Description
Oppfinnelsen vedrører forbindelser med formel I
hvor
R<1> er et fenyl- eller naftylradikal som hver er usubstituert eller substituert med
R<2> og/eller R<3>, eller er Het<1>,
R<2> og R<3> er hver, uavhengig av hverandre, Hal, A, OA, OH eller CN,
R<4> er H, CN, Ci.6-alkylkarbonyl, Hal, A, OA, OH, CONH2, CONHA eller
CONA2,
R<5> erH,
R<4> og R<5> er til sammen alternativt alkylen med 3-5 karbonatomer,
Het<1> er tienyl som er usubstituert, eller monosubstituert eller disubstituert med
Hal, A, OA eller OH,
A er alkyl med 1-6 karbonatomer, og
Hal er F, Cl, Br eller I,
og hvor indolringen også kan være erstattet av en isåtinenhet,
og fysiologisk akseptable salter og solvater derav,
hvor (lH-indol-5-yl)-(4-fenetylpiperazin-l-yl)metanon og l-((5-metoksy-lH-indol-7-yl)-karbonyl)-4-(2-fenetyl)-piperazin er utelukket.
Til grunn for oppfinnelsen lå den oppgave å finne frem til nye forbindelser med verdifulle egenskaper, særlig slike som kan anvendes til fremstilling av legemidler.
Det ble funnet at forbindelsene med formel I og deres fysiologisk akseptable salter og solvater har verdifulle farmakologiske egenskaper med god forenlighet ettersom de har virkninger på sentralnervesystemet. Forbindelsene oppviser en sterk affinitet for 5-HT2A-reseptorer, og dessuten utviser de 5-HT2A-reseptorantagonistiske egenskaper.
For in vitro påvisning av affiniteten til S-HT^-reseptorer kan det eksempelvis trekkes frem følgende test (eksempel Al). 5-HT2A-reseptorene eksponeres for så vel [<3>H]-ketanserin (et stoff som er kjent for sin affinitet til reseptoren) som også testforbindelsen. Reduksjonen i affinitet for [<3>H]-ketanserin til reseptoren er en indikasjon på affiniteten for teststoffet til 5-HT2A-reseptoren. Påvisningen skjer analogt med beskrivelsen til J.E. Leysen et al., Molecular Pharmacology, 1982,21: 301-314, eller som f.eks. også beskrevet i EP 0320983.
Aktiviteten av forbindelsene ifølge oppfinnelsen som 5-HT2A-reseptoranta-gonister kan måles in vitro analogt med W. Feniuk et al., Mechanisms of 5-Hydroxytryptamine-induced vasoconstriction, i: The Peripheral Actions of 5-Hydroxytryptamine, red. Fozard JR, Oxford University Press, New York, 1989, p. 110. Således formidles kontraktiliteten til rottehalearterie, fremkalt ved hjelp av 5-hydroksytryptamin, gjennom 5-HT2A-reseptorer. For dette testsystemet underkastes karringer, preparert fra den ven-trale rottehalearterie, en perfusjon i et organbad med en oksygenmettet oppløsning. Gjennom innføring av økende konsentrasjoner av 5-hydroksytryptamin i oppløsningen får man et svar på den kumulative konsentrasjon av S-HT. Deretter tilsettes testforbindelsen i egnede konsentrasjoner i organbadet og det måles en andre konsentrasjonskurve for 5-HT. Styrken til testforbindelsen på den 5-HT-induserte konsentrasjonskurve tit høyere 5-HT-konsentrasjoner er målt for den 5-HT2A-reseptorantagonistiske egenskap in vitro.
Den 5-HT2A-antagonistiske egenskap kan bestemmes in vivo analogt med M.D. Serdar et al., Psychopharmacology, 1996,128:198-205.
Andre forbindelser som likeledes utviser 5-HT2-antagonistiske virkninger, er beskrevet f.eks. i EP 0320983. Lignende piperazinderivater med antiarytmiske egenskaper er åpenbart f.eks. i EP 0431945. Andre indolkarbonylderivater med analgetiske egenskaper er beskrevet i EP 0599240.1WO 99/11641 er det beskrevet fenylindol-derivater med 5-HT2-antagonistiske egenskaper. Når det gjelder de to forbindelsene som er utelukket ved disclaimer i krav 1, så er den førstnevnte kjent fra EP 431 945 A2 som et antiarytmisk middel og den andre forbindelsen er kjent fra EP 599 240 Al som et analgetisk middel.
Forbindelsene med formel I egner seg således så vel innen veterinær- som også innen humanmedisinen til behandling av funksjonsforstyrrelser i sentralnervesystemet, samt av betennelser. De kan anvendes til profylakse og til bekjempelse av følgende cerebrale infarkthendelser (apoplexia cerebri) som slaganfall og cerebrale iskemier, samt til behandling av ekstrapyrimidal-motoriske bivirkninger av neuroleptika, samt Parkinsons sykdom, til akutt og symptomatisk terapi av Alzheimers sykdom samt til behandling av amyotrof lateral sklerose. Likeledes egner de seg som terapeutika for behandling av hjerne- og ryggmargstraumer. Særlig er de imidlertid egnet som aktive legemiddelforbindelser for anxiolytika, antidepressiva, antipsykotika, neuroleptika, antihypertonika og/eller til positiv påvirkning av tvangsnevroser (obsessiv-kompulsive forstyrrelser, ODC; f.eks. WO 9524194). Angsttilstander som fysiologiske forandringer som følger med angsttilstander, som f.eks. tachycardi, tremor eller svetting (f.eks. EP 319962), panikkanfall, psykoser, schizofreni, anoreksi, tvangsforestillinger, agorafobi, migrene, Alzheimers sykdom, søvnforstyrrelser som også søvnapne, tardive dyskinesier, lære-forstyrrelser, aldersavhengige hukommelsesforstyrrelser, spiseforstyrrelser som bulemi, rusmiddelmisbruk som f.eks. av alkohol, opiater, nikotin, psykiske stimuleringsmidler som f.eks. kokain eller amfetaminer (f.eks. US 6004980), seksualfunksjonsforstyrrelser, smertetilstander av alle typer og fibromyalgi (f.eks. WO 9946245).
Forbindelsene med formel I egner seg til behandling av ekstrapyrimidale bivirkninger (EPS) ved den neuroleptiske rusmiddelterapi. EPS er kjennetegnet ved Parkinson-lignende syndromer, akathisi og dystoniske reaksjoner (f.eks. EP 337 136). Videre er de egnet til behandling av anorexia nervosa, angina, Reynauds fenomen, koronare vasospasmer, ved profylaksen av migrene (f.eks. EP 208235), smerte og neuralgier (f.eks. EP 320983), til behandling av Rett-syndromet med autistiske karaktertegn, Asperger-syndromet, autisme og autistiske forstyrrelser, med konsentrasjonsmangeltilstander, utvik-lingsforstyrrelser, hyperaktivitetstilstander med mental underutvikling og stereotype adferdstilstander (f.eks. WO 9524194).
Dessuten er de egnet til behandling av endokrine sykdommer som hyperpro-laktinemi, videre ved vasospasmer, trombotiske sykdommer (f.eks. WO 9946245), hypertensjon og gastrointestinale sykdommer. Videre er de egnet til behandling av kardiovaskulære sykdommer samt ekstrapyrimidale symptomer som beskrevet i WO 99/11641 på side 2, linjene 24-30.
Forbindelsene ifølge oppfinnelsen egner seg videre til reduksjon av det indre trykk i øyet og til glaukombehandling. De er også egnet til profylakse og behandling av forgiftnings-fenomener ved administrering av ergovalin hos dyr. Forbindelsene egner seg dessuten til behandling av sykdommer i hjerte-kar-systemet (WO 99/11641, side 3, linjene 14-15). Forbindelsene ifølge oppfinnelsen kan også anvendes sammen med andre aktive forbindelser ved behandlingen av schizofreni. Som andre aktive forbindelser kommer de i WO 99/11641 på side 13, linjene 20-26 nevnte forbindelser på tale.
Videre kan de anvendes som mellomprodukter til fremstilling av ytterligere aktive legemiddelforbindelser.
Oppfinnelsens gjenstand er N-(indolkarbonyl)-piperazinderivatene med formel I samt deres fysiologisk akseptable syreaddisjonssalter. Oppfinnelsens gjenstand er også solvatene, f.eks. hydratene eller alkoholatene, av disse forbindelsene.
Oppfinnelsens gjenstand er således forbindelsene med formel I samt en fremgangsmåte for fremstilling av forbindelser med formel I ifølge krav 1.
Fremgangsmåten for fremstilling av forbindelser med formel I ifølge krav 1, idet (lH-indol-5-yl)-(4-fenetyl)-piperazin-l-yl)-metanonog l-((5-metoksy-lH-indol-7-yl)-karbonyl)-4-(2-fenetyl)-piperazin utelukkes, er kjennetegnet ved
a) en forbindelse med formel II
hvor L betyr Cl, Br, I eller en fri eller reaksjonsdyktig, funksjonelt omdannet OH-gruppe,
og
R4 og R<5> har de i krav 1 angitte betydninger,
omsettes med en forbindelse med formel III
hvor R<1> har den i krav 1 angitte betydning,
eller
b) en forbindelse med formel IV
hvor R4 og R<5> har de i krav 1 angitte betydninger,
omsettes med en forbindelse med formel V
hvor L betyr Cl, Br, I eller en fri eller reaksjonsdyktig, funksjonelt omdannet OH-gruppe, og R<1> har den i krav 1 angitte betydning,
eller
c) eventuelt omdannes én av restene R<1>, R<4>og/eller R<5> til en annen rest R<1>, R4 og/eller Rs, idet man f.eks. spalter en OA-gruppe under dannelse av en OH-gruppe,
og/eller omdanner en CHO-gruppe til en CN-gruppe,
og/eller omdanner en erholdt base med formel I ved behandling med en syre til et av dens salter.
Oppfinnelsens gjenstand er også forbindelsene med formel I ifølge krav 1, samt deres fysiologisk akseptable salter og solvater, idet (lH-indol-5-yl)-(4-fenetyl)-piperazin-l-yl)-metanon og l-((5-metoksy-lH-indol-7-yl)-karbonyl)-4-(2-fenetyl)-piperazin er utelukket, som legemidler.
Oppfinnelsens gjenstand er særlig forbindelsene med formel I ifølge krav 1 samt deres fysiologisk akseptable salter og solvater som legemiddel med 5-HT2A-reseptoranta-gonistisk virkning.
Oppfinnelsens gjenstand er også forbindelsene med formel I samt deres enantio-merer samt diastereomerer og deres salter.
Indolringen kan også erstattes med en isatinenhet. Isatin er en i 2- og 3-stillingen med okso substituert indol = indol-2,3-dion.
For alle restene som opptrer flere ganger, samt f.eks. A eller Hal, gjelder det at deres betydninger er uavhengige av hverandre.
Resten A betyr alkyl og har 1-6, fortrinnsvis 1,2,3 eller 4, særlig 1 eller 2, C-atomer. Alkyl betyr således særlig f.eks. metyl, dessuten etyl, n-propyl, isopropyl, n-butyl, sek-butyl eller tert-butyl, videre ogsåpentyl, 1-, 2- eller 3-metylbutyl, 1,1-, 1,2-eller 2,2-dimetylpropyl, 1-etylpropyl, heksyl, 1-, 2-, 3- eller 4-metylpentyl, 1,1-, 1,2-, 1,3-, 2,2-, 2,3-eller 3,3-dimetylbutyl, 1-eller 2-etylbutyl, 1-etyl-l-metylpropyl, l-etyl-2-metylpropyl, 1,1,2- eller 1,2,2-trimetylpropyl, videre trifluormetyl eller pentafluoretyl.
Acyl har 1-6 C-atomer og betyr f.eks. formyl, acetyl, propionyl, butyryl, videre trifluoracetyl eller pentafluorpropionyl.
Alkylen er propylen, butylen eller pentylen.
OA er fortrinnsvis metoksy, videre også etoksy, n-propoksy, isopropoksy, n-butoksy, isobutoksy, sek-butoksy eller tert-butoksy.
Hal betyr fluor, klor, brom eller jod, særlig fluor eller klor.

Oppfinnelsens gjenstand er således særlig slike forbindelser med formel I hvor minst én av de nevnte rester har en av de ovenfor angitte, foretrukne betydninger. Noen foretrukne grupper av forbindelser kan uttrykkes gjennom de følgende delformlene Ia tit li som tilsvarer formel I, og hvor de ikke nærmere angitte rester har de for formel I angitte betydninger, hvor imidlertid
og idet indolringen også kan være erstattet med en isatinenhet.
Forbindelsene med formel I og også utgangsforbindelsene for deres fremstilling fremstilles for øvrig etter i og for seg kjente fremgangsmåter, slik som de er blitt beskrevet i litteraturen (f.eks. i standardverkene som Houben-Weyl, Methoden der Organischen Chemie, Georg Thieme Verlag, Stuttgart; Organic Reactions, John Wiley & Sons, Inc., New York;), og da under reaksjonsbetingelser som er kjent og egnet for de nevnte omsetninger. Derved kan man også gjøre bruk av i og for seg kjente, her ikke nærmere beslyste varianter.
Utgangsforbindelsene for den krevde fremgangsmåte kan om ønsket også dannes in situ slik at man ikke isolerer dem fra reaksjonsblandingen, men straks omsetter dem videre til forbindelsen med formel I. På den annen side er det mulig å gjennomføre reaksjonen trinnvis.
I forbindelse med formel II og V er resten L fortrinnsvis Cl eller Br; den kan imidlertid også være I, OH eller også foretrukket en reaksjonsdyktig, funksjonell omdannet OH-gruppe, særlig alkylsulfonyloksy med 1-6 (f.eks. metansulfonyloksy) eller arylsulfonyloksy med 6-10 C-atomer (f.eks. benzensulfonyloksy, p-toluensulfonyloksy, 1- eller 2-naftalinsulfonyloksy) eller også triklormetoksy, alkoksy, som f.eks. metoksy, etoksy, propoksy eller butoksy, videre også fenoksy.
Forbindelsene med formel I kan fortrinnsvis fås idet man omsetter forbindelser med formel II med forbindelser med formel III.
Utgangsforbindelsene med formlene II og III er som regel kjent; de ikke kjente forbindelser med formlene II og III kan lett fremstilles analogt med de kjente forbindelser.
Omsetningen av forbindelsene med II og III forløper etter fremgangmåter som er kjent fra litteraturen for alkyleringen henholdsvis acyleringen av aminer. Det er imidlertid også mulig å omsette forbindelsene i nærvær av et inert oppløsningsmiddel. Som oppløs-ningsmiddel egner seg f.eks. hydrokarboner som benzen, toluen eller xylen; ketoner som aceton eller butanon; alkoholer som metanol, etanol, isopropanol eller n-butanol; etere som tetrahydrofuran (THF) eller dioksan; amider som dimetylformamid (DMF) eller n-metyl-pyrrolidon; og nitriler som acetonitril; eventuelt også blandinger av disse oppløs-ningsmidlene med hverandre eller blandinger med vann. Tilsetning av et syrebindende middel, f.eks. et alkali- eller jordalkalimetallhydroksid, -karbonat eller -bikarbonat, eller et annet salt av en svak syre og alkali- eller jordalkalimetaller, fortrinnsvis kalium, natrium etler kalsium, eller tilsetning av en organisk base som trietylamin, dimetylanilin, pyridin eller kinolin, eller et overskudd av piperazin-derivat med formel II kan være gunstig. Reaksjonstiden ligger alt etter de anvendte betingelser mellom noen minutter og 14 dager, reaksjonstemperaturen mellom ca. 0 og 150 °C, vanligvis mellom 20 og 130 °C.
Dessuten kan man fremstille forbindelser med formel I idet man omsetter aminer med formel IV med en komponent med formel V som inneholder resten R<1>. De enkelte komponenter er som regel kjent eller kan fremstilles etter kjente fremgangsmåter som allerede beskrevet.
Oppfinnelsens gjenstand er også et farmasøytisk preparat som inneholder minst ett legemiddel ifølge oppfinnelsen samt eventuelt bærer- og/eller hjelpestoffer, og eventuelt andre aktive forbindelser. Legemidlet kan være bragt sammen med minst et fast, flytende og/eller halvflytende bærer- eller hjelpestoff, og eventuelt i kombinasjon med et eller flere ytterligere aktive forbindelser, i en egnet doseringsform.
Oppfinnelsens gjenstand er dessuten anvendelsen av forbindelsene ifølge oppfinnelsen og/eller av deres fysiologisk akseptable salter og solvater til fremstilling av et legemiddel av 5-HT2A-reseptoarntagonistisk virkning.
Oppfinnelsens gjenstand er også anvendelsen av forbindelsene ifølge oppfinnelsen og/eller av deres fysiologisk akseptable salter og solvater til fremstilling av et legemiddel med 5-HT2A-reseptorantagonistisk virkning for behandling av psykoser, schizofreni, depresjon, neurologiske forstyrrelser, hukommelsesforstyrrelser, Parkinsons sykdom, amyotrof lateral sklerose, Alzheimers sykdom, Huntingtons sykdom, spiseforstyrrelser som bulemi, anorexia nervosa, premenstruelt syndrom og/eller til positiv påvirkning av tvangsnevrotisk adferd (obsessiv-kompulsiv forstyrrelse, OCD).
De farmasøytiske preparatene kan anvendes som legemidler innen human- og veterinærmedisinen. Som bærerstoffer kommer det på tale med organiske eller uorgan-iske stoffer som egner seg for enteral (f.eks. oral), parenteral eller topisk applikasjon, og som ikke reagerer med de nye forbindelsene, f.eks. vann, planteoljer, benzylalkoholer, polyetylenglykoler, gelatin, karbohydrater som laktose eller stivelse, magnesiumstearat, talkum, vaselin. For enteral applikasjon tjener særlig tabletter, drageer, kapsler, siruper, safter, dråper eller suppositorier, for parenteral applikasjon oppløsninger, fortrinnsvis olje- eller vannoppløsninger, videre suspensjoner, emulsjoner eller implantater, for topisk anvendelse salver, kremer eller pudder. De nye forbindelsene kan også Iyofiliseres og de erholdte lyofilisater anvendes f.eks. til fremstilling av injeksjonspreparater.
De angitte preparater kan være sterilisert og/eller inneholde hjelpestoffer som glide-, konserverings-, stabiliserings- og/eller fuktemidler, emulgatorer, salter for påvisning av det osmotiske trykk, bufferstoffer, farve-, smaks- og/eller aromastoffer. De kan om ønsket også inneholde én eller flere ytterligere aktive forbindelser, f.eks. ett eller flere vitaminer.
Derved administreres forbindelsene ifølge oppfinnelsen som regel analogt med kjente preparater, fortrinnsvis ved doseringer mellom ca. 0,1 og 500 mg, særlig mellom 5 og 300 mg pr. doseringsenhet. Den daglige dosering ligger fortrinnsvis mellom ca. 0,01 og 250 mg/kg, særlig mellom 0,02 og 100 mg/kg kroppsvekt.
Derved administreres forbindelsene ifølge oppfinnelsen som regel fortrinnsvis ved doseringer mellom ca. 1 og 500 mg, særlig mellom 5 og 100 mg pr. doseringsenhet. Den daglige dosering ligger fortrinnsvis mellom ca. 0,02 og 10 mg/kg kroppsvekt. Den spesielle dose for hver bestemt pasient avhenger imidlertid av de forskjelligste faktorer, f.eks. av aktiviteten til den anvendte spesielle forbindelse, av alderen, kroppsvekten, den generelle helsetilstand, arv, av kosten, av administreringstidspunktet og -veien, av ut-skillingshastigheten, legemiddelkombinasjonen og alvorligheten av den enkelte sykdom som behandlingen gjelder for. Den orale applikasjon er foretrukket.
Ovenfor og nedenunder er alle temperaturer angitt i °C. I de etterfølgende eksempler betyr "vanlig opparbeidelse": Om nødvendig fjerner man oppløsningsmidlet, om nødvendig tilsettes vann, om nødvendig innstilles, alt etter sluttproduktets konstitu-sjon, pH-verdiene mellom 2 og 10, de ekstraheres med etylacetat eller diklormetan, separeres, den organiske fase tørkes over natriumsulfat, filtreres, inndampes og renses ved kromatografi på silikagel og/eller ved krystallisasjon.
Eksempel Al
Fremstilling av en suspensjon av 5-HT2A-reseptorer:
Fremre rottehjernebark homogeniseres i iskald buffer. Homogenatet sentrifugeres i 10 minutter ved 4 °C og 50.000 x g. Pelleten oppslemmes på nytt i 2,5 ml iskald tris-buffer, fylles opp med 10 ml ytterligere buffer og sentrifugeres som beskrevet. Deretter oppslemmes pelleten på nytt i buffer og fortynnes til et homogenat som inneholder 60 mg materiale/ml.
I inkubasjonsrørene tilsettes 0,1 ml av suspensjonen, 100 ul av 5 nM oppløsning av [<3>H]-ketanserin, 100 ul av oppløsning av testforbindelsen (konsentrasjon i området fra 10'<5> til 10'10 mol pr. liter) og det fylles opp med en buffer til 1 ml. Rørene inkuberes i 15 minutter ved 37 °C. Etter avbrudd av inkubasjonen ved nedsenking av rørene i et isbad filtreres den avkjølte suspensjon gjennom et glassfilter under vakuum. Filteret vaskes tre ganger med 5 ml kald buffer og overføres så i scintillasjonsrør. Filteret analyseres ved hjelp av væskescintillasjonsspektrometri i 8 ml triton-X-scintillatorvæske.
Eksempel 1
En oppløsning av 2,0 g 4-karboksyindol og 8,1 g 2-klor-l-metyl-pyridiniumjodid i 60 ml N-metylpyrrolidon (NMP) tilsettes en oppløsning av 2,36 g 4-fenetylpiperazin og 8,2 g etyldiisopropytamin (EDIPA) i 20 ml NMP og det omrøres i 3 timer ved romtem-peratur. Det opparbeides på vanlig måte og råproduktet fås. Dette oppløses i aceton og hydrokloridet utfelles med vandig saltsyre. Etter tørking får man 4,59 g (lH-indol-4-yl)-(4-fenetylpiperazin-1 -y l)-metanon-hydroklorid

Eksempel A: Injeksjonsglass
En oppløsning av 100 g av en aktiv forbindelse med formel I og 5 g dinatrium-hydrogenfosfat i 31 dobbeltdestillert vann innstilles på pH 6,5 med 2 N saltsyre, sterilfiltreres, fylles i injeksjonsglass, lyofiliseres og lukkes sterilt. Hvert injeksjonsglass inneholder 5 mg aktiv forbindelse.
Eksempel B: Suppositorier
Man smelter en blanding av 20 g av en aktiv forbindelse med formel I sammen med 100 g soyalecithin og 1400 g kakaosmør, heller i former og lar det avkjøles. Hvert suppositorium inneholder 20 mg aktiv forbindelse.
Eksempel C: Oppløsning
Det lages en oppløsning av 1 g av en aktiv forbindelse med formel 1,9,38 g NaH2P04 x 2 H20,28,48 g NaH2P04 x 12 H20 og 0,1 g benzalkoniumklorid i 940 ml dobbeltdestillert vann. Det innstilles på pH 6,8, fylles opp til 1 liter og steriliseres ved bestråling. Denne oppløsningen kan anvendes i form av øyedråper.
Eksempel D: Salve
Man blander 500 mg av en aktiv forbindelse med formel I med 99,5 g vaselin under aseptiske betingelser.
Eksempel E: Tabletter
En blanding av 1 kg aktiv forbindelse med formel 1,4 kg laktose, 1,2 kg potetstivelse, 0,2 kg talkum og 0,1 kg magnesiumstearat presses på vanlig måte til tabletter slik at hver tablett inneholder 10 mg aktiv forbindelse.
Eksempel F: Drageer
Analogt med eksempel E presses det tabletter som deretter belegges på vanlig måte med et belegg av sakkarose, potetstivelse, talkum, tragant og fargestoff.
Eksempel G: Kapsler
2 kg aktiv forbindelse med formel I fylles på vanlig måte i hardgelatinkapsler slik at hver kapsel inneholder 20 mg av den aktive forbindelse.
Eksempel H: Ampuller
En oppløsning av 1 kg aktiv forbindelse med formel I i 60 1 dobbeltdestillert vann fylles i ampuller, lyofiliseres under aseptiske betingelser og lukkes sterilt. Hver ampulle inneholder 10 mg aktiv forbindelse.
