CAMPO DE DE LA INVENCIÓN
Las modalidades de la presente invención se refieren en general al tratamiento de heridas utilizando terapia para heridas con presión negativa, y más específicamente a un aparato y un método mejorados del mismo para manejar heridas abdominales abiertas.
DESCRIPCIÓN DE LA TÉCNICA RELACIONADA
El tratamiento de heridas abiertas o crónicas por medio de aplicar presión negativa al sitio de herida, donde las heridas son demasiado grandes para que se cicatricen espontáneamente o dejan de sanar de algún otro modo, es bien conocido en la técnica. Los sistemas para el tratamiento de heridas con presión negativa actualmente conocidos en la técnica implican comúnmente colocar una cubierta que sea impermeable a líquidos sobre la herida, utilizar varios mecanismos para sellar la cubierta al tejido del paciente que rodea la herida, y conectar una fuente de presión negativa (como una
bomba de vacío) a la cubierta, con lo cual se crea un área de presión negativa bajo la cubierta en el área de la herida.
BREVE DESCRIPCIÓN DE LA INVENCIÓN
Las modalidades de la invención expuestas en la presente están dirigidas a un aparato de presión reducida y a métodos de tratamiento que utilizan un aparato de presión reducida, y pueden ser útiles en el tratamiento de heridas que utilizan presión reducida.
Ciertas modalidades de la invención están dirigidas a métodos mejorados de tratar heridas o incisiones abdominales con presión negativa. Por ejemplo y con propósitos ilustrativos solamente, algunas modalidades emplean una almohadilla porosa con secciones desprendibles que permiten la conformación deseada de la almohadilla al sitio de herida. La conformación de la almohadilla de material alveolar se puede realizar en algunas modalidades de manera dimensionalmente independiente, para que por ejemplo la anchura y/o longitud se puedan modificar independientemente una de otra. Modalidades adicionales proveen también una capa de contacto con heridas para colocarlas en el contacto con el sitio de herida, donde la capa de contacto con heridas es de preferencia mínimamente adherente o no adherente al sitio de herida y está provista de hendiduras u otras aberturas para la remoción del material exudado de una herida o fluidos y la aplicación de presión negativa al sitio de herida.
Ciertas modalidades proveen un sistema de tratamiento con presión negativa, que comprende una capa de contacto con heridas colocada sobre la herida, una almohadilla porosa configurada para ser conformada y para ser posicionada sobre la capa de contacto con heridas, una venda flexible configurada para ser colocada sobre la herida y sellada a la piel que rodea la herida, y que comprende además un conducto configurado para ejercer presión negativa a la herida a través de una abertura en la venda flexible y a través de la almohadilla porosa y la capa de contacto con heridas.
En una modalidad preferida, se provee una almohadilla porosa para el tratamiento de heridas con presión negativa, en donde la almohadilla porosa está constituida de un material poroso adecuado para canalizar el material exudado de una herida de un sitio de herida a una fuente de presión negativa. La almohadilla porosa comprende preferiblemente una forma generalmente plana con un grosor menor a su anchura y longitud, y comprende preferiblemente por lo menos un corte que extiende por lo menos a través de una porción del grosor de la almohadilla, con lo cual el corte define una sección de almohadilla desprendióle del resto de la almohadilla para permitir la modificación del tamaño de la almohadilla (por ejemplo su longitud y/o anchura). En ciertas modalidades, los cortes pueden estar constituidos de cortes arqueados y/o elípticos, y pueden comprender además cortes adicionales, interiores y exteriores. En modalidades adicionales, pueden estar presentes también cortes intermedios adicionales.
En otra modalidad preferida, un sistema para el tratamiento de
un sitio de herida comprende una capa de contacto con heridas provisto de aberturas para canalizar el material exudado de una herida y distribuir presión negativa, una almohadilla porosa generalmente plana, adecuada para transmitir presión negativa a un sitio de herida y que comprende por lo menos un corte que se extiende a través de una porción del grosor de la almohadilla para definir una almohadilla desprendible, una venda flexible, un conducto y una fuente de presión negativa configurada para ejercer presión negativa a través del conducto al sitio de herida.
En otra modalidad preferida todavía, un método de tratar un sitio de herida usando presión negativa puede comprender colocar una capa de contacto con heridas sobre el sitio de herida; colocar una almohadilla porosa sobre la capa de contacto con heridas, donde la almohadilla porosa está perforada para permitir la remoción de porciones de almohadilla para permitir conformar de la almohadilla de manera dimensionalmente independiente para que ésta quede justa en el sitio de herida; sellar el sitio de herida con una venda flexible configurada para posicionarla sobre la herida y sellarla a la piel que rodea la herida; conectar una fuente de presión negativa al sitio de herida; y mantener la aplicación de presión negativa hasta que el sitio de herida haya cicatrizado apropiadamente.

DESCRIPCIÓN DETALLADA DE LAS MODALIDADES PREFERIDAS
Las modalidades preferidas expuestas en la presente se refieren a terapias para heridas para un cuerpo humano o animal. Por lo tanto, cualquier referencia a una herida en la presente puede referirse a una herida en un cuerpo humano o animal, y cualquier referencia a un cuerpo en la presente puede referirse a un cuerpo humano o animal. El término "herida" como se utiliza en la presente, además de tener su significado ordinario amplio, incluye cualquier parte del cuerpo de un paciente que sea tratado utilizando presión reducida. Las heridas y/o los sitios de herida incluyen, pero sin limitación, heridas abiertas, llagas por decúbito, úlceras y quemaduras.
Las heridas abiertas y/o los sitios de herida pueden incluir también incisiones (por ejemplo incisiones abdominales) u otras aberturas, desgarres o fístulas, por ejemplo en la cavidad abdominal o peritoneal. Se puede realizar el tratamiento de tales heridas utilizando terapia para heridas con presión negativa, en donde se puede aplicar una presión reducida o negativa a la herida para facilitar y promover la cicatrización de la herida. Se apreciará también que los sistemas y métodos con presión negativa, como se ha expuesto en la presente, se pueden aplicar a otras partes del cuerpo y no están limitados necesariamente al tratamiento de heridas.
Volviendo a la figura 1 , el tratamiento de una herida con presión negativa en ciertas modalidades utiliza un sistema de tratamiento con presión negativa 101 , como se ilustra aquí esquemáticamente. En esta modalidad, un sitio de herida 1 10, ilustrado aquí como un sitio de herida abdominal, puede beneficiarse mediante el tratamiento con presión negativa. Tales sitios de heridas abdominales pueden ser el resultado por ejemplo de un accidente o debido a alguna intervención quirúrgica. A veces, ciertas condiciones médicas, tales como síndrome de compartimiento abdominal, hipertensión abdominal, sepsis o edema por fluidos, pueden requerir la descompresión del abdomen con una incisión quirúrgica a través de la pared abdominal para exponer el espacio de peritoneal, después de lo cual pueda ser necesario mantener la abertura en estado abierto y accesible hasta que ceda la condición. Otras condiciones pueden necesitar también que una abertura— particularmente en la cavidad abdominal— se quede abierta, por ejemplo si se requieren múltiples procedimientos quirúrgicos (posiblemente consecuencia del trauma) o hay evidencia de condiciones clínicas como peritonitis o fascitis necrosante.
En casos en que hay una herida, particularmente en el abdomen, se requiere el manejo de posibles complicaciones relacionadas con la exposición de órganos y el espacio peritoneal, sin tener en cuenta si la herida ha de permanecer abierta o si será cerrada. Se puede aplicar la terapia, utilizando preferiblemente la aplicación de presión negativa, para reducir al mínimo el riesgo de infección, a la vez que promover la viabilidad de tejido y la remoción de sustancias deletéreas del sitio de herida. Se ha observado que la aplicación de presión reducida o negativa a un sitio de herida promueve generalmente una cicatrización más rápida, riego sanguíneo incrementado, carga bacteriana disminuida, tasa incrementada de formación de tejido de granulación, para estimular la proliferación de fibroblastos, estimular la proliferación de células de endoteliales, cerrar heridas abiertas crónicas, inhibir la penetración de quemaduras y/o aumentar la fijación de solapa e injerto, entre otras cosas. Se ha informado también que las heridas que han exhibido respuesta positiva al tratamiento mediante la aplicación de presión negativa incluyen heridas abiertas infectadas, úlceras por decúbito, incisiones abiertas, quemaduras de grosor parcial y varias lesiones a las cuales se les ha agregado colgajos o injertos. Por consiguiente, la aplicación de presión negativa a un sitio de herida 1 10 puede ser beneficioso para un paciente.
De acuerdo con ello, ciertas modalidades proveen una capa de
contacto con heridas 105 para que se coloquen sobre el sitio de herida 110. Preferiblemente, la capa de contacto con heridas 105 pueden ser un material delgado y flexible que no se adhiera al sitio de herida ni a las visceras expuestas en proximidad inmediata. Por ejemplo, se pueden utilizar polímeros tales como poliuretano, polietileno, politetrafluoroetileno o mezclas de los mismos. En una modalidad, la capa de contacto con heridas es permeable. Por ejemplo, la capa de contacto con heridas 105 puede estar provista de aberturas, tales como agujeros, hendiduras o canales, para permitir la remoción de fluidos del sitio de herida 1 10 o la transmisión de presión negativa al sitio de herida 1 10. Se describen modalidades adicionales de la capa de contacto con heridas 105 con detalle adicional más adelante.
Ciertas modalidades del sistema de tratamiento con presión negativa 101 también pueden utilizar una almohadilla 103 porosa, la cual puede quedar dispuesta sobre la capa de contacto con heridas 105. Esta almohadilla 103 pueden estar fabricada de un material poroso, por ejemplo material alveolar, que sea blando, elásticamente flexible y generalmente conformable al sitio de herida 1 10. Tal material alveolar puede incluir un material alveolar de célula abierta y reticular, hecho por ejemplo de un polímero. Los materiales alveolares adecuados incluyen materiales alveolares compuestos por ejemplo de poliuretano, silicona y alcohol polivinílico. Preferiblemente, esta almohadilla 103 puede canalizar el material exudado de una herida y otros fluidos a través de sí misma, cuando se le aplica presión negativa a la herida. Algunas almohadillas 103 pueden incluir canales o aberturas previamente formados con tales propósitos. En ciertas modalidades, la almohadilla 103 pueden tener un grosor de entre aproximadamente 2.54 y aproximadamente 5.08 centímetros (por ejemplo entre 2.54 y 5.08 centímetros). La almohadilla puede tener también una longitud de entre aproximadamente 40.64 y aproximadamente 43.18 centímetros (por ejemplo 40.64 y 43.18 centímetros), y una anchura de entre aproximadamente 27.94 y aproximadamente 30.48 centímetros (por ejemplo, entre 27.94 y 30.48 centímetros). En otras modalidades, el grosor, la anchura y/o la longitud pueden tener otros valores adecuados. Se discuten otros aspectos de la almohadilla 103 con mayor detalle más adelante.
Preferiblemente, se utiliza una venda 107 para sellar el sitio de herida 1 10. La venda 107 puede ser por lo menos parcialmente impermeable a líquidos, de tal manera que se puede mantener por lo menos una presión negativa parcial en el sitio de herida. Los materiales adecuados para la venda 07 incluyen, sin limitación, materiales poliméricos sintéticos que no absorben fluidos acuosos de manera notable, inclusive poliolefinas tales como polietileno y polipropileno, poliuretanos, polisiloxanos, poliamidas, poliésteres y otros copolímeros y mezclas de los mismos. Los materiales utilizados en la venda pueden estar hidrófobos o hidrófilos. Ejemplos de materiales adecuados incluyen Transeal®, obtenible de DeRoyal, y OpSite®, obtenible de Smith & Nephew. Para ayudar al bienestar del paciente y evitar la maceración de la piel, las vendas son en ciertas modalidades por lo menos en parte transpirables, de tal manera que el vapor de agua puede atravesar sin quedarse atrapado bajo el vendaje. Se puede proveer una capa adhesiva por lo menos en una porción de la cara inferior de la venda 107 para asegurar la venda a la piel del paciente, aunque ciertas modalidades puedan utilizar en su lugar un adhesivo o tira adhesiva separados. Opcionalmente, una capa de la liberación puede quedar dispuesta sobre la capa adhesiva para protegerla antes de ser usada y para facilitar el manejo de la venda 107; en algunas modalidades, la capa de la liberación puede estar compuesta de múltiples secciones.
El sistema de presión negativa 101 puede estar conectado a una fuente de presión negativa, por ejemplo una bomba 1 14. Un ejemplo de bomba adecuada es la bomba de Renasys EZ, obtenible de Smith & Nephew. La venda 107 puede conectarse a la fuente de presión negativa 1 14 a través de un conducto 112. El conducto 1 12 puede conectarse a una vía de acceso 1 13 situada sobre una abertura 109 en la venda 107, o bien el conducto 1 12 puede conectarse directamente a través de la abertura 109 sin el uso de una vía de acceso. En una alternativa adicional, el conducto puede pasar debajo de la venda y extenderse de un lado de la venda. La patente de No. 7,524,315 expone otros aspectos semejantes de sistemas con presión negativa y se incorpora mediante la presente como referencia en su totalidad y se debe considerar parte de esta especificación.
En muchas aplicaciones, se puede interponer un recipiente u otra unidad de almacenamiento 1 15 entre la fuente de presión negativa 1 14 y el conducto 1 12 para permitir que el material exudado de una herida y otros fluidos removidos del sitio de herida se almacenen sin que entren a la fuente de presión negativa. Ciertos tipos de fuentes de presión negativa — por ejemplo, bombas peristálticas— pueden permitir también colocar un recipiente 1 15 después de la bomba 1 14. Algunas modalidades pueden utilizar también un filtro para evitar salgan fluidos, aerosoles y otros contaminantes microbianos del recipiente 1 15 y/o entren a la fuente de presión negativa 114. Modalidades adicionales pueden incluir también una válvula de cierre o un filtro de oclusión hidrófobo y/u oleófobo en el recipiente para evitar el flujo excesivo; otras modalidades pueden incluir medios de detección, tales como sensores de capacidad u otros detectores de nivel de fluido que actúan para detener o cerrar la fuente de presión negativa, si el nivel de fluido en el recipiente se acerca a la capacidad. En el escape de bomba, puede ser también preferible proveer un filtro de olor, como un bote de carbón activado.
Con referencia a las figuras 2 y 3, se muestran vistas en perspectiva y superior de una modalidad de la almohadilla 103 porosa. La almohadilla 103 tiene preferiblemente una o más perforaciones realizadas en la misma, ilustradas por ejemplo en los cortes arqueados 202, 204, 208 y 210. Se pueden realizar estos cortes en la almohadilla 103 utilizando algún mecanismo adecuado, incluyendo por ejemplo, pero sin limitación, cuchillas de corte, troquelado o corte con alambre caliente, y estos cortes se extienden preferiblemente por lo menos a través de una porción del grosor de la almohadilla 103. No es necesario que los cortes sean continuos y pueden
consistir, por ejemplo, en múltiples perforaciones pequeñas. En una modalidad, las perforaciones se extienden enteramente a través del grosor de la almohadilla 103. Para asegurar que la almohadilla 103 permanezca estructuralmente intacta durante el manejo y el uso, los cortes hechos a través de la almohadilla 103 retienen preferiblemente una o más pequeñas porciones de puente, como la porción de puente 206.
En una modalidad, la almohadilla 103 tiene forma sustancialmente rectangular con una longitud L, una anchura W y un grosor T, definidos alrededor de un eje mayor X, un eje menor Y y un eje vertical Z, y tiene cuatro esquinas redondeadas. Se puede formar una primera serie de cortes exteriores arqueados 202 en la almohadilla con forma elíptica. En la modalidad ilustrada, hay cuatro cortes exteriores 202a, 202b, 202c y 202d, cada posicionado en uno de los cuadrantes definidos por los ejes X y Y, con cuatro porciones de puente 206 posicionadas en extremos los opuestos a lo largo de los ejes mayor y menor. Dentro de los cortes exteriores 202 hay una serie de cortes interiores arqueados 210 también con forma elíptica, conformado similarmente a la serie de cortes exteriores arqueados 202. Como se ilustra, en una modalidad hay cuatro cortes interiores 210a, 210b, 210c, 21 Od, cada uno posicionado también en uno de los cuadrantes definidos por los ejes X y Y, con cuatro porciones de puente 222 posicionadas en los extremos opuestos a lo largo de los ejes mayor y menor.
Ubicado entre los cortes exteriores e interiores 202 y 210 hay una serie de cortes intermedios 204 y 208. A partir de la vista en perspectiva de superior de la figura 3, un corte arqueado superior 204a y un corte arqueado inferior 204b están dispuestos simétricamente alrededor del eje menor Y ubicado en los extremos opuestos de la almohadilla 103. Los cortes 204a y 204b se extienden generalmente a través de la anchura W de la almohadilla, simétricamente alrededor del eje mayor X, en que estos cortes 204a, 204b tienen un radio de curvatura más grande que el de los cortes arqueados 202 alrededor del eje mayor X. Unos cortes izquierdo y derecho arqueados 208 están provistos entre los cortes arqueados 204a, 204b, extendiéndose por lo general a lo largo a través de la almohadilla. Como se ilustra, puede haber cuatro cortes arqueados 208a, 208b, 208c, 208d, cada extendiéndose por lo general en paralelo a las porciones de los cortes arqueados 202, 210 que los rodean, con porciones de puente 220 ubicadas en el eje menor Y. Se apreciará que se puede variar la forma y el número de cortes, y que puede haber más de una serie de cortes intermedios entre los cortes interiores y exteriores 210, 202.
Ventajosamente, los cortes realizados en la almohadilla 103 se pueden utilizar para conformar selectivamente la almohadilla 103 al sitio de herida en el que se ha de colocar la almohadilla 103. Por ejemplo, se puede conformar la almohadilla 103 quitando secciones desprendibles de la almohadilla 103, por ejemplo la sección exterior 218 que rodea los cortes exteriores 202, las secciones interiores 212a, 212b ubicadas entre los cortes exteriores 202 y los cortes intermedios 204a y 204b, y las secciones interiores 214a, 214b entre los cortes exteriores 202 y los cortes intermedios 208.
Aunque se pueden hacer cortes adicionales y diferentes de los cortes 202, 204, 208 y 210 en la almohadilla 103, estos cortes representan ejemplos de tipos y ubicaciones de cortes que se pueden utilizar para conformar una almohadilla de manera dimensionalmente independiente. Los tipos de cortes que se pueden hacer en la almohadilla 103 incluyen cortes por ejemplo arqueados, circulares, ovoides, en zigzag y/o lineales. Aún más, aunque los cortes mostrados aquí estén a lo largo la longitud L y la anchura W de la almohadilla, se pueden hacer cortes semejantes a lo largo del grosor T de la almohadilla 103, de tal manera que se puede utilizar una almohadilla más delgada en un sitio de herida. Se pueden hacer cortes también en un ángulo no alineado con ninguno de los ejes X, Y o Z, por ejemplo diagonalmente a través de la almohadilla 103.
En uso, la almohadilla 103 puede ser demasiado grande para el sitio de herida 1 10 y puede necesitar ser conformada quitando el área desprendible 218 delimitada por las orillas de la almohadilla 103 y los cortes 202 hechos en la misma. Para heridas más pequeñas, se pueden quitar todas las áreas desprendióles 212a, 212b, 214a y 214b, para dejar solamente las áreas desprendibles 216 y 217. En heridas aún más pequeñas, se puede quitar el resto de la almohadilla 103 para dejar solamente el área desprendible central 216. Típicamente, se puede realizar tal conformación manualmente, usando por ejemplo tijeras, pero tales métodos implican desventajas concomitantes, tales como dificultades para manipular un utensilio cortante en una sala de operaciones ocupada, cortes desiguales e imprecisos, y la
posibilidad de dejar caer partículas extranjeras en un sitio de herida. En vez de eso, los cortes previamente practicados en la almohadilla 103 se pueden desprender con la mano o con un corte mínimo a lo largo de las varias porciones de puente 206, 220, 222.
Con referencia continua a las figuras 2-3, ciertas modalidades permiten conformar una almohadilla 103 de manera dimensionalmente independiente. Aquí, las secciones de la almohadilla 103 se pueden desprender o cortar a lo largo de las delineaciones entre los varios cortes, por ejemplo las secciones 212a, 212b y 214a, 214b. Estos cortes 204 y 208 permiten la conformación de la almohadilla 103 según se desee para aproximarse en la medida de lo posible a las dimensiones reales de un sitio de herida. Por ejemplo, se puede efectuar la conformación de una almohadilla 103 para que quede justa en una herida que sea más ancha en el lado izquierdo y más estrecha en el lado derecho, quitando una sección de almohadilla 214a delineada entre los cortes 208a, 208b y 202a, 202b. En otro ejemplo en el cual la almohadilla 103 es más larga sobre su porción superior que el sitio de herida 1 10, se puede quitar una sección de almohadilla 212a, delineada entre los cortes 202a, 202b y 204a, de un extremo de la almohadilla 03. En estos ejemplos anteriores, ya se ha quitado preferiblemente la porción desprendible exterior 218, aunque no se requiere esto necesariamente. Por consiguiente, se puede lograr la conformación dimensionalmente independiente de la almohadilla 103 (por ejemplo modificando la longitud de la almohadilla sin alterar la anchura de la
almohadilla, y viceversa) desprendiendo las secciones 212, 214 delineadas por los cortes 204 o 208. Se contemplan secciones desprendióles adicionales delimitadas por cortes adicionales para permitir la conformación dimensionalmente independiente de la almohadilla 103 y no se pretende que las modalidades ilustradas en la presente sean limitativas. Obviamente, para sitios de herida más pequeños, puede ser útil todavía la remoción de secciones simétricas de la almohadilla 103 y las modalidades de la almohadilla 103 pueden proveer tales secciones, ilustradas aquí como las secciones 218, 217, 216. Por ejemplo, puede ser necesaria la remoción de la sección exterior 218 de la almohadilla 103 a lo largo de los cortes 202. Similarmente, para incisiones más pequeñas se puede requerir sólo la sección interior 216 delineada dentro de los cortes 210.
Las figuras 6-56 ilustran varias modalidades diferentes y vistas de una capa de contacto con heridas 105. Como se indica anteriormente, tal capa de contacto con heridas 105 está diseñada y fabricada preferiblemente de manera que sea mínimamente adherente a un sitio de herida, y más preferiblemente no adherente a un sitio de herida. En el caso de una herida abdominal, la capa de contacto con heridas 105 es de preferencia mínimamente adherente o no adherente a las visceras y otros órganos internos expuestos. La capa de contacto con heridas 105 está fabricada más preferiblemente construidos de un material flexible, por ejemplo polímeros, tales como poliuretano (inclusive Elastollan®), polietileno, politetrafluoroetileno o las mezclas de los mismos.
La capa de contacto con heridas 105 es preferiblemente más grande que la almohadilla 103 de material alveolar porque, cuando se usa, se puede plegar entonces la capa de contacto con heridas 05 alrededor y en un sitio de herida. Por ejemplo, cuando se usa en una herida abdominal, se inserta la capa de contacto con heridas 105 preferiblemente en la cavidad abdominal entre los intestinos. Preferiblemente, se dispone la capa de contacto con heridas 105 de manera que se evite que la almohadilla 103 haga contacto con las visceras abdominales y otros órganos internos, aunque puede ser aceptable el contacto con las orillas de la incisión abdominal.
En el transcurso del tratamiento que utiliza el sistema descrito anteriormente, la capa de contacto con heridas 105 es preferiblemente permeable, por ejemplo provista de aberturas, tales como agujeros, hendiduras o canales. Estas aberturas pueden ser útiles en particular en el tratamiento del síndrome compartimiento abdominal, donde se pueden utilizar estas aberturas para canalizar las cantidades a menudo copiosas de material exudado y otros líquidos que se puedan producir. Además de ayudar a la remoción de material exudado y otros líquidos de un sitio de herida, las aberturas son útiles para transmitir y distribuir la presión negativa al sitio de herida. Preferiblemente, las aberturas son suficientemente pequeñas para evitar el crecimiento del tejido hacia dentro, pero suficientemente grandes para evitar la oclusión. Adicionalmente, algunas modalidades de la capa de contacto con heridas 105 pueden estar provistas de una capa microperforada de contacto con heridas. Diferentes modalidades de la capa de contacto con heridas 105 (por ejemplo como se ilustra en las figuras 6, 1 1 , 16, 21 , 26, 32, 37, 42, 47, 52) pueden conferir también ventajas durante la fabricación, como facilidad de producción. La fabricación de la capa de contacto con heridas 105 puede implicar aberturas u agujeros cortantes por ejemplo con troquel o cuchillas de troquelado, perforadores rotatorios, chorros de agua, cortes con láser, o con ultrasonido.
El tratamiento de heridas con presión negativa requiere generalmente limpiar la herida de manera médicamente aceptable, llenarla opcionalmente de un material de relleno para heridas de algún tipo (tal como material alveolar), sellarla con una venda y conectarla a una fuente de presión negativa. El tratamiento de heridas que exponen órganos internos, vasos sanguíneos y nervios, y en particular aquéllas en la cavidad abdominal, pueden necesitar consideraciones particulares. En primer lugar, los típicos materiales de relleno para heridas, tales como material alveolar o gasa, no se pueden poner convenientemente en contacto directo con las visceras abdominales, tales como los intestinos o el estómago, ya que estos materiales pueden adherirse inconvenientemente a las visceras. En vez de eso, se coloca preferiblemente una capa de contacto con heridas 105 no adherente o mínimamente adherente, descrita anteriormente, en la cavidad abdominal o el sitio de herida 10. Esta capa de contacto con heridas 105 se corta preferiblemente al tamaño exacto (si es necesario) y se pliega entre las visceras y la fascia abdominal, con algún exceso de material doblado sobre sí mismo para evitar atrapar algo del contenido abdominal. Subsiguientemente,
la almohadilla 103 de material alveolar, después de ser conformada como se describe anteriormente, se coloca sobre la capa de contacto con heridas y preferiblemente hacia el centro del sitio de herida 10. Se corta una venda 107 al tamaño exacto (si es necesario), otra vez como se describe anteriormente, y se coloca preferiblemente de manera que se traslape por lo menos sobre una porción de piel sana que rodee el sitio de herida 110. En algunos casos, si es insuficiente una venda 107, se pueden proveer vendas adicionales; éstas se traslapan preferiblemente por lo menos en parte para permitir realizar un sello seguro. Preferiblemente, la venda 107 está provista de una capa adhesiva en su cara inferior, la cual se puede proteger con una capa de refuerzo. Se quita tal capa de refuerzo preferiblemente antes de que se utilice para permitir que la venda 107 se adhiera a la piel que rodea el sitio de herida así como a la almohadilla 103 de material alveolar. Se puede realizar luego una abertura 109 a través de la venda 107, aunque algunas modalidades pueden comprender una venda 107 provista de una o más aberturas 109 previamente realizadas. Un conducto 1 12 conectado a una fuente de presión negativa se puede conectar luego a la abertura 109 o, en algunas modalidades, bajo un lado de la venda 107, de tal manera que se cree una conexión de fluido entre el sitio de herida 1 10 y la fuente de presión negativa. La conexión de fluido permite la aplicación terapéutica de presión negativa al sitio de herida 1 10 y se puede aplicar según sea necesario hasta que el sitio de herida 1 10 haya alcanzado un nivel deseado de cicatrización o hasta que se requiera otra intervención quirúrgica.
Por supuesto, la descripción precedente es la de ciertas características, aspectos y ventajas de la presente invención, a los cuales se les puede hacer varios cambios y modificaciones sin apartarse del espíritu y el alcance de la presente invención. Además, el sistema de tratamiento con presión negativa expuesto en la presente no debe representar todos los objetos, las ventajas, las características y los aspectos discutidos anteriormente. Los expertos en la técnica reconocerán que se puede modalizar la invención o que se puede llevar a cabo de manera que logre u optimice una ventaja o un grupo de ventajas como s enseña en la presente sin lograr necesariamente otros objetos o ventajas como se puede enseñar o sugerir en la presente. Por ejemplo, en algunas modalidades se puede utilizar la almohadilla 103 sin la capa de contacto con heridas 105 y/o la venda 107. Además, si bien se han mostrado y descrito con detalle ciertas variaciones de la invención, otras modificaciones y métodos de uso, que estén dentro del alcance de esta invención, serán fácilmente evidentes para los expertos en la técnica con base en esta descripción. Se contempla que se pueden hacer varias combinaciones o combinaciones secundarias de estas características y aspectos específicos de modalidades y figurar todavía dentro del alcance de la invención. Por consiguiente, se debe entender que varias características y aspectos de las modalidades expuestas se pueden combinar uno con otro o sustituir uno por otro para formar los varios modos del sistema de tratamiento con presión negativa que se discute.
Claims (1)
- NOVEDAD DE LA INVENCIÓN REIVINDICACIONES 1. - Una almohadilla porosa para el tratamiento de heridas mediante la aplicación de presión negativa, en donde la almohadilla comprende: un material poroso adecuado para canalizar el material exudado de una herida del sitio de herida y presión negativa al sitio de herida; una forma generalmente plana con un grosor menor a la anchura y la longitud; y por lo menos un corte o una perforación que se extiende por lo menos a través de una porción del grosor de la almohadilla, en que el corte o la perforación define una sección de almohadilla desprendible de la almohadilla para modificar el tamaño de la almohadilla de manera dimensionalmente independiente, en donde que una longitud y una anchura de la almohadilla se pueden modificar independientemente una de otra, permitiendo desprender una porción de la almohadilla. 2. - La almohadilla de conformidad con la reivindicación 1 , caracterizada además porque la almohadilla comprende un material alveolar reticulado con alvéolos abiertos. 3. - La almohadilla de conformidad con cualquiera de las reivindicaciones 1 y 2, caracterizada además porque la almohadilla comprende a material alveolar de poliuretano. 4. - La almohadilla de conformidad con cualquiera de las reivindicaciones precedentes, caracterizada además porque la almohadilla comprende por lo menos dos perforaciones o cortes arqueados que se extienden sobre la longitud de la almohadilla y por lo menos dos perforaciones o cortes arqueados que se extienden sobre la anchura de la almohadilla. 5.- La almohadilla de conformidad con cualquiera de las reivindicaciones precedentes, caracterizada además porque comprende una pluralidad de perforaciones o cortes exteriores que tienen forma elíptica y una pluralidad perforaciones o cortes interiores que tienen forma elíptica similar. 6. - La almohadilla de conformidad con cualquiera de las reivindicaciones precedentes, caracterizada además porque comprende adicionalmente una pluralidad de perforaciones o cortes intermedios entres los cortes exteriores y las perforaciones o cortes interiores, en que por lo menos algunos de las perforaciones o cortes intermedios se extienden por lo general a lo largo a través de la almohadilla y por lo menos algunos de las perforaciones o cortes intermedios se extienden por lo general a lo ancho a través de la almohadilla. 7. - Un sistema para el tratamiento de un sitio de herida mediante la aplicación de presión negativa, sistema que comprende: una capa de contacto con heridas, en donde la capa de contacto con heridas está provista de una o más aberturas para canalizar el material exudado de una herida y distribuir la presión negativa; una almohadilla porosa generalmente plana, adecuada para la transmisión de presión negativa a un sitio de herida y provista de un grosor menor a su anchura y longitud, en donde la almohadilla comprende adicionalmente por lo menos un corte que se extiende por lo menos a través de una porción del grosor de la almohadilla para definir una sección de almohadilla desprendible de la almohadilla para permitir conformar la almohadilla de manera dimensionalmente independiente, en donde la longitud y la anchura de la almohadilla se pueden modificar independientemente una de otra; una venda flexible configurada para colocarla sobre la almohadilla porosa; una fuente de presión negativa; y un conducto configurado para transmitir presión negativa de la fuente a la venda flexible. 8.- El sistema de conformidad con la reivindicación 7, caracterizado además porque la capa de contacto con heridas está fabricada de un material que es mínimamente adherente la sitio de herida. 9. - El sistema de conformidad con cualquiera de las reivindicaciones 7 u 8, caracterizado además porque la capa de contacto con heridas comprende poliuretano. 10. - El sistema de conformidad con cualquiera de las reivindicaciones 7-9, caracterizado además porque la almohadilla porosa comprende por lo menos dos cortes arqueados que se extienden sobre la longitud de la almohadilla y por lo menos dos cortes arqueados que se extienden sobre la anchura de la almohadilla. 1 1. - El sistema de conformidad con cualquiera de las reivindicaciones 7-10, caracterizado además porque la almohadilla porosa comprende un material alveolar poroso de poliuretano. 12.- El sistema de conformidad con cualquiera de las reivindicaciones 7-1 1 , caracterizado además porque comprende adicionalmente una vía de acceso conectable a una abertura formada en la venda y al conducto. 3 - Un método de tratar un sitio de herida mediante la aplicación de presión negativa, en donde el tratamiento comprende: colocar una capa de contacto con heridas sobre el sitio de herida; colocar una almohadilla porosa sobre la capa de contacto con heridas, en donde la almohadilla porosa comprende una pluralidad de perforaciones que dispuestas de manera que permitan la remoción de porciones de la almohadilla porosa de manera dimensionalmente independiente, en donde una longitud y una anchura de la almohadilla se pueden modificar independientemente una de otra y en donde el paso de colocar la almohadilla porosa comprende remover porciones de la almohadilla porosa por lo menos a lo largo de uno de dichas perforaciones para conformar la almohadilla porosa para que ésta quede justa dentro del sitio de herida; sellar el sitio de herida con una venda flexible configurada para ser posicionada sobre la herida y sellada a la piel que rodea la herida; conectar una fuente de presión negativa al sitio de herida, en donde la fuente de presión negativa se conecta a través de un conducto conectado fluidicamente entre la venda y la fuente de presión negativa; y mantener la aplicación de presión negativa al sitio de herida hasta que el sitio de herida haya alcanzado un nivel deseado de cicatrización. 14.- El método de conformidad con la reivindicación 13, caracterizado además porque comprende adicionalmente almacenar el material exudado de la herida y otros fluidos removidos de la herida en un recipiente. 15. - El método de conformidad con cualquiera de las reivindicaciones 13 o 14, caracterizado además porque comprende adicionalmente conectar el conducto a la venda a través de una vía de acceso aplicada a una abertura en la venda. 16. - El método de conformidad con cualquiera de las reivindicaciones 13-15, caracterizado además porque comprende colocar la capa de contacto con heridas sobre un sitio de herida abdominal. 17. - El método de conformidad con cualquiera de las reivindicaciones 13-16, caracterizado además porque el paso de colocar la almohadilla porosa comprende remover porciones la almohadilla porosa por lo menos a lo largo de una de una pluralidad de perforaciones abdominales formadas en un patrón elíptico sobre la almohadilla porosa.
