Description
- Hintergrund der Erfindung
- Diese Erfindung betrifft im Allgemeinen einen bioabsorbierbaren Marker mit röntgendichten Bestandteilen, einen ”bioabsorbierbaren-röntgendichten Marker”, zur Verwendung auf einer implantierbaren Endoprothese, wie einem Stent. Der bioabsorbierbare Marker enthält dispergierbare, röntgendichte Bestandteile, die weder bioabsorbierbare noch abbaubar sind, sondern vom Körper ausgeschieden oder im Körper gespeichert werden.
- Implantierbare Endoprothesen, die Stents, Stent-Grafts und Implantate enthalten, werden in der perkutanen transluminalen Koronarangioplastie und in anderen medizinischen Prozeduren zur Reparatur und Stützung erkrankter oder schadhafter Arterien und Körperlumina verwendet. Implantate werden zum Bedecken oder Überbrücken von Leckstellen oder Dissektionen in Gefäßen verwendet. Stent-Grafts sind Stents, die im Allgemeinen eine poröse Beschichtungsbefestigung haben. Ungestützte Implantate sind poröse Röhren, die für gewöhnlich über einen offenen chirurgischen Zugang implantiert werden.
- Zur Sichtbarmachung des Durchgangs und der Platzierung der implantierbaren Endoprothese in Arterien und Körperlumina werden viele chirurgische Prozeduren mit Hilfe der fluoroskopischen Angiographie durchgeführt. Die chirurgische Abgabevorrichtung und implantierbare Endoprothese können sichtbar gemacht werden, wenn sie röntgendicht sind und einen radiografischen Kontrast relativ zu dem Körper bieten. Zum Beispiel kann Röntgenstrahlung zur Sichtbarmachung chirurgischer Abgabevorrichtungen und der Entfaltung des Implantats im Körper verwendet werden. Ebenso kann eine radiografische Kontrastlösung in das Körperlumen injiziert werden, so dass das Lumen in dem fluoroskopischen Bild erkennbar ist.
- Damit die implantierbare Endoprothese röntgendicht ist, muss sie aus einem Material bestehen, das eine radiografische Dichte aufweist, die höher als das umgebende Wirtsgewebe ist, und eine ausreichende Dicke haben, um den Durchgang von Röntgenstrahlen zu beeinflussen, um einen Kontrast im Bild zu erzeugen. Es wird auf den plattierten Verbund-Stent Bezug genommen, der im
US Patent Nr. 5,630,840 dargestellt ist. Eine implantierbare Endoprothese kann aus Metallen, einschließlich Tantal oder Platin, mit relativ hohen radiografischen Dichten hergestellt sein. Andere Metalle, wie rostfreier Stahl, Superlegierungen, Nitinol und Titan, mit geringeren radiografischen Dichten können ebenso verwendet werden. Es wird auf implantierbare Vorrichtungen verwiesen, die in denUS Patenten Nr. 4,655,771 ,4,954,126 und5,061,275 dargestellt sind. - Eine implantierbare polymere Endoprothese ist im Allgemeinen röntgendurchlässig und weist keine ausreichende radiografische Dichte auf, um durch Fluoroskopie leicht dargestellt zu werden. Zur Verbesserung der Abbildung solcher polymeren Materialien können Polymere mit röntgendichten Füllmaterialien vor dem Formen oder Extrudieren gemischt werden, um die radiografische Dichte zu verbessern. Ein Nachteil bei der Verwendung von Füllmitteln mit Polymeren besteht jedoch darin, dass Änderungen in den Eigenschaften des Polymers eintreten können. Zum Beispiel kann die Zugabe von Füllstoffen die Stärke oder Verformbarkeit des Polymers verringern.
- Es besteht ein Bedarf an einem verbesserten bioabsorbierbaren-röntgendichten Marker zur Verwendung in medizinischen Vorrichtungen, insbesondere in temporären medizinischen Vorrichtungen mit geringer Röntgendichte. Der Bedarf zur Verbesserung der Röntgendichte einer relativ gering röntgendichten, implantierbaren Endoprothese oder zur Verbesserung der Abbildung unter schwachen röntgendichten Bedingungen ist besonders für die Prozeduren in der Chirurgie, Mikrochirurgie, Neurochirurgie und herkömmlichen Angioplastie wichtig, die unter Fluoroskopie durchgeführt werden. Ärzte stehen ständig der Herausforderung gegenüber, kleine Implantate an bestimmten intraluminalen Stellen zu platzieren. Verschiedene Vorrichtungen mit Röntgendichte sind in der Technik bekannt, wie in den
US Patenten Nr. 4,447,239 ,5,354,257 und5,423,849 dargestellt ist. -

- Daher besteht ein Bedarf an bioabsorbierbaren-röntgendichten Markern zur Verwendung auf implantierbaren Endoprothesen zur Verbesserung der Röntgendichte und der Lokalisierbarkeit einer Endoprothese während verschiedener medizinischer Prozeduren. Dieser Bedarf wird befriedigt von einem System gemäß Anspruch 1 und einem Verfahren gemäß Anspruch 21. Die Bereitstellung einer temporären Röntgendichte ist besonders für implantierbare Endoprothesen mit geringer oder ohne Röntgendichte vorteilhaft. Die bioabsorbierbaren-röntgendichten Marker ermöglichen eine radiografische Identifizierung einer oder mehrerer Stellen von Interesse auf einer implantierbaren Endoprothese. Bioabsorbierbare-röntgendichte Marker in dem Stoff- oder Beschichtungsmaterial einer implantierbaren Endoprothese sind zur Anzeige der Stelle des Stoffs oder der Beschichtung während der Implantation vorteilhaft.
- Alternative Verwendungszwecke enthalten das Einfädeln der Marker: neben einem spiralförmigen Strang in der implantierbaren Endoprothese; um den Umfang der implantierbaren Endoprothese; oder in einer geraden Linie in die axiale Richtung der implantierbaren Endoprothese. Ein oder mehrere bioabsorbierbare-röntgendichte Marker können auf der implantierbaren Endoprothese mit geringer oder ohne Röntgendichte verwendet werden. Nach der Implantation kann der bioabsorbierbare-röntgendichte Marker absorbiert, aufgelöst oder vom Körper ausgeschieden werden, so dass er die Funktion der Endoprothese nicht beeinträchtigt.
- Ein Nachteil gewisser permanenter röntgendichter Marker ist, dass sie die strukturelle Integrität beeinträchtigen können, nicht biokompatibel oder biostabil sein mögen, und thrombogener als die implantierbare Endoprothese sein können.
- Der bioabsorbierbare-röntgendichte Marker der vorliegenden Erfindung ermöglicht in vorteilhafter Weise, dass die meisten implantierbaren Endoprothesen eine temporäre Röntgendichte gegenüber einem vorbestimmten Abschnitt ihrer Struktur haben, und hilft in vorteilhafter Weise bei der richtigen Positionierung und Lokalisierbarkeit der implantierbaren Endoprothese in einem Körperlumen.
- Die Verwendung des bioabsorbierbaren-röntgendichten Markers ist vorteilhaft, dass die röntgendichte Eigenschaft nur für eine gewünschte Zeitperiode auf einer implantierbaren Endoprothese vorhanden sein kann. Zum Beispiel kann es wünschenswerter sein, die implantierbare Endoprothese, sobald sie implantiert ist, mit Techniken, wie Ultraschall, Magnetresonanz und Endoskopie abzubilden, und eine weitere Strahlenbelastung für den Patienten zu vermeiden. Wenn sich das bioabsorbierbare Polymer abbaut, dispergiert röntgendichtes Material gleichzeitig oder anschließend in den Körper. Die Dispersion des röntgendichten Materials von dem Marker führt zu einem Verlust der Röntgendichte in dem Marker. Eine vorbestimmte Freisetzungsrate des röntgendichten Materials kann in dem bioabsorbierbaren Marker auf der Basis des Abbaus des Polymers in dem Körper oder der Gestaltung der Markerstruktur festgelegt werden.
- Das bioabsorbierbare Material in den bioabsorbierbaren-röntgendichten Markern kann Polymere oder Copolymere, wie Polylactid (Poly-L-Lactid (PLLA), Poly-D-Lactid (PDLA)], Polyglycolid, Polydioxanon, Polycaprolacton, Polygluconat, Polymilchsäure-Polyethylenoxid-Copolymere, modifizierte Zellulose, Collagen, Poly(hydroxybutyrat), Polyanhydrid, Polyphosphoester, Poly(aminosäuren), Poly(alpha-hydroxysäure) oder verwandte Copolymermaterialien enthalten, von welchen jedes eine charakteristische Abbaurate im Körper aufweist. Zum Beispiel sind Polyglycolid und Polydioxanon relativ schnell bioabsorbierende Materialien (Wochen bis Monate) und PLA ist ein relativ langsam bioabsorbierendes Material (Monate bis Jahre). Für ein PLA-Element ist der Masseabbau mit einer vollständigen Absorption der polymeren Endoprothese in etwa 1,5 bis 3 Jahren nach der Implantation beendet.
- Bioabsorbierbare Harze, wie PLLA, PDLA, PGA und andere sind im Handel von mehreren Quellen erhältlich, einschließlich PURRC America, Inc., Lincolnshire, Illinois. Röntgendichte Materialien, wie Bariumsulfat und Wismuttrioxid, sind im Handel erhältlich und mit dem bioabsorbierbaren Harz von New England Urethane, Inc., North Haven, Connecticut, versetzt. Das bioabsorbierbare Harz oder bioabsorbierbare-röntgendichte Harz kann zu einem Filament von Albany International Research Co, Mansfield, Massachusetts, extrudiert werden.
- Die Bioabsorptionsrate des Markers kann so gestaltet werden, dass sie für Anwendungen schnell ist, wo eine akute Röntgendichte erwünscht ist, wie während der Positionierung und Platzierung des Implantats. Als Alternative kann die Bioabsorptionsrate so gestaltet sein, dass sie für Anwendungen langsamer ist, wo das Implantat radiografisch für zumindest einen Abschnitt seiner funktionsfähigen Zeit abgebildet werden muss, zum Beispiel bei Implantaten, wo die Heilung Monate dauern kann. Es sind auch andere Bioabsorptionsraten möglich. Die Bioabsorptionsrate des Markers kann durch Kontrolle der Art des bioabsorbierbaren Polymers, der chemischen Zusammensetzung des bioabsorbierbaren Polymers, des Molekulargewichts des bioabsorbierbaren Polymers, der Dicke und Dichte des bioabsorbierbaren Polymers, der Oberflächengröße des Markers, des Austrittsbereichs für das röntgendichte Material und die Gestaltung der Markerstruktur abgestimmt werden.
- Die Abbauprodukte von dem bioabsorbierbaren Marker und dem dispergierten röntgendichten Material werden vom Körper metabolisiert, ausgeschieden oder gespeichert. Der Metabolismus ist der chemische Prozess in lebenden Zellen, durch den Energie für vitale Prozesse und Aktivitäten bereitgestellt wird und neues Material zur Verarbeitung des Abfalls assimiliert wird. Es ist die Summe der Prozesse, durch die eine bestimmte Substanz in dem lebenden Körper verarbeitet wird.
- Die Ausscheidung ist die Trennung und Entfernung oder Abgabe aus dem Blut oder den Geweben unbrauchbaren, überflüssigen oder schädlichen Materials, das aus dem Körper eliminiert wird.
- Die Biokompatibilität absorbierbarer Polymere während des Abbaus hängt von der Ansammlungsrate ab wie auch davon, wie gut das umgebende Gewebe oder Fluid die Abbauprodukte puffert oder metabolisiert. Wenn die Produkte metabolisierbar sind, hängt die Rate, bei der dies stattfindet, von der Blutzirkulation im Gewebe ab. Eine gut mit Blutgefäßen versorgte Lumenwand könnte Abbauprodukte puffern und metabolisieren, sobald sie von dem Implantat freigesetzt werden. Dieser biologische Prozess ist wichtig, um eine nachteilige Gewebereaktion gegenüber dem sich abbauenden Implantat zu minimieren.
- Die Abbauprodukte von PLLA und PGA sind Milch- beziehungsweise Glycolsäure, die normalerweise im menschlichen Körper vorhanden sind. Die Säuren werden von Zellen um das Implantat metabolisiert. Der Metabolisierungsprozess ist ein Citratzyklus, der die Säuren zu Kohlendioxid umwandelt, das aus dem Körper ausgeatmet wird.
- Die röntgendichten Mittel, die dem bioabsorbierbaren Marker zugesetzt werden, sind im Allgemeinen im Körper unlöslich und somit nicht metabolisierbar. Wenn diese Materialien im Gewebe gefangen sind, reagiert der Wirt im Allgemeinen durch Einkapselung und Annahme der biologisch inaktiven Partikel. Wenn das Material von dem Implantat in den systemischen Kreislauf freigesetzt wird, wandert es mit dem Fluidstrom, bis es ausgeschieden oder von Organen oder Gewebe gesammelt und gespeichert wird. Die Vorstellung ist, dass nur geringe Mengen der röntgendichten Substanzen in dem Implantat vorhanden sind, indem der gesonderte bioabsorbierbare-röntgendichte Marker eingearbeitet und nicht das gesamte Implantat mit dem röntgendichten Material belastet wird. Die Minimierung der Menge des röntgendichten Materials, das von dem Marker bei Absorption des Polymers freigesetzt wird, muss berücksichtigt werden, wenn die prozentuelle Belastung auf der Basis der radiografischen und mechanischen Eigenschaften bestimmt wird.
- Um röntgendicht zu sein sollten die Marker Material mit atomaren Elementen einer ausreichend hohen Atomzahl enthalten und ausreichend dick sein, um eine ausreichende Röntgendichte für die Abbildung bereitzustellen. Der bioabsorbierbare-röntgendichte Marker kann einen oder mehrere hohle, hohlraumartige oder poröse Abschnitte aufweisen, in welchen röntgendichtes Material angeordnet werden kann.
- Eine Schwächung ist die Änderung in der Anzahl von Photonen im einfallenden Röntgenstrahl aufgrund der Wechselwirkung mit einem Absorptionsmittel. Zur Abbildung eines Objekts, das in dem Körper implantiert ist, wäre es wünschenswert, dass das Objekt Röntgenstrahlen stärker abschwächt als Körpergewebe, Knochen und Fett, so dass der Kontrastunterschied in einer Radiografie offensichtlich ist. Die Schwierigkeit in der Wahl eines röntgendichten Materials für chirurgische Implantate ist, dass das Material erwünschte radiografische Eigenschaften und Biokompatibilität aufweisen muss.
- Damit ein Implantat röntgendichter wird, kann eine Substanz, die mehr Röntgenstrahlen absorbiert, auf dem Implantatmaterial aufgebracht oder in dieses eingemischt werden. Wenn das Implantat mehr Röntgenstrahlen als das umgebene Medium (zum Beispiel Gewebe in dem Körper) absorbiert, ist es als scharfe Änderung im Kontrast auf einem Röntgenfilm oder einem Fluoroskopiebild erkennbar.
- Der Anteil der Röntgenstrahlenergie, der durch das Absorptionsmittel durchgeht, wird quantitativ durch die folgende Gleichung vorhergesagt, die in The Physics of Radiology, vierte Ausgabe, H. Johns, J. Cunningham, 1983, S. 137–142, beschrieben ist.

- N/N0 wäre der Anteil der einfallende Röntgenstrahlenergie, der durch das Absorptionsmittel durchgeht. Ein röntgendichteres Material hatte einen geringeren Anteil durchgehender Energie als ein röntgendurchlässigeres Material. Daher wäre zur Erhebung der Röntgendichte eines Materials, wie des Markermaterials, wünschenswert, ein Material mit einer hohen Röntgenstrahl absorbierenden Fähigkeit zu wählen, um den Anteil der durchgehenden Energie zu minimieren. Diese Fähigkeit zur Röntgendichte ist zu dem linearen Schwächungskoeffizienten und der Dicke des Absorptionsmaterials proportional. Je höher der Schwächungskoeffizient des Absorptionsmaterials für eine bestimmte Dicke ist, um so röntgendichter ist das Absorptionsmaterial. Die Schwächung, die von einem Absorptionsmaterial erzeugt wird, ist von der Anzahl von Elektronen und Atomen abhängig, die in dem Absorptionsmaterial vorhanden sind. Eine Möglichkeit zur Quantifizierung dieser Absorptionseigenschaft ist mit dem atomaren Schwächungskoeffizienten, der zu dem linearen Schwächungskoeffizienten und der Atomzahl des Absorptionselements direkt proportional ist. Die Röntgendichte ist daher im Allgemeinen zu der Atomzahl (Anzahl von Elektronen im Atom) des Materials proportional. Kandidatmaterialien zur Erhöhung der Röntgendichte chirurgischer Implantate hätten höhere Atomzahlen als die Elemente, die im Körper vorhanden sind, und mussten biokompatibel sein. Die Atomzahl muss ausreichend hoch sein, so dass eine relativ geringe Dicke des Absorptionsmaterials in dem Körper verwendet werden kann. Es wird auch auf den linearen Schwächungskoeffizienten verwiesen, der in
US Patent Nr. 5,628,787 beschrieben ist. Tantal, Zirkon, Titan, Barium, Wismut und Jod sind als nicht oxisch in bestimmten Konzentrationen bekannt, und sind daher Kandidatelemente zur Erhöhung der Röntgendichte eines Polymermarkers oder eines Implantats. Diese Elemente können dem Polymer in verschiedenen prozentuellen Anteilen zugesetzt werden, und der Schwellenwert, über dem der Anteil unzufriedenstellende Änderungen in den Polymereigenschaften bewirkt, kann durch Material- und Vorrichtungstestung bestimmt werden. Die Elemente, die in Mengen zugesetzt werden können, die ausreichen, um die Röntgendichte zu erhöhen und einen annehmbaren Wert der Polymereigenschaften aufrecht zu erhalten, und die biokompatibel sind, könnten in den Markern verwendet werden. Die biokompatiblen Elemente mit einem Bereich von Atomzahlen von 22 bis 83 und mit linearen Schwächungskoeffizienten im Bereich von 5,46 bis 149,08 cm–1 bei 50 KeV sollten eine ausreichende Erhöhung in der Röntgendichte bieten, ohne dass eine übermäßige Dicke notwendig wäre, um in Markern nützlich zu sein. Diese Elemente würden zumindest Titan, Vanadium, Chrom, Eisen, Kobalt, Nickel, Kupfer, Brom, Zirkon, Niob, Molybdän, Silber, Jod, Barium, Tantal, Wolfram, Platin, Gold und Wismut enthalten. Die bevorzugten metallischen Elemente für eine Biokompatibilität und Röntgendichte sind Titan, Zirkon, Tantal und Platin. Die bevorzugten organischen Elemente für eine Biokompatibilität und Röntgendichte sind Brom, Jod, Barium und Wismut. Besonders bevorzugte Elemente sind Tantal, Platin, Barium und Wismut wegen ihrer hohen Atomzahlen und Biokompatibilität (Atomzahlen von 56 bis 83 und linearer Schwächungskoeffizient von etwa 50 bis 149,08). Tantal und Platin werden als Stent-Komponenten verwendet und Bariumsulfat und Wismuttrioxid werden zur Erhöhung der Röntgendichte für Polymerkatheter verwendet.
- Der bioabsorbierbare-röntgendichte Marker kann in eine Subanordnung oder eine fertige implantierbare Endoprothese während der Herstellung integriert werden. Röntgendichte längliche Elemente können mit den nicht röntgendichten bioabsorbierbaren länglichen Elementen zur Bildung eines röhrenförmigen verflochtenen Stents verflochten werden, oder die bioabsorbierbaren und röntgendichten länglichen Elemente können zu einem fertig verflochtenen Stent gewebt werden.
- Der bioabsorbierbare-röntgendichte Marker wurde in vorteilhafter Weise einer implantierbaren Endoprothese eine temporäre Röntgendichte verleihen, so dass der temporäre Marker keine medizinische Prozedur zur Entfernung aus dem Patienten benötigt.
- In Summe betrifft die Erfindung eine implantierbare Endoprothese und ein Markersystem gemäß Anspruch 1.
- Das bioabsorbierbare Material kann ein Polymer oder ein Copolymer enthalten. Das bioabsorbierbare Material kann Poly-L-lactid, Poly-D-lactid. Polyglycolid, Polydioxanon, Polycaprolacton und Polygluconat, Polymilchsäure-Polyethylenoxid-Copolymere, modifizierte Zellulose, Collagen, Poly(hydroxybutyrat), Polyanhydrid, Polyphosphoester, Poly(aminosäuren), Poly(alpha-hydroxysäure) und Kombinationen davon enthalten. Das röntgendichte Material kann einen linearen Schwächungskoeffizienten von 5,46 cm–1 bei 50 KeV bis 149,08 cm–1 bei 50 KeV haben. Der Marker kann eine durchschnittliche Dicke von 20 Mikron bis 500 Mikron haben und das röntgendichte Material enthält mindestens ein Element mit einer Atomzahl von 22 bis 83. Das röntgendichte Material kann Bariumsulfat, Wismuttrioxid, Brom, Jod, Jodid, Titanoxid, Zirkoniumoxid, Tantal und Kombinationen davon enthalten. Das röntgendichte Material kann ein Oxid oder Salzmaterial sein. Eines von dem bioabsorbierbaren Material oder dem röntgendichten Material kann mit dem anderen beschichtet oder vermischt sein und das röntgendichte Material kann einen linearen Schwächungskoeffizienten von 5,46 cm–1 bei 50 KeV bis 149,08 cm–1 bei 50 KeV haben. Der Marker kann ein röntgendichtes Material in Gewichtsprozent in dem bioabsorbierbaren Material von 1% bis 80% haben. Das bioabsorbierbare Material kann aus PLLA bestehen und das röntgendichte Material kann aus Wismuttrioxid bestehen, und die Gewichtsprozent des Wismuttrioxids in dem PLLA können mindestens etwa 10% betragen. Das bioabsorbierbare Material kann aus PLLA bestehen und das röntgendichte Material kann Bariumsulfat sein, und die Gewichtsprozent des Bariumsulfats in dem PLLA können mindestens etwa 10% betragen. Der Marker kann im Wesentlichen in weniger als 3 Jahren abgebaut sein. ”Wesentlicher Abbau des Markers” bedeutet, dass der Marker mindestens 50% seiner strukturellen Stärke verloren hat. Es ist bevorzugt, dass der Marker 100% seiner strukturellen Stärke verloren hat. Das bioabsorbierbare Material kann aus Polylactid bestehen und das röntgendichte Material kann aus Bariumsulfat, Wismuttrioxid, Jod, Jodid und Kombinationen davon bestehen, und der Marker baut sich im Wesentlichen in 1 Jahr bis 2 Jahren ab. Das bioabsorbierbare Material kann Poly-L-Lactid, Poly-D-Lactid, Polyglycolid und Kombinationen davon enthalten und das röntgendichte Material kann Bariumsulfat, Wismuttrioxid, Brom, Jod, Jodid und Kombinationen davon enthalten, und der Marker baut sich im Wesentlichen in 3 Monaten bis 1 Jahr ab. Das bioabsorbierbare Material kann Polyglycolid, Polygluconat, Polydioxanon und Kombinationen davon enthalten und das röntgendichte Material kann Bariumsulfat, Wismuttrioxid, Brom, Jod, Jodid und Kombinationen davon enthalten und der Marker baut sich im Wesentlichen in 1 Woche bis 3 Monate ab. Der Marker kann ein Monofilament, Multifilament, Faden, Band, Nahtmaterial und Kombinationen davon sein. Der Marker kann ein oder mehrere hohle, hohlraumartige, poröse Abschnitte und Kombinationen davon enthalten und das röntgendichte Material kann darin angeordnet sein. Der Marker kann eine Röntgendichte für eine vorbestimmte Zeit aufweisen. Die Endoprothese kann ein Stent, ein Stent-Graft, ein Implantat, ein Filter, eine Verschlussvorrichtung oder ein Ventil sein. Die Endoprothese kann eine röhrenförmige, radial erweiterbare Struktur und axial flexible Struktur aufweisen mit einer Mehrzahl von länglichen Elementen, die zu einer verflochtenen Konfiguration verwoben sind.
- Vorzugsweise umfasst der röntgendichte Marker ein bioabsorbierbares Material und ein röntgendichtes Material, das mit Gewichtsprozent W in das bioabsorbierbare Material eingearbeitet ist, länglich ist, und eine durchschnittliche Dicke T (gemessen in mm) über die Länge des Markers (
14 ) hat; und wobei die Gewichtsprozent W gleich: - (i) [10 + (950T – 208,5)] ± 5 für röntgendichtes Material mit einem Atomgewicht von 20 bis 100;
- (ii) (950T – 208,5) ± 5 für röntgendichtes Material mit einem Atomgewicht von 100 bis 150; oder
- (iii) [(950T – 208,5) – 10] ± 5 für röntgendichtes Material mit einem Atomgewicht größer 150 sind.
- Vorzugsweise umfasst der Marker 20 Gewichtsprozent bis 99 Gewichtsprozent des bioabsorbierbaren Materials, und 1 Gewichtsprozent bis 80 Gewichtsprozent des röntgendichten Materials, wobei das röntgendichte Material mindestens eines von einer Flüssigkeit oder Partikel mit einem durchschnittlichen Partikeldurchmesser von weniger als 8 Mikron und einem maximalen Durchmesser von weniger als 10 Mikron umfasst, wobei das röntgendichte Material einen linearen Schwächungskoeffizienten von 5,46 cm–1 bei 50 KeV bis 149,08 cm–1 bei 50 KeV hat.
- Vorzugsweise ist das röntgendichte Material röntgendichter als Knochen.
- Die Erfindung betrifft auch ein Verfahren zum Modifizieren einer implantierbaren Endoprothese zur vorübergehenden Verstärkung einer Sichtbarmachung der Endprothese während und nach deren Implantation in ein Körperlumen wie in Anspruch 21 definiert. Vorteilhafte Ausführungsformen werden in den abhängigen Ansprüchen beschrieben.
- Weitere Aufgaben und Vorteile der vorliegenden Erfindung und Verfahren zu deren Konstruktion werden für den Fachmann aus der folgenden ausführlichen Beschreibung offensichtlich, wobei nur die bevorzugten Ausführungsformen dargestellt und beschrieben sind, einfach anhand einer Darstellung der als am besten angesehenen Ausführungsform der Erfindung. Wie erkennbar ist, kann die Erfindung andere und unterschiedliche Ausführungsformen und Konstruktionsverfahren beinhalten, und ihre diversen Einzelheiten können in verschiedener, offensichtlicher Hinsicht modifiziert werden, ohne von der Erfindung abzuweichen. Daher sind die Zeichnung und Beschreibung als Veranschaulichung und nicht als Einschränkung zu verstehen.

- Es wird auf
1 Bezug genommen, die eine Stent-Abgabevorrichtung10 mit einem oder mehreren bioabsorbierbaren-röntgendichten Markern14 zeigt, die in einem spiralförmigen Muster auf einer implantierbaren Endoprothese16 angeordnet sind. Der bioabsorbierbare-röntgendichte Marker14 wird auf der Endoprothese16 vorzugsweise vor dem Einbringen der Anordnung in die äußere Röhre einer Abgabevorrichtung10 angeordnet. Es wird auf eine Abgabevorrichtung verwiesen, die imUS Patent Nr. 5,026,377 dargestellt ist. -
2 zeigt eine implantierbare Endoprothese16 mit einem bioabsorbierbaren-röntgendichten Marker14 , der in einem spiralförmigen Muster darauf angeordnet ist, in einem Körperlumen12 . Implantierbare Endoprothesen16 , die in der Technik bekannt sind, umfassen Stents, Stent-Grafts, Implantate, Filter, Verschlussvorrichtungen, Ventile und Kombinationen davon, die alle den bioabsorbierbaren-röntgendichten Marker enthalten können. -
3a bis3c zeigen drei alternative Stellen auf einer implantierbaren Endoprothese16 zum Anordnen des bioabsorbierbaren-röntgendichten Markers. Der bioabsorbierbare-röntgendichte Marker14 kann an Abschnitten der Innenfläche17 , der Außenfläche19 angeordnet sein, oder kann um und durch die länglichen Elemente der implantierbaren Endoprothese16 gewebt oder geflochten sein. Als Alternative kann der bioabsorbierbare-röntgendichte Marker14 durch die Filamente der Endoprothese16 in einem relativ um den Umfang verlaufenden Muster geflochten zeigt.5 zeigt einen Marker14 , der durch die Filamente der Endoprothese16 in einem relativ spiralförmigen Muster geflochten ist. Andere Muster und Anordnungen des bioabsorbierbaren-röntgendichten Markers14 auf der Endoprothese16 sind ebenso möglich. Ein oder mehrere Marker14 können temporär auf der implantierbaren Endoprothese16 angeordnet sein, um in vorteilhafter Weise eine temporäre Röntgendichte an vorbestimmten Stellen auf der implantierbaren Endoprothese16 bereitzustellen. - Wie in
3a und3c dargestellt ist, kann der bioabsorbierbare-röntgendichte Marker14 an einer oder mehreren Oberflächen der implantierbaren Endoprothese16 mit einem relativ schwachen bioabsorbierbaren Klebstoff oder Gelatine angeordnet werden. - Der bioabsorbierbare-röntgendichte Marker
14 kann längliche Elemente, wie ein Band, einen Faden, ein Filament, ein Nahtmaterial oder Kombinationen davon enthalten. Der bioabsorbierbare-röntgendichte Marker14 kann zur Bildung eines Seils oder Kabels geflochten sein. - Wenn die implantierbare Endoprothese
16 aus der Abgabevorrichtung10 entfaltet wird, kann sich der bioabsorbierbare-röntgendichte Marker14 der Erweiterung der implantierbaren Endoprothese16 anpassen und in vorteilhafter Weise eine Röntgendichte bereitstellen und die Sichtbarmachung der Position oder Größe der implantierbaren Endoprothese16 während der Fluoroskopie verbessern. Sobald die implantierbare Endoprothese16 vollständig entfaltet ist, kann die Abgabevorrichtung10 aus dem Körper entfernt werden und der bioabsorbierbare-röntgendichte Marker14 kann auf der implantierbaren Endoprothese16 verbleiben, wo er bioabsorbiert, aufgelöst, dispergiert oder aus dem Körper ausgeschieden wird. Der bioabsorbierbare-röntgendichte Marker14 kann so gestaltet sein, dass er auf der implantierbaren Endoprothese16 für eine vorbestimmte Zeitperiode verbleibt, wenn ein Bedarf an einer späteren Angiografie besteht. - Es wird auf
6 Bezug genommen, die einen bioabsorbierbaren-röntgendichten Marker14 zeigt, der vorzugsweise aus einem relativ flexiblen, länglichen, polymeren Material besteht, das ein röntgendichtes Material enthält, das mindestens ein Element mit einer Atomzahl von 22 bis 83 beinhaltet. Das röntgendichte Material hat vorzugsweise einen linearen Schwächungskoeffizienten von 5,46 cm–1 bei 50 KeV bis 149,08 cm–1 bei 50 KeV. -

14 kann ein bioabsorbierbares Polymer enthalten, das mit einer röntgendichten Substanz, wie Jodid, Jod, Zirkoniumoxid, Bariumsulfat, Wismuttrioxid oder einer verwandten Oxid- oder Salzsubstanz beschichtet, versetzt, gefüllt, beladen oder gemischt ist. Röntgendichte Verbundmaterialien können mindestens ein Element mit einer Atomzahl, die vorzugsweise höher als etwa 22 ist, enthalten. Andere röntgendichte Materialien können Gold, Platin, Tantal, metallische Biomateriallegierungen zur Beschichtung und kleine Partikel dieser Materialien, mit einer Größe von vorzugsweise weniger als 10 Mikron, zum Vermischen enthalten. Zum Vermischen von röntgendichten Bestandteilen und bioabsorbierbaren Harzen zur Bildung des extrudierten bioabsorbierbaren-röntgendichten Filaments reicht der Gewichtsprozentsatz von röntgendichten Harzen zu bioabsorbierbaren Harzen von etwa 1 Prozent bis etwa 80 Prozent. Zum Vermischen röntgendichter metallischer Füllstoffe und bioabsorbierbarer Harze zur Bildung des extrudierten bioabsorbierbaren-röntgendichten Filaments reicht der Gewichtsprozentsatz von röntgendichten metallischen Füllmitteln zu bioabsorbierbaren Harzen von etwa 1 Prozent bis etwa 40 Prozent. Der bevorzugte Gewichtsprozentsatz von Wismuttrioxid und Bariumsulfat im PLLA-Filament ist ein Minimum von etwa 10%. Bevorzugte Ausführungsformen des bioabsorbierbaren-röntgendichten Markers sind in der Folge in Tabelle 2 angeführt. Tabelle 2 - Die Spalte für die Art des Markers in Tabelle 2 enthält eine Beschreibung der physischen Aspekte des Markers, wie ein Strang der einer Drahtspirale, folgend in und aus den geflochtenen Stent-Zwischenräumen, oder in und aus den geflochtenen Stent-Zwischenräumen um den Umfang, oder in und aus den geflochtenen Stent-Zwischenräumen in einer geraden Linie in der Axialausrichtung gefädelt wird. Ein Zwischenraum ist die Stelle, wo zwei Stent-Drähte in dem Geflecht übereinander kreuzen. Die Funktion des Markers ist in Tabelle 2 beschrieben um anzugeben, wo der Marker in der Endoprothese verwendet wird, zum Beispiel, um die Enden eines Stents anzuzeigen oder um eine radiografische Sichtbarmachung des Stents zu ermöglichen, der aus einem eingeengten Zustand in einen erweiterten Zustand beim Entfalten wechselt. Eine Liste von Vorrichtungen, wo der Marker eingearbeitet werden konnte, ist in Tabelle 2 angeführt und enthält im Allgemeinen verschiedene Arten intraluminaler Endoprothesen. Die bevorzugten röntgendichten metallischen Bestandteile (Ta, Pt, Zr, Ti) sind als biokompatibel bekannt und haben relativ hohe Atomzahlen und lineare Schwächungskoeffizienten. Diese Elemente würden dem bioabsorbierbaren Polymer zugegeben, um das Material röntgendicht und für eine radiografische Markierung geeignet zu machen. Die angrenzende Spalte ”Anteil des röntgendichten metallischen Bestandteils, Gew.-%” zeigt den bevorzugten Bereich des Anteils der röntgendichten Metallbestandteile im bioabsorbierbaren Polymer an, um dieses ausreichend röntgendicht zu machen, wie von etwa 1 bis etwa 20 Gewichtsprozent Tantal oder Platin, das mit dem Polymer vermischt oder auf dieses aufgetragen ist. Dieselbe Art von Informationen findet sich in den nächsten zwei Spalten für organische röntgendichte Bestandteile. Der Marker kann entweder mit metallischen oder organischen Bestandteilen hergestellt werden, wobei Metall für dünne Marker bevorzugt ist und organische Substanzen für dickere Marker geeigneter sind, wo höhere Anteile akzeptiert werden können (um den Marker nicht signifikant zu schwächen). Die letzten zwei Spalten in der Tabelle enthalten bevorzugte absorbierbare Polymere für das Marker-Matrixmaterial PLLA und PDLA sind für langsam absorbierende Marker bevorzugt, da die Abbaurate dieser Polymere ziemlich langsam ist (Monate bis Jahre). PGA und Polydioxanon sind für schnell absorbierende Marker bevorzugt, da die Abbaurate dieser Polymere ziemlich schnell ist (Wochen bis Monate).
- Zum Zwecke der Beschreibung können die Marker der Erfindung in Arten unterteilt werden: gefädelte und gesonderte bioabsorbierbare-röntgendichte Marker. Ein gefädelter Marker ist im Allgemeinen ein Strang oder Stränge aus Material mit Röntgendichte, der/die in die implantierbare Vorrichtung durch Weben oder Flechten des Strangs durch die Streben oder Drähte der Endoprothese eingearbeitet wird/werden. Ein gesonderter bioabsorbierbarer-röntgendichter Marker ist im Allgemeinen ein polymerer bioabsorbierbarer-röntgendichter Materialstrang, der sicher an einem lokalisierten Bereich der implantierbaren Vorrichtung befestigt ist und sich nicht signifikant über einen großen Abschnitt der Vorrichtung erstreckt.
- Ein Beispiel für einen gefädelten Marker in einem geflochtenen, röhrenförmigen Draht-Stent ist ein bioabsorbierbarer-röntgendichter Polymerstrang, der mit einem röntgendichten Bestandteil geladen ist, der in und aus den Drahtkreuzungspunkten gewebt ist und dem spiralförmigen Pfad eines einzelnen Drahtstrangs in dem Stent folgt.
- Ein Beispiel für einen gesonderten bioabsorbierbaren-röntgendichten Marker ist eine Spirale, ein Knoten oder ein Ring aus einem bioabsorbierbaren-röntgendichten Polymerstrang um ein Element eines Stents, wie einen Stent-Drahtkreuzungspunkt. Der Strang ist um den Stent-Draht geschlungen, gewickelt oder gebunden und wird dadurch mechanisch an der Vorrichtung befestigt. Die Strangenden werden abgezwickt, so dass der Marker als kleiner, fester Ring um ein Element des Stents vorhanden ist. Der Stent mit den befestigten Markern wird geladen und von der Abgabevorrichtung entfaltet.
- Die absorbierbaren, röntgendichten Marker werden in einer Reihe von intraluminalen Endoprothesen, wie Stents, Implantaten, Filtern, Verschlussvorrichtungen und Ventilen verwendet. Die Endoprothesen werden in Luftwege, das Verdauungssystem und das Gefäßsystem implantiert. Wenn die Marker implantiert und Körperfluida ausgesetzt werden, erfährt die absorbierbare Polymermatrix einen Abbau und zersetzt sich schließlich, wodurch die nicht abbaubaren röntgendichten Bestandteile in den Körper freigesetzt werden. Wenn die Endoprothese und die Marker vollständig in der Gefäßwand integriert sind, werden die röntgendichten Substanzen in dem lokalen Gewebe (wie bei einem Stent) gehalten. Wenn die Endoprothese und Marker nicht eingewachsen und integriert werden, können die röntgendichten Substanzen in das Körperfluid freisetzt werden. Das Freisetzen ist im Verdauungssystem kaum bedenklich, da die geringe Konzentration der freigesetzten Partikel wahrscheinlich eine geringe Wirkung auf die Galle haben und schnell ausgeschieden werden. Die Freisetzung von Partikeln in das Gefäßsystem ist weniger wünschenswert, aber dies kann durch die Verwendung geringer prozentueller Anteile und feine Partikelgrößen für vaskuläre Vorrichtungsindikationen vermieden werden.
- Die Funktion der absorbierbaren, gefädelten, röntgendichten Marker besteht darin, auf einem radiografischen Bild die Stelle des Stents innerhalb der Behandlungsstelle anzuzeigen, und die Länge des erweiterten Stents kann durch Messen der Länge des Markers bestimmt werden, da dieser der Stent-Form folgt, wenn der Marker entlang einer Stent-Drahtspirale oder axial entlang einer Linie im Stent gefädelt wurde. Der Marker kann um den Umfang an jedem Ende der Stent-Umhüllung in einem umhüllten Stent oder Stent-Graft gefädelt werden, um die Stelle des strahlendurchlässigen Umhüllungsmaterials anzuzeigen. Die Stent-Erweiterung während der Entfaltung kann radiografisch beobachtet werden, indem verfolgt wird, wie sich der spiralförmige oder um den Umfang verlaufende Strang des röntgendichten Markers öffnet, während der sich selbst ausweitende Stent aus seinem radial eingeengten Zustand freigegeben wird.
- Gesonderte bioabsorbierbare-röntgendichte Marker haben denselben funktionalen Zweck wie die gefädelten Marker, können aber einfacher zur Markierung spezifischer Stellen von Elementen von Interesse an dem Stent verwendet werden. Zum Beispiel kann ein gesonderter bioabsorbierbarer-röntgendichter Marker in der Mitte der Stent-Länge angeordnet werden, um dem Arzt beim Zentrieren des Stents in der Struktur zu helfen. Einzelne bioabsorbierbare-röntgendichte Marker könnten dazu verwendet werden, umhüllende Stoffe oder Filme an Stents zu befestigen, um Stent-Grafts so zu gestalten, dass die Stelle der Umhüllung auf dem Stent radiografisch bestimmt werden kann.
- Die gesonderten bioabsorbierbaren-röntgendichten Marker können aus biokompatiblen absorbierbaren Polymeren hergestellt werden, die Elemente mit relativ hohen Atomzahlen enthalten, wie Titan, Tantal, Zirkon und Platin. Die röntgendichten Elemente können durch die Herstellung metallurgischer Legierungen oder plattierter Verbundstrukturen hinzugefügt werden. Röntgendichte Bestandteile können in hohle Kerne, Hohlräume oder Poren in der Polymermatrix gefüllt werden. Organische röntgendichte Pulver, die Elemente oder Salze oder Oxide von Elementen, wie Brom, Jod, Jodid, Barium und Wismut enthalten, könnten anstelle von Metallpulvern verwendet werden.
- Die Menge eines röntgendichten Bestandteils, die der absorbierbaren Polymermatrix hinzugefügt wird, ist im Allgemeinen etwa 1 bis 80 Gewichtsprozent, aber der spezifische Anteil hängt von der Atomzahl des röntgendichten Bestandteils und der Dicke des Markers ab. Metallische Elemente, wie Tantal und Platin, die hohe Atomzahlen haben, können in geringen Prozentsätzen hinzugefügt werden (etwa 1 bis 20 Gewichtsprozent), während metallische Elemente mit niederen Atomzahlen, wie Titan und Zirkon, in höheren Prozentsätzen (etwa 20 bis 40%) hinzugefügt werden müssen. Organische röntgendichte Bestandteile mit relativ geringen Atomzahlen, wie Jod und Brom, erfordern prozentuelle Anteile von etwa 40 bis 80 Gewichtsprozent, während organische Substanzen mit höheren Atomzahlen nur 10% in dicken Markern darstellen könnten. Es ist wünschenswert, dass die Partikelgröße des röntgendichten Bestandteils geringer als 10 Mikron ist, so dass bei der Dispersion in den Körper die Partikel nicht so groß sind, dass sie eine Behinderung oder Embolisation verursachen.

8a bis8c Bezug genommen, die alternative Ausführungsformen eines Abschnitts eines bioabsorbierbaren röntgendichten Markers14 zeigen. Der bioabsorbierbare-röntgendichte Marker14 kann mindestens einen Abschnitt zur temporären Aufnahme eines röntgendichten Materials enthalten. Das röntgendichte Material kann in einem oder mehreren hohlen, hohlraumartigen oder porenförmigen Abschnitten in dem Marker14 angeordnet sein. Zum Beispiel zeigt8a einen massiven bioabsorbierbaren-röntgendichten Marker14 . Wie in8b bis8c dargestellt ist, kann der bioabsorbierbare-röntgendichte Marker14 einen röntgendichten Kern13 aufnehmen, der in dem einst hohlen15 Abschnitt angeordnet ist. Der röntgendichte Kern13 kann langsam von den offenen Enden14a ,14b des hohlen Abschnitts15 in den Körper freigesetzt werden. Als Alternative kann der röntgendichte Kern13 von dem röntgendichten Kern13 durch Poren in den Wänden des Markers14 in den Körper freigesetzt werden. -
9 ist eine Darstellung eines bioabsorbierbaren-röntgendichten Markers14 mit Poren35 . Die Poren können mit einem Reservoir von röntgendichtem Material in einem Hohlraum25 oder einem hohlen Bereich15 verbunden sein, oder die einzelnen Poren35 können mit röntgendichtem Material gefüllt sein. Die Poren35 ermöglichen, dass das röntgendichte Material in dem Marker14 angeordnet wird, um aus dem Marker14 über einen Zeitraum auszutreten. - Das röntgendichte Material kann massiv sein oder ein bioabsorbierbares Gehäuse enthalten, das eine Flüssigkeit, einen Feststoff, ein Gel, ein Pulver oder eine Kombination davon umgibt und in einem hohlen Abschnitt
15 , einem Hohlraum25 oder einem porösen Abschnitt35 durch einen relativ schwachen bioabsorbierbaren Klebstoff, bioabsorbierbare Gelatine, Reibung oder durch ein anderes mechanisches oder chemisches Mittel, das in der Technik bekannt ist, gehalten wird. Das röntgendichte Material kann so gestaltet sein, dass es von dem bioabsorbierbaren-röntgendichten Marker14 nach einer vorbestimmten Zeitperiode dispergiert wird. Das röntgendichte Material hat vorzugsweise mindestens ein Element mit einer Atomzahl von etwa 22 bis etwa 83 und ist lösbar in mindestens einem hohlen15 , hohlraumartigen25 oder porösen35 Abschnitt in dem Marker14 befestigbar. Der bioabsorbierbare-röntgendichte Marker14 kann des Weiteren eine Wand oder mehrere Wände30 umfassen, die Wände zwischen dem hohlen15 , hohlraumartigen25 oder porösen35 Abschnitt, proximale und distale Wände und Kombinationen davon umfassen, die zur in vivo Bioabsorption ausgebildet sind. - Es wird auf

Der bioabsorbierbare-röntgendichte Marker14 reagiert mit Körperfluida und zersetzt sich, und dann werden die Bestandteile absorbiert oder aus dem Körper ausgeschieden. -
11 zeigt gesonderte bioabsorbierbare-röntgendichte Marker14 , die durch die Bildung kleiner Ringe oder Spiralen von bioabsorbierbarem-röntgendichtem Filament um Elemente der implantierbaren Endoprothese16 hergestellt werden. Relativ kleine und gesonderte Filamentschlaufen (Pigtail) der bioabsorbierbaren-röntgendichten Marker14 sind an den Drahtkreuzungspunkten auf dem röhrenförmigen Geflecht dargestellt. -
12 zeigt detaillierter einen Teil der Endoprothese in11 , wobei ein bioabsorbierbarer-röntgendichter Marker14 um einen Drahtkreuzungspunkt einer implantierbaren Endoprothese16 dargestellt ist. -
13 zeigt den bioabsorbierbaren-röntgendichten Marker14 von12 und13 und zeigt Filamentenden14a ,14b , die einfach übereinander laufen, um eine geschlossene Schlaufe zu bilden, die des Weiteren an den Enden14a ,14b vorzugsweise geknotet, verdrillt oder gebunden ist. Die bioabsorbierbaren-röntgendichten Marker14 können relativ klein sein und eine einzelne Schlaufe oder ein Pigtail aus Filament um einen Filamentkreuzungspunkt, ein Filament, eine Emboliationsspirale oder dergleichen umfassen. Der bioabsorbierbare-röntgendichte Marker14 besteht vorzugsweise aus PGA, Polydioxanon, PLLA, PDLA oder Kombinationen davon. Biokompatible röntgendichte Metallbestandteile enthalten vorzugsweise Titan, Zirkon, Tantal und Platin. Bevorzugte organische röntgendichte Bestandteile enthalten Brom, Barium, Wismut, Jod oder Kombinationen davon. - Der bioabsorbierbare-röntgendichte Marker
14 ist vorzugsweise aus einem länglichen Element, wie einem Filament, gebildet und entsprechend auf der implantierbaren Endoprothese16 geformt. Der bioabsorbierbare-röntgendichte Marker14 ermöglicht in vorteilhafter Weise eine kundenspezifische Markierung der implantierbaren Endoprothese16 , ohne vorgeformte Markerbänder zu benötigen, oder einen komplizierten Herstellungsvorgang entwickeln zu müssen. Die bioabsorbierbaren-röntgendichten Marker14 können einfach und rasch zu der implantierbaren Endoprothese16 hinzugefügt werden. Ebenso werden nur kleine, spezifische Stellen durch den bioabsorbierbaren-röntgendichten Marker14 markiert, so dass eine minimale Menge an Fremdkörpermaterial der implantierbaren Endoprothese16 zugefügt wird. - Die bioabsorbierbaren-röntgendichten Marker
14 sollten vorzugsweise kleiner als die Größe des Elements in der implantierbaren Endoprothese16 sein. Ein bioabsorbierbarer-röntgendichter Marker14 geringeren Durchmessers sollte durch die meisten Webungsarten passen, verformbar sein und auf die Größe zugeschnitten werden können. - Es wird auf
12 und13 Bezug genommen, die gesonderte bioabsorbierbare-röntgendichte Marker14 zeigen, die einmal oder mehrere Male um ein Filament oder einen Filamentkreuzungspunkt geschlungen sind, um eine Lösung von diesen zu verhindern. Die Enden14a ,14b werden abgezwickt und so positioniert, dass sie in einer Ebene parallel zu der Längsachse der implantierbaren Endoprothese16 liegen. Der bioabsorbierbare-röntgendichte Marker14 kann auf einer oder mehreren Filamentkreuzungen oder jedem zweiten Filamentkreuzungspunkt um den Umfang des Geflechts in einer kreisförmigen Querebene angeordnet werden. Der bioabsorbierbare-röntgendichte Marker14 kann in Form eines oder mehrerer um den Umfang verlaufender Ringe auf der implantierbaren Endoprothese16 positioniert werden. Als Alternative kann der bioabsorbierbare-röntgendichte Marker14 entlang einer intravaskularen Spiralenvorrichtung für einen Embolisationsverschluss oder einem Filament an vorbestimmten Stellen positioniert werden. Die Enden14a ,14b können dann gebunden, verdrillt, verknotet oder zusammengeklebt werden und danach abgezwickt und so positioniert werden, dass sie in einer nicht hinderlichen Position mit kleinem Profil liegen. - Aus den vorangehenden Überlegungen geht hervor, dass der bioabsorbierbare-röntgendichte Marker
14 unter Verwendung einer Reihe von Verfahren und Materialien in einer Vielzahl verschiedener Größen und Arten für eine höhere Effizienz und Annehmlichkeit für den Benutzer gebildet werden kann. - Ein bioabsorbierbarer Marker, der in vorteilhafter Weise in Verbindung mit der vorliegenden Erfindung verwendet werden kann, ist in der US Patentanmeldung von J. Stinson und Claude Clerc mit dem Titel ”Radiopaque Markers And Methods Of Using Same”, Seriennr. 08/905,821 (nun
US Patent Nr. 6,340,367 ) offenbart. - Ein bioabsorbierbarer Stent, der in vorteilhafter Weise in Verbindung mit der vorliegenden Erfindung verwendet werden kann, ist in der US Patentanmeldung von J. Stinson mit dem Titel ”Bioabsorbable Implantable Endoprothesis With Reservoir And Method Of Using Same”, Seriennr. 08/905,806 (nun
US Patent 5,980,564 ) offenbart. - Ein anderer bioabsorbierbarer Stent, der in vorteilhafter Weise in Verbindung mit der vorliegenden Erfindung verwendet werden kann, ist in der US Patentanmeldung von J. Stinson mit dem Titel ”Bioabsorbable Self-Expanding Stent”, Seriennr. 08/904,467 (nun
US Patent 6,245,103 ) offenbart. - Die zuvor beschriebenen Ausführungsformen der Erfindung sind nur eine Beschreibung der Prinzipien und werden nicht als Einschränkung angesehen. Weitere Modifizierungen der hierin offenbarten Erfindung sind für den Fachmann in den betreffenden Wissensbereichen offenkundig, und alle diese Modifizierungen werden als im Umfang der Erfindung liegend angesehen, die durch die folgenden Ansprüche definiert ist.
Claims (24)
- Implantierbare Endoprothese und Markierungssystem umfassend: eine implantierbare Endoprothese (
16 ), ausgelegt zur Zuführung an eine Behandlungsstelle innerhalb eines Körperlumens; und eine röntgendichte Markierung (14 ), um die Röntgendichte und die Lokalisierbarkeit der Endoprothese (16 ) während einer medizinischen Prozedur zu verbessern, wobei die röntgendichte Markierung (14 ) ein proximales Ende, ein distales Ende und eine Dicke aufweist, wobei die Markierung bezüglich der Endoprothese in einer Weise gesichert ist, um die Lokalisierung der Endoprothese in vivo durch fluoroskopische Bildgebung der Markierung (14 ) zu ermöglichen, wobei die röntgendichte Markierung (14 ) ein bioabsorbierbares Material und ein röntgendichtes Material umfasst, wobei das bioabsorbierbare Material geeignet ist, durch den Körper metabolisiert zu werden, wobei die Markierung in vivo degradierbar ist, und wobei nur kleine bestimmte Stellen durch die bioabsorbierbare-röntgendichte Markierung markiert werden, sodass eine minimale Menge von Fremdkörpermaterial zu der implantierbaren Endoprothese (14 ) hinzugefügt wird. - System gemäß Anspruch 1, wobei die Endoprothese (
16 ) röhrenförmig, radial aufweitbar und axial flexibel ist, und durch eine Mehrzahl von länglichen Filamenten gebildet wird, die miteinander verwoben sind. - System gemäß Anspruch 2, wobei die röntgendichte Markierung (
14 ) ein längliches Element umfasst, dass mit den Filamenten verwoben ist. - System gemäß Anspruch 1, wobei die implantierbare Endoprothese (
16 ) bioabsorbierbar ist. - System gemäß Anspruch 1, wobei das bioabsorbierbare Material ausgewählt ist von der Gruppe bestehend aus: Poly-L-Milchsäure, Poly-D-Milchsäure, Polyglykolid, Polydioxanon, Polycaprolakton, Polyglukonat, Polymilchsaüre-Polyethylenoxid-Copolymeren, modifizierter Cellulose, Collagen, Poly(-Hydroxybutyrat), Polyanhydrid, Polyphosphorester, Poly(-aminosäuren) und Kombinationen daraus.
- System gemäß Anspruch 1, wobei die röntgendichte Markierung (
14 ) eine durchschnittliche Dicke von 20 Mikrometern bis 500 Mikrometern aufweist. - System gemäß Anspruch 1, wobei das röntgendichte Material beinhaltet: Tantal, Bismut, Platin oder Gold.
- System gemäß Anspruch 1, wobei eines der bioabsorbierbaren Materialien und das röntgendichte Material beschichtet oder vermischt ist mit dem anderen.
- System gemäß Anspruch 1, wobei die röntgendichte Markierung (
14 ) im Wesentlichen in weniger als 3 Jahren degradiert. - System gemäß Anspruch 1, wobei das röntgendichte Material im Bereich von 1 bis 80 Gewichtsprozent der Markierung liegt.
- System gemäß Anspruch 1, wobei das röntgendichte Material ausgewählt ist aus der Gruppe bestehend aus: Bariumsulfat, Bismuttrioxid, Brom, Jod, Jodid, Titanoxid, Zirkoniumoxid, Tantal und Kombinationen daraus
- System gemäß Anspruch 1, wobei das bioabsorbierbare Material ausgewählt ist aus der Gruppe bestehend aus Poly-L-Milchsäure, Poly-D-Milchsaüre, Polyglykolid und Kombinationen daraus; und das röntgendichte Material ausgewählt ist aus der Gruppe bestehend aus Bariumsulfat, Bismuttrioxid, Brom, Jod, Jodid und Kombinationen daraus.
- System gemäß Anspruch 1, wobei: das bioabsorbierbare Material aus einem Material ausgewählt aus der Gruppe bestehend aus Polyglykolid, Polyglukonat, Polydioxanon und Kombinationen daraus besteht; und das röntgendichte Material aus einem Material ausgewählt aus der Gruppe bestehend aus Bariumsulfat, Bismuttrioxid, Brom, Jod, Jodid und Kombinationen daraus besteht.
- System gemäß Anspruch 1, wobei die röntgendichte Markierung (
14 ) eine Form hat ausgewählt aus der Gruppe bestehend aus einem Mono-Filament, Multi-Filament, Faden, Streifen, Naht und Kombinationen daraus. - System gemäß Anspruch 1, wobei die Markierung (
14 ) zumindest einen Hohlraum umfasst, der das röntgendichte Material beinhaltet. - System gemäß Anspruch 1, wobei das röntgendichte Material einen linearen Dämpfungskoeffizienten von 10 cm–1 bei 50 KeV bis 120 cm–1 bei 50 KeV hat.
- System gemäß Anspruch 1, wobei das röntgendichte Material ein Element beinhaltet, das eine Ordnungszahl von 22–83 hat.
- System gemäß Anspruch 1, wobei die röntgendichte Markierung (
14 ) ein bioabsorbierbares Material und ein röntgendichtes Material, das in das bioabsorbierbare Material mit einem Gewichtsanteil W eingeschlossen ist, umfasst, länglich ist und eine durchschnittliche Dicke T (gemessen in mm) über der Länge der Markierung (14 ) aufweist; und wobei der Gewichtsanteil W gleich ist zu: (i) [10 + (950T – 208,5)] ± 5 für röntgendichtes Material mit einem Atomgewicht von 20–100; (ii) [(950T – 208,5)] ± 5 für röntgendichtes Material mit einem Atomgewicht von 100–150; oder (iii) [(950T – 208,5) – 10] ± 5 für röntgendichtes Material mit einem Atomgewicht größer als 150: wobei das minimale W ungefähr 1 beträgt und das maximale W ungefähr 80 beträgt. - System gemäß Anspruch 1, wobei die Markierung 20 Gewichtsprozent bis 99 Gewichtsprozent des bioabsorbierbaren Materials umfasst; und von 1 Gewichtsprozent bis 80 Gewichtsprozent des röntgendichten Materials, wobei das röntgendichte Material zumindest eines einer Flüssigkeit oder Partikel mit einem durchschnittlichen Partikeldurchmesser von weniger als 8 Mikrometern und einem maximalen Partikeldurchmesser von weniger als 10 Mikrometern umfasst, wobei das röntgendichte Material einen linearen Dämpfungskoeffizienten von 10 cm–1 bei 50 KeV bis 120 cm–1 bei 50 KeV hat.
- System gemäß Anspruch 1, wobei das röntgendichte Material röntgendichter ist als Knochen.
- Verfahren zum Modifizieren einer implantierbaren Endoprothese, um zeitweise eine Visualisierung der Endoprothese während und nach der Implantation dieser in ein Körperlumen zu verbessern, umfassend: Bereitstellen einer Endoprothese (
16 ), angepasst zum Zuführen in eine Behandlungsstelle in einem Körperlumen; Bereitstellen einer röntgendichten Markierung (14 ), die ein proximales Ende, ein distales Ende und eine Dicke aufweist, die ein bioabsorbierbares Material und ein röntgendichtes Material umfasst und angepasst ist, um in vivo zu degradieren; und Befestigen, vor einer Implantation der Endoprothese (16 ), der röntgendichten Markierung (14 ) bezüglich der Endoprothese (16 ) in einer Weise, die Lokalisieren der Endoprothese (16 ) in vivo durch fluoroskopische Bildgebung der Markierung (14 ) ermöglicht; wobei nur kleine bestimmte Stellen durch die bioabsorbierbare-röntgendichte Markierung markiert sind, sodass eine minimale Menge von Fremdkörpermaterial zu der implantierbaren Endoprothese (16 ) hinzugefügt wird. - Verfahren gemäß Anspruch 21, wobei das Bereitstellen der Endoprothese (
16 ) Bereitstellen einer röhrenförmigen, radial aufweitbaren und axial flexiblen Struktur umfasst, die durch eine Mehrzahl von länglichen Filamenten gebildet ist, die miteinander verwoben sind, und Bereitstellen der röntgendichten Markierung (14 ) Bereitstellen der Markierung in Form eines länglichen Elements umfasst; und das Befestigen der Markierung (14 ) zumindest eines umfasst von: Verweben der Elemente mit den Filamenten, Anordnen der Elemente entlang einer inneren Oberfläche der röhrenförmigen Struktur‚ und Abscheiden Anordnen der Elemente entlang einer äußeren Oberfläche der röhrenförmigen Struktur. - Verfahren gemäß Anspruch 22, wobei das Bereitstellen der Endoprothese (
16 ) Verweben der Filamente umfasst, um eine Vielzahl von Filamentkreuzungspunkten in der röhrenförmigen Struktur zu bilden, und das Befestigen Umschlingen der länglichen Elemente um einen der Filamentkreuzungspunkte umfasst. - Verfahren gemäß Anspruch 21, wobei das Befestigen der röntgendichten Markierung (
14 ) Anordnen der Markierung (14 ) auf der Oberfläche der Endoprothese (16 ) mit einem bioabsorbierbaren Klebstoff umfasst.
