Abstract
Description

いくつかの実施形態では、本明細書に記載の方法に従って修飾された免疫細胞は、ワクチン開発のための抗原をスクリーニングするために使用される。例えば、腫瘍細胞溶解物を、抗原提示細胞、T細胞、またはB細胞に、本明細書に記載の圧搾法を用いて送達する。APCを患者由来のT細胞またはT細胞クローン/株と共にインキュベートして、ワクチン候補抗原を特定する。別のアプローチでは、腫瘍溶解物を負荷したAPCによって処理され提示された抗原は、質量分析によって特定される。このようにして特定された抗原は、その後、ワクチン接種のために使用される。
本開示のある特定の態様は、本明細書に記載の方法に従ってT細胞に標識を送達し、標識T細胞を患者に投与することを含む、患者内のT細胞輸送を決定する方法に関し、患者内のT細胞輸送は、標識されたT細胞を検出することによって決定することができる。いくつかの実施形態では、標識は蛍光標識または放射性標識である。いくつかの実施形態では、T細胞輸送は腫瘍に輸送することである。例えば、T細胞は、患者から単離され、同位体をT細胞に送達するためにマイクロ流体デバイスを通過させて、患者に注射し戻すことができる。次に、PETスキャン等のイメージング方法を使用して、標識を検出し、身体を通してのT細胞の輸送を追跡することができる。

アジュバントまたは免疫応答活性化剤/増強剤は、ワクチン抗原に対する免疫細胞、例えばT細胞の応答の誘出を促進するために使用される。いくつかの実施形態では、免疫細胞がマイクロ流体デバイスを通過した後、免疫細胞をアジュバントと接触させる。例えば、細胞は、マイクロ流体デバイスを通過した後約5分〜約2時間、或いはこれらの間の任意の時間または範囲でアジュバントと接触させる。例えば、マイクロ流体デバイスを通過した後、約5分〜約1.5時間、約5分〜約1時間、約5分〜約45分、約5分〜約30分、約5分〜約15分、または約5分〜約10分、細胞をアジュバントと接触させる。いくつかの実施形態では、マイクロ流体デバイスを通過した後約10分〜約2時間、約15分〜約2時間、約30分〜約2時間、約45分〜約2時間、約1時間〜約2時間、または約1.5時間〜約2時間、細胞をアジュバントと接触させる。ミョウバンまたは油中水型エマルジョン(例えば、フロイントの不完全アジュバント及びMF59(登録商標))等の古典的なアジュバントに加えて、パターン認識受容体(PRR)のリガンド等の他のアジュバントは、先天性免疫を誘導することによって作用し、APCを標的にし、その結果、適応免疫応答に影響を及ぼす。ほぼ全てのPRRファミリーのメンバーがアジュバントの標的である。これらとしては、Toll様受容体(TLR)、NOD様受容体(NLR)、RIG−I様受容体(RLR)及びC型レクチン受容体(CLR)が挙げられる。これらは異なる転写因子の活性化につながる異なるアダプター分子を含む経路を介してシグナル伝達する。転写因子(NF−κB、IRF3)は、免疫応答のプライミング(刺激)、拡大及び分極において重要な役割を果たすサイトカイン及びケモカインの産生を誘導する。NLRP3及びNLRC4等のNLRファミリーのいくつかのメンバーの活性化は、前炎症性サイトカインIL−1β及びIL−18の誘導に関与する、インフラマソームと呼ばれるタンパク質複合体の形成を誘発する。NLRP3及びNLRC4インフラマソームは、ある種のアジュバントによって誘発された先天性免疫に関与しているが、それらの作用機構は不明である。
本開示のある特定の態様は、記載された方法に従って修飾された免疫細胞を患者に導入することにより患者を治療する方法に関する。いくつかの実施形態では、免疫細胞は免疫療法に使用するためのものである。例えば、T細胞に多様な物質の送達を可能にすることにより、T細胞機能は目的の疾患を標的とするように操作される。例えば、目的の抗原を標的とするTCRのためのキメラ抗原レセプターまたはDNA/mRNAを送達することにより、疾患抗原に対して特異的であり、キラー(及び/またはヘルパー)T細胞応答を促すT細胞が生成される。NF−κB、Bcl−2、Bcl−3、Bcl−xl、CD3/CD28の上方制御因子、CpG、R848、PD−1のサプレッサー、PDL−1の抑制遺伝子(サプレッサー)、CTLA−4のサプレッサー等の他の物質が送達され、細胞活性及び生存を増強する。
細胞提示抗原に対する寛容性を誘導するために、細胞は、胸腺間質リンパ球ポエチン、デキサメタゾン、ビタミンD、レチノイン酸、ラパマイシン、アスピリン、トランスフォーミング増殖因子ベータ、インターロイキン−10または血管作動性腸管ペプチド等の寛容原と抗原と一緒に接触し、代わりに寛容性を誘発するであろう。
腫瘍微小環境において、腫瘍反応性T細胞は寛容化され得る。これは、腫瘍発生に関連する他の細胞の寛容原性活性を含む複数の抑制機構に起因する(Andersonら,J Immunol 2007,178:1268−1276;Probstら,Nat Immunol 2005,6:280−286)。CTLA−4及びPD−1等のチェックポイント受容体を介してシグナル伝達を遮断する抗体ベースの薬物は、原発性及び転移性疾患の両方において抗腫瘍応答をもたらしている。チェックポイント遮断に応答する悪性腫瘍は、高い突然変異頻度を有し、がん抗原に反応するT細胞によって浸潤される(Taneja、J Urol 2012,188:2148−2149;Brahmerら,N Engl J Med 2012,366:2455−2465;Wolchokら,N Engl J Med 2013,369:122−133)。このアプローチはある特定の適応症には成功しているが、複数の阻害性チェックポイント表面受容体の存在は、免疫療法に利用可能な現在限定された機能遮断抗体のパネルの適用を弱体化させ得る。さらに、複数の遮断抗体を用いた全身治療の必要性は、毒性を増加させ得(Ribasら,N Engl J Med 2013,368:1365−1366;Weberら,Cancer 2013、119:1675−1682)、特に、組み合せて用いられた時に毒性を増加させ得る(Postow et al.,J Clin Oncol 2015,33:1974−1982及びGaoら,Oncogene 2015に概説されている)。本発明のいくつかの態様において、患者の転帰における劇的な改善が、腫瘍反応性T細胞のみにおいて阻害経路を抑制することにより達成される。非限定的な例において、これは、腫瘍浸潤リンパ球(TIL)、組換えTCR及びキメラ抗原受容体(CAR)T細胞等の養子移入アプローチで使用されるT細胞内の阻害経路の遺伝子ノックダウンまたはノックアウトを阻害または可能にすることによって成し遂げられ得る。
自己免疫性疾患は、しばしば健康な組織に損傷を与える自己反応性免疫細胞を含む。T細胞へのFoxP3(及び/または他の因子)の細胞内送達が、自己免疫に対抗するための制御性T細胞を生じさせるために使用される。これらのTregは、広範な全身免疫抑制のため、または自己抗原特異的T細胞をTreg表現型に再プログラムするために産生される。後者は、自己免疫を永続させ、それらの活性の局所的抑制を誘導する細胞と同じ標的部位へのTregホーミングをもたらす。
自己増幅RNAは、目的のタンパク質を発現するだけでなく、宿主ゲノムへの組み込みの危険性を伴わず細胞質内でその配列を複製するので、独自の能力を提供する。従って、自己増幅RNAの免疫細胞への導入は、免疫細胞機能を操作するのに有用である。いくつかの具体的な現れの形態としては、カミカゼ免疫細胞、タンパク質送達の代替、または細胞機能の継続的な調節が挙げられ、それぞれが以下に記載される。
上記のものと同様のワクチン接種または免疫抑制戦略のために、目的の抗原をコードするsaRNAを、所望の位置にある免疫細胞に送達する。例えば、saRNAをコードするがん抗原のT細胞(またはB細胞、単球、DCまたはマクロファージ)への送達及びそれらの細胞の患者への注入は、T細胞における抗原の迅速な産生と、迅速なsaRNA複製に起因するこれらT細胞の最終的な死とをもたらす。標的抗原を負荷したこれらの死滅するT細胞の爆発は、感染をシミュレートし、標的組織中の先天性細胞による物質の取り込みをもたらし、標的抗原に対するワクチン応答を感作する。標的組織、アジュバントの存在、及び患者の全体的な炎症状態に依存して、この戦略を用いて寛容性も誘導する。寛容を誘導するために、前記カミカゼ細胞は、寛容誘発因子及び抗原を放出するように操作されるか、またはTGF−ベータ及びIL−10等の寛容誘発因子と接触させる。例えば、カミカゼT細胞またはB細胞には、寛容原性である因子(例えば、TGF−β及びIL−10の分泌)をも発現しながら、所望の抗原を過剰発現するmRNA、DNAまたはsaRNAを負荷することができる。これらの細胞は、その後、死滅する前にリンパ系器官に移動して、抗原及び寛容原性因子を環境に放出し、標的抗原に対する寛容性を誘導するのを助ける。これは、自己反応性エフェクターT細胞を排除するのに役立ち、自己反応性に対して保護することができるT regの産生を促進する。1つの非限定的な例において、このアプローチは、関節リウマチ抗原を用いて使用される。
saRNA、mRNAまたは発現ベクター、例えばプラスミドは、タンパク質送達のパルスではなく、持続した時間のワクチン治療のための連続的なタンパク質産生を提供する。
一例としてT細胞養子免疫伝達療法を使用すると、より良好なT細胞療法の開発は、免疫抑制経路の阻害剤をコードするsaRNAを用いることによって達成される。saRNAはまた、刺激タンパク質を発現して、T細胞活性化及び/または抗アポトーシスタンパク質の高い状態を維持して生存を延長するためにも使用される。抑制インヒビターの非限定的な例としては、PD−1、PD−L1、CTLA−4を遮断する任意の物質または他のチェックポイント阻害剤が挙げられる。IL−2、NF−Kb、IL−7、IL−15、IL−12等を発現して、高T細胞活性を維持することができ、Bcl2及びBclx1のようなタンパク質は生存延長に役立ち得る。
免疫細胞は、自己細胞の供給源として使用される。細胞は、数多くの疾患治療機能を実行するように再プログラムされる。例えば、関節炎の場合、物質をT細胞に送達して、関節ホーミングフェノタイプの発現を誘導し、症状を緩和する因子(例えば、IL−4、IL−6、IL−10、IL−13、IL−11、TGFb、レチノイン酸、及びチェックポイント覚醒剤)の放出をプログラミングし、或いは、パーキンソン病の場合には、材料をT細胞またはB細胞に送達して脳ホーミング受容体の発現を誘導し、患者の転帰を改善する因子の分泌を誘導する。関節炎に関して、炎症誘発性サイトカインを除去することも有用であり、例えば、IL−2に対して高い親和性を有するT細胞は、自己反応性エフェクター細胞が必要とするサイトカインを吸収する。
CellSqueezeプラットフォームは、3つの主要成分、すなわちa)並列に複数のチャネルを含み、各々が少なくとも1つの狭窄点を含むシリコン及びガラスのマイクロ流体チップ、b)チップとインターフェースし、細胞懸濁液を負荷/収集することを可能にするリザーバシステム、c)リザーバを加圧し、チップを通る流体の流れを容易にするための圧力調整システム、からなる。典型的なワークフロー(図1A)において、標的送達材料を所望の細胞(懸濁液中)と混合し、それらをリザーバ(貯蔵器)に装填する。次に、圧力配管がリザーバに接続され、チャンバが所望のレベルで加圧されて流体の流れが開始される。処理後、細胞をアウトプットリザーバから収集し、所望の温度である時間、例えば少なくとも1、2、3、4、5分またはそれ以上インキュベートすることができ、さらなる処理の前に適切な膜回収を保証する。
がん特異抗原を認識するキメラ抗原受容体(CAR)を発現するようにT細胞を修飾(改変)することにより、免疫検出を回避するであろう腫瘍細胞を認識して殺傷するように細胞をプライミングすることができる。このプロセスは、患者のT細胞を抽出し、それらをCARの遺伝子でトランスフェクトし、次にトランスフェクトされた細胞を患者に再注入することを含む。
化合物または組成物を細胞質ゾルに導入するために上記のように処理された免疫細胞は、例えば、腫瘍特異的T細胞媒介性免疫応答を誘発して腫瘍を殺すまたは腫瘍の増殖を阻害することによって、腫瘍を治療するために使用される。本方法のデバイス及び方法は、腫瘍細胞特異的/腫瘍関連抗原またはその混合物、例えば腫瘍生検細胞溶解物調製物を免疫細胞に導入するので、本方法はどのような腫瘍型にも適用可能である。例えば、腫瘍の種類としては、米国でまん延している膀胱がん、乳がん、結腸及び直腸がん、子宮内膜がん、腎臓がん、白血病、肺がん、メラノーマ、非ホジキンリンパ腫、膵臓がん、前立腺がん、甲状腺がんが挙げられる。(米国がん学会:Cancer Facts and Figures、2015,Atlanta,Ga:American Cancer Society,2015。オンラインで入手可能)。処理された免疫細胞を使用して治療される他の腫瘍の種類には、脳(膠芽細胞腫)、肝臓(肝細胞がん)、並びに原発腫瘍の部位または組織とは異なる体内の解剖学的部位または組織に生じる転移性のがんが含まれる。
精製された腫瘍関連抗原または腫瘍細胞溶解物(抗原の異種混合物)は、細胞の圧搾を用いて免疫細胞に送達される抗原として使用される。腫瘍細胞溶解物は、腫瘍と診断された、及び/または病理学的悪性腫瘍の治療が予定されている、対象から得られた腫瘍細胞株または腫瘍生検組織から産生される。腫瘍細胞溶解産物の産生は当該分野で公知である。このような腫瘍溶解物調製物は、免疫細胞の細胞質ゾルへの細胞圧搾に基づく送達に使用するのに適している
ウイルス関連抗原は、細胞の圧搾を用いて免疫細胞に送達され得る。ウイルス関連抗原は当技術分野で公知であり、このような抗原は生化学的に精製され、組換え発現され、及び/またはそうでなければ精製/単離され得る。ウイルス抗原の例を以下の表に示す。
細菌抗原もまた、細胞圧搾を用いて免疫細胞に送達され得る。好ましい実施形態では、細菌抗原は、マイコプラズマ種(Mycoplasma sp.)、マイコバクテリウム種(Mycobacterium sp.)(例えば結核菌)またはリステリア菌(Listeria monocytogenes)等の細胞内細菌と関連している。
T細胞及びB細胞を、製造者の指示(ネガティブ選択技術)に基づいて、Stemcell Technologies(Vancouver,Canada)の細胞特異的単離キットの既知の方法を用いて、野生型C57BL/Jマウスの脾臓から単離した。単球/マクロファージを、チオグリコール酸溶液1mlの腹腔内注射の3日後に野生型C57BL6/Jマウスの腹腔から単離した。Stemcell Technologies(Vancouver,Canada)のStemcell TechnologiesのCD11b陽性選択キットを用いて、製造者の指示に基づいて細胞を精製した。細胞を、10%ウシ胎仔血清、1%抗生物質/抗真菌剤、0.5%ベータ−メルカプトエタノール、1%非必須アミノ酸、1mMピルビン酸ナトリウム、及び10mM HEPES緩衝液を含有するグルタミン含有RPMI1640培地中でインキュベートした(全てLife Technologies、NY、USA製)。
ヒトPBMCを、既知の方法、例えばFicoll−Paque(GE Healthcare、Uppsala、Sweden)を用いて、全血から密度勾配遠心分離を用いて分離した。CD14+T細胞を、CD14及びCD4磁気マイクロビーズ(MACS Miltenyi Biotec、Auburn、CA)を用いてPBMCのCD14陰性画分から分離した。10%ヒト血清(AB)(GemCell、West Sacramento、CA)、100U/mlペニシリン及び100μg/mlストレプトマイシン・スルフェート(H10培地)を含み、5ng/ml rhIL−15(R&D Systems、Minneapolis、MN)が追加されたRPMI1640培地(Cellgro、VA、Manassas、VA)において、T細胞を培養し、細胞活性化を行わずに細胞生存率を維持した。抗CD14磁気マイクロビーズ(MACS Miltenyi Biotec)を用いて末梢血単核細胞から選択されたCD14陽性単球からヒト単球由来樹状細胞(MDDC)を調製し、100ng/mlインターロイキン−4及び50ng/ml顆粒球−マクロファージコロニー刺激因子(R&D Systems)で6日間培養した。

記載されたエレクトロポレーション実験では、Amaxa Nucleofector II(Lonza Inc.、Allendale、NJ)を製造業者の推奨に従って使用した。ヒトT細胞の実験は、ヒトT細胞キットを用いて、ヒトの非刺激T細胞、高い生存率、U−014についてのプログラムを使用して行った。ヒトMDDCについては、MDDC用のヒト樹状細胞キットを用いてヒト樹状細胞U−002用プログラムを使用した。簡潔には、2×106個の細胞を200pmolのsiRNAを含む100μlのヌクレオフェクション溶液中に懸濁させ、機械によってヌクレオフェクトした。タンパク質送達を試験するために、Cellsqueeze及びヌクレオフェクション実験の両方にAPC標識マウスIgG1(cl.MOPC−21、Biolegend)を0.02mg/mLで用いた。また、3kDaカスケードブルー標識デキストラン及び70kDフルオレセイン標識デキストランを0.2mg/ml(Invitrogen)で使用した。
調節性T細胞(Treg)を単離し、公知の方法を用いて増殖させた。例えば、CD4+T細胞RosetteSep濃縮キット(Sigma−Aldrich及びSTEMCELL Technologies)を用いた密度遠心分離によって健康な個体の末梢血からCD4+T細胞富化PBMCを単離し、抗CD3−PE−Cy7、CD4−FITC 、CD25−APC及びCD127−PEで標識した。CD3+CD4+CD25+CD127low Tregを、FACS Aria細胞選別機(BD Biosciences)で選別し、抗CD3/抗CD28被覆マイクロビーズ(Invitrogen)で刺激し、IL−2(300U/mL)で培養した。
抗CD8−Pacific Blue、抗CD4−APC、抗CD11b−PE(抗M1/70)、抗CD11c−APCを用いてマウス細胞を染色した。死んだ細胞を排除するためにヨウ化プロピジウムを用いた。データは、FACS CantoII、LSR II、またはLSRFortessa(BD Biosciences)を用いて取得し、FlowJo(Tree Star、Ashland、OR)を用いて分析した。
一次CD4+T細胞を、10−4チップを用いて5μMsiRNAで処理した。CD4のノックダウンのために、siRNAは感染の48時間前に送達され、ウイルス遺伝子vif及びgagを標的とするsiRNAは感染の24時間前に送達された。次いで、細胞を5μg/mlのPhytohaemagglutinin(PHA)で一晩刺激し、HIVIIB(400ng/ml p24)を用いて2×105細胞/ウェルで96ウェルプレートにおいてHIVIIIBに感染させた。HIV IIIBは、NIH AIDS試薬プログラムから得られ、ウイルスストックは、以前に記載されたように調製された(18)。感染は、5μg/mlのポリブレン及び1200xgのスピノキュレーションの37℃で2時間での添加により増強された(19)。Fix&Perm Kit for Cell permeabilization(Invitrogen)で抗p24 KC57−FITC抗体(Beckman Coulter、Fullerton、CA)を用いて24時間後に細胞内p24抗原染色を行い、フローサイトメトリーにより分析した。
RNeasy Mini Kit(Qiagen)を用いてT細胞から全RNAを単離し、Superscript III及びランダムヘキサマー(Invitrogen)を用いてコピーDNAを合成した。リアルタイムPCRは、SsoFast EvaGreen Supemix及びBio−Rad CFX96 Real−Time PCR System(Bio−Rad Laboratories,Hercules,CA)を用いて行った。プライマーは以下の通りであった:Gapdhフォワード:5’−AGCCACATCGCTCAGACAC−3’(配列番号8)、Gapdhリバース:5’−GCCCAATACGACCAAATCC−3’(配列番号9)、CD4フォワード:5’−GGCAGTGTCTGCTGAGTGAC−3’(配列番号10)、CD4リバース:5’−GACCATGTGGGCAGAACCT−3’(配列番号11)。
GraphPad Prism 4ソフトウェア(GraphPad Software、San Diego,CA)を用いて、一元配置分散分析(ANOVA)が、ボンフェローニ(Bonferroni)の多重比較試験と共に、複数の群を比較するときに実施され、或いは、両側スチューデントT検定を、2グループを比較する時に用いた。*、**及び***は、BonferroniのMultiple比較試験を使用した場合の0.05,0.01及び0.001未満のP値を示し、###は、両側スチューデントT検定を使用した場合の0.001未満のP値を示す。別段の指示がない限り、データは平均±1標準偏差として表される。
試験したマイクロ流体デバイスは、狭窄長(10〜50μm)、幅(4〜9μm)及びチャネル当たりの狭窄部数(1〜5狭窄部)を変化させた45〜75の平行なマイクロ流体チャネルを含んでいた。(Sharei et al.,2013,Proceedings of the National Academy of Sciences of the United States of America 110:2082−2087;Sharei et al.,2014,Integrative biology:quantitative biosciences from nano to macro 6:470−475)。
一次免疫細胞への細胞内送達を可能にするこのプラットフォームの可能性を評価するために、マウスT細胞、B細胞、及び単球/マクロファージを、蛍光標識デキストラン(3及び70及び2,000kDa)、及び抗体(約150kDa)の存在下にマイクロ流体デバイスに通した。これらの物質は、それぞれ小分子、多糖類、及びタンパク質のモデルとして選択された。細胞タイプごとに、少なくとも4つのデバイス設計と2つの動作圧力が試験された。収縮の大きさの最初の選択は、細胞株、初代線維芽細胞及び胚性幹細胞での作業によって導かれた。(Sharei et al.,2013, Proceedings of the National Academy of Sciences of the United States of America 110:2082−2087)。具体的には、狭窄部の幅は、標的細胞の平均直径の約50%であるように選択された。リンパ球及び骨髄細胞では、30−4デザイン(すなわち、狭窄は長さ30μm及び幅4μmを有する)を使用した送達が最も効果的であった(図2A〜C)。
ヒト免疫細胞に対するこのアプローチの適用性を調べるために、T細胞について4〜6μm及び単球由来樹状細胞(MDDC)について6〜9μmの範囲の狭窄幅を有するデバイス設計を試験した。異なる生体分子に対する抗体送達及び効率データの代表的なヒストグラムを図3Aに示す。最も効果的な設計は、3kDaのデキストランをT細胞の70%±9%(4μmの狭窄サイズ)及びMDDC(7μmの狭窄サイズ)の60%±4.5%に送達した(図3A)。70kDaのデキストランをT細胞の71%±11%及びMDDCの40%±14%に送達し、タンパク質(抗体)をT細胞の65%±15%及びMDDCの38%±7%に送達した。蛍光標識されたsiRNA(CD45RA siRNA−Alexa−Fluor−488)の送達も同様の結果を与えた(図12、13)。
マイクロ流体アプローチが、ウイルス遺伝子を標的とするsiRNAを送達することによってヒト初代CD4+T細胞におけるHIV感染及び複製を阻害するのに有用であるかどうかを決定するために試験を行った。以前にウイルス複製を抑制することを示したvif及びgagに対するsiRNAを、HIV感染24時間前に細胞に送達した。陽性対照として、HIV受容体CD4を標的とするsiRNAを使用した。CD4siRNAを感染の48時間前に送達して、表面CD4レベルの低下を確実にし、従って感染を阻害した。HIV複製は、細胞内p24抗原を染色することによって決定し、フローサイトメトリーによって測定した。独立した感染実験の代表的なヒストグラム及び集計結果を図4A〜Bに示す。vif及びgag標的化siRNAで処理されたT細胞において、p24抗原の有意な減少が、別々にまたは組み合わせて観察された(p<0.01)。阻害効果は、CD4ノックダウン(p<0.05)により誘導されたものよりも大きかった(図4A〜B)。
免疫細胞への巨大分子の細胞内送達は、本発明より以前には多大な課題であった。本明細書に示される結果は、マウスやヒトの免疫細胞のような真核細胞に小分子だけでなく大型の巨大分子も送達するベクターフリー膜破壊技法の有用性を示している。結果は、免疫細胞はこれらの細胞の細胞質に組成物を送達する他の技術にとって扱いにくいため、特に臨床用途に関連して驚くべきことである。本明細書に記載のデータは、(i)多様な生物学的に関連する巨大分子(多糖類、タンパク質、及び核酸)を送達する能力;(ii)最も臨床的に関連する免疫細胞サブセット(T細胞、B細胞、DC、単球/マクロファージ)における有効性(図2A〜C及び図3A〜D);(iii)ベクター物質及び電場からの独立性、従ってエンドサイトーシスの捕捉及びエレクトロポレーションレベルの毒性に関連するいくつかの課題を克服すること;及び(iv)複数の種類の巨大分子の標的細胞への同時送達(図2C、9、12、13)を示す。
感染性病原体は、兵士にも一般市民にも同様に重大な脅威をもたらす。潜在的な生物学的攻撃や流行性ウイルスの進化を防ぐために、頑強で迅速応答の予防接種能力を開発しなければならない。最先端の技術で、毒性のある菌株を科学者が分離して特徴付けることができたとしても、効果的なワクチンを開発するには長い年月と数十億ドルかかることがある。エボラやHIV等の多くの致命的な病原体は、何十年もの研究にもかかわらず、現代のワクチン接種法では対応できない。本発明は、本明細書に記載のベクター無しの細胞内送達プラットフォームを使用して個々の免疫細胞を直接操作することにより、病原体に対する迅速応答、多標的、個別保護のための方法及びデバイスを提供している。この方法は、新たに感染した個体を同定してから数時間以内に地域の危険性のある集団に効果的にワクチン接種することを可能にし、以下の表3に示すように以前のアプローチに比べていくつかの優位性を有する。
抗原提示細胞(APC)は、組織から外来または自己のタンパク質及びペプチドを捕捉し、適応免疫細胞を活性化して、これらの抗原に対する炎症性または寛容性免疫応答を生じさせる免疫細胞(樹状細胞、マクロファージ、B細胞を含む)の多様なサブセットである。タンパク質は、それらの周囲の液相サンプリング、または外来微生物または死細胞破片の受容体媒介摂取を介して、インビボでAPCによって摂取される。摂取されたタンパク質は、ペプチドフラグメント(抗原)に分解されるが、このペプチドフラグメントは、APC及び提示された抗原によって受け取られた特異的なシグナルに基づいてナイーブT細胞活性化を指示する、共刺激シグナルと共に処理されてT細胞に提示されるものである。T細胞活性化におけるこの重要な役割のために、抗原が負荷され、体外で(ex vivo)で活性化された精製APCは、培養物(例えば、養子T細胞療法のため)で機能性T細胞を拡大するために、またはインビボでの有効な細胞ワクチンとして、使用され得る。マイクロ流体システムを使用して、APCの体外操作は、特異的タイプの免疫、特にがん及びHIV等の疾患(これらの疾患では、病原細胞の標的死滅が重大であり、そして内因性のAPC機能が活発に抑制される)において細胞傷害性Tリンパ球(CTL)を生成するための代替アプローチとして有効であることが示された。有望な前臨床試験にもかかわらず、細胞ベースのワクチンの臨床的翻訳は、複数の制限によって妨げられてきた。免疫細胞の細胞質ゾルへ抗原を送達するためのマイクロ流体デバイス及び関連する方法は、以前のアプローチの多くの問題及び欠点を解決する。
抗原を用いてB細胞を負荷するための機械的破壊のプロセスは、本明細書に記載のデバイスを用いて達成された。生細胞を、シリコンデバイス内の平行なマイクロ流体チャネルに通した;各チャネルにおいて、1つ以上の狭窄部が、デバイスを通過する細胞の膜に一時的な細孔または乱れを生じさせる。周囲の流体中に存在する巨大分子積み荷は、この過渡的乱れ/膜破壊の間に細胞内に拡散し、細胞内負荷をもたらす。機械的破壊は、原発性ネズミB細胞を含む多種多様な細胞型への巨大分子の細胞質送達を促進するために有効である。長さ30μm及び幅5μmの寸法を有する5つの連続した狭窄部で効率的な送達が達成された。B細胞へのタンパク質送達及びその後の抗原提示のための機械的破壊パラメータを個別化するのを促進するために、デバイス設計を変更することができる(平行狭窄チャネルの数を75に増やし、侵入領域を長くする、可逆性等)。最適化予備実験は、120psiで作動する30−4×1マイクロ流体チップ(長さ30μm、直径4μmのチャンネル当たり1つの狭窄)が、5×106B細胞/mLの濃縮イオンで効率的な送達及び高い細胞生存率の両方を有するマウスB細胞の機械的破壊のための効果的なチップ設計であることを示した。本明細書には他の構成、例えば30−5×5対30−4×1が記載されている。この圧力の細胞は、約100万個/秒の速度でデバイスを通過した。マイクロ流体圧搾は、エンドサイトーシスと比較してデキストランの取り込みを大きく向上させ、内在化が3及び40kDaのデキストランについて、それぞれ約65倍及び約25倍に増加した。これは、両方のデキストランについて全細胞の75〜90%への送達を示し、それに対し、エンドサイトーシスによる休止B細胞は10%未満の検出可能な量の積み荷である。機械的破壊後の回収細胞の生存率は、エンドサイトーシス対照と同様に約95%であった。これらの使い捨てマイクロ流体チップを通過できる最大数の細胞の容量は、1デバイスあたり100万〜500万細胞の範囲であったが、デバイスは、詰まるまで複数のアリコートの細胞(1回運転あたり100万の細胞)で運転され、所与の実験において各個々のデバイスを通過する細胞の最大数はしばしば500万個をかなり超える細胞であった。最初の運転からデバイスが詰まるまでの送達細胞のパーセンテージの変動が低く、デバイス内変動が最小であることが示された。同じ実験セッション内の複数のデバイスの送達性能(デバイス間のばらつき)は非常に一貫していた。いくつかの用途では、より多くのB細胞数、異なる細胞密度でのマイクロ流体デバイスの有効性を必要とすることがあることを認識する。細胞内デキストラン送達の効率は、少なくとも50×106細胞/mLまでは、細胞濃度とはほとんど無関係であり、強固な拡張性の可能性を示している。
機械的破壊が細胞質クラス1 MHC抗原プロセシング及び提示機構へのタンパク質の送達を促進し得るかどうかを決定するために、周囲の培地中において過剰のモデルタンパク質オボアルブミン(OVA)の存在下に細胞をマイクロ流体デバイスに通すことにより、OVAを休止した精製ポリクローナルB細胞に送達するよう、前述の最適条件を利用した。その後、圧搾されたB細胞を、MHCクラスI制限OVAペプチドSIINFEKLに特異的なトランスジェニックT細胞受容体を有するCFSE標識OT−I CD8+T細胞と、B細胞活性化刺激としてのCpGの存在下または非存在下に共培養した。OT−I T細胞でのCFSE希釈を、フローサイトメトリーで分析し、B細胞提示抗原に応答するT細胞の増殖/拡大を評価した。細胞の圧搾による全タンパク質のB細胞への送達は、インビトロでの強力なMHCクラスI抗原提示及び抗原特異的CD8+T細胞プライミングを可能にすることが見出された。本明細書に記載されるような細胞の圧搾は、主に抗原を細胞質ゾルに向かわせ、MHCクラスII負荷が生じるエンドソーム区画には誘導しない。これと一致して、データは、休止中でもCpG活性化され機械穿孔された(mechano−porated)B細胞も、4日間の共培養の後にOVA特異的OT−II CD4+T細胞を拡大できないことを示した。B細胞プライミングCD8+T細胞集団の機能性を、共培養の2日目及び4日目のエフェクター分子の分泌を測定することにより評価した。エンドサイトーシスによって抗原を負荷したB細胞は基礎細胞レベルのサイトカインを産生したが、圧搾により抗原を負荷したB細胞は、休止していても活性化していても、グランザイムB、IFN−γ及びTNF−αの実質量を分泌させるためにT細胞をプライミング(prime)した。
さらに、インビトロ抗原特異的T細胞増殖プラットフォームでは、記載されたように処理されたB細胞は、細胞ワクチンとして使用するための樹状細胞の代替物として有用である。インビボの性能を評価するために、コンジェニックマーカーとしてThy1.1を発現するCFSE標識OT−I CD8+T細胞を、抗原提示のレポーターとして受容マウスに養子移入した。1日後、機械破壊抗原負荷直後に休止B細胞を注入したか、または続いてインビトロにおいてCpGで24時間活性化された機械破壊負荷B細胞を注入した。エンドサイトーシスによって抗原を負荷した休止B細胞を、直ちに、またはCpGで24時間の活性化後のいずれかで、対照として用いた。B細胞移植の4日後、マウスを犠牲にし、脾臓及び鼠径リンパ節をフローサイトメトリーによりOT−I増殖について分析した。インビトロの結果と一致して、機械穿孔されたB細胞は、養子移入されたOT−I T細胞の分裂を誘発することができたが、エンドサイトーシス対照は基礎分裂のみを示した。CpG活性化及び休止圧搾B細胞の両方が、脾臓におけるOT−I増殖を引き起こした(それぞれ、SQZ対エンドサイトーシスを比較して、p<0.001及びp=0.001の注入OT−I T細胞の約45%及び約35%分裂)。エンドサイトーシスB細胞と比較して、同様にCpG活性化B細胞及び休止圧搾B細胞もリンパ節における向上したOT−I増殖を誘発した(それぞれ、CpG B及び休止圧搾B細胞による、注入されたOT−I T細胞の約40%及び約35%の分裂;休止時とCpGの両方についてSQZ対エンドサイトーシスを比較する。p<0.001)。エンドサイトーシス対照は、リンパ節で約4%のベースライン分割を示した。これらの結果は、マイクロ流体機械破壊によって負荷されたB細胞が細胞ワクチンにおいて有用であることを示した。
従って、B細胞を、細胞を圧搾処理して、インビトロ及びインビボの両方で抗原特異的T細胞をプライミングする自己抗原提示細胞(APC)を得た。このマイクロスケール細胞の圧搾プロセスは、原形質膜に一時的な乱れまたは細孔を生じさせ、機械的破壊を介して周囲の媒体からB細胞への全タンパク質の細胞内送達を可能にする。休止B細胞及び活性化B細胞の両方が、CD4+T細胞ではなく、専ら抗原特異的CD8+T細胞に機械的破壊により送達される抗原を処理し、提示する。圧搾したB細胞は、グランザイムB及びインターフェロン−γを含む細胞溶解性機能にとって重要なエフェクターサイトカインを産生するインビトロで多数のエフェクターCD8+T細胞をプライミングし、拡大した。圧搾された抗原負荷B細胞はまた、マウスに養子移入された場合、インビボで抗原特異的CD8+T細胞をプライミングすることができた。これらのデータは、機械破壊/膜攪乱が、B細胞抗原負荷、抗原特異的CD8+T細胞のプライミング、及びB細胞活性化からの抗原取り込みの分離に有用であり、また非常に効率的な方法であることを実証している。
本発明以前には、免疫刺激または寛容のための抗原提示は、一般に、例えばB細胞、樹状細胞及びマクロファージ等の抗原提示細胞と呼ばれる、選択されたサブセットの細胞の範囲であると仮定され、そして免疫刺激または寛容性を促進する点で抗原を処理及び提示するのに必要な機構が欠如していると推定されたため、T細胞は含まれなかった。細胞のデバイス及び圧搾媒介抗原負荷は、T細胞の細胞質ゾルに抗原性タンパク質を直接送達することによって、このような処理をされた細胞に抗原提示表現型を付与するという驚くべき発見をもたらした。
0日目:CFSE/CellTraceViolet標識OT−I CD45.1T細胞をナイーブB6宿主に注入する。
1日目:T細胞APCS(Tc−APC)に抗原を送達し、B6宿主にTc−APC及びAPC対照を注入する。
5/6日目:免疫化マウスの脾臓及びリンパ節を採取し、フローサイトメトリーを介してT細胞の増殖を分析する。
細胞の圧搾はまた、後で患者に戻すために生体外(ex vivo)で患者由来の免疫細胞を富化または拡大するために有用である。例えば、抗原は、機械的破壊を介して抗原提示細胞(樹状細胞、B細胞、T細胞、マクロファージ)の細胞質ゾルに送達される。これらの抗原提示細胞は、その表面にMHC/HLAヘテロダイマーという観点で処理された抗原を処理して提示し、T細胞を刺激し及び拡大する。T細胞の拡大された集団は、その後、治療免疫応答を増大させるために患者に再注入される。例えば、抗原は、抗腫瘍応答を増強するための腫瘍抗原(または溶解物)である。あるいは、微生物感染症を増強するための抗原または細菌もしくはウイルス抗原(またはそれらの断片もしくは弱毒化もしくは死滅した細菌細胞またはウイルス粒子)。別の例では、抗原は、寛容化された免疫細胞の集団を寛容化させてその後に膨張させて患者に再注入して戻し、異常な(例えば自己免疫の)応答を下方制御するための寛容原(上記のような)と共に提示される自己抗原である。
抗体治療薬を使用して患者のT細胞の活性化に基づくがん免疫療法は、高頻度の突然変異を有し、且つ腫瘍関連抗原(TAA)に対する既存のT細胞応答を有するこれらの適応症において成功していることを示している(Topalian et al.,Cancer Cell 2015;27:450−461;Hodi et al.,N Engl J Med 2010;363:711−723;Topalian et al.,Cell 2015;161:185−186)。多くのがんは突然変異の頻度が低く、自発的T細胞応答の率も低いので、がんワクチンの使用はTAAに対するT細胞応答を増強するのに役立つかもしれない(Melief et al.,J Clin Invest 2015;125:3401−3412)。がんワクチンは、単独療法(Ly et al.,Cancer Res 2010;70:8339−8346;Kantoff et al.,N Engl J Med 2010;363:411−422)として、チェックポイント封鎖と組み合わせて、(Agarwalla et al.,J Immunother 2012;35:385−389)、または養子T細胞療法と組み合わせて(Lou et al.,Cancer Res 2004;64:6783−6790)使用される可能性がある
自己寛容を自己抗原に誘導することができるので、治療用ワクチン接種は、今やネオ(neo)(Gubin et al.,J Clin Invest 2015;125:3413−3421;Gubin et al.,Nature 2014;515:577−581;Yadav et al.,Nature 2014;515:572−576;Quakkelaar et al.,Adv Immunol 2012;114:77−106;Castle et al.,Cancer Res 2012;72:1081−1091)及びウイルス抗原(Melief et al.,J Clin Invest 2015;125:3401−3412;Quakkelaar et al.,Adv Immunol 2012;114:77−106)を標的とすることが好ましい。
抗原特異的T細胞を拡大させる能力についていくつかの条件下でペプチドカクテルを負荷したDCの機能を試験する。例えば、抗原特異的T細胞は、HPV16−SLP及びCpGでマウスを免疫することによって誘導される。追加免疫の8日後、T細胞を免疫化マウスの脾臓から単離し、抗原特異的CD4及びCD8T細胞増殖及びサイトカイン産生を引き起こす負荷細胞の能力を決定するために、HPV16−SLP負荷DCとの共培養に使用する。ヒト負荷DCを試験するために、HPV16に反応性のヒトT細胞クローンを使用して、いくつかの条件下でHPV16−SLPカクテルを負荷したヒトDCと共培養する。抗原特異的T細胞拡大及びサイトカイン産生を評価する。ヒト及び/またはマウス負荷DCが機能的であり、抗原特異的T細胞を拡大することができるか否かを、インビトロで試験される。HLA特異的クラスI短ペプチドを負荷したDCを対照として使用する。HLA適合T細胞は応答物として使用される。
