Abstract
Description
本発明は、アセチルコリンエステラーゼ治療の補助療法としてのアルツハイマー病の治療方法であって、N−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンまたは薬学的に許容される塩を有効1日量(effective daily dose)、このような治療を必要とする患者に投与することを含み、患者に投与される有効1日量が約30〜約60mgである方法をさらに提供する。
別の実施形態では、用量は速放性(immediate release)製剤として投与される。
さらに別の実施形態では、方法は、軽度から中度のアルツハイマー病の治療に関する。
別の実施形態では、アセチルコリンエステラーゼ阻害剤はドネペジルである。

本発明はさらに、治療有効量の化合物Iと、任意選択により薬学的に許容される担体または賦形剤とを含む医薬組成物を提供する。化合物Iは、単独でまたは薬学的に許容される担体、賦形剤もしくは医薬品添加物と組み合わせて、単回投与または複数回投与することができる。本発明の医薬組成物は、薬学的に許容される担体または賦形剤ならびに他の任意の既知の補助剤および医薬品添加物と共に、Remington:The Science and Practice of Pharmacy,19th Edition,Gennaro,Ed.,Mack Publishing Co.,Easton,PA,1995に開示されているものなどの従来の技術に従って製剤化することができる。

同様に、医薬組成物は、投与される用量範囲が約30mg〜約60mgとなる化合物Iの投与を含むように製造されてもよい。
本明細書では、軽度、中度および重度のアルツハイマー病、ならびに軽度〜中度のアルツハイマー病に関連する症状の治療に有用な併用療法を提供する。後述のように、本明細書で提供される本方法には幾つかの利点がある。
前駆体は、3−フェニルスルホニル−8−(ピペラジン−1−イル)キノリン(C19H19N3O2S;MW:353.4)であり、既知の公開された化合物である。この前駆体をアセトニトリルに溶解し、BioScan Autoloop Systemに移し、次のように製造した[11C]−ヨードメタンと反応させた。0.5〜1.0%の酸素を含有する高純度窒素ガスに加速プロトンを照射することにより製造した11CO2を、水素とモレキュラーシーブ:ニッケル触媒カラム上で380℃で反応させて11CH4を生成し、それを740℃に加熱したヨウ素蒸気と反応させて11CH3Iを生成した。[11C]−ヨードメタンを、銀トリフラートを収容する炉を通過させて、放射標識したヨードメタンを[11C]−メチルトリフラートに変換した。周囲温度で約20mL/分の流量のヘリウムガスを用いて、[11C]−メチルトリフラートをループメチル化装置に送入した。放射能がプラトーに達するまで、局所放射線監視装置でループ内の[11C]−放射能の集積を監視した。ループ内の反応混合物を室温で4.5分間残留させた。Waters XBridge Prep OBD C18 10μm 10×150mmカラムを使用し、30%アセトニトリル:70%水性緩衝液(o−リン酸でpH7.2に調節した57mM TEA)を用い、10mL/分の流量を使用する分取高圧液体クロマトグラフィー(HPLC)により、粗[11C]LuPETを精製した。インライン放射測定検出器で決定した[11C]LuPET画分を水のリザーバ内に回収した。リザーバを加圧して[11C]LuPETをC18 Sep−Pakにロードした。次いで、C18 Sep−Pakを注射用の0.9%塩化ナトリウム10mLで洗浄した。エタノール1mL、続いて0.9%塩化ナトリウム注射液10mLで[11C]LuPETをC18 Sep−Pakから溶出し、塩化ナトリウム注射液4mLを予めロードしたパイロジェンを含まない滅菌バイアル内の滅菌0.22μフィルタ―を通過させる。
放射性リガンドはこれまでにヒトに投与されてこなかったため、定量に最適な方法を特定し、PETトレーサーとしての放射性リガンドのバリデーションを行うために、それを最初にヒトの脳内で評価した(A部)。この試験の主要な目的は、健常対象に化合物Iを複数回経口投与した後の5−HT6受容体の占有率を、PETを使用し、[11C]−LuPETを放射性リガンドとして用いて評価することであった(B部)。
8人の健常男性対象(年齢:30.6±7.7才;範囲:22〜44才)がこの試験のA部に参加し、16人の健常男性対象(年齢:32.3±7.6才;範囲:21〜44才)がB部に参加した。
PET試験は、GE Advance Tomograph(GE Medical Systems,Waukesha,WI,USA)で行った。対象は、放射性リガンド注入用の静脈カテーテルを1つと、血漿中の放射能を測定するための動脈血試料を得るための動脈カテーテルを1つ有した。次いで、対象は、PETデータ取得中の頭部の動きを低減するために、注文製の熱可塑性マスクで頭部を固定して走査装置内に位置決めされた。次いで、減衰補正用の回転68Ge源を使用して10分間の減衰走査を行った。次いで、[11C]LuPETの静脈内ボーラス注入を行った後、90分間三次元モードでダイナミックPET取得を行った。合計30のPETフレームを得た(4×15、4×30、3×60、2×120、5×240、および12×300秒)。最初は動脈血試料を非常に短い間隔(<5秒)で採取し、血漿中放射能を測定するためにPET試験全体を通して間隔を徐々に長くした(30分後には15分毎)。0分、5分、10分、30分、45分、60分および90分の時点で採取した選択された試料について、放射性リガンドおよび他の箇所(Hilton et al.,2000)に記載するその放射性代謝物の有無をHPLCで分析した。
別の機会に、次のパラメータ:繰り返し時間、35ms;エコー時間、6ms;フリップ角、458°;スライス厚、1.5mm、ギャップなし;視野、24×18cm2;画像取得マトリクス、256×192、256×256に再フォーマットを使用して、関心構造の解剖学的同定を行うために各対象についてスポイルド・グラジエント(SPGR)シーケンスMRIを得た。
関心容積(VOI):Freesurferソフトウェアを使用して皮質VOIを自動的に画定し、前頭(Fr)皮質、側頭(Tp)皮質、頭頂(Pa)皮質、および後頭(Oc)皮質、紡錘状回(Fs)、帯状(Cg)、および島(In)を含む標準領域に組み合わせした。皮質下領域をFIRSTソフトウェア(Patenaude et al.,2011)で画定し、個々のMRIについて手動で調節を行った。皮質下領域には、被殻(Pu)、尾状核(CN)、腹側線条体(vS)、淡蒼球(GP)、視床(Th)、海馬(Hp)、および扁桃(Am)が含まれた。SPM5(Ashburner J,Friston 2003;Maes et al.,1997)により得られる、MRIからPETへの位置合わせ(coregistration)パラメータに従ってVOIをMRI空間からPET空間に移行させ、領域の時間放射能曲線(TAC)を得た。方法論的評価を行うために1走査当たり合計25の領域を採用した(A部)が、占有率の算出はPu、CN、およびvSに限定された(B部)。
式中、v試験およびv再試験はそれぞれ、領域中の試験走査および再試験走査のVTまたはBPNDの推定値を指す。幾つかの論文では、放射性リガンドの許容可能な再現性の判定規準としてTRV10%が採用された(Hirvonen et al.,2009)。従って、本報告ではこのレベルを参照として使用した。この項では、A部の試験走査および再試験走査を単独で使用した。
式中、上付きはそれぞれベースライン(B)および投与後(D)のBPNDを示す。
式中、Omaxは到達可能な最高占有率を表し、EC50はOmaxの50%を達成するPKを指す。赤池情報量規準(AIC;Akaike 1974;Burnham and Anderson 2004)を使用して、コンパートメントモデルに関する、および占有率−PK関係の検定に関する適合度を、式3を使用して検定した。
A部およびB部からの次の結果については、本発明の図面の図を参照されたい。組織TAC:ベースライン走査で、PuおよびCNのTACは10分より前にピークを形成し、その後、90分間、連続した増加を示した(図1)が、他の脳領域のTACは20分より前に各ピークに達し、その後、単調に減少した。Cbは脳領域の中で最も低い放射能集積を示した。投与後の走査では、PuおよびCNのTACの形状は、用量依存的に他の脳領域のTACに近付いた(パネルB)。CbのTACはベースライン走査と投与後の走査との間で比較的類似したままであった。
この試験の方法論的評価の項では、広く認識されている標準的なPETデータ解析方法の中で、放射性リガンドPETデータからVT、BPNDおよび占有率を導出するのに最適な方法として、PRGAが特定された。プロットが成功した場合、CbのVT推定値はLassenプロットにより得られるVND推定値より僅かに低かった(が、それと相関した)ため、[11C]LuPETに関して、参照領域としてのCbの使用が正当化された。
実施例2のPET試験から得られたデータを使用し、アルツハイマー病母集団で、化合物Iを5.0〜60mgの範囲の1日1回投与量で複数回投与した後の5−HT6受容体占有率を年齢に応じて推定することがモデル構築およびシミュレーション演習の目的であった。
自己抑制モデルを使用したが、これはPlockらによる論文(Plock N.et al.Drug Metab Disp 2007,35:1816−1823)に基づいている。使用するpopPKモデルの構造を下記に記載する。
化合物Iを複数回投与した後の5−HT6占有率の評価に、16人の健常対象が含まれた。対象に化合物Iを少なくとも3日間、120(60BID)、60(30BID)、30mg/日(QD)または5mg(QD)の用量で投与し、各用量コホートに4人の対象が含まれた。合計で、対象1人当たり3回PET走査を行い、最初はベースライン時に、2回目は投与最終日のtmax付近で、最後は投与最終日の投与後10〜51時間の間隔を空けて行った。
試験12936Aのものに対して年齢に関するアルツハイマー病の母集団のシミュレーションを行った(範囲54〜90才、中央値75才)。5mg/日、10mg/日、15mg/日、20mg/日、25mg/日、30mg/日および60mg/日の用量に関して、且つ用量当たり1000人の患者で、化合物Iの定常状態での血漿中特性および対応する5−HT6受容体占有率のシミュレーションを行った。定常状態における1日間の尾状核領域内の平均5−HT6受容体占有率を各患者に関して推定した。中央値、5%および95%パーセンタイルを各用量群に関して個々の値から推定した。従って、患者の90%は、5%パーセンタイル〜95%パーセンタイルの区間に入ることになる。
PK/PDモデルからのデータを使用して、図12は5−HT6受容体占有率と化合物Iの血漿中濃度との投影(projected)相関を示す。AD試験での化合物Iの固定用量により得られる暴露(X軸上の強調表示された範囲を参照)から、試験の対象は5−HT6受容体の受容体占有率レベルが高かったことが分かる。

Claims (15)
- アセチルコリンエステラーゼ阻害剤の効果を改善または増強することによるアルツハイマー病の治療に使用されるN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンまたは薬学的に許容される塩であって、前記治療が前記化合物を有効1日量、このような治療を必要とする患者に投与することを含み、前記患者に投与される有効1日量が約30〜約60mgである、N−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンまたは薬学的に許容される塩。
- アセチルコリンエステラーゼ治療の補助療法としてアルツハイマー病の治療に使用されるN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンまたは薬学的に許容される塩であって、前記治療がN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンまたは薬学的に許容される塩を有効1日量、このような治療を必要とする患者に投与することを含み、前記患者に投与される有効1日量が約30〜約60mgである、N−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンまたは薬学的に許容される塩。
- 前記薬学的に許容される塩が塩酸塩である、請求項1または2に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 前記用量が速放性製剤として投与される、請求項1〜3のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 軽度〜中度のアルツハイマー病を治療するための、請求項1〜4のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 前記アセチルコリンエステラーゼ阻害剤がドネペジルである、請求項1〜5のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 前記アセチルコリンエステラーゼ阻害剤がリバスチグミンである、請求項1〜5のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- アセチルコリンエステラーゼ阻害剤がガラタミンである、請求項1〜5のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 前記有効1日量が30mgである、請求項1〜8のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 有効1日量が40mg以下である、請求項1〜8のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 前記有効1日量が50mg以下である、請求項1〜8のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 前記有効1日量が60mg以下である、請求項1〜8のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 前記有効1日量が60mgである、請求項1〜8のいずれか一項に記載のN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミン。
- 化合物Iを含む医薬組成物であって、前記組成物をヒトに投与する場合、化合物Iの有効1日量約60mg以下が提供されるように前記組成物を投与すると、定常状態の血漿中レベルで化合物Iの血漿中濃度が約56ng/mL〜約310ng/mLの範囲となり;化合物IがN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンである医薬組成物。
- 化合物Iを含む医薬組成物であって、前組成物をヒトに投与する場合、化合物Iの有効1日量約60mg以下が提供されるように前記組成物を投与すると、定常状態の血漿中レベルで5HT−6受容体における化合物Iの受容体占有率が約90%以上となり;化合物IがN−(2−(6−フルオロ−1H−インドール−3−イル)エチル)−3−(2,2,3,3−テトラフルオロプロポキシ)ベンジルアミンである医薬組成物。
