(発明の属する技術分野)
本発明は、概ね、人体の器官内において上昇した圧力を低減させるために改良した医療装置及び方法に関する。さらに詳細には、本発明は、繊維柱帯バイパス形成外科術による緑内障治療に関し、該繊維柱帯バイパス形成外科術は、マイクロステント、シャントなどのようなインプラントまたはセトンを用いて、繊維柱帯レベルで病んだ繊維柱帯にバイパスを形成し現存の流出経路を使用及び修復する手段である。

(発明の背景)
米国国民の約2パーセントが、緑内障である。緑内障は、視神経円板を病理学的に変化させ、それに伴い、治療を怠れば失明してしまう視野の欠損を生じる眼病の一群である。眼内圧の上昇は、全ての緑内障の主な病因学的要素である。

眼圧の上昇を伴う緑内障において、流出抵抗の原因は、繊維柱帯にある。繊維柱帯の組織によって、「水様液」が、シュレム管に入り、それから、シュレム管の後壁内において水様液コレクタチャネル内へ次に房水静脈内へ注がれる。水様液または房水は、眼球の前方の角膜と水晶体との間の領域を満たす透明な液体である。房水は、水晶体の周りにおいて毛様体により分泌されて、房水は、毛様体から眼球の前房へ連続して流れる。眼内圧は、水様液の産生と、繊維柱帯からの流出(主な経路)またはブドウ膜強膜を介した流出(二次的な経路)との間のバランスにより決定される。繊維柱帯は、虹彩の外側縁と角膜の内側周縁との間に配置されている。シュレム管に隣接した繊維柱帯の部分が、水様液流出に対する抵抗の殆どを引き起こす(近傍小管網(juxtacanilicular meshwork))。

緑内障は、閉塞隅角緑内障及び開放隅角緑内障の2つの種類に大きく分類される。閉塞隅角緑内障は、虹彩と繊維柱帯内面とが接触して隅角が閉鎖されることにより引き起こされる。この解剖学的角が閉鎖されることによって、眼球の前房からの房水の通常の排出が妨げられる。開放隅角緑内障は、前房の隅角は開放したままだが繊維柱帯からの水様液の流出が減少する種々の緑内障である。フィルトレーション作用が減少する厳密な原因は、開放隅角緑内障の殆どの症例について分かっていない。しかしながら、(ステロイド使用による)繊維柱帯間隙の浮腫または腫れ、異常な色素分散、または血管の鬱血を生じる甲状腺機能亢進症のような病気を含み得る第2の開放隅角緑内障が存在する。

緑内障の現在の治療は、全て、眼内圧を低減させることに向けられている。これは、房水の産生を低減するかまたは水様液の流出を増加させる点眼薬または錠剤による薬物療法から始める。しかしながら、これらの緑内障への種々の薬物療法には、頭痛、目のかすみ、アレルギー反応のような重い副作用、心肺合併症及び起こり得る他の薬物との相互作用による死亡を伴う場合がある。薬物療法がうまくいかない場合には、外科治療が利用される。開放隅角緑内障に対する外科治療は、レーザー(繊維柱帯形成術)と、繊維柱帯切開術と、繊維柱帯切開術が成功しなかった後にはまたは繊維柱帯切開術が成功する見込みがない場合には水様液バイパスインプラントとからなる。繊維柱帯切開術は、もっとも広く利用されるとともに、5−フルロウラシルまたはマイトマイシン−Cのような抗ガン剤を局所的に適用して傷が低減しかつ外科術の成功が多くなったことにより増加している主な外科術である。

米国において一年に約100,000の繊維柱帯切開術が、老人医療保険世代の患者に行われている。繊維柱帯切開術により病的状態発生率を低減可能となれば、この数は増加するだろう。繊維柱帯切開術に伴う現在の病的状態発生率は、不成功(10−15%)、感染(終生続くリスク約2−5%)、脈絡膜出血(1%、ひどい内出血により、圧力が低下し過ぎて視力低下に至る)、白内障形成、及び低眼圧黄斑症(圧力が低下し過ぎて可逆性の視力低下が起こり得る)からなる。

抵抗部位において水様液を流出させ現存の流出メカニズムを利用するように、局所的抵抗にバイパス形成することができれば、外科手術で病的状態発生率は著しく低減することになる。これの理由は、強膜の房水静脈が、眼内圧の低減超過を妨げることになる背圧を有することである。これが可能であれば、低眼圧黄斑症及び脈絡膜出血の危険が実質的に排除されるだろう。さらには、視力回復が、非常に急速となり、感染の危険性が、非常に低くなるだろう(2−5%から0.05%に低くくなる)。これらの理由によって、外科医は、何十年間、繊維柱帯に対して効果的な外科術を開発しようとしてきた。

これまでに試されてきた技術は、隅角切開術及び線維柱帯切開術、及び繊維柱帯穿刺、隅角光切除、レーザー繊維柱帯切除及び隅角掻爬術のような、繊維柱帯の他の機械破壊である。それらを、以下に簡単に説明する。

隅角切開術及び線維柱帯切開術 隅角切開術及び線維柱帯切開術は、繊維柱帯の機械破壊を伴い、顕微手術による切開という簡易な管理を受ける技術である。これらは、最初は、開放隅角緑内障の治療において初期には好適な反応であった。しかしながら、外科手術の結果を長期にわたり見直すと、成人における成功がごく限られたものであることが示された。過去へ遡ると、これらの処置では、おそらくは、メカニズム及び、「閉塞(filling in)」というプロセスが二次的にうまく修復されなかった。閉塞は、繊維柱帯中に生成された開口部が崩壊し閉鎖されるという有害な作用を有する治癒過程の結果である。一度、生成された開口部が閉じると、圧力が高まり、外科手術は失敗する。

繊維柱帯穿刺 Qスイッチネオジウム(Nd):YAGレーザーもまた、繊維柱帯内に十分な厚さの孔を生成するための光学式侵襲性技術として研究されてきた。しかしながら、この繊維柱帯穿刺技術により生成される比較的小さい孔は、実質的に閉塞し失敗に終わる。

隅角光切除及びレーザー繊維柱帯切除 隅角光切除は、Berlinによる米国特許第4,846,172号明細書に開示され、エキシマレーザーを使用して、繊維柱帯を切除することにより緑内障を治療することが説明されている。これには、臨床試験が行われたが、成功はしなかった。Hill et al. は、エルビウム:YAGレーザーを使用して、繊維柱帯に十分な厚さの孔を生成した(Hill et al.、Lasers in Surgery and Medicine 11:341-346, 1991)。この技術は、アーヴィンのUniversity of Californiaにおいて霊長類の動物をモデルにまた限定的に人間の臨床試験において研究された。病的状態発生率は、両方の試験においてゼロとなったが、成功率によって、さらに人間の試験を行うことは是認されなかった。失敗が繰り返されたのは、修復メカニズムにより、繊維柱帯において生じた欠損が閉塞されることによるものだった。これらのいずれも、緑内障に効果的な外科技術ではない。

隅角掻爬術 これは、ab-interno(内側からの)機械破壊技術である。これは、先端にマイクロキュレットを有する毛様体解離スパチュラに類似した器具を使用する。初期の結果は、メカニズム及び閉塞というプロセスを二次的に修復できない線維柱帯切開術と同様である。繊維柱帯切開術では、フィルタリング外科術が最もよく行われるが、ヴィスコカニュロストミー(VC)及び非穿孔繊維柱帯切開術(NPT)は、フィルタリング外科術の2つの新しい変種である。これらは、大きく非常に深い強膜弁を形成することによりシュレム管が外科術で露出される、ab-externo(外側からの)、主な目の処置である。VC処置において、シュレム管には、カニューレが挿入され、(シュレム管及び水様液コレクタチャネルを膨張させる)粘弾性物質が注入される。NPT処置においては、シュレム管の内壁は、外科術により管が露出された後に取り除かれる。

背景を示す目的のために、図1は、眼球10の断面図を示し、図2は、繊維柱帯、前房、及びシュレム管の解剖学的な相対位置を表す拡大図を示す。強膜11として知られる厚い膠原組織が、角膜12により覆われた部分を除いて、眼球10全体を覆う。角膜12は、焦点を合わせ、かつ、眼球と、虹彩13の中央の円形孔(眼の色のついた部分)である瞳孔14との中に光を透過させる薄い透明な組織である。角膜12は、角膜縁15と呼ばれる接合点で強膜11に合体する。毛様体16は、眼球内において内部に始まり、強膜11の内面に沿い延び、脈絡膜17になる。脈絡膜17は、下に網膜18がある眼球の血管層である。視神経19が、視覚情報を脳に伝達するが、緑内障により結果として損なわれる。

眼球10の前房20は、角膜12により前部を、虹彩及び水晶体26により後部を包まれ、水様液で満たされている。水様液は、最初に毛様体16により産生され、瞳孔14を通り、虹彩13と角膜12との間に形成された前房隅角25に到達する。正常な眼において、水様液は、繊維柱帯21を通じて除去される。水様液は、繊維柱帯21からシュレム管22内へ、それから血液を運ぶ静脈と合流する房水静脈23へ通過し静脈還流が行われる。眼球10の内圧は、上述のような水様液の分泌及び流出の複雑なバランスによって維持される。緑内障は、内圧の増加を生じる、前房内における水様流体の蓄積超過という特徴を有する(流体は、比較的圧縮性が低く、圧力は、眼球の全領域に等しく向けられる)。

図2に示したように、繊維柱帯21は、強膜11の狭い一部分を構成する。結膜24の組織を通して装置を埋め込むための孔または開口部を形成することが、繊維柱帯21のみを通して装置を埋め込む外科手術に比べて比較的主要な外科手術であることは、理解できる。繊維柱帯21から配置される、現存の流出経路を使用または修復するための本発明のセトンインプラント31が、図5に示されている。

図3は、本発明の原理によるセトンインプラント31の実施形態を示す。セトンインプラントは、生体適合性を有する材料、例えば、SilasticTMの商標で販売されミシガン州ミッドランドのDow Corning Corporationから入手可能な材料のようなメディカルグレードのシリコン、または、Pellethane TMという商標で販売されDow Corning Corporationから入手可能なポリウレタンを含み得る。代替実施形態において、他の生体適合性を有する材料(生体適合性物質)が使用可能であって、ポリビニルアルコール、ポリビニルピロリドン、コラーゲン、ヘパリン化コラーゲン、テトラフルオロエチレン、フッ素化ポリマー、フッ素化エラストマー、可撓性のある石英ガラス、ポリオレフィン、ポリエステル、ポリシリコン、生体適合性を有する材料の混合物など、がある。さらなる代替実施形態において、上述の生体適合性物質の表面をコーティングすることによる合成生体適合性材料を、利用可能であり、コーティング材料は、ポリテトラフルオロエチレン(PTFE)、ポリイミド、ヒドロゲル、ヘパリン、治療薬などからなる群から選択可能である。

セトンインプラントの主目的は、内圧のバランスがとれるように、水様液が外側方向40にシュレム管内へ、続いて水様液コレクタ、房水静脈内へ流出することを容易にする援助を行うことである。一実施形態において、セトンインプラント31は、遠位部32とインレット部44とを有した細長い管状エレメントを備える。剛性または可撓性のある遠位部32は、現存の流出経路の一つの内部に配置される。遠位部は、テーパーのついた出口端部33を備えるか、或いは、埋め込み後に前記現存の流出経路内にセトンインプラントを安定させるために半径方向外側に突出する少なくとも1つの縁部37または他の保持装置を備え得る。安定化のために、遠位部32の外面は、短突出面、リブ付き面、ピラー付き面、テキスチャード加工面などを含み得る。セトンインプラントの外面36は、出口端部33に外側領域35及び内側領域34を含み、外面と周囲の組織との間の相互作用及び刺激が最小限となるように生体適合性及び組織適合性を有する。セトンインプラントは、2つ以上の方向に水様液が流れ得るように、出口端部33から離れた、遠位部32に隣接した場所に少なくとも1つの開口部を備え得る。少なくとも1つの開口部は、遠位部32の、出口端部33の反対側あたりに配置可能である。

別の典型的な実施形態において、セトンインプラント31は、一方向の水様液の流れ40を可能とするために一方向流制御手段39を備え得る。一方向流制御手段39は、逆止弁、スリットバルブ、マイクロポンプ、半透膜などからなる群から選択可能である。流出効率を高めるためには、出口端部33から離れた場所において、典型的な実施形態においては出口端部33の反対側に、遠位部32の近位部分内に、少なくとも1つの任意の開口部41が備えられる。

図4は、図3の上部断面図である。出口端部33の開口部及び、遠位部32の残りのボディの形状は、現存の流出経路の形状に一致するように適合させた楕円形、円形または他の形状とすることができる。この外形は、回転を妨げることにより虹彩及び角膜についてインレット部が安定するようにシュレム管の外形に適合する。

図3に示したように、本発明のセトンインプラントの長さは、セトンインプラントが使用されるボディ孔により異なるが、約0.5mmから1ミリメートルを超える。セトンインプラントの外径は、約30μmから約500μmの範囲とすることができる。内腔の直径の範囲は、好ましくは、約20μmと約150μmとの間である。セトンインプラントは、複数の流れを輸送することを容易にするために複数の内腔を備え得る。遠位部は、インレット部44について約30度から約150度の角度で、典型的な実施形態においては70から110度で、湾曲させることができる。

図5は、本発明の原理によって構成したセトンインプラント45の別の実施形態を示す。典型的実施形態において、セトンインプラント45は、少なくとも2つの部分、インレット部47とアウトレット部46とを含み得る。アウトレット部は、セトンインプラント45の出口端部にある出口開口部48を有する。出口開口部48の形状は、好ましくは、現存の流出経路の外形に適合する楕円形である。アウトレット部46に連結領域が隣接した、インレット部47の一部分は、インレット部47の残り及びアウトレット部46を繊維柱帯の外側にしたまま、必ず病んだ繊維柱帯の中に配置される。図5に示したように、楕円形開口部48の長軸線は、X軸線及びY軸線により形成される第1の面内にある。インレット部47は、前房20、繊維柱帯21及び現存の流出経路の解剖学的外形により適合するように、好ましくは、想像上のインレット部47A及びアウトレット部46により形成される、第1の面から角度θ高くなった第2の面に配置することができる。角度θは、約30度と約150度との間とすることができる。

図6は、眼球10の組織内に配置された、本発明のセトンインプラント31、45を図示する斜視図を示す。孔及び開口部が、病んだ繊維柱帯21の中に生成される。セトンインプラント31の遠位部32が、該孔内に挿入されて、入口端部38が、前房20に露出され、それと同時に、出口端部33は、前記病んだ繊維柱帯21の外面43の近くに配置される。さらなる実施形態において、出口端部33は、現存の流出経路の流体コレクタチャネル内にさらに入り込み得る。

繊維柱帯外科術の典型的実施形態において、患者は、仰向けにされ、手術の準備処置を施され、患部を無菌布でおおわれ、麻酔をかけられる。一実施形態において、小さい(1mm未満の)セルフシール切開が行われる。セトン配置部位の反対側の角膜から、洗浄ナイフで繊維柱帯内の切開が行われる。セトン31は、次に、ゴニオスコープ(レンズ)または内視鏡の案内の下、洗浄アプリケータ51内に保持されて、角膜切開部52から前房20を横切って前進させられる。アプリケータは、引き抜かれて、外科手術が終わる。洗浄ナイフの太さは、20から40ゲージの範囲の、好ましくは約30ゲージとすることができる。

Claims (19)
- 眼球の繊維柱帯の一部を貫通して延びるように形成されたインレット部と、
前記眼球のシュレム管内に延びるように形成されたアウトレット部と、
当該セトン内を延びる内腔であって、使用時に流体が前房からシュレム管に流れるよう、前記インレット部からアウトレット部までの流体の伝達を可能にする内腔と、
を備えることを特徴とするセトンであって、
さらに、前記インレット部が、前記アウトレット部に対して、前記セトンが概ねL字状となるような角度をつけて配設されていることを特徴とする前記セトンを特徴とする眼球に埋め込むための装置。 - 前記インレット部と前記アウトレット部との間の角度は、70°から110°であることをさらに特徴とする請求項1に記載の装置。
- 前記アウトレット部は、長軸線を持つ楕円形断面を有する内腔を備えることをさらに特徴とする請求項1に記載の装置。
- 前記アウトレット部は長手方向軸線を有し、前記アウトレット部の前記長軸線及び長手方向軸線は面を画定し、前記インレット部は前記面の外側であって該面に対して角度θで位置する長手方向軸線を有することをさらに特徴とする請求項3に記載の装置。
- 前記角度θは、30°から150°であることをさらに特徴とする請求項4に記載の装置。
- 前記アウトレット部は、シュレム管の内面に対して摩擦係合を作用させるように形成された少なくとも1つの突出部を有することをさらに特徴とする請求項1から5のいずれかに記載の装置。
- 前記突出部は、少なくとも1つのかえしを含むことをさらに特徴とする請求項6に記載の装置。
- 前記突出部は、少なくとも1つのうね状隆起部を含むことをさらに特徴とする請求項6に記載の装置。
- 前記インレット部及びアウトレット部の少なくとも1つに逆止弁を備えることをさらに特徴とする請求項1から8のいずれかに記載の装置。
- 前記セトンは、細長い部材内に配置されることをさらに特徴とする請求項1から9のいずれかに記載の装置。
- 切断部材が、前記細長い部材の前記遠位端部に配置されることをさらに特徴とする請求項10に記載の装置。
- 前記切断部材は、ナイフ、レーザープローブ、鋭利なガイド部材、前記管の鋭利な遠位端部、及び超音波カッターからなる群から選択されることをさらに特徴とする請求項11に記載の装置。

- 前記細長い部材は、前記細長い部材の遠位端部へ流体を送る内腔を含むことをさらに特徴とする請求項14に記載の装置。
- 前記セトンは、剛性または半剛性を有することをさらに特徴とする請求項1から15のいずれかに記載の装置。
- 前記セトンは、唯一のインレット及び唯一のアウトレットを有することをさらに特徴とする請求項1から16のいずれかに記載の装置。
- 前記セトンは、前記インレット部とアウトレット部との間に肘状屈曲部を有することをさらに特徴とする請求項1から16のいずれかに記載の装置。
- 前記セトン内に配置されたマイクロポンプをさらに特徴とする請求項1から18のいずれかに記載の装置。
