Description
- Diese Erfindung betrifft allgemein Dilatatoren (auch als Einspreizkörper bezeichnet), die verwendet werden, um ein Körperlumen, wie einen Kanal bzw. eine Röhre oder ein Gefäß, in einer offenen Konfiguration zu halten, und insbesondere Dilatatoren bzw. Einspreizkörper mit einer hohlen und aufpumpbaren Wand.
- Die Verwendung von Einspreizkörpern, um ansonsten verschlossene oder okkludierte Körperlumina, wie z.B. Kanäle/Röhren oder Gefäße, in einer offenen Konfiguration zu halten, ist eine bekannte Behandlungsmethode. Derzeit üblicherweise verwendete Dilatatoren schließen selbstexpandierende Einspreizkörper, wie sie z.B. im US-Patent 4 655 771 beschrieben sind, und Einspreizkörper ein, die am Lumen-Standort durch einen Ballon expandiert werden, der innerhalb des Einspreizkörpers aufgepumpt wird. In jedem Fall sind die Einspreizkörper gewöhnlich aus Metall konstruiert und besitzen daher einen gewissen Grad an Steifigkeit und eine minimale Biegsamkeit.
- Es ist daher eine Hauptaufgabe der vorliegenden Erfindung, einen Einspreizkörper zur Verfügung zu stellen, dessen Wand beim Einsetzen weich und biegsam ist, der jedoch nach Positionierung am Behandlungsort das richtige Maß an Steifigkeit und Starrheit annehmen kann. Eine andere Aufgabe der vorliegenden Erfindung ist die Bereitstellung eines derartigen Dilatators bzw. Einspreizkörpers, bei dem mindestens ein Teil von dessen Wandstruktur eine hohle und aufpumpbare Wand, hergestellt aus einer semipermeablen Membran, ist. Eine weitere Aufgabe der vorliegenden Erfindung ist die Bereitstellung eines solchen Einspreizkörpers, bei dem in der hohlen Wand ein hydrophiles Material angeordnet ist, das eine Flüssigkeit absorbieren und dadurch das Volumen des Materials erhöhen und demzufolge die Wand aufpumpen kann. Noch eine weitere Aufgabe der vorliegenden Erfindung ist die Bereitstellung eines Einspreizkörpers, bei dem das hydrophile Material ein Arzneimittel mitführt. Diese und weitere Aufgaben der Erfindung werden aus der folgenden Beschreibung ersichtlich.
- Der erfindungsgemäße Einspreizkörper zur Positionierung in einem Körperlumen hat also eine Wandstruktur, bei der wenigstens ein Teil der Struktur eine hohle Wand ist. In dieser befindet sich hydrophiles Material, das beispielsweise in Gelform vorliegen kann und bei Einführung einer Flüssigkeit in die hohle Wand anschwillt, wodurch die hohle Wand "aufgepumpt" oder aufgebläht wird. Die hohle Wand mit dem hydrophilen Material besteht aus semipermeablem Membraninaterial, so daß Flüssigkeit aus dem den Einspreizkörper am Anordnungsort umgebenden Gewebe durch die Membran hindurchtreten und das hydrophile Material zum Aufschwellen bringen kann, wodurch das Aufpumpen erfolgt. Mit dem hydrophilen Material kann eine therapeutische Droge eingeschlossen sein, die am Ort des Einspreizkörpers durch die Membran abgegeben wird. Körperlumina, in denen der erfindungsgemäße Einspreizkörper verwendet werden kann, sind beispielsweise, jedoch nicht ausschließlich, Arterien, Venen, Harnröhre und Harnleiter, Gallen-, Leber- und Bauchspeicheldrüsenkanäle, Luftröhren-, Speiseröhren- und Darmabschnitte, Samenleiter und Eileiter, Ohrtrompete und Tränenkanäle. Es kann die gesamte Wandstruktur des Einspreizkörpers aus einer hohlen Wand bestehen oder die Wandstruktur kann sowohl hohle als auch nicht-hohle Teile umfassen, beispielsweise hohle und massive Fasern, die dadurch zusammengehalten werden, daß sie geflochten, verwoben oder zusammengewickelt sind.
- Die vorliegende Erfindung gemäß Anspruch 1 stellt einen Einspreizkörper bereit, der nach Positionieren und anschließendem Aufpumpen ein Lumen abstützt, der aber auch, weil er in nicht aufgepumpter Konfiguration an seinen Platz verbracht werden kann, während des Einbringens eine dementsprechende kompakte Größe aufweist, so daß die Positionierung innerhalb eines Lumens verbessert wird.
- Derzeit bevorzugte Ausführungsformen der Erfindung werden in den beigefügten Zeichungen erläutert, die zeigen:
- Fig. 1 in perspektivischer Darstellung einen rohrförmigen Einspreizkörper, teilweise geschnitten, dessen gesamte Wandstruktur eine hohle Wand nach Art eines Aufpumpballons ist;
- Fig. 2 eine Seitenansicht einer zweiten Ausführungsform eines rohrförmigen Einspreizkörpers, dessen gesamte Wandstruktur eine hohle Wand aus geflochtenen aufpumpbaren Fasern ist;
- Fig. 3 eine Seitenansicht einer dritten Ausführungsform eines rohrförmigen Einspreizkörpers, teilweise im Schnitt, dessen Wandstruktur einen hohlen Wandteil aus aufpumpbaren Fasern und einen nicht- hohlen Wandteil aus massiven Fasern umfaßt, wobei die hohlen und die massiven Fasern miteinander verflochten sind; und
- Fig. 4 eine perspektivische Ansicht einer vierten Ausführungsform eines Einspreizkörpers, teilweise im Schnitt, dessen gesamte Wandstruktur eine hohle Wand ist, umfassend eine aufpumpbare Helix.
- Fig.1 1 zeigt einen rohrförmigen Dilatator/Einspreizkörper 10 zur Anordnung in einem Körperlumen. Die gesamte Wandstruktur des Einspreizkörpers 10 besteht aus einer hohlen und aufpumpbaren Wand 12 mit einer Ballonstruktur 18, die eine Vielzahl radialer hindurchgehender Öffnungen 16 aufweist, um ein Einwachsen von Gewebe zu erleichtern, wenn der Einspreizkörper 10 sich innerhalb des Körperlumens an Ort und Stelle befindet. Die Wand 12 besteht Aus semipermeablem Membranmaterial, beispielsweise aus zu semipermeablen Membranen bildbaren Polymeren, wie sie an sich bekannt sind, die einem geeigneten Aufpumpdruck standhalten können. Als nicht- beschränkende Beispiele seien genannt Polyamide, Polyester, Polyurethane und Äthylenvinylalkohl. In der hohlen Wand 12 des Einspreizkörpers 10 befindet sich ein hydrophiles Material 32, das über osmotische Verdünnung eine Flüssigkeit absorbieren oder anziehen kann, um hierdurch sein Volumen bzw. den durch das Material 32 ausgeübten Druck zu erhöhen. Als hydrophiles Material 32 kommt jedes beliebige biokompatible Mittel in Frage, das einen osmotischen Druck erzeugt. Als nicht beschränkende Beispiele seien anorganische Salze, organische Salze, verschiedene Zucker, Polysaccharide, polymere Hydrogele oder amphotere Moleküle genannt. Ein bevorzugtes Material ist ein Hydrogel wie Polyvinylalkohol.
- Im Gebrauch wird der Einspreizkörper 10 zunächst in nicht-aufgepumpten Zustand am gewünschten Ort im Körperlumen durch übliche und zweckmäßige Anordnungs- und Transportmittel wie beispielsweise durch einen (nicht dargestellten) Katheter passender Größe verbracht. Durch dieses innerkörperliche Transportmittel wird der Einspreizkörper für eine gegebene Zeitspanne am betreffenden Ort festgehalten, die ausreichend lang ist, um das Eindiffundieren einer zweckentsprechenden Menge von umgebender Gewebeflüssigkeit in die Wand 12 zu ermöglichen, wodurch das hydrophile Material 32 sein Volumen vergrößert und den Einspreizkörper 10 ausfüllt bzw. durch Aufblähen aufpumpt, so daß er durch Andrücken an die innere Lumenwand unabhängig an Ort und Stelle bleibt. Die zur Herstellung der Wand 12 verwendete semipermeable Membran muß verständlicherweise eine ausreichende Festigkeit haben, um nicht durch den Druck zu zerreißen, der durch das expandierende hydrophile Material im Inneren erzeugt wird. Das Einwachsen von Gewebe erfolgt schließlich durch die radialen Öffnungen 16.
- Der Einspreizkörper 10 kann außerdem als Depotvorrichtung zur zeitverzögerten Abgabe von Arzneistoffen verwendet werden. Speziell kann ein Arzneistoff zusammen mit dem hydrophilen Material 32 eingesetzt werden, und zwar entweder als getrennte Komponente oder mit dem Material 32 vermischt. Der Arzneistoff wird dann durch die semipermeable Membran über eine Zeitspanne hinweg in die umgebenden Gewebe abgegeben. Der so eingeschlossene Arzneistoff ist in einer zweckmäßigen Konzentration vorhanden und kann auch noch mit einem Träger kombiniert sein, um die gewünschte Abgabegeschwindigkeit zu erreichen. Das Molekulargewicht des Arzneistoffs sollte niedriger sein als das des hydrophilen Materials. Ein Beispiel für einen solchen Arzneistoff ist Piroxicam, das unter der Handelsbezeichnung Felden von der Pfizer Inc., New York, New York, hergestellt wird und kommerziell erhältlich ist und das in einer Menge von etwa 20 bis 500 mg je Einspreizkörper vorhanden sein kann. Der Arzneistoff kann eine getrennte Komponente sein oder kann im hydrophilen Material durch Mischen eingeschlossen oder in einer Lösung des hydrophilen Materials 32 aufgelöst sein, um später für den therapheutischen Zweck zeitgesteuert vom Einspreizkörper 10 abgegeben zu werden. Für unterschiedliche Anwendungen des Einspreizkörpers können verständlicherweise unterschiedliche Arzneistoffe verwendet werden. Als nicht-beschränkende Beispiele für solche Arzneistoffe seien antithrombische Drogen für cardiovaskuläre Anwendungen, Anti-Verkalkungs-Drogen für urologische Behandlungen und entzündungshemmende oder das Wachstum unterdrückende Drogen für die Unterdrückung von biologischer Reaktion auf die Einspreiz- oder Ballon-Angioplastie genannt.
- Der Einspreizkörper 10 kann konstruiert sein durch Vorsehen von zwei konzentrischen rohrförmigen Membranen, wobei die Innenfläche der äußeren Membran und die Außenfläche der inneren Membran die inneren Wandflächen der hohlen Struktur bilden. Zwischen die beiden Membrane wird Gel eingeführt, woraufhin ein an sich bekannter Membranverschlußvorgang durchgeführt wird, durch den die Enden des Einspreizkörers 10 geschlossen werden und gleichzeitig die radialen Öffnungen 16 geschnitten und abgedichtet werden.
- Fig. 2 zeigt eine zweite Ausführungsform eines rohrförmigen Einspreizkörpers 20 zur Anordnung in einem Körperlumen. Die gesamte Wandstruktur des Einspreizkörpers 20 besteht aus einer hohlen und aufpumpbaren Wand 24 aus einer Vielzahl von geflochtenen hohlen Fasern 26. Während, wie in Fig. 2 dargestellt ist, im wesentlichen die gesamte Wandstruktur aus den geflochtenen hohlen Fasern 26 bestehen kann, zeigt Fig. 3 einen rohrförmigen Einspreizkörper 40 mit einer Wand 44, von der nur ein Teil aus hohlen Fasern 26 besteht, die somit mit massiven Vollfasern 28 verflochten sind. Durch die Wände 24 bzw. 44 verlaufen radiale Öffnungen 29, die das Einwachsen des Gewebes erleichtern, wenn der Einspreizkörper 20 bzw. 40 innerhalb eines Körperlumens angeordnet wurde. Ebenso wie die Ballonstruktur 18 des Einspreizkörpers 10 nach Fig. 1 bestehen auch die hohlen Fasern 26 der Einspreizkörper 20 und 40 aus semipermeablem Membranmaterial, beispielsweise aus Polymeren, die in an sich bekannter Weise zu semipermeablen Membranen geformt werden können. Als nicht begrenzende Beispiele seien in gleicher Weise genannt Polyamide, Polyester, Polyurethane und Äthylenvinylalkohl. In den hohlen Fasern 26 befindet sich ein hydrophiles Material, wie es unter Bezugnahme auf Fig. 1 beschrieben wurde, wobei dieses Material eine Flüssigkeit absorbieren kann, um hierdurch sein Volumen zu erhöhen und dadurch das Aufblähen oder Aufpumpen der Fasern 26 zu bewirken. In gleicher Weise wie bei der oben beschriebenen Ausführungsform kann sich beim hydrophilen Material ein Arzneistoff befinden, der in das umgebende Gewebe über die semipermeable Membran der Fasern 26 über eine Zeitspanne freigegeben wird.
- Der Einspreizkörper 20, 40 wird in gleicher Weise wie bei der Ausführungsform nach Fig. 1 am gewünschten Ort innerhalb des Lumens angeordnet. In ebenfalls gleicher Weise wird diese Stelle durch das Transportmittel am Behandlungsort oder Ort der gewünschten Positionierung für eine Zeitspanne aufrechterhalten, die die Diffusion einer angemessenen Menge von umgebender Gewebeflüssigkeit in die Fasern 26 ermöglicht, wodurch das hydrophile Material anschwillt und den Einspreizkörper 20 bzw. 40 so aufpumpt oder aufbläht, daß er durch Andrücken an die innere Lumenwand an seiner Stelle verbleibt. Es ist klar, daß die zur Herstellung der hohlen Fasern 26 verwendete semipermeable Membran von ausreichender Festigkeit sein muß, um nicht aufgrund des darin durch das expandierende hydrophile Material erzeugten Drucks zu zerreißen. Das Einwachsen von Gewebe erfolgt durch die radialen Öffnungen 29.
- Eine hier beschriebene Möglichkeit der Herstellung des Einspreizkörpers 20 besteht in der Anwendung von an sich bekannten Lösungsmittelgießtechniken. So wird eine entsprechend geformte Form bereitgestellt, wobei eine Lösung eines Polymeres von einem Teil der Form gepumpt wird, um eine hohle Wand zu bilden. Gleichzeitig wird ein hydrophiles Material etwa in Gelform von einem anderen Teil der Form zentral zur Polymerlösung gepumpt. Wenn die Polymerlösung und das Gel ein Koagulationsbad erreichen, das bei einem solchen Lösungsmittelgießen vorgesehen ist, ist das Gel vom Polymer umgeben, während die struktur abbindet. Alternativ kann natürlich das Gel auch unter Druck in eine Faserlänge eingegeben werden, woraufhin das Faserende verschlossen wird.
- Fig. 4 zeigt eine vierte Ausführungsform mit einem Einspreizkörper 50, dessen gesamte Wandstruktur aus einer hohlen Wand 52 besteht. Speziell hat die hohle und aufpumpbare Wand 52 des Einspreizkörpers 50 eine Ballonstruktur 54, die im aufgepumpten Zustand wie dargestellt eine Helixkonfiguration aufweist, jedoch zum Behandlungsort innerhalb eines Lumens in nicht-aufgepumpter, gerader Konfiguration transportiert werden kann. Ebenso wie bei den Einspreizkörpern gemäß den Fig.n 1 bis 3 besteht auch die Wand 52 des Einspreizkörpers 50 aus semipermeablem Membranmaterial, beispielsweise aus Polymerem, das in an sich bekannter Weise zu semipermeablen Membranen geformt werden kann. Als nicht begrenzende Beispiele seien Polyamide, Polyester, Polyurethane und Äthylenvinylalkohl genannt. Innerhalb der Wand 52 befindet sich, wie es unter Bezugnahme auf Fig. 1 beschrieben wurde, ein hydrophiles Material, das Flüssigkeit absorbieren kann und hierdurch sein Volumen erhöht, wodurch das Aufpumpen erfolgt. Zumindest ein Teil der Wand 52 kann mit einer Faserverstärkung 56 wie beispielsweise Polyester, Nylon, Polypropylen, vorzugsweise jedoch durch einen Polyester, verstärkt sein. Eine Art, die Verstärkung 56 bei der Herstellung an der Wand 52 zu schaffen, besteht darin, Fasern um die Struktur zu flechten und dann eine überbeschichtung der semipermeablen Membran aufzubringen. Eine solche Verstärkung gibt dem Einspreizkörper 50 eine größere Festigkeit.
- In der hohlen Wand befindet sich hydrophiles Material, und der Einspreizkörper 50 wird in der im Zusammenhang mit den Fig.n 1 bis 3 beschriebenen Weise am gewünschten Platz innerhalb des Lumens angeordnet. Diese Position wird durch das Transportmittel am Ort der gewünschten Positionierung für eine Zeitspanne aufrecht erhalten, die ausreicht, um die Diffusion einer passenden Menge von Flüssigkeiten der umgebenden Gewebe in die Wand 52 zu ermöglichen, wodurch das hydrophile Material anschwillt und den Einspreizkörper 50 so aufbläht, daß er seine Helixkonfiguration annimmt und durch den Andruck auf die innere Lumenwand unabhängig an Ort und Stelle verbleibt. Natürlich muß die verwendete semipermeable Membran, aus der die Wand 52 besteht, von ausreichender Festigkeit sein, um nicht durch den im Inneren durch das expandierende hydrophile Material erzeugten Druck zu platzen.
- Der in Fig. 4 dargestellte Einspreizkörper 50 kann auch als Depot für die über die Zeit verteilte Abgabe von Arzneistoffen verwendet werden. Der Arzneistoff kann im hydrophilen Material innerhalb der Wand 52 in der beschriebenen Weise angeordnet sein und wird über eine Zeitspanne hinweg durch die semipermeable Wandstruktur in die umgebenden Gewebe abgegeben.
Claims (19)
1. Dilatator bzw. Einspreizkörper zur Positionierung
innerhalb eines Körperlumens, der eine Wandstruktur umfaßt, bei
der mindestens ein Teil derselben eine hohle Wand (12) ist,
dadurch gekennzeichnet, daß die hohle Wand (12) aus einer
semipermeablen Membran hergestellt ist.
2. Einspreizkörper nach Anspruch 1, bei dem in der hohlen
Wand (12) ein hydrophiles Material (32) angeordnet ist, das
eine Flüssigkeit absorbieren kann, um dadurch das Volumen des
Materials zu erhöhen.
3. Einspreizkörper nach Anspruch 2, bei dem das hydrophile
Material (32) ein Gel ist.
4. Einspreizkörper (10) nach Anspruch 2, bei dem dessen
Konfiguration röhrenformig ist.
5. Einspreizkörper (10) nach Anspruch 4, bei dem die hohle
Wand (12) einen Ballon (18) umfaßt.
6. Einspreizkörper (10) nach Anspruch 5, bei dem die hohle
Wand (12) eine Vielzahl hindurchgehender radialer Öffnungen
(16) aufweist.
7. Einspreizkörper (10) nach Anspruch 2, bei dem die hohle
Wand (12) eine Vielzahl hindurchgehender radialer Öffnungen
(16) aufweist.
8. Einspreizkörper (20) nach Anspruch 2, bei dem die hohle
Wand (24) hohle Fasern (26) umfaßt die durch Verflechten,
Verweben oder Zusammenwickeln zusammengehalten sind.
9. Einspreizkörper (40) nach Anspruch 8, bei dem die
Wandstruktur zusätzlich kompakte Fasern (28) umfaßt, die mit den
hohlen Fasern (26) durch Verflechten, Verweben oder
Zusammenwickeln zusammengehalten sind.
10. Einspreizkörper nach Anspruch 2, bei dem mit dem
hydrophilen Material (32) ein Arzneimittel angeordnet ist.
11. Einspreizkörper nach Anspruch 10, bei dem das
hydrophile Material (32) ein Gel ist.
12. Einspreizkörper (10) nach Anspruch 10, bei dem die
hohle Wand (12) einen Ballon (18) umfaßt.
13. Einspreizkörper (20) nach Anspruch 10, bei dem die
hohle Wand (24) hohle Fasern (26) umfaßt, die durch
Verflechten, Verweben oder Zusammenwickeln zusammengehaltn sind.
14. Einspreizkörper (40) nach Anspruch 13, bei dem die
Wandstruktur zusätzlich kompakte Fasern (28) umfaßt, die mit
den hohlen Fasern (26) durch Verflechten, Verwerben oder
Zusammenwickeln zusammengehalten sind.
15. Einspreizkörper (50) nach Anspruch 2, bei dem dessen
Konfiguration nach dem Aufblasen eine Spirale ist.
16. Einspreizkörper (50) nach Anspruch 15, bei dem die
hohle Wand (52) einen Ballon (54) umfaßt.
17. Einspreizkörper (50) nach Anspruch 16, bei dem
mindestens ein Teil der hohlen Wand (52) verstärkt ist.
18. Einspreizkörper (50) nach Anspruch 15, bei dem das
hydrophile Material (32) ein Gel ist.
19. Einspreizkörper (50) nach Anspruch 18, bei dem mit dem
hydrophilen Material (32) ein Arzneimittel angeordnet ist.
