Pyridyl-N-oxiden.
De uitvinding heeft betrekking op N-oxiden van pyridylcarbonzuuresters, op een werkwijze voor de bereiding ervan, op farmaceutische preparaten met dergelijke verbindingen erin, alsmede op een werkwijze voor de toepassing ervan.
5 Bekend is, dat atherosclerosis wordt veroorzaakt door de ophoping van lipiden in de aorta en de coronaire, cerebrale en perifere arteriën. Dit resulteert in een verhoogd risico op trombose of arterieverstopping. Afhankelijk van de aard van de verhoogde plasmalipoproteïnespiegels is hetzij de 10 verhoogde cholesterolspiegel, hetzij de verhoogde triglyce-ridespiegel van belang. In dit verband blijken zelfs cholesterolspiegels van 200-300 mg/100 ml serum en triglyceridespie-gels van 150-200 mg/100 ml serum te hoog te zijn.
De twee meest algemeen bekende werkzame stoffen, 15 die tot nu toe werden gebruikt voor de behandeling van hyper-lipidema's zijn de ethylester van 2-(p-chloorfenoxy)isoboter-zuur, bekend als clofibraat, alsmede zouten ervan; en nicoti-nezuur. Deze verbindingen werken in op de serumlipiden via verschillende werkingsmechanismen. Bij proefdieren blijkt een 20 dosis van 100-500 mg/kg lichaamsgewicht in hoofdzaak een verlaging van de cholesterolspiegel teweeg te brengen naast een geringe verlaging van de vrije vetzuren, terwijl de toediening van 3-pyridylcarbinol of nicotinezuur of een zout ervan een aanzienlijke verlaging van de vetzuurspiegel teweeg blijkt te 25 brengen, zelfs bij een dosis tussen 10 en 100 mg/kg. Echter geen van deze beide stoffen heeft een significant verlagende werking op de triglyceridespiegel. Bovendien kan nicotinezuur ten gevolge van zijn onaangename en bekende bijwerkingen {blozen, hoofdpijn, braken), slecht in beperkte mate worden 30 gebruikt, zodat de therapie vaak voortijdig dient te worden afgebroken.
Bovendien is bekend, dat clofibraat in feite bewerkstelligt dat de triglyceride- en pre-g-lipoproteïnewaarden zakken tot 50 S beneden de aanvankelijke waarden, welke ver-35 laging echter niet wordt bereikt in geval van cholesterol.

Nicotinezuur en derivaten daarvan blijken daarentegen in hoofdzaak in te werken op verhoogde cholesterol- en verhoogde vet-zuurwaarden, terwijl de afname van endogene triglyceride-resynthese via remming van de weefsellipolyse slechts een 5 secundaire werking blijkt te zijn (cf. Negotiations of the German Association for Internal Medicine, 82ste congres, gehouden te Wiesbaden van 25-29 april 1976, deel I, J.F. Berg-mann Verlag, München).
De boven genoemde werkzame stoffen hebben der- 10 halve het vermogen de bloedspiegels van slechts één lipidecom-ponent significant te verlagen, bijv. van de triglyceriden, terwijl de andere lipidecomponenten slechts in geringe mate of helemaal niet therapeutisch worden beïnvloed. Een dergelijke therapeutische werking kan slechts worden bereikt via een ver- 15 hoogde dosering.
Overeenkomstig de meest recente ontdekkingen op het gebied van het lipide-onderzoek dient een geschikt anti-hyperlipidemisch geneesmiddel niet slechts de triglyceride- en cholesterolwaarden te verlagen, maar ook een toename van de 20 HDL-lipoproteïnen (atherogene beschermende factor) te verhogen en een afname van de LDL- en VLDL-lipoproteïnen in verloop van de behandeling te bewerkstelligen. Bovendien mag er geen proliferatie van peroxisomen optreden, zodat het levergewicht constant blijft.
25 Het Duitse Offenlegungsschrift 30 09 099 be schrijft nicotinezuuresters, die ofschoon sommige van de boven genoemde vereisten voldoen, zoals verlaging van beide lipidecomponenten, toch nog veel te wensen over laten met betrekking tot de overige vereisten.
30 Dientengevolge is een doel van de uitvinding het verschaffen van andere verbindingen, die naast hun lipide-verlagende werking bij dezelfde dosering of zelfs bij een lagere dosering, een gunstige invloed uitoefenen op de andere factoren, geschikt voor de bestrijding van atherosclerosis.
35 Verrassenderwijs is gebleken, dat N-oxiden van pyridylcarbonzuuresters een krachtige werking hebben op zowel verhoogde serumtriglyceridewaarden als op de verhoogde cholesterolwaarden door een simultaan teweeg brengen van een sterke 8401357 • * .-3- toename van de vasobeschermende HDL en een reductie van de atherogene LDL en VLDL-lipoprotexnewaarden. Aangezien de verbindingen volgens de uitvinding geen proliferatie van peroxi-somen teweeg brengen blijft het levergewicht praktisch con-5 stant.
Dienovereenkomstig omvat de uitvinding N-oxiden van pyridylcarbonzuuresters met formule 1 van het formuleblad, waarin R de groep met formule 2 van het formuleblad voorstelt, waarin X een waterstofatoom, een al dan niet vertakt C^-C^-10 alkyl, -C^“cycloalkyl, 1-adamantyl of cyclohexylacetyl voorstelt, terwijl Y een waterstofatoom, een al dan niet vertakt C^-C^-alkyl, 2-ethylbenzofuranoyl-3, of kyl, of 2-{_ (4-chloor fenoxy) -2-methylpropanoyloxy7~ethyl is; een werkwijze voor de bereiding ervan, farmaceutische prepara-15 ten met dergelijke verbindingen erin, alsmede de toepassing ervan als anti-hyperlipidemische middelen.

De -C-OR-groep kan voorkomen in de positie 2, 3 of 4, waarbij de positie 3 de voorkeur verdient. De fenyl-groep van R kan mono- of digesubstitueerd zijn, waarbij de 20 substituent X of Y bij voorkeur een cyclohexylgroep, cyclo-pentylgroep of 2-ethylbenzofuranoyl-3-groep is. Voorbeelden van geschikte X- en Y-substituenten zijn methyl-, ethyl-, propyl-, isopropyl-, butyl-, tert-butyl-, sec-butyl- en pentyl-groepen.
25 De werkwijze voor de bereiding van de verbindingen volgens de uitvinding heeft het kenmerk, dat een pyridylcar-bonzuurester, die aan de stikstof niet geoxideerd is, met een peroxyzuur of ijsazijn-waterstofperoxide N-geoxideerd wordt overeenkomstig bekende methoden, bij voorkeur in aanwezigheid 30 van een niet-reactief organisch oplosmiddel, waarbij het gebruikte oplosmiddel niet kritisch is.
De verbindingen volgens de uitvinding kunnen worden verwerkt tot farmaceutische preparaten, die een gebruikelijke farmaceutisch aanvaardbare drager of verdunningsmiddel 35 bevatten, naast de werkzame stof. Zij zijn geschikt voor orale en parenterale toediening.
Vaste preparaten voor orale toediening zijn kap-sules, tabletten, pillen, poeders en korrels. Bij dergelijke 8401357 - 4 - vaste preparaten wordt het werkzame bestanddeel gemengd met tenminste één inert bindmiddel, bijv. rietsuiker, lactose of zetmeel. Er kunnen ook andere stoffen aanwezig zijn, zoals smeermiddelen of buffers. De tabletten en pillen kunnen bedekt 5 worden met een enterische of andere coating.
Voor parenterale toediening kunnen de verbindingen volgens de uitvinding gebruikt worden in de vorm van emulsies, oplossingen en suspensies, die de gebruikelijke inerte verdunningsmiddelen, bijv. water, kunnen bevatten. Der-10 gelijke vloeibare middelen kunnen bovendien bevochtingsmidde-len, emulgeermiddelen en dispergeermiddelen bevatten, alsmede zoetstoffen, smaakstoffen, alsmede geur verlenende stoffen in orale vorm.
Afhankelijk van de voorgenomen toedieningsvorm en 15 behandelingsduur kan de dosering van de werkzame stoffen in het preparaat variëren. In het algemeen bevat een therapeutisch middel dat geschikt is voor de behandeling van hyper-lipidema, een hyperlipidemisch werkzame dosis van tenminste ëén verbinding volgens de uitvinding, gewoonlijk gecombineerd 20 met een farmaceutisch aanvaardbare drager of verdunningsmiddel.
Het aantonen van de werking
Methode
Er werd gebruik gemaakt van vrouwelijke CFY-rat-ten (50-60 g) afkomstig van Hacking and Churchill Ltd. Bij 25 aankomst werd elk dier gewogen en vervolgens overgebracht in een afzonderlijk hok van metaaldraad, waar het dier vier dagen kreeg om te acclimatiseren.
De kamertemperatuur werd gehandhaafd op 23°C + 2°C, terwijl de relatieve vochtigheid en kamertemperatuur 30 dagelijks werden genoteerd. De belichting werd zodanig geregeld dat er gedurende 12 uur licht was (van 's-morgens 7 uur tot 's-avonds 7 uur), terwijl gedurende 12 uur het donker was per 24 uur.
Tijdens de studie kregen de dieren water toege-35 diend ad libitum.
Tijdens de vier dagen durende acclimatiserings-periode werden de ratten gevoed met gepoederd laboratorium-dierendiëet nr. 2, afkomstig van Spratt (laag vetgehalte) ad 8401357 - 5 - libitum. Tijdens de volgende 14 dagen kregen alle dieren ad libitum Spratt's gepoederd laboratorium-dierendiëet toegediend, waaraan 2 % (gew./gew.) cholesterol en 1 % (gew./gew.) choli-nezuur waren toegevoegd. Gedurende deze periode werd de af-5 zonderlijke voerconsumptie drie keer per week geregistreerd.
Tussen 6 en 7.30 uur in de ochtend van de veertiende dag werd een bloedmonster getrokken van elk van de ratten en wel onder retro-orbitale plexus onder lichte ether-anesthesie. Alle bloedmonsters werden geanalyseerd op de 10 totale serumcholesterol, HDL-cholesterol, 8-lipoproteïnen en triglycerideconcentraties.
Op basis van de totale serumcholesterolspiegels werden de dieren ingedeeld bij de behandelingsgroepen, zodanig dat elke groep een soortgelijke verdeling van de totale cho-15 lesterolspiegels had, terwijl de aanvankelijke hoofdcholesterolspiegel voor alle groepen praktisch gelijk was.
Tijdens de volgende veertien daagse behandelings-periode kregen alle dieren, met uitzondering van de controle-dieren, het hypercholesterolemische diëet toegediend, waaraan 20 de geschikte concentratie van de proef verbinding (0,1 %) was toegevoegd.
Tijdens de veertien dagen durende toediening van het geneesmiddel werd de individuele voerconsumptie dagelijks genoteerd, terwijl het lichaamsgewicht van de dieren een keer 25 per week werd waargenomen.
Op de laatste dag van de studie werd van de ratten opnieuw een bloedmonster getrokken en wel van de retro-orbitale plexus en wel op hetzelfde tijdstip als op dag veertien van de pre-dosisperiode.
30 De bloedmonsters werden wederom geanalyseerd op totale serumcholesterol-, HDL-cholesterol-, β-lipoproteïne- en triglycerideconcentraties. De dieren werden vervolgens gedood, de levers verwijderd en gewogen.
Vergeleken met de controledieren konden zeer 35 significante positieve veranderingen van alle parameters, behalve levergewicht, worden waargenomen (zoals in tabel A is weergegeven). Het levergewicht bleef constant.

De volgende voorbeelden dienen ter verduidelij-20 king van de uitvinding, echter zonder de uitvinding daartoe te beperken.
Algemene procedure voor de bereiding van pyridyl-carbonzuurester-N-oxiden
Er werd aan een ijs gekoelde oplossing van het 25 desbetreffende nicotinaat +) (20 mmol) in 50-100 ml dichloor-ethaan, geroerd met behulp van een magneetroerder, 4,22 g (22 mmol) van een 90 % 3-chloorperbenzoezuur toegevoegd in verloop van 15 min. en wel portiegewijs. De oplossing werd nog eens gedurende 30 min. bij 0°C geroerd. De overige reactie-30 omstandigheden zijn in tabel B vermeld (TLC-controle: Si02? CHClg/MeOH 9 : 1). De reactietemperatuur lag gewoonlijk tussen -10°C en ca. 60°C, met als optimaal een temperatuursgebied tussen 0°C en 40°C.
Vervolgens werd het reactiemengsel verdund met 35 dichloormethaan tot ca. 250 ml. De organische fase werd gewassen met een verzadigde oplossing van Na2S02 (2 x 100 ml), verzadigde NaHCO^-oplossing (3 x 100 ml) en verzadigde NaCl-oplossing (2 x 100 ml). Na drogen boven Na2S04 werd het oplos- 8401357 - 7 - middel onder vacuum verwijderd, waarna het residu werd omgekristalliseerd desgewenst onder gebruikmaking van actieve koolstof (zie tabel B).
In plaats van het perbenzoezuur kunnen andere 5 perbenzoezuren en andere peroxyzuren worden gebruikt, dan wel waterstofperoxide in ijsazijn.
De volgende verbindingen werden bereid volgens de boven beschreven methode:

20 Er werden talrijke andere produkten volgens de uitvinding bereid op de boven beschreven wijze uitgaande van de niet-N-geoxideerde uitgangsverbinding, bijv. welke beschreven zijn in het Amerikaanse octrooischrift 4.321.268, waaronder in het bijzonder de homologen en analogen van de verbin-25 ding van voorbeeld nr. III, waarin een andere lagere alkyl-graep, bij voorkeur een vertakte lagere alkylgroep, met tot en 'met 6 koolstofatomen aanwezig is, waaronder de eerder genoemde groepen, zoals X- of Y-substituenten, in plaats van de t-butylgroep, hetzij in de 2- of 3-positie en vanzelfsprekend in 30 dergelijke verbindingen de positie van de cyclohexylgroep op de fenylring eveneens gevariëerd kan worden, terwijl de lagere alkylgroep op de fenylring eveneens in een andere positie kan zijn, bijv. in de 3- of 4-positie.

8401357 Het uit?an9Smateri-aal voor de verbinding uit - 8 - voorbeeld VII wordt bereid als volgt:
Een mengsel van 0,2 ml benzaron en 0,24 ml nico-tinezuurchloridehydrochloride in 400 ml droge pyridine werd bewaard bij kamertemperatuur gedurende vijftig uur. Het reac-5 tiemengsel werd uitgegoten in 800 ml ijswater en vervolgens gedurende vier uur geëxtraheerd met 100 ml methyleenchloride. De organische laag werd twee keer gewassen met 50 ml water, boven watervrij magnesiumsulfaat gedurende 24 uur gedroogd en vervolgens ingedampt. De verkregen olie werd opgenomen in 80 10 ml warme ethanol. Het produkt kristalliseerde na afkoeling uit, waarna het oplosmiddel onder vacuum werd verwijderd en het produkt werd uitgekristalliseerd uit ethanol, onder oplevering van gele naaldvormige kristallen met een smeltpunt van 83°C in een opbrengst van 88 %.

In het algemeen induceren alle standaard lipide verlagende middelen/ die clofibraatderivaten zijn, zoals nico-tinezuur, proliferatie van peroxisomen. Ofschoon dit een significant deel van hun werkingsmechanisme is, leidt de induce-5 rende werking op peroxisomen in het algemeen tot een verhoging van het levergewicht (hepactomegalie) en wordt zelfs door sommige wetenschappelijke groeperingen besproken als een pre-neoplastische transformatie van de levercellen. Daarentegen bleken de N-oxideverbindingen volgens de uitvinding op ver-10 rassende wijze dergelijke peroxisomale proliferatiewerking te missen.
Voor het opnieuw binden van vrije vetzuren en triglyceriden bij toepassing van het standaardnicotinezuur alsmede van bepaalde andere pyridylcarbonzuuresters, blijken 15 dergelijke verbindingen triglyceriden en vetzuren sterk te verlagen na elke opeenvolgende toediening en gedurende een bepaalde tijdsperiode. Na het ophouden van deze gunstige werking blijft er een scherpe stijging in de vrije vetzuren en triglyceriden, een zogenaamde nieuwe binding, tot concentraties die 20 veel hoger zijn dan de concentraties vóór de behandeling. Deze hogere concentraties gaan gepaard met een aanzienlijk hoger risico voor atherosclerosis. Verrassenderwijze blijken de N-oxiden volgens de uitvinding geen aanleiding te geven tot dit opnieuw-bindingsfenomeen en maken derhalve een meer efficiënte 25 behandeling van atherosclerosis mogelijk.
Het standaardnicotinezuur, alsmede bepaalde pyridylcarbonzuuresters hebben voorts een geringe stimulerende werking op prostacyclinen, onder voorkoming van platelet-aggregatie, gevolgd door atherosclerotische plakvorming. De 30 overeenkomstige N-oxiden van de uitvinding blijken echter een werking uit te oefenen op prostaglandinebiosynthese op een verschillende manier, hetgeen positief resulteert in een extra farmacologische werking, te weten een anti-edema- of antiflogistische werking. Aangezien edema van de arteriële 35 vaatwand beschouwd wordt als de eerste stap in de atherosclerotische plakvorming, blijken de verbindingen volgens de uitvinding een interessant anti-atherosclerotisch profiel teweeg te brengen, vergeleken met hun uitgangsnicotinezuur- 8401357

- 10 - esters. De uitgangsverbindingen blijken te reageren tegen atherosclerosis op een later tijdstip, namelijk plateletaggre-gatie, terwijl de N-oxiden volgens de uitvinding actief blijken te zijn reeds tijdens de eerste stap van de atherogenese, 5 namelijk tegen de edema van de arteriële vaatwand.
Bovendien bleken de verbindingen volgens de uitvinding bij het diermodel van Gustafson en Kiessling, gepubliceerd in Z. Versuchstierkunde 2£, 271-277 (1978), met de titel "The Mouse as a Model for Evaluation of Hypotriglyceridemic 10 Drugs", bij een behandeling van vijf dieren per groep over een periode van een week, vermengd met een standaarddieet in een concentratie van 0,3 % een toename in het levergewicht teweeg te brengen, die kleiner was dan bij hun uitgangsverbindingen, terwijl met betrekking tot het totale cholesterol, gemeten in 15 millimolen per liter, de verbindingen volgens de uitvinding telkens geleid hebben tot verlaging van de totale cholesterol-waarden in een sterkere mate dan hun desbetreffende uitgangsverbindingen.
Uit het bovenstaande kan worden geconcludeerd dat 20 het duidelijk is, dat de onderhavige uitvinding nieuwe pyri-dylcarbonzuurester-N-oxiden verschaft, naast een methode voor de bereiding ervan, alsmede farmaceutische preparaten daarvan en een methode voor de behandeling van hyperlipidemia, met de boven genoemde eigenschappen en voordelen.

Claims (16)
1. Een N-oxide van een pyridylcarbonzuurester met formule 1 van het formuleblad, waarin R de groep met formule 2 van het formuleblad is, waarin X een waterstofatoom, 5 een al dan niet vertakt C^Cg-alkyl, C^-C^-cycloalkyl, 1-adamantyl of cyclohexylacetyl voorstelt, terwijl Y een waterstofatoom, een al dan niet vertakt C^-C^-alkyl, 2-ethylbenzo-furanoyl-3 of C5-C12 -cycloalkyl voorstelt, of 2-/T4-chloor-fenoxy) -2- (methy Ipropanoy loxy7 ethyl.
2. Verbinding volgens conclusie 1, met het kenmerk , dat de verbinding 2-cyclohexylfenylnicoti-naat-N-oxide is.
3. Verbinding volgens conclusie 1, m e t het kenmerk , dat de verbinding 4-cyclohexylfenylnicoti- 15 naat-N-oxide is.
4. Verbinding volgens conclusie l, met het kenmerk , dat de verbinding een cyclohexylfenylnicoti-naat-N-oxide is, waarin op de fenylring een lagere alkylsub-stituent aanwezig is.
5. Verbinding volgens conclusie 1, met het kenmerk , dat de verbinding 2-t-butyl-4-cyclohexyl-fenylnicotinaat-N-oxide is.
6. Verbinding volgens conclusie 1, met het kenmerk , dat de verbinding 4-(2-ethyl-3-oxo-benzo- 25 furan)-fenylnicotinaat-N-oxide is.
7. Verbinding volgens conclusie 1, met het kenmerk , dat de verbinding 1-/2-(4-chloorfenoxy)-2-methylpropanoyloxy7"2-(nicotinoyloxy)ethaan-N-oxide is.
8. Farmaceutisch preparaat, geschikt voor de be- 30 handeling van hyperlipidema, met het kenmerk, dat dit preparaat als werkzaam bestanddeel een effectieve antihyperlipidemische hoeveelheid van tenminste één verbinding volgens conclusie l bevat, naast een farmaceutisch aanvaardbare drager.
9. Werkwijze voor de bereiding van een pyridyl- carbonzuurester-N-oxide met formule 1 van het formuleblad, waarin R een groep met formule 2 van het formuleblad is, waarin X een waterstofatoom, een al dan niet vertakt C^-C^-alkyl, 8401357 - 12 - Cg-C^2~cycloalkyl/ 1-adamantyl of cyclohexylacetyl voorstelt, terwijl Y een waterstofatoom, een al dan niet vertakt C^-C^-alkyl, 2-ethylbenzofuranoyl-3, of C^-C^-cycloalkyl of 2-^T4-chloorfenoxy)-2-methylpropanoyloxyZethyl voorstelt, met 5het kenmerk, dat een pyridylcarbonzuurester met de boven genoemde formule, die echter aan de stikstof niet geoxideerd is, wordt N-geoxideerd door omzetting met een peroxy-zuur of ijsazijn/waterstofperoxide, onder oplevering van de gewenste N-oxideverbinding.
10. Werkwijze volgens conclusie 9, met het kenmerk , dat als uitgangsverbinding 2-cyclohexylfenyl-nicotinaat wordt gebruikt, terwijl als produkt het N-oxide ervan wordt verkregen.
11. Werkwijze volgens conclusie 9, met het 15. e n m e r k , dat als uitgangsverbinding 4-cyclohexylfenyl- nicotinaat wordt gebruikt, waarbij als produkt het N-oxide ervan wordt verkregen.
12. Werkwijze volgens conclusie 9, met het kenmerk, dat als uitgangsverbinding een cyclohexylfe- 20 nylnicotinaat wordt gebruikt, waarin op de fenylring een lager alkylsubstituent aanwezig is, terwijl als produkt het N-oxide ervan wordt verkregen.
13. Werkwijze volgens conclusie 9, met het kenmerk , dat als uitgangsverbinding 2-t-butyl-4-cyclo- 25 hexylfenylnicotinaat wordt gebruikt, waarbij als produkt het N-oxide ervan wordt verkregen.
14. Werkwijze volgens conclusie 9, met het kenmerk , dat als uitgangsverbinding 4-(2-ethyl-3-oxo-benzofuran)fenylnicotinaat wordt gebruikt, terwijl als produkt 30 het N-oxide ervan wordt verkregen.
15. Werkwijze volgens conclusie 9, met het kenmerk , dat als uitgangsverbinding 1-^2-(4-chloor- fanoxy)-2-methylpropanoyloxy7“2-(nicotinoyloxy)ethaan wordt gebruikt, terwijl als produkt het N-oxide ervan wordt verkre- » 35 gen.
16. Werkwijze volgens conclusies 9-15, met het kenmerk, dat de reactie wordt uitgevoerd bij een temperatuur tussen -10 en 60°C in aanwezigheid van een 8401357 5 - 13 - niet reactief organisch oplosmiddel, waarbij het peroxyzuur of waterstofperoxide in overmaat wordt gebruikt. 8401357
