataan sopivasti steriiliin pakkaukseen yhdessä pienen kompressisiteen kanssa, joka on kooltaan sellainen, että se asettuu sopivasti katetrin siipien alle ja estää katetria ärsyttämästä potilaan ihoa. Käytettäessä sidettä avataan ensin pakkaus, jolloin steriili kompressiside sijoitetaan katetrin siipien alle. Tämän jälkeen side kiinnitetään yllä kuvatulla tavalla.
Keksinnön mukainen side on hyvin helppo kiinnittää ja vaihtaa.
Katetri pysyy lujasti paikallaan johtuen siitä, että siteen kaksi liuskaa kiinnittävät katetrin siivet lujasti sekä katetrireiän vlä-että alapuolelle. Erityisen tärkeätä on se, että liuskat ovat riittävän pitkät. Sitä vastoin imukappaleen ympärillä olevan liimausosan ei tarvitse olla kovin suuri. Imukappale sijoitetaan tästä syystä siteen toiselle puolikkaalle, jolloin liuskat voivat olla jonkin verran pidemmät kuin puolet siteestä.
Nestettä läpipäästävässä imemättömässä kalvossa olevalla imukappa-leella pidetään haava puhtaana verestä ja mahdollisista katetrin läpi ruiskutetuista liuoksista, jotka voivat tunkeutua ylöspäin haavan läpi. Imemätön kalvo saa aikaan sekä haavan pysymisen kuivana että estää sidettä takertumasta kiinni haavaan.
Esillä olevan siteen avulla on erittäin helppoa pitää suonihatetrin ympäristö steriilinä. Tähän vaikuttavat useat tekijät. Siteenä esimerkiksi käytetään ainoastaan yhtä komponenttia, jolloin se toimitetaan sopivasti steriilissä pakkauksessa yhdessä kompressisiteen kanssa, joka sijoitetaan katetrin siipien alle. Side on helppo kiinnittää eikä sitä tarvitse tarpeettomasti sormeilla. Edelleen laastari- 69756 aineen tai kiinnityskerroksen kantomateriaali on muodostettu sulku-kerrokseksi, joka edullisesti päästää läpi ilmaa mutta ei nestettä. Tällöin bakteereja ei pääse nesteen mukana tunkeutumaan siteen läpi ja likaamaan pistokohtaa. Edelleen siteessä oleva rako ei ulotu imukappaleeseen asti. Siten bakteerit eivät myöskään pääse tunkeutumaan haavaan imukappaleen ja ympäröivän ilman tai kosteuden välisen kosketuksen vuoksi.
Siteessä olevat kalvokerrokset, eli sulkukalvo, nestettä läpäisevä imemätön kalvo ja suojaavat päästökalvot, on muodostettu sopivasta arkkimaisesta materiaalista, kuten esimerkiksi tiiviistä muovifo-lioista tai kalvoista, kuitumateriaalista tai paperista tai kankaasta.
Sulkukalvossa oleva materiaali on sopivasti kyllästettyä kutomatonta materiaalia, kuten kutomatonta raionia, ja absorptiokappale voidaan muodostaa esimerkiksi sopivan paksuisesta kehräämällä sidotusta selluloosaregeneraatista.
Imemätön nestettä läpipäästävä kalvo voi olla esimerkiksi perforoitu, mahdollisesti aluminoitu muovikalvo, kutomatonta keinokuitua tai vastaavaa. Kiinnityskerroksena voidaan käyttää akrylaattioohjaista kiinnemassaan. Päästökalvot voidaan valmistaa sopivasti silikonipääl-lysteisestä paperista.

Selvyyden vuoksi kuvioissa esitetään joidenkin kerrosten paksuus liioiteltuna.
Kuvio 1 esittää perspektiivikuvan keksinnön mukaisen siteen eräästä 6 69756 suoritusimiodosta. Siteeseen kuuluu suorakulmainen, edullisesti vesi-tiivis ja ilmaa läpäisevä sulkukalvo 1. Kalvo on päällystetty itse-kiinnittyvällä liimalla 2. Keskellä kalvon toista puolikasta sijaitsee neliömäinen imukappale 3, jonka päällä on yhtä suuri perforoitu, imemätön arkki 4. Koko sulkukalvoa ja sen päällä olevaa kerrosta peittää kaksi suojapaperia 5 ja 6. Nämä kohtaavat imukappaleen päällä ja niiden reunaosat muodostavat kaksi tartuntaliuskaa 7 ja 8. Toinen tartuntaliuska 7 on taitettu taaksepäin ja toinen liuska 8 sijaitsee taaksepäin taivutetun liuskan päällä. Siteen reunassa sijaitsee nelikulmainen repäisyperforointi 18.
Kuvattu side voi olla esimerkiksi noin 95 mm pitkä ja 60 mm leveä. Tällöin imukerros voi olla noin 20 x 20 mm ja sijoittua noin 15 mm päähän lähimmästä lyhyestä sivusta ja noin 20 mm päähän kummastakin pitkästä sivusta. Rako ulottuu sopivasti kauimpana imukappaleesta olevasta lyhyestä sivusta keskeisesti aina noin 5 mm päähän imukap-paleesta.
Kuviossa 2 on side esitetty kiinnitettynä suonieflonkatetrin päälle. Katetriin kuuluu letku 15, putkimainen yhdvskappale 17 ja kaksi sivulle suuntautuvaa siipeä 10. Toisen siiven 10 ulkoreunassa on tulppapidin 12 ja tulppa 13. Tulppaa 13 voidaan käyttää putken 17 sulkemiseksi. Putken 17 päällä on tulpalla 14 varustettu lisäaukko. Tulpan 14 alla Olevassa aukossa on kalvo, joka estää verta tunkeutumasta ylöspäin aukon läpi, mutta joka samanaikaisesti mahdollistaa esimerkiksi lääkkeen annon aukon läpi samalla kun putki 17 on auki ja esimerkiksi verta annetaan sen läpi. Siipien alla on kompressi-side 20. Katetrin letku 15 on esitetty pisteviivoin siinä kohdassa, josaa se ei ole paljaana, ja se työntyy potilaan ihon 16 läpi pisto-kohdassa 19. Pistokohdan 19 päällä on perforoitu kalvo 4 ja imukerros 3. Raon 9 muodostamat siteen kaksi liuskaa sijaitsevat siipien 10 päällä ja kumpikin omalla puolellaan putkea 17.
Kuviossa 3 side on esitetty kiinnitettynä keskikatetriin, johon kuuluu putkimainen vhdyskappale 17 ja hana 11, joka voi sulkea ja avata yhdyskappaleen. Tässä tapauksessa ei ole välttämätöntä paljastaa aukkoa 18 repäisemällä. Muuten side on kiinnitetty samalla tavoin tähän katetriin kuin suonieflonkatetriin. Siteen ja katetrin ne osat, jotka sijaitsevat sulkukalvon 1 alla, on esitetty kätkovii-voin.

Suonikatetriside, johon kuuluu kantokalvo (1) jonka toisella puolella on tarrakerros (2), jota peittää suojaava, irtirepäistävä päästökalvo (5, 6) jolloin kantokalvon (1), tarrakerroksen (2) ja päästökerroksen läpi on järjestetty rako (9) siteen toisesta reunasta sen keskiosaan, tunnettu siitä, että tarrakerroksen (2) ja päästökerroksen (5, 6) väliin on järjestetty nestettä absorboiva imukerros (3), että sopivimmin imukerroksen ja päästö-kerroksen väliin on järjestetty ei-imevä nesteen läpäisemä kalvo (4), joka peittää imukerroksen (3), jolloin imukerros tai kyseessä olevassa tapauksessa ei-imevä kalvo, sidettä käytettäessä on tarkoitus asettaa suonikatetrin sisäänpistokohtaa vastaan, että imukerros (3) on oleellisesti pienempi kuin puolet kantokalvosta ja kokonaisuudessaan sijoitettu kantokalvon (1) toiselle puoliskolle, kun se jaetaan puoliksi, ja välimatkan päähän kantokalvon reunoista, että päästökerros on jaettu kahdeksi päästökalvoksi (5, 6), jotka ulottuvat kantokalvon (1) molemmista päädyistä ja kohtaavat imukerroksen (3) kohdalla ja muodostavat kaksi tartunta-liuskaa, toinen (7) taaksepäin käännettynä ja toinen (8) sen päällä, ja että rako (9) ulottuu siteen siitä päästä, joka on kauempana imukerroksesta (3) välimatkan päähän imukerroksesta.
2. Patenttivaatimuksen 1 mukainen side, tunnettu siitä, että imukerros (3) on sijoitettu keskelle sulkukalvon toista puolikasta ja peittää noin kolmasosan siteen leveydestä ja noin viidesosan ja pituudesta.
3. Patenttivaatimuksen 1 tai 2 mukainen side, joka on tarkoitettu suonieflonkatetriin, jossa on liitosputki (17), kaksi sivusuun-taista siipeä (10) ja toisessa siivessä oleva tulppapidin (12) ja tulppa (13), tunnettu siitä, että vastaavan aukon paljastamiseen tarkoitettu sopivasti nelikulmainen repäisyperforointi (18) on järjestetty siteen toiseen sivureunaan sulkukalvon (1), kiinnityskerroksen (2) ja päästökalvon (5) läpi ja sijoitettu siten, että side on sijoitettu suonieflonkatetrin päälle siten, että imemätön nestettä läpäisevä kalvo (4) sijaitsee katetrin 8 69756 pistokohdan (19) päällä ja siteen rako (9) katetrin liitosputken (17) kohdalla, jolloin suonieflonkatetrin tulppapidin (12) sijaitsee repäisyperforoinnin avulla muodostetussa aukossa (18).
4. Jonkin patenttivaatimuksen 1-3 mukainen side, tunnettu siitä, että sen pituus on noin puolitoista kertaa suurempi kuin sen leveys.
