Description
本出願は、2014年1月21日に出願した「COLLAPSIBLE DRESSING FOR NEGATIVE PRESSURE WOUND TREATMENT」という名称の米国仮出願第61/929,870号の利益を主張するものである。前述の出願の内容は、本明細書に完全に記載されているかのように、その全体が参照により本明細書に組み込まれる。前述の出願の優先権の利益は、限定するものではないが、米国特許法第119条(e)項を含む、適切な法的根拠の下で主張される。

安定化構造物は、垂直方向平面内よりも水平方向平面内でより大きく潰れることで、創傷被覆材が陰圧下に設置されたときに創傷の周囲の皮膚に水平力がかかるように構成されている。
創傷の周囲の皮膚の上に安定化構造物を位置付けて、創傷の上に創傷被覆材を設置するステップと、

安定化構造物は、陰圧下に設置されたときに、創傷の周囲の皮膚に水平力をかける。
裏打ち層、吸収層、及び吸収層の下に位置付けられた安定化構造物を備える創傷被覆材であって、裏打ち層、吸収層、及び安定化構造物が、創傷の上に同時に設置させるための単一のユニットとして構成され、安定化構造物が、創傷の周囲の皮膚の上に設置させるように構成されている、創傷被覆材

吸収層は、複数の貫通孔を備え、
安定化構造物は、垂直方向平面内よりも水平方向平面内でより大きく潰れることで、創傷被覆材が陰圧下に設置されたときに創傷の周囲の皮膚に水平力がかかるように構成されている。
図4A〜図4Dは、安定化構造物1701の実施形態の様々な図を示している。安定化構造物は、創傷の上に設置されたときにどの方向に向けられてもよいが、より好ましくは、水平面において優先的に潰れるように方向付けられる。図4A〜図4Dに示された構造物が、創傷被覆材に使用される安定化構造物の一部分だけを含み得るように、或いは、図4A〜図4Dに示された構造物の一部分だけが、創傷被覆材に使用され得るように、安定化構造物1701は、創傷被覆材の設置面積の中に適合するように適切に寸法決めされ得る。
図6A〜図6Eは、安定化構造物1100の追加実施形態を示す。図6Aは、安定化構造物1100の一実施形態の斜視図を示す。ここで、安定化構造物1100は、好ましくは、実質的に潰されない構成であるとき互いにほぼ直交する方向に延びる(以下で図6Bに関してより詳細に説明する)2つ以上のインターロッキングストリップからなる。この安定化構造物は、好ましくは、依然として相対的に剛性かつ第1の方向または第1の平面に垂直な方向に潰れ抵抗性でありながらも、一方向に、または第1の平面に沿って、潰れるように構成される。
図15Aは、創傷の上に設置され得ると共に創傷被覆材の中に組み込まれ得る安定化構造物2100の一実施形態の写真である。ここで、このデバイスは、略平面状構成で並んで設けられた複数のセル2102を備える。好ましくは、安定化構造物2100は、平面2101に垂直な方向に大きく潰れることなく、デバイスの幅によって画定された平面2101に沿った方向に潰れるように構成される。すなわち、図で見ると、安定化構造物2100は水平方向に潰れるが、垂直方向に圧縮されない。いくつかの実施形態では、安定化構造物は、組織の動きに関連して潰れる。ここで、セル2102は、好ましくは、両端で平面2101に垂直な方向に開いている。
また、いくつかの実施形態は、壁2104または継手2106の中に穴も備えてもよいし、多孔性材料から構築されてもよい。
これらの測定値は、本項または本明細書の別の場所で説明する任意の安定化構造物に適用されてよい。
いくつかの実施形態では、本項又は本明細書の他の箇所で説明した安定化構造物の潰れはゆっくりと発生し、それによって、増加しつつある長手方向張力を長期間にわたって加えることができる。特定の実施形態では、構造物の潰れおよび伸長は、陰圧の印加直後に発生することができる。さらなる実施形態では、潰れは、いかなる速度でも発生することができる。
バーは、好ましくは、隣接する細長いストリップ間の列の中で等間隔に離間し、隣接する列の中で段差があるかまたはずらされてよく、したがって、隣接する列では、バーは、第1の列のバーの間のある場所で細長いストリップに接続される。他の実施形態では、介在部材は、隣接する細長いストリップの間に延びるように構成されたワイヤまたは他の細長い構造を備えることができる。
図29A〜図29Fは、図22A〜図25に関して上記で説明した実施形態に類似した安定化構造物4200の実施形態を示す。安定化構造物は、並列に配置された複数の細長いストリップ4202を備えてよく、ストリップ4202の長手方向長さは、創傷の上に設置されたとき、創傷の長手方向軸と位置合わせすることができる。安定化構造物は、継手4206を介して細長いストリップ4202に接続された複数の介在部材4204をさらに備えることができる。特定の実施形態では、安定化構造物4200は、陰圧の印加を用いてまたは用いずに、本項または本明細書の別の場所で説明する任意の形で潰れることができる。たとえば、安定化構造物は、一平面内で、別の平面内よりも著しく大きく潰れてよい。いくつかの実施形態では、安定化構造物は、シリコーンなどの軟質プラスチック、ポリウレタン、ポリ塩化ビニルなどの硬質プラスチック、半硬質プラスチック、半軟質プラスチック、生体適合性材料、複合材料、金属、およびフォームを含む、本項または本明細書の別の場所で説明する任意の材料からなることができる。
図34A〜図34Bは、2種類の組織アンカー5702,5704を備える固着層5700の実施形態の写真である。本明細書で説明されたような1つ以上の固着層又はアンカーは、本明細書で説明されたいずれかの安定化構造物の任意の適切な表面上に設けられることで、組織への接着を促進し得る。例えば、1つ以上の固着層又はアンカーは、安定化構造物のうち、皮膚に面する表面上に設けられ得る。特定の実施形態では、組織アンカー5702,5704は、Velcro社によって生産されたものや、種々のバーブ、及び/又は種々のフックなどのアンカーを備えてもよい。アンカー、例えば図34A〜図34B又は本明細書のその他の箇所に関して説明されたものなどは、皮膚組織などの種々の組織をグリップ又は穿通するのに使用され得る。さらに、アンカーの構造は、組織に応じて様々な形状を有することが可能であり、それらは、穿通及びグリップするように意図されている。例えば、より長いアンカーは、脂肪または連結組織などの緩く結合された組織に対して使用され得る一方で、より短いアンカーは、筋肉などのより密度の高い組織に対して使用され得る。アンカーの形状に応じて、より短いアンカーが、軟質の脂肪組織にとってより望ましいものとなる場合があり、より長いアンカーが、より密度の高い組織対して使用される。より高い剛性のステムを有するアンカーが、より高い密度の組織に穿通するために使用され得る。
図35は、図1〜図3Bに関して説明された創傷被覆材と同様に、陰圧創傷療法に使用される被覆材6000の断面図を示している。この図は1つの特定の形状を有する被覆材を示しているが、複数の層の構造は本項や本明細書の他の箇所で説明された実施形態のいずれかに適用することができる。本項又は本明細書の他の箇所でより詳細に説明されているように、特定の実施形態では、被覆材の種々の構成要素が、任意的であり得る。例えば、被覆材は、本項や本明細書の他の箇所で説明された全ての層及び構成要素を含んでもよく、或いは、被覆材は、複数の層のいくつかだけを含んでもよい。

Claims (15)
- 裏打ち層及び前記裏打ち層の下に位置付けられた安定化構造物を備える創傷被覆材であって、前記裏打ち層及び前記安定化構造物が、創傷の上に同時に設置させるための単一のユニットとして構成され、前記安定化構造物が、前記創傷の周囲の皮膚の上に設置させるように構成されている、創傷被覆材と、
前記創傷被覆材に陰圧を伝えるためのポートと、
を備える陰圧創傷治療装置であって、
前記安定化構造物は、前記創傷被覆材が陰圧下に設置されたときに、垂直方向平面内よりも水平方向平面内で大きく潰れることで、前記創傷の周囲の皮膚に水平力がかかるように構成されている、装置。 - 前記創傷被覆材が、創傷接触層をさらに備えており、前記安定化構造物が、前記裏打ち層と前記創傷接触層との間に位置付けられている、請求項1に記載の装置。
- 前記創傷被覆材が、前記安定化構造物と前記裏打ち層との間に取得分散層をさらに備えている、請求項1又は2に記載の装置。
- 前記創傷被覆材が、前記安定化構造物と前記裏打ち層との間に吸収層をさらに備えている、請求項1から3のいずれか一項に記載の装置。
- 前記創傷被覆材を前記創傷の周囲の皮膚に取り付けるように構成された接着剤をさらに備える、請求項1から4のいずれか一項に記載の装置。
- 前記創傷被覆材が、前記創傷に適用された縫合糸に印加される応力を解放させるように構成されている、請求項1から5のいずれか一項に記載の装置。
- 前記裏打ち層は、透明又は半透明である、請求項1から6のいずれか一項に記載の装置。
- 前記安定化構造物は、長さ又は幅の20%未満の厚さを有する、請求項1から7のいずれか一項に記載の装置。
- 裏打ち層、吸収層、及び前記吸収層の下に位置付けられた安定化構造物を備える創傷被覆材であって、前記裏打ち層、前記吸収層、及び前記安定化構造物が、創傷の上に同時に設置させるための単一のユニットとして構成され、前記安定化構造物が、前記創傷の周囲の皮膚の上に設置させるように構成されている、創傷被覆材
を備える陰圧創傷治療装置であって、
前記吸収層が、複数の貫通孔を備え、
前記安定化構造物が、前記創傷被覆材が陰圧下に設置されたときに、垂直方向平面内よりも水平方向平面内で大きく潰れることで、前記創傷の周囲の皮膚に水平力がかかるように構成されている、装置。 - 前記複数の貫通孔のうちの少なくともいくつかには、プラグ材料が詰められ、垂直方向の剛性が増大した前記吸収層が提供されている、請求項9に記載の装置。
- 前記プラグ材料が透明である、請求項10に記載の装置。
- 前記吸収層は、垂直方向平面内よりも水平方向平面で大きく潰れるように構成されている、請求項9から11のいずれか一項に記載の装置。
