Den foreliggende opfindelse angår visse hidtil ukendte N-oxider af nikotinsyreestere, samt farmaceutiske præparater indeholdende sådanne forbindelser.
5 Det er velkendt at atherosclerose forårsages af ak kumulering af lipider i aorta og de coronare, cerebrale og perifere arterier. Dette bevirker en forøget risiko for thromboser og arterietilstopning. Afhængig af arten af de forhøjede plasmalipoproteinniveauer, er enten det 10 forhøjede kolesterol eller triglyceridniveau af betydning. I denne forbindelse betragtes selv kolesterolniveauer på 200-300 mg/100 ml serum og tri- glyceridniveauer på 150-200 mg/100 ml serum som værende for høje.

De to bedst kendte aktive forbindelser, der hidtil har været anvendt ved behandlingen af hyperlipidæmi, er ethylesteren af 2-(p-chlorphenoxy)isosmØrsyre, betegnet clofibrat, salte heraf og nikotinsyre. Disse forbindelser 20 virker på serumlipiderne ved forskellige indvirk ningsmåder. Medens hos forsøgsdyr doser på 100-500 mg/kg legemsvægt især bevirker en reduktion af kolesterolniveauet udover en ringe reduktion af indholdet af de frie fedtsyrer, udvirker administreringen af 3-pyri-25 dylcarbinol eller nikotinsyre eller saltene heraf en væsentlig sænkning af fedtsyreniveauet selv i lave doser mellem 10 og 100 mg/kg. Imidlertid har ingen af disse to forbindelser en signifikant sænkende virkning på tri-glyceridniveauet. Derudover kan nikotinsyre på grund af 30 forbindelsens velkendte og ubehagelige bivirkninger (rødmen, hovedpine, kvalme, opkastning) kun anvendes i begrænset udstrækning, således at behandlinger ofte må opgives for tidligt.
35 Herudover er det kendt, at clofibrat bevirker et fald af triglycerid og præ-0-lipoproteinværdierne på indtil 50 % under de oprindelige værdier, hvorimod denne sænkning DK 159105 8 2 ikke opnås, når det drejer sig om kolesterol. Nikotinsyre og derivaterne heraf virker i modsætning hertil overvejende på de forhøjede kolesterol og forhøjede fedtsyreværdier, medens nedgangen af endogentriglyceridre-5 syntese ved hjælp af hæmning af vævslipolyse kun er en sekundær virkning (jvf. Ovennævnte aktive forbindelser har derfor evnen til signifikant at reducere blodniveauerne af kun én lipidkomponent f.eks. triglyceriderne, medens den anden lipidkomponent terapeutisk kun er påvirket i ringe grad 15 eller slet ikke. Sådanne terapeutiske, virkninger kan kun nås ved at forøge dosis.
De seneste fund inden for lipidområdet har vist, at et passende antihyperlipidæmisk lægemiddel ikke alene bør 20 reducere triglyeerid- og· kolesterol værdrerne, men også” bevirke en tilvækst i HDL lipoproteinerne (atherogen-beskyttende virkning), og en nedgang af LDL og VLDL lipoproteinerne under behandlingen. Herudover bør opformering af peroxi'somer ikke forekomme, således at 25 levervægten forbliver konstant.
I tysk offentliggørelsesskrift nr. 3 009 099 er beskrevet nikotinsyreestere, der skønt de løser visse af de foranstillede krav som f.eks. sænkning af begge lipid-30 komponenter lader meget tilbage, hvad angår de øvrige krav.
Den foreliggende opfindelse har derfor til formål at tilvejebringe andre forbindelser, der udover deres 35 lipidsænkende virkning ved samme dosis eller endog ved lavere dosis udviser en fordelagtig indflydelse på de andre faktorer, der er velegnet til at bekæmpe 3

atherosclerose.
Det har nu uventet vist sig, at N-oxider af visse nikotinsyreestere bevirker både en stærk sænkende virk-5 ning på de forhøjede serum triglycerid og kolesterolværdier og samtidig en stærk stigning af det vasobeskyttende HDL og en reduktion af de atherogene LDL og VLDIi lipoproteinværdier. Da de omhandlede forbindelser ikke forårsager opformering af peroxisomer, forbliver 10 levervægten i det væsentlige konstant.

Phenyl-gruppen i R kan være mono- eller disubstitueret, substituenten X eller Y er fortrinsvis cyclohexyl, cyclo-pentyl eller en 2-ethylbenzofuranoyl-3 gruppe. Eksempler på velegnede X og Y substituenter er methyl, ethyl, pro-10 pyl, isopropyl, butyl, tert-butyl, sec-butyl og pen-tylgrupper.
Fremgangsmåden til fremstilling af de omhandlede forbindelser kan bestå i at en nikotinsyreester, der er 15 ikke-oxideret ved nitrogenet, N-oxideres med peroxysyre eller iseddikesyre-hydrogenperoxid ved velkendte metoder, fortrinsvis i nærværelse af et ikke-reaktivt organisk opløsningsmiddel, idet det pågældende opløsningsmiddel ikke er specielt kritisk.

De omhandlede forbindelser kan oparbejdes til farmaceut-tiske præparater, der kan indeholde et normalt anvendt farmaceutisk acceptabelt bærestof eller fortyndingsmiddel udover den aktive forbindelse. Præparaterne er velegnede 25 til peroral og parenteral administrering.
Faste præparater til peroral administrering er kapsler, tabletter, piller, pulvere og granuler. I de faste præparater kan den aktive ingrediens blandes med mindst 30 ét inert bindemiddel f.eks. rørsukker, lactose eller stivelse. Yderligere forbindelser som f.eks. smøremidler eller puffer kan forefindes. Tabletterne og pillerne kan være udstyret med enterisk eller andet overtræk.
35 Til parenteral administrering kan de omhandlede forbindelser anvendes i form af emulsioner, opløsninger og suspensioner indeholdende almindelig anvendte inerte for-

5 tyndingsmidler såsom vand. De flydende præparater kan også indeholde befugtningsmidler, emulgeringsmidler og dispergeringsmidler samt sødemidler, smags- og lugtkorrigerende midler til perorale former.

Afhængigt af den påtænkte administreringsmåde og behandlingens varighed kan doseringen af den aktive forbindelse i præparatet variere.
10 Undersøgelser over virkning
Fremgangsmåde
Hun CFY rotter (50 til 60 g) indkøbtes fra Hacking and 15 Churchill Ltd. Dyrene vejedes straks, og anbragtes i enkelte netbure og fik en fire dages akklimatiseringsperiode .
Ydertemperaturen holdtes på 23 °C τ 2 °C; den relative 20 fugtighed og den omgivende temperatur registreredes dagligt og lyset kontrolleredes ved at give 12 timer lys (7 morgen til 7 aften) og 12 timer mørke pr. hver 24 timer.
25 Under hele undersøgelsen kunne dyrene drikke frit.
Efter fire dages akklimatiseringsperiode indgaves rotterne Spratt pulveriseret laboratoriedyrefoder nr. 2 (lav fedt) ad libitum. De næste 14 dage fik alle rotter 30 ad libitum Spratt pulveriseret laboratoriedyrefoder nr.
2, hvortil var sat 2 vægt/vægt-% kolesterol og 1 vægt/-vægt-% kolsyre. I løbet af dette tidsrum registreredes det enkelte dyrs foderoptagelse tre gange om ugen.
35 Mellem kl. 6 og 7.30 om morgenen på den 14. dag udtoges en blodprøve fra hver rotte fra det retro-orbitale plexus under let æteranæsthesi. Blodprøverne analyseredes for

6 total indhold af serumkolesterol, HDL kolesterol, Ø-lipo-proteiner og triglycerider, idet LDL og VLDL lipo-proteinerne som angivet i tabel X aftager.
5 På basis af de totale serumkolesterolmængder opdeltes dyrene i behandlingsgrupper, således at hver gruppe havde en ensartet fordeling af det totale kolesterolindhold og at det oprindelige gennemsnitskolesterolniveau var omtrent ens i alle grupper.

Under de efterfølgende 14 dages behandling modtog alle dyrene, bortset fra kontroldyrene, hyperkolesterolæmisk diæt, hvortil passende koncentrationer af forsøgsforbindelsen (0,1 %) var sat.
15 I løbet af de 14 dages lægemiddelbehandling registreredes det enkelte dyrs fødeoptagelse, medens dyrenes legemsvægt registreredes en gang om ugen.
20 På forsøgets sidste dag udtoges atter blod fra rotterne fra det retro-orbitale plexus på “samme tid som på dag 14 i førdoseringsperioden.
Blodprøverne analyseredes atter for totalt serumkole-25 sterolindhold, HDL kolesterol, Ø-lipoproteiner og triglycerider. Herefter aflivedes dyrene, og leverne fjernedes og vejedes.
X forhold til kontroldyrene sås højsignifikante positive 30 forandringer af alle parametre, bortset fra levervægtene (jvf. tabel 1). Levervægtene forblev konstant.

Almene metoder til fremstilling af nikotinsyreester 25 N-oxider
Til en iskold opløsning af det pågældende nikotinat +) (20 mmol) i 50-100 ml dichlorethan omrørt ved hjælp af en magnetisk omrøring sattes portionsvis i løbet af 15 30 minutter 4,22 g (22 mmol) 90 % 3-chlorperbenzoesyre.
Opløsningen omrørtes yderligere 30 minutter ved 0 °C. De øvrige reaktionsbetingelser kan ses i tabel 2 (tyndt-lagskromatografisk kontrol; SXO2; CHClg/MeOH 9:1). Reaktionstemperaturen lå generelt fra -10 °C til ca. 60 °C, 35 idet 0 °C til 40 °C var det sædvanlige optimale område.

Herefter fortyndedes reaktionsblandingen med dichlorme-than til ca. 250 ml. Den organiske fase vaskedes med mættet Na2SC>3-opløsning (2 x 100 ml), mættet NaHCOg opløsning (3 x 100 ml) og mættet NaCl opløsning (2 x 100 5 ml). Efter tørring over Na2SC>4 fjernes opløsningsmidlet i vakuum, remanensen omkrystalliseres eventuelt under anvendelse af aktivt kul (se tabel 2).
Andre perbenzoesyrer og andre peroxysyrer eller hydro-10 genperoxid i iseddikesyre kan anvendes i stedet for den anvendte benzoesyre, afhængig af hvad man ønsker.

10 +) Udgangsmaterialet til forbindelsen fra eksempel 7 blev fremstillet på følgende måde:
En blanding af 0,2 ml benzaron og 0,24 ml nikotin-15 syrechloridhydrochlorid i 400 ml tør pyridin henstår 50 timer ved stuetemperatur. Reaktionsblandingen udhældes i 800 ml isvand, og der extraheredes fire gange i 100 ml methylenchlorid. Det organiske lag vaskes to gange med 50 ml vand, tørres over vandfrit MgSO^ i 24 timer og ind-20 dampes. Den fremkomne olie optages i 80 ml ^varm ethanol. Forbindelsen udkrystalliserer ved afkøling, hvorefter opløsningsmidlet fjernes under vakuum, og forbindelsen omkrystalliseres med ethanol, hvorved der med 88 % udbytte opnås gule nåle med smp. 83 °C.

Sædvanligvis inducerer alle almindelige lipid-sænkende midler, der er clofibratderivater såvel som nikotinsyre 25 opformeringen af peroxisomer. Skønt dette kan være en signifikant del af deres virkningsmåde, fører påvirkningen på peroxisomerne sædvanligvis til en forøgelse af levervægten (hepatomegali) og omtales endog som en præneoplastisk transformation af levercellerne af visse 30 videnskabsmænd. I modsætning hertil har de her omhandlede forbindelser overraskende vist sig slet ikke at have nogen peroxisomal-prolifererende virkning.
Hvad angår genopdukken (fænomenet rebound) af frie fedt-35 syrer og triglycerider, når man anvender almindelige nikotinsyrer såvel som visse andre pyridylcarboxyl-syreestere, reducerer disse forbindelser kraftigt tri- 11

glyceriderne og fedtsyrerne efter hver enkelt administrering og i et vist givet tidsrum. Efter at den helbredende virkning er ophørt, sker der imidlertid en stærk stigning i de frie fedtsyrer og triglycerider, en såkaldt rebound 5 til niveauer, der er betydelig højere end de niveauer, der ses før behandlingen. Disse højere niveauer bevirker en betydelig højere risiko for atherosclerose. Overraskende producerer de her omhandlede N-oxider ikke dette specielle fænomen, og giver derfor en betydelig effek-10 tivere behandling af atherosclerose uden reboundfæ- nomener.
Herudover frembringer almindelig nikotinsyre såvel som visse pyridylcarboxylsyreestere en let stimulerende ef-15 fekt på prostacycliner, hvilket forhindrer blodpladeag-gregation efterfulgt af atherosclerotisk plaguedannelse.
De her omhandlede N-oxider viser sig at udøve deres virkning på prostaglandinbiosyntesen på en anden måde, hvilket positivt bevirker en yderligere farmakologisk 20 effekt, nemlig en anti-ødematisk eller antiphlogistisk virkning. Da ødem i arteriekarvæggen antages at være det første trin i atheroscleroseplaquedannelsen, frembyder de her omhandlede forbindelser en interessant anti-athero-sclerotisk profil i forhold til deres stamnikotin- 25 syreestere. Stamforbindelserne synes at reagere over for atherosclerose på et senere tidspunkt, nemlig ved blod-pladeaggregationen, medens de her omhandlede N-oxider synes at være aktive allerede under atherogenesens første trin, nemlig over for ødemer i arteriekarvæggen.

Herudover viser de her omhandlede forbindelser i et dyreforsøg af Gustafson og Kiessling, publiceret i Z. Versuchstierkunde 24, 271-277 (1978), benævnt "The Mouse as a Model for Evaluation of Hypotriglyceridemic Drugs", 35 ved behandling af 5 dyr pr. gruppe over en tidsperiode på 1 uge med de omhandlede forbindelser blandet i en standarddiætkoncentration på 0,3 %, en forøgelse i den 12
