Abstract

Description
Aparato para tratar el glaucoma.
La presente invención se refiere en general a
procedimientos y dispositivos médicos mejorados para la reducción
de la presión elevada en órganos del cuerpo humano. Más
particularmente, la presente invención se refiere al tratamiento
del glaucoma mediante cirugía de derivación trabecular, que es un
medio para utilizar un implante o sedal, tal como una
microendoprótesis, derivación o similar, para sortear la malla
trabecular enferma al nivel de la malla trabecular y
utilizar/restablecer rutas de flujo de salida existentes.
Aproximadamente el dos por ciento de la
población en los Estados Unidos presentan glaucoma. El glaucoma es
un grupo de enfermedades oculares que produce cambios patológicos en
el disco óptico y pérdida del campo visual correspondiente dando
como resultado ceguera si no se trata. La elevación de la presión
intraocular es el principal factor etiológico en todos los
glaucomas.
En los glaucomas asociados con una elevación en
la presión en el ojo, la fuente de resistencia al flujo de salida
está en la malla trabecular. El tejido de la malla trabecular
permite que el "humor acuoso" entre en el conducto de Schlemm,
que luego se vacía hacia el interior de los canales colectores de
humor acuoso en la pared posterior del conducto de Schlemm y luego
hacia el interior de las venas acuosas. El humor acuoso o acuoso es
un líquido transparente que llena la región entre la córnea, en la
parte frontal del ojo, y el cristalino. El humor acuoso se secreta
constantemente por el cuerpo ciliar alrededor del cristalino, por lo
que hay un flujo continuo del humor acuoso desde el cuerpo ciliar
hasta la cámara frontal del ojo. La presión en el ojo está
determinada por un equilibrio entre la producción de humor acuoso y
su salida a través de la malla trabecular (vía principal) o
mediante el flujo de salida uveoescleral (vía secundaria). La malla
trabecular está ubicada entre el borde externo del iris y la
periferia interna de la córnea. La parte de la malla trabecular
adyacente al conducto de Schlemm produce la mayor parte de la
resistencia al flujo de salida de humor acuoso (malla
yuxtacanalicular).
El glaucoma se clasifica de manera aproximada en
dos categorías: glaucoma de ángulo cerrado y glaucoma de ángulo
abierto. El glaucoma de ángulo cerrado está producido por el cierre
del ángulo anterior mediante el contacto entre el iris y la
superficie interna de la malla trabecular. El cierre de este ángulo
anatómico impide el drenaje normal del humor acuoso desde la cámara
anterior del ojo. El glaucoma de ángulo abierto es cualquier
glaucoma en el que el ángulo de la cámara anterior permanece
abierto, pero disminuye la salida de humor acuoso a través de la
malla trabecular. Se desconoce la causa exacta para la filtración
disminuida para la mayoría de los casos de glaucoma de ángulo
abierto. Sin embargo, hay glaucomas de ángulo abierto secundarios
que pueden incluir edema o hinchazón de los espacios trabeculares
(debido a la utilización de esteroides), dispersión pigmentaria
anómala o enfermedades tales como hipertiroidismo que producen
congestión vascular.
Todos los tratamientos actuales para el glaucoma
se dirigen a la disminución de la presión intraocular. Esto se
realiza inicialmente mediante el tratamiento médico con gotas o
píldoras que reducen la producción de humor acuoso o aumentan el
flujo de salida del humor acuoso. Sin embargo, estos diversos
tratamientos farmacológicos para el glaucoma se asocian a veces con
efectos secundarios significativos, tales como cefalea, visión
borrosa, reacciones alérgicas, muerte debida a complicaciones
cardiopulmonares y posibles interacciones con otros fármacos.
Cuando fracasa el tratamiento farmacológico, se utiliza un
tratamiento quirúrgico. El tratamiento quirúrgico para el glaucoma
de ángulo abierto consiste en láser (trabeculoplastia),
trabeculectomía e implantes de derivación de humor acuoso tras
fracaso de trabeculectomía o si no es probable que tenga éxito la
trabeculectomía. La trabeculectomía es una cirugía mayor que se
utiliza lo más ampliamente y se amplía con fármacos contra el
cáncer aplicados por vía tópica tales como
5-fluorouracilo o mitomicina-c para
disminuir la formación de cicatrices y aumentar el éxito
quirúrgico.

Se realizan aproximadamente 100.000
trabeculectomías en pacientes con edad para ser beneficiarios del
seguro Medicare al año en los Estados Unidos. Este número
aumentaría si pudiera disminuirse la morbilidad asociada con la
trabeculectomía. La morbilidad actual asociada con la
trabeculectomía consiste en fracaso (10-15%),
infección (un riesgo durante toda la vida de aproximadamente el
2-5%), hemorragia coroidea (1%, una grave
hemorragia interna debida a presión demasiado baja que da como
resultado pérdida visual), formación de cataratas y maculopatía por
hipotonía (pérdida visual potencialmente reversible a partir de una
presión demasiado baja).
Si fuera posible sortear la resistencia local al
flujo de salida de humor acuoso en el punto de la resistencia y
utilizar mecanismos de flujo de salida existentes, disminuiría
enormemente la morbilidad quirúrgica. El motivo para esto es que
las venas acuosas epiesclerales presentan una contrapresión que
impediría que la presión ocular disminuyera demasiado. Esto
eliminaría prácticamente el riesgo de maculopatía por hipotonía y de
hemorragia coroidea. Además, la recuperación visual sería muy
rápida y el riesgo de infección sería muy pequeño (una reducción
desde el 2-5% hasta el 0,05%). Debido a estas
razones, los cirujanos han intentado durante décadas desarrollar
una cirugía factible para la malla trabecular.
Las técnicas anteriores, que se han intentado,
son la goniotomía/trabeculotomía y otra rotura mecánica de la malla
trabecular, tal como trabeculopunción, goniofotoablación, ablación
trabecular con láser y goniocuretaje. Éstas se describen brevemente
a continuación.
Goniotomía/trabeculotomía: La goniotomía
y la trabeculotomía son técnicas sencillas y dirigidas de disección
microquirúrgica con rotura mecánica de la malla trabecular. Éstas
presentaron inicialmente respuestas favorables tempranas en el
tratamiento del glaucoma de ángulo abierto. Sin embargo, la revisión
a largo plazo de los resultados quirúrgicos mostró sólo un éxito
limitado en adultos. En retrospectiva, estas intervenciones
quirúrgicas fracasaron probablemente debido a los mecanismos de
reparación y a un proceso de "relleno". El "relleno" es
el resultado de un proceso de curación que presenta el efecto
perjudicial de colapso y cierre de la abertura producida en toda la
malla trabecular. Una vez que se cierran las aberturas producidas,
se acumula de nuevo presión y fracasa la cirugía.
Trabeculopunción: También se han
investigado los láseres de neodimio (Nd)-YAG con
interrupción Q como técnica ópticamente invasiva para producir
orificios de espesor completo en la malla trabecular. Sin embargo,
el orificio relativamente pequeño producido mediante esta técnica de
trabeculopunción muestra un efecto de relleno y fracasa.
Goniofotoablación/ablación trabecular con
láser: La goniofotoablación se da a conocer por Berlin en la
patente US nº 4.846.172 y describe la utilización de un láser de
excímero para tratar el glaucoma mediante la ablación de la malla
trabecular. Se demostró que esto no tenía éxito mediante un ensayo
clínico. Hill et al. utilizaron un láser de erbio:YAG para
producir orificios de espesor completo a través de la malla
trabecular (Hill et al., Lasers in Surgery and Medicine
11:341-346, 1991). Se investigó esta técnica en un
modelo de primate y un ensayo clínico limitado con seres humanos en
la Universidad de California, Irvine. Aunque la morbilidad fue cero
en ambos ensayos, las tasas de éxito no justificaron otros ensayos
con seres humanos. El fracaso se debió de nuevo al relleno de los
defectos producidos en la malla trabecular mediante los mecanismos
de reparación. Ninguna de éstas es una técnica quirúrgica válida
para el tratamiento del glaucoma.
Goniocuretaje: Ésta es una técnica de
rotura mecánica ab interno (desde el interior). Utiliza un
instrumento similar a una espátula de ciclodiálisis con una
microcureta en la punta. Los resultados iniciales son similares a
los de la trabeculotomía que fracasó debido a los mecanismos de
reparación y a un proceso de relleno.
Aunque la trabeculectomía es la cirugía
filtrante realizada más comúnmente, la viscocanalostomía (VC) y la
trabeculectomía no penetrante (NPT) son dos nuevas variaciones de
cirugía filtrante. Éstas son intervenciones quirúrgicas oculares
mayores ab externo (desde el exterior), en las que se deja al
descubierto quirúrgicamente el conducto de Schlemm realizando un
colgajo escleral grande y muy profundo. En la intervención
quirúrgica de VC, se canula el conducto de Schlemm y se inyecta una
sustancia viscoelástica (que dilata el conducto de Schlemm y los
canales colectores de humor acuoso). En la intervención quirúrgica
de NPT, se extrae la pared interna del conducto de Schlemm después
de dejar al descubierto quirúrgicamente el conducto.
La trabeculectomía, VC y NPT se realizan bajo un
colgajo escleral y de la conjuntiva, de manera que el humor acuoso
se drena sobre la superficie del ojo o hacia los tejidos ubicados
dentro de la pared lateral del ojo. No se utilizan flujos de salida
fisiológicos normales. Estas operaciones quirúrgicas son
intervenciones quirúrgicas mayores con morbilidad ocular
significativa. Cuando se cree que la trabeculectomía, VC, y NPT
presentan una baja probabilidad de éxito, se han utilizado varios
dispositivos de drenaje implantables para garantizar que
continuarán la filtración y el flujo de salida de humor acuoso
deseados a través de la abertura quirúrgica. El riesgo de
colocación de un implante de drenaje para el glaucoma también
incluye hemorragia, infección y visión doble posoperatoria que es
una complicación única de los implantes de drenaje.

El documento W09830181 da a conocer un implante
de filtración para glaucoma que comprende un cuerpo tubular con un
diámetro externo alargado que forma una curva continua.
Todas las formas de realización anteriores y
variaciones de las mismas adolecen de numerosos inconvenientes y
niveles de éxito moderados. Suponen un traumatismo sustancial para
el ojo y requieren gran experiencia quirúrgica creando un orificio
a través del espesor completo de la esclerótica/córnea hacia el
espacio subconjuntival. Además, no se utilizan las rutas del flujo
de salida fisiológico normal. Las intervenciones quirúrgicas se
realizan en su mayor parte en un quirófano generando una tarifa por
la instalación, una tarifa para el profesional anestesiólogo y
presentan un tiempo de recuperación prolongado para la visión. Las
complicaciones de la cirugía de filtración han impulsado a los
cirujanos oftálmicos a considerar otros enfoques para disminuir la
presión intraocular.
La malla trabecular y el tejido yuxtacanalicular
proporcionan en conjunto la mayor parte de la resistencia al flujo
de salida de humor acuoso y, como tal, son dianas lógicas para la
extirpación quirúrgica en el tratamiento del glaucoma de ángulo
abierto. Además, se alteran cantidades mínimas de tejido y se
utilizan rutas de flujo de salida fisiológico existentes. La
cirugía de derivación trabecular presenta la posibilidad de riesgos
mucho menores de hemorragia coroidea, infección y utilizaciones de
mecanismos de flujo de salida fisiológico existentes. Esta cirugía
podría realizarse bajo anestesia tópica en la consulta de un médico
con una rápida recuperación visual.
Por tanto, existe una gran necesidad clínica
para el tratamiento del glaucoma mediante un procedimiento que sea
más rápido, más seguro y más económico que las modalidades
disponibles actualmente. La cirugía de derivación trabecular es una
cirugía innovadora que utiliza una microendoprótesis, derivación, u
otro implante para sortear la malla trabecular enferma sólo al
nivel de la malla trabecular y utilizar o restablecer rutas de flujo
de salida existentes. El objetivo de la presente invención es
proporcionar unos medios y procedimientos para tratar la presión
intraocular elevada de una manera que es sencilla, eficaz específica
del sitio de la enfermedad y puede realizarse de manera
ambulatoria.
Los aspectos de la invención se definen en las
reivindicaciones adjuntas.
Según una forma de realización de la invención,
puede proporcionarse un aparato que comprende un sedal completamente
implantable tal como se define en la reivindicación 1. El sedal
puede estar recubierto con un fármaco. En algunas formas de
realización, la parte de salida presenta una luz con una sección
transversal ovalada que presenta un eje largo.
La parte de salida en ciertas formas de
realización presenta un eje longitudinal, de manera que el eje largo
de la sección transversal ovalada y el eje longitudinal de la parte
de salida definen un plano, presentando la parte de entrada un eje
longitudinal que se encuentra fuera del plano en un ángulo \theta
(theta) con el mismo.
En algunas disposiciones preferidas, el sedal
comprende una parte de entrada, configurada para extenderse a
través de una parte de la malla trabecular; una parte de salida,
configurada para extenderse hacia el interior del conducto de
Schlemm; y por lo menos un resalte sobre la parte de salida,
configurado para ejercer tracción contra una superficie interior
del conducto de Schlemm. Este resalte puede comprender por lo menos
un reborde o nervadura.
Algunas formas de realización preferidas
comprenden una parte de entrada configurada para extenderse a través
de una parte de la malla trabecular, una parte de salida
configurada para extenderse hacia el interior del conducto de
Schlemm y una válvula de una vía dentro de la parte de entrada y/o
salida.
Se da a conocer un procedimiento para
suministrar un sedal dentro de un ojo, que comprende proporcionar un
elemento de guiado alargado, hacer avanzar un extremo distal del
elemento de guiado a través de por lo menos una parte de la malla
trabecular del ojo, hacer avanzar el sedal a lo largo del elemento
de guiado hacia el extremo distal y colocar el sedal para conducir
el humor acuoso entre la cámara anterior del ojo y conducto de
Schlemm.
En ciertas formas de realización, hacer avanzar
el elemento de guiado comprende hacerlo avanzar desde la cámara
anterior hacia la malla trabecular. En formas de realización
adicionales, la colocación comprende colocar un extremo del sedal
dentro del conducto de Schlemm adyacente a un canal de recogida de
humor acuoso.
Ciertas formas de realización preferidas
incluyen un aparato para suministrar un sedal a la cámara anterior
de un ojo que comprende un tubo alargado que presenta una luz, una
superficie externa y un extremo distal; un elemento de guiado
alargado, desmontable, dentro de la luz, configurado para permitir
hacer avanzar y colocar el sedal en la malla trabecular del ojo.
Este aparato puede comprender además un elemento cortante colocado
en el extremo distal del tubo. El elemento cortante puede
seleccionarse de entre el grupo constituido por una cuchilla, una
sonda láser, un elemento de guiado puntiagudo, un extremo distal
afilado de dicho tubo y un instrumento de corte ultrasónico. El
aparato puede comprender además una abertura en la superficie
externa del tubo, configurada para permitir la infusión de fluido
hacia el interior del ojo.
En formas de realización preferidas adicionales,
un aparato para suministrar un sedal en un ojo, comprende un
elemento alargado adaptado para su inserción en el interior de una
cámara anterior del ojo, presentando el elemento alargado una parte
de extremo distal configurada para retener el sedal en la misma,
comprendiendo la parte de extremo distal un elemento cortante
configurado para formar una abertura en la malla trabecular del ojo
para la recepción del sedal, de manera que un extremo del sedal está
en el conducto de Schlemm. El elemento alargado puede comprender
además una luz que conduce fluido hacia dicha parte de extremo
distal.
La forma de realización preferida proporciona
tratamiento quirúrgico adicional del glaucoma (cirugía de derivación
trabecular) al nivel de la malla trabecular y restablece rutas de
flujo de salida fisiológico existentes. Un implante sortea la malla
trabecular enferma al nivel de la malla trabecular y restablece las
rutas de flujo de salida fisiológico existentes. El implante
presenta un extremo de entrada, un extremo de salida y una luz
entre ellos. La entrada está colocada en la cámara anterior al nivel
de la malla trabecular interna y el extremo de salida está colocado
aproximadamente en la superficie exterior de la malla trabecular
enferma y/o hacia el interior de los canales de recogida de fluido
de las rutas de flujo de salida existentes.
Según un procedimiento preferido, la cirugía de
derivación trabecular produce una abertura o un orificio a través
de la malla trabecular enferma con microcirugía menor. Para impedir
el "llenado" del orificio, se sitúa un implante alargado
biocompatible dentro del orificio tal como un sedal, que puede
incluir, por ejemplo, una varilla sólida o un tubo hueco. En una
forma de realización a título de ejemplo, el implante de sedal
puede colocarse sólo a través de la malla trabecular enferma y no se
extiende hacia el interior de la esclerótica o pared del ojo. En
otra forma de realización, el extremo de entrada del implante está
expuesto a la cámara anterior del ojo mientras que el extremo de
salida está colocado en la superficie exterior de la malla
trabecular. En otra forma de realización a modo de ejemplo, el
extremo de salida está colocado en y sobre la superficie exterior
de la malla trabecular y hacia el interior de los canales de
recogida de fluido de las rutas de flujo de salida existentes.
Todavía en otra forma de realización, el extremo de salida está
colocado en el conducto de Schlemm. En una realización alternativa,
el extremo de salida entra hacia el interior de los canales de
recogida de fluido hasta el nivel de las venas acuosas con el sedal
insertado de una forma retrógrada o anterógrada.
Según la forma de realización preferida, el
implante de sedal está fabricado de material biocompatible, que o
bien es hueco para permitir el flujo del humor acuoso o bien es
material biocompatible sólido que se empapa del humor acuoso. El
material para el sedal puede seleccionarse de entre el grupo
constituido por material poroso, material semirrígido, material
blando, material hidrófilo, material hidrófobo, hidrogel, material
elástico y similares.
Según la forma de realización preferida, el
implante de sedal puede ser rígido o puede estar fabricado de
material relativamente blando y algo curvado en su sección distal
para ajustarse en el interior de las rutas de flujo de salida
fisiológico existentes, tal como el conducto de Schlemm. La sección
distal dentro de las rutas de flujo de salida puede presentar una
forma ovalada para estabilizar el sedal en su sitio sin sutura
excesiva. La estabilización o retención del sedal puede reforzarse
adicionalmente mediante un extremo de sección decreciente y/o
mediante por lo menos una nervadura o nervio sobre la superficie
exterior de la sección distal del sedal, u otras alteraciones de
superficie diseñadas para retener el sedal.
En una forma de realización, el sedal puede
incluir una microbomba, válvula de una vía o membrana semipermeable
si el reflujo de los glóbulos rojos o proteínas séricas se convierte
en un problema clínico. También puede ser útil utilizar un material
biocompatible que se hidrate y se expanda tras la implantación de
modo que el sedal se bloquee en su sitio alrededor de la abertura
de la malla trabecular o alrededor de la sección distal del
sedal.
Una de las ventajas de la cirugía de derivación
trabecular, tal como se da a conocer en la presente memoria, y de
la utilización de un implante de sedal para sortear la malla
trabecular enferma al nivel de la malla trabecular y utilizar de
ese modo rutas de flujo de salida existentes es que el tratamiento
del glaucoma es sustancialmente más sencillo que en los
tratamientos existentes. Una ventaja adicional de la invención es
la utilización de microcirugía sencilla que puede realizarse de
manera ambulatoria con rápida recuperación visual y morbilidad
enormemente disminuida. Finalmente, se utiliza un enfoque claramente
diferente al que se encuentra en los implantes existentes. El
implante de la presente invención utiliza o reestablece los
mecanismos de flujo de salida fisiológico, en contraposición con las
metodologías dadas a conocer anteriormente.
Los objetivos y las características adicionales
de la presente invención se pondrán de manifiesto y la propia
invención se entenderá mejor a partir de la siguiente descripción
detallada de las formas de realización a título de ejemplo, cuando
se lee haciendo referencia a los dibujos adjuntos.
La figura 1 es una vista en sección de un ojo
para fines de ilustración.

plano, que muestra el diagrama anatómico de la malla trabecular y
la cámara anterior del ojo.

implante de sedal construido según los principios de la
invención.

La figura 7 es un procedimiento a título de
ejemplo alternativo para situar un implante de sedal en el sitio
del implante.
Haciendo referencia a las figuras 1 a 7, lo que
se muestra es un procedimiento para el tratamiento del glaucoma
mediante cirugía de derivación trabecular. En particular, se utiliza
un implante de sedal para sortear la malla trabecular enferma al
nivel de la malla trabecular para utilizar o restablecer rutas de
flujo de salida existentes y procedimientos de los mismos.
A título ilustrativo de los antecedentes, la
figura 1 muestra una vista en sección de un ojo 10, mientras que la
figura 2 muestra una vista en primer plano, que muestra las
ubicaciones anatómicas relativas de la malla trabecular, la cámara
anterior y el conducto de Schlemm. El tejido colagenoso grueso
conocido como esclerótica 11 cubre la totalidad del ojo 10, excepto
la parte cubierta por la córnea 12. La córnea 12 es un tejido
transparente delgado que enfoca y transmite luz hacia el interior
del ojo y de la pupila 14 que es el orificio circular en el centro
del iris 13 (parte coloreada del ojo). La córnea 12 se fusiona con
la esclerótica 11 en una unión denominada el limbo 15. El cuerpo 16
ciliar comienza internamente en el ojo y se extiende a lo largo del
interior de la esclerótica 11 y llega a la coroides 17. La coroides
17 es una capa vascular del ojo subyacente a la retina 18. El
nervio 19 óptico transmite información visual al cerebro y se
destruye secuencialmente por el glaucoma.
La cámara 20 anterior del ojo 10, que se une de
manera anterior mediante la córnea 12 y de manera posterior
mediante el iris 13 y el cristalino 26, está llena con humor acuoso.
El humor acuoso lo produce principalmente el cuerpo 16 ciliar y
alcanza el ángulo 25 de la cámara anterior formado entre el iris 13
y la córnea 12 a través de la pupila 14. En un ojo normal, el humor
acuoso se elimina a través de la malla 21 trabecular. El humor
acuoso pasa a través de la malla 21 trabecular hacia el interior del
conducto 22 de Schlemm y a través de las venas 23 acuosas, que se
fusionan con venas transportadoras de sangre y hacia la circulación
venosa. La presión intraocular del ojo 10 se mantiene mediante el
complicado equilibrio de secreción y flujo de salida del humor
acuoso de la manera descrita anteriormente. El glaucoma se
caracteriza por la acumulación excesiva de fluido acuoso en la
cámara 20 anterior, lo que produce un aumento en la presión
intraocular (los fluidos son relativamente incompresibles y la
presión se dirige por igual a todas las zonas del ojo).
Tal como se muestra en la figura 2, la malla 21
trabecular constituye una pequeña parte de la esclerótica 11.
Resulta comprensible que producir un orificio o abertura para
implantar un dispositivo a través de los tejidos de la conjuntiva
24 y de la esclerótica 11 es relativamente una cirugía mayor en
comparación con una cirugía para implantar un dispositivo sólo a
través de la malla 21 trabecular. Un implante de sedal 31 de la
presente invención o bien para utilizar o bien para restablecer
rutas de flujo de salida existentes colocadas a través de la malla
21 trabecular se ilustra en la figura 5.
En la misma, se da a conocer un procedimiento
para aumentar el flujo de salida de humor acuoso en un ojo de un
paciente para reducir la presión intraocular. El procedimiento
comprende sortear la malla trabecular enferma al nivel de la malla
trabecular y restablecer de ese modo rutas de flujo de salida
existentes. Alternativamente, se da a conocer un procedimiento para
aumentar el flujo de salida de humor acuoso en un ojo de un paciente
para reducir una presión intraocular en el mismo. El procedimiento
comprende sortear la malla trabecular enferma al nivel de dicha
malla trabecular con un implante de sedal y utilizando rutas de
flujo de salida existentes. El implante de sedal 31 puede ser un
sedal alargado o de otra forma, tamaño o configuración apropiados.
En una realización de un implante de sedal alargado, el sedal
presenta un extremo de entrada, un extremo de salida y una luz
entre ellos, en el que el extremo de entrada está colocado en una
cámara anterior del ojo y el extremo de salida está colocado
aproximadamente en una superficie exterior de dicha malla trabecular
enferma. Además, el extremo de salida puede estar colocado dentro
de canales de recogida de fluido de las rutas de flujo de salida
existentes. Opcionalmente, las rutas de flujo de salida existentes
pueden comprender el conducto 22 de Schlemm. El extremo de salida
puede estar colocado adicionalmente dentro de canales de recogida de
fluido hasta el nivel de las venas acuosas con el sedal insertado o
bien de una forma retrógrada o bien anterógrada con respecto a las
rutas de flujo de salida existentes.
También se da a conocer un procedimiento para
aumentar el flujo de salida de humor acuoso en un ojo de un
paciente para reducir una presión intraocular en el mismo. El
procedimiento comprende (a) producir una abertura en la malla
trabecular, en el que la malla trabecular comprende un lado interior
y un lado exterior; (b) insertar un implante de sedal en el
interior de la abertura; y (c) transportar el humor acuoso mediante
dicho implante de sedal para sortear la malla trabecular al nivel
de dicha malla trabecular desde el lado interior hasta el lado
exterior de la malla trabecular.
La figura 3 muestra una forma de realización del
implante de sedal 31 construido según los principios de la
invención. El implante de sedal puede comprender un material
biocompatible, tal como una silicona de calidad médica, por
ejemplo, el material vendido con la marca comercial Silastic^{TM},
que está disponible de Dow Corning Corporation de Midland,
Michigan, o poliuretano, que se vende con la marca comercial
Pellethane^{TM}, que también está disponible de Dow Corning
Corporation. En una forma de realización alternativa, pueden
utilizarse otros materiales biocompatibles (biomateriales), tales
como poli(alcohol vinílico), polivinilpirrolidona, colágeno,
colágeno heparinizado, tetrafluoroetileno, polímero fluorado,
elastómero fluorado, sílice fundida flexible, poliolefina,
poliéster, polisilicio, mezcla de materiales biocompatibles, y
similares. En una forma de realización alternativa adicional, puede
utilizarse un material biocompatible compuesto mediante el
recubrimiento en superficie del biomaterial mencionado
anteriormente, en el que el material de recubrimiento puede
seleccionarse de entre el grupo constituido por
politetrafluoroetileno (PTFE), poliimida, hidrogel, heparina,
fármacos terapéuticos y similares.
El fin principal del implante de sedal es ayudar
a facilitar el flujo de salida del humor acuoso en una dirección 40
hacia el exterior dentro del conducto de Schlemm y posteriormente
hacia el interior de colectores acuosos y de las venas acuosas de
modo que se equilibra la presión intraocular. En una forma de
realización, el implante de sedal 31 comprende un elemento tubular
alargado que presenta una sección distal 32 y una sección de entrada
44. Una sección distal 32 rígida o flexible está colocada dentro de
una de las rutas de flujo de salida existentes. La sección distal o
bien puede presentar un extremo de salida de sección decreciente 33
o bien puede presentar por lo menos una nervadura 37 u otro
dispositivo de retención que sobresale radialmente hacia el exterior
para estabilizar el implante de sedal dentro de dichas rutas de
flujo de salida existentes tras la implantación. Para fines de
estabilización, la superficie externa de la sección distal 32 puede
comprender una superficie estriada, una superficie con nervaduras,
una superficie con pilares, una superficie texturada, o similares.
La superficie externa 36, incluyendo la región externa 35 y la
región interna 34 en el extremo de salida 33, del implante de sedal
es biocompatible y compatible con el tejido, de modo que se minimiza
la interacción/irritación entre la superficie externa y el tejido
circundante. El implante de sedal puede comprender por lo menos una
abertura en una ubicación proximal a la sección distal 32, alejada
del extremo de salida 33, para permitir el flujo de humor acuoso en
más de una dirección. Dicha por lo menos una abertura puede estar
ubicada en la sección distal 32 aproximadamente en el lado opuesto
del extremo de salida 33.
En otra forma de realización ejemplificativa, el
implante de sedal 31 puede presentar medios 39 de control de flujo
unidireccional para permitir el flujo 40 de humor acuoso
unidireccional. Los medios 39 de control de flujo unidireccional
pueden seleccionarse de entre el grupo constituido por una válvula
de contención, una válvula de rendija, una microbomba, una membrana
semipermeable, o similar. Para potenciar la eficacia del flujo de
salida, se proporciona por lo menos una abertura 41 opcional en la
parte proximal de la sección distal 32, en una ubicación
ale-
jada del extremo de salida 33, y en una realización a modo de ejemplo en el extremo opuesto del extremo de salida 33.

La figura 4 muestra una vista en sección
transversal desde arriba de la figura 3. La forma de la abertura
del extremo de salida 33 y el cuerpo restante de la sección distal
32 puede ser ovalada, redonda o de alguna otra forma adaptada para
ajustarse a la forma de las rutas de flujo de salida existentes.
Esta configuración se corresponderá con el contorno del conducto de
Schlemm para estabilizar la sección de entrada con respecto al iris
y la córnea evitando la rotación.
Tal como se muestra en la figura 3, el implante
de sedal de la presente invención puede presentar una longitud de
entre aproximadamente 0,5 mm hasta más de un metro, dependiendo de
la cavidad del cuerpo al que se aplica el implante de sedal. El
diámetro exterior del implante de sedal puede oscilar desde
aproximadamente 30 \mum hasta aproximadamente 500 \mum. El
diámetro de la luz está preferentemente en el intervalo de entre
aproximadamente
20 \mum hasta aproximadamente 150 \mum. El implante de sedal puede presentar una pluralidad de luces para facilitar el transporte de flujo múltiple. La sección distal está curvada en un ángulo de aproximadamente 70 - 110 grados, con respecto a la sección de entrada 44.

La figura 5 muestra otra realización del
implante de sedal 45 construido según los principios de la
invención. En una forma de realización ejemplificativa, el implante
de sedal 45 puede comprender por lo menos dos secciones: una
sección de entrada 47 y una sección de salida 46. La sección de
salida presenta una abertura de salida 48 que está en el extremo de
salida del implante de sedal 45. La forma de la abertura de salida
48 es preferentemente una forma ovalada para corresponderse con el
contorno de las rutas de flujo de salida existentes. Una parte de
la sección de entrada 47 adyacente a la región de unión para la
sección de salida 46 estará colocada esencialmente a través de la
malla trabecular enferma mientras que el resto de la sección de
entrada 47 y de la sección de salida 46 están fuera de la malla
trabecular. Tal como se muestra en la figura 5, el eje largo de la
abertura de forma ovalada 48 se encuentra en un primer plano
formado por un eje X y por un eje Y. Para corresponderse mejor con
el contorno anatómico de la cámara 20 anterior, a la malla 21
trabecular y a las rutas de flujo de salida existentes, la sección
de entrada 47 puede encontrarse preferentemente en un segundo plano
elevado, en un ángulo \theta, con respecto al primer plano
formado por una sección de entrada imaginaria 47A y la sección de
salida 46. El ángulo 9 puede ser de entre aproximadamente 30 grados
y aproximadamente 150 grados.
La figura 6 muestra una vista en perspectiva que
ilustra el implante de sedal 31, 45 de la presente invención
colocado dentro del tejido de un ojo 10. Un orificio/abertura se
produce a través de la malla 21 trabecular enferma. La sección
distal 32 del implante de sedal 31 está insertada dentro del
orificio, en el que el extremo de salida 38 está expuesto a la
cámara 20 anterior mientras que el extremo de salida 33 está
colocado aproximadamente en una superficie exterior 43 de dicha
malla 21 trabecular enferma. En una realización adicional, el
extremo de salida 33 puede entrar adicionalmente en canales de
recogida de fluido de las rutas de flujo de salida existentes.
En una forma de realización, los medios para
formar un orificio/abertura en la malla 21 trabecular pueden
comprender una incisión con una microcuchilla, una incisión mediante
un hilo guía acabado en punta, un aplicador afilado, un aplicador
con forma de tornillo, un aplicador de irrigación o un aplicador con
lengüetas. Alternativamente, la malla trabecular puede disecarse
con un instrumento similar a un gancho o microcureta para retina. La
abertura puede producirse alternativamente mediante la ablación con
láser de fibra óptica de manera retrógrada.
La figura 7 muestra un procedimiento ilustrativo
para colocar un implante de sedal en el sitio del implante. Un
aplicador 51 o cuchilla con irrigación comprende una parte de
jeringuilla 54 y una parte de cánula 55. La sección distal de la
parte de cánula 55 presenta por lo menos un orificio 53 de
irrigación y un espacio 56 distal para contener un implante de
sedal 31. El extremo proximal 57 de la luz del espacio 56 distal
está sellado desde la luz restante de la parte de cánula 55.
Para colocar el sedal 31 en el orificio o
abertura a través de la malla trabecular, puede hacerse avanzar el
sedal sobre el hilo guía o una fibra óptica (de manera retrógrada).
En otra forma de realización, el sedal se coloca directamente sobre
el aplicador de suministro y se hace avanzar hasta el sitio del
implante, en el que el aplicador de suministro contiene el sedal de
manera segura durante la fase de suministro y lo libera durante la
fase de despliegue.
En una forma de realización ejemplificativa de
la cirugía de malla trabecular, se coloca al paciente en decúbito
supino, preparado, cubierto con un paño quirúrgico y anestesiado. En
una forma de realización, se practica una pequeña incisión
(inferior a aproximadamente 1 mm) autosellante. A través de la
córnea opuesta al sitio de colocación del sedal, se practica una
incisión en la malla trabecular con una cuchilla con irrigación.
Entonces se hace avanzar el sedal 31 por la incisión 52 de la córnea
a través de la cámara 20 anterior contenida en un aplicador 51 con
irrigación bajo guiado gonioscópico (lente) o endoscópico. Se retira
el aplicador y se concluye la cirugía. La cuchilla con irrigación
puede estar dentro de un intervalo de tamaños de calibre de 20 a
40, preferentemente de aproximadamente calibre 30.
A partir de la descripción anterior, debe
apreciarse que se ha dado a conocer un enfoque nuevo para el
tratamiento quirúrgico del glaucoma para liberar la presión
intraocular excesiva. Aunque la invención se ha descrito haciendo
referencia a una forma de realización específica, la descripción es
ilustrativa de la invención y no debe interpretarse como limitativa
de la misma. A los expertos en la materia se les pueden ocurrir
diversas modificaciones y aplicaciones, sin apartarse, por ello,
del alcance de la invención, tal como se describe mediante las
reivindicaciones adjuntas.
Claims (21)
1. Aparato que comprende un sedal completamente
implantable (31, 45), comprendiendo el sedal:
- una parte de entrada (44, 47) configurada para extenderse desde la cámara (20) anterior de un ojo (10) a través de una parte de la malla (21) trabecular del ojo;
- una parte de salida (32, 46) configurada para colocarse dentro del conducto (22) de Schlemm de dicho ojo; y
- una luz que se extiende a través del sedal, permitiendo la luz la comunicación fluídica desde una sección de entrada de la luz, situada dentro de la parte de entrada del sedal, hasta una sección de salida de la luz, situada dentro de la parte de salida del sedal,
- en el que la parte de entrada está dispuesta en un ángulo comprendido entre 70º y 110º con relación a la parte de salida, y en el que la parte de salida presenta por lo menos un resalte (37) hacia el exterior radial configurado para retener la parte de salida en el conducto (22) de Schlemm.
2. Aparato según la reivindicación 1, en el que
dicha sección de salida de la luz presenta una sección transversal
ovalada que presenta un eje largo.
3. Aparato según la reivindicación 2, en el que
la parte de salida (32, 46) presenta un eje longitudinal, en el que
dicho eje largo y dicho eje longitudinal de dicha parte de salida
definen un plano, y en el que dicha parte de entrada presenta un
eje longitudinal que se encuentra fuera de dicho plano en un ángulo
\theta con el mismo.
4. Aparato según la reivindicación 3, en el que
el ángulo \theta está comprendido entre 30º y 150º.
5. Aparato según cualquiera de las
reivindicaciones anteriores, en el que dicho por lo menos un resalte
(37) está configurado asimismo para ejercer tracción contra una
superficie interior del conducto de Schlemm.
6. Aparato según la reivindicación 5, en el que
dicho resalte (37) incluye por lo menos un reborde.
7. Aparato según la reivindicación 5 ó 6, en el
que dicho resalte (37) incluye por lo menos una nervadura.
8. Aparato según cualquiera de las
reivindicaciones anteriores, que comprende asimismo una válvula (39)
de una vía en por lo menos una de las partes de entrada y
salida.
9. Aparato según cualquiera de las
reivindicaciones anteriores, que comprende asimismo un elemento (55)
alargado en el que está dispuesto el sedal.
10. Aparato según la reivindicación 9, que
comprende asimismo un elemento cortante colocado en un extremo
distal de dicho elemento (55) alargado.
11. Aparato según la reivindicación 10, en el
que dicho elemento cortante se selecciona de entre el grupo
constituido por una cuchilla, una sonda láser, un elemento de
guiado puntiagudo, un extremo distal afilado de dicho tubo y un
instrumento de corte ultrasónico.
12. Aparato según la reivindicación 9, 10 u 11,
en el que dicho elemento (55) alargado presenta una abertura
configurada para permitir la infusión de fluido hacia el interior de
dicho ojo.
13. Aparato según cualquiera de las
reivindicaciones 9 a 12, en el que el elemento alargado es un tubo y
comprende asimismo un elemento de guiado alargado dentro de una luz
de dicho elemento (55) alargado, configurado para permitir que
dicho sedal (31, 45) se haga avanzar a través de dicho elemento
