Antecedentes
Campo de utilizacion
Las realizaciones de la presente invencion se refieren en general al tratamiento de heridas utilizando en heridas terapia de presion negativa, y mas espedficamente a un aparato mejorado para el tratamiento de heridas abdominales abiertas.
Descripcion de la tecnica relacionada
El tratamiento de heridas abiertas o cronicas mediante la aplicacion depresion negativa en el sitio de la herida, en los casos en que las heridas que son demasiado grandes para el cierre espontaneo o similar no cicatrizan, es bien conocido en la tecnica. Sistemas de tratamiento de presion negativa en la herida actualmente conocidos en la tecnica corriente implican la colocacion de una cubierta que es impermeable a los lfquidos sobre la herida, utilizando diversos mecanismos para sellar la cubierta al tejido del paciente que rodea la herida, y conectar una fuente de presion negativa (tal como una bomba de vado) a la cubierta mediante lo cual se crea un area de presion negativa debajo de la cubierta en la zona de la herida. El documento US2009/0099519 describe un aparato para apositos abdominales.
Compendio
Las realizaciones de la invencion descrita en el presente documento se dirigen a un dispositivo de presion reducida y puede ser util en el tratamiento de heridas utilizando presion reducida.

Ciertas realizaciones de la invencion se dirigen a mejorar la utilizacion en el tratamiento de heridas o incisiones abdominales con presion negativa, que emplean un aposito poroso con secciones desmontables que permiten adaptar el tamano del aposito al sitio de la herida. El dimensionamiento del aposito de espuma puede realizarse en algunas realizaciones de una manera dimensionalmente independiente, de manera que, por ejemplo, se pueden modificar la anchura y/o la longitud independientemente entre sr Realizaciones adicionales tambien aportan que se pueda colocar una capa de contacto con la herida en contacto con el sitio de la herida, donde la capa de contacto con la herida sea preferentemente mmimamente o no adherente a la zona de la herida y provista de ranuras u otras aberturas para la eliminacion del exudado o fluidos de la herida y para la aplicacion de presion negativa a la zona de la herida.
Ciertas realizaciones aportan un sistema de tratamiento de presion negativa que comprende una capa de contacto con la herida colocada sobre la herida, un aposito poroso configurado para ser dimensionado y posicionado sobre la capa de contacto con la herida, una cubierta flexible configurada para ser colocada encima de la herida y sellada a la piel que rodea la herida, y que comprende ademas un conducto configurado para suministrar presion negativa a la herida a traves de una abertura en la cubierta flexible y a traves del aposito poroso y la capa de contacto con la herida.
Se puede proveer un aposito poroso de la invencion para el tratamiento de heridas con presion negativa, en la que el aposito poroso esta compuesto de un material poroso adecuado para la canalizacion del exudado de la herida desde el sitio de la herida a una fuente de presion negativa. El aposito poroso tiene preferiblemente una forma generalmente plana con un espesor menor que su anchura y su longitud, y preferiblemente comprende al menos un corte que se extiende a traves de al menos una parte del espesor del aposito, por lo que el corte define una seccion del aposito separable del resto del aposito a fin de permitir la modificacion del tamano del aposito (por ejemplo, su longitud y/o su anchura). En ciertas realizaciones, los cortes pueden estar constituidos por cortes arqueados y/o elfpticos, y pueden comprender ademas cortes interiores y cortes exteriores adicionales. En realizaciones adicionales, puede haber tambien cortes intermedios adicionales.
En otro sistema de la invencion, el sistema puede ser utilizado para el tratamiento de un sitio de la herida que comprende una capa de contacto con la herida provista de aberturas para la canalizacion del exudado de la herida y la distribucion de presion negativa, un aposito poroso, generalmente plano, adecuado para la transmision de la presion negativa a un sitio de la herida y que comprende al menos un corte que se extiende a traves de una porcion del espesor del aposito a fin de definir un aposito separable, una cubierta flexible, un conducto y una fuente de presion negativa configurada para suministrar presion negativa a traves del conducto a la zona de la herida.

Descripcion detallada de las realizaciones preferidas
Las realizaciones preferidas descritas en este documento se refieren a la terapia en heridas en un cuerpo humano o animal. Por lo tanto, cualquier referencia a una herida en el presente documento puede referirse a una herida en un cuerpo humano o animal, y cualquier referencia a un cuerpo en el presente documento puede referirse a un cuerpo humano o animal. El termino "herida" como se usa en este documento, ademas de tener su amplio sentido normal, incluye cualquier parte del cuerpo de un paciente que puede ser tratado utilizando presion reducida. Las heridas y/o sitios de la herida incluyen, pero no se limitan a, heridas abiertas, llagas por presion, ulceras y quemaduras. Las heridas abiertas y/o los sitios de la herida tambien pueden incluir incisiones (por ejemplo, incisiones abdominales) u otras aberturas, desgarros, o fistulas, por ejemplo, en la cavidad abdominal o peritoneal. El tratamiento de tales heridas se puede realizar utilizando la terapia de presion negativa en heridas, en la que una presion reducida o negativa se puede aplicar a la herida para facilitar y promover la cicatrizacion de la misma. Tambien se apreciara que los sistemas de presion negativa y los metodos como los se describen aqrn pueden ser aplicados a otras partes del cuerpo, y no se limitan necesariamente al tratamiento de heridas.
Volviendo a la figura 1, el tratamiento de una herida con presion negativa en ciertas realizaciones utiliza un sistema de tratamiento de presion negativa 101 como se ilustra esquematicamente aqrn. En esta realizacion, un sitio de la herida 110, ilustrado aqrn como un sitio de herida abdominal, puede beneficiarse del tratamiento con presion negativa. Tales sitios de heridas abdominales pueden ser el resultado de, por ejemplo, un accidente o una intervencion quirurgica. En algunos casos, las condiciones medicas, tales como el smdrome de compartimento abdominal, hipertension abdominal, sepsia o edema fluido pueden requerir la descompresion del abdomen con una incision quirurgica a traves de la pared abdominal para exponer el espacio peritoneal, despues de lo cual puede necesitar que la abertura se mantenga en un estado accesible abierta hasta que desaparezca la condicion. Otras condiciones pueden tambien necesitar que una apertura permanezca abierta - especialmente en la cavidad abdominal-, por ejemplo, si se requieren multiples procedimientos quirurgicos (posiblemente incidentales al trauma), o si existe evidencia de condiciones clmicas como peritonitis o fascitis necrotizante.
En los casos en que hay una herida, particularmente en el abdomen, el tratamiento de las posibles complicaciones relacionadas con la exposicion de los organos y el espacio peritoneal precisa, sf se desea, que la herida permanezca abierta o cerrada. La terapia, preferiblemente utilizando la aplicacion de presion negativa, puede estar dirigida a minimizar el riesgo de infeccion, al tiempo que fomenta la viabilidad del tejido y la eliminacion de sustancias nocivas desde el sitio de la herida. La aplicacion de presion reducida o negativa a un sitio de la herida se ha encontrado generalmente que fomenta una curacion mas rapida, un aumento del flujo sangumeo, una disminucion de la carga bacteriana, un aumento de la tasa de formacion de tejido de granulacion, para estimular la proliferacion de fibroblastos, para estimular la proliferacion de celulas endoteliales, para cerrar heridas abiertas cronicas, para inhibir la penetracion de las quemaduras, y/o para mejorar la sujecion de la solapa y del injerto, entre otras cosas. Tambien se ha informado de que en las heridas que han exhibido respuesta positiva al tratamiento mediante la aplicacion de presion negativa se incluyen heridas infectadas abiertas, ulceras de decubito, incisiones dehiscentes, quemaduras de espesor parcial y diversas lesiones a las que se han unido solapas o injertos. En consecuencia, la aplicacion de presion negativa al sitio de una herida 110 puede ser beneficiosa para un paciente.
De acuerdo con ello, ciertas realizaciones aportan una capa de contacto con la herida 105 para ser colocada sobre el sitio de la herida 110. Preferiblemente, la capa de contacto con la herida 105 puede ser de un material delgado y flexible que no se adhiera a la zona de la herida o las vfsceras expuestas en las proximidades. Por ejemplo, se pueden usar polfmeros tales como poliuretano, polietileno, politetrafluoroetileno o mezclas de los mismos. En una realizacion, la capa de contacto con la herida es permeable. Por ejemplo, la capa 105 de contacto con la herida puede estar provista de aberturas, tales como agujeros, rendijas o canales, para permitir la eliminacion de los fluidos desde el sitio de la herida o para la transmision de presion negativa a la zona de la herida 110. Las realizaciones adicionales de la capa 105 de contacto con la herida se describen con mas detalle a continuacion.
Ciertas realizaciones del sistema de tratamiento de presion negativa 101 pueden tambien utilizar un aposito poroso 103, que puede estar dispuesto sobre la capa de contacto con la herida 105. Este aposito 103 puede construirse a partir de un material poroso, por ejemplo de espuma, que es suave, elasticamente flexible, y generalmente conformado a la zona de la herida 110. Tal espuma puede incluir una espuma de celdas abiertas y reticulada hecha, por ejemplo, de un polfmero. Las espumas adecuadas incluyen espumas compuestas de, por ejemplo, poliuretano, silicona y alcohol polivinilico. Preferiblemente, este aposito 103 puede canalizar el exudado de la herida y otros fluidos a traves de sf mismo cuando se aplica presion negativa a la herida. Algunas apositos 103 pueden incluir canales preformados o aberturas para tales fines. En ciertas realizaciones, el aposito 103 puede tener un espesor de entre aproximadamente 25 y 50 mms. (es decir, entre 25 y 50 mms.). El aposito tambien puede tener una longitud

de entre aproximadamente 406 y 432 mms., (por ejemplo, entre 406 y 432 mms.), y una anchura de entre aproximadamente 280 y 305 mms. (por ejemplo, entre 280 y 305 mms.). En otras realizaciones, el grosor, la anchura y/o la longitud pueden tener otros valores adecuados. Otros aspectos del aposito 103 se discuten con mas detalle a continuacion. Preferiblemente, se usa una cubierta 107 para sellar el sitio de la herida 110. La cubierta 107 puede ser al menos parcialmente impermeable a los lfquidos, de modo que al menos una presion parcial negativa pueda ser mantenida en el sitio de la herida. Los materiales adecuados para la cubierta 107 incluyen, sin limitacion, materiales polimericos sinteticos que no absorban significativamente fluidos acuosos, incluyendo poliolefinas tales como polietileno y polipropileno, poliuretanos, polisiloxanos, poliamidas, poliesteres y otros copolfmeros y mezclas de los mismos. Los materiales utilizados en la cubierta pueden ser hidrofobos o hidrofilos. Ejemplos de materiales adecuados incluyen Transeal® disponible de DeRoyal y OpSite® disponible de Smith & Nephew. Con el fin de ayudar a la comodidad del paciente y evitar la maceracion de la piel, las cubiertas en ciertas realizaciones son, al menos en parte, transpirables, de tal manera que el vapor de agua es capaz de pasar a traves de ellas sin quedar retenido bajo el aposito. Se puede proporcionar una capa adhesiva sobre al menos una porcion de la parte inferior de la cubierta 107 para asegurar la cubierta a la piel del paciente, aunque ciertas realizaciones pueden utilizar en su lugar un adhesivo separado o una cinta adhesiva. Opcionalmente, una capa desprendible puede estar dispuesta sobre la capa adhesiva para protegerla antes de su uso y para facilitar el manejo de la cubierta 107; En algunas realizaciones, la capa desprendible puede estar compuesta de multiples secciones.
El sistema de presion negativa 101 se puede conectar a una fuente de presion negativa, por ejemplo una bomba 114. Un ejemplo de una bomba adecuada es la bomba Renasys EZ disponible de Smith & Nephew. La cubierta 107 puede estar conectada a la fuente de presion negativa 114 a traves de un conducto 112. El conducto 112 puede estar conectado a un puerto 113 situado sobre una abertura 109 en el pano 107, o bien el conducto 112 puede ser conectado directamente a traves de la abertura 109 sin el uso de un puerto. En una alternativa adicional, el conducto puede pasar por debajo de la cubierta y extenderse desde un lado de la cubierta. La patente US n° 7.524.315 describe otros aspectos similares de sistemas de presion negativa y se incorpora en este documento como referencia en su totalidad y se debe considerar una parte de esta especificacion.
En muchas aplicaciones, un recipiente u otra unidad de almacenamiento 115 puede estar interpuesto entre la fuente de presion negativa 114 y el conducto 112 a fin de permitir almacenar el exudado de la herida y otros fluidos desechados del sitio de la herida sin que entren en la fuente de presion negativa. Ciertos tipos de fuentes de presion negativa -por ejemplo, bombas peristalticas- tambien pueden permitir colocar un recipiente 115 despues de la bomba 114. Algunas realizaciones tambien pueden utilizar un filtro para evitar que los fluidos, aerosoles y otros contaminantes microbianos puedan salir del recipiente 115 y/o entrar en la fuente de presion negativa 114. Otras realizaciones pueden incluir tambien una valvula de corte o un filtro de obstruccion hidrofobo y/o oleofobo en el recipiente para evitar el desbordamiento; otras realizaciones pueden incluir medios sensores, tales como sensores capacitivos u otros tipos de detectores de nivel de lfquidos que actuan para detener o cortar la fuente de presion negativa si el nivel de fluido en el recipiente se aproxima a su capacidad. En el escape de la bomba, puede tambien ser preferible proporcionar un filtro contra los olores, tal como un deposito de carbon activado.
Con referencia a las figuras 2 y 3, se muestran las vistas en perspectiva y desde arriba de una realizacion del aposito poroso 103. El 103 tiene preferiblemente una o mas perforaciones hechas en el mismo, ilustradas por ejemplo en forma de cortes en arco 202, 204, 208 y 210. Estos cortes pueden hacerse en el aposito 103 utilizando cualquier mecanismo adecuado, incluyendo, por ejemplo, pero sin limitacion, cuchillas de corte, troquelado o corte por hilo caliente y estos cortes se extienden preferiblemente a traves de al menos parte del espesor del aposito 103. Los cortes no tienen que ser continuos, y pueden consistir, por ejemplo, en multiples pequenas perforaciones. En una realizacion, las perforaciones se extienden totalmente a traves del espesor del aposito 103. Con el fin de garantizar que el aposito 103 permanezca estructuralmente intacto durante la manipulacion y el uso, los cortes hechos a traves del aposito 103 retienen preferiblemente una o mas pequenas porciones en puente, tales como la porcion de puente 206.
De acuerdo con la invencion, el aposito 103 tiene una forma sustancialmente rectangular que tiene una longitud L, una anchura W y un espesor T, definidos con respecto de un eje mayor X, un eje menor Y y un eje vertical Z y tiene cuatro esquinas redondeadas. Se pueden formar unas primeras series de cortes exteriores arqueados 202 en el aposito en forma elfptica. Hay cuatro cortes exteriores 202a, 202b, 202c y 202d, posicionado cada uno en uno de los cuadrantes definidos por los ejes X e Y, con cuatro partes en puente 206 posicionadas en los extremos opuestos a lo largo de los ejes mayor y menor. Dentro de los cortes exteriores 202 hay una serie de cortes interiores arqueados 210 que tienen tambien una forma eliptica de forma similar a las series de cortes exteriores arqueados 202. Existen cuatro cortes internos 210a, 210b, 210c, 210d posicionados tambien cada uno en uno de los cuadrantes definidos por los ejes X e Y, con cuatro porciones en puente 222 posicionadas en los extremos opuestos a lo largo de los ejes mayor y menor.
Situado entre los cortes exterior e interior 202 y 210 aparecen una serie de cortes intermedios 204 y 208. Desde la vista en perspectiva superior de la figura 3, se aprecian un corte superior arqueado 204a y un corte inferior arqueado 204b simetricamente situados alrededor del eje menor Y en los extremos opuestos del aposito 103. Los cortes 204a y 204b se extienden generalmente a traves de la anchura W del aposito, simetricamente alrededor del eje mayor X, teniendo estos cortes 204a, 204b un radio de curvatura mayor que los de los cortes arqueados 202 cerca del eje mayor X. Los cortes arqueados izquierdo y derecho 208 se situan entre los cortes arqueados 204a, 204b, que se

extienden generalmente en sentido longitudinal a traves del aposito. Como se ilustra, hay cuatro cortes arqueados 208a, 208b, 208c, 208d que se extienden cada una generalmente en paralelo a las porciones de los cortes arqueados 202, 210 que los rodean, con las porciones en puente 220 situadas en el eje menor Y. Se apreciara que la forma y el numero de cortes se pueden variar, y que puede haber mas de una serie de cortes intermedios entre los cortes interior y exterior 210, 202.
De manera ventajosa, los cortes realizados en el aposito 103 se pueden utilizar para dimensionar selectivamente el aposito 103 con respecto a la zona de la herida en la que el aposito 103 se va a colocar. Por ejemplo, el aposito 103 puede dimensionarse eliminando las secciones desprendibles del aposito 103, por ejemplo, la seccion exterior 218 que rodea los cortes exteriores 202, y las secciones interiores 212a, 212b situadas entre los cortes exteriores 202 y los cortes intermedios 204a y 204b, y las secciones interiores 214a, 214b entre los cortes exteriores 202 y los cortes intermedios 208. Aunque se pueden hacer en el aposito 103 cortes adicionales y diferentes de los cortes 202, 204, 208, y 210, estos cortes representan ejemplos de ubicaciones de los cortes que se pueden utilizar para dimensionar el tamano de un aposito de una manera dimensionalmente independiente. Los tipos de cortes que se pueden hacer en el aposito 103 son arqueados. Ademas, aunque los cortes mostrados aqrn son a lo largo de la longitud L y a lo ancho de la anchura W del aposito, se pueden hacer cortes similares a lo largo del espesor T del aposito 103, de tal manera que se puede utilizar un aposito mas fino en un determinado sitio de la herida. Los cortes tambien se pueden hacer en un angulo no alineado con ninguno de los ejes X, Y o Z, por ejemplo diagonalmente a traves del aposito 103.
Durante su uso, el aposito 103 puede ser demasiado grande para el sitio de la herida 110, y puede necesitar ser dimensionado mediante la eliminacion de la zona desprendible 218 abarcada por los bordes del aposito 103 y los cortes 202 realizados en el mismo. Para heridas mas pequenas, se pueden retirar las areas desprendibles 212a, 212b, 214a y 214b, para dejar solo las areas desprendibles 216 y 217. En las heridas aun mas pequenas, se puede retirar el resto del aposito 103 para dejar solo la zona central desprendible 216. Por lo general, tal dimensionamiento puede realizarse manualmente, por ejemplo con unas tijeras, pero tales metodos incurren en desventajas concurrentes tales como dificultades en la manipulacion de un utensilio de corte en un quirofano ocupado, cortes irregulares e imprecisos y la posibilidad de arrojar partmulas extranas en el sitio de la herida. En su sitio, los trepados en el aposito 103 pueden separarse manualmente o con un mmimo corte a lo largo de las distintas porciones en puente 206, 220, 222.
Con referencia continua a las figuras 2-3, ciertas realizaciones permiten el dimensionamiento de un aposito 103 de una manera dimensionalmente independiente. Aqrn, las secciones del aposito 103 se pueden separar o cortar a lo largo de las delineaciones entre los diferentes cortes, por ejemplo, las secciones 212a, 212b y 214a, 214b. Estos cortes 204 y 208 permiten el dimensionamiento del aposito 103 a voluntad para adaptarla mas ajustadamente a las dimensiones reales de un sitio de la herida. Por ejemplo, el tamano de un aposito 103 para fijarlo en una herida que es mas ancha en el lado izquierdo y mas estrecho en el lado derecho se puede adecuar eliminando una seccion del aposito 214a delineada entre los cortes 208a, 208b y 202a, 202b. En otro ejemplo, cuando el aposito 103 es mas largo a lo largo de su porcion superior que el sitio de la herida 110, se puede desprender una seccion del aposito 212a, delineada entre los cortes 202a, 202b, 204a desde un extremo del aposito 103. En estos ejemplos anteriores, la porcion desprendible exterior 218 ya se ha eliminado preferiblemente, aunque esto no es estrictamente necesario. En consecuencia, el conformado dimensionalmente independiente del aposito 103 (por ejemplo, la modificacion de la longitud del aposito sin alterar el ancho del aposito, y viceversa) se puede lograr mediante secciones desprendibles 212, 214 delineadas por los cortes 204 o 208. Se contemplan secciones adicionales desprendibles abarcadas por cortes adicionales a fin de permitir el conformado dimensionalmente independiente del aposito 103, y no se pretende que las realizaciones ilustradas en el presente documento sean limitativas. Obviamente, para sitios de heridas mas pequenos, puede ser util la eliminacion de secciones simetricas del aposito 103, y las realizaciones del aposito 103 pueden proporcionar tales secciones, ilustradas aqrn como secciones 218, 217, 216. Por ejemplo, puede ser necesario eliminar la seccion exterior 218 del aposito 103 a lo largo de los cortes 202. Del mismo modo, para incisiones mas pequenas solo la seccion interior 216 puede necesitar los cortes 210 delineados en el interior.
Las figuras 6-56 ilustran diversas realizaciones diferentes y vistas de una capa de contacto con la herida 105. Como se establecio anteriormente, la capa de contacto con la herida 105 esta disenada y construida preferiblemente de manera que sea mmimamente adherente al sitio de la herida, y mas preferiblemente no adherente al sitio de la herida. En el caso de una herida abdominal, la capa de contacto con la herida 105 es con preferencia mmimamente adherente o no adherente a las vfsceras expuestas y a otros organos internos. La capa de contacto con la herida 105 se construye mas preferiblemente de un material flexible, por ejemplo, polfmeros tales como poliuretano (incluyendo Elastollan®), polietileno, politetrafluoroetileno o mezclas de los mismos.
La capa de contacto con la herida 105 es preferentemente mayor que el aposito de espuma 103, porque cuando se utiliza, la capa de contacto con la herida 105 puede entonces ser remetida alrededor y dentro del sitio de la herida. Por ejemplo, cuando se utiliza en una herida abdominal, la capa de contacto con la herida 105 se inserta preferiblemente en la cavidad abdominal entre los intestinos. preferiblemente permeable, por ejemplo, provista de aberturas tales como agujeros, hendiduras o canales. Estas aberturas pueden ser utiles en particular en el tratamiento de smdrome compartimental abdominal, donde estas aberturas pueden ser utilizadas para canalizar las cantidades a menudo copiosas de exudado y otros fluidos que se pueden producir. Ademas de ayudar en la eliminacion del exudado y de otros fluidos desde un sitio de la herida, las aberturas son utiles para la transmision y distribucion de la presion negativa a la zona de la herida. Preferiblemente, las aberturas son lo suficientemente pequenas para evitar el crecimiento de tejido hacia el interior, pero lo suficientemente grandes para evitar la oclusion. Ademas, algunas realizaciones de la capa de contacto con la herida 105 pueden estar provistas de una capa micro perforada de contacto con la herida. Diferentes realizaciones de la capa de contacto con la herida 105 (por ejemplo, las ilustradas en las figuras 6, 11, 16, 21, 26, 32, 37, 42, 47, 52) tambien pueden aportar ventajas durante la fabricacion, tales como la facilidad de produccion. La fabricacion de la capa de contacto con la herida 105 puede implicar cortar ranuras o agujeros, por ejemplo, con una matriz u hojas para troquelar, perforadoras rotativas, chorro de agua, corte por laser o por ultrasonidos.
El tratamiento de las heridas con presion negativa, requiere en general que la herida se pueda limpiar de una manera medicamente aceptable, llenando opcionalmente la herida con un material de relleno de algun tipo (tal como espuma), sellado con un pano, y conectada a una fuente de presion negativa. El tratamiento de heridas exponiendo los organos internos, vasos sangumeos y nervios, y en particular los de la cavidad abdominal, puede requerir consideraciones particulares. En primer lugar, los materiales de relleno de la herida tfpicos tales como espuma o gasa puede que no sea deseable que se coloquen en contacto directo con las vfsceras abdominales tales como los intestinos o el estomago, ya que estos materiales se pueden adherir indeseablemente a las vfsceras. En su sitio, se coloca preferiblemente en la cavidad abdominal o en el sitio de la herida 110 una capa no adherente o mmimamente adherente en contacto con la herida 105, descrita anteriormente. Esta capa de contacto con la herida 105 se corta preferiblemente a medida (si es necesario), y se remete entre las vfsceras y la fascia abdominal, con cualquier exceso de material plegado sobre sf mismo para evitar retener cualquiera de los contenidos abdominales. Posteriormente, el aposito de espuma 103, despues de ser dimensionado como se describio anteriormente, se coloca sobre la capa de contacto con la herida y, preferiblemente, hacia la mitad de la zona de la herida 110. Un pano 107, de nuevo como se ha descrito anteriormente, se corta a medida (si es necesario) y se coloca preferentemente de tal manera que se superpone sobre al menos una parte de la piel sana que rodea el sitio de la herida 110. En algunos casos, si una cubierta 107 es insuficiente, se pueden poner cubiertas adicionales; estas se superponen preferiblemente al menos parcialmente a fin de permitir realizar un sellado seguro. Preferiblemente, la cubierta 107 esta provista de una capa adhesiva en su lado inferior, que puede estar protegida por una capa auxiliar de proteccion. Tal capa auxiliar se elimina, preferiblemente antes de su uso a fin de permitir que la cubierta 107 se adhiera a la piel que rodea el sitio de la herida, asf como al aposito de espuma 103. Se puede hacer entonces una abertura 109 a traves de la cubierta 107, aunque algunas realizaciones pueden comprender una cubierta 107 provista de una mas pre abertura o aberturas 109. Se puede conectar entonces un conducto 112 conectado a una fuente de presion negativa a la abertura 109, o, en algunas realizaciones, bajo un lateral de la cubierta 107, de tal manera que se cree una conexion para fluidos entre el sitio de la herida 110 y la fuente de presion negativa. La conexion para fluidos permite la aplicacion terapeutica de presion negativa a la zona de la herida 110, y se puede aplicar como sea necesario hasta que el sitio de la herida 110 haya alcanzado un nivel deseado de curacion o hasta que se requiera otra intervencion quirurgica. Por supuesto, lo anterior describe ciertas caractensticas, aspectos y ventajas de la presente invencion, a la que se le pueden hacer diversos cambios y modificaciones sin apartarse del alcance de la presente invencion. Por otra parte, el sistema de tratamiento de presion negativa descrito en este documento no incorpora todos los objetos, ventajas, caractensticas y aspectos descritos anteriormente. Los expertos en la tecnica reconoceran que la invencion puede realizarse o llevarse a cabo de una manera que obtenga u optimice una ventaja o un grupo de ventajas como se plantea en este documento sin obtener necesariamente otros objetivos o ventajas como pueden ser mostradas o sugeridas en este documento. Por ejemplo, en algunas realizaciones, el aposito 103 se puede utilizar sin la capa de contacto con la herida 105 y/o la cubierta 107. Ademas, aunque se han mostrado y descrito con detalle un numero de variaciones de la invencion, otras modificaciones, que esten dentro de la alcance de esta invencion, seran facilmente evidentes para los expertos en la tecnica, basandose en esta descripcion. Se contempla que se pueden hacer diversas combinaciones o sub combinaciones de estas caractensticas y aspectos espedficos de realizaciones que caigan dentro del alcance de la invencion. En consecuencia, debe entenderse que las diversas caractensticas y aspectos de las realizaciones descritas se pueden combinar o sustituir por otras con el fin de formar diferentes modos del sistema de tratamiento de presion negativa descrito.
Claims (2)
- 5101520253035404550REIVINDICACIONES1. Un aposito poroso (103) para el tratamiento de heridas utilizando presion negativa, teniendo el aposito (103) una forma sustancialmente rectangular que tiene una longitud (L), una anchura (W) y un espesor (T) definidos alrededor de un eje mayor (X), un eje menor (Y) y un eje vertical (Z) y cuatro esquinas redondeadas, estando hecho el aposito de:un material poroso adecuado para canalizar el exudado de la herida desde el sitio de la herida y la presion negativa a la zona de la herida; caracterizado porque hay:una pluralidad de cortes o perforaciones externas arqueadas (202) que se extienden a traves de al menos una porcion del espesor del aposito (103) que tiene una forma eliptica, teniendo la pluralidad de cortes o perforaciones externas arqueadas cuatro cortes externos (202a, 202b, 202c y 202d) cada uno posicionado en uno de los cuadrantes definidos por los ejes mayor (X ) y menor (Y); con cuatro porciones en puente (206) colocadas en los extremos opuestos a lo largo de los ejes mayor (X) y menor (Y);una pluralidad de cortes o perforaciones internas arqueadas (210) que se extienden a traves de al menos una porcion del espesor del aposito y que tiene una forma elfptica de forma similar a la pluralidad de cortes o perforaciones externas arqueadas (202), teniendo la pluralidad de cortes o perforaciones internas arqueadas cuatro cortes interiores (210a, 210b, 210c y 210d) situado cada uno en uno de los cuadrantes definidos por los ejes mayor y menor; con cuatro porciones en puente (222) colocadas en los extremos opuestos a lo largo de los ejes mayor (X) y menor (Y); situados entre los cortes externo (202) e interno (210) existe una serie de cortes intermedios (204, 208), en la que los cortes o perforaciones (202, 210, 204, 208) definen una seccion desprendible del aposito (103).
- 2. El aposito segun la reivindicacion 1, en el que el aposito (103) comprende una espuma de celda abierta, reticulada.3 El aposito segun cualquiera de las reivindicaciones 1 y 2, en el que el aposito (103) comprende una espuma de poliuretano.4 El aposito segun cualquier reivindicacion precedente, en el que al menos algunos de los cortes o perforaciones intermedias (204, 208) se extienden en general longitudinalmente a traves del aposito (103) y al menos algunos de los cortes o perforaciones intermedias (204, 208) se extienden generalmente a lo ancho a traves el aposito (103).5 Un aposito poroso (103) como se reivindica en cualquiera de las reivindicaciones precedentes que tiene mas de una serie de cortes intermedios (204, 208) entre los cortes internos y externos (202, 210).6 Un aposito poroso (103) como se reivindica en cualquiera de las reivindicaciones precedentes en el que se realizan cortes a lo largo del espesor T del aposito (103), de tal manera que se puede utilizar un aposito mas fino en el sitio de la herida.7 Un aposito poroso (103) como se reivindica en cualquiera de las reivindicaciones precedentes, en el que existen cortes realizados segun un angulo no alineado con los ejes mayor (X), menor (Y) o vertical (Z).8 Un aposito poroso (103) segun la reivindicacion 7, en la que los cortes realizados segun un angulo no alineado con los ejes (X, Y, Z) se encuentran diagonalmente a traves del aposito (103).9 Un sistema para el tratamiento de un sitio de la herida usando presion negativa, comprendiendo el sistema:una capa de contacto con la herida (105), en la que la capa de contacto con la herida (105) esta provista de una o mas aberturas para canalizar el exudado de la herida y la distribucion de la presion negativa; un aposito poroso (103) segun cualquiera de las reivindicaciones precedentes;una cubierta flexible (107) configurada para ser colocada sobre el aposito poroso; una fuente de presion negativa; yun conducto configurado para transmitir la presion negativa desde la fuente a la cubierta flexible (107).10 El sistema segun la reivindicacion 9, en el que la capa de contacto con la herida (105) esta construida de un material que es mmimamente adherente a la zona de la herida (110).11 El sistema segun cualquiera de las reivindicaciones 9 o 10, en el que la capa de contacto con la herida comprende poliuretano.12 El sistema segun cualquiera de las reivindicaciones 9 a 11, en el que el aposito poroso (103) comprende una espuma de poliuretano porosa.13 El sistema segun cualquiera de las reivindicaciones 9 a 12, que comprende ademas un puerto acoplable a una abertura formada en la cubierta (107) y en el conducto.
