Description
本発明は、外科処置を実行する方法および装置に関し、詳細には、外科処置中、可視性を維持し、灌注および/または吸引をもたらす方法および装置に関する。
腹腔鏡下手術の間、1つ以上の小さな切開部が腹部に形成され、トロカールが、その切開部を通して挿入されて、腹腔へのアクセスを提供する経路を形成する。トロカールは、様々な器具および道具を腹腔内に導入するため、ならびに、器官より上に腹壁を持ち上げるよう通気(insufflation)をもたらすために、使用される。そのような処置の間、内視鏡または腹腔鏡などの観察装置(scoping device)は、トロカールのうち1つを通して挿入されて、外科医が、観察装置に連結された外部モニター上で手術野を見ることを可能にする。
本発明は、一般に、外科処置中、灌注および/または吸引を手術部位に対して実行し、かつ、観察装置のレンズを通した明瞭な可視性を維持するための方法および装置を提供する。一実施形態では、外科装置が提供され、この外科装置は、環状シースであって、体腔に配されるように、また環状シースの内腔に外科器具を受容するように構成された、環状シースと、流体導管と、を含む。シースはまた、近位開口部および遠位開口部を有する通路であって、流体をその通路に流すように構成された、通路を有する。流体導管は、シースの遠位部分に少なくとも部分的に位置しており、シースが体腔に配されているときに、通路の遠位開口部の場所を変えるように少なくとも2つの位置の間で動くことができるように構成されている。例えば、遠位開口部の第1の場所は、シースの遠位端部にあってよく、遠位開口部の第2の場所は、シースの遠位端部に対して近位であってよい。様々な外科器具、例えばスコープの遠位端部に観察要素を有するスコープ、をシースの内腔に配することができる。

環状シースであって、体腔に配されるように、また前記環状シースの内腔に外科器具を受容するように構成されており、前記シースは、流体が前記シースを通って流れることを可能にするように構成された通路をさらに有し、前記通路は、近位開口部および遠位開口部を有する、環状シースと、
前記シースの遠位部分に少なくとも部分的に位置する流体導管であって、前記シースが体腔に配されているときに、前記通路の前記遠位開口部の場所を変えるように少なくとも2つの位置の間で動くことができるように構成された、流体導管と、
を含む、装置。
(2) 実施態様1に記載の装置において、
前記遠位開口部の第1の場所が、前記シースの遠位端部にあり、前記遠位開口部の第2の場所が、前記シースの前記遠位端部に対して近位である、装置。
(3) 実施態様1に記載の装置において、
前記流体導管は、前記通路を通って流れる流体を実質的に前記内腔の方に向けるのに有効な第1の位置、および、前記通路を通って流れる流体を前記内腔から離して、かつ実質的に手術野の方に向ける第2の位置、にくるように構成される、装置。
(4) 実施態様1に記載の装置において、
前記流体導管は、前記通路を通した流体灌注および前記通路を通した流体吸引を選択的に可能にするように構成される、装置。
(5) 実施態様1に記載の装置において、
前記外科器具は、スコープであって、前記スコープの遠位端部に観察要素を有する、スコープを含む、装置。
(6) 実施態様1に記載の装置において、
少なくとも1つの位置における前記流体導管は、流体が、前記通路を通って、前記通路の長さ方向軸と実質的に整列した方向に向けられることを可能にする、装置。
(7) 実施態様1に記載の装置において、
前記流体導管は、軸方向に調節可能なアームを含む、装置。
(8) 実施態様7に記載の装置において、
前記アームは、前記アームの遠位端部が前記シースの遠位端部に対して近位となる第1の位置と、前記アームの前記遠位端部が前記シースの前記遠位端部に対して遠位となる第2の位置との間で動くことができるように構成される、装置。
(9) 実施態様7に記載の装置において、
前記シースの遠位端部において配された自己密封要素、
をさらに含み、
前記アームは、前記自己密封要素に対して近位の位置と、前記自己密封要素に対して遠位の位置との間で動くことができるように構成される、装置。
(10) 実施態様7に記載の装置において、
前記アームの近位部分に連結されたアクチュエータ、
をさらに含み、
前記アクチュエータの操作が、少なくとも2つの位置の間で前記アームを動かすのに有効である、装置。
(11) 実施態様1に記載の装置において、
前記流体導管は、前記シースの前記遠位部分において配された可動リングを含む、装置。
(12) 実施態様11に記載の装置において、
第1の回転された位置における前記リングは、流体が、前記シースの遠位端部に対して近位の前記通路の遠位開口部を通って前記シースの前記内腔に向かって流れることを可能にするように構成され、
第2の回転された位置における前記リングは、流体が、前記シースの前記遠位端部における前記通路の前記遠位開口部を通って前記シースから流れ出ることを可能にするように構成される、装置。
(13) 実施態様12に記載の装置において、
前記第1の回転された位置における前記リングは、前記シースの前記遠位端部から離れて前記通路の前記遠位開口部を通る流体流動を防ぐように構成され、
前記第2の回転された位置における前記リングは、前記シースの前記遠位端部に対して近位の前記通路の前記遠位開口部を通る流体流動を防ぐように構成される、装置。
(14) 実施態様11に記載の装置において、
前記リングは、前記シースに取り付けられた外科器具の遠位端部に対して遠位に位置するように構成される、装置。
(15) 外科装置において、
環状シースであって、前記環状シースを通って延びる第1および第2の通路を有しており、前記第1の通路は、外科器具を受容するように構成され、前記第2の通路は、流体が前記第2の通路を通って流れることを可能にするように構成されている、環状シースと、
流体導管であって、前記第2の通路の遠位開口部が、前記第1の通路に配された外科器具の遠位端部の方に実質的に向けられることを可能にする第1の位置と、前記第2の通路の前記遠位開口部が、前記シースの遠位端部から離れて、かつ体腔内へと実質的に向けられることを可能にする第2の位置との間で動くように構成された、流体導管と、
を含む、装置。
(16) 実施態様15に記載の装置において、
前記流体導管は、前記シースの遠位部分に近接して少なくとも部分的に位置している、装置。
(17) 手術方法において、
シースを体腔に通すことであって、前記シースは、前記シースの内腔に配された外科器具を有し、かつ前記シースを通って延びる流体通路を有しており、前記外科器具の遠位端部が、前記シースの遠位端部に対して近位に位置し、前記シースの遠位部分に少なくとも部分的に位置する流体導管が、前記流体通路の遠位開口部の場所を変えるため、少なくとも2つの位置の間で動くことができるように構成されている、シースを通すこと、
を含む、方法。
(18) 実施態様17に記載の方法において、
前記シースを体腔に通すことは、体腔内に延びるワーキングチャネルを有する導入器装置に前記シースを通すことを含む、方法。
(19) 実施態様17に記載の方法において、
前記流体導管は、前記シースに対して遠位の部位を選択的に灌注および吸引するのに有効な前記流体通路の遠位開口部を露出するように動くことができる、方法。
(20) 実施態様17に記載の方法において、
前記流体導管は、前記シースの前記内腔に配された外科器具の遠位端部を選択的に灌注および吸引するのに有効な前記流体通路の遠位開口部を露出するように動くことができる、方法。
Claims (8)
- 外科装置において、
環状シースであって、体腔に配されるように、また前記環状シースの内腔に外科器具を受容するように構成されており、前記環状シースは、流体が前記環状シースを通って流れることを可能にするように構成された通路をさらに有し、前記通路は、近位開口部、前記環状シースの前記内腔と連通している第1の遠位開口部、及び、該第1の遠位開口部よりも遠位側であって前記環状シースの遠位端部に位置する第2の遠位開口部を有する、環状シースと、
前記環状シースの前記通路内に少なくとも部分的に位置する流体導管であって、前記環状シースが体腔に配されているときに、前記通路を通って流れる流体を前記第1の遠位開口部を通って前記内腔に導入するのに有効な第1の位置、および、前記通路を通って流れる流体を前記内腔に導入せず、かつ前記第2の遠位開口部を通って体腔内に導入するのに有効な第2の位置の間で動くことができるように構成された、流体導管と、
を含み、
前記流体導管が前記第1の位置にあるとき、前記流体導管の遠位端は前記第1の遠位開口部よりも近位側にあり、前記流体導管が前記第2の位置にあるとき、前記流体導管の遠位端は前記第2の遠位開口部よりも遠位側にある、外科装置。 - 請求項1に記載の外科装置において、
前記流体導管は、前記通路を通した流体灌注および前記通路を通した流体吸引を選択的に可能にするように構成される、外科装置。 - 請求項1に記載の外科装置において、
前記外科器具は、スコープであって、前記スコープの遠位端部に観察要素を有する、スコープを含む、外科装置。 - 請求項1に記載の外科装置において、
少なくとも1つの位置における前記流体導管は、流体が、前記通路を通って、前記通路の長さ方向軸と実質的に整列した方向に向けられることを可能にする、外科装置。 - 請求項1に記載の外科装置において、
前記流体導管は、軸方向に調節可能なアームを含む、外科装置。 - 請求項5に記載の外科装置において、
前記アームは、前記アームの遠位端部が前記環状シースの前記遠位端部に対して近位となる第1の位置と、前記アームの前記遠位端部が前記環状シースの前記遠位端部に対して遠位となる第2の位置との間で動くことができるように構成される、外科装置。 - 請求項5に記載の外科装置において、
前記環状シースの前記遠位端部において配された自己密封要素、
