Abstract
Description

能性大腸疾患(FBD)、例えば消化不良および他の内蔵痛のような胃腸病を治
療するための方法に関する。

する好ましいNSAIDはスリンダク、ナプロキセン、インドメタシン、メフェ
ナム酸、ジクロフェナック、フェノプロフェン、ジフルニサル、エトドラック、
イブプロフェン、ピロキシカム、アセチルサリチル酸、オキサプロジン、および
ブロムフェナックである。使用されるNSAIDの大部分は、一般的にはカルシ
ウム、ナトリウムまたはカリウムの塩として、例えば、フェンスプロフェンカル
シウムおよびブロムフェナックナトリウムとして、市販されている。特に好まし
い組み合わせはナプロキセンナトリウムまたはイブプロフェンと一緒になったプ
レガバリンまたはガバペンチンである。組成物は上記した一般的な製薬用賦形剤
を含有してよい。

タシン単独投与群)であり、棒グラフ2はガバペンチン40mgを1回投与した動
物であり、棒グラフ3はガバペンチン60mgを1回投与した動物であり、そして
、棒グラフ4はガバペンチン10mgを2回投与した動物である。

存した。6mmの生検標本2検体を胃の腺部の各側に位置する胃粘膜の一定領域か
ら採取し、そのPGE2含量を市販のELISAキット(Assay Designs Inc., A
nn Arbor, Michigan, USA)を用いて測定した。胃損傷の存在は再生した電子画 像を用いて測定し、損傷の範囲はCUE3画像形成ソフトウエアを用いて求めた
。データは損傷を受けた胃の領域の%およびPGE2含量(pg/ml)として記載 した。データを下記の表1に示す。

よびIBSのような一般的な胃腸の疾患も治療する。 以下の試験はGABA類縁体がエタノール摂取中止症候群の治療に有用である

腔内(ip)注射した。標準的な上昇プラス迷路を用いた実験において、ガバペ
ンチンまたは生理食塩水をエタノール飼料撤去後8時間に腹腔内注射し、マウス
を注射後60分にプラス迷路上に乗せた。運動協調(運動失調)および運動活動
性に関するこの試験において、試験直前に、ガバペンチンまたは生理食塩水をエ
タノール感受性の給餌マウスに注射した。次に運動失調試験では60分、運動活
動性については30分間、測定を行った。

5×0.25cm)および2本の対向する閉鎖アーム(30×5×15cm)が中央 のプラットホーム(5×5cm)から伸びているパースペクス製の装置を用いた。

化させた後に実験を行った。実験は薄暗い赤色電灯下で行い、各5分の試行は前
の投与を知らない観察者がビデオ撮影して後に分析した。この分析の間(Observ
er 3.0,Noldus Information Technology, Wageningen, Netherlands)、迷路の
各アーム上で経過した時間、各アームへの侵入回数、および直立挙動に関して測
定を行った。測定は表3の定義に従って行った。

留したことを確認しておいた(極めて少数が残留しなかったが、それらは試験か

行った。各投与群で8匹のマウスを使用した。

2A)で、12時間の試験期間に渡って結果を比較した場合に、上記評点増加を

薬剤の各用量による取り扱い曲線下部面積の検討のために使用した。より低用量
のガバペンチンの作用は測定の12時間に渡り有意ではなかったが、試験の最初
の4時間について曲線下部面積を求めたところ(図2B)、20および50mg/
kgの用量(p<0.05)並びに100mg/kgの用量(p<0.01)で有意な作
用が見とめられた。

た(p<0.05)。より低用量では作用は観察されなかった(表4)。エタノ ール投与停止から12時間後の試験では何れの用量でも作用は認められなかった
(データ示さず)。

意(生理食塩水の作用と比較してp<0.01)に減少した(F(4,50)=6. 53,p<0.001)。対照群の動物では保護没頭数は100mg/kgのガバペ ンチンにより有意に減少した(生理食塩水投与後の対照群の数値と比較してp<
0.05)。解放アーム上の平均時間は上記用量のガバペンチン投与後に対照群 動物で増加したが、これは生理食塩水を与えた対照群と有意差は無かった。
、GABA類縁体を評価した。GABA類縁体の評価に用いたアッセイは、ラッ
トにおけるリポ多糖類誘発遅延直腸異痛に対する化合物の作用を測定するもので
ある。内毒素リポ多糖類(LPS)の腹腔内(IP)注射は身体的疼痛モデルに
おける長時間持続する痛感亢進を誘発することが知られている。以下のアッセイ
、即ちLPS結腸過敏症アッセイは直腸膨張の実験モデルにおける疼痛の内臓閾
値に対するLPSのIP注射の作用を評価するために考案された。
筋電図を測定できるようにした。ラットにそれぞれ0.6および120mg/kgの 用量でアセプロマジンおよびケタミン(Imalgene 1000, Rhoene-Merieux,Lyon,
France)を腹腔内注射して麻酔した。ニクロム腺(長さ60cm、直径80μm)
の電極4本を2セット、鼠径部の靭帯の真上の外腹斜筋内の両側に移植した。電
極を頸部後方から露出させ、皮膚に装着したガラス管で保護した。動物をポリプ
ロピレンケージに個別収容し、温度制御された室内(21℃)で飼育した。水お
よび飼料は自由摂取させた(UARペレット、Epinay,France)。
定装置(Mini VIII,Alvar,Paris,France)で記録し、その際、低周波数信号 (<3Hz)を除去するために短時間定数(0.03秒)を用い、ペーパースピー ドは3.6cm/分とした。
プラスチック性のトンネル(直径6cm、長さ25cm)にラットを入れた。実験中
のストレス反応を最小限にするために、直腸膨張(RD)の前3〜4日間この操
作に適応させた。動物は以下の2つの基準、即ち(i)挙動要素:5分間に1回
まで動物が逃避あるいは反転を試みようとした場合、(ii)腹部基礎活動:腹部
横紋筋が膨張不存在下5分間当たり5回未満の腹部収縮を示した場合、を用いて
、プラスチックトンネルに適応していることを判断した。膨張用のバルーンとし
て動脈塞栓切除術用のカテーテル(Fogarty、Edwards Laboratories,Inc., San
ta Ana,USA)を用いた。直腸膨張(RD)はカテーテルを尾部に固定し、肛門 1cmにおいて、直腸内にバルーン(直径2mm、長さ2cm)を挿入することにより
行った。これを0〜1.6mlとなるまで0.4mlずつ段階的に漸次膨張させ、各膨
張段階は5分間持続させた。漏れが無いか調べるために、バルーンの内部に導入
した水の容量を、膨張期の終了時にシリンジを用いて完全に除去することにより
確認した。

ヒクル(NaCl 9%水溶液)を投与しておいた。 第2組の実験では、同じ群の8匹のラットに対し、対照直腸膨張の1時間後に
1mg/kgIPの用量で、リポ多糖類(E.coli,血清型0111:B4)または そのビヒクルを腹腔内投与した。次にRDをLPS注射後12時間に実施し、そ
の際予め(30分間)ガバペンチン(30mg/kg)またはそのビヒクル(0.3m
L/ラット)をIP投与した。

腸疾患の軽快に有効であることを示している。

数を低減した。直腸膨張の120分前の注射では、10および30mg/kgの両用
量のプレガバリンは、全膨張容量で同様の作用をもたらした。LPSはその投与
後12時間に0.4mLの容量で腹部収縮の回数の増加をもたらした(9.7±1. 0 vs 3.7±1.0)。この作用は直腸膨張の前30分に30mg/kgの用量でプ
レガバリンを投与した動物では抑制された(1.8±0.9 vs 9.7±1.0)。
これらの結果は、プレガバリンがラットにおいて基礎直腸感受性の低下において
、そして、LPS誘発直腸異痛の遮断において、有効であることを示している。
有するラットにおいてGABA類縁体を評価した。動物の結腸にTNBSを注射
することにより慢性の結腸炎が生じることが解かっている。ヒトにおいては消化
器系疾患は内臓痛が伴う場合が多い、このような病態において、内臓痛の閾値は
低下し、内臓過敏症となる。従って、結腸膨張の実験モデルにおける内臓痛の閾
値に対する結腸へのTNBSの注射の作用を評価するために以下の試験を考案し
た。

尾部基部にテーピングすることにより固定(バルーン先端を肛門から5cm)した
。バルーンを0mmHgから75mmHgまで5mmHgずつ段階的に漸次膨張させ、各膨張
段階は30秒間持続させた。結腸膨張の各サイクルは標準的な圧力調節器により
制御した。閾値は、初回腹部収縮が得られたときの圧力に相当するものとし、そ
の時点で膨張サイクルを中断した。結腸閾値を測定するために、4サイクルの膨
張を同一動物で実施した。
群を結腸膨張処置に付した。 第2組の実験においては、TNBSを投与した8匹のラットの群を結腸膨張処
置に付した。 第3組ではTNBSを投与した8匹のラットの群に結腸膨張サイクル開始前3
0分にガバペンチンまたはCI−1008を皮下注射した。 TNBSを除く全ての被験化合物は生理食塩水に溶解した。TNBSは30%
EtOH(w/v)に溶解した。ビヒクルの皮下注射は2mg/kgの容量で行った。 各群の間の統計学的有意差は一元ANOVAついでStudentの非対t検定を用 いて行った。p<0.05で統計的有意差ありとした。
の覚醒ラット、即ち、対照群動物およびTNBS投与動物で第7日に測定した。
TNBS投与動物では疼痛閾値の有意な低下が観察された。炎症パラメータ(結
腸重量、充血および壊死の面積、および、結腸ミエロペルオキシダーゼ含量)を
TNBS投与後第7日に近位の結腸で測定した。壊死面積を除き、パラメータは
全て有意に増加していた。
存的に抑制した。500mg/kg scでは、ガバペンチンは結腸疼痛に対するTN BSの作用を完全にブロックした。CI−1008もまた疼痛の閾値の低下の用
量依存的抑制を示した。100mg/kgでは、CI−1008はTNBSにより誘
発された異痛を完全に抑制した。モルヒネ(0.1mg/kg sc)は結腸膨張後の疼
痛閾値のTNBS誘発低下を完全に抑制した(図4)。一方、ガバペンチンおよ
びCI−1008の何れも上記実験条件下でTNBSの結腸炎症作用を抑制しな
かった。
値を有意に上昇させたが、同じ条件下で、ガバペンチン(500mg/kg sc)お よびCI−1008(200mg/kg sc)の何れも結腸疼痛閾値を変えなかった (図5)。結果を更に表7および8に示す。
がTNBS誘発結腸異痛を抑制し、従って、IBSにおける慢性疼痛に反映され
る腹部結腸過敏症において効果的であることを示している。
定するために別のモデルにおいて評価を実施した。 ラット結腸壁部へのホルマリンの投与により、急性の炎症と内臓疼痛が起こる
。本試験の目的はホルマリンの結腸壁内注射により誘発された内臓疼痛において
ガバペンチンおよびCI−1008の抗疼痛活性を評価することである。 体重240〜260gの成熟雌性Sprague-Dawleyラットを試験に用いた。試験
に使用する前には調節された環境下(20±1℃、50±5%湿度、照明8:0
0AM〜8:00PM)でケージ当たり3匹を飼育した。

結腸鏡を挿入した。この専用の内視鏡は、肛門辺縁部から約35mmで51mmの長
さの針(26ga)を用いて、目視制御下に腸壁の穿孔を可能にする側方スロット
を有している。50μlの5%ホルマリン水溶液(v/v)または同じ容量の等張
生理食塩水を注射した。注射終了直後(約1分)に動物を麻酔から覚醒させ、観
察を開始し、2時間継続した。観察試験の終了後30分、エバンズブルー(1%
)を静脈内注射し、その30分後に動物を屠殺した。腹部を切開した。注射部位
およびエバンズブル−の拡散領域を画像分析ソフトウエアで記録した。染料の拡
散がS字結腸壁に限局していないラットのデータは廃棄した。

次に疼痛評点(S)を以下の式: S=1L+2B+3C+4W1+5W2+6W3 を用いて連続15分の期間の各々につき計算し、疼痛評点は(i)各選択挙動の
事例の回数および(ii)所定の挙動による1〜6の係数に比例するようにした。

を表9に示す。 この試験は、結腸内ホルマリン誘発疼痛に対する抗疼痛作用をGABA類縁体 が示し、これにより、IBDおよびIBS、そして、膵炎および腸嚢胞症を含む
内臓痛の治療において有効であることを明らかにするものである。

評価した。このアッセイは海藻抽出液(カラゲナン)を用いており、この物質は
、供試動物の脚肉趾に注射すると滅菌性炎症を誘発し、これにより疼痛の閾値を
低下させる。ガバペンチンのようなGABA類縁体を含む鎮痛剤は疼痛閾値を正
常値まで上昇させ、これにより、動物は、未投与の対照動物と比較して長時間、
疼痛の外的発生源に忍容性を示すことができるようになる。GABA類縁体約5
0重量部とNSAID1重量部から1:1の組み合わせまでの濃度範囲における
、幾つかの固定された組み合わせのガバペンチンおよびナプロキセンナトリウム
を上記アッセイにおいて評価した。結果は図6(種々の用量における固定された
1:1組み合わせ)および図7(種々の用量における固定された50:1組み合
わせ)に示す。データによれば、GABA類縁体とNSAIDの組み合わせは急
性および慢性の疼痛を緩解し、鎮痛作用をもたらす能力において、相乗作用的で
あることが明らかである。

- 【請求項3】 ガバペンチンを使用する請求項2記載の方法。
- 【請求項4】 (1−アミノメチル−3−メチルシクロヘキシル)酢酸、(
1−アミノメチル−3−メチルシクロペンチル)酢酸、および(1−アミノメチ
