Description

技术领域
本发明总体上涉及用于进行内窥镜手术的系统和方法之领域,具体地,涉及对体腔内的组织进行内窥镜钉合的系统和方法。
背景技术
图1A示出了人胃S及相关特征的解剖学视图。食道E把食物从嘴输送到胃S的近端部分。Z-线或胃-食道接合处Z是在食道的薄组织和胃壁的较厚组织之间的形状不规则的边界。胃-食道接合区域G是包围食道E的远端部分、Z-线、以及胃S的近端部分的区域。
胃S包括在其近端的胃底F和在其远端的胃窦A。胃窦A供给到幽门P中,幽门P附接到十二指肠D,即小肠的近端区域。在幽门P内有括约肌,该括约肌防止食物从十二指肠D回流到胃中。小肠的中间区域,即位于十二指肠D远端的区域,是空肠J。

对于通过内窥镜将紧固件(例如缝合钉)施用到体腔内的组织而言,存在许多申请。其中的一些申请涉及在体腔的组织中形成诸如褶皱或折叠的组织结构。
一些现有申请,包括国际申请日为2004年10月8日的国际申请No.WO2005/037152和2006年5月23日提交的美国申请No.11/439461(这两个申请均通过引用并入本文),描述了这样的方法,即根据该方法,将医用植入物联接到形成于胃中的组织结构。根据这些申请,用于导致体重减轻(例如,通过限制和/或阻碍食物流入胃中,和/或通过占据胃容积的一部分)的装置可被联接到由胃组织形成的组织管道或褶皱。
例如,美国申请No.11/439461描述了一种用于引起体重减轻的限制性和/或阻碍性植入物系统。在一个实施例中,柔性环被联接到在胃的胃-食道接合区域中形成的组织褶皱。植入物(例如限流和/或阻流植入物)经过环2,从而保持在胃中。
在其它例子中,组织褶皱本身即可足以提供所需的处理。例如,如在WO2005/037152和2006年10月3日提交的、申请人的共同待决的申请No.11/542457(美国公开号为2007-0219571)中所公开的那样,褶皱可用于减小胃的容积或在胃中形成限流物,其中所述申请通过引用并入本文。
出于各种目的,可将其他类型的植入物联接到这样的褶皱或其它组织结构。这些植入物包括但不限于:用于治疗胃-食道回流疾病的人工瓣膜、胃刺激器、pH监视器、以及用于把药物、生物制剂或细胞释放到胃中或胃肠道中其他地方的药物洗脱装置。这些药物洗脱装置可包括释放莱普亭(一种产生饱腹感的激素)、胃饥素(一种产生饥饿感的激素)、奥曲肽(降低胃饥素水平从而减轻饥饿)、胰岛素、化疗剂、有助于术后创伤、溃疡、裂口等的天然生物制剂(例如,生长因子、细胞因子)的药物洗脱装置。另外一些植入物可以是如下类型的:其可提供特定细胞类型能附着、生长并向胃肠道提供生物活性基因产物的平台,和/或能出于治疗目的提供局部辐射源的辐射源所用平台,或者提供由其固定诊断配位体并将之用于对胃肠道采样以便获得特定的正常或病态状况的证据的平台,或者提供用于通过照相机和其它图像采集装置对胃肠道进行成像的锚固点。
以上所列的现有申请针对的是以如下方式形成组织褶皱、容腔或管道这一期望:浆膜组织(即,胃的外表面上的组织)的各区域被保持成相互接触。随着时间的推移,在相对的浆膜层之间形成的附着物会产生强键,该强键可有利于长时间保持褶皱/容腔/组织,尽管存在胃运动和植入装置施加给它们的力。
尽管存在形成褶皱的申请,但还是非常期望借助于采用通过食道下去的器具来从胃内执行的步骤形成该褶皱,而不是借助于更具侵袭性的手术或腹腔镜方法来形成该褶皱。本申请描述的内窥镜钉合器可经口进入胃中并可用于在胃壁中形成“浆膜到浆膜褶皱”。
发明内容
在一个方面,本发明包括一种用于向组织施用缝合钉的钉合器装置12。所述装置包括:(i)第一部件或者说缝合钉部件25,其具有第一部件壳体或者说缝合钉壳体28和能够相对于所述缝合钉壳体独立运动的缝合钉保持器78;(ii)第二部件或者说砧部件27,其具有第二部件壳体或者说砧壳体30和所述砧壳体30上所设置的砧96;以及(iii)驱动组件29,其包括驱动部件68和臂组件32,其中,所述驱动部件可操作地连接到所述缝合钉保持器78以便在所述缝合钉壳体28内从第一、收回位置运动到第二、伸出位置,所述臂组件可操作地联接到所述缝合钉部件和砧部件25、27,使得所述驱动部件68从其收回位置到其伸出位置的运动能够有效地(i)使所述缝合钉保持器78相对于所述缝合钉壳体28朝着所述砧96运动(A1),以及(ii)使所述砧部件27朝着所述缝合钉部件运动(A2)。

所述缝合钉保持器78和所述砧96可以具有相对的表面,所述相对的表面与所述臂组件32一起限定室21,并且所述驱动部件68从其第一位置到其第二位置的运动的效果是挤压被捕获在所述室内的组织,从而形成被压缩的组织褶。

所述臂伸展器113可以可枢转地连接到所述销和所述臂组件,使得所述销在其槽内从收回位置到伸出位置的运动能够有效地向外移动所述臂组件。因此,所述臂伸展器可在所述缝合钉主体和相关联的臂组件之间提供了桁架状支撑。

当所述驱动部件从其收回位置运动到其伸出位置时,所述驱动部件68能够有效地使所述缝合钉保持器78在其壳体28内前进;当所述驱动部件从其收回位置运动到其伸出位置时,所述臂组件32可有效地使所述砧96在其壳体内前进。

具体实施方式
本申请描述了内窥镜紧固件施用装置,在优选实施例中,所述装置可以经口进入胃中并可用于使胃组织形成褶皱。
在所公开的实施例中,组织被向内吸入到真空室中,但也可以利用不涉及使用真空的其它构件(例如,抓紧器)来向内抽吸组织。当胃内壁的一部分被向内抽吸时,胃外部的浆膜组织的各部分定位成彼此面对。所公开的紧固件施用装置允许相对的组织部分移动成彼此接触,并且输送将会把这些组织部分保持在一起的紧固件,直到至少浆膜键在它们之间形成之时。这些步骤中的每一个均可以完全从胃的内部执行,因此可以消除对任何手术或腹腔镜介入的需求。在一个或多个褶皱形成之后,可将医疗器械(包括但不限于任何以上所列类型)联接到褶皱以便保持在胃中。
所公开的实施例包括利用紧固件施用装置在褶皱中形成孔或切口的可选特征。该孔或切口可以形成为使得医用植入物的一部分可以通过或联结到所述孔/切口,或者可以形成为引起将会对所得到的组织键的强度做出贡献的愈合反应。
在下文给出的实施例描述中,紧固件施用装置被描述为钉合器,并且给出关于在胃组织中形成褶皱的示例性方法。然而,应该理解,本文所述实施例包括的特征对于施用其它类型的紧固件、以及出于形成褶皱之外的目的而施用缝合钉或其它紧固件而言具有均等的适用性。更具体地,术语“缝合钉”在本文中用于表示任何类型的紧固件,所述紧固件(i)能够被推动穿过组织,并且(ii)具有一个或多个支腿部件,所述支腿部件在被迫抵靠砧时会褶曲以便把紧固件固定到组织并把组织紧固的组织褶保持在一起。所公开的实施例和方法在胃肠系统之外的身体部分中也会有用。此外,虽然所公开的实施例以同心孔的圆形钉合和切割为特征,但可以想到能实现直线钉合以及无需切割的圆形或直线钉合的修改。
图2示出了用于组织钉合的系统或器具10的一个实施例,其适合于内窥镜用途,以及手术或腹腔镜用途,如果期望的话。
一般来说,系统12包括钉合器或钉合器器具12,钉合器或钉合器器具12具有定位在轴16远端部分上的钉合器头或装置14。轴16上的手柄18控制钉合器头14的关节运动以及钉合器头14的组织获取、组织压缩和钉合功能的致动。该系统中的真空源20和流体源31流体联接到手柄18以便在如下所述的组织获取、压缩和钉合中使用。真空源20可以是通过手术室的壁上的联接可通达的“室真空”或者是辅助抽吸泵。钉合器可包括开关21,开关21允许使用者控制真空源和钉合器之间的气流。
钉合器装置还用于捕获用于钉合的组织褶,因此在本文中也被称为用于固定组织褶(例如,用于紧固组织褶的各边)的组织捕获装置。组织捕获装置可以独立地工作以用于捕获组织(例如缺少单独的钉合机构),或者可以如图所示那样与钉合元件组合。
流体源31可以是单个的驱动流体(例如,水、盐水、油、气体)源或者是多个源,但是在每种情况下,该流体源优选地包括两个致动器,这两个致动器分别用于控制进入两个液压管路(一个用于组织压缩,一个用于钉合)中每一个的流。系统中的内窥镜22可通过轴16中的腔插入,从而允许看到褶皱形成过程。任选地,该系统可包括具有用于接收钉合器12的腔的外套管,例如内窥镜引导管23。
参照图3A,钉合器中的覆盖物或膜24围封钉合器机构,从而在钉合器头14内形成真空室21(图17-23)。暴露于待形成褶皱的组织的那侧保持不被膜24覆盖,从而允许在使用期间将组织吸入到所述室中。例如,膜24可包括如图3B所示的侧面开口26。膜24优选地由硅酮、弹性体材料、或者任何其它非弹性或弹性的柔性或可变形并且能够形成真空室21的生物相容材料(例如,打褶的麦拉膜)形成,真空室21的容积会扩展以容纳吸入到真空室21中的组织。在图3A-3C中还示出了关节连接部分128,关节连接部分128把所述器具轴连接到钉合器头,将在下文中参照图26和17对其进行描述。
所述膜的至少一部分是至少部分透明的。在至少部分透明时,所述膜由这样的材料形成或者包括这样的材料部分,即所述材料允许使用者透过所述膜足够清楚地看到以确认(经由内窥镜观测)在施用缝合钉之前已经把合适量的组织获取到钉合器头中。开口26可由加强部分27围绕,加强部分27由会加强开口26周围区域的材料形成。加强部分27可由所述膜材料的较厚部分和/或较高硬度的材料形成。可替代地,加强肋或其它结构或元件可以形成到所述膜材料中或上,或者嵌入在所述膜材料中。
钉合器头或装置
钉合器头14被设计成在插入褶皱位置期间具有最小的形貌(profile),然后变换成具有大的内部容积的较大形貌装置。例如,在一个实施例中,真空室可具有0.2立方英寸的初始内部容积,以及0.6立方英寸的扩展后容积(即,在减去了定位于真空室内的钉合器头构件所占据的容积之后的真空室内部容积)。该大的内部容积允许大量的组织被吸入真空室中并被钉合。这样,钉合器头实现了大的褶皱而无需侵袭性插入技术。钉合器头的独有特征允许钉合器头利用最小的运动和力输入就能进行原位容积扩展。特别地,如将在下文中参照图32A-32C和图33所见到的,褶皱的尺寸可以设计成使得施用到组织的缝合钉(例如在图33中所见的两个环形缝合钉阵列)与被钉合组织的边缘很好地隔开,从而最小化缝合钉周围的组织撕裂的风险。
钉合器头的特征示于图4-10。为了清楚起见,在这些图中没有示出膜。参照图4,钉合器头14通常包括:第一、缝合钉部件25,其包括近端缝合钉壳体28;第二、砧部件27,其包括远端砧壳体30;以及至少一个细长部件但优选地是一对铰接臂组件32,其如下所述那样可操作地连接所述两个壳体。
缝合钉壳体和砧壳体布置成允许组织在位于缝合钉壳体和砧壳体各自上的接触表面之间受压缩。在所公开的实施例中,接触表面位于缝合钉壳体的缝合钉保持部分上,即缝合钉保持器的外表面,以及砧壳体上的砧。只考虑所述装置的组织捕获操作,缝合钉保持器78(示于图7)充当具有组织接触前表面83的组织捕获板,而砧96(示于图13和14)充当第二组织捕获板,该第二组织捕获板具有面对表面83的组织接触表面103,其中,这些表面用于在装置工作期间捕获组织褶,这将参照图32A-32C进行更详细地描述。
臂组件32在位于钉合器头14相对两侧的缝合钉壳体28和砧壳体30之间延伸。近端销34和远端销36把每个臂组件32可枢转地联接到缝合钉壳体28和砧壳体30。包括升膜器37的扩展部件也在缝合钉壳体28和砧壳体30之间延伸。虽然在图4中没有示出膜24,但应该理解,升膜器37定位成与该膜中的开口26(图3B)相对。在所示实施例中,升膜器37包括:联结件38,其通过销42可枢转地安装到缝合钉壳体;相应的联结件40,其通过销44可枢转地安装到砧壳体;以及弹簧丝46,其把联结件38和40联接到彼此。
缝合钉壳体
现在转向对钉合器头构件的更详细论述,在图5和6中可以看到缝合钉壳体28已与其它构件分开。如图5所示,缝合钉壳体的近端面48包括输入端口50a、50b,引导流体通过输入端口50a、50b以便对钉合器头的组织压缩、钉合及任选的切割操作进行液压致动。密封件51环绕端口50a、50b以便最小化流体泄漏。
真空端口52流体联接于真空源20(图2),真空源20被选择性地激活以在真空室中产生负压,以便进行组织获取。真空端口52通过钉合器轴16(图2)中的柔性管(未示出)连接到真空源20。安装孔54用于把钉合器头14通过关节连接部分128安装到轴16。
缝合钉壳体28包括在开口侧部56上方和下方的上部58a和下部58b。上部58a包括凹进60,在凹进60内安装有用于联结件38(图4)的枢销42。如在图6中最佳示出的,孔62设置于上部58a和下部58b中,用于接收销34(图4),销34充当臂组件32的近端枢转点。导槽64纵向延伸通过上部58a和下部58b。
参照图6,液压室66设置在缝合钉壳体28内。在液压室66(图6)内有用于驱动钉合器的组织压缩和钉合功能的专用液压回路。液压室66流体联接于流体输入端口50a、50b(图5)。如将结合图11A-11D详细论述的,经由输入端口50a、50b而被驱动进入液压室66中的流体相继地推动液压活塞系统(未示出),所述液压活塞作用于其它构件以便压缩组织并且驱动缝合钉和切割元件通过被压缩的组织。
图7示出了被液压系统驱动以便进行压缩、钉合和切割的钉合器头构件。为了清楚起见,这些构件被示为与缝合钉壳体分离并且彼此分离。在此论述中,将描述被液压系统驱动的构件。液压系统本身则在稍后的部分中结合图11A-11D进行描述。
特别地,图7示出了缝合钉壳体中采用盘68形式的驱动部件。在组装好的壳体中,盘68定位成将会被液压压缩活塞(图11A-11E中的活塞106)向远端推动。如将在图11A-11E中看到的,驱动部件可在图11A所示的第一、收回位置和图11C及11D所示的第二、伸出位置之间运动。虽然这里所示的驱动部件被活塞106驱动且与活塞106分开,不过将会认识到,这两个构件可由单件式部件形成,即作为包括活塞和盘这两者的单件式驱动部件。如将会在下文中看到的,驱动部件联接到臂组件32、砧壳体、和缝合钉壳体,使得向远端推动驱动部件(朝其伸出位置)会通过把缝合钉壳体和砧壳体的接触表面相对朝彼此带动而实现组织压缩。盘68、它的驱动活塞106、以及联接所述两个壳体的臂组件32的组合在本文中也统称为驱动组件,在图10中以29标出。驱动组件还可以包括砧部件中的驱动联结件114,如下文所述,驱动联结件114可操作地联结于组件32。
如在图7中最佳所见的,盘68包括安装孔70、中央开口72、以及对准柱74。暂时参照图10,在组装好的钉合器头中,盘68联接到缝合钉壳体28,并且它在该壳体中的轴向运动被销84限制,销84延伸通过该壳体的导槽64和盘68中的安装孔70。
缝合钉壳体28的一部分容纳(即被装载以容纳)有待发射到组织中的缝合钉。缝合钉被容纳在缝合钉壳体上的缝合钉保持器(例如,缝合钉储筒78)中。缝合钉保持器可具有许多不同的构造。例如,它可以是缝合钉壳体的一体化部分,或者是安装或附接到缝合钉壳体的独立零件,和/或它可以能够相对于缝合钉壳体的主体运动,从而在钉合之前实现组织压缩。在任何这些示例中,缝合钉保持器均可以是可移除/可更换的储筒,和/或可以通过向该保持器中插入另外的缝合钉而对其进行再填充。在其它实施例中,缝合钉保持器可以既不是可更换的也不是可再填充的,即,拟用于一次性使用。
在所公开的实施例中,缝合钉保持器是可移除的缝合钉储筒78,缝合钉储筒78可用另一个填充了缝合钉后的储筒来替换。在这个实施例中,缝合钉储筒可相对于缝合钉壳体的主体运动以便在缝合钉发射之前压缩组织。
再次参照图7,缝合钉储筒78可定位在缝合钉壳体内、在盘68的远侧,使得压缩活塞对所述盘进行的向远端的推动会把储筒从第一、收回位置向远端推到第二、伸出位置,从而压缩布置在储筒和砧之间的组织。在储筒插入到钉合器头中的过程中,储筒外部上的凹槽79在相应的对准柱74上滑动。图10示出了在把储筒装载到缝合钉壳体中之前的对准柱。如图所示,对准柱74可具有锥形端部,以便于把储筒装载到对准柱上。将会认识到,在钉合器工作期间,对准柱保持住储筒以防其在壳体28中发生角运动。
再次参照图7,储筒78包括众多缝合钉位置80,每一位置收纳一缝合钉,例如图33中所见的缝合钉158。缝合钉储筒设有凸起81,用于保持如图8所示类型的缝合钉线加强装置83,该加强装置在2006年10月3日提交的、标题为“ENDOSCOPICPLICATIONDEVICESANDMETHODS”、于2007年9月20日公布为US20070219571的、共同拥有的美国申请No.11/542457中被详细地公开。做一简短的总结,这种类型的加强装置83可以是能够抵靠缝合钉储筒的远端面定位的环或其它元件。当该环被置于储筒上时,该环中的开口85与储筒中的一些缝合钉的凸部对准。当从储筒驱动缝合钉时,这些凸部穿过相关联的开口85并且抵靠着相邻的身体组织捕获环83。
参照图7和9,位于储筒的面向砧的那侧上的众多底切凸起81可用于把加强装置83锁定就位在缝合钉储筒的面上。其它正形状(例如蘑菇状、钩状、和倾斜凸起)可用于实现相同目的。形成于储筒表面中的负形状(例如,容腔或凹槽)也可用于接合加强装置83上的相应特征部。作为另一种替代方案,加强装置可利用粘合剂在储筒上保持就位。
在所示实施例中,切割器元件86延伸通过盘68的中央开口72(图7)。切割器元件被示出为具有锋利壁和腔87的管状冲,但可以以替代形式来提供。如可在图10的组装图中看到的,缝合钉推动器76在盘的远侧安装到切割器元件。缝合钉推动器76包括推动器元件82,当缝合钉推动器76被推入缝合钉储筒78中时,推动器元件82配合滑入储筒的缝合钉位置80,从而把缝合钉从储筒驱出。液压室66(被设置在由活塞106形成的液压室内)中的液压驱动缝合钉活塞(在图11A-11E中以116示出)联接到切割器元件86,使得缝合钉活塞的前进会使缝合钉推动器76和切割器元件86沿远端方向前进。
流体驱动系统
用于致动压缩、钉合和切割的流体驱动系统可以多种方式构造。以下段落描述了流体驱动系统的一种示例性构造,在这个实施例中,流体驱动系统是液压系统。图11A和11B示意地示出了在致动的压缩阶段和钉合阶段期间缝合钉壳体28的液压室66中的流体流。参照图11A,压缩活塞106布置于液压室66内。盘68(也示于图7和10)定位成与活塞106接触或者在活塞106的稍远侧。压缩活塞106为大致杯形,具有围封内部111的侧壁110和后壁108。O形环密封件112在侧壁110的近端部分上间隔开。通道115在侧壁110中形成,位于O形环密封件112之间。
第二活塞(称为缝合钉活塞116)位于压缩活塞106的内部111中,抵靠着后壁108。虽然在图11A-11D中未示出,但其上带有缝合钉推动器76的切割元件86(图7)定位成与缝合钉活塞116接触或者在缝合钉活塞116的稍远侧。O形环密封件118环绕缝合钉活塞116的在压缩活塞的通道115远侧的那部分。
第一流体通道120从缝合钉壳体28中的流体端口50a延伸至液压室66的近端部分。第二流体通道122从缝合钉壳体中的流体端口50b延伸至液压室66的更远端部分。在图11A中示出了来自端口50a和流体通道120的流体流抵靠压缩活塞缸。液压室66内的流体压力推动压缩活塞106(其中有缝合钉活塞116)沿远端方向从图11A所示的第一、收回位置到图11C和11D所示的第二、伸出位置。图11B示出接近其行程末端(即,完全伸出位置)的压缩活塞106。一旦压缩活塞到达其如图11C所示的行程末端,压缩活塞106中的通道115就与壳体中的通道122对准,从而允许通过流体端口50b引入的流体经由通道122进入压缩活塞106的内部。进入压缩活塞内部的流体如图11D所示那样向远端驱动缝合钉活塞从图11A-11C所示的第一、收回位置到图11D所示的第二、伸出位置。在图11E所示的替代性实施例中,提供了提三活塞117,用于单独驱动切割元件86。在该实施例中,引入第三驱动流体端口50c中的流体使第三活塞117从第一、收回位置前进到第二伸出位置(未示出)。活塞106、116和117及相关联的流体路径可以布置成使得流体不能进入缝合钉活塞内部以推动切割活塞117,直到压缩活塞106已经运动到组织压缩位置并且缝合钉活塞116已经进而运动到钉合位置。
接下来将参照图12描述砧部件27中的砧壳体(在图4中以附图标记30表示)。砧壳体30包括安装孔88,用于接收位于铰接臂组件32远端的枢销36。砧壳体30的上部包括部分94,用于联结件40(图4)的枢销44安装通过该部分94。
中央孔90纵向延伸通过砧壳体30。砧支撑件92(图13)可在所述孔内纵向滑动。孔90和砧支撑件92都优选地形成为具有非圆形的截面(例如所示的矩形截面),带有平的支承表面用以防止活塞在所述孔内旋转。
图13示出了与砧壳体30分离开的砧支撑件92。砧支撑件92的远端部分被分成上板95a和下板95b。板95a具有孔93,孔93与板95b中的类似孔轴向对准。砧支撑件92的近端部分承载砧96。如图14所示,砧96包括多个凹痕98,凹痕98定位成使得当从缝合钉储筒驱动缝合钉时,每个缝合钉支腿与一个凹痕接合,这导致缝合钉支腿弯折或褶曲。在所示实施例中,所述砧被设计用于具有两个由错开缝合钉构成的环形圈的缝合钉阵列,其中每个圈有五个缝合钉。中央开口97延伸通过砧96并且与砧支撑件92中的腔毗邻。
砧96和缝合钉储筒78(图7)是钉合器头的向待钉合组织施力的两个部分。如图9和14所示,优选的砧和储筒被设计成使用环绕砧96的凹痕98和储筒78的缝合钉位置80的极少量的材料,使得接触组织的砧/储筒表面积的量尽可能的小。当受到恒定力时,较小的印迹将会比较大的印迹损伤更少的组织,因为较小的组织区域被挤压在砧和储筒之间。然而,确实被挤压的组织会经历更多来自给定力的压强,因为所述力分布在较小的面积上。换句话说,最小化的印迹会以较小的力在组织上造成较大的压强。从力学角度来说,这是有利的,因为所述钉合器头不需要供应或经受像具有较大印迹的储筒和砧所需力那么大的力。
参照图7,在所示实施例中,缝合钉储筒78具有追随其中所收纳的缝合钉的轮廓的外壁,从而形成多个环绕外缝合钉位置或槽80a(与内缝合钉位置80b相邻)的瓣73,瓣之间设有凹槽79。不是把每个缝合钉位置设置成被储筒材料完全环绕,而是缝合钉位置80a、80b优选地各自包括后壁71a和附接于所述壁的保持元件,所述保持元件定位成把缝合钉保持在所述保持元件和后壁之间。在图7中,保持元件包括一对翼71b,这对翼从后壁71向内弯曲,从而限定出被充分约束以便把缝合钉保持在缝合钉位置的槽,但是优选地不围绕所述槽的整个周边对其进行约束。砧具有类似的瓣式布置,如图13所示。
再次参照图13,板99定位在砧96上,使得在组织切割过程中,向远端前进的切割元件86将会前进至与板99接触。在一个实施例中,板99可安置在砧中的开口97内。板99(也将被称为“切割板”)中具有孔101,孔101缓解被捕获组织的压力并防止液压锁定(所述冲和板形成封闭容积的情形)。如果期望在接触发生之后移动切割元件86,则该封闭容积内的压力会增大并且将会抵抗进一步的运动。这会阻止或负面影响组织切割。
切割板优选地被设计成不充当针对切割元件86的前进的硬止挡件。如果切割元件86被切割板停止,则缝合钉活塞也将会被停止,从而可导致未完成的缝合钉形成。因此,优选的是,在切割组织期间或之后允许切割元件86刺入或移动切割板;一旦与板发生了初始接触,切割器就能够稍微前进。
图15A和15B示出了前进至与切割板的不同实施例接触时的切割元件86。在图15A的实施例中,切割板99a的材料是相对较软的材料,例如弹性体硅酮,其被前进的切割元件切割或刺入,如图所示。在缝合钉形成的最终阶段,该材料允许切割元件的锋利远端运动进入切割板。在图15B的实施例中,切割板99b可由较硬的材料制成,在其后面置有可压缩的物体,例如弹性体弹簧99c。在该图中,该弹簧为O形圈。切割元件86抵着切割板99b的前进使得切割板抵着弹簧99c向远端移动。随着O形圈被压缩,前进的切割元件86会受到越来越大的阻力。其它的弹簧形状和材料,例如盘丝、弹簧垫圈和板簧,可用于实现相同的结果。当切割元件86被迫与切割板接触时,切割板99b表面上的斜面99d可帮助对准切割元件86。
臂组件
以下是对臂组件32的特征的论述。图16示出了与钉合器头的其它元件分离开的组件32。一般来说,每个臂组件具有:第一臂部分100,其可枢转地联接到缝合钉壳体;和第二臂部分102,其可枢转地联接在第一臂部分和砧壳体之间。虽然在所示实施例中不存在,但是可在第一臂部分和第二臂部分之间设置另外的臂部分。
也就是说,每个臂组件包括近端臂100和远端臂102,它们彼此连接以形成铰链104。每个近端臂100具有纵向切口108和可枢转地安装在切口108内的臂伸展器113。每个臂伸展器113的远端包括孔112。销84置于孔112内。如结合图10所公开的,销84延伸通过盘68并具有在位于缝合钉壳体的下部和上部上的槽64(图6)内移动的端部。盘68在缝合钉壳体内的纵向运动将会使销84在其相应的槽64内前进,从而使臂伸展器113相对于销84枢转,并且由此向外驱动臂组件32。在标题为“钉合器头的操作”的部分中阐述了关于臂组件32的运动的更多具体情况。
臂组件的远端臂102包括销36,如所论述过的,销36可枢转地安装到砧壳体30(图4)。提供了一对驱动联结件114,它们各自具有可枢转地附接于相应的一个远端臂102的第一端、以及可枢转地联接到共用销116的第二端。在组装好的钉合器头中,销116位于砧支撑件的上板95a和下板95b(参见图12中的板95a、95b)的孔93内。如在下文的“钉合器头的操作”这一部分中详细阐述的,当臂伸展器113向外驱动臂组件32时,驱动联结件114作用于销116以便沿近端方向推动砧支撑件,从而导致砧朝着缝合钉储筒向近端前进。
可替代地,可由装在砧壳体中的直接液压驱动机构来以独立于臂组件的方式在砧壳体内驱动砧。优选地,协调位于所述两个壳体中的驱动机构,使得缝合钉保持器和砧在其各自的壳体内同时朝彼此运动。
钉合器头的操作
以下论述集中于臂组件以何工作方式来扩展真空室并对已利用抽吸被吸入真空室内的组织进行压缩。在真空室扩展之前的初始步骤中,把钉合器头定位成膜24中的开口26与处于期望形成褶皱的位置处的组织接触。激活真空源20(图2)以向由所述膜限定的真空室内部施加真空。与开口26(图3B)接触的组织将会被吸入到缝合钉壳体28和砧壳体30之间的真空室内。在组织被吸入之后,改变钉合器的形貌,扩展所述膜内的真空室容积。
在图4、17和18中示出了扩展之前的钉合器头28的流线型位置。特别地,铰接臂组件32和升膜器37处于大致直的取向。近端臂100充当用于真空室扩展和组织压缩的驱动臂。当处于压力下的水被迫使进入缝合钉壳体的液压回路中时,启动这些臂的运动。参照图19,流体压力推动盘68(通过压缩活塞106的作用,该活塞在图19中未示出)。如图19-21所示,盘68转而把缝合钉储筒78推向砧96,从而使缝合钉储筒78进一步从缝合钉壳体28中伸出。
盘68和臂伸展器113均联接到销84。由于这个原因,盘68在缝合钉壳体28内的纵向运动将会把销84在其相应的槽64内带向远端。结果,臂伸展器113将相对于销84枢转,从而向外驱动近端臂100。近端臂100在铰链104处的向外运动会使远端臂102也在铰链104处向外枢转,从而在近端臂100和远端臂102之间形成角度。自然,在臂100和臂102之间形成角度会缩短所述两臂远端之间的有效距离,从而导致远端臂102的远端销36把砧壳体30带向缝合钉储筒。远端臂102的枢转运动还使驱动联结件114作用于销116,从而沿近端方向推动砧支撑件。这使砧支撑件相对于砧壳体沿近端方向运动,同时砧壳体也向近端运动。
其实,液压驱动压缩活塞的一个运动会引起至少三个运动,如图19-21中的箭头A1、A2和A3所示。这三个运动包括:缝合钉储筒78相对于缝合钉壳体沿着朝向砧96的方向运动(箭头A1);砧壳体30朝着缝合钉壳体28运动(箭头A2);以及,砧96本身相对于砧壳体30沿着朝向储筒的方向运动(箭头A3)。这种砧朝着缝合钉储筒的复合运动使得压缩活塞发生小位移,从而快速地压缩被钉合器抓住的组织。运动的增加也通过把铰链104处近端(从动)臂和远端(驱动)臂间的角度保持得尽可能大来增强所述两个壳体之间的力传递。
所述两个壳体28、30朝向彼此的相对运动还会向上驱动联结件38、40以及它们位于钉合器头14顶部的互连弹簧丝46。所述联结件和弹簧丝一起使膜的顶部升高,从而形成更大的容积来适应压缩期间组织的膨胀。
如图22-24所示,当在缝合钉壳体28的槽64内移动的销84到达行程极限时,停止组织的压缩。这样,设计所述槽和相关构件的尺寸以便设定在钉合器头的钉合器侧和砧侧的组织接触表面之间的期望分隔距离。用于胃壁褶皱的示例性分隔距离可包括大约0.06-0.07英寸(例如,用于和支腿长度为5.5mm的缝合钉一起使用)或者0.109英寸(用于支腿长度为6.5mm的缝合钉)。向液压回路施加更多压力不会再进一步压缩组织。
此外,由于活塞布置,钉合功能被有效地锁定,直到完成组织压缩。在采用这种布置的情况下,在完成组织压缩之前经由流体端口50b(图11A)引入缝合钉流体通道122中的流体会泄露,直到压缩活塞106的两个O形圈112跨置在入口114上。这种设计防止了过早的缝合钉发射。
在完全压缩位置时,臂伸展器113几乎垂直于钉合器头的纵向中心线。一旦组织被压缩在储筒78和砧96之间,该组织就准备好进行钉合。
通过经由端口50b(图5)引入液压流体来起动钉合。缝合钉活塞前进,把切割元件86(图7和10)推向砧96。由于缝合钉推动器76安装到切割器86,因此这个动作会带动缝合钉推动器76通过储筒78,在储筒78中它同时推动所有的缝合钉通过组织。缝合钉活塞的行程由内部止挡件限制,并且被预设以实现最佳的缝合钉形成。
在压缩过程中,当臂组件32在铰链104处的角度达到其最小值时,抵抗缝合钉壳体和砧壳体分离所需的力增大。当缝合钉活塞在砧上施加缝合钉挤压力时,这些力会进一步增大。为了补偿,臂伸展器113充当位移支柱以将至少一部分这些力导入盘68中。如果未被推动器盘反作用的话,这些力会把臂100、102推入并且可能松开对组织的压缩,从而导致无法完成缝合钉形成或组织切割。这样,形成了用于力位移的桁架状结构。
当缝合钉已经形成时,释放缝合钉压力并且弹簧(未示出)使缝合钉推动器72返回其基础位置。释放流体压力将会允许升膜器37上偏转的弹簧丝46使钉合器头返回其最小形貌结构并且从钉合器释放褶皱。一旦到患者的体外,就可以排出用过的缝合钉储筒并安装新的储筒。
图25A-25C示出了一种用于把可移除的缝合钉储筒78保持在缝合钉壳体内的方法。储筒被弹簧装载到缝合钉壳体中并被两个锁闩170(可看到一个)保持住,每个锁闩均可相对于支点172枢转。如图所示,支点172可通过销84联接到盘68。每个锁闩170均包括钩174,钩174接合储筒上的相应钩176。优选地,锁闩170被弹簧偏置以朝着储筒向内推动钩174。
如图25B中的箭头P所示那样按压每个锁闩170的近端175会使该锁闩抵抗着所述偏置发生枢转,从而导致缝合钉储筒被排出。然后可以放置新的缝合钉储筒,并如图25C所示那样,让新储筒的凹槽79与对准柱74对准,随后把新储筒推向缝合钉壳体。当新储筒滑动到位时,钩174在钩176的锥形近端部分178上移动。一旦钩174越过钩176的远端180,钩174就由于其弹簧偏置而朝着储筒向内落下,从而接合储筒。当储筒被正确地安置好时,由于锁闩接合新储筒,将会感觉到或听到咔嚓声。
在钉合期间的钉合器对准
在操作中,当所述两个钉合器部件被带至一起时,组织褶被捕获在缝合钉储筒的外表面和砧的相对表面之间。随着该组织捕获的发生,恰在组织褶钉合之前及期间,被捕获组织的区域的厚度和/或可压缩性的变化可能使钉合操作偏离中心,即,偏离轴线,从而导致缝合钉支腿未在适当的位置被接收到它们相关联的砧凹痕中,结果是一个或多个缝合钉可能紧固得有缺陷,例如,没有完成缝合钉支腿的弯折,和/或缝合钉阵列可能相对于装置的中心轴线偏移,从而导致一些缝合钉例如离已钉合组织褶的中心中切出的孔太近。
为了确保针对储筒中的每个缝合钉阵列(例如,两个各具有五个缝合钉的同心阵列)都能成功地进行钉合操作,本发明的装置可包括对准结构,用于恰在钉合操作之前及期间,保持该装置的近端部件和远端部件轴向对准。在图36和37中以侧剖视图示出了所述对准结构。图36中所示的是缝合钉装置12的包括远端部件27上的砧96和切割板99a以及近端部件25上的切割器86的那部分。在操作中,首先使所述两个部件以及其上带有的缝合钉储筒和砧移动到一起,以便在储筒和砧的相对两表面之间捕获组织褶,如图22和23所示。然后,让缝合钉活塞116从其收回位置运动到其伸出位置(如图24中所见),以便使切割器迎着切割板99a运动(如图所示),并且迎着砧驱动缝合钉储筒中的缝合钉。
对准结构包括:销,该销被轴向保持在切割器86中以随其运动;以及形成在衬套164中的销接收槽,其中衬套164被支撑在壳体27的一部分中,壳体27附接到砧96以随其轴向运动。在图36所示的实施例中,通过弹簧166沿着抵抗所述销朝着衬套运动的方向把衬套164弹簧装载到壳体27中,从而允许切割器和所附接的销(现在已经刺入穿过被捕获的组织)安置在衬套的坡口端中。当切割器、销和缝合钉推动器继续朝第二部件运动,最终在组织中形成孔并且穿过组织并迎着砧从缝合钉储筒中射出缝合钉时,将所述两个装置部件保持成轴向对准,使得钉合操作在所有缝合钉定位成与相关砧凹痕对准的情况下发生。当这个操作完成时,收回缝合钉活塞和驱动部件以释放钉合后的组织并使所述装置返回其线形状态。
图37中所示的装置12的实施例与刚才描述的相似,但是在该实施例中,弹簧174使切割器的壳体中所带的对准销168沿着第二部件的方向发生弹簧偏置,并且销接收槽170位于固定位置衬套172中,固定位置衬套172在部件27中被支承在砧(未示出)下方。在这个实施例中,一旦开始时已将销安置在衬套槽中,那么当如上所述那样缝合钉活塞继续朝其完全伸出位置运动以便切割和钉合被捕获在所述两个部件之间的组织时,所述两个部件朝彼此的运动就被所述销沿远离第二部件方向的运动所适应。
钉合器轴和手柄
再次参照图2,连接手柄18和钉合器头14的钉合器轴16足够柔韧,以顺应上消化道的曲度,却保持传递足够的扭矩来旋转钉合器头的能力。所述轴形成为具有足够的刚度,从而允许把它沿着食道引导管23推下去。合适的材料包括
图26示出了轴16的远端部分,钉合器头从该轴移除了。如图所示,轴16包括内窥镜腔124,通过内窥镜腔124使内窥镜前进以便允许看到钉合操作。还可提供侧腔126,用于接收在手术过程中有用的其它器具。
关节连接部分128位于轴16的远端,在轴16和钉合器头14之间,以便允许钉合器头相对于轴进行关节运动。联接到真空源和液压流体源的管道从手柄延伸并穿过轴16和关节连接部分128。
图27A示出了可用于液压流体管路130的一种构造。在使用期间,液压流体管路在钉合器的关节连接部分遭受明显的偏转和拉长。它们有时也会遭受可能超过1500psi的流体压强。通常,工业应用中的液压管路是柔韧的并且具有额外管道的工作环路,其在使用过程中适应长度变化。所示的液压管路构造是一种较小形貌方案,其尤其适合于具有空间限制的内窥镜装置。优选的液压管路是这样的管130,即该管的一部分被形状设计成纵向可扩展的形状,使得它在弯曲过程中可以适应有效长度的变化。该管纵向可扩展的部分优选地设置在钉合器12的关节连接部分128中。在优选设计中,纵向可扩展形状是图27A所示的线圈形状。在替代实施例中,管130可以形成为其他纵向可扩展形状,例如规则或不规则的波状(图27B)。
管130的优选材料是不锈钢海波管(hypotube),但也可改用其他材料。在优选的钉合器构造中,提供了两个驱动流体管路,一个用于致动组织压缩,另一个用于施用缝合钉(以及在被使用时用于切割)。在本实施例中,所述管如图27A所示盘绕在一起。在替代实施例中,可将两个或更多个盘绕管一个放置在另一个中。当关节连接部分弯曲时,它迫使盘绕管130弯曲并响应于该弯曲而改变长度。在这些运动中,盘绕管表现得就像盘丝一样,因此能够改变长度、偏转以及追随关节连接部分的轮廓,而不会伤及通过管腔的流或者向位于液压系统任一端的连接处施加过度的应力。
流体管路的纵向可扩展形状可适合用来实现向其他类型的关节连接医疗器械(例如用于输送治疗剂或冲洗流体经过装置的关节连接部分或可弯曲部分的内窥镜装置或导液管)的操作端输送流体。
再次参照图26,关节连接部分128由脊柱构成,该脊柱由多个在一对拉缆134(在图26中只示出了一个)上延伸的联结件132形成。在一个实施例中,拉缆的接合会允许钉合器头14在两个方向上进行关节运动,运动范围大约是从一个方向上(参见图3B)的90度到相反方向上的175度(参见图3C)。每根拉缆被锚固在钉合器头处或其附近,例如在缝合钉壳体28的最远端联结件132处。
拉缆134的较近端部分延伸轴16的长度并终止于手柄18中。参照图28,手柄18包括旋钮136,旋钮136可以选择性地沿顺时针方向或逆时针方向旋转,以使钉合器头向上或向下进行关节运动。沿一个方向的旋转会向其中一根拉缆施加张力,从而使钉合器头向下弯曲,而沿相反方向的旋转会在其它拉缆上施加张力,从而导致钉合器头向上弯曲。
在优选的手柄构造中,旋钮136包括内螺纹孔138。旋钮136被部分限制在手柄18内,因此它保持固定在手柄中但能够自由旋转。具有螺纹外表面的承载架140被置于旋钮的螺纹孔128内。孔138内的螺纹与承载架140上的螺纹接合,使得旋钮的旋转会让承载架140在手柄内发生平移而不旋转。
所述两根拉缆中的每一根(在图28中标示为缆线134a和134b)终止在手柄中的不同部件上。缆线134a安装在滑动承载架上,而缆线134b安装到手柄18的固定部分。每根缆线均延伸通过相应的护套。缆线134a延伸通过护套135a,护套135a的近端固定到手柄18的固定部分。缆线134b延伸通过护套135b,护套135b的近端安装到滑动承载架。
缆线134a和134b以及护套135a和135b布置成使得承载架沿一个方向的平移将会导致钉合器头沿一个方向的偏转,而承载架沿另一个方向的平移将会使钉合器头沿另一个方向偏转。
参照图28,如果旋转旋钮136以使承载架140平移到页面的左边,则缆线134a将会被张紧,而缆线134b将会被放松,从而导致钉合器头沿第一方向(例如,向上)进行关节运动。旋钮136沿相反方向的旋转将会使承载架移动到页面的右边,于是释放了缆线134a上的张力并且推动缆线134b上的护套135b朝向钉合器头的远端,从而导致当护套135b迎着轴16的远端部分前进时发生沿第二方向(例如,向下)的关节运动。护套135b的近端部分设有足够的工作长度,防止其在承载架向远端运动时被置于张力之下。旋钮定位的有利之处在于,使钉合器发生关节运动所需的手的运动始终是相同的,而无论钉合器的旋转取向如何。而且,使用螺纹旋钮可防止无意地松开偏转角,即使提供的旋钮没有用于保持其旋转位置的锁也是如此。
参照图28和29,内窥镜腔124沿着钉合器的中心轴线延伸。该腔的定位关节运动旋钮相对于内窥镜124的同轴关系允许内窥镜和钉合器互补干涉地独立旋转。因此,如果使用者选择改变钉合器头14在身体内的旋转取向,那么他/她可旋转手柄18和轴16,同时保持内窥镜的旋转位置。
为了节约成本,钉合器12可被设计成允许丢弃钉合器头14,而允许消毒和再使用轴16和手柄18。图中示出了一种用于把钉合器头可移除地联接到轴16的机构,但可以容易地想到其它机构(例如,滑动联接型布置)。参照图26,端板142安装到最远端的一个联结件132。端板142和钉合器头的相应后表面中的每个均设有允许端板和钉合器头彼此接合的锁闩特征部。
端板142包括具有栓145(其可以是弹簧销)的悬臂销144、中央开口146、以及一对沿其边缘的U形钩或接合边缘148。液压供给孔或开口156a、156b形成穿过端板142。向钉合器头输送液压流体的液压管(参见图27中的管130)优选地被焊接到端板,从而允许把来自所述管的流体引导通过供给孔156a、156b。
图30A和30B示出了缝合钉壳体的后表面48a,其与图5相比已进行了一些修改。在后表面48a的这个变型中,液压输入端口或开口50a、50b被如图所示那样重新定位。此外,后表面48a已被修改成包括一对呈底切凸起150形式的钩或接合边缘,再加上对准销152和孔154。轴与钉合器装置的卡合式连接
图30A和30B示出了定位成抵靠缝合钉壳体的后表面48a的端板142。为了清楚起见,在图30A和30B中没有示出关节连接部分128的其它特征。为了把钉合器头附接到轴16,如图30A所示那样,把附接于手柄组件的端板142按压在缝合钉壳体的后表面48a上。当推动该板时,它沿顺时针方向旋转,从而导致悬臂销144的栓145(图26)与缝合钉壳体的后表面中的孔154接合。当该锁闩接合时,如图30A所示那样,端板142的液压供给孔156a、156b与钉合器头上的液压入口50a、50b对齐。同时,端板的环绕U形钩148的那些部分在底切凸起152下方滑动。按压端板会压缩环绕液压输入端口50a、50b的表面密封O形圈。通过钩与悬于端板之上的底切凸起的接合来保持对O形圈的压缩。为了从壳体中移除钉合器头,沿逆时针方向旋拧缝合钉壳体以使端板142与后表面48a脱离。然后可以对钉合器轴和手柄消毒,以便准备安装新的钉合器头。
示例性过程
接下来将在于胃壁组织中形成褶皱的背景下,特别参照图31-33,来描述系统10的使用方法的一个示例。
在初始步骤(图2)中,让内窥镜引导管23经由嘴和食道前进到胃中。把内窥镜22插入钉合器手柄中的内窥镜通道里,并使其沿着钉合器手柄的腔往下前进。使钉合器/内窥镜同时通过内窥镜引导管朝向胃移动。一旦钉合器和内窥镜到达胃的胃-食道接合区域,就保持钉合器的位置而让内窥镜进一步前进到胃中。
使钉合器头14前进到胃中的期望深度和位置。如图31A所示,利用钉合器手柄上的关节连接控制件,把钉合器头的角度取向调整为允许将钉合器头12定位到预定目标组织处。把膜24中的开口26定位在目标组织上。把内窥镜22置于如图所示的后弯位置。
把真空源20(图2)联接到在身体外的手柄上的真空端口,并施加真空压力以便通过开口26把组织17吸入由膜24限定的真空室中,如图31B和32A所示。可以通过钉合器头上的透明膜24的壁借助于内窥镜容易地确认目标组织的获取。
把流体源(示出)联接到手柄。一旦已经从视觉上确认获取了足够量的组织,就引入流体以引起组织的压缩和臂组件32及升膜器37的扩展,如图32B和31C所示。如可以看到的,臂组件和膜的扩展允许大量的组织被获取到真空室内并在组织压缩过程中进一步移动到真空室内。如之前所述,在操作期间组织被吸入扩展后的真空室内、在缝合钉保持器和砧“之上”很多(图32B和32C),这提供了已钉合组织部分周围相对较大的组织余量,从而降低了在已钉合组织部分附近的组织褶薄弱或组织撕裂的风险。被捕获的组织褶以17a表示。
一旦组织已被压缩,就引入另外的液压流体,以便引起组织的钉合和切割(如图31D和32C所示),从而形成褶皱P(在图33中以17h表示)。然后,停用压缩和钉合液压源以释放液压回路中的流体压力。缓解了液压压力之后,升膜器37的弹簧丝帮助钉合器头14恢复其初始流线型构造,从而允许钉合器头从组织撤离,如图31E所示。可使钉合器头相对于轴发生关节运动从而有助于把钉合器头移离褶皱P。
在图33所示的优选褶皱构造中,缝合钉158布置成两个同心的由五个缝合钉构成的圈,缝合钉加强装置83由缝合钉保持并且围绕如图所示的缝合钉图案分布力。褶皱P包括由切割元件形成的孔H,穿过该孔可放置各种植入物或用于各种植入物的锚固装置。
如果需要多个褶皱,则把钉合器12暂时从内窥镜引导管中撤出并以结合图25A-25C所描述的方式更换缝合钉储筒。重复这个过程直到形成所有期望的褶皱。
可给所述系统封装使用说明书,用于指导使用者利用各种所公开的特征来借助于本文公开的方法执行钉合过程。
替代实施例
以上所公开的钉合器的基本架构可用作其他钉合工具的基础。图34-35示出了经修改的钉合器,其中移除了膜和升膜器,并且已经对缝合钉壳体28进行了修改以便附接工具。如图34所示,缝合钉壳体28包括一对用于接收工具162的凹槽160。工具162可安置在这些凹槽160中并安装到缝合钉壳体,如图35所示。该附接将会提供稳定的基座,从该基座致动工具。工具可以是自我关节运动式,或者缝合钉壳体28可以配备有用于使工具在流线型位置(用于把组件插入体腔)和如图35所示的展开位置之间运动的装置164。类似于图35中工具的工具可用于通过到达储筒和砧之间而进行组织获取,并可用于接合组织和把组织拉到储筒和砧之间的合适位置以便该组织可被钉合或者被加到砧和储筒或取代砧和储筒的各种特征作用。可受益于钉合器调整的过程包括但不限于胃形成术、人造口调整术、息肉去除术、电极置入术、流血控制、穿孔或孔闭合、活组织检查或肿瘤去除。
所公开的系统提供了用于实施所公开的压缩和钉合功能的便利实施例。然而,还存在很多其他各种非常不同的可以在本发明范围内替代性地使用的器具或系统。此外,所公开的实施例的特征可以以不同的方式相互组合以及与其它特征组合,从而产生更多实施例。因此,本文所描述的实施例应该被当作可用于形成内窥镜组织褶皱的系统的代表性示例,而不应该被用于限制所要求保护的本发明的范围。

Claims (10)

1.一种用于向组织施用缝合钉的手术钉合器装置(14),包括:
缝合钉保持器(78);
砧(96),其与缝合钉保持器沿纵轴隔开,所述砧设置在砧壳体上并可相对于所述砧壳体独立运动;
驱动部件(68),所述驱动部件可操作地连接到所述缝合钉保持器(78)并且构造成从收回位置运动到伸出位置,以及
臂组件(32),所述臂组件可操作地联接到缝合钉保持器(78)和砧(96),所述臂组件(32)包括通过铰链(104)联接在一起的两个臂(100,102),使得所述驱动部件(68)从其收回位置到其伸出位置的运动能够有效地使两个臂(100,102)的每个臂向外枢转以使铰链(104)远离纵轴运动并且朝向彼此驱动缝合钉保持器(78)和砧(96)。
2.如权利要求1所述的装置,进一步包括联接到缝合钉保持器(78)的缝合钉壳体(28),其中,缝合钉保持器(78)相对于缝合钉壳体(28)可独立地运动,使得所述驱动部件(68)从其收回位置到伸出位置的运动能够有效地使所述缝合钉保持器(78)相对于缝合钉壳体(28)运动。
3.如权利要求1所述的装置,其中,所述缝合钉保持器(78)和所述砧(96)具有相对的表面,所述相对的表面与所述臂组件(32)一起限定室(21),并且所述驱动部件(68)从其收回位置到其伸出位置的运动的效果是挤压被捕获在所述室内的组织,从而形成被捕获组织褶。
4.如权利要求3所述的装置,其中,所述室(21)被膜(24)覆盖,从而允许在对所述室施加真空的情况下把组织吸入到所述室内。
5.如权利要求4所述的装置,其中,所述膜(24)在其一侧上具有开口(26),用于把组织抽吸到所述室内,并且在对所述室施加真空的情况下,所述驱动部件(68)从其收回位置到其伸出位置的运动能够有效地把组织通过所述开口吸入到所述室中。
6.如权利要求1所述的装置,其中,所述驱动部件包括连接到驱动部件(68)的驱动活塞(106),并且所述装置还包括:缝合钉活塞(116),其被设置以便在所述驱动活塞(106)内在收回位置和伸出位置之间运动;以及缝合钉推动器(76),其适于接合所述缝合钉保持器(78)中的一个或多个缝合钉,并当所述缝合钉推动器因所述缝合钉活塞从收回位置到伸出位置而运动时,把所述一个或多个缝合钉从所述保持器中迎着所述砧(96)射出。
7.如权利要求6所述的装置,其中,缝合钉保持器(78)被设计成保持环形缝合钉阵列,并且所述缝合钉推动器(76)被设计成同时接合和射出所述保持器中的缝合钉阵列。
8.如权利要求1所述的装置,还包括布置成抵靠所述缝合钉保持器(78)的加强环(83),所述加强环具有用于接收通过其中的缝合钉的开口(85),以便将所述加强环(83)附接到已钉合组织的面向所述缝合钉保持器的那侧。
9.如权利要求1所述的装置,其中,两个臂包括可枢转地联接到缝合钉保持器的近端臂(100),以及可枢转地联接到砧的远端臂(102)。
10.如权利要求1所述的装置,其中,近端臂(100)和远端臂(102)在铰链(104)处可枢转地联接在一起。
