Abstract

Description

これは、ここに全体が援用される、2003年11月12日に出願された、「オーバーモールド把持ジョーおよび二重カム作動機構」と題する仮出願番号60/519,849号の優先権を主張する非仮出願である。
本発明は、一般的に、対向したジョーを有する外科用把持器に関し、特に、複合把持ジョーおよびジョー突起を作動させるための機構に関する。
過去には、把持ジョーは、強度と信頼性を提供するために、金属で形成された。金属を形成する様々な工程は、プレス加工、ＥＤＭ(Electrical Discharge Machining)、光化学エッチング、ウォータージェット切断、レーザー切断、および機械加工を含む。これらの工程の全ては、ジョーが、体の組織に対して非外傷性の外面および端部で終わらせなければならないから、比較的高価で、時間のかかるものである。加えて、過去の作動機構は、典型的に、各々がジョーの各々と関連した二つのスロット内で軸方向に移動できる単一のピンで作動していた。二つのスロットは、二つのジョーの各々を開閉するために四つのカム面を設けている。

図１に、外科用把持器を図示し、これを参照番号１０で指示する。把持器は、近位端１４と遠位端１６の間に延びる軸線１２を有する細長い装置である。遠位端１６に配置された一対の対向したジョー突起１８，２１は、作動機構２３によって開位置と閉位置の間で作動可能である。シャフト組立体２５は、軸線１２に沿って延び、外チューブ２７と内作動ロッド３０と、を含む。突起１８，２１は、共通の枢軸ピン３２によって外チューブ２７に枢動可能に取り付けられる。

本発明の上の実施形態の各々において、ジョー突起１８ｄ，２１ｄのようなジョー突起は、複合構造を形成するために、プラスチックでオーバーモールドされるのがよい。図９に示す例では、複合ジョーの一つを、金属製内部ジョー突起１８ｄと外プラスチックオーバーモールド９０で形成することができる。同様な外プラスチックオーバーモールド９２を、金属製ジョー突起２１ｄ上に形成することもできる。気づくように、プラスチックオーバーモールド９０，９２に、非外傷性ジョーパッド９６を受け入れるようになった連結移動止め９４のような部分を、容易に、且つ安価に設けることができる。これらの部分は、容易に且つ安価に、プラスチックオーバーモールド９０に成形することができるが、金属製ジョー上に形成することは経済的に不可能であろう。かくして、ジョー突起１８ｄとオーバーモールド９０で形成された堅固な複合ジョーは、現実的であるばかりでなく、費用対効果が高く、したがって、使い捨て非外傷性パッド９６のような追加的な部分の適用を容易にする。
図１１の実施形態は、図９のものと同様であり、先に述べたものと同様の構造の要素を、同じ参照番号に続いて小文字「ｅ」を付けて指示する。かくして、この実施形態は、外チューブ２７ｅと、作動ロッド３０ｅと、ジョー突起１８ｅ，２１ｅと、それぞれ関連したオーバーモールド９０ｅ，９２ｅと、を含む。
この実施形態は、図９のものと同様であり、個々のピン７８，８１と、共通のピン８３と、を含む。しかしながら、この実施形態では、共通のピン８３は、ジョー突起１８ｅ，２１ｅを作動ロッド３０ｅに枢動可能に連結するように配置される。言い換えれば、共通のピン８３は、スロット内に配置されず、ジョー突起１８ｅ，２１ｅと関連した孔内に配置される。比較によって、個々のピン７８，８１が、図９の実施形態の場合のように、孔内に配置されず、関連したスロット内に配置されることに気づくであろう。
作動に際して、作動ロッド３０ｅを外チューブ２７ｅに対して遠位側に移動させると、ジョー突起１８ｅ，２１ｅは、個々のピン７８ｅ，８１ｅがそれぞれのスロット１０１，９８内で動くので、共通のピン８３ｅを中心に枢動する。この動きにより、ジョー１８ｅ，２１ｅを開かせる。ジョー１８ｅ，２１ｅの閉鎖は、作動ロッド３０ｅを外チューブ２７ｅに対して近位側に引くことによって達成される。
本発明のさらなる実施形態を、図１３に図示し、先に開示されたものと同様の構造の要素を、同じ参照番号に続いて小文字「ｆ」を付けて指示する。かくして、この実施形態は、外チューブ２７ｆと、作動ロッド３０ｆと、ジョー突起１８ｆ，２１ｆと、オーバーモールド９０ｆ，９２ｆと、共通の枢軸ピン８３ｆと、スロット９８ｆ，１０１ｆと、を含む。しかしながら、この場合には、スロット９８ｆ，１０１ｆは、図１１の実施形態の個々のピン８１ｅ，７８ｅ（図示せず）を組み合わせて、外チューブ２７ｆに固定された共通のピン１０３にするように、交差する。この例外が、共通のピン１０３と組み合わて交差するスロット９８ｆ，１０１ｆを含むので、図１３の実施形態は、図１１のものと同様の方法で機能する。

作動ロッド３０ｇは、その遠位端で分岐して、外面１０７，１１０の外方にそれぞれ延びる二つの延長部１１６，１１８を形成する。これらの延長部１１６，１１８は、それぞれスロット１２１，１２３を備え、これらは、軸１２ｇに交差し、また互いに交差している。この実施形態では、延長部１１６と関連したスロット１２１は、ジョー突起１８ｇと関連した短い突出部１１２を受け入れるように寸法決めされ、且つ構成されている。同様に、延長部１１８と関連したスロット１２３は、ジョー突起２１ｇと関連した短い突出部１１４を受け入れるように寸法決めされ、且つ構成されている。
作動に際して、ジョー突起１８ｇ，２１ｇが、開位置と閉位置の間で、共通のピン１０５を中心に枢動する。作動ロッド３０ｇを外チューブ２７ｇに対して遠位側に移動させると、短い突出部１１２，１１４がそれぞれのスロット１２１，１２３の遠位端まで押される。これにより、ジョー突起１８ｇ，２１ｇが互いに向かって移動するので、ジョー突起を閉鎖させる。作動ロッド３０ｇが外チューブ２７ｇに対して近位側に引かれると、短い突出部１１２，１１４がそれぞれのスロット１２１，１２３の遠位端に移行するので、内方に移動する。これにより、関連したジョー突起１８ｇ，２１ｇが分離するので、これらを開位置に移動させる。作動ロッド３０ｇが外チューブ２７ｇに対して遠位側に押されると、短い突出部１１２，１１４がそれぞれのスロット１２１，１２３の近位端に移行するから、外方に移動する。これにより、関連したジョー突起１８ｇ，２１ｇが、一緒についてくるから、これらを閉位置に移動させる。
外チューブに対して可動な作動ロッドを作動できる本発明の多数の他の実施形態がある。これらの実施形態の中で、突起１８のようなジョー突起の一方を作動ロッド３０で移動させ、次いで突起２１のような他のジョー突起の他方を突起１８のような第一突起で移動させることによって機能するものがある。共通のピン、個々のピン、およびスロットの様々な構造をこれらの実施形態に使用することができる。

Claims (29)

  1. カムロッド(３０)と、カムピン(３４)およびカム突出部(３６)を有するカムロッド(３０)に作動可能に連結された作動機構(２３)と、を含む、軸線(１２)に沿って延びる細長いチューブと、
    第一カムスロット、第一内部カム面(４１)、および第一外部カム面(４５)を有する第一ジョー突起(１８)と、
    第二カムスロット、第二内部カム面(３８)、および第二外部カム面(４３)を有する第二ジョー突起(２１)と、を含み、第二ジョー突起(２１)は、作動機構(２３)の移動に応答してジョー突起(１８，２１)を開閉するために、細長いチューブに作動可能に連結された共通の枢軸ピン(３２)で、第一ジョー突起(１８)に枢動可能に連結され、
    カムピン(３４)は、カムロッド(３０)を近位側に移動させるとき、第一および第二の内部カム面(４１，３８)に沿って動き、且つジョー突起(１８，２１)を閉じるように作動する、
    ことを特徴とする外科用具。
  2. 第一外部カム面(４５)は、第一ジョー突起(１８)の近位側に形成され、第二外部カム面(４３)は、第二ジョー突起(２１)の近位側に形成され、カムロッド(３０)が遠位側に移動するとき、カム突出部(３６)が第一および第二の外部カム面(４５，４３)で動き、且つジョー突起(１８，２１)を開くように作動する、

    第二ジョー突起(２１ａ)の近位端に連結された遠位端と、作動ロッド(３０ａ)の枢軸ピン(５２)に枢動可能に連結された近位端とを有する第一リンク(４７)と、
    第一ジョー突起(１８ａ)の近位端に枢動可能に連結された遠位端と、作動ロッド(３０ａ)の枢軸ピン(５２)に枢動可能に連結された近位端とを有する第二リンク(５０)と、
    ジョー突起の各々の上の非外傷性プラスチックオーバーモールドと、を含む、
    ことを特徴とする外科用具。
  7. 作動ロッド(３０ａ)が細長いチューブに対して近位側に引かれるとき、枢軸ピン(５２)が共通のピン(３２ａ)から離れる方向に移動し、リンク(４７，５０)の遠位端を内側に引いてジョー突起(１８ａ，２１ａ)を閉じる、
    請求項６に記載の外科用具。
  8. 作動ロッド(３０ｂ)を含む、軸線(１２)に沿って延びる細長いチューブと、
    近位端および遠位端を有し、近位端が第一孔(６５)の周りに形成された第一連のギア歯(６３)を有する第一ジョー突起(１８ｂ)と、
    近位端および遠位端を有し、近位端が第二孔(７０)の周りに形成された第二連のギア歯(６７)を有する第二ジョー突起(２１ｂ)と、を含み、第二ジョー突起(２１ｂ)は、作動ロッド(３０ｂ)の移動に応答してジョー突起(１８ｂ，２１ｂ)を作動可能に開閉する、第一および第二の孔(６５，７０)を貫く枢軸ピン(３２ｂ)によって第一ジョー突起(１８ｂ)に枢動可能に連結され、
    細長いチューブは、チューブの一方側で軸方向に延びる第一窓孔(５８)と、第一窓孔(５８)に面するチューブの反対側で延びる第二窓孔()６１と、を有し、第一連のギア歯(６３)が第一窓孔(５８)内で動くように配置され、且つ第二連のギア歯(６７)が第二窓孔(６１)内で動くように配置される、
    ことを特徴とする外科用具。
  9. ジョー突起(１８ｂ，２１ｂ)の各々の上に非外傷性プラスチックオーバーモールドをさらに含む、
    請求項８に記載の外科用具。
  10. 作動ロッド(３０ｂ)は、細長いチューブ内で近位側に引かれる、
    請求項８に記載の外科用具。
  11. 作動ロッド(３０ｂ)が細長いチューブ内で近位側に引かれるとき、第一連および第二連のギア歯(６３，６７)がそれぞれの第一および第二の窓孔(５８，６１)によって枢軸ピン(３２ｂ)を中心に枢動する、

  13. 作動ロッド(３０ｄ)を含む、軸線(１２)に沿って延びる細長いチューブと、
    第一カムスロット(８７)を有し、且つ第一枢軸ピン(７８)によって細長いチューブに連結された第一ジョー突起(１８ｄ)と、
    第二カムスロット(８５)を有し、第二枢動ピン(８１)によって細長いチューブに連結された第二ジョー突起(２１ｄ)と、を含み、第二ジョー突起(２１ｄ)は、それぞれの第一および第二のジョー突起(２１ｄ，１８ｄ)と関連した第一および第二のカムスロット(８７，８５)内で動く、作動ロッド(３０ｄ)に取り付けられた共通のピン(８３)で第一ジョー突起(１８ｄ)に連結される、
    ことを特徴とする外科用具。
  14. 作動ロッド(３０ｄ)を遠位側に移動させるとき、共通のピン(８３)が第一および第二のカムスロット(８７，８５)内で動き、それぞれの第一および第二のジョー突起(１８ｄ，２１ｄ)をそれぞれ第一および第二の枢軸ピン(７８，８１)を中心に開位置まで回転させる、

  15. 作動ロッド(３０ｄ)が細長いチューブに対して近位側に引かれると、共通のピン(８３)が第一および第二のカムスロット(８７，８５)の近位端まで移動し、ジョー突起(１８ｄ，２１ｄ)をそれぞれの第一および第二の枢軸ピン(７８，８１)を中心にほぼ閉位置まで枢動させる、
