Description
技术领域

背景技术
含有肽的药物制剂中包含等渗剂是公知的,这样制剂中所用的较常见等渗剂之一是甘露醇。然而,本发明者观察到甘露醇在生产肽制剂过程中引起了问题,因为其结晶导致生产设备和成品中的沉积物。这样的沉积物增加了生产制剂过程中清洗装填设备的需要,且这导致了降低的生产能力。此外,这样的沉积物还导致了降低的成品产量,因为含有肽制剂的小瓶/药筒如果存在颗粒就需要废弃。最后,本发明者观察到在通过注射给药的肽制剂中,甘露醇的存在导致注射装置的阻塞。
因此,需要鉴定甘露醇的替代等渗剂,用于包含于含有肽的制剂中,尤其是用于包含于通过注射给药的肽制剂中。
发明内容
本发明者发现含有一定浓度丙二醇的肽制剂呈现出生产设备和成品中减少的沉积物,还呈现出注射装置减少的阻塞。本发明的组合物可以与任何肽配制,并且还是物理和化学稳定的,因此使得它们耐存储并适于入侵方式的给药(例如,注射,皮下注射,肌内,静脉内或灌输)以及非入侵方式的给药。
因此本发明涉及包含肽和丙二醇的药物制剂,其中丙二醇的存在浓度为1-100mg/ml,制剂的pH为7-10。在优选实施方案中,本发明的药物制剂进一步含有缓冲剂和防腐剂。
本发明还涉及制备本发明药物制剂的方法。
在一个实施方案中,制备肽制剂的方法包括:
a)将防腐剂、丙二醇和缓冲剂溶解于水中制得第一种溶液;
b)将肽溶解于水中制得第二种溶液;
c)将第一种和第二种溶液混合;和
d)将c)中混合物的pH调节至所需的pH。
在另一个实施方案中,制备肽制剂的方法包括:
a)将防腐剂和缓冲剂溶解于水中制得第一种溶液;
b)将丙二醇加入第一种溶液中;
c)将第一种溶液与含有溶解于水中的肽的第二种溶液混合;和
d)将c)中混合物的pH调节至所需的pH。
在再一个实施方案中,制备肽制剂的方法包括:
a)将防腐剂、缓冲剂和丙二醇溶解于水中制得溶液;
b)将肽加入步骤a)的溶液中;和
c)将步骤b)溶液的pH调节至所需的pH。
本发明进一步涉及使用本发明药物制剂的治疗方法,其中给予有效量的组合物来抗击疾病、病症或障碍,为此制剂中所含肽的给药得到表明。

发明详述
本发明涉及含有肽或肽混合物和丙二醇的药物制剂,其中制剂中丙二醇的终浓度为1-100mg/ml,制剂的pH为7-10。
发现本发明的药物制剂对于生产是最佳的,因为它们相对于含有其他等渗剂的制剂呈现出生产设备中减少的沉积物,如通过实施例中所述的模拟装填研究所测量的。此外,发现本发明的药物制剂对于用于注射装置中是最佳的,因为它们相对于含有其他等渗剂的制剂呈现出注射装置减少的阻塞,如通过实施例中所述的模拟使用研究所测量的。

本发明的制剂对于生产和用于注射装置中是最佳的,因为它们呈现出生产设备上沉积物的减少和注射装置中阻塞的减少,上述的生产方法可以用于生产适用于生产和/或用于注射装置中的肽制剂。
本发明的制剂适于给药于哺乳动物,优选人。本发明制剂的给药途径可以是将制剂中所含肽有效地运送至合适或所需作用部位的任何途径,如口服、鼻、口腔含化、肺、经皮或非肠道给药。
当和其他等渗剂尤其是和甘露醇相比较时,由于丙二醇减少注射装置阻塞的能力,在优选实施方案中,将本发明的制剂非肠道给药于有此需要的患者。通过注射器,任选地笔式注射器的皮下、肌内、或静脉内注射来进行非肠道给药。或者,可以通过灌输泵来进行非肠道给药。
另外的选择是适于制剂以鼻或肺喷雾形式给药的组合物,其可以是粉末或液体。作为再进一步的选择,还可以经皮来给予制剂,例如,从贴片,任选地离子电渗贴片,或经粘膜给药,例如,通过口腔含化。本发明制剂上述可能的给药方式不被认为是对本发明范围的限制。

实施例1
甘露醇替代侯选物的模拟装填实验,点滴试验和阻塞试验
由于实验室实验已经表明,对于针的阻塞和针上的沉积物,没有肽的制剂(“安慰剂”)获得与含有0.3-5.0mg/ml肽的制剂相同的结论,所以已经使用安慰剂(除非另外表明)进行了实施例1中的筛选研究。
制备含有不同等渗剂的制剂
将防腐剂(5.5mg/ml苯酚)和缓冲剂(1.24mg/ml磷酸氢二钠,二水合物)溶解于水中,边搅拌边加入等渗剂。用氢氧化钠和/或盐酸将pH调节至pH7.9。最后,将制剂通过0.22μm滤器过滤。每种制剂中测试的等渗剂和它们的浓度显示于表1中。

摩尔渗透压浓度
测定不同安慰剂的摩尔渗透压浓度并将结果显示于表2中。
等渗溶液具有约0.286osmol/L的摩尔渗透压浓度。从表2中可以看出,三种制剂(PEG400,蔗糖和木糖醇)离等渗超出20%(0.229-0.343osmol/l),然而,对于这些种类的实验,不期望摩尔渗透压浓度影响结果,尽管,应该在将来的实验中调节制剂的张力。

点滴试验
将每种制剂的液滴放置于显微镜载玻片上并使其干燥。通过眼睛和光学显微镜目视检查沉积物。
一些制剂干燥液滴的照片显示于图1中。在该图中,清楚地观察到甘露醇当使其干燥时在显微镜载玻片上形成沉积物。对于山梨糖醇、木糖醇、蔗糖和甘油没有观察到沉积物。最右边的液滴(形状1)含有甘露醇和Arg34,Lys26(Nε-(γ-Glu(Nα-十六烷酰)))-GLP-1(7-37)。
在图2中,显示了在显微镜载玻片上形成最多沉积物的侯选物。为了比较,显示了甘油,其没有形成沉积物(甘露醇,精氨酸,肌醇)。
阻塞试验
在该试验中,用装有(G30针)的10个1.5ml测试每种制剂,将它们中的5个垂直放置,5个水平放置。在测试期间将Pensystems存储于室温。每天,检查针上的沉积物,并在注射进组织之前进行空打。如果存在,记录阻力和阻塞的程度。用相同的针每天进行注射,对于所有制剂,进行9个工作日。
阻塞试验的结果显示于表3中。

在表3和图3中,观察到肌醇和麦芽糖阻塞了针。为了比较,图3中显示了没有阻塞针的甘油。在图4和表3中,观察到含有甘氨酸、乳糖和甘露醇的制剂在针上形成了很多沉积物。对于甘氨酸,沉积物是沿着针往下沉积的液滴,而对于乳糖和甘露醇,沉积物出现在针尖上。
模拟装填
对每种制剂的1L进行持续24小时的模拟装填实验。24小时后,检查装填设备上沉积物的存在。
基于模拟装填研究的结果(数据未显示),安慰剂可以分成三种类型。1.没有在装填设备上形成沉积物的那些等渗剂:木糖醇、甘油、葡萄糖一水合物、麦芽糖、PEG400和丙二醇。2.形成少量沉积物且与甘露醇相比较具有优异装填特性的那些等渗剂:山梨糖醇、蔗糖和甘氨酸。3.与甘露醇类似或比甘露醇更差的那些等渗剂:甘露醇、乳糖一水合物,精氨酸,肌醇和二甲砜。
结论
在模拟装填实验中,发现木糖醇、甘油、葡萄糖、麦芽糖、PEG400、丙二醇、山梨糖醇、蔗糖和甘氨酸适于作为甘露醇的替代侯选物。然而,因为葡萄糖是还原糖,因此能够启动制剂中不需要的降解,所以排除该张力调节剂。此外,由于针的阻塞,排除麦芽糖。这样产生以下的侯选物:甘油、木糖醇、山梨糖醇、蔗糖、甘氨酸、丙二醇和PEG400,对于点滴试验、针的阻塞和模拟装填,发现它们作为肽制剂中甘露醇的替代侯选物具有合适的特性。
然而,基于以下的考虑,相对于有待于在与甘露醇的逐一比较研究中进一步研究的其他侯选物选择丙二醇作为等渗剂:
a.观察到丙二醇对含有Arg34,Lys26(Nε-(γ-Glu(Nα-十六烷酰)))-GLP-1(7-37)的制剂的物理和化学稳定性没有影响;
b.观察到丙二醇对抗微生物防腐测试没有影响;以及
c.使用丙二醇不需要进行进一步的毒性研究试验
实施例2
在模拟装填研究和模拟使用研究中比较含有甘露醇和丙二醇的安
慰剂制剂
制剂的制备
将防腐剂和缓冲剂溶解于水中,并边搅拌边加入等渗剂。使用氢氧化钠和/或盐酸将pH调节至目标pH。最后,将制剂通过0.22μm的滤器过滤。制剂的组成如下:
磷酸氢二钠,二水合物:1.42mg/ml
苯酚: 5.5mg/ml
丙二醇或甘露醇: 13.7或35.9mg/ml

模拟装填研究
如实施例1中所述的进行持续24小时的模拟装填研究,24小时后,检查装填设备上沉积物的存在。对于丙二醇制剂,没有观察到装填设备上的沉积物。相比之下24小时后,对于甘露醇制剂,观察到装填设备上很多的沉积物(参见图6)。
模拟使用研究
对于模拟使用研究,如实施例1中所述的进行阻塞试验。在十个工作日的研究时段中使用相同的针,每天检查针上沉积物的存在。图7显示了用含有甘露醇(上图)或丙二醇(下图)的制剂给药的针的照片。当甘露醇用作等渗剂时,在48%的情况中观察到针上的沉积物,而当丙二醇用作等渗剂时,没有观察到沉积物。
实施例3

制剂的制备

模拟使用研究
对于模拟使用研究,如实施例1中所述的进行阻塞试验,不同的是使用G31针。在十个工作日的研究时段中使用相同的G31针,每天检查针上沉积物的存在。图7显示了当用含有丙二醇(下图)的制剂给药时,没有沉积物的针的照片,或当用含有甘露醇(下图)的制剂给药时,显示沉积物的针的照片。
对于含有甘露醇的制剂,在第4天观察到10个情况中有1个针阻塞,第5天10个情况中有2个,第8天10个情况中有3个,第9天10个情况中有4个。相比之下,对于含有丙二醇的制剂,没有观察到针的阻塞。
据认为如果将pH调节至7.40、7.70或7.90,也将会获得与用上述含有丙二醇的制剂所获得的那些相似的结果。此外,可能被测试的其他制剂包括具有以下组成的那些:
缓冲剂:甘氨酰甘氨酸(1.32mg/ml),L-组氨酸(1.55mg/ml),Hepes(2.38mg/ml),或二甘氨酸(1.63mg/ml)

Lys β29(Nε-十四烷酰)脱(B30)人胰岛素(2400nmol),苯酚(1.80mg/ml),间-甲酚(2.06mg/ml),甘露醇(30.0mg/ml),磷酸二钠,二水合物(0.890mg/ml),氯化钠(1.17mg/ml),醋酸锌(65.4μg/ml),注射用水(达1.0ml),pH:7.4。
如下制备含有NovoMix30的制剂:
a)将缓冲剂、氯化钠、苯酚、甘露醇和氢氧化钠溶解于水中制得溶液
b)制得氯化钠、苯酚和甘露醇的水溶液
c)制得鱼精蛋白硫酸盐的水溶液
d)制得胰岛素、盐酸和锌的水溶液
e)将溶液b)、c)和d)混合
f)将溶液e)加入步骤a)的溶液中
g)将溶液的pH调节至所需的pH,并在室温下结晶
h)将间-甲酚、苯酚和甘露醇溶解于水中制得溶液
i)将溶液h)加入步骤g)的晶体部分中;和
j)将pH调节至所需的pH
以上述方式制得的含有NovoMix 30的制剂的组成如下:
速效胰岛素制剂(100单位/ml),鱼精蛋白硫酸盐(约0.33mg/ml),苯酚(1.50mg/ml),间-甲酚(1.72mg/ml),甘露醇(30.0mg/ml),磷酸二钠二水合物(1.25mg/ml),氯化钠(0.58mg/ml),锌(19.6μg/ml),注射用水(达1.0ml),pH:7.3。
结果
如实施例3中所述的进行模拟使用研究,使用G31针,其中研究20个针10天。结果如下:观察Lys β29(Nε-十四烷酰)脱(B30)人胰岛素对针的阻塞:第2天(5%),第3天(70%)和第4天(100%)。观察NovoMix 30对针的阻塞:第3天(5%),第4天(10%),第5天(35%),第6天(40%),第8天(50%),第9天(55%)和第10天(80%)。因此,甘露醇对针阻塞的作用与制剂中所包括肽的类型无关,因为用Arg34,Lys26(Nε-(γ-Glu(Nα-十六烷酰)))-GLP-1(7-37),Lys β29(Nε-十四烷酰)脱(B30)人胰岛素和NovoMix 30观察到类似的阻塞作用。
实施例6
含有丙二醇的Lys β29(Nε-十四烷酰)脱(B30)人胰岛素和
NovoMix 30制剂的测试
Lys β29(Nε-十四烷酰)脱(B30)人胰岛素和NovoMix 30制剂的制备和组成如实施例5中所述,不同的是用一定浓度的确保张力的丙二醇替代甘露醇。然后如实施例5中所述的进行模拟使用试验。
基于含有甘露醇的Lys β29(Nε-十四烷酰)脱(B30)人胰岛素和NovoMix 30制剂的阻塞作用与用含有甘露醇的Arg34,Lys26(Nε-(γ-Glu(Nα-十六烷酰)))-GLP-1(7-37)制剂观察到的相似的事实,认为丙二醇对含有Lys β29(Nε-十四烷酰)脱(B30)人胰岛素和NovoMix 30制剂阻塞作用的效果与用含有Arg34,Lys26(Nε-(γ-Glu(Nα-十六烷酰)))-GLP-1(7-37)的制剂观察到的相似。
Claims (18)
1.包括Arg34,Lys26(N-ε-(γ-Glu(N-α-十六烷酰)))-GLP-1(7-37)和丙二醇的药物制剂,其中所述丙二醇以1mg/ml至100mg/ml的终浓度存在于所述制剂中,且其中所述制剂具有7.0至10.0的pH。
2.根据权利要求1的制剂,其中丙二醇的浓度为1mg/ml至50mg/ml。
3.根据权利要求1的制剂,其中丙二醇的浓度为5mg/ml至25mg/ml。
4.根据权利要求1的制剂,其中丙二醇的浓度为8mg/ml至16mg/ml。
5.根据权利要求1的制剂,其中所述制剂的pH为7.0至9.5。
6.根据权利要求1的制剂,其中所述制剂的pH为7.0至8.3。
7.根据权利要求1的制剂,其中所述制剂的pH为7.3至8.3。
8.根据权利要求1的制剂,进一步包括防腐剂。
9.根据权利要求8的制剂,其中所述防腐剂的存在浓度为0.1mg/ml至20mg/ml。
10.根据权利要求1的制剂,进一步包括缓冲剂。
11.根据权利要求10的制剂,其中所述缓冲剂选自甘氨酰甘氨酸,L-组氨酸,Hepes,N-二甘氨酸和磷酸二钠二水合物。
12.制备权利要求1-11任一项所述的药物制剂的方法,其中将Arg34,Lys26(N-ε-(γ-Glu(N-α-十六烷酰)))-GLP-1(7-37)、所述丙二醇与缓冲剂和防腐剂如下混合在一起来生产所述的制剂:
a)将防腐剂、丙二醇和缓冲剂溶解于水中制得第一种溶液;
b)将Arg34,Lys26(N-ε-(γ-Glu(N-α-十六烷酰)))-GLP-1(7-37)溶解于水中制得第二种溶液;
c)将第一种和第二种溶液混合;和
d)将c)中混合物的pH调节至pH7.0至10.0。
13.根据权利要求12的方法,其中丙二醇的浓度为1mg/ml至50mg/ml。
14.根据权利要求12的方法,其中丙二醇的浓度为5mg/ml至25mg/ml。
15.根据权利要求12的方法,其中丙二醇的浓度为8mg/ml至16mg/ml。
16.根据权利要求12的方法,其中所述制剂的pH为7.0至9.5。
17.根据权利要求12的方法,其中所述制剂的pH为7.0至8.0。
18.根据权利要求12的方法,其中所述制剂的pH为7.2至8.0。
