この実施例では、本開示は、IL-15 / IL-15sushiを同時発現するAPRIL CAR操作細胞を、それを必要とする患者に投与することによって、癌患者における長期耐久性寛解を提供する方法を提供する。理論に拘泥するものではないが、IL-15 / IL-15sushiとAPRIL CARポリペプチドとの同時発現は、標的癌細胞のCAR認識の感受性を増加させるか、または自然免疫細胞を漸加することによって、患者において長期耐性寛解をもたらすと考えられている。APRIL受容体、CD269(BCMA)は、形質細胞および骨髄腫細胞において弱く発現される。

別の実施例では、本発明は、癌治療における同種異系移植の前におけるコンディショニングのため、CD45を標的化する方法を提供する。 CD45は、白血球共通抗原(LCA)としても知られており、赤血球および血小板を除いて造血起源の事実上すべての細胞上で発現されるチロシンホスファターゼである。ほとんどの血液悪性腫瘍はCD45を発現する。例えば、85%〜90%の急性リンパ性白血病および骨髄性白血病は、CD45を発現する。 CD45は非造血起源には見出されない。さらに、CD45は、悪性細胞および白血球上の細胞当たり約200,000分子の平均コピー数の高密度で発現される。 CD45は、様々な血液悪性腫瘍に対する理想的な標的である。しかしながら、CAR TおよびNK細胞もCD45を発現する。内因性CD45の不活性化がなければ、CD45を標的とすとした武装したCAR TまたはNK細胞は、結果、自己殺傷を引き起こし得るかもしれない。

CARsの二重特異性に関する前臨床研究において、CD3ζ細胞内シグナル伝達ドメインは、第二世代のCARからの共刺激ドメインから離れている。換言すれば、1つのCARは、共刺激ドメインのない第1世代のCARを含み、別のCARは、CD3ゼータ細胞内ドメインを欠いている。したがって、両方の標的抗原の存在が、T細胞の活性化および強力な殺傷のために必要とされる。したがって、それらは、標的特異性を増加させるが、感度を犠牲にして、2つの標的抗原のうちの1つの健康な組織での発現によって引き起こされる腫瘍外の毒性を減少させる方法として提案された。この実施例では、compound CARは、compound CD123CD19 CARである。 B-ALLの集団のサブセットの90%以上が CD123を発現することが示されている。 AMLやMDSと同様に、B-ALLにはまれなLSC集団が存在すると考えられています。したがって、本発明に従って白血病性幹細胞および巨大な白血病集団の両方を標的とすることは、B-ALLsに適用することができる。本発明によれば、CD19がB細胞リンパ球集団の異なる段階で広く発現されるので、B-ALLにおいて発現されるCD123およびCD19表面抗原が標的であり得る。
1) CD19またはCD20またはCD22に対するscFvを有するCAR操作細胞と前記細胞を接触させることにより、成熟、記憶B細胞および短命、長命の形質細胞の枯渇
2) BCMAまたはTACIまたはBAFF-Rに対するscFvを有するCAR操作細胞と前記細胞とを接触させることによる、短命および長命の形質細胞の枯渇、または
3) BCMAまたはTACIまたはBAFF-R結合ドメイン(BAFFまたはAPRIL)を含む抗原認識ドメインを有するCAR操作細胞と前記細胞とを接触させることによる、短命および長命の形質細胞の枯渇。
4) 抗体媒介臓器拒絶または自己免疫障害を有する患者の疾患の活動の低下を提供するために、1つ以上の異なる抗原を標的とするcompound CAR操作細胞と前記細胞とを接触させることでの、成熟、記憶B細胞および短命、長命の形質細胞の欠失。
5) CD19、CD20、CD22、BCMA、TACI、APRILおよびBAFFから選択する1つ以上の異なる抗原を標的とするCAR操作細胞を接触させることによる、成熟、記憶B細胞および短命、長命の形質細胞の欠失。
1) 異なる抗原を標的とするCARの少なくとも2つの別々のコンストラクトを作製する。
2) 個々のコンストラクトをTまたはNK細胞に形質導入し、それらを標準培地中のex vivoで増殖させる。
3) 適切な比率で異なる増殖させたT細胞またはNK細胞をプールする。そして
4) プールされたCAR TまたはNK細胞を被験体に投与する。
あるいは、異なる操作された細胞を個々に増殖させ、独立してまたは連続的に投与することができる。
いくつかの実施例では、安全スイッチは、RSV F糖タンパク質A2株(NSELLSLINDMPITNDQKKLMSNN)の 254〜277残基に対応する24残基ペプチドを含むことができる。
CAR TおよびNK細胞機能を増強するIL-15およびその受容体
最近の研究により、T細胞の持続性がCAR T細胞の治療効果と良好に相関することが実証されている。最近の試験は、注入された産物の量よりむしろ品質が抗腫瘍活性に寄与する上でより重要であることを示す、注入された少量のCAR T細胞によって強力で持続性の抗腫瘍活性が生じ得ることを実証している。 インターロイキン(IL)-15は、リンパ球の発生および止血を促進するサイトカインである。 IL-15レベルの増加は、T細胞増殖を促進し、T細胞エフェクター応答を増強する。最近の研究からのデータは、IL-15が、抗腫瘍活性に関連する重要な因子の1つである記憶CD8 T細胞の産生および維持にとって重要であることを示している。 IL-15は、T細胞の生存、増殖および記憶T細胞の産生などのIL-15媒介作用に寄与するIL-15受容体アルファ鎖(IL-15RAまたはRAとも呼ばれる)に結合する。
IL-15単独のトランスフェクションはT細胞機能に有意な影響を与えないが、IL-15 / IL-15RAのトランスフェクションはT細胞が生存し、自律的に増殖することを可能にする。
サプレッサーT細胞とも呼ばれる制御性T細胞(Tregs)は、免疫系を調節し、自己抗原の耐性を維持するT細胞の亜集団である。 Tregsは、移植、アレルギー、喘息、感染症、移植片対宿主病(GVHD)、および自己免疫における多様な臨床応用を伴い、血液中の全CD4 + T細胞の約1-5%を構成する。 Tregのバイオマーカーは、CD4、Foxp3およびCD25である。 Tregは、ナイーブCD4細胞に由来すると考えられている。
Tリンパ球(T細胞)は、細胞性免疫において重要な役割を果たす白血球のサブタイプである。 T細胞はCD4およびCD8細胞に分けられる。ナチュラルキラー細胞(NK細胞)は、自然免疫に重要なタイプの細胞傷害性細胞である。
いくつかの実施例では、TまたはNK細胞は、異なるまたは同じ抗原を標的とするTreg CARを含む。
(1) 被験者またはドナーからT / NK細胞を得る。
(2) リンパ球/ T細胞またはNK細胞を培養する。
(3) 培養したT細胞またはNK細胞にTreg CARコンストラクトを導入する。
(4) Treg CAR T細胞またはNK細胞を増殖させる。
(5) 対象に治療上有効な量を投与することによって対象を治療する。
腫瘍浸潤性リンパ球(TIL)のエクスビボでの増殖は、現在の養子細胞療法でうまく使用されている。この実施例では、TILを採取し、エクスビボで首尾よく増殖させる。
操作されたcCARは、CD33またはCD123または両方を発現する細胞を標的とする
compound CAR (cCAR)の作成
CD33CD123 cCARの構造は、図1および図2Aの概略図に従う。それは、2つの異なるCARのユニットを有する機能的なcompound CAR(cCAR)の発現を誘導するSFFV(脾臓フォーカス形成ウイルス)プロモーターを含む。抗原受容体は、抗CD33および抗CD123のscFv(一本鎖可変断片)ヌクレオチド配列を頭部につける。ピコルナウイルス由来のP2Aペプチドは、バイシストロン性遺伝子構造の自己切断ダイナミクスの非常に効率的な機構のために利用される。自己切断P2Aペプチドは、 発現中における2つの独立したCARユニット、CD33CAR、およびCD123CARを 一緒に連結するのに役立つ。文献中で一般的に使用されている内部リボソーム進入部位(IRES)に対するこのアプローチの利点は、小さいサイズおよび高い切断効率もつ2Aペプチドを上流および下流の2つの単位タンパク質間にはさめることである。さらに、自己切断P2Aペプチドの使用は、IRESが適用された場合に、IRESの前後の遺伝子間の発現レベルの差の問題を回避することができる。
Compound CARレンチウイルスは 、図2に示すように大きなサイズのインサートを含むベクターのため、力価を増加させるために2倍量のベクターDNAを使用した以外は、 Lipofectamine 2000 の製造業者の指示に従った方法を用いてHEK-293FT細胞へトランスフェクションさせることで作製した。 12から16時間のインキュベーション後、Lipofectamineを含む培地を除去し、10%FBS、20mM HEPES、1mMピルビン酸ナトリウムおよび1mM酪酸ナトリウムを含むDMEM培地と交換した。約24時間後、上清を採取し、冷蔵し、新鮮な培地と交換した。さらに約24時間後、これを回収し、前の上清と合わせ、0.45μMフィルターディスクで濾過した。上清を一定分量にに分割し、液体窒素で急速凍結し、-80℃で保存した。 HEK-293 FT細胞を回収し、凍結保存し、その後の電気泳動およびウェスタンブロッティングのために溶解した(図2)。
Compound CARの 構築を確認するため、トランスフェクトされたCD33CD123 cCAR HEK293T細胞をウェスタンブロットで解析を行った。抗CD3ゼータモノクローナル抗体を用いたイムノブロットは、compund CAR CD3ゼータ融合タンパク質の予測されるサイズのバンドを示した(図2B)。重要なことに、同レベルの強度の2つの異なるバンドシグナルがブロット上で観察され、P2Aペプチドによる高い切断作用が成功した と予期 された。期待されるように、GFPコントロールベクターについては予期されたように、CD3ゼータの発現は見られなかった。 scFvの表面発現を、HEK293細胞(図2C)および初代T細胞(図2C)についてもテストした。
CD33CD123 cCAR T細胞またはGFP T細胞(コントロール)を、0.5:1から50:1から、好ましくは約2:1、5:1、10:1、20:1、50:1、約100,000、200,000、500,000、約100万、または200万個のエフェクター細胞と約50,000、100,000、200,000個の標的細胞、それぞれ)の範囲の比率で標的細胞とともにIL-2を含まない約1-2mLのT細胞培養培地中で約24時間インキュベートした。標的細胞は、白血病細胞株および白血病患者由来の白血病細胞であった。共培養の約24時間後、細胞をマウス抗ヒトCD33、CD123、CD34およびCD3抗体で染色した。
細胞株での実験に加えて、個々のCARユニットの機能を試験するために、患者の試料についての研究も実施した。侵攻性の急性骨髄性白血病(AML)であるAML-9は、CD33CD123 cCARの有効性を試験するために用いられた。 AML-9サンプル中の複数の細胞型を含む患者細胞集団の不均一性のために、白血病芽球は、2つの陽性マーカーであったCD34およびCD33でゲートされた。白血病細胞のこのCD33 + CD34 +集団の枯渇は、GFPコントロールと比較してCAR T細胞では48%であることが観察された(図6)。
ナチュラルキラー(NK)細胞はCD56+ CD3-であり、CD8+ T細胞のような感染細胞および腫瘍細胞を効率的に殺すことができる。 CD8+ T細胞とは異なり、NK細胞は、細胞を殺すために活性化を必要とせずに腫瘍に対して細胞傷害性を発揮する。 NK細胞は、サイトカインストームによる潜在的に致命的な合併症を回避できるので、より安全なエフェクター細胞である。しかしながら、CD33またはCD123または両方のCAR NK細胞を用いて、白血病を死滅させることに関しては完全に未開発である。
NK-92細胞に、CD33CD123 CARレンチウイルス上清を、2度にわたる連続したオーバーナイトでの形質導入方で形質導入を行い、方法としてはレトロネクチンおよびウイルスをコーティングしたプレートをそれぞれの形質導入時に交換した。形質導入した細胞を3または4日間増殖させ、次いでCARの発現をフローサイトメトリーによって分析した。細胞を採取し、ヤギ抗マウスF(Ab ')2と約1:250の割合で約30分間インキュベートした。細胞を洗浄し、懸濁させ、ストレプトアビジン-PEで約30分間染色した。細胞を洗浄し、2%ホルマリン中に懸濁させ、フローサイトメトリーにより分析した。次いで、CD33CD123cCARを発現するNK-92細胞を上記のように標識し、FACSAria上で選別し、F(Ab ')2でラベルされた細胞の上部0.2%を収集し、培養した。選別して培養した細胞のその後の標識では、抗マウスF(Ab ')2についてNK-92細胞は 約89%の陽性を示した(図8)。
最初に、我々は、共培養においてHL-60癌細胞株を死滅させる能力を測定することにより、CD33CD123 cCAR NK-92細胞の機能を試験した。実質的にすべてのHL-60細胞はCD33を高発現するが、この細胞株でのCD123発現は10%(弱)未満である。したがって、CD33CD123cCARの殺傷能力は、cCARがCD33を適切に標的化している能力 に依存する可能性が高い。
エンハンサーであるIL-15 / IL-15sushiもCD33CD123 CARコンストラクトに含まれていた。ompound CAR、CD33CD123およびIL-15 / IL-15sushiの両方が単一コンストラクトであった(図14)。 IL-15 / IL-15sushiは、CAR T / NK細胞の増殖を促進し、自然免疫細胞の腫瘍部位への浸潤をもたらし、腫瘍破壊をより良好に行うことができる。
CD19CD20、CD19CD22およびCD19CD138 cCARの作製
3つのcCARは、上記のCD33CD123cCARと同様の手順を用いて作製された(図15)。
compound CAR(cCAR)のコンストラクトは、図16Aの模式図に従う。これは、CARの2つの異なるユニットを有する機能性cCARの発現を誘導するSFFV(脾臓フォーカス形成ウイルス)プロモーターを含む。最初のCARは、ピコルナウイルス由来である高効率のP2A自己切断ペプチドによって完全な第2のCAR(CD20-2GまたはCD22-2Gのいずれか)に連結された、完全なL8-CD19-2G CAR(ヒトCD8aリーダー配列を使用し、L-8と呼ばれる)である。全配列はフレーム内にあり、結果として最初に細胞表面発現の半分前に切断される1つの大きな融合タンパク質である。この方法は両方のCARの発現レベルが等しいことを保証する。続いてcCAR DNA分子を上記と同じレンチウイルスプラスミドにサブクローニングした。
レンチウイルスベクター上清は、CD19CD20-2GまたはCD19CD22-2GベクターコンストラクトまたはコントロールベクターのいずれかでトランスフェクトしたHEK293T細胞から作製した。レンチウイルス上清の回収を集めた後、細胞を回収し、溶解し、ウェスタンブロット転写の為に電気泳動した。ブロット膜を抗ヒトCD3ゼータ抗体とインキュベーションすることで、切断後の各CAR単位を表す2つの異なるバンドが得られた。 CD19CARはCD20またはCD22 CARユニットよりもわずかに大きい(図18B)。次に、末梢血単核バフィーコート細胞を3日間活性化し、濃縮CD19CD20-2G、CD19CD22-2Gまたはコントロールベクターレンチウイルス上清をレトロネクチンでコーティングした非組織培養プレート上で形質導入した。形質導入処置を、最初の形質導入の24時間後に繰り返した。 T細胞表面上のCAR発現は、形質導入の3日後に示された ヤギ抗マウスF(Ab ')2抗体およびマウス抗ヒトCD3で形質導入されたT細胞を染色することで示した。
次に、cCARの最高レベルの細胞表面発現をもたらすリーダー配列を決定し、ヒトCD8a(L8)、CD45(L45)、およびコロニー刺激因子(CSF)のリーダー配列を組み込んだ3つのコンストラクトを作製した(図17 )。ヒト末梢血T細胞をこれらのベクターのそれぞれから作製したレンチウイルス上清で形質導入した後、T細胞の形質導入効率を上記のようなF(Ab ')2抗体を用いて測定した。 L8リーダー配列は再び最高の形質導入効率(43.8%)をもたらし、その後L45(9.8%)およびCSF(1.3%)が続いた。 (図17)。これは、単一CARのように、compound CARの最適な設計が、表面CAR発現のためのリーダー配列に部分的に依存することを示す。
形質導入されたT細胞のCAR効率を改善するために、CD19CD20-2GおよびCD19CD22-2Gのレンチウイルス上清を3,880×gで24時間遠心分離した。得られたウイルスペレットを、元の容量の3分の1で培地に懸濁し、3倍に濃縮した。この濃縮物を用いて、非濃縮ウイルスと同じ用量を用い活性化T細胞に形質導入した。図10aは、3倍濃縮CD19CD22-2Gレンチウイルス上清で形質導入したT細胞のCAR効率はほぼ3倍増加し、2.5x濃縮CD19CD20-2Gレンチウイルス上清で形質導入されたT細胞のCAR効率は約10倍増加した(図18)。これは、より長いcCARコンストラクトのためにレンチウイルスベクターの濃縮の重要性を示している。
CD19 +細胞系、SP53およびJeKo-1(両方のマントル細胞リンパ腫系)を効果的に溶解するL8-CD19CD22-2GおよびL8-CD19CD20-2G CAR T細胞の能力を決定するために、T細胞共培養殺傷アッセイを実施した。簡潔言えば、各標的細胞株をCMTMR膜色素で予め染色し、次いでベクターコントロール、L8-CD19CD22-2GまたはL8-CD19CD20-2G CAR T細胞のいずれかと2:1および5:1エフェクター:標的細胞の比で共培養した(血清またはIL-2を含まない1mLのT細胞培地中に200,000または500,000エフェクター細胞に対し100,000の標的細胞)。一晩インキュベートした後、細胞を抗ヒトCD3-PerCpおよびCD19-APCで30分間標識し、洗浄し、フローサイトメトリーによる分析のために2%ホルマリンに懸濁した。 L8-CD19CD22-2G CAR T細胞は、腫瘍細胞に対し強力な溶解を示し(図19)、それぞれ2:1および5:1の比でSP53細胞の53.4%および93%を溶解した。同じ比率で、L8-CD19CD22-2G CAR T細胞は、JeKo-1細胞の69%および97.3%を溶解することができた(図20)。
患者サンプルを用いて再度研究を行った。これらの初代細胞のバフィコート画分をCMTMRで予め標識し、腫瘍細胞株と同じ様式で、ベクターコントロールまたはL8-CD19CD22-2G CAR T細胞と共培養した。 L8-CD19CD22-2G CAR T細胞は、一晩の共培養において、それぞれ2:1および5:1の比でCD19の異常発現を伴うAML患者細胞の54.3%および77%を溶解し、2.5%FBSおよびIL-2が加えられた培地において4日間の共培養、1:1の比でB-ALL腫瘍細胞の84.3%を溶解した(図21,22A)。これらのAML患者細胞は65%の芽細胞しか含まず、その75%がCD19を発現したので、L8-CD19CD22-2G CAR T細胞はCD19陽性芽細胞集団全体を排除することができた可能性が高い。
野生型K562細胞への形質導入による人工的CD22発現K562細胞株(K562xp22)を作製した。続いて、K562細胞のマイナーCD22 +集団を標的とするために、CD19CD22cCARの抗腫瘍特性を試験した。 1:1の比(有効:標的)での共培養実験は、対照と比較してK562を発現するCD22集団に対して穏やかな有意な細胞毒性効果を示す。細胞傷害性の結果は、人工抗原提示細胞株に対する抗腫瘍活性に関して他に報告された数と一致している(図22B)。
多発性骨髄腫または自己免疫疾患の治療のためのBCMA CS1 cCARおよびBCMA CD19 cCARを含むcCARの作製
CD38、CS1、CD138、B細胞成熟抗原(BCMA)およびCD38を含む標的抗原に対するcCARsの前臨床研究が展開されている。 CD19 CARはまた、第I相臨床試験において多発性骨髄腫の治療に対していくつかの有効性を示した。しかし、表面抗原発現の不均一性が悪性形質細胞で一般的に生じることから、単一の標的がこの疾患を治療するのに十分であるとは考えにくい。 BCMA CS1 cCAR、BCMA CD19 cCAR、BCMA CD38 cCARおよびBCMA CD138 cCARが作製され、実験デザインは上記のCD33CD123 cCARのものと同様であった。
BCMA-CS1 cCAR(BC1cCAR)コンストラクトの作製と特性評価
本発明者らは、compound CARコンストラクトの形質導入は、一般に、単一抗原CARと比較して高い効率の遺伝子導入率を欠いていることが認められた。コンストラクトのサイズまたはエフェクター細胞への代謝ストレスまたは他の要因に起因しようが、compound CARの形質導入スキームの最適化は依然として必要である。我々は、形質導入に3つの異なるプロトコールを比較、またインキュベーションがウイルス上清内で起こるかどうか、形質導入手順の頻度、および処理当たりの最終細胞密度数を含む主要な差異を含め比較した。方法1は毎回24時間2回形質導入においてレトロネクチン被覆プレートで最初にウイルスとともにインキュベートし、吸引し、次いで0.5×10 6細胞/ mlの最終濃度までT細胞と共にインキュベートした。方法2は同じウィルスおよびレトロネクチンの手法を使用したが、最終細胞密度を0.3x10 6細胞/ mlで、第2の形質導入期間を合計48時間までの継続的なインキュベーションに変えた。方法2は、0.3×10 6細胞/ mlの細胞密度でレトロネクチン被覆プレート上でウイルス上清を細胞と48時間直接インキュベートするインキュベーションスキームを使用する(図23)。
BC1cCARのモジュールデザインは、自己切断P2Aペプチドを挟んで、抗CD269(BCMA、B細胞成熟抗原)一本鎖可変断片(scFv)領域と抗CD319(CS1)scFvと融合した 、CD8由来のヒンジ(H)および膜貫通(TM)領域、そしてタンデム4-1BB共活性化ドメイン、ならびにCD3ζシグナル伝達ドメインに連結した形を含むように設計される(図24A)。強力な脾臓フォーカス形成ウイルスプロモーター(SFFV)およびCD8リーダー配列を、BC1cCARのCAR分子のT細胞表面上での効率的な発現のために使用した。ヒト末梢血軟膜から単離したT細胞を、2日活性化させた後にBC1cCARレンチウイルスで形質導入した。上記の異なる形質導入スキームによれば、各技術について様々な形質導入効率が報告されている(図24B)。我々は、一般に、減少した細胞密度(0.3×10 6細胞/ ml)で48時間ウイルス上清と共にインキュベートした細胞が最も高い遺伝子移入効率を支持することを見出した(図24B)。したがって、我々の形質導入スキームを改善するにつれて、対応してより高い遺伝子導入率が観察される(図24C)。
BC1cCAR T-cells による細胞障害活性を評価するため、MM1S(BMCA + CS1 +)、RPMI-8226(BCMA + CS1-)およびU266(BCMA + CS1dim)らの骨髄腫細胞株と共培養アッセイを行った。標的細胞を溶解するBC1cCAR T細胞の能力は、フローサイトメトリー解析によって定量化し、標的細胞をCytotracker色素(CMTMR)で染色した。 24時間の共培養において、BC1cCARはMM1S細胞の実質的に完全な溶解を示し、2:1のE:T比で標的細胞の90%以が減少し、および5:1のE:Tでの95%以上の減少を示した(図24Aおよび図 25C)。 RPMI-8226細胞において、BC1cCARはE:T比2:1で70%以上のBCMA +標的細胞を溶解し、5:1のE:Tで75%以上を溶解した(図25Aおよび図 25C)。 U266標的細胞との24時間の共培養において、BC1cCARはBCMA+U266細胞の80%を2:1のE:T比で溶解し、飽和状態に達した(図25Bおよび図 25C)。骨髄腫細胞株はすべてほぼBCMA +であるので、これらの結果はBC1cCAR T細胞によるBCMAターゲティングの大部分が有効な細胞溶解を促進することを示唆している。
原発性腫瘍細胞に対するBC1cCART細胞を用いて共培養を行い、多様な原発性骨髄腫細胞を殺傷させる能力を評価した。MM10-G患者サンプルのフローサイトメトリー分析で、明瞭かつ一貫したBCMA +およびCS1 +集団サブセットを明らかにする。 MM7-Gサンプルは完全なBCMA + CS1 +表現型を示し、一方、MM11-Gは骨髄吸引物であるという性質に起因すると思われるノイズの多いBCMAdim CS1dimの表現型を示す。 BC1cCAR T細胞は、E:T比5:1で75%以上の溶解を示し、10:1で85%以上に増加し、MM7-Gの一次患者試料の強い用量依存的アブレーションを示す(図26A)。
その抗原標的化に関してBC1cCARの生物学的特性を評価し特徴付けるために、本発明者らは、BC1cCAR scFv機能性を独立して試験することができるモデルを確立した。骨髄腫マーカー(K562)に陰性のCML細胞株を使用することで、 K562細胞にCS1 cDNAを導入し、続いてピューロマイシン選択(図27A)により安定した発現を促進することにより、安定したCS1発現K562細胞株(CS1xpK562)を樹立した。 BCMA scFv機能性を試験するために、NIH(Kochenderfer Lab)からBCMA発現K562細胞株(BCMAxpK562)を得た。本発明者らは、各抗原発現細胞株(図27A)に対する各抗原の独立した発現を確認した後、BC1cCAR T標的機能性を決定するための共培養実験においてそれらを使用した。
次に、BC1cCAR T細胞が、細胞溶解、破片および腫瘍の再負荷によって引き起こされる好ましくない微環境下で腫瘍細胞を連続的に殺傷する能力を調べた。図28Aのスキームを使用して、MM1S細胞を骨髄腫腫瘍のモデルとして用いて長期共培養を行い、新鮮なMM1Sを有するBC1cCAR T細胞および他のCARコンストラクトを腫瘍増殖または再発をシミュレートするために定期的に再負荷した。最初の共培養条件は、1:1のE:T比で行った。外因性サイトカインが存在しない場合、標的抗原の枯渇は、48時間後の全てのCAR細胞によって達成され、有意なクラスター化およびT細胞増殖が見られる(図28B)。対照的に、コントロールT細胞は応答せず、腫瘍個体群を生じる増殖は現在はその最初の数の2倍に増殖している。新鮮なMM1S細胞を用いてすべての処理中のウェルに再負荷した後、我々は外因性サイトカインなしでも強い細胞傷害性を保持していることがわかった。 108時間までに、新たに入力されたMM1S細胞は、CS1-CARから依然として観察される有意な細胞傷害性を伴って、BCMA-CARおよびBC1cCARの両方によって実質的に枯渇した。しかし、この段階では、フローサイトメトリーはCS1-CAR集団の減少および腫瘍抗原集団の相対的増殖が約17%を示し(図28C)、これはCS1-CAR T細胞が虚脱している可能性があることを示唆している。この時点で、コントロールT細胞は腫瘍細胞によって完全に増殖している。しかし、CAR腫瘍溶解および細胞毒性はすべて168時間後に停止したが、BCMA-CARおよびBC1cCARは検出可能な少数のT細胞集団を示し、コントロールT細胞およびCS1-CAR T細胞はすべて実質的に消失した(データは示さず)。
BC1cCAR T細胞のインビボでの抗腫瘍活性を評価するために、我々は、 多発性骨髄腫細胞株であるルシフェラーゼ発現MM.1S細胞を致死量以下で照射したNSGマウスに静脈注射し、測定可能な白血病形成を誘発することができる、 異種マウスモデルを開発した。腫瘍細胞の注射3日後に、マウスに、5×106個のBC1cCAR T細胞またはコントロールGFPT細胞を単回用量で静脈注射した。 3日目、6日目、8日および11日目に、マウスにRediJect D-Luciferin(Perkin Elmer)を皮下注射し、腫瘍負荷をIVISイメージングにより測定した(図21)。BC1cCAR処置による腫瘍増殖のコントロールを決定するために、BC1cCAR T細胞注射マウスについて測定した平均光強度を、GFPコントロールマウスのそれと比較した(図29B)。Unpaired T検定は、コントロールグループと比較して、BC1cCAR T細胞の注射グループでは、6日目までに、光強度が小さく、したがって腫瘍の負荷がより少なく、2つの群の間に非常に有意な差(P <0.01)を示した(図29B)。全てのBC1cCAR T細胞注射マウスは過去50日に生存し、1/4以上は65日を超えたままであった。コントロールマウスと処置マウスとの間のP値は、Long Rank Mantel-Cox試験に基づいて0.0011である。コントロールT細胞注射マウスの生存率は、早ければ50日目までに短期間で減少し始め、55日目までに死亡した。要約すると、これらのインビボデータは、BC1cCAR T細胞が、MM1S注射NSGマウスにおける腫瘍負荷を有意に減少させ、生存を延長することを示している。
本発明者らは、安定して形質導入されたBCMAまたはCS1のいずれかを発現するルシフェラーゼ発現K562細胞を致死量以下に照射し静脈注射したNSGマウスを用いて、異種マウスモデルを計画した。 BCMAおよびCS1発現K562細胞を、ピューロマイシンセレクション後に、さらに選別し、安定な均一な単一抗原集団として樹立した。 BCMAおよびCS1を発現するK562細胞を、潜在的な抗原逃避のモデルとして、注射の前にそれぞれ4:1の比率で混合した。腫瘍細胞注射の3日後、マウスに、1コース15×10 6のコントロールT細胞、BCMA特異的CAR、またはBC1cCAR T細胞を静脈注射した。 2匹のコントロールマウスは、T細胞注入および細胞凝集などの技術的問題の結果として、注射処置の結果として死亡した。3,7,10および12日目に、マウスにRediJect D-Luciferin(Perkin Elmer)を皮下注射し、腫瘍負荷を視覚化するためにIVISイメージングで解析した(図19D)。 BC1cCAR T細胞注射マウスについて測定された平均光強度(腫瘍負荷を示す)は、治療による腫瘍増殖の制御を測定するために、BCMA特異的CARおよびGFP対照注射マウスのそれと比較された(図29D)。10日目までに、BCMA特異的CARおよびBC1cCAR T細胞の両方がコントロールと比較して47%を超える腫瘍の減少を示した。しかし、BC1cCARに有利な腫瘍負荷の減少には、マウスの背側に10日早くに6%の差があった。 12日目までに、背側のBC1cCAR(図19DおよびE)のために腫瘍減少に17%の差があった。この数は、BCMA-K562(80%)に対して注入されたCS1-K562細胞の割合(20%)に近づく。おそらく、CS1発現K562細胞が抗原逃避のモデルとして生存および増殖した結果であろう。要約すると、これらのインビボデータは、BC1cCAR T細胞が、複数の抗原集団について改善された腫瘍負荷制御を示すようであることを示している。
異なる状況におけるBC1cCARの強さをさらに評価するために、本発明者らはBC1cCARコンストラクトをNK細胞株モデルのNK-92に形質導入した。NK-92細胞への48時間に及ぶレンチウイルスインキュベーションによって、このコンストラクトは、形質導入することで成功し、遺伝子導入後の62.1%の表面発現特性をもたらした(図30Aおよび30B)。0.3〜0.5×10 6細胞/ mlの密度でのNK-92細胞のメンテナンスは、安定した集団をもたらした。インビトロでBC1cCAR抗腫瘍活性をテストするために、本発明者らは、骨髄腫細胞株および一次患者サンプルとの共培養を行った。BC1cCARは、MM1S、U266およびRPMI-8226細胞株に対して、E:T比が5:1の培養液で80%溶解に近づいた。それは、さらに主要なMM7-G腫瘍の60%以上を溶解した(図31Aおよび31B)。これらの結果は、BC1cCAR T細胞との比較可能性の点において同様である。次に、我々はBC1cCARがBCMA +またはCS1 +細胞を単独で溶解する能力におけるBC1cCARの抗原特異性を測定した。 BC1cCAR T細胞についても同様のアッセイを行った(図27)。 BCMAを発現するK562(BCMAxpK562)またはCS1を発現するK562(CS1xpK562細胞)のいずれかを用いた4時間の培養において、BC1cCAR NK細胞はいずれの集団に対しても細胞傷害性効果を有することができることを見出した(図31C)。
BCCMA-CD19 cCARまたはBCMA-CD19b cCARコンストラクトの作製および特性評価
BC1cCARのモジュールデザインは、自己切断P2Aペプチドを挟んで、抗CD269(BCMA、B細胞成熟抗原)一本鎖可変断片(scFv)領域と抗CD319 (CS1) scFvと融合した 、CD8由来のヒンジ(H)および膜貫通(TM)領域、そしてタンデム4-1BB共活性化ドメイン、ならびにCD3ζシグナル伝達ドメインに連結した形を含むように設計される(図35)。強力な脾臓フォーカス形成ウイルスプロモーター(SFFV)およびCD8リーダー配列を、BC1cCAR分子のT細胞表面上での効率的な発現のため、および上記の同様のアプローチを用いたin vitroおよびin vivoでの抗腫瘍活性のために使用した。
形質導入されたT細胞は、scFv配列に基づいて異なるレベルでCD19-2Gを発現する
CD19-2G CARの最高レベルの細胞表面発現をもたらすであろうCD19のscFv配列を決定するために、CD19-2G CAR(図39A)のデザインにおいて、CD19およびCD19bの2つの異なる配列を使用した。両方ともL8リーダー配列を使用した。ヒト末梢血T細胞を同じ条件下でこれらのベクターのそれぞれから作製したレンチウイルス上清で形質導入した後、上記のようにF(Ab ')2抗体を用いてT細胞の形質導入効率を測定した。 CD19-2Gコンストラクトは18.2%CAR細胞を生じたが、CD19b-BB-2G構築物は54.7%CAR効率をもたらした(図39b)。これは、CD19-2G CARの最適な設計もまた、使用されるscFvの配列に部分的に依存することを示す。
CD19 +細胞系、SP53およびJeKo-1(両方ともマントル細胞リンパ腫系)を効果的に溶解するCD19-2GおよびCD19b-BB-2G CAR T細胞の能力を決定するために、T細胞共培養死滅アッセイを実施した。簡潔には、各標的細胞株をCMTMR膜色素で予め標識し、次いでベクターコントロール、L8-CD19-2GまたはL8-CD19b-BB-2G CAR T細胞のいずれかと2:1および5:1エフェクター:標的細胞の比で共培養した(血清またはIL-2を含まない1mLのT細胞培地中に200,000または500,000エフェクター細胞に対し100,000の標的細胞)。一晩インキュベートした後、細胞を抗ヒトCD3-PerCpおよびCD19-APCで30分間標識し、洗浄し、フローサイトメトリーによる分析のために2%ホルマリンに懸濁した。 CD19-2GおよびCD19b 2G CAR T細胞の両方が、B細胞株SP53およびJeko-1の強力な溶解を示した(図41)。
患者サンプルを用いて研究を行った。 CD19 +細胞を有する2人の患者、すなわち、AML(CD19の異常発現)と診断された患者、およびB-ALLを有する患者がこの研究で使用された。患者の血液は、それぞれ26.4%および90%のCD19 +細胞を含んでいた(図41A、42A)。これらの初代細胞のバフィコート画分をCMTMRで予め標識し、ベクターコントロール、L8-CD19-2GまたはL8-CD19b-BB-2G T細胞のいずれかと、腫瘍細胞株と同じ方法および比率で共培養した。 L-8-CD19-2G CARおよびL-8-CD19b-2G細胞の両方は、CD19を発現する標的細胞を完全に除去することができた(図42および43)。 ウイルス力価は一般に、インサートのサイズが増加するにつれて減少し、CD19b scFvの配列はCD19b CARに対してより高い力価を提供した(図39)。したがって、CD19b scFvを用いてompound BCMA CD19b CARを作製した(図44)。 BCMA CD19b CAR。
我々は、様々なB細胞活性化因子受容体に結合するリガンドを発現するCARを設計した。腫瘍集団に結合する可能性のある潜在的な抗原またはリガンド因子についてCARを設計することは論理的な飛躍のように思われるが、CAR技術における技術的トラブルシューティングは依然として高くて永続的な障壁である。すべてのCARコンストラクトが、未定義の分子相互作用または設計上の問題の結果として、一定のあるまたは十分な表面発現を達成することはできない。我々は、CD28細胞内シグナル伝達ドメインを有するCD45リーダー配列BAFF-CARが約21%の表面発現を達成することができた(図45A)。しかし、CD8またはCSF由来の代替リーダー配列を有するBAFF-CARは、有意な発現を達成しなかった(図45B)。 CAR設計を考慮した場合、さらに別の要因が観察された。我々は、1つのケースにおいて、刺激経路として支持する4-1BBLリガンド結合ドメインを用いてBAFF-CARコンストラクトを設計した。別の方法では、コンストラクトにアームを発現するIL-15 / IL-15sushiアーマーを加えた。両方の場合において、4-1BBLまたはIL-15 / IL-15sushiと対になったCD8リーダー配列は、両方ともCSFリーダー配列よりも高い表面発現を達成した(図45C)。
我々は、BAFFがリガンド結合複合体である血漿細胞マーカーCD138の構成成分を発現している形質細胞骨髄腫細胞(MM1S)またはマントル(MCL)細胞(SP53)のいずれかを用いてそれらを培養することにより、様々なBAFF-CARの生物学的特性を特徴付けた。L45-BAFF-28 CARは48時間後にE:T比が3:1で60%に近づくとMM1S腫瘍細胞を溶解することができた(図46)。さらに、L8-BAFF-28 IL-15 / IL-15sushiおよびL8-BAFF-28 4-1BBL CARも同等の程度の細胞傷害性を達成することができた(図47A、47C)。 B細胞マントル細胞株SP53との共培養は、約25%の細胞傷害性を有する限定された効果をL8-BAFF-28 IL-15 / IL-15 CARのみについて見られた(図47)。
目的の遺伝子を標的とするために、3対のsgRNAをCHOPCHOPで設計する。次いで、遺伝子特異的sgRNAを、ヒトCas9およびE2A自己切断リンカーと連結したピューロマイシン耐性遺伝子を発現するレンチウイルスベクター(Lenti U6-sgRNA-SFFV-Cas9-puro-wpre) にクローニングする。U6-sgRNAカセットはCas9エレメントの前にある。 sgRNAおよびCas9puroの発現は、それぞれU6プロモーターおよびSFFVプロモーターによって誘導される(図48)。

レンチウイルスに保有された遺伝子特異的sgRNAを用いてNK-92細胞を形質導入した。 NK-92細胞におけるCD45発現の消失は、フローサイトメトリー解析によって判定した。 NK-92細胞のCD45陰性集団を選別し、増殖させた(図49B)。選別後に増殖させたCD45陰性NK-92細胞を用いてCD45CAR NK細胞を作製した。得られたCD45CAR NK細胞を用いて、CD45 +細胞を殺傷させる能力においてテストした。
我々は、CD45のCRISPR / Casヌクレアーゼによる不活性化後、NK45i-92細胞の細胞増殖が野生型NK-92細胞の増殖と同様であることを実証した(図26)。 CD45の不活性化は、NK-92の細胞増殖に全く有意に影響しなかった。さらに、NK45i-92細胞の溶解能は、細胞を白血病細胞であるCCRFと共培養した場合、野生型NK-92の溶解能と一致していることを示した(図51)。
我々は次に、NK45i-92細胞中のCD45CARが白血病細胞中のCD45抗原に応答することを調べる。我々はCD45CARを作製した。 CD45CARは、CD3ζシグナル伝達ドメインが連結された抗CD45一本鎖可変断片(scFv)領域、CD8由来ヒンジ(H)および膜貫通(TM)領域およびタンデムCD28および4-1BB共活性化ドメインからなる29A)。強力な脾臓フォーカス形成ウイルスプロモーター(SFFV)およびCD8リーダー配列を使用した。 CD45CARタンパク質は、CD45CARレンチウイルスプラスミドでトランスフェクトされたHEK293-FT細胞を用いたウエスタンブロットで適切なベクターコントロールと比較によって明らかにされた。さらに、抗CD3ゼータモノクローナル抗体を用いたイムノブロットにより、ベクターコントロールはバンドが見られなかったが、CD45CARタンパク質の予測されるサイズのバンドが目的のサンプルからみられた(図53B)。
蛍光活性化細胞選別(FACS)によるCD45NK45i-92細胞が多く含まれる領域を回収後、ソーティング後にけるCD45CAR NK-92導入効率は87%であるとフローサイトメトリー(図54)によって測定された。 CD45NK45i -92細胞のFACS収集後、CD45CAR発現レベルは、少なくとも10継代にわたって一貫して安定したままであった。
CD45CAR NK45i -92抗白血病活性を評価するために、T-ALL細胞株である、CCRF-CEMおよびJurkat、ならびにNK細胞株およびNK-92細胞を用いた共培養アッセイを実施した、何故ならばそれら細胞はCD45を発現しているからである(図55、56そして 57)。本発明者らは、我々はCD45CAR NK45i -92細胞が一貫して白血病細胞の強力な溶解を示したことを実証した。CD45CAR NK45i -92細胞は、標的細胞に対する低有効性(E:T比5:1)で6時間インキュベートした後、60%を超えるCCRF-CEM細胞を効果的に溶解した(図55)。 6時間の共培養後、CD45CAR NK45i -92細胞はまた、E:T、2:1または5:1の比で約60%のJurkat細胞を殺傷することができた(図56)。 6時間の共培養の後、CD45CAR NK45i-92細胞は、E:T比2:1で20%CD45陽性NK-92細胞を効率的に溶解し、E:T = 5:1で60%に近い溶解度を示した(図57A~57C)。
我々はまた、CD28を介した共刺激を受けるだけでなく、4-1BBリガンド(4-1BBLまたはCD137L)を単一のコンストラクトに共発現させて、より良い治療効果を提供する改変CD45 CAR細胞を作製した(図58A)。以下で説明する。
CD4IL-15 / IL-15sushi -CARが作製され、それはIL-15 / IL-15sushiに連結した第3世代のCD4CARを含む(図59)。 CAR、(第3世代)、sushi / IL-15の組み合わせを発現ベクター上に組み立て、それらの発現はSFFVプロモーターにより誘導される(図59)。 IL-15 / IL-15sushiを有するCARは、P2A切断配列と連動している。 IL-15 / IL-15sushi部分は、26アミノ酸のポリ - プロリンリンカーを介してIL-15susiに連結されたIL-15に融合されたIL-2シグナルペプチドからなる(図59)。 IL-2シグナルペプチドは、より良い分泌シグナルを提供する。 IL-15 / IL-15sushiの安定な機能的複合体は、形質導入された細胞から分泌され、分泌はIL-2シグナルペプチドによって命令される。
NK-92細胞に、濃縮したCD4IL-15 / IL-15sushi-CARレンチウイルス上清を形質導入した。 5日間のインキュベーション後、細胞を採取し、ヤギ抗マウスF(Ab ')2と1:250で30分間インキュベートした。細胞を洗浄し、懸濁させ、ストレプトアビジン-PEで30分間染色した。細胞を洗浄し、2%ホルマリン中に懸濁させ、フローサイトメトリーで分析し、その結果、形質導入細胞のほぼ70%近くがCD4IL-15 / IL-15sushi-CARを発現するを生じた(丸で囲まれた領域、図61。CD4IL-15 / IL-15sushi-CARのさらなる実験的試験には、インビトロおよびインビボでの白血病/リンパ腫殺傷アッセイ、ならびにCD4CARで形質導入された細胞とでの標的殺傷および増殖速度の比較検討が含まれる。CD4IL-15 / IL本発明者はまた、CD19IL-15 / IL-15sush CAR、CD20IL-15/IL-15sush CAR そして CD22IL-15/IL-15sush CARを作製するために上記の同じ方法を使用した。
ヒト臍帯バフィーコート細胞にCD4IL-15/IL-15sushi -CARレンチウイルス上清を形質導入した。 5日間のインキュベーション後、細胞を採取し、ヤギ抗マウスF(Ab ')2と1:250の比率で30分間インキュベートした。細胞を洗浄し、懸濁させ、ストレプトアビジン-PE抗体で30分間染色した。細胞を洗浄し、2%ホルマリン中に懸濁させ、フローサイトメトリーで分析し、その結果、形質導入細胞の63%がCD4IL-15/IL-15sushi -CARを発現する ことがわかった(図62の丸で囲んだ領域 )。 CD4IL-15/IL-15sushi -CARのさらなる実験的試験には、インビトロおよびインビボでの白血病/リンパ腫殺傷アッセイ、ならびにCD4CARで形質導入された細胞とでの標的殺傷能力および細胞増殖速度の比較検討が含まれる。
Karpas 299細胞株は、患者由来の未分化大型T細胞リンパ腫である。 CD4を発現するMOLT4細胞株は、19歳の患者の末梢血から樹立された急性リンパ芽球性白血病(T-ALL)である。 4時間の共培養実験の間に、エフェクター:標的の5:1比率において、CD4IL-15/IL-15sushi CAR NK NK細胞は、CD4CAR NK細胞の殺傷能力(82%、図63)よりもさらに高い割合で、Karpas 299細胞への著しい殺傷能力(95%)を示した。同様に、MOLT4細胞と1:1で共培養した場合、CD4IL-15/IL-15sushi CAR NK細胞は、オーバーナイトアッセイ(図64)においてCD4CAR NK細胞よりも高い速度(84%〜65%)で標的細胞を溶解した。これらの結果は、CD4IL-15/IL-15sushi NK細胞が、 CD4CAR NK細胞同様に腫瘍細胞をアブレートすることができることを示す。
CD4CARおよびCD4IL-15/IL-15sushi CAR T細胞のインビボでの抗腫瘍活性を評価し、 CD4IL-15/IL-15sushi CAR T細胞がCD4CAR T細胞に対して、より持続性が増加する可能性を検討するため、本発明者らは、致死以下に照射したNSGマウスに 100%CD4である急性骨髄性白血病細胞株(M5)であるルシフェラーゼ発現MOLM13細胞を静脈注射することで測定可能な腫瘍形成を可能とする異種マウスモデルを開発した(図65 )。腫瘍細胞注射の3日後、6匹のマウスに、8×10 6個のCD4CAR、CD4IL-15/IL-15sushi CAR T細胞またはベクターコントロールT細胞を1コースで静脈注射した。 3、6、9および11日目に、マウスにRediJect D-Luciferin(Perkin Elmer)を皮下注射し、腫瘍量を IVISイメージングにて測定した(図65B)。処置マウスとコントロールマウスを比較した時における腫瘍細胞のパーセンテージを決定するために、CD4CARおよびCD4IL-15 / IL-15sushi CAR T細胞注入マウスについて測定した平均光強度を、ベクターコントロールT細胞注入マウスと比較した(図65C)。6日目において、CD4CAR T細胞での治療マウスは、コントロールと比較して腫瘍量が52%低かったが、一方、CD4IL-15/IL-15sushi CAR T細胞での治療マウスは腫瘍負荷量が74%低かった。11日目に、ほとんどすべての腫瘍細胞がこれらのグループの両方において溶解された。Unpaired T検定では、コントロールと比較して、CD4CARおよびCD4IL-15/IL-15sushi CAR T細胞での治療グループは、9日目までで光強度が弱く、より少ない腫瘍量であり、非常に有意な統計差(P = 0.0045)を示した。要約すると、これらのインビボデータは、CD4CARおよびCD4IL-15 / IL-15sushi CAR T細胞が、ベクターコントロールT細胞と比較して、腫瘍負荷およびMOLM13を注入したNSGマウスを有意に減少させることを示している。
CD4IL-15 / IL-15sushiCARの機能をさらに評価するために、我々は、NK CAR細胞およびJurkat腫瘍細胞を利用してストレス状態を作り出した。 NK細胞は短い半減期特性を有し、白血病Jurkat細胞は60%未満のCD4 +表現型を示す(図66A)。このような条件下で、分泌性可溶IL-15sushiがその持続性および殺傷能力に関してどのようにCAR機能に影響を及ぼすかを調べることができる。次に、致死量以下に照射したNSGマウスにルシフェラーゼ発現Jurkat細胞を静脈注射して、測定可能な腫瘍形成を誘導する、異種NSGマウスモデルを使用した。 MOLM-13細胞とは対照的に、Jurkat細胞は60%未満のCD4 +表現型を示す(図66A5)。Jurkat細胞注射の3日後に、マウスに、CD4CAR、CD4IL-15 / IL-15sushi、またはベクターコントロールNK細胞のいずれか10x10 6の1コースで静脈注射した。 3日目(処置前日)、7、10および14日目に、腫瘍負荷を測定するためにマウスにIVISイメージングを行った(図66B)。 CD4CARおよびCD4IL-15 / IL-15sushi NK注射マウスについて測定された平均光強度を、ベクターコントロールNK注射マウスのそれと比較して、Jurkat細胞の溶解率を決定した(図66C)。両方の症状は、7日目までに有意な腫瘍細胞溶解を示したが、CD4CAR NK細胞の溶解率は、14日目と同じままであったが、CD4IL-15 / IL-15sushi NK細胞は97%以上に増加した。 (図66D)。Unpaired T検定は、14日目までに2つのグループ間に非常に有意な差(P <0.0001)を示した。これらの結果は、Jurkat腫瘍細胞のCD4CAR NKでの細胞溶解がCD4 - Jurkat細胞の増殖に追いつくことができなかったことを示し、IL-15 / IL-15sushiを分泌するNK CAR細胞の継続的な増殖が効果的に溶解した。分泌型IL-15 / IL-15sushiとCARとの同時発現は、CAR TまたはNK細胞がdim発現癌細胞または非標的癌細胞を排除できないという欠点を補うことができる。実験の繰り返し(図67)は、図66に記載したものと同様の結果を示した。
IL-15 / IL-15sushi分泌NK細胞の細胞生存への効果を測定した。 CD4CARまたはCD4IL-15 / IL-15sushiのいずれかで安定に形質導入されたNK-92細胞をIL-2の存在下または非存在下で培養し、IL-15 / IL-15sushi分泌のみが生存および増殖につながるかどうかを調べた。IL-2なしで培養したCD4CAR発現NK細胞は7日目までに死滅したが、IL-2を添加しないで培養したCD4IL-15 / IL-15sushi発現NK細胞は、IL-2を添加し培養したCD4CARまたはCD4IL-15 / IL-15sushi細胞のいずれかとほぼ同じ速度で増殖した図68B)、これは分泌されたIL-15 / IL-15sushiがIL-2に代わることができることを示す。さらに、我々は、IL-15 / IL-15sushiを分泌するNK細胞が、共培養中の非形質導入NK-92細胞の生存および増殖を助けることができることを実証することができた。この実験では、等しい割合のNK GFP発現細胞を、IL-2の存在下または非存在下で、CD4CARまたはCD4IL-15 / IL-15を発現するNK細胞のいずれかとともに培養した。細胞を2〜3日ごとに数えた(図68A)。7日目までに、IL-2を与えられていないCD4CAR NK細胞は死滅したが、IL-2を含まないCD4IL-15 / IL-15sushi NK細胞はIL-2と培養したCD4CARまたはCD4IL-15 / IL-15sushi細胞とほぼ同じ速度で生存し、 増殖した。GFP発現細胞の数がCD4IL-15 / IL-15sushi NK細胞と伴い増殖した(図68B)、このことは分泌されたIL-15 / IL-15sushiがGFP NK細胞の生存にポシティブな影響を及ぼしたことを示す。 GFP陽性細胞のパーセンテージは、実験の過程にわたって50%から70%を超えて上昇した(データは示さず)。第2の実験(図69)では、GFP NKおよび対照ベクターNKまたはCD4IL-15/IL-15sushi NK細胞のいずれかを10対1の比率で、IL-2を用いずに混合した。 6日目までに、対照細胞と共培養した細胞はすべて死滅したが、IL-15/IL-15sushiを分泌するNK細胞で培養した細胞の生存は10日まで生存した。
我々はまた、IL-2の存在下または非存在下で、CD4CARおよびCD4IL-15 / IL-15sushi形質導入T細胞の細胞増殖を比較した。 IL-2の存在下または非存在下で、形質導入された細胞を実験を通して総細胞数が計算された(17日目まで)。 CD4IL-15 / IL-15sushi形質導入T細胞は、CD4CAR形質導入T細胞よりもIL-2の不存在に対してより耐性であるようであった。
Treg CAR標的Treg細胞の作製
Treg CAR(CD4zetaCD25CARまたはC4-25zとも呼ばれる)は、図70の模式図に従った。これは、P2A切断ペプチドによって連結された2つの異なる不完全CARのの発現を誘導するするSFFV(脾臓フォーカス形成ウイルス)プロモーターを含む。 CD4キメラ抗原レセプターポリペプチド単位は、CD45シグナルペプチド、CD4抗原認識ドメイン、ヒンジ領域(誘導CD8a)、膜貫通ドメイン(CD8a)およびCD3ゼータ鎖を含み、 CD25キメラ抗原レセプターポリペプチド単位は、CD45シグナルペプチド、CD25抗原認識ドメイン、ヒンジ領域(CD8a)、膜貫通ドメイン(CD8a)、共刺激ドメイン、CD28を含む。 Treg CARは、CD4またはCD25抗原のみを有する細胞を最小限に抑えながら、CD4およびCD25を同時発現する細胞を溶解する活性を増強することができる。
操作されたCD4ゼータCD25CAR細胞は、本開示に従って調製された。 細胞殺傷アッセイを行われる。 4-1BBLまたはIL-15 / IL-15sushiまたはIL-15 / IL-15RAと共発現すると、CD4zetaCD25CARによる標的細胞殺傷が改善される。
「安全スイッチ」の導入は安全性の側面を大幅に向上させ、「安全スイッチ」は、限定するものではないが、カスパーゼ9遺伝子、チミジンキナーゼ、シトシンデアミナーゼ(CD)またはシトクロムP450のような誘導性自殺遺伝子であり得る。
T細胞悪性腫瘍に対するCAR T細胞を用いた臨床治療では、腫瘍枯渇後またはCAR療法による予期せぬ副作用のために、緊急的にCAR T細胞を患者から排除する安全方法を確立する必要がある。 T細胞およびB細胞は、細胞表面上でCD52を発現し、CD52特異的抗体であるCAMPATH(アレムツズマブ)は、CD52 +細胞を循環系から排除することができる。したがって我々は、CD5CARベクターコンストラクトにヒトCD52配列を組み込んだ(図72A)。この追加のCD52コンストラクトは、ネイティブのCD52からのシグナル伝達と機械的に隔てている。その目的は、CAMPATH処置後にCAR T細胞表面上のネイティブのCD52抗原逃避の可能性を先取りすることである。 CD5CAR-CD52レンチウイルスタンパク質および発現を、形質導入されたHEK293細胞上のCD52抗体を用いたウエスタンブロットおよびフローサイトメトリー分析によって確認した。我々はまた、CD52を共発現することがCAR T細胞機能に影響しないことも見出した。
CAMPATH(アレムツズマブ)処置によるCAR除去の効果を評価するために、我々は、記載されているインビボ処置を行った(図72Bと72C)。 5×106個のCD5CAR-52T細胞を照射マウスに静脈注射した。翌日、各グループ、3匹のマウスに対して、0.1mg / kgのCAMPATHまたはPBSをIP注射により注入した。 CAMPATH処理の6時間後および24時間後、我々はマウス尾から末梢血を採取し、FACS分析によりCD5CAR-52T細胞の存在を判定した。 CAMPATH注入は、6時間および24時間の両方で、血液中のCD5CAR-CD52 T細胞を実質的に完全に枯渇させる(図72C)。 CAMPATH投与の5日後、CD5CAR-CD52細胞は、骨髄および脾臓の両方においても完全に枯渇させた(図72D)。これらの知見は、循環およびリンパ器官からCAR-T細胞を枯渇させる安全トリガーとして作用する有用な戦略としてのCAMPATHの使用を支持する。
HEK293FT細胞に、SFFV、EF1またはCAGプロモーターの下でGFPを発現するレンチウイルスプラスミドでトランスフェクトした。トランスフェクションの約60時間後、上清をそれぞれから回収した。相対的ウイルス力価は、最初に、3つのプロモーターのそれぞれからの上清をHEK293細胞に導入することによって決定した。HEK-293FT細胞から得られた、それぞれ3つのGFPウイルス上清を用いてHEK-293細胞に形質導入した。ポリブレンを4μL/ mLまで添加した。 16時間後に培地を交換し、ウイルス上清またはポリブレンを含まない培地と交換した。形質導入の3日後、細胞を回収、洗浄し、2%ホルマリン中に懸濁し、GFP発現(FITC)の状態をフローサイトメトリーにより分析した。 GFP発現は各試料で見られたが、SFFVプロモーターを用いて作製したウイルスで形質導入した細胞で最も高かった。
各上清中のウイルスベクター粒子の機能的力価を決定するために、HEK293細胞に、12ウェル組織培養処理プレートの1ウェルあたり30μL(低)、125μL(中)または500μL(高)のいずれかの量で、EF1-GFPまたはSFFV-GFPウイルス上清のいずれかで形質導入した。翌朝、培地を10%FBSを含む DMEMに変更した(図73)。
初代T細胞におけるプロモーター形質導入効率および表面発現の持続性を決定するために、活性化された臍帯血バフィーコートT細胞を、レトロネクチン(Clontech)でプレコートした12ウェルの組織培養プレート中で50μLのSFFV-GFPまたは1mLのEF1-GFP EF1-GFPウイルス上清のいずれかを用い形質導入した。 2度にわたるオーバーナイトでの形質導入の後、細胞を300 IU / mLのIL-2(Peprotech)を含むT細胞培養培地で培養し、1.0〜4.0×10 6 / mLの細胞密度を維持した。細胞を洗浄し、ホルマリン中に懸濁させ、形質導入後7、14、21および28日におけるGFP +細胞の割合をFITCチャンネルを用いてフローサイトメトリー分析を行うことで決定した。 GFP +細胞のパーセンテージは、SFFV-GFPで形質導入されたT細胞の方がEF1-GFP形質導入されたT細胞(図75A)と比較して一貫して高かった。さらなる比較により、より多い量(1mL)のEF1-GFP上清で形質導入されたT細胞が、より少ない量(50μLまたは20倍少ない)のSFFV-GFP上清で形質導入された細胞よりも、7日目と28日目の間で、GFP+の細胞の割合が60%超から40%未満まで減少した(図75B)。このことは、SFFVプロモーターを用いた形質導入が、形質導入された細胞の形質発現のより良い持続性を維持することを示唆している。

- 請求項27-41において記載の操作細胞を、それを必要とする患者に投与する、第1の抗原認識ドメインの標的がCD19を含み、第2の抗原認識ドメインの標的はCD123を含む、B細胞急性リンパ芽球性白血病(B-ALL)を治療する方法。
- 請求項27-42のいずれか一項において記載の操作細胞を、それを必要とする患者に投与する、ここにおいて第1の抗原認識ドメインの標的がCD19、 CS1、BCMA そして CD38のグループから選ばれ、第2の抗原認識ドメインの標的はCS1、 BCMA そして CD38のグループから選ばれることを含む、多発性骨髄腫を治療する方法。
- 請求項27-41のいずれか一項においての記載の方法にて、それを必要とする患者に、第1の抗原認識ドメインの標的がBCMA、TAC1、CS1 、およびBAFF-Rの中から選ばれ、第2の抗原認識ドメインの標的は、BCMA、TAC1、CS1、およびBAFF-Rからなるグループから選択される。細胞増殖性疾患を治療する方法。

- 細胞増殖性疾患を治療する方法であって、それを必要とする患者に、インターロイキン6受容体、NY-ESO-1、アルファフェトプロテイン(AFP)、グリピカン-3(GPC3)、BCMA、BAFF-R、TACI、LeY、CD5、CD13、CD14、CD15、CD45、CD19、CD20、CD22、CD33 、CD41、CD61、CD64、CD68、CD117、CD123、CD138、CD30、CD269、CD38、Flt3受容体およびand CS1、およびCAR増強剤を含む、これらグループから選択される標的に対して選択的な抗原認識ドメインを有するキメラ抗原レセプターポリペプチドを含む操作された細胞を投与することを含む方法。
- ここにおいて前述のCAR増強剤が、免疫チェックポイント経路を標的とする薬剤、コロニー刺激因子-1受容体(CSF1R)、PD-1、PD-L1、IL-2、IL-12、IL-15、CSF1R、CTAL-4、TIM-3、およびTGFRベータからなるグループより選択される、請求項71に記載の細胞増殖性疾患の治療方法。
- 請求項39-49のいずれか一項において記載の操作されたポリペプチドを含む操作された細胞を、それを必要とする患者に投与することを含む、細胞増殖性疾患を治療する方法。
- 操作されたキメラ抗原レセプターポリペプチドであって、シグナルペプチド、CD45抗原認識ドメイン、ヒンジ領域、膜貫通ドメイン、少なくとも1つの共刺激ドメイン、およびシグナル伝達ドメインを含むポリペプチド。

- 請求項74-80のいずれか一項において記載のポリペプチドを含む操作された細胞。
- ここにおいて細胞がT細胞またはナチュラルキラー細胞である、請求項83-84のいずれか一項において記載の操作された細胞。
- ここにおいてT細胞がCD4 T細胞またはCD8 T細胞である、請求項85に記載の操作された細胞。
- ここにおいてナチュラルキラー細胞がNKT細胞またはNK-92細胞である、請求項59に記載の操作された細胞。
- 標的細胞の数を減少させる方法であって、以下の方法に含まれる、
i. 請求項27-40、55-60および83-87のいずれか一項において記載の、有効量の操作細胞と前述での標的細胞を接触させること、そして
ii. 場合により、前述の細胞の数の減少を測定すること、標的細胞が、インターロイキン6受容体、NY-ESO-1、アルファフェトプロテイン(AFP)、グリピカン-3(GPC3)、BCMA、BAFF-R、TACI、LeY、 CD5、 CD13、CD14、CD15、 CD19、 CD20、CD22、 CD33、CD41、CD45、CD61、 CD64、 CD68、CD117、CD123、CD138、CD267、CD269、CD38、Flt3レセプター、およびCS1からなるグループから選択される少なくとも1つの細胞表面抗原を含む、操作された細胞。 - ここにおいて第1の抗原認識ドメインの標的がBCMAを含み、第2の抗原認識ドメインはCS1を含む、請求項27-32のいずれか一項において記載の操作された細胞。
- ここにおいて第1の抗原認識ドメインの標的がCD19を含み、第2の抗原認識ドメインの標的がBCMAを含む、請求項27-32のいずれか一項において記載の操作された細胞。
- ここにおいて第1の抗原認識ドメインの標的がCD19を含み、第2の抗原認識ドメインの標的がCD22を含む、請求項27-32のいずれか一項において記載の操作された細胞。
- ここにおいて第1の抗原認識ドメインの標的がCD19を含み、第2の抗原認識ドメインの標的がCD20を含む、請求項27-32のいずれか一項において記載の操作された細胞。
- ここにおいて第1の抗原認識ドメインの標的がCD19を含み、第2の抗原認識ドメインの標的がCD123を含む、請求項27-32のいずれか一項において記載の操作された細胞。
- ここにおいて第1の抗原認識ドメインの標的がCD33を含み、第2の抗原認識ドメインの標的がCD123を含む、請求項27-32のいずれか一項において記載の操作された細胞。
- ここにおいて第1の抗原認識ドメインの標的がCD269を含み、第2の抗原認識ドメインの標的がCS1を含む、請求項27-32のいずれか一項において記載の操作された細胞。
- 第1の抗原認識ドメインの標的がCD4を含み、エンハンサーはIL-15/IL-15sushiを含む、請求項55-60のいずれか一項において記載の操作された細胞。
- ここにおいてTAC1抗原認識ドメインがAPRILリガンドまたはBAFFリガンドまたはその一部を含む、請求項27-32のいずれか一項において記載の操作された細胞。

- ここにおいて第1の共刺激ドメインおよび第2の共刺激ドメインが同じであることを含む、請求項27-32のいずれか一項において記載の操作された細胞。
- ここにおいて第1の共刺激ドメインおよび第2の共刺激ドメインが4-1BB共刺激ドメイン含む、請求項27-32及び37のいずれか一項において記載の操作された細胞。
- 自己免疫障害または抗体媒介臓器拒絶反応を治療する以下を含む方法、
それを必要とする対象に、抗原認識ドメインを有する第1のキメラ抗原レセプターポリペプチドを含む第1の操作された細胞、そして第2の抗原認識ドメインを有する第2のキメラ抗原受容体ポリペプチド、そして第3の抗原認識ドメインを有する第3のキメラ抗原レセプターポリペプチドを含む第2の操作された細胞、そして第4の抗原認識ドメインを有する第4のキメラ抗原レセプターポリペプチド、ここにおいて第1の抗原認識ドメインおよび第2の抗原認識ドメインは、CD19、CD20、およびCD22からなる群から独立して選択される、そして第3の抗原認識ドメインおよび第4の抗原認識ドメインは、BCMA、CS1、BAFF、APRIL、BAFF-R、およびTACIからなる群から独立して選択され、第1の抗原認識ドメインおよび第2の抗原認識ドメインは異なり、第3の抗原認識ドメインおよび第4の抗原認識ドメインは異なる、これらのの有効量を投与すること。 - ここにおいて第1の操作された細胞および第2の操作された細胞が、NK細胞またはT細胞である、請求項102に記載の方法。
