CD123(インターロイキン3受容体α、IL‐3Ra)は、40kDa分子であり、インターロイキン3受容体複合体の一部である(非特許文献1)。インターロイキン3(IL‐3)は、赤血球、骨髄及びリンパ系前駆細胞の細胞への、多能性幹細胞の初期分化を開始させる。CD123は、CD34+コミット前駆細胞上に発現する(非特許文献2)が、CD34+/CD38‐正常造血幹細胞によって発現されない。CD123は、好塩基球、肥満細胞、形質細胞様樹状細胞によって発現され、単球、マクロファージ及び好酸球によってある程度発現され、好中球及び巨核球によってはあまり又は全く発現されない。いくつかの非造血組織(胎盤、精巣のライディッヒ細胞、特定の脳細胞要素及びいくつかの内皮細胞)はCD123を発現するが、発現の大半は細胞質内である。
CD3は、4つの別個の鎖で構成されたT細胞補助受容体である(非特許文献13)。哺乳類において、この複合体はCD3γ鎖、CD3δ鎖及び2つのCD3ε鎖を含有する。これらの鎖は、T細胞受容体(TCR)として公知である分子と連動して、Tリンパ球の活性化信号を生成する。CD3が存在しない場合、TCRは適切に集合せず、劣化する(非特許文献14)。CD3は、全ての成熟T細胞の膜に結合し、実質的に他の細胞タイプには結合しないことが知られている(非特許文献15、16、17)。

本発明の組成物は、医薬組成物の製造に使用できるバルク薬剤組成物(例えば不純又は非滅菌組成物)、及び単位剤形の調製に使用できる医薬組成物(即ち被験体又は患者への投与に好適な組成物)を含む。これらの組成物は、予防的又は治療的有効量の本発明の配列最適化CD123×CD3二重特異性ダイアボディ、又はこのような作用剤と薬学的に許容可能なキャリアとの組み合わせを含む。好ましくは、本発明の組成物は、予防的又は治療的有効量の本発明の配列最適化CD123×CD3二重特異性ダイアボディと、薬学的に許容可能なキャリアとを含む。
本発明の組成物は、有効量の本発明の融合タンパク質若しくはコンジュゲート分子又は本発明の融合タンパク質若しくはコンジュゲート分子を含む医薬組成物を被験体に投与することによる、疾患、障害又は感染症に関連する1つ又は複数の症状の治療、予防及び改善のために提供できる。好ましい態様では、上記組成物は実質的に精製される(即ち上記組成物の効果を制限する、又は望ましくない副作用を生成する物質を実質的に含まない)。ある具体的実施形態では、被験体は動物、好ましくは非霊長類(例えばウシ属、ウマ科、ネコ科、イヌ科、齧歯類等)又は霊長類(例えばカニクイザル等のサル、ヒト等)といった哺乳類である。ある好ましい実施形態では、被験体はヒトである。
本発明の配列最適化CD123×CD3二重特異性ダイアボディは、CD123の発現に関連する又はCD123の発現を特徴とするいずれの疾患又は状態を治療する能力を有する。従ってこのような分子は、限定するものではないが:急性骨髄性白血病(AML);CML及びCMLに関連したアベルソン癌遺伝子の急性転化(Bcr‐ABL転座)を含む、慢性骨髄性白血病(CML);骨髄異形成症候群(MDS);急性Bリンパ芽球性白血病(B‐ALL);リヒター症候群又はCLLのリヒター転化を含む、慢性リンパ性白血病(CLL);有毛細胞白血病(HCL);芽形質細胞様樹状細胞腫瘍(BPDCN);マントル細胞白血病(MCL)及び小リンパ球性リンパ腫(SLL)を含む非ホジキンリンパ腫(NHL);ホジキンリンパ腫;全身性肥満細胞症;並びにバーキットリンパ腫(実施例2参照);自己免疫性狼瘡(SLE);アレルギー;喘息並びに慢性関節リウマチの診断又は治療において採用してよい。本発明の二重特異性ダイアボディは更に、上述の状態の治療のための薬品の製造に使用してよい。
表2は、発現及び精製後の二重特異性ダイアボディのリストを含む。配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A)及び非配列最適化CD123×CD3二重特異性ダイアボディ(DART‐B)は、CD123及びCD3に同時に結合できる。対照二重特異性ダイアボディ(対照DART)は、FITC及びCD3に同時に結合できる。これら二重特異性ダイアボディは、挙げられているアミノ酸配列のヘテロ二量体又はヘテロ三量体である。二重特異性ダイアボディを形成するための方法は:国際公開第2006/113665号;国際公開第2008/157379号;国際公開第2010/080538号;国際公開第2012/018687号;国際公開第2012/162068号;国際公開第2012/162067号において提供される。
合計106個の標的細胞を培地から採取し、FACS緩衝液中の10%ヒトAB血清(PBS+1%BSA+0.1%Naアジド)中に再懸濁し、Fc受容体をブロックするために5分間インキュベートした。異なる抗体結合性能を有するマイクロスフェア(Quantum(商標)Simply Cellular(登録商標)(QSC)、Bangs Laboratories,Inc.,インディアナ州フィッシャーズ)及び標的細胞の抗体標識を、製造元の指示に従って抗CD123 PE抗体(BD Biosciences)で標識した。要するに、1滴の各QSCマイクロスフェアを5mLポリプロピレンチューブに添加し、PE標識抗CD123抗体を1μg/mLの濃度で標的細胞及びマイクロスフェアの両方に添加した。チューブを暗所において4℃で30分間インキュベートした。2mLのFACS緩衝液を添加して2500×Gで5分間遠心分離することによって、細胞及びマイクロスフェアを洗浄した。洗浄後に、1滴のブランクマイクロスフェア集団を添加した。マイクロスフェアを、まずフローサイトメトリーで分析して、試験専用の機器設定(PMT電圧及び補償)を設定した。同一の機器設定を用いて、マイクロスフェア及び標的細胞の幾何学的平均蛍光値を記録した。マイクロスフェア集団上の抗体結合部位の標準曲線を、マイクロスフェア集団の幾何学的平均蛍光値から生成した。標的細胞上の抗体結合部位を、QuickCalスプレッドシート(Bangs Laboratories)内でマイクロスフェアに関して生成された上記標準曲線を用いて、標的細胞の幾何学的平均蛍光値に基づいて算出した。
接着性標的腫瘍細胞を、0.25%トリプシン‐EDTA溶液を用いて引き離し、1000rpmで5分間の遠心分離によって回収した。懸濁した標的細胞株を培地から採取し、アッセイ媒体で洗浄した。細胞濃度及び生存力を、Beckman Coulter Vi‐Cellカウンタを使用して、トリパンブルー除外によって測定した。標的細胞を、アッセイ媒体中で4×105細胞/mLに希釈した。希釈した細胞懸濁液50μLを、96ウェルU字型底細胞培養処置済みプレート(BD Falcon Cat#353077)に添加した。
1)MR:CD123×CD3二重特異性ダイアボディを有しない200μLのアッセイ媒体及び50μLの標的細胞;実験の最後に洗浄剤を添加して、最大LDH放出を決定する。
2)AICC:CD123×CD3二重特異性ダイアボディを有しない50μLのアッセイ媒体、50μLの標的細胞、及び100μLのT細胞。
3)SR:CD123×CD3二重特異性ダイアボディを有しない150μLのアッセイ媒体及び50μLの標的細胞。

CD123×CD3二重特異性ダイアボディは、高いCD123発現を有する標的細胞株Kasumi‐3(EC50=0.01ng/mL)(図4パネルD)、中程度のCD123発現を有する標的細胞株Molm13(EC50=0.18ng/mL)及びTHP‐1(EC50=0.24ng/mL)(それぞれ図4パネルC及びE)、並びに中低程度の又は低いCD123発現を有する標的細胞株TF‐1(EC50=0.46ng/mL)及びRS4‐11(EC50=0.5ng/mL)(それぞれ図4パネルB及びA)におけるCD3エピトープ結合特異性(DART‐A対DART‐B)とは無関係に、50%の最大活性(EC50)を達成するために必要なng/ml以下の範囲での濃度を用いて、潜在的な再指向殺滅能力を呈した。同様に、CD123×CD3二重特異性分子媒介性再指向殺滅は、異なる供与体からのT細胞を有する複数の標的細胞株を用いて観察され、またCD123を発現しない細胞株では再指向殺滅活性は観察されなかった。結果を表4にまとめる。
配列最適化CD123×CD3二重特異性ダイアボディは、高いCD123発現を有する標的細胞株Kasumi‐3及び中程度のCD123発現を有するTHP‐1(それぞれ図5パネルA及びB)において、半減期延長技術の存在又は不在に関係なく(DART‐A対DART‐A w/ABD対DART‐A w/Fc)、潜在的な再指向殺滅能力を呈した。配列最適化CD123×CD3二重特異性ダイアボディ媒介性再指向殺滅プロセス中のT細胞活性化を特性決定するために、再指向殺滅アッセイからのT細胞を、T細胞活性化マーカCD25に関して染色し、FACSで分析した。図5パネルDに示すように、CD25は、CD8T細胞中において用量依存的に発現増加され、これは配列最適化CD123×CD3二重特異性ダイアボディが、再配向殺滅のプロセスにおいてT細胞活性化を誘発することを示している。対照的に、標的細胞の不在時、CD8T細胞の活性化は発生せず(図5パネルC)、これは、配列最適化CD123×CD3二重特異性ダイアボディが、標的細胞の不在時にT細胞を活性化させないことを示している。同様に、CD8T細胞は、標的細胞及び対照二重特異性ダイアボディ(対照DART)を用いてインキュベートした場合に活性化されず(図5パネルD)、これは、配列最適化CD123×CD3二重特異性ダイアボディを用いてT細胞及び標的細胞を架橋する必要があることを示している。
T細胞におけるグランザイムB及びパーフォリンの細胞内レベルを決定するために、CTLアッセイを上述のように構築した。約18時間後、4℃で30分間インキュベートすることにより、アッセイプレートからの細胞を抗CD4及び抗CD8抗体で染色した。表面染色に続いて、細胞を、100μLの固定及び易透化緩衝液(BD BioSciences)中において4℃で20分間インキュベートした。細胞を易透化/洗浄緩衝液(BD BioSciences)で洗浄し、(1×易透化/洗浄緩衝液中で調製した)グランザイムB及びパーフォリン抗体混合物50μL中において、4℃で30分間インキュベートした。続いて細胞を、250μLの易透化/洗浄緩衝液で洗浄して、FACS取得のために易透化/洗浄緩衝液中に再懸濁した。
T細胞による配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A)媒介性細胞毒性に関する可能性のある機序を調査するために、再指向殺滅後のT細胞において、細胞内グランザイムB及びパーフォリンレベルを測定した。DART‐Aを用いたT細胞及びKasumi‐3のインキュベーション後、CD8及びCD4T細胞両方において、グランザイムB及びパーフォリンレベルの用量依存的発現増加が観察された(図6パネルA)。興味深いことに、発現増加はCD8T細胞において、CD4T細胞に比べて略2倍高かった(図6パネルA)。グランザイムB及びパーフォリン阻害剤の存在下でアッセイを実施した場合、細胞殺滅は観察されなかった。T細胞をKasumi‐3標的細胞及び対照二重特異性ダイアボディ(対照DART)でインキュベートした場合、CD8又はCD4T細胞においてグランザイムB又はパーフォリンの発現増加は発生しなかった(図5パネルB)。これらのデータは、DART‐A媒介性標的細胞殺滅が、グランザイムB及びパーフォリン機序を通して媒介され得ることを示している。
ヒト全血からのPBMC及びT細胞の単離
Ficoll勾配遠心分離を用いて、健康なヒト供与体からのPBMCを全血から単離した。要約すると、全血を無菌PBSで1:1に希釈した。希釈した血液35mLを、50mLチューブ中の15mLのFicoll‐Paque(商標)Plusに積層し、このチューブを1400rpmで20分間、制動を外して遠心分離した。2つの相の間の軟膜層を50mLチューブ内に回収し、45mLのPBSを用いて、このチューブを600×g(1620rpm)で5分間遠心分離することにより洗浄した。上清液を廃棄し、細胞ペレットをPBSで1回洗浄し、トリパンブルー染料排除によって生存細胞数を決定した。PBMCを、完全培地(RPMI 1640、10%FBS、2mMグルタミン、10mM HEPES100μ/100μ/mLペニシリン/ストレプトマイシン(P/S))中で、2.5×106細胞/mLの最終濃度に再懸濁した。
Dynabeads未結合ヒトT細胞単離キット(Life Technologies)を製造元の指示に従って使用して、ヒト全血からのPBMCからのネガティブ選択によって、未結合T細胞を単離した。単離後、10%FBS、1%ペニシリン/ストレプトマイシンを含むRPMI媒体中でT細胞を一晩培養した。
ヒトT細胞及び腫瘍細胞(Molm13又はRS4‐11)を1:5の比(それぞれ1×106及び5×106)で組み合わせ、200μLの無菌食塩水中に懸濁し、研究日数0日目(SD0)に皮下(SC)注射した。配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A)又は対照二重特異性ダイアボディ(対照DART)を、表5(MOLM13)及び表6(RS4‐11)に概説したような100μLの尾静脈注射によって静脈内(IV)投与した。
動物の体重‐個々の動物の体重を、腫瘍細胞注射時に開始して研究の完了まで1週間に2回記録した。
動物を、全体的な瀕死状態に関して1週間に2回、死亡率に関して毎日観察した。動物の死は、全体的観察及び体重減少を含む、薬剤関連又は技術的ベース因子として評価した。動物の死を毎日記録した。
個々の腫瘍体積を、腫瘍埋入の1週間以内に開始して1週間に2回記録し、研究の完了まで継続した。
腫瘍成長阻害(TGI)値を、処置される動物を包含する各グループに関して、以下の式を用いて算出した。
1日目に1mm3未満と測定された腫瘍を有する個々のマウスを、部分的応答(PR)を有するものとして分類し、%腫瘍退縮(%TR)値を、以下の式を用いて決定した。
処置対象のグループと対照グループとの間で統計的分析を実施し、腫瘍体積を比較した。これらの分析に関して、分散の双方向分析及びこれに続くBonferroniポスト試験を採用した。全ての分析は、GraphPad PRISM(登録商標)ソフトウェア(バージョン5.02)を用いて実施した。技術的な又は薬剤関連の死を経験した個々の動物からの重量及び腫瘍データを分析から削除した。しかしながら、部分的な又は完全な応答が報告された動物からの腫瘍データはこの算出に含めた。
AML細胞株MOLM13を、活性化したT細胞と予備混合し、上述のようにSD0において、NOD/SCIDガンマ(NSG)ノックアウトマウス(N=8/グループ)にSC埋入した。ビヒクル処置グループのMOLM13腫瘍(MOLM13細胞単独、又はT細胞と共に)は、インビボで比較的激しい成長プロファイルを示した(図7パネルA及びB)。SD8において、ビヒクル処置グループの腫瘍の平均体積は129.8±29.5mm3であり、SD15までに腫瘍は平均体積786.4±156.7mm3に達した。SD18における実験の終了までに、腫瘍は1398.8±236.9mm3に達した。
ALL細胞株RS4‐11を、活性化したT細胞と予備混合し、上述のようにSD0において、NOD/SCIDガンマノックアウトマウス(N=8/グループ)にSC埋入した。ビヒクル処置グループのRS4‐11腫瘍(RS4‐11細胞単独、又はT細胞と共に)は、インビボで比較的激しい成長プロファイルを示した(図8)。
AML患者1初代サンプルにおけるCD123発現パターンを定義するために、低温保存された初代AML患者骨髄及びPBMCサンプルを、白血病芽細胞におけるCD123表面発現に関して評価した。

T細胞による配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A)媒介性細胞毒性に関する可能性のある機序を調査するために、再指向殺滅後のT細胞において、細胞内グランザイムB及びパーフォリンレベルを測定した。対照二重特異性ダイアボディ(対照DART)を用いてT細胞をインキュベートした場合、グランザイムB及びパーフォリンの発現増加は発生しなかった。配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A)を用いて、CD8及びCD4T細胞の両方におけるグランザイムB及びパーフォリンレベルの発現増加が観察された(図17パネルC及びD)。興味深いことに、発現増加はCD8T細胞において、CD4T細胞に比べて略2倍高かった(図17パネルC及び図17パネルD)。これらのデータは、DART‐A媒介性標的細胞殺滅が、グランザイムB及びパーフォリン経路を通して媒介されることを示している。
配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A)とヒト又はカニクイザルCD3との間の結合の程度を定量化するために、BIACORE(商標)分析を実施した。BIACORE(商標)分析は、解離オフレートkdを測定する。抗体とその標的との間の結合アフィニティ(KD)は、式:KD=[kd]/[ka]に従った、会合に関する動力学定数(オンレート、ka)及び解離(オフレート、kd)の関数である。BIACORE(商標)分析は、表面プラズモン共鳴を使用して、これらの動力学的パラメータを直接測定する。組み換えヒト又はカニクイザルCD3を支持体に直接固定した。精製されたヒト又はカニクイザルCD123を捕捉して支持体に固定した。解離の期間を測定し、データの2価フィットを実施した。結合定数及びアフィニティを、1:1結合フィットを用いて得た。ヒト対カニクイザルCD123及びCD3タンパク質への結合を比較するBIACORE(商標)分析の結果を、図18に示す。カニクイザルCD123(図18D)及びCD3(図18B)タンパク質への結合アフィニティは、ヒトCD123(図18C)及びCD3(図18A)タンパク質に関する結合アフィニティに匹敵する。
ヒト又はカニクイザル全血サンプルからのPBMCを、アッセイ媒体150μL中に200,000細胞/ウェルの細胞密度で、U字型底プレートに添加した。配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A又はDART‐A w/ABD)の希釈物を、アッセイ媒体中で調製した。DART‐A又はDART‐A w/ABD希釈物それぞれ50μLを、複製ウェル中にPBMCを含有するプレートに添加した。プレートを37℃で〜18〜24時間に亘ってインキュベートした。上清液を用いて、上述のように細胞毒性を決定した。図19(パネルA及びB)に示すように、pDC細胞の枯渇を、ヒトPBMC(図19パネルA)及びカニクイザルPBMC(図19パネルB)において観察した。これらの結果は、循環pDCを、カニクイザルにおける前臨床毒性研究に関する薬物動態マーカとして使用できることを示している。
用量範囲を見出すための毒性研究の一部として、カニクイザルに、配列最適化CD123×CD3二重特異性ダイアボディ(DART‐A)を、0.1、1、10、30、100、300又は1000ng/kgの用量での4日間の点滴として投与した。対照DARTを100ng/kgで投与した。カニクイザルPBMC中のpDc及び単球集団を識別するために、細胞をCD14‐FITC抗体で標識した。単球はCD14+集団として識別され、pDCはCD14‐CD123+集団として識別された。図20パネルK及びLに示すようにpDCは、僅か10ng/kgのDART‐Aを用いた4日間の点滴後すぐに枯渇した。対照二重特異性ダイアボディ(対照DART)で処置されたサル、又はビヒクル+キャリアで処置されたサルにおいては、4日目の時点でpDCの枯渇は見られなかった(それぞれ図20パネルG、H、C及びD)。インターフェロン‐γ、TNF‐α、IL6、IL5、IL4及びIL2のサイトカインレベルを、点滴後4時間の時点で決定した。DART‐Aで処置された動物においては、対照DART又はビヒクルで処置された動物と比べて、サイトカインレベルの上昇は殆ど又は全く見られなかった。
配列最適化CD123×CD3二重特異性ダイアボディの予期しない利点及び属性

非ヒト霊長類の実験を、Charles River Laboratories(ネバダ州レノ)において、その地域の動物実験委員会(IACUC)のガイドラインに従って実施した。バッテリ駆動プログラマブル点滴ポンプ(CADD‐Legacy(登録商標)、SIMS Deltec, Inc.,ミネソタ州セントポール)を用いた、大腿及び頸静脈ポートを通した静脈内点滴により、研究用に繁殖させた中国産のナイーブなカニクイザル(Macaca fascicularis)(年齢2.5〜9歳、体重2.7〜5kg)にビヒクル又はDART‐Aを供給した。示されている時点において、末梢血又は骨髄サンプルを抗凝固剤含有チューブに回収した。488nm、640nm及び405nmレーザ並びに以下の抗体:CD4‐V450、CD8‐V450、CD123‐PE‐Cy7、CD45‐PerCP、CD4‐APC‐H7、CD8‐FITC、CD25‐PE‐Cy7、CD69‐PerCP、PD‐1‐PE、TIM3‐APC、CD3‐PacificBlue、CD95‐APC、CD28‐BV421、CD16‐FITC、CD3‐Alexa488、CD38‐PE、CD123‐PE‐Cy7、CD117‐PerCP‐Cy5.5、CD34‐APC、CD90‐BV421、CD45RA‐APC‐H7及びCD33‐APC(BD Biosciences)を備えたLSR Fortessaアナライザ(BD Biosciences)を用いて、細胞表面表現型分析を実施した。TruCOUNT(BD Biosciences)を用いて細胞の絶対数を決定した。非ヒト霊長類Th1/Th2サイトカインサイトメトリックビードアレイキット(BD Biosciences)を用いて、IL‐2、IL‐4、IL‐5、IL‐6、TNF‐α及びIFN‐γサイトカインの血清レベルを測定した。電気化学発光検出によるサンドイッチイムノアッセイ(MesoScale Diagnostics、MSD、メリーランド州ロックヴィル)を用いて、サル血清サンプル中のDART‐Aの濃度を測定した。要約すると、アッセイプレート(MSD)は、組み換えヒトIL‐3Ra(R&D System)でコーティングし、5%BSAでブロックした。較正標準又は希釈試験サンプルを適用し、続いて、本分子の上述のEコイル(配列番号34)及びKコイル(配列番号35)ドメインに対する特異的結合を呈するビオチン化モノクローナル抗体を添加した。SULFO‐TAG(商標)標識ストレプトアビジンコンジュゲート(MSD)を添加し、錯体の構成をMSD SECTOR(登録商標)イメージャで分析した。5パラメータロジスティックモデルに光強度データをフィッティングすることによって生成された標準曲線から、DART‐A濃度を決定した。
DART‐Aは、ヒト又はサルエフェクタ細胞による、CD123+Kasumi‐3白血病細胞株に対する再指向標的細胞殺滅を媒介し(図27A〜27D)、これは活性化マーカの誘発を伴った。CD123‐ネガティブ標的(U937細胞)に対しては、又は対照DARTを用いると、活性は観察されず、これはT細胞活性化が厳密に標的細胞エンゲージメントによるものであること、及びDART‐AによるCD3の1価エンゲージメントはT細胞活性化をトリガするには不十分であることを示している。CD123は、pDC及び単球を含む正常な循環白血球のサブセットによって発現されるため(図27E)、正常なヒト及びサルのPBMCにおいてDART‐Aの効果を更に調査した。
カニクイザルは、前駆細胞mAbを用いた免疫組織化学的検査に基づき、公開されている情報(Munoz, L. et al.(2001)“Interleukin‐3 Receptor Alpha Chain (CD123) Is Widely Expressed In Hematologic Malignancies,” Haematologica 86:1261‐1269; Korpelainen, E.I. et al. (1996) “IL‐3 Receptor Expression, Regulation And Function In Cells Of The Vasculature,” Immunol. Cell Biol. 74:1‐7)の通り、この種において2つの標的抗原両方がヒトに比べて均等に分布していることに基づいて、DART‐A分析のための適切な薬学的モデルとして選択された。
DART‐AのT細胞活性化特性を仮定すると、点滴によって発生する循環サイトカインの増大が予想され、従って同様の化合物を用いた過去の実験に基づき、「脱感作」戦略として、低い開始用量を使用した(例えばTopp, M.S. et al.(2011) “Targeted Therapy With The T‐Cell‐Engaging Antibody Blinatumomab Of Chemotherapy‐Refractory Minimal Residual Disease In B‐Lineage Acute Lymphoblastic Leukemia Patients Results In High Response Rate And Prolonged Leukemia‐Free Survival,” J. Clin. Oncol. 29:2493-2498; Bargou, R. et al.(2008)“Tumor Regression In Cancer Patients By Very Low Doses Of A T Cell-Engaging Antibody,” Science 321:974‐977を参照)。試験対象のサイトカインについて、IL‐6は、最小量の点滴によって、実際には一過性のものではあるが最大の変化を示し、動物個体間及びグループ間の変動が大きかった(図29A〜29C)。また、ビヒクル点滴(グループ1の全ての点滴及び1日目の全ての点滴)後にはIL‐6の小さな一時的増加が観察され、これはこのサイトカインの、操作によるストレスに対する感受性を示している。それにも関わらず、血清IL‐6のDART‐A依存性の増加(<80pg/mL)が、最初のDART‐A点滴(100ng/kg/日)の後に一部のサルにおいて観察され、これは72時間後までにベースラインに戻った。興味深いことに、用量レベルを最大1000ng/kg/日まで上昇させた場合でさえ、連続的なDART‐A点滴それぞれによってIL‐6放出の量は低下した。血清TNF‐αの極微で一時的なDART‐A関連増加(<10pg/mL)も観察され、IL‐6と同様に、TNF‐α放出の最大量は、最初の点滴の後に観察された。IL‐5、IL‐4、IL‐2又はIFN‐γのレベルにおいては、研究全体を通して、対照に比べてDART‐A関連の変化は発生しなかった。結論として、DART‐Aを用いたサルの処置に応答したサイトカイン放出は極微で一時的であり、最初の投薬による効果を被験体内での用量の段階的上昇によって管理できることが示された。
CD14‐/CD123+細胞の循環絶対レベルを、薬力学的エンドポイントとして研究全体を通して測定した。対照グループ1のCD123+細胞の数が経時的に安定したままであるのに対して、DART‐A処置は、全てのアクティブな処置グループ全体の全ての動物において、最初のDART‐A点滴(100ng/kg/日)の開始後に測定された最初の時点(72時間)から観察可能な、循環CD14‐/CD123+細胞の大幅な枯渇(研究前ベースラインから94〜100%)に関連した(図30A〜30C)。この枯渇は、グループ2〜5の週3日の投薬を行わない日の間にも持続するため、永続的なものであり、長期の回復期間中にやっとベースラインレベルに戻った。DART‐AがCD123をマスキング又は変調する可能性(これは低い循環DART‐Aレベルの場合には発生しにくい状況である)を排除するために、直交マーカCD303によってpDCを計数した。CD123のデータと矛盾することなく、CD303+pDCは、DART‐Aを用いて処置されたサルにおいて同様に枯渇した(図30D〜30F)。
循環CD123+細胞の持続性枯渇とは対照的に、4日オン/3日オフスケジュール(グループ2〜5)で投与されたDART‐Aは、循環T細胞の週単位での変調に関連し、その一方で7日連続の点滴としての投与は同様に、最初の投与に続いて循環T細胞レベルの低下をもたらし、これは投薬期間中であっても変動することなく徐々に回復した(図31A〜31C)。これら2つの投薬戦略間のこのような違いは、Tリンパ球に対するDART‐Aの効果が、枯渇ではなく輸送及び/又は辺縁趨向に整合するものであることを示している。投薬の注視後、T細胞は、回復期間の間にベースラインの約2倍高いレベルまでリバウンドした。DART‐Aの点滴は、特にCD4+細胞において、T細胞が後期活性化マーカPD‐1を発現する、曝露量依存性の、漸進的に上昇する頻度に関連しており、投薬グループ6が最も高い全体的なレベルを示した(図31D〜31I及び図32A〜32F及び図33A〜33F)。T細胞の消耗に関連するマーカであるTim‐3はCD4+T細胞では検出されず、CD8+細胞では低い頻度(5.5〜9.7%)に留まり、CD8+/PD‐1+二重陽性細胞の20.5〜35.5%を含んでいた。早期T細胞活性化マーカCD69については一貫した変化は見られず、循環細胞中のCD25発現については穏やかな変動しか見られなかった。
DART‐Aは、試験した全ての用量においてサルにおいて十分に許容されたが、最も高い用量においては、赤血球パラメータの可逆的な低下が観察された(図36A〜36C)。ビヒクルで処置された動物は赤血球量の減退が穏やかであったため、頻繁な血液のサンプリングが潜在的な寄与因子であった可能性がある。網状赤血球応答が全ての動物において観察された。しかしながら、曝露量が最高の場合(グループ6)、上記応答は、赤血球量の同様の減少に関して、僅かに堅牢性が低かった(図36D〜36F)。研究全体を通しての骨髄塗抹標本の形態学的分析は、特筆すべきものではなかった。しかしながら、フローサイトメトリー分析によって、未熟系統ネガティブ(Lin‐)骨髄集団内のCD123+細胞の頻度は、DART‐Aで処置された動物において、投薬期間の終了時に低下しており、回復期間の終了までにベースラインレベルに戻った(図37A〜37B)。(Lin‐/CD34+/CD38‐/CD45RA‐/CD90+細胞(Pang, W.W. et al.(2011) “Human Bone Marrow Hematopoietic Stem Cells Are Increased In Frequency And Myeloid‐Biased With Age,” Proc. Natl. Acad. Sci. (U.S.A.) 108:20012‐20017)として定義される)HSCは、大きなグループ間変動を示した。グループ4〜6のDART‐Aで処置されたサルは、対応する投薬前レベルに比べてある程度明らかな減少を示すが、全ての処置グループにおいて、ビヒクルで処置された動物に比べて減少は見られなかった。これらのデータは、HSCが、DART‐Aによる標的化を受けにくいことを示しており、また、造血に対するDART‐A処置の負の影響の観察された可逆性と整合する。
Claims (20)
- CD123のエピトープ及びCD3のエピトープに特異的に結合できる、配列最適化ダイアボディであって、
前記ダイアボディは、互いに共有結合した第1のポリペプチド鎖及び第2のポリペプチド鎖を含み:
A.前記第1のポリペプチド鎖は、N末端からC末端の方向に:
i.ドメイン1であって:
(1)CD3に結合できるモノクローナル抗体のVLドメイン(VLCD3)(配列番号21)を含むサブドメイン(1A);及び
(2)CD123に結合できるモノクローナル抗体のVHドメイン(VHCD123)(配列番号26)を含むサブドメイン(1B)
を含み、前記サブドメイン1A及び前記サブドメイン1Bはペプチドリンカー(配列番号29)によって互いから分離されている、ドメイン1;
ii.ドメイン2であって、前記ドメイン2はEコイルドメイン(配列番号34)又はKコイルドメイン(配列番号35)であり、また前記ドメイン2はペプチドリンカー(配列番号30)によって前記ドメイン1から分離されている、ドメイン2
を含み;
B.前記第2のポリペプチド鎖は、N末端からC末端の方向に:
i.ドメイン1であって:
(1)CD123に結合できるモノクローナル抗体のVLドメイン(VLCD123)(配列番号25)を含むサブドメイン(1A);及び
(2)CD3に結合できるモノクローナル抗体のVHドメイン(VHCD3)(配列番号22)を含むサブドメイン(1B)
を含み、前記サブドメイン1A及び前記サブドメイン1Bはペプチドリンカー(配列番号29)によって互いから分離されている、ドメイン1;
ii.ドメイン2であって、前記ドメイン2はKコイルドメイン(配列番号35)又はEコイルドメイン(配列番号34)であり、また前記ドメイン2はペプチドリンカー(配列番号30)によって前記ドメイン1から分離されており;かつ前記第1のポリペプチド鎖の前記ドメイン2及び前記第2のポリペプチド鎖の前記ドメイン2は、両方がEコイルドメインではなく、又は両方がKコイルドメインではない、ドメイン2
を含み、かつ;
(a)前記第1のポリペプチド鎖の前記VLドメイン及び前記第2のポリペプチド鎖の前記VHドメインは、CD3のエピトープに特異的に結合できる抗原結合ドメインを形成し;
(b)前記第2のポリペプチド鎖の前記VLドメイン及び前記第1のポリペプチド鎖の前記VHドメインは、CD123のエピトープに特異的に結合できる抗原結合ドメインを形成する、配列最適化ダイアボディ。 - 前記第1のポリペプチド鎖は更に、ペプチドリンカー(配列番号31)を介して前記ドメイン2に連結した、アルブミン結合ドメイン(配列番号36)を含む、請求項1に記載のダイアボディ。
- 前記第2のポリペプチド鎖は更に、免疫グロブリンFcドメインのCH2及びCH3ドメイン(配列番号37)を含むドメイン3を含み、
前記ドメイン3は、ペプチドリンカー(配列番号33)を介して前記ドメイン1Aに連結する、請求項1に記載のダイアボディ。 - 前記第2のポリペプチド鎖は更に、免疫グロブリンFcドメインのCH2及びCH3ドメイン(配列番号37)を含むドメイン3を含み、
前記ドメイン3は、ペプチドリンカー(配列番号32)を介して前記ドメイン2に連結する、請求項1に記載のダイアボディ。 - 前記第1のポリペプチド鎖の前記ドメイン2はKコイルドメイン(配列番号35)であり、前記第2のポリペプチド鎖の前記ドメイン2はEコイルドメイン(配列番号34)である、請求項1〜4のいずれか1項に記載のダイアボディ。
- 前記第1のポリペプチド鎖の前記ドメイン2はEコイルドメイン(配列番号34)であり、前記第2のポリペプチド鎖の前記ドメイン2はKコイルドメイン(配列番号35)である、請求項1〜4のいずれか1項に記載のダイアボディ。
- 前記ダイアボディは、ヒト及び霊長類両方のCD123及びCD3タンパク質と交差反応できる、請求項1〜6のいずれか1項に記載のダイアボディ。
- CD123のエピトープ及びCD3のエピトープに特異的に結合できる二重特異性ダイアボディであって、
前記ダイアボディは、互いに共有結合した第1のポリペプチド鎖及び第2のポリペプチド鎖を含み、
前記二重特異性ダイアボディは:
A.配列番号1のアミノ酸配列を有する第1のポリペプチド鎖;及び
B.配列番号3のアミノ酸配列を有する第2のポリペプチド鎖
を含み、
前記第1のポリペプチド鎖及び前記第2のポリペプチド鎖は、ジスルフィド結合によって互いに共有結合する、二重特異性ダイアボディ。 - 医薬として使用するための、請求項1〜8のいずれか1項に記載のダイアボディ。
- CD123の発現に関連する又はCD123の発現を特徴とする疾患又は状態の治療において使用するための、請求項8に記載のダイアボディ。
- CD123の発現に関連する又はCD123の発現を特徴とする前記疾患又は状態は、癌である、請求項10に記載のダイアボディ。
- 前記癌は:急性骨髄性白血病(AML);CML及びCMLに関連したアベルソン癌遺伝子の急性転化(Bcr‐ABL転座)を含む、慢性骨髄性白血病(CML);骨髄異形成症候群(MDS);急性Bリンパ芽球性白血病(B‐ALL);リヒター症候群又はCLLのリヒター転化を含む、慢性リンパ性白血病(CLL);有毛細胞白血病(HCL);芽形質細胞様樹状細胞腫瘍(BPDCN);マントル細胞白血病(MCL)及び小リンパ球性リンパ腫(SLL)を含む非ホジキンリンパ腫(NHL);ホジキンリンパ腫;全身性肥満細胞症;並びにバーキットリンパ腫からなる群から選択される、請求項11に記載のダイアボディ。
- CD123の発現に関連する又はCD123の発現を特徴とする前記疾患又は状態は、炎症状態である、請求項10に記載のダイアボディ。
- 前記炎症状態は:自己免疫性狼瘡(SLE);アレルギー;喘息;及び関節リウマチからなる群から選択される、請求項11に記載のダイアボディ。
- 請求項1〜8のいずれか1項に記載のダイアボディ及び生理学的に許容可能なキャリアを含む、医薬組成物。
- CD123の発現に関連する又はCD123の発現を特徴とする疾患又は状態の治療における、請求項15に記載の医薬組成物の使用。
- CD123の発現に関連する又はCD123の発現を特徴とする前記疾患又は状態は、癌である、請求項16に記載の使用。
