Claims (44)
- 対象とする組織欠損の開口部または腔内を修復し、小さな直径の形状を呈する搬送状態からより大きな直径の形状を呈する展開状態へと変形することのできる植え込み型デバイスであって、
前記デバイスが前記展開状態であるときに前記開口部または腔内を覆う寸法の閉塞構造体と、
前記デバイスが前記搬送状態であるときに前記閉塞構造体から第1の方向へ広がる第1のアンカー構造体および前記閉塞構造体から第1の方向と反対側の第2の方向へ広がる第2のアンカー構造体とを有し、
前記第1および第2のアンカー構造体は前記デバイスが前記展開状態であるときに閉塞構造体の直径よりも大きな直径の円周方向に広がる構造体をなす、植え込み型デバイス。 - 前記第1および第2のアンカー構造体の少なくとも一方に設けられた少なくとも1つの放射線透過性マーカーを含む、請求項1に記載の植え込み型デバイス。
- 前記第1および第2のアンカー構造体のそれぞれに設けられた少なくとも1つの放射線透過性マーカーを含む、請求項2に記載の植え込み型デバイス。
- 前記第1および第2のアンカー構造体の中間に設けられた放射線透過性マーカーを含む、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体に設けられた放射線透過性マーカーを含む、請求項1に記載の植え込み型デバイス。
- 前記放射線透過性マーカーは、タンタル、金、銀、バリウム、プラチナ、タングステンおよびこれらの化合物のグループから選択される1つの組成物を含む、
請求項2−5に記載の植え込み型デバイス。 - 前記閉塞構造体は、連続した十分な閉塞表面積を有する、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体は、他のデバイスの通過を容易にする少なくとも1つの開口部を有する、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体は、前記構造を再現し対象組織へ移動するのに十分な半径方向の可撓性を有する、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体は、シリコン材料;ゴム材料;織布または不織布;フッ素重合体組成物;ポリマー材料、ポリウレタン材料、金属材料およびこれらの化合物のグループから選択される1つの材料を含む、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体は薄膜形状記憶合金を含む、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体はニッケルチタニウム合金を含む、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体は、少なくともその表面の一部にポーラス表面構造を有する、
請求項1に記載の植え込み型デバイス。 - 前記閉塞構造体は、液体を透過させるが気泡は透過させない材料を含む、請求項1に記載の植え込み型デバイス。
- 前記閉塞構造体は、補強構造体を組み込んでいる、請求項1に記載の植え込み型デバイス。
- 前記補強構造体は、前記閉塞構造体の周辺部近傍にフレームワーク構造体を含んでいる、請求項15に記載の植え込み型デバイス。
- 前記閉塞構造体の周辺部近傍に設けられた襟構造体を含み、前記襟構造体は、ほぼ円筒形状の構造をなし、搬送状態において前記閉塞構造体の面とほぼ直角の面内に拡張する、請求項1に記載の植え込み型デバイス。
- 前記襟構造体は、可撓性メンブレン構造体に設けられたほぼ円筒形状の補強構造体を含む、請求項17に記載の植え込み型デバイス。
- 前記襟構造体は、前記デバイスが展開状態にあるときにほぼ円筒形状をなす、請求項17に記載の植え込み型デバイス。
- 前記襟構造体の少なくとも一部は、展開状態においてデバイス中央から角をなしている、もしくは湾曲している、請求項17に記載の植え込み型デバイス。
- 前記アンカー構造体は、非外傷性でかつ前記閉塞構造体の表面部分における密度よりも低い物質密度からなる表面部分を有している、請求項1に記載の植え込み型デバイス。
- 前記アンカー構造体は生体適合形状記憶合金材料を含む、請求項1に記載の植え込み型デバイス。
- 前記アンカー構造体はニッケルチタン合金を含む、請求項22に記載の植え込み型デバイス。
- 前記第1および第2のアンカー構造体は、ほぼ円形、ほぼ楕円形、ほぼ曲線、ほぼ多角形、ほぼ三角形、およびこれらを組み合わせた形状のグループから選択される1つの形状を有する、請求項1に記載の植え込み型デバイス。
- 前記第1および第2のアンカー構造体は、ほぼ同じ形状を有している、請求項1に記載の植え込み型デバイス。
- 前記第1および第2のアンカー構造体は、ほぼ同じ寸法を有している、請求項1に記載の植え込み型デバイス。
- 前記第1および第2のアンカー構造体は、異なる形状および/または寸法を有している、請求項1に記載の植え込み型デバイス。

- 細胞の成長と前記対象部分への付着を促進する物質;親水性物質;疎水性物質;接着剤;減摩剤;放射線不透過性物質;抗生物質製剤;血栓形成物質:非血栓形成物質;治療薬;ヒドロゲル成分:非炎症性物質;非再狭窄物質;放射性物質;これらを組み合わせた物質、のグループから選択される1つの材料を内蔵する、請求項1に記載の植え込み型デバイス。
- 対象とする組織欠損の開口部もしくは腔内を修復する植え込み型デバイスを有する搬送システムであって、
対象とする組織欠損の近傍へ搬送するための遠位端を有する搬送カテーテルと、植え込み型デバイスとを含み、
該植え込み型デバイスは、搬送状態においてほぼ円筒形状をなし、前記開口部もしくは前記腔内を覆う大きさの閉塞構造体と、該閉塞構造体から違いに反対方向へ伸びる第1および第2のアンカー構造体とを含む植え込み型デバイスとを含み、前記搬送カテーテルの遠位端部の内腔に位置している、搬送システム。 - 前記植え込み型デバイスの近傍に位置するプッシャーを含む、請求項34に記載の搬送システム。
- 搬送状態において植え込み型デバイスの近位端部に設けられたワイヤおよび取り外し可能な要素を有する離脱機構を含む、請求項34に記載の搬送システム。
- 少なくとも1つの放射線透過性マーカーが前記搬送カテーテルの遠位端に設けられている、請求項34に記載の搬送システム。
- 対象とする組織欠損の開口部もしくは腔内の修復方法であって、
(a) 第1のアンカー部と、第2のアンカー部と、前記第1および第2のアンカー部の中央に位置する閉塞構造体とを対象とする前記組織欠損部へ低侵襲性技術を用いて修復デバイスを案内すること、
(b) 前記第1のアンカー部を対象とする前記組織欠損部の開口部内で展開し、前記第1のアンカー部を前記開口部を囲む第1の組織表面の近傍に位置させもしくは接触させた状態で位置させること、
(c) 前記閉塞構造体を前記開口部を覆うように展開させること、
(d) 前記第2のアンカー部を対象とする前記組織欠損部の開口部外で展開し、前記第2のアンカー部を前記開口部を囲む第2の組織表面の近傍に位置させもしくは接触させた状態で位置させることとを含む、修復方法。 - ガイドワイヤを対象とする前記組織欠損部の近傍に案内することと、前記修復デバイスを前記ガイドワイヤに沿って前記欠損部に案内することとを含む、請求項38に記載の修復方法。
- 細胞の成長と前記対象部分への付着を促進する物質;接着剤;放射線不透過性物質;抗生物質製剤;血栓形成物質:非血栓形成物質;治療薬;ヒドロゲル成分:非炎症性物質;非再狭窄物質;放射性物質;これらを組み合わせた物質、のグループから選択される1つの物質を対象とする前記組織欠損部に導入することを含む、請求項38に記載の修復方法。
- (a) 第1および第2のアンカー部の中間に位置する閉塞構造体を有する閉塞デバイスを小さな直径の搬送状態で動脈瘤の頸部近傍へ低侵襲性の技術を使用して案内すること、
(b) 前記第1のアンカー部の遠位領域を、前記動脈瘤の前記頸部を通して前記動脈瘤の内部に展開し、前記動脈瘤の頸部を囲む内側動脈瘤組織表面の近傍に、前記開口部よりも大きい直径を有した第1の円周状アンカー構造体を位置させること、
(c) 前記中間閉塞構造体を前記動脈瘤の頸部に渡って展開し、前記開口部を被覆および閉塞すること、
(d) 前記第2のアンカー部を前記動脈瘤の頸部外に展開し、前記動脈瘤の前記頸部近傍の血管表面近傍に、前記動脈瘤の頸部よりも大きい直径を有した第2の円周状アンカー構造体を位置させることとを含む、動脈瘤を閉塞する方法。 - ガイドワイヤを対象とする前記組織欠損部の近傍に案内することと、前記修復デバイスを前記ガイドワイヤに沿って前記欠損部に案内することとを含む、請求項41に記載の動脈瘤を閉塞する方法。
- 細胞の成長と前記対象部分への付着を促進する物質;接着剤;放射線不透過性物質;抗生物質製剤;血栓形成物質:非血栓形成物質;治療薬;ヒドロゲル成分:非炎症性物質;非再狭窄物質;放射性物質;これらを組み合わせた物質、のグループから選択される1つの物質を対象とする前記組織欠損部に導入することを含む、請求項41に記載の動脈瘤を閉塞する方法。
- 開口部を覆い、動脈瘤と患者の血管との間の細胞伝達を制限する閉塞構造体を前記動脈瘤の前記開口部に配置させること、
前記動脈瘤の前記開口部の内部および/または外部に配置される少なくとも1つのアンカー構造体を用いて、前記閉塞構造体を適切な位置に固定することとを含む、患者の血管から動脈瘤を除去し、前記動脈瘤の質量軽減を促進させるための方法。
