Abstract
Oppfinnelsen tilveiebringer antipsykotiske fremgangsmåter og preparater.
Description
Foreliggende oppfinnelse vedrører anvendelse av olanzapin i kombinasjon med fluoxetin for fremstilling av et farmasøytisk preparat og farmasøytiske preparater omfattende denne kombinasjon.
Psykoser er alvorlige mentale sykdommer karakterisert ved mangelfull eller tapt kontakt med virkelighet. Psykotiske pasienter kan også lide av hallusinasjoner og vrangforestillinger som del av sykdommer. Psykoser utløser en enorm emosjonell og økonomisk byrde på pasientene, deres familier og samfunnet som helhet. Mens mekanismene som er underliggende for disse sykdomstilfellene er lite kjente, gir terapier, oppdaget i den senere tid, nytt håp for behandling av psykotiske pasienter. Fremskritt i behandlingen av psykotiske tilstander har blitt oppnådd gjennom introduseringen av nye, atypiske antipsykotiske midler. Selv om bivirkningsprofilen til disse atypiske antipsykotiske midlene er langt overlegen i forhold til tradisjonelle midler, er vektøkning en bivirkning som har blitt observert hos pasienter behandlet med de atypiske antipsykotiske midlene.
Virkningen av olanzapin på schizofreni når administrert alene er blant annet kjent fra V. P. Bakshi, Psychopharmacology, vol. 122, nr. 2,1995, s. 198-201 og C. M. Beasley et a., Neuropsychopharmacology, vol. 14, nr. 2, 1996, s. 111-123.
Videre beskriver F. Centorrino et al., American Journal of Psychiatry, vol. 153, nr. 6, 1996, s. 820-822 virkningen av serotorin gjenopptaksinhibitoren fluoxetin på serumnivået av klozapin og norklozapin når fluoxitin og klozapin koadministreres.
Selv om disse nye midlene gir håp om betydelig forbedring av livenskvaliteten til psykotiske pasienter, er de ikke nødvendigvis gode nok til å behandle enhver psykotisk pasient. Siden psykotiske tilstander viser seg å ha en kompleks etiologi, kan noen schizofrene som viser depressive episoder under sykdomsforløpet, eller deprimerte individer som også har psykotiske trekk, ikke finne fullkommen hjelp ved anvendelse av kun ett atypisk antipsykotisk middel.
Oppfinnelsen tilveiebringer anvendelse av olanzapin i kombinasjon med fluoxetin for fremstilling av et farmasøytisk preparat for behandling av en pasient som lider av eller er mottakelig for psykose, akutt mani, milde angsttilstander eller depresjon i kombinasjon med psykotiske episoder.
Oppfinnelsen tilveiebringer også et farmasøytisk preparat kjenntegnet ved at det omfatter en første komponent som er olanzapin i kombinasjon med en andre komponent som er fluoxetin.
I foreliggende dokument, blir alle temperaturer uttrykt i grader Celsius, og alle mengder, mengdeforhold og konsentrasjoner blir uttrykt i vektdeler med mindre annet er angitt.
I de generelle uttrykkene i foreliggende oppfinnelse, er den første komponenten olanzapin som virker som et atypisk antipsykotisk middel. Det vesentlige trekket til et atypisk antipsykotisk middel er færre akutte ekstrapyramidale symptomer, spesielt dystoni, relatert til behandlingen sammenlignet med et typisk antipsykotisk middel som f.eks. haloperidol. Clozapin, det prototypiske atypiske antipsykotiske middelet, skiller seg fra de typiske antipsykotiske midlene med de følgende egenskapene: (1) større virkning i behandlingen av generell psykopatologi hos pasienter med schizofreni som ikke er mottakelige for typiske antipsykotiske midler; (2) større virkning i behandlingen av negative symptomer av schizofreni; og (3) mindre hyppige og kvantitativt mindre økning av serum prolaktinkonsentrasjoner forbundet med behandling (Beasley, et al., Neuropsychopharmacology. 14(2), 111-123, (1996)).
Olanzapin, 2-metyl-4-(4-metyl-l-piperazinyl)-10H-tieno[2,3-b][l,5] benzodiazepin er en kjent forbindelse som er nyttig i behandlingen av schizofreni, forstyrrelse av schizofreniform, akutt mani, milde angsttilstander og psykoser og blir beskrevet i U.S. patent nr. 5.229.382.
Likeledes, når oppfinnelsen betraktes i dens videste forstand, er den andre komponent-forbindelsen fluoxetin som virker som en serotonin gjenopptaksinhibitor. Målingen av en forbindelses aktivitet ved den anvendelsen er nå et standard farmakologisk analyse. Wong, et al., Neuropsychopharmacolog<y> 8,337 - 344 (1993).
Fluoxetin, N-metyl-3-(p-trifluormetylfenoksy)-3-fenylpropylamin, blir markedsført i form av hydrokloridsaltet, og som den racemiske blandingen av dens to enantiomerer. U.S. patent 4.314.081 er en tidlig referanse på forbindelsen. Robertson et al., J. Med. Chem. 31. 1412 (1988), beskrev separasjonen av R og S enantiomerene til fluoxetin og viste at deres aktivitet som serotonin opptaksinhibitorer ligner hverandre. I foreliggende beskrivelse vil ordet "fluoxetin" bli anvendt i betydning av ethvert syreaddisjonssalt eller den frie basen, og for å innbefatte enten den racemiske blandingen eller begge R og S enantiomerene;
Kombinasjonen av olanzapin og fluoxetin kan kombineres med andre atypiske antipsykotiske midler og andre serotonin gjenopptaksinhibitorer.
Det blir spesielt foretrukket at olanzapin er den Form II olanzapin polymorfe forbindelsen som har et typisk røntgenpulver-diffraksjonsmønster som beskrives ved de følgende avstandene mellom planene:

Røntgendireaksjonsmønstrene oppført heri, ble oppnådd ved anvendelse av et Siemens D5000 røntgenpulverdiffraktometer som har en kobber Ka strålingskilde med bølgelengde X = 1,541 Å.
Det blir videre foretrukket at den Form II olanzapin polymorfe forbindelsen administreres som den vesentlig rene Form II olanzapin polymorfe forbindelsen.
"Vesentlig ren" som anvendt heri, betyr at Form II er bundet med mindre enn ca. 5% av Form I, foretrukket mindre enn ca. 2% av Form I, og mer foretrukket mindre enn ca. 1% av Form I. Videre vil "vesentlig ren" Form II inneholde mindre enn ca. 0,5% av liknende stoffer, hvori "liknende stoff' betyr uønskede kjemiske urenheter eller rest-løsemiddel eller vann. Spesielt bør "vesentlig ren" Form II inneholde mindre enn ca. 0,05% acetonitril, mer foretrukket, mindre enn ca. 0,005% acetonitril. Ytterligere bør den polymorfe forbindelsen ifølge oppfinnelsen inneholde mindre enn 0,5% av bundet vann.
Den polymorfe forbindelsen som er oppnåelig ved fremgangsmåten beskrevet i patentet '382 vil bli betegnet som Form I og har hovedsakelig et typisk røntgenpulver-diffraksjonsmønster som følger, oppnådd ved anvendelse av en Siemens D5000 røntgenpulverdiffraktometer, hvori d betyr avstanden mellom planene:

Et typisk eksempel på et røntgendiffraksjonsmønster for Form I er som følger, hvori d betyr avstanden mellom planene og I/Ii betyr de typiske relative intensitetene:
Røntgenpulverdiffraksjonsmønstrene heri ble oppnådd med en kobber med bølgelengde X = 1,541 Å. Avstandene mellom planene i kolonnen merket "d" er i ångstrøm. De typiske relative intensitetene er i kolonnen merket "I/Ii".
Selv om Form II olanzapin blir foretrukket, vil det bli forstått at betegnelsen "olanzapin" som anvendt heri, omfatter alle solvater og polymorfe former med mindre annet er spesielt angitt.
Mellomprodukt 1
I en egnet trehalskolbe ble det følgende tilsatt:
Dimetylsulfoksid (analytisk): 6 volumdeler
Mellomprodukt 1: 75 g
N-Metylpiperazin (reagens): 6 ekvivalenter
Mellomprodukt 1 kan bli fremstilt idet for fagmannen kjente fremgangsmåter anvendes. Fremstillingen av mellomproduktet I blir f.eks. beskrevet i patent '382.
En nitrogen sprederlinje ble tilført under overflaten for å fjerne ammoniakk dannet under reaksjonen. Reaksjonen ble oppvarmet til 120°C og opprettholdt ved den temperaturen gjennom reaksjonsvarigheten. Reaksjonene ble fulgt ved HPLC inntil 5% av mellomproduktet 1 var uomsatt tilbake. Etter fullstendig reaksjon, fikk blandingen anledning til å avkjøle sakte til 20°C (ca. 2 timer). Reaksjonsblandingen ble deretter overført til en egnet trehalset rundbundet kolbe og et vannbad. Under omrøring ble det til denne løsningen tilsatt 10 volumdeler metanol av reagenskvalitet og reaksjonen ble omrørt ved 20°C i 30 minutter. Tre volumdeler vann ble langsomt tilsatt over ca. 30 minutter. Reaksjonsoppslemmingen ble avkjølt til 0 til 5°C og omrørt i 30 minutter. Produktet ble filtrert og den våte filterkaken ble vasket med avkjølt metanol. Den våte filterkaken ble tørket over natten under vakuum ved 45°C. Produktet ble identifisert som teknisk olanzapin.
Utbytte: 76,7%; Virkestyrke: 98,1%.
Preparat 2
Form II olanzapin polymorf forbindelse
En 270 g prøve med 2-metyl-4-(4-metyl-l-piperazinyl)-10H-tieno[2,3-b][l,5]-benzodiazepin av teknisk kvalitet ble suspendert i vannfri etylacetat (2,7 1). Blandingen ble oppvarmet til 76°C og opprettholdt ved 76°C i 30 minutter. Blanding ble avkjølt til 25°C. Det resulterende produktet ble isolert ved anvendelse av vakuumfiltrering. Produktet ble identifisert som Form n ved anvendelse av røntgenpulveranalyser. Utbytte: 197 g.
Fremgangsmåte for fremstilling av Form TJ beskrevet ovenfor, tilveiebringer et farmasøytisk glimrende produkt som har virkestyrke > 97%, sum av liknende stoffer < 0,5% og et isolert utbytte på > 73%.
Det vil av fagmannen bli forstått at de fleste eller alle forbindelsene anvendt i foreliggende oppfinnelse er i stand til å danne salt, og at de saltformene vanligvis blir anvendt i farmasøytiske midler, ofte fordi de kan krystalliseres og renses lettere enn det de frie basene kan. Anvendelsen av de farmasøytiske midlene beskrevet ovenfor som salter, blir i alle tilfeller omfattet av foreliggende beskrivelse, og blir ofte foretrukket, og de farmasøytisk egnede saltene til alle forbindelsene blir innbefattet deres betegnelser.
Mange av forbindelsene anvendt i foreliggende oppfinnelse er aminer, og reagerer følgelig med en hvilken som helst av et antall av uorganiske og organiske syrer for å danne farmasøytisk egnede syreaddisjonssalter. Siden noen av de frie aminene i forbindelsene ifølge foreliggende oppfinnelse er typiske oljer ved romtemperatur, blir det foretrukket at de frie aminene blir omdannet til deres farmasøytisk akseptable syreaddisjonssalter for å lette håndtering og administrering, siden disse ordinært er faste ved romtemperatur. Eksempler på slike farmasøytisk akseptable salter er således sulfat, pyrosulfat, bisulfat, sulfitt, bisulfitt, fosfat, monohydrogenfosfat, dihydrogenfosfat, metafosfat, pyrofosfat, klorid, bromid, jodid, acetat, propionat, dekanoat, kaprylat, akrylat, format, isobutyrat, kaproat, heptanoat, propiolat, oksalat, malonat, succinat, suberat, sebacat, fumarat, maleat, butyn-l,4-dioat, heksyn-l,6-dioat, benzoat, klorbenzoat, metylbenzoat, dinitrobenzoat, hydroksybenzoat, metoksybenzoat, ftalat, sulfonat, xylensulfonat, fenylacetat, fenylpropionat, fenylbutyrat, citrat, laktat, 6-hydroksybutyrat, glykollat, tartrat, metansulfonat, propansulfonat, naftalen-1-sulfonat, naftalen-2-sulfonat, mandelat og lignende. Foretrukne farmasøytisk akseptable salter er de dannet med hydroklorsyre, oksalsyre eller fumarsyre.
Administrering
Doseringene av legemidlene anvendt ifølge foreliggende oppfinnelse må, i de endelige analysene, fastsettes av den behandlende legen som leder saken, som anvender kunnskap om legemidlene i kombinasjon med egenskapene til legemidlene som er fastsatt ved kliniske prøver, og pasientenes særpreg, innbefattende andre sykdommer enn den som legen behandler pasienten for. Generelle rammer for doseringene, og noen foretrukne doseringer, kan og vil bli gitt her. Doseringsretningslinjer for noen av legemidlene vil først bli gitt separat; for å skape en retningslinje for enhver ønsket kombinasjon, én ville velge retningslinjene for hvert av komponentlegemidlene.
Olanzapin: fra ca. 0,25 til 60 mg, en gang/dag; foretrukket fra 1 til 30 mg, en gang/dag; og mest foretrukket 1 til 25 mg en gang/dag;
Fluoxetin: fra ca. 1 til ca. 80 mg, en gang/dag; foretrukket, fra ca. 10 til ca. 40 mg en gang/dag; foretrukket for bulimi og tvangs-kompulsiv sykdom, fra ca. 20 til ca. 80 mg en gang/dag;
I mer generelle vendinger, ville en skape en kombinasjon ifølge foreliggende oppfinnelse ved å velge en dosering av første og andre komponentforbindelser ifølge betydningen av den ovennevnte retningslinjen.
Den sammensatte terapien som anvender et preparat ifølge foreliggende oppfinnelse utføres ved administrering av en første komponent sammen med den andre komponenten på enhver måte som tilveiebringer virksomme nivåer av forbindelsene i kroppen på samme tid. Alle de gjeldende forbindelsene er oralt tilgjengelige og blir normalt administrert oralt, og således er oral administrering av den sammensatte kombinasjonen foretrukket. De kan administreres sammen, i en enkel doseringsform, eller administreres separat.
Oral administrering er imidlertid ikke den eneste administreringsveien eller enda den eneste foretrukne administreringsveien. Transdermal administrering kan f.eks. være svært ønskelig for pasienter som er glemsomme eller vrange når det gjelder å ta oral medisin. Administreirngsveien kan varieres på enhver måte, begrenset av de fysiske egenskapene til legemiddelet og komforten til pasienten og omsorgsgiveren.
Den sammensatte kombinasjonen kan administreres som en enkel farmasøytisk sammensetning, og sådan er farmasøytiske sammensetninger som innlemmer begge forbindelsene viktige utførelser av foreliggende oppfinnelse. Slike sammensetninger kan anta enhver fysisk form som er farmasøytisk egnet, men oralt anvendelige farmasøytiske sammensetninger blir fortrinnsvis foretrukket. Slike sammensatte farmasøytiske sammensetninger inneholder en virksom mengde av hver av forbindelsene, der effektiv mengde vedrører den daglige dosen av sammensetningene som skal bli administrert. Hver sammensatt doseringsenhet kan inneholde den daglige dosen av alle forbindelser, eller kan inneholde en fraksjon av de daglige dosene, som f.eks. en tredjedel av dosene. Hver doseringsenhet kan alternativt inneholde hele dosen med en av forbindelsene, og en fraksjon av dosen med de andre forbindelsene. Ved et slikt tilfelle ville pasienten daglig ta én av kombinasjonsdoseringsenhetene og én eller flere enheter inneholdende kun de andre forbindelsene. Mengdene av hvert legemiddel som hver doseringsenhet inneholder, avhenger av identiteten til legemidlene valgt for behandlingen, og andre faktorer som f.eks. indikasjonen for hvilken sammensatt behandling blir gitt.
De inerte bestanddelene og fremstillingsmåten til de sammensatte farmasøytiske sammensetningene er konvensjonelle, bortsett fra nærværet av kombinasjonen ifølge foreliggende oppfinnelse. De vanlige fremgangsmåtene for fremstilling anvendt i farmasøytisk vitenskap kan anvendes her. Alle de vanlige sammensetningstypene kan anvendes, innbefattende tabletter, tyggbare tabletter, kapsler, løsninger, parenterale løsninger, intranasale sprayer eller pulver, trokéer, stikkpiller, transdermale plaster og suspensjoner. Sammensetningene kan generelt inneholde fra ca. 0,5% til ca. 50% av de samlede forbindelsene, avhengig av de ønskede dosene og preparattypen som skal anvendes. Mengden av forbindelsene blir imidlertid best definert som den virksomme mengden, det betyr, mengden av hver forbindelse som tilveiebringer den ønskede dosen til pasienten som trenger slik behandling. Aktiviteten til de sammensatte kombinasjonene avhenger ikke av naturen til preparatet, slik blir sammensetningene valgt og formulert utelukkende for komfort og økonomi. Enhver av kombinasjonene kan fremstilles i enhver ønsket sammensetningsform. En viss redegjørelse for forskjellige sammensetninger vil bli gitt, fulgt av noen typiske formuleringer.
Kapsler blir fremstilt ved at forbindelsen blandes med et egnet fortynningsmiddel og ved at det fylles korrekt mengde av blandingen på kapsler. De vanlige fortynningsmidlene innbefatter inerte pulveriserte substanser som f.eks. stivelse av mange forskjellige slag, pulverisert cellulose, spesielt krystallinsk- og mikrokrystallinsk cellulose, sukker som f.eks. fruktose, mannitol og sukrose, mel av korn og lignende spiselige pulver.
Tabletter blir fremstilt ved direkte kompresjon, ved våt granulering eller ved tørr granulering. Formuleringene deres innlemmer vanligvis fortynningsmidler, bindemidler, smøremidler og disintegratorer såvel som forbindelsen. Typiske fortynningsmidler innbefatter f.eks. forskjellige typer stivelse, laktose, mannitol, kaolin, kalsiumfosfat eller -sulfat, uorganiske salter som f.eks. natriumklorid og pulverisert sukker. Pulveriserte cellulosederivater er også nyttige. Typiske tablett-bindemidler er substanser som f.eks. stivelse, gelatin og sukker som f.eks. laktose, fruktose, glukose og lignende. Naturlige og syntetiske gummier er også egnet, innbefattende akasie, alginater, metylcellulose, polyvinylpyrrolidin og lignende. Polyetylenglykol, etylcellulose og voks kan også fungere som bindemidler.
Ved en tablettfremstilling er et smøremiddel nødvendig for å hindre tabletten og stansene i å sitte fast i pressformen. Smøremiddelet blir valgt fra faste slippmidler som talkum, magnesium- og kalsiumstearat, stearinsyre og hydrogenerte vegetabilske oljer.
Tablettdisintegratorer er substanser som sveller når de blir fuktet for å løse opp tabletten og frigjøre forbindelsen. De innbefatter stivelser, leirer, celluloser, alginer og gummier. Mer spesielt kan f.eks. mais- og potetstivelser, metylcellulose, agar, bentonitt, trecellulose, pulverisert naturlig svamp, kation-bytterharpiks, alginsyre, guargummi, sitrusmasse og karboksymetylcellulose anvendes, såvel som natriumlaurylsulfat.
Enteriske preparater blir ofte anvendt for å beskytte en aktiv bestanddel fra det sterke syreinnholdet i magen. Slike preparater blir dannet ved å belegge en fast doseringsform med et belegg av en polymer som er uløselig i sure miljø, og løselig i basiske miljø. Belegg som celluloseacetatftalat, polyvinylacetatftalat,
hydroksypropylmetylcelluloseftalat og hydroksypropylmetylcelluloseacetatsuccinat tjener som eksempler. Det blir foretrukket å fremstille duloksetin- og duloksetin-
inneholdende kombinasjoner som enteriske preparater, og enda mer foretrukket å fremstille dem som enteriske piller.
Tabletter blir ofte belagt med sukker som et aroma- og tetningsmiddel. Forbindelsene kan også bli fremstilt som tyggbare tabletter, ved anvendelse av store mengder av behagelig-smakende substanser som f.eks. mannitol i formuleringen, som nå er veletablert praksis. Øyeblikkelig oppløselige tablett-lignende preparater blir nå også ofte anvendt for å forsikre at pasienten konsumerer doseringsformen, og for å unngå problemet som noen av pasientene har med å svelge faste objekter.
Når det ønskes å administrere kombinasjonen som en stikkpille, kan de vanlige basisene anvendes. Kakaosmør er en tradisjonell stikkpillebasis, som kan bli modifisert ved tilsetning av vokser for å heve dens smeltepunkt noe. Vannblandbare stikkpillebasiser innbefattende, spesielt polyetylenglykoler med forskjellige molekylvekter blir også utstrakt anvendt.
Transdermale plaster har i den senere tid blitt populære. Typisk omfatter de en harpiksholdig sammensetning hvori legemidlene vil løses opp, eller delvis løses opp, som holdes i kontakt med huden ved et belegg som beskytter sammensetningen. Mange patenter har i den senere tid blitt meddelt på dette området. Andre, mer kompliserte plasterpreparater anvendes også, spesielt de som har en membran gjennomboret med uttallige porer som legemidlene blir pumpet gjennom ved osmotisk virkning.
De følgende typiske preparatene/formuleringene blir tilveiebrakt for å tilfredsstille interessen og informasjonen til den farmasøytiske fagmannen.

Foreliggende oppfinnelse tilveiebringer preparater for behandling av psykotiske tilstander og mild angst med atypiske antipsykotiske midler uten den ledsagende vekt-økningen som typisk blir observert ved slik behandling, hvilket gir et marked og en uventet fordel for pasienten. Foreliggende oppfinnelse tilveiebringer videre en potensiering av konsentrasjonsøkningen av noradrenalin observert som en virkning av administrering av en første komponentforbindelse, ved administrering av en andre komponentforbindelse.
Foreliggende oppfinnelse er spesielt egnet for anvendelse i behandlingen av bipolare forstyrrelser, mani (blandet tilstand), schizoaffektive forstyrrelser karakterisert ved forekomsten av en depressiv episode i løpet av sykdomsperioden, og depresjoner med psykotiske trekk. Slike forstyrrelser kan ofte være resistente mot behandling med et anti-psykotisk middel alene.
Foreliggende oppfinnelse er også nyttig for behandlingen av premenstruelt syndrom (PMS) og anorexia nervosa. Videre er foreliggende oppfinnelse nyttig for behandling av aggresjonen/volden som kan forbindes med visse forstyrrelser. Disse forstyrrelsene innbefatter, men er ikke begrenset til, mani, schizofreni, schizoaffektive forstyrrelser, stoffmisbruk, hodeskade og mental retardasjon.
Psykotiske tilstander som kan behandles med foreliggende preparat ved adjunktiv terapi, innbefatter schizofreni, sykdommer av schizofreniform, akutt mani, schizoaffektive forstyrrelser og depresjon med psykotiske trekk. Navnene som er gitt disse tilstandene representerer mangfoldige sykdomstilstander. Den følgende fortegnelsen illustrerer et antall av disse sykdomstilstandene, mange av dem er klassifisert i "Diagnostic and Statistical Manual of Mental Disorders", 4. utgave, publisert av "American Psychiatric Association" (DSM). DSM kodenumrene til disse sykdomstilstandene blir gitt nedenfor, når tilgjengelige, til fordel for leseren.
Psykoser blir ofte forbundet med andre sykdommer og tilstander, eller forårsaket av slike andre tilstander. De blir f.eks. forbundet med neurologiske tilstander, endokrine tilstander, metabole tilstander, væske- eller elektrolyttubalanser, hepatiske- eller nyresykdommer, og autoimmune forstyrrelser med involvering av sentralt nervesystem. Psykoser kan også forbindes med bruk eller misbruk av visse stoff. Disse stoffene innbefatter, men er ikke begrenset til, kokain, metylfenidat, deksmetason, amfetamin og liknende stoff, hallucinogener, innåndende midler, opioider, fencyklidin, seditativer, hypnotika og anxiolytika. Psykotiske forstyrrelser kan også oppstå i forbindelse med avvenning fra visse stoff. Disse stoffene innbefatter, men er ikke begrenset til, seditativer, hypnotika og anxiolytika. Utførelsesformene ifølge foreliggende oppfinnelse er nyttig for behandling av psykotiske tilstander forbundet med enhver av disse tilstandene.
Mikrodialvseforsøk med monoaminer
Sprague-Dawley rotter (Harlan eller Charles River) som veier 270 - 300 gram får mikrodialyseprober kirurgisk implantert under kloralhydrat-/pentobarbital anestesi (170 og 36 mg/kg i.p. i 30% propylenglykol, 14% etanol) (Perry og Fuller, virkning av fluoxetin på serotonin og dopaminkonsentrasjon i rotte-hypothalamus etter administrering av fluoxetin pluss L-5-hydroksytryptofan, Life Sei., 50,1683 - 90
(1992)). Et David Kopf stereotaksisk instrument blir anvendt for å implantere proben unilateralt i hypothalamusen ved koordinater rostral -1,5 mm, lateral -1,3 mm og ventral -9,0 mm (Paxinos og Watson, 1986). Etter en 48 timers rekonvalisenttid blir rottene plassert i en stor plastbolle med et montert væskesvivelsystem (CMA/120 system for fritt bevegende dyr, Bioanalytical Systems, West Lafayette, IN). Filtrert kunstig cerebrospinalvæske (CSF) (150 mM NaCl, 3,0 mM KC1,1,7 mM CaC12 og 0,9 mM MgC12) blir perfundert gjennom proben ved en hastighet på 1,0 ml/min. Den utgående dialysatledningen blir tilpasset til en tiports HPLC ventil med en 20 ul sløyfe. Ved slutten av hver 30 minutters prøvetakingsperiode, blir dialysatet som er samlet i sløyfen, injisert i en analytisk kolonne (Spherisorb 3 u ODS2,2 x 150 mm, Keystone Scientific).
Fremgangsmåten anvendt for å måle monoaminer er som beskrevet av Perry og Fuller
(1992). I korthet blir dialysat samlet i den 20 ul sløyfen analysert for 5-HT, NE og DA.
20 ul injeksjonen går videre til kolonnen med en mobil fase som oppløser NE, DA og 5-HT: 75 mM kaliumacetat, 0,5 mM etylendiamintetraeddiksyre, 1,4 mM natriumoktansulfonsyre og 8% metanol, pH 4,9. Den mobile fasen til aminkolonnen blir levert med en strømningsprogrammerbar pumpe med en innledende strømningshastighet på 0,2 ml/min. som øker til 0,3 ml/min. ved 5 minutter og som deretter reduseres tilbake til 0,2 ml/min. i 26 minutter med en total driftstid på 30 min. Strømningsprogrammering blir anvendt for å eluere 5-HT innen en 25 minutters tidsperiode. Den elektrokjemiske detektoren (EG&G, Model 400) til aminkolonnen blir innstilt til et potensial på 400 mV og en sensitivitet på 0,2 nA/V. Basisnivå blir målt minst 90 minutter før legemiddeladministrering. Legemidlene blir fremstilt i filtrert avionisert vann (volum 0,25 - 0,3 ml) for administrering av de ønskede dosene.
